Abstract (sommario):
ObjectiveSince 2014 Senegal has benefited from regular awareness-raising Short Message Service (SMS) campaigns (Be He@lthy, Be Mobile initiative) directed at people who have signed up, for free, to the ‘mDiabète’ programme. We report on an evaluation of its impact on diabetes control.DesignThe clinical trial was designed to send daily SMS during 3 months to people with type 2 diabetes. Due to centre randomisation, SMS were sent from inclusion (M0) to month 3 (M3) to people in centre S and from M3 to month 6 (M6) to people in centre P.SettingMedical centre S in the northwestern suburbs of Dakar; centre P in Popenguine, 70 km south of Dakar.ParticipantsIn February 2017, people with type 2 diabetes were consecutively recruited in the two centres. Complete data were available from 186 of these people.Main outcome measuresHbA1c was measured in the two centres with the same assay throughout the study. The primary end point was the difference between centres for the change in HbA1c from M0 to M3. Secondary end points were the evolution of HbA1c in centres S and P between M3 and M6.ResultsThe HbA1c change from M0 to M3 in centre S was better than in centre P, with a median difference of −0.4%, quartiles (−1.0; 0.3) versus 0.2% (-0.5; 0.8), respectively (p=0.0038). HbA1c decreased over the 3 months after having stopped SMS in centre S and was confirmed in centre P. The campaign cost was €2.5 (US$3.1) per person.ConclusionsIn Senegal, SMS sending was associated with an improved glycaemic control in people with type 2 diabetes. As SMS has a high penetration in low-income, middle-income countries where medical resources are scarce, health interventions using mobile telephones should be developed to facilitate exchanges between people with diabetes and medical teams; this may reduce diabetes-related complications.

Abstract (sommario):
IntroductionSevere aortic stenosis is a major cause of morbidity and mortality. The existing treatment pathway for transcatheter aortic valve implantation (TAVI) traditionally relies on tertiary Heart Valve Centre workup. However, this has been associated with delays to treatment, in breach of British Cardiovascular Intervention Society targets. A novel pathway with emphasis on comprehensive patient workup at a local centre, alongside close collaboration with a Heart Valve Centre, may help reduce the time to TAVI.MethodsThe centre performing local workup implemented a novel TAVI referral pathway. Data were collected retrospectively for all outpatients referred for consideration of TAVI to a Heart Valve Centre from November 2020 to November 2021. The main outcome of time to TAVI was calculated as the time from Heart Valve Centre referral to TAVI, or alternative intervention, expressed in days. For the centre performing local workup, referral was defined as the date of multidisciplinary team discussion. For this centre, a total pathway time from echocardiographic diagnosis to TAVI was also evaluated. A secondary outcome of the proportion of referrals proceeding to TAVI at the Heart Valve Centre was analysed.ResultsMean±SD time from referral to TAVI was significantly lower at the centre performing local workup, when compared with centres with traditional referral pathways (32.4±64 to 126±257 days, p<0.00001). The total pathway time from echocardiographic diagnosis to TAVI for the centre performing local workup was 89.9±67.6 days, which was also significantly shorter than referral to TAVI time from all other centres (p<0.003). Centres without local workup had a significantly lower percentage of patients accepted for TAVI (49.5% vs 97.8%, p<0.00001).DiscussionA novel TAVI pathway with emphasis on local workup within a non-surgical centre significantly reduced both the time to TAVI and rejection rates from a Heart Valve Centre. If adopted across the other centres, this approach may help improve access to TAVI.

Abstract (sommario):
Background: Ventriculoperitoneal (VP) shunts are an established treatment modality for CSF diversion. An alternative to VP shunting is lumboperitoneal (LP) shunting. There is a paucity of evidence on LP shunt use in the literature, but available studies demonstrate that it is a safer and similarly efficacious method for conditions such as normal pressure hydrocephalies (NPH) and idiopathic -intracranial hypertension (IIH). Methods: Ventriculoperitoneal (VP) shunts are an established treatment modality for CSF diversion. An alternative to VP shunting is lumboperitoneal (LP) shunting. There is a paucity of evidence on LP shunt use, but available studies demonstrate that it is a safer and similarly efficacious method for conditions such as normal pressure hydrocephalies (NPH) and idiopathic intracranial hypertension (IIH). Results: 95 patients were treated with lumboperitoneal shunt, 71 of which were for hydrocephalus and 24 for IIH. 39 male and 58 female patients were included with mean age 55 (range from 20 to 96 years old). 26 patients had laparoscopic placement of the peritoneal catheter. Mechanical issues with distal end was less with laparoscopic approach. Conclusions: We will review disease-specific scores for NPH and IIH, and compare laparoscopic with non-laparoscopic placement of peritoneal catheter. We will also compare outcomes and complications with rates for VP shunting.

Abstract (sommario):
This research studies the p-centre problem in the continuous space. This problem is particularly useful in locating emergency facilities, such as fire-fighting stations, police stations and hospitals where it is aimed to minimise the worst-case response time. This problem can be divided into a single facility minmax location problem (1-centre) and multi-facility minmax location problem (p-centre). The solution of the 1-centre location problem can be found optimally in polynomial time by using the well known Elzinga-Hearn algorithm for both the weighted and the unweighted case. The objective of the p-centre problem is to locate p facilities (p>1) so as to minimise the radius of the largest circle. However, in this case, we cannot always guarantee optimality as the problem is known to be NP hard. The aim of the research is to develop and analyse powerful meta-heuristics including the hybridisation of exact methods and heuristics to solve this global optimisation problem. To our knowledge this is the first study that meta-heuristics are developed for this problem. In addition larger instances previously used in the literature are tested .This is achieved by designing an efficient variable neighbourhood search, adapting a powerful perturbation method and extending a newly developed reformulation local search. Large instances are used to evaluate our approaches with promising results.

Abstract (sommario):
This thesis will analyse, investigate and develop new and interesting ideas to optimally solve a location problem called the continuous p−centre problem. This problem wishes to locate p facilities in a plane or network of n demand points such that the maximum distance or travel time between each demand point and its closest facility is minimised. Several difficulties are identified which are to be overcome to solve the continuous p−centre problem optimally. These relate to producing a finite set of potential facility locations or decreasing the problem size so that less computational time and effort is required. This thesis will examine several schemes that can be applied to this location problem and its related version with the aim to optimally solve large problems that were previously unsolved. This thesis contains eight chapters. The first three chapters provide an introduction into location problems, with a focus on the p−centre problem. Chapter 1 begins with a brief history of location problems, followed by the various classifications and methodologies used to solve them. Chapter 2 provides a review of the methods that have been used to solve the p−centre problem, with a focus on the continuous p−centre problem. An overview of the location models used in this research is given in Chapter 3, alongside an initial investigative work. The next two chapters enhance two well-known optimal algorithms for the continuous p−centre problem. Chapter 4 develops an interesting exact algorithm that was first proposed over 30 years ago. The original algorithm is reexamined and efficient neighbourhood reductions which are mathematically supported are proposed to improve its overall computational performance. The enhanced algorithm shows a substantial reduction of up to 96% of required computational time compared to the original algorithm, and optimal solutions are found for large data sets that were previously unsolved. Chapter 5 develops a relatively new relaxation-based optimal method. Four mathematically supported enhancements are added to the algorithm to improve its efficiency and its overall computational time. The revised reverse relaxation algorithm yields a vast reduction of up to 87% of computational time required, which is then used to solve larger data sets where n ≤ 1323 optimally. Chapter 6 creates a new relaxation-based matheuristic, called the relaxed p' matheuristic, by combining a well-known heuristic and the optimal method developed in Chapter 5. The unique property of the matheuristic is that it deals with the relaxation of facilities rather than demand points to establish a sub-problem. The matheuristic yields a good, but not necessarily optimal, solution in a reasonable time. To guarantee optimality, the results found from the matheuristic are embedded into the optimal algorithms developed in Chapters 4 and 5. Chapter 7 adapts the optimal algorithm developed in Chapter 5 to solve two related location problems, namely the α−neighbour p−centre problem and the conditional p−centre problem. The α−neighbour p−centre problem is investigated and solved where α = 2 & 3. A scenario analysis is also conducted for managerial insights by exploring changes in the number of facilities required to cover each demand point. Furthermore, an existing algorithm for the conditional p−centre problem is enhanced by incorporating the optimal algorithm proposed in Chapter 5, and it is used to solve large data sets where the number of preexisting facilities is 20. This chapter therefore demonstrates that an algorithm developed in this research can be adapted or used to enhance existing algorithms to optimally solve more practical and challenging related location problems. Finally, Chapter 8 summarises the findings and main achievements of this research, and outlines any further work that could be worthwhile exploring in the future.

Abstract (sommario):
Over time, the Town Centre has always had an important role in the development of local economies (McAteer and Stephens, 2011), experiencing periods of growth and decline that influenced its physiognomy. The Town Centre’s decline was caused by several different factors, such as the decentralization of retail activities away from, and on the edge of, the Town Centre (Schiller, 1986; 1988), and the development of new forms of selling over the Internet (Bhatti, 2007; Ickler, et al., 2011). All in all, this has led to a crisis for Town Centre’s retailers, prejudicing their development and enhancement (Alzubaidi et al., 1997; Thomas and Bromley, 2003). In order to contrast the pauperization of the Town Centre and to improve its growth, during the last decades in several European Cities was created the Town Centre Management (TCM) schemes, to find practical responses to the emerging complexities of urban revitalisation (Coca- Stefaniak et al., 2009). Taking into consideration this scenario, the aim of this work is to further the understanding of the role-played by TCM’s over time in revitalizing the commercial activities in the Town Centre. Studying TCM programs and activities carried out in different European cities will work towards that better understanding of their effectiveness. By investigating the TCM programs in some Swedish and Italian cities, this study aims at identifying the core elements in the TCM strategies related to improving and developing the Town Centre. This thesis is organized in three autonomous papers, where each of them constitutes a specification of one general topic. The first paper aims to analyse the existing literature to highlight scholars’ point of view and, to verify whether the retail activity maintains the central role in the perspective of the Town Centre revitalization, or if the attention has shifted to other activities. One question of this analysis is: What are the research lines that emerge as attracting interest for future development regarding the Town Centre revitalization? This paper built the review of the literature (Roley and Slack, 2004) by using a retrieval procedure based on the combination of several keywords in order to identify the journals where the considered topic assumes relevance. The purpose of the second paper is to understand the way in which the TCM, through the adoption of practical strategies, tackles problems related to the decentralization of retail activities away from the Town Centre. More precisely, the research questions are: what are the answers that Cities gave to the development of Town Centre problems? In the European context what kind of organizations has been created to pursue the goal to revitalize the Town Centre? Does an accord exist between scholars to the most effective initiatives that Cities have to adopt? To answer these questions a qualitative method based on multiple case study methodology was adopted (Eisenhardt, 1989; 1991; Yin, 1994; 2009). The multiple, case study permitted to make a comparison between different realities, providing a stronger base to clarify the several multifaceted aspects of the same phenomenon. From the analysis of the literature and empirical cases, studied in the first two papers, it emerged that the use of Internet and ways to sell through it (Bhatti, 2007; Ickler et al., 2009; Curty, and Zhang, 2011) are rarely adopted as tools able to revitalize the Town Centre. Inasmuch, the third paper aims to highlight what kind of e-technologies - after a specific analysis of each of ecommerce, s-commerce and m-commerce - are more suitable than others to help Town Centres retail activities to regain their past glory. The research questions are: What are the reasons for the development of new forms of selling? What are the new forms of sale over the Internet developed in the last decades? And, in what way is it possible to connect old and new forms of selling in order to improve the retail attractiveness of Town Centres? To find a way by which it is possible to connect old and new forms of selling that could improve the retail experience in Town Centres, a model has been proposed. The implications of this thesis are both theoretical, since it contributes to a better understanding of the relationship between TCM and the renaissance of the retailing activities (in order to support the urban revitalization), and practical, as it emphasizes the role of the internet and new forms of selling over it that could be integrated within traditional forms of selling within fixed location stores in the Town Centre.

Abstract (sommario):
This thesis focuses on the exact solution of the NP-hard problems p-median and p-center, combinatorial optimization problems that quickly become difficult to solve as the instance size increases. These discrete location problems involve opening a defined number p of facilities and then allocating to them a set of clients according to an objective function to be minimized.First, we study the p-median problem, which seeks to minimize the sum of distances between clients and the open facilities to which they are allocated. We develop an algorithm based on Benders decomposition that outperforms state-of-the-art exact methods. The algorithm considers a two-stage approach and an efficient algorithm for separating Benders cuts. The method has been evaluated on over 230 benchmark instances with up to 238025 clients and sites. Many instances are solved to optimality for the first time or have their best known solution improved.Secondly, we explore the p-center problem, which seeks to minimize the largest distance between a client and its nearest open facility. We first compare the five main MILP formulations in the literature. We study the Benders decomposition and also propose an exact algorithm based on a client clustering procedure based on the structure of the problem. All the proposed methods are compared with the state-of-the-art on benchmark instances. The results obtained are analyzed, highlighting the advantages and disadvantages of each method. Finally, we study a robust two-stage p-center problem with uncertainty on node demands and distances. We introduce the robust reformulation of the problem based on the five main deterministic MILP formulations in the literature. We prove that only a finite subset of scenarios from the infinite uncertainty set can be considered without losing optimality. We also propose a column and constraint generation algorithm and a branch-and-cut algorithm to efficiently solve this problem. We show how these algorithms can also be adapted to solve the robust single-stage problem. The different proposed formulations are tested on randomly generated instances and on a case study drawn from the literature

Abstract (sommario):
<p>The Majestic Centre is a 30-storey office tower in the centre of Wellington, New Zealand. The structure has a dual lateral system (reinforced concrete (RC) moment frame + shear cores) and hollow-core floors. The building’s assessed seismic performance was found to be below expected levels, leading to a strengthening exercise. Over a period of 5 years, the structures performance was raised to meet current seismic loading requirements, at a cost of €50M.</p>

Abstract (sommario):
AbstractThe recent health crisis and the phenomena linked to environmental upheavals have aroused great interest in the theme of emergency and the need to systematise it.The “Design for Emergency”, not yet well defined and in continuous evolution within the scientific discipline of Design, is today at the centre of the cultural debate to find a more precise definition, limits and possibilities, good practices and necessary interdisciplinary relations. After an initial introduction to the topic, the essay intends to define and discuss, through selected case studies, the design’s contribution within critical contexts, through the reinterpretation of two terms, the “cycle” and the “wave”, discussed in the essay “Designing in the Post-COVID Era” (2021) by M. Bianchini, P. Bolzan and S. Maffei [1].The two terms, used by the authors to describe a range of phenomena, would influence differently the actions, methods and design strategies aimed at developing innovative and effective solutions and products to cope with emergency conditions.

Abstract (sommario):
AbstractThe formation of cities, and the relocation of workers to densely populated areas reflect a spatial equilibrium, in which the higher real consumption levels of urban areas are offset by lower non-monetary amenities [1]. However, as the society progress toward a post-COVID stage, the prevailing decentralized delivery systems and location-based services, the growing trend of working from home, with citizens’ shifting preference of de-appreciating densities and gathering, have not only changed the possible spatial distribution of opportunities, resources, consumption and amenities, but also transformed people’s preference regarding desirable urban spatial qualities, value of amenities, and working opportunities [2, 3].This research presents a systematic method to evaluate the perceived trade-off between urban spatial qualities and urban utilities such as amenities, transportation, and monetary opportunities by urban residence in the post-COVID society. The outcome of the research will become a valid tool to drive and evaluate urban design strategies based on the potential self-organization of work-life patterns and social profiles in the designated neighbourhood. To evaluate the subjective perception of the urban residence, the study started with a comparative survey by asking residence to compare two randomly selected urban contexts in a data base of 398 contexts sampled across Hong Kong and state their living preference under the presumption of following scenarios: 1. working from home; 2. working in city centre offices. Core information influencing the spatial equilibrium are provided in the comparable urban context such as street views, housing price, housing space, travel time to city centre, adjacency to public transport and amenities, etc. Each context is given a preference score calculated with Microsoft TrueSkill Bayesian ranking algorithm [4] based on the comparison survey of two scenarios. The 398 contexts are further analysed via GIS and image processing, to be deconstructed into numerical values describing main features for each of the context that influence urban design strategies such as composition of spatial features, amenity allocation, adjacency to city centre and public transportations. Machine learning models are trained with the numerical values of urban features as input and two preference scores for the two working scenarios as the output. The correlation heat maps are used to identify main urban features and its p-value that influence residence’s preference under two working scenarios in post–COVID era. The same model could also be applied to inform the direction of urban design strategies to construct a sustainable community for each type of working population and validate the design strategies via predicting its competitiveness in attracting residence and developing target industries.

Abstract (sommario):
This work aims to investigate, based on the Strouhal number St and the flow structure, the dependence of flow classification on the Reynolds number Re in the wake of two staggered cylinders, with Re varying from 1.5×103 and 2.0×104. The cylinder centre-to-centre pitch, P* = P/d examined is 1.2 ∼ 6.0 (d is the cylinder diameter), and the angle (α) between the incident flow and the line through the cylinder centres is 0° ∼ 90°. Two single hotwires were used to measure simultaneously the St behind each of the two cylinders over 2.5d ∼ 15d. Whilst the present data reconfirms the flow structure modes previously reported, the dependence of the flow modes on P* and α exhibits an appreciable dependence on Re. The observation is connected to the Re effect on the generic features of a two-cylinder wake such as flow separation, boundary layer thickness, gap flow deflection and vortex formation length.

Abstract (sommario):
The last weeks have seen a flurry of activity across the Australian university sector with the release of the Universities Accord Final Report. While I write, there is vigorous debate occurring, deliberating both the Accord’s strengths and its inherent limitations, one of the latter being the apparent perpetuation of low expectations for students with disability (see McLennan, 2024). Whilst the relative merits of some of the report’s recommendations have been questioned, there is little doubt that both this document—and the associated review process—have succeeded in placing educational equity at the centre of any future imaginings of the Australian higher education system. This is welcome news for many of us who have worked alongside students from more diverse backgrounds and witnessed the many obstacles encountered in their higher education journeys. To address these and other enduring issues, the Accord recommends significant changes that include accessible preparatory programs for future students, a better integrated tertiary system, and—of course—increases in student funding support. But I would argue there are other, far more fundamental, issues that also need to be addressed if we are to achieve the Accord’s ultimate goal of a “better and fairer education system”. Abstract (sommario):

Abstract (sommario):
Background and Objectives- Symptomatic Primary Hyperparathyroidism (PHPT) is common in India in comparison to the western population. But there is very little data on the inuence of age on the presentation of PHPT. In the present study we aimed to analyse the clinical and metabolic prole among different age groups of symptomatic primary hyperparathyroidism. Methods: This retrospective analysis was done in PHPT patients who attended Department of Endocrinology, Gauhati Medical college and Hospital. Thirty-one PHPT subjects who presented to us over a period of last ve years were divided into three different age groups i.e, children and adolescents <18yrs, adults ≥18-50 years, and older group >50years. All major clinical, metabolic and imaging parameters were compared among these groups. Appropriate statistical methods were used to compare different variables. The age distribution ranged from 13 to Results: 72 years with mean age of 38.6±16.3years and with equal female to male ratio. Bony deformity (Rickets) as initial manifestation was seen in three adolescents and bone pain was common in adolescents(p=0.05). Prevalence of renal stones were higher in adult group(p=0.002), gastrointestinal manifestations were higher in older group (p=0.02). There was no signicant difference in fracture rate(P=0.17), brown tumours(P=0.56) and other symptoms among different age groups. Alkaline phosphatase(p=0.006) and iPTH(p=0.01) were signicantly higher in adolescent group. There was no signicant difference in serum calcium, phosphate, 25(OH)Vitamin-D3 and haemoglobin levels among different age groups. Age has substantial inuence on PHPT presentation. Bone Interpretation & Conclusion: pain and deformity was common in adolescents, while renal stones and gastrointestinal manifestations were common in middle aged and elderly group respectively

Abstract (sommario):
The Australian Council for Educational Research (ACER) was commissioned by the Teachers and Teaching Research Centre (TTRC) at the University of Newcastle to conduct an independent randomised controlled trial (RCT), with the goal of examining effects of Quality Teaching Rounds (QTR) on student outcomes and teachers’ practice in Victorian high schools. A total of 19 schools participated in Quality Teaching Rounds in 2022, with 20 schools in the wait list control. Data were gathered in an ongoing manner during the evaluation with: Progressive Assessment Tests in Mathematics (PAT-M) and reading (PAT-R) – baseline and follow up; student self-efficacy and aspiration surveys – baseline and follow up; teacher surveys – one questionnaire administered every term; implementation fidelity check surveys for teachers to complete for each QT Round; and implementation fidelity checks with onsite visits from ACER staff for 33% of the treatment schools. Key findings include: The mixed model analysis showed that treatment was not a significant predictor of PAT-R and PAT-M outcomes. Differences in student responses to the self-efficacy and aspiration surveys were identified. The control group showed a significant increase in the level of education that they aspired to complete (p = 0.037). Teachers in the control group had statistically significant growth in teacher efficacy, while those in the treatment group showed statistically significant lower teacher student support. Within the QTR process, the longest time was spent on discussing the coding and the individual coding process. Key observations identified from analysis of the fidelity check data are: teacher stress due to high rates of absenteeism, varied use of the Classroom Practice Guide, and analytical conversations about some elements and terms.

Abstract (sommario):
This research was performed at Petrovsky waste rock dump, the Central part of the Kryvyi Rih iron-ore & metallurgical district (Dnipropetrovsk region, Ukraine). The macronutrients (K, Ca, Mg, P and S) and heavy metals (Fe, Mn, Zn, Cu, Pb and Cd) contents in the leaves of three species of the trees (Ash-leaved maple, Silver Birch and Black locust) that were collected on devastated lands were assessed. It was established that trees which grow on the Petrovsky dump take place under evident shortage of nutrients (especially K and P) and excess of metals (especially Fe, Mn and Zn). It is revealed that Ash-leaved maple and Black locust are more resistant to the geochemical conditions of devastated lands. In this regard, these species of trees can be recommended for forest restoration & reforestation on devastated lands.

Abstract (sommario):
New potential for engineering chloroplasts to express novel traits has stimulated research into relevant techniques and genetic processes, including plastid transformation and gene regulation. This proposal continued our long time BARD-funded collaboration research into mechanisms that influence chloroplast RNA accumulation, and thus gene expression. Previous work on cpRNA catabolism has elucidated a pathway initiated by endonucleolytic cleavage, followed by polyadenylation and exonucleolytic degradation. A major player in this process is the nucleus-encoded exoribonuclease/polymerasepolynucleotidephoshorylase (PNPase). Biochemical characterization of PNPase has revealed a modular structure that controls its RNA synthesis and degradation activities, which in turn are responsive to the phosphate (P) concentration. However, the in vivo roles and regulation of these opposing activities are poorly understood. The objectives of this project were to define how PNPase is controlled by P and nucleotides, using in vitro assays; To make use of both null and site-directed mutations in the PNPgene to study why PNPase appears to be required for photosynthesis; and to analyze plants defective in P sensing for effects on chloroplast gene expression, to address one aspect of how adaptation is integrated throughout the organism. Our new data show that P deprivation reduces cpRNA decay rates in vivo in a PNPasedependent manner, suggesting that PNPase is part of an organismal P limitation response chain that includes the chloroplast. As an essential component of macromolecules, P availability often limits plant growth, and particularly impacts photosynthesis. Although plants have evolved sophisticated scavenging mechanisms these have yet to be exploited, hence P is the most important fertilizer input for crop plants. cpRNA metabolism was found to be regulated by P concentrations through a global sensing pathway in which PNPase is a central player. In addition several additional discoveries were revealed during the course of this research program. The human mitochondria PNPase was explored and a possible role in maintaining mitochondria homeostasis was outlined. As polyadenylation was found to be a common mechanism that is present in almost all organisms, the few examples of organisms that metabolize RNA with no polyadenylation were analyzed and described. Our experiment shaded new insights into how nutrient stress signals affect yield by influencing photosynthesis and other chloroplast processes, suggesting strategies for improving agriculturally-important plants or plants with novel introduced traits. Our studies illuminated the poorly understood linkage of chloroplast gene expression to environmental influences other than light quality and quantity. Finely, our finding significantly advanced the knowledge about polyadenylation of RNA, the evolution of this process and its function in different organisms including bacteria, archaea, chloroplasts, mitochondria and the eukaryotic cell. These new insights into chloroplast gene regulation will ultimately support plant improvement for agriculture
