Abstract (sommario):
Background/Aims: Several pathological classification systems were commonly used in clinical practice to predict the prognosis of IgA nephropathy (IgAN). However, how prognostic value differs between these systems is unclear. The aim of this study was to compare the Lee grade, the Oxford classification, and the Haas classification and to find a simplified classification. Methods: We retrospectively analyzed IgAN cases diagnosed between January 2002 and December 2007. The endpoints were progression to end-stage renal disease (ESRD) or a ≥50% decline in estimated glomerular filtration rate (eGFR). The predictive capabilities were evaluated by comparing the ability of discrimination (continuous net reclassification) and calibration (Akaike information criterion [AIC]). Results: A total of 412 IgAN patients were included in the study. The average follow-up period was 80.62 ± 23.63 months. A total of 44 (10.68%) patients progressed to ESRD, and 70 (16.99%) patients showed a ≥50% decline in eGFR. All multivariate Cox regression models had limited power for high AIC values. The prognostic values of the Lee grade and the Oxford classification were higher than those of models containing only established baseline clinical indicators for progression to ESRD or a ≥50% decline in eGFR (Lee grade 0.50, 95% CI 0.21–0.74; Oxford classification 0.48, 95% CI 0.28–0.71). The prognostic value of the Haas classification was lower than that of the other pathological classification systems for progression to ESRD or a ≥50% decline in eGFR (Lee grade 0.53, 95% CI 0.23–0.92; Oxford classification 0.59, 95% CI 0.10–0.74). The prognostic value of hierarchical classification (Beijing classification) using M and T lesion was similar to the Oxford classification. Conclusions: Both the Lee grade and the Oxford classification showed incremental prognostic values beyond established baseline clinical indicators. The Haas classification was slightly inferior to the Lee grade and the Oxford classification. The hierarchical classification (Beijing classification) using less pathological parameters does not lose predictive efficiency.

Abstract (sommario):
Immunoglobulin A vasculitis nephritis (IgAVN) or Henoch-Schönlein purpura nephritis (HSPN) is the most common type of secondary IgA nephropathy (IgAN), particularly in children. Overall, its prognosis is good, but in a significant number of cases, it does lead to chronic kidney disease (CKD) and end-stage renal disease (ESRD). Various classifications exist to predict the long-term outcome of kidney disease in these diseases, but none is universally accepted. We searched PubMed, Web of Science Embase, EBSCO, Scopus and directory of open access journals (DOAJ) with keywords of Henoch-Schönlein purpura nephritis, immunoglobulin A vasculitis nephritis, immunoglobulin A nephropathy, IgA vasculitis, extracapillary proliferation, IgA nephropathy, leukocytoclastic vasculitis, endstage renal disease, Oxford classification, crescent and childhood IgA vasculitis. IgAVN in children presents most often with crescents and endocapillary proliferation, with relapsing and remitting course clinically. Due to morphological resemblance of IgAVN and IgAN, the Oxford MEST-C scores could be applied for determining the long-term outcomes in the former disease. A critical concern in applying Oxford classification for IgAVN is that limited number of children with IgAVN exists in any one center and also a relatively short period of follow-up. Hence, further work in this regard is necessary. Preliminary evidence suggests that Oxford MEST-C classification is valid in predicting long-term kidney outcomes in children with IgAVN and the classification can also be used in adults. However, further, large scale, multicenter, international collaborative studies are needed to address the unmet issues.

Abstract (sommario):
Abstract Background Complement activation has been highlighted in immunoglobulin (Ig) A nephropathy pathogenesis. However, whether the complement system can affect the downstream phenotype of IgA nephropathy remains unknown. Herein, we investigated the association of mesangial C3 deposition with the Oxford classification and their joint effects on worsening kidney function. Methods We investigated 453 patients with biopsy-proven IgA nephropathy. C3 deposition was defined as an immunofluorescence intensity of C3 ≥2+ within the mesangium. The subjects were classified according to the combination of C3 deposition and Oxford classification lesions. The primary endpoint was a composite of ≥30% decline in the estimated glomerular filtration rate or an increase in proteinuria ≥3.5 g/g during follow-up. Results Among the Oxford classification lesions, mesangial hypercellularity (M1), segmental glomerulosclerosis (S1) and tubulointerstitial fibrosis (T1–2) and crescentic lesion significantly correlated with C3 deposition. During a median follow-up of 33.0 months, the primary endpoint occurred more in patients with M1, S1, T1–2 and mesangial C3 deposition than in those without. In individual multivariable-adjusted Cox analyses, the presence of M1, S1, T1–2 and C3 deposition was significantly associated with higher risk of reaching primary endpoint. In the combined analyses of C3 deposition and the Oxford classification lesions, the hazard ratios for the composite outcome were significantly higher in the presence of C3/M1, C3/S1 and C3/crescent than in the presence of each lesion alone. Conclusions Complement deposition can strengthen the significance of the Oxford classification, and the presence of both components portends a poorer prognosis in IgA nephropathy.

Abstract (sommario):
This thesis explores the history of botany as a global collection-based science by tracing parallels between utopian traditions and botanical collecting, from their sixteenth-century beginnings to the present. A range of botanical collections, such as gardens, herbaria and classification systems, have played a central role in the struggle to discover a global or universal scientific order for the chaotic, diverse and locally shaped kingdom of plants. These collections and utopia intersect historically, and are characterised by the same epistemology of collecting: the creation of order through confined collecting spaces or “no-place.” They are manipulations of space and time. Between chaos and order, both seek to make a whole from – often unruly – parts.   The long history of botanical collecting is characterised by a degree of continuity of practice that is unusual in the sciences.  For instance, the basic technology of the herbarium – preserving plants by mounting and labelling dried specimens on paper – has been in use for almost five centuries, from sixteenth-century Italy to ongoing digitisation projects. The format of the compilation thesis is well-suited to handling the historiographical challenge of tracing continuity and discontinuity with such a long chronological scope.   The thesis is structured as a walled quadripartite garden, with the Kappa enclosing four research papers and an epilogue. The papers take a diachronic approach to explore different perspectives on botanical collections: botanical collecting in seventeenth-century Oxford, pressed plants in books that are not formally collections; and the digitisation of botanical collections. These accounts are all shaped by the world of books, text and publication, historically a male-dominated sphere. In order to acknowledge marginalisation of other groups and other ways of knowing plants, the epilogue is an explanation of an embroidered patchwork of plant-dyed fabric, which forms the cover of the thesis.

Abstract (sommario):
The Oxford Textbook of Neuro-Oncology examines the epidemiology, pathogenesis, clinical features, and management of adult and childhood central nervous system (CNS) tumours. The book is organized according to the World Health Organization (WHO) classification of CNS tumours and includes an introductory chapter discussing the new, revised WHO classification that will be used in future editions of this textbook. The book reviews primary CNS tumours, metastatic CNS tumours, and familial tumour syndromes. Each chapter is organized according to a uniform framework, addressing epidemiology, aetiology, pathogenesis, clinical presentation, imaging, and treatment, with a focus on different treatment modalities and specific consideration of paediatric aspects, where applicable. Each chapter is authored by a multidisciplinary, international group of senior specialists representing the United States, Canada, Europe, and the Asia/Pacific region. It is written for generalists and specialists managing these various conditions. It provides an outlook on how current developments in molecular diagnostics, which have in part been integrated into the new WHO classification, may shape clinical decision-making in the future.

Abstract (sommario):
Widely recognised as the standard text for trainee psychiatrists, the Shorter Oxford Textbook of Psychiatry stands head and shoulders above the competition. Honed over five editions it displays a rare fluency, authority and insight, and it makes the process of assimilating information as smooth and enjoyable as possible. The resource provides an introduction to all the clinical topics required by the trainee psychiatrist, including all the sub-specialties and major psychiatric conditions. Throughout, the authors emphasize the basic clinical skills required for the full assessment and understanding of the patient. Discussion of treatment includes not only scientific evidence, but also practical problems in the management of patients their family and social context. It emphasizes an evidence-based approach to practice and gives full attention to ethical and legal issues. Introductory chapters focus on recognition of signs and symptoms, classification and diagnosis, psychiatric assessment, and aetiology. Further chapters deal with all the major psychiatric syndromes as well as providing detailed coverage of pharmacological and psychological treatments. It also gives equal prominence to ICD and DSM classification - often with direct comparisons.

Abstract (sommario):
‘Classification’ introduces concepts of mental illness before briefly reviewing the reasons for, and criticisms of, psychiatric classification. Key issues of reliability, validity, and stigma associated with classification are covered. After an overview of the history of classifications, the organizing principles of the two current major classification systems used in psychiatry (ICD-10 and DSM-5) are then outlined: the World Health Organization’s International Classification of Diseases (‘ICD-10’), and the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (‘DSM-5’). Additional schemes, used in particular countries, are also briefly mentioned. Finally, the chapter summarizes how psychiatric classification may develop in the future, with particular reference to ICD-11, which is due in 2018 or 2019.

Abstract (sommario):
Abstract Diagnosis is simple. Diagnostic systems, generally called classifi cations, are lists of terms for conventionally accepted concepts that are used to describe psychopathology. Within the broad framework of classifi cation, there are three different processes for which specifi c names can be assigned. Classification, when the term is used specifi cally, refers to the activity of forming groups.

Abstract (sommario):
Chapter 2 discusses the principles of psychiatric diagnosis and classification, since this provides the framework within which this clinical process happens. The term nosology is sometimes used to refer to classification and its study. It also covers why classification is needed in psychiatry, including enabling clinicians to communicate with one another about the diagnoses given to their patients, understanding the implications of these diagnoses in terms of their symptoms, prognosis, and treatment, and sometimes their aetiology, relating the findings of clinical research to patients seen in everyday practice, facilitating epidemiological studies and the collection of reliable statistics, and ensuring that research can be conducted with comparable groups of subjects.

Abstract (sommario):
This chapter surveys previous attempts to classify the genetic relationships among the Tungusic languages. The set of sound correspondences that can be employed in this classification is examined and it is argued that, if one assumes binary branching for a cladistic classification, there are three plausible classifications that result from the application of the classical comparative method. Next, a Bayesian phylogenetic analysis of basic vocabulary is undertaken to determine whether that analysis provides any further evidence for which of the three classifications is preferred. The conclusion is that it does and that one of the best classifications of Tungusic places Manchu, Xibe, and Jurchen in a Southern Branch together with Udihe and Nanai complexes, and the Even-Evenki complex in a Northern Branch. Though our analysis does not exclude the most common classification in which the Manchuric branch separated first from all other Tungusic languages.

Abstract (sommario):
Abstract This chapter deals with the genetic classification of Ethiopian languages. It emphasizes that northeast Africa is a region of great genetic and linguistic diversity, which is a principal difficulty in the classification of Ethiopian languages. This chapter shows that Ethiopian linguistics has seen great progress in recent decades which resulted in a growing number of languages identified, recorded, and described. Nevertheless, two languages in southwest Ethiopia that remain unclassified, so-called isolates: Ongota (Birale), on the verge of extinction, and Shabo (Mikeyir), with at most a few hundred speakers. The chapter concludes with a concise discussion of the general method of language classification.

Abstract (sommario):
Several studies in the area of education have recently applied metaphor analysis as an analytical tool for investigating the perceptions and attitudes of various stakeholders, such as learners, practicing and pre-service teachers, language teachers, etc. about different aspects of teaching and learning (Nikitina and Furuoka, 2008; Oxford et al., 1998; Chen, 2003). The primary aim of the study presented in this paper is to identify the ways pre-service teachers perceive the teacher‟s role through metaphors. The theoretical framework first looks at metaphor from a cognitive perspective (Lakoff, 1993). We then turn to the conceptualisation of teachers through metaphors, with particular reference to the typology of teacher metaphors developed by Oxford et al. (1998), Oxford (2001), and Chenʼs (2003) classification system for metaphors about teaching. The second part presents the results of qualitative research which was carried out based on the analysis of studentsʼ personal documentary material in the form of argumentative essays. The results of the content analysis are presented in different categories and include a selection of direct quotations by the participants. The results indicate that future primary school teachers perceive the teacher‟s role through a variety of metaphors, such as teachers as friends, parents or role models. We would like to argue that their perceptions expressed through metaphors reflect different educational paradigms.
