ABSTRACTThe use of psychometric tools to assess various psychological aspects of malocclusion and treatment is increasing in orthodontics. Mere evaluation of an orthodontic patient with normative criteria is not enough; instead, the psychological status should be assessed using a questionnaire. Many generic and few condition-specific tools are available for assessing quality of life (QoL) in orthodontics. The steps involved in the development of such tools are complex and unknown to many. This article outlines the methodology involved in the development and validation of a psychometric tool for dental and orthodontic use. It also helps the clinician to translate and cross-culturally adapt an existing QoL tool to a different setting.

AbstractBackground:This correlational study explored the psychosocial aspects related to eating behavior in different age samples of adolescents in treatment from 0 to 60 months at the Clinic of Orthodontics and Dentistry of Messina, Messina, Italy. The aim of the study was to investigate the relationship between psychosocial impact, levels of self-esteem, and the possible connection with eating habits of adolescents under orthodontic treatment.Methods:Sixty-one adolescents, aged between 12 and 22 years (mean=15.6±2.8) participated to the study. Each adolescents was interviewed with the Eating Attitudes Test, the Rosenberg Self Esteem Scale, and the Psychosocial Impact of Dental Aesthetics Questionnaire.Results:Data did not show a direct connection between eating disorder and dental aesthetics, nevertheless, adolescents under orthodontic treatment, especially in the earliest phase of wearing braces, showed peculiar eating habits and underwent a higher psychological impact of dental aesthetics. Eating behaviors are strictly linked to global self-esteem. The processing of the results was made through the Student’s t-test and using Pearson’s correlation analysis.Conclusion:Increased knowledge of the psychological aspects involved in orthodontic treatment compliance may have positive effects in the relationship between adolescent patients and orthodontists. More attention should be paid to aspects that are often underestimated in clinical practice, thus, influencing the outcome of treatment and patient satisfaction, not only in terms of dental health, but also of mental health.

Case report: This article presents the case of an 11-year-old patient who sought orthodontic treatment, complaining of crooked teeth. In anamnesis, an emotional and physical dependence was reported regarding the act of breastfeeding. Considerations on the diagnosis, psychological implications, and clinical approach in this rare case were addressed. An orthodontic treatment and myofunctional therapy were performed to resolve the occlusal and functional aspects of the patient. Psychological therapy for the child and her mother was required to handle emotional sequelae. Conclusion: Multidisciplinary treatment was prescribed. Dentists should be aware of this need in cases of patients with special characteristics for treatment beyond oral problems. In orthodontics, this may be the difference between effective treatment outcomes or not.

ABSTRACT Aims and objectives To determine the psychological response of the patient to twin-block therapy. To design and formulate a questionnaire to patient on twin-block therapy in the orthodontic clinic and the result will be evaluated. Materials and methods A total of 14 patients within the age group of 12 to 14 years on twin-block therapy for treatment of skeletal class II malocclusion with a mandibular deficiency in the Department of Orthodontics were selected. A questionnaire was formulated at the end of twin-block therapy. The completed questionnaire was analyzed and the patient responses to the different aspects of twin-block treatment were recorded. Results Of the 14 patients, 12 patients liked wearing the appliance. All the patients wore it comfortably full time. About 3 out of 14 had pain, 1 had ulcer, 9 14 had difficulty in eating, 6 patients had altered taste, and 3 had altered speech. A total of 13 patients observed improvement in lower jaw position. A total of 13 patients considered recommending the appliance to families and friends. Conclusion The response of the patient to twin-block appliance was positive. The twin block appears to be well accepted by the patients despite their age. Clinical significance Patient compliance directly affects the treatment outcome of the appliance. Summary Background Orthognathic surgery involves a change in the patient’s functional and aesthetic aspects. Objective The objective was to answer the following focused question: what is the impact on quality of life (QoL; aesthetic, function, social, and psychological aspects) in patients undergoing orthodontic-surgical treatment? Search methods Appropriate word combinations and truncations were selected and tailored specifically for each electronic database: PubMed/Medline, Scopus, Web of Science, PsycInfo, and Latin American and Caribbean Health Sciences Literature and gray literature. Selection criteria Studies that met the following criteria was included: patients with dentofacial deformity (P); surgical correction through orthodontic-surgical treatment (E/I); before orthodontic-surgical treatment or patients with no dentofacial deformity (C); QoL (O); cross-sectional, cohort, case-control and randomized or non-randomized clinical trial (S). Data collection and analysis In phase 1, two reviewers independently reviewed the titles and abstracts of all references. All articles that did not meet the inclusion criteria were excluded. In phase 2, the same reviewers completely read the selected articles independently. Results A total of 2879 articles were retrieved during the final database search. Fifty-two articles were selected for full reading, of which 16 were excluded, resulting in 36 included articles. The meta-analysis was performed using 13 of the selected studies. When comparing the period before any treatment with the post-operative period of 4–8 weeks, there was an association only for facial aesthetics (mean difference = 3.00; 95 per cent confidence interval = 1.10–4.89; inconsistency index = 63 per cent). The comparison between the period before any treatment with the 6 month post-operative period showed an improved QoL in all of the domains evaluated and, when comparing data after the orthodontic-surgical preparation (before surgery) and after 5–12 months of surgery, there was also statistical significance with an increased QoL for all of the domains evaluated. Conclusions In conclusion, based on the results of this systematic review and meta-analysis, the evidence suggests an improvement in the QoL of patients undergoing orthodontic-surgical treatment regarding aesthetic, functional, social, and psychological aspects. Introduction: This article proposes a standardized method for myofunctional evaluation and treatment for patients undergoing orthognathic surgery. Patients selected for surgery typically exhibit a malocclusion or facial disharmony due to abnormal bone development. [excerpt] <br />Conclusion: Orthognathic surgery, together with orthodontic treatment to align the teeth, provide a harmonious maxillomandibular relationship. Surgery and orthodontics also improve the relationship between the bones and soft tissues. There is an anatomical and aesthetic improvement. However, function in itself is not modified. If the individual has hypotonic lips and tongue thrust, these conditions will persist after surgery. The anatomic harmony of the face will improve function, but it will not solve the problem completely. Myofunctional therapy adds a new dimension to orthognathic surgery. The functional aspects have begun to receive their due value. Some surgical techniques have also become revised in view of a new understanding of physiology, and the once common relapses due to muscle imbalances can now be avoided. The main goal of myofunctional therapy for patients undergoing orthognathic surgery is to restore normal function and to obtain muscular static and dynamic equilibrium. A successful stable outcome from the surgical, orthodontic, and myofunctional treatments has a great impact on the patient's psychological well-being. Myofunctional therapy's relationship to orthognathic surgery is a field ripe for future research.

This study analyzed the academic, sociodemographic, and labor conditions related to the quality of life (QOL) of orthodontics postgraduate students in Colombia. A mixed study (explanatory sequential design) was conducted. An online cross-sectional survey (n = 84; 64.3% females) was carried out with sociodemographic, academic, social support, health, labor, and QOL (WHOQOL-BREF) variables. Descriptive, bivariate analyses, and multivariate linear regression were performed. Focus groups (FGs) delved into aspects of relevance regarding QOL and determinants, through qualitative content analysis and triangulation of information. The median score in the four WHOQOL-BREF dimensions surpasses 50 points, with the highest score being in the psychological dimension (62.5 ± 16.7). According to the multivariate linear regression models, the variables significantly associated with QOL scores were playing sports, being married/living together, normal BMI, low social support, and medium/low socioeconomic status. The qualitative results explained the determinants of QOL in the personal, academic, and social dimensions of the participants. The discourses showed that the postgraduate course represents a resignification of their life, where their QOL is affected by the difficulties of their academic development, by the difficulty of reconciling the personal academic load with their affective, work, and social life, and by the stress they experience in their staff process. In conclusion, the participants’ QOL was moderate and affected by different factors. The findings highlighted the importance of mental health promotion and well-being strategies in students of orthodontic postgraduate training programs in Colombia for improving QOL.

Health indices are important to measurement of the presence and severity of diseases. The objective of this study was to perform a literature review about malocclusion indices and analyze their applications in the public health field. The research was performed by consulting Pubmed, SciELO, Web of Science, Scopus, Bireme and Embase databases, using the following terms: public health, epidemiological methods, indices, epidemiological surveys, preventive dentistry, malocclusion, and orthodontics. Publications on the development and use of malocclusion indices in clinical and epidemiological studies were included, without restrictions of methodology and language. Fifty-two indices and their variations were identified. Of these, most were intended for individual assessments and their use in public health was difficult due to the requirements for their application, including the need for specialists, plaster models analysis, complementary exams such as cephalometric radiographs and photographs, specific equipment, the need for longitudinal monitoring of cases, and exclusively objective or subjective assessments. Some malocclusion indices present positive aspects and partially evaluate physical, functional, psychological, and social conditions. However, it is still challenging to find a unanimous index that fulfills the requirements for assessing the severity, treatment need, and impact of malocclusions on the individual’s quality of life with applicability in public health.

Relevance. Previous studies have shown an improvement in orthodontic patients’ health-related quality of life. It is interesting to study the effect of various orthodontic appliances on the patients’ quality of life. Purpose – to study the changes in dental parameters of the quality of life in patients with malocclusion before orthodontic treatment, during treatment with metal and ceramic vestibular braces and after treatment, using removable retainers.Materials and methods. Using a validated Russian version of Oral Health Impact Profile (OHIP-49RU), we surveyed the quality of life in four patient groups: 1 - with malocclusion (n = 35), 2 – with removable retainers (n = 32), 3 – with metal brackets (n = 30), 4 – with ceramic brackets (n = 33). Statistical processing was carried out in Statistics 10. The study used descriptive statistics methods, e.g., nonparametric test methods determined the significance of sign differences: using the Kruskal-Wallis H, z tests and the χ2 test with Yates' correction.Results. OHIP-49 score analysis revealed statistically significant differences among the studied groups of pa- tients (1 - functional limitation (H = 9.15; p <0.05), 2 - physical pain (H = 13.98; p < 0.01), 3 - psychological discomfort (H = 23.04; p <0.001), 4 - physical disability (H = 24.73; p <0.001), 5 - psychological disability (H = 11.03; p < 0.05), 6 - social disability (H = 16.21; p <0.01), 7 - handicap (H = 7.91; p <0.05)).Conclusions. The patients, who had completed the orthodontic treatment and used removable retainers at night, demonstrated the best score in most sections of the questionnaire and the patients with malocclusion, who had presented to an orthodontist - the worst. Patients with the brackets had a higher score on the "Physical pain" section of the questionnaire (6.5 (3.0-8.0) with metal braces and 4.0 (2.0-7.0) with ceramic bracket system). The quality of life in patients with metal and ceramic brackets did not have statistically significant differences.

OBJECTIVE: The purpose of this study was to analyze the psychological aspects of adult patients who sought and underwent orthodontic treatment, evaluating their expectations and discomfort during treatment, as well as their satisfaction after completion of dental movement. RESULTS AND CONCLUSIONS: Data obtained from previous published papers, and also from questionnaires answered by 54 patients, showed that adult patients stood out for their attention to details and high interest in the esthetic improvements provided by treatment, and also for a greater perception of their initial malocclusion. On the other hand, the same data showed that adult patients, once informed about the limitations of their treatment and having confidence on the orthodontist, presented a high level of satisfaction with treatment results, revealing themselves as good patients for indication and execution of orthodontic procedures.

Orthodontic treatment, especially during the period of active growth, can significantly affect the development of the jaws and teeth. Treatment with extraction of molars is more complicated because the space that needs to be closed is larger than the premolar spaces and the time of treatment will be longer. However, in cases where we have a lack of space, teeth protrusion or dental asymmetry, and the extraction of permanent teeth is indicated, the damaged molars should be the first option for extraction when premolars are in better condition. The third molars in different ways are affected by orthodontic therapy. One of them is orthodontic treatment by applying the extraction of the third molars, this situation being widely explored. Extraction of third molar for orthodontic reasons is justified when it prevents the eruption of the second molar or otherwise affects the other teeth. The indications of extraction and selection of teeth to be removed depend on the specific aspects of malocclusion. Premolars are teeth that are usually extracted from orthodontic reasons, but this is not the rule and depends on individual treatment needs. The decision to correct the malocclusion with teeth extraction should be based on the ratio of the size of the dental arch with the size of the teeth, facial appearance and other factors arising from the diagnosis itself. Indications for extraction of the third molars during orthodontic treatment are: mandibular incisor crowding, if they in some way constitute a obstruction of orthodontic treatment as well as preparation for orthognathic surgery. The success of an orthodontic treatment involving the extraction of molars is directly related to the patient complaints and psychological profile, the absence of diseases and skeletal discrepancies, the professional experience as well as the fact whether the patient underwent previous orthodontic treatment with extraction. Patients with partially or completely non-erupted teeth are significantly more likely to have certain consequences: before, during or after the orthodontic treatment. Therefore, orthodontists need to raise patient awareness of these consequences in the context of orthodontic treatment.

Background: The influence of orthodontic treatment on self-confidence and quality of life is a major reason for patients to seek treatment. This study was designed to assess the relationship between orthodontic treatment need and psychosocial impact of dental aesthetics in patients seeking orthodontic treatment. Methods: This descriptive analytical cross-sectional study was performed on 165 patients referred to orthodontic clinics in Kerman to start treatment. Patients were selected by census method to reach the specified sample size. The data collection tools included 3 forms: demographic information form, Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ) and a form for recording information obtained from clinical examination to determine orthodontic treatment need based on Index of Orthodontic Treatment Need (IOTN). Data analysis was performed through SPSS 22 and using t test, ANOVA, and linear regression at a significance level of 0.05. Results: The educational level of subjects and their parents and economic status showed no significant relationship with total PIDAQ score and any of its domains (P>0.05). In psychological impact domain, women’s score was significantly higher (P=0.016) and there was a significant relationship between the treatment need based on aesthetic component (AC) and the domains of dental self-confidence (P=0.003), social impact (P=0.049) and psychological impact (P=0.066), as well as the total score of questionnaire (P=0.012). Treatment need based on dental health component had no statistically significant relationship with PIDAQ score (P>0.05). Conclusion: AC of IOTN had significant relationship with dental self-confidence, social impact and psychological impact as well as the total score of PIDAQ. In orthodontic treatments, in addition to malocclusion, the psychological aspects should also be considered.

Background. COVID-19, a viral infection, has negatively impacted the physical and psychological health of the people worldwide. This was a descriptive, cross-sectional study, which aimed to investigate orthodontic postgraduate residents’ knowledge regarding COVID-19, its association with anxiety around quality of their training, and performance of routine orthodontic procedures during the pandemic. A questionnaire was circulated online through WhatsApp to the orthodontic residents enrolled in the four-year postgraduate training program across the country which comprised the following sections: demographic data, questions to test knowledge of COVID-19, self-grading of anxiety around performing routine orthodontic procedures, academic aspects of the training program, and residents’ thoughts on the current status of their training program in general during the pandemic. The residents were grouped based on their awareness on COVID-19, with >70% score taken as having adequate knowledge and ≤70% as inadequately updated. The association between COVID-19-related knowledge and the anxiety level of the residents experienced during training was assessed. Results were analyzed using SPSS (Statistical Package for Social Sciences) version 23.0. Counts and percentages were reported on baseline characteristics of studied samples. Descriptive methods were used to report the outcomes of this survey on knowledge of COVID-19, anxiety around performing orthodontic procedures, and different academic aspects of training. Statistical significance was set at p < 0.05 using the Pearson chi square test. Results. Most of the residents (51.5%) were adequately updated about the COVID-19 virus. Increased awareness was associated with more anxiety about the following: contracting the virus ( p < 0.01 ), performing different orthodontic procedures ( p < 0.05 ), disturbed patients’ appointments ( p < 0.01 ), timely completion of their cases ( p < 0.01 ), and exam’s preparation ( p = 0.04 ). The group with <70% COVID-19 related knowledge opted for extension of their training period (51.8%). Conclusion. COVID-19 related knowledge was clearly associated with anxiety experienced by the orthodontic residents during training in the pandemic. Awareness regarding the infection led to more anxiety around working during the pandemic, preparing for postgraduate exams, and concerns about its negative influence on the overall quality of the training program.

The high prevalence of dental anomalies and deformities requires a careful approach to orthodontic treatment, especially to improve quality of life. The purpose of the study was to determine the effectiveness of orthodontic care by index assessment of the results of correction of dental anomalies and determination of the psychosocial profile in adolescents of the Transcarpathian region. Material and methods. The object of the study were 68 adolescents from the Transcarpathian region. Determination of the correction of dental and maxillary apparatus anomalies was performed by calculating the relevant part of the ICON index, calculating the evaluation of the results. The quality of life of adolescents was assessed using the OHIP-14 quality of life questionnaire. Statistical analysis was performed after consolidation of results using the application package Statistica 10.0 (StatSoft, Inc., USA) and Microsoft Office Excel 2010. Results and discussion. Many studies in the field of psychology have determined that self-esteem is a core characteristic of the individual and together with the level of claims is a regulator of human activity and significantly affects its development. The result of self-esteem depends on how a person evaluates his/her success in joint activities where he/she is a member. This is especially true for teenagers and adolescence. The most important aspect of impaired quality of life in patients of this subgroup were problems of psychological and social nature, issues of external attractiveness, which prevailed even over the physical aspects of the impact, which is quite typical of adolescence. In patients with moderate orthodontic treatment there was a statistically significant improvement on the scales of psychological discomfort (from 6.4±0.6 points to 3.96±0.6 points; p <0.05), psychological disability (from 6.5±0.5 points to 2.18±0.5 points; p <0.05) and social disability (from 5.4±0.5 points to 3.36±0.5 points; p <0.05). Patients in this group after solving problems related to the aesthetics of the smile, as well as discomfort during communication, felt greater psychological comfort, relief in interaction with others and peers. There was a noteworthy shift of accentuation in such patients from problems with appearance to study and active social life. Among patients with severe orthodontic treatment, the most pronounced dynamics was observed on all scales of dental quality of life: on the scale of masticatory dysfunction (from 5.7±1.0 points to 3.12±1.0 points; p <0.05), physical pain (from 7.5±0.7 points to 2.01±0.7 * points; p <0.05), psychological discomfort (from 7.9±0.6 points to 3.48±0.6 points; p <0.05) physical disability (from 7.7±0.5 points to 3.20±0.5 points; p <0.05), psychological disability (from 7.5±0.4 points to 3.41±0.5 points; p <0.05). Conclusion. According to the study results, the success of orthodontic treatment (indicators of severity and effectiveness of orthodontic treatment according to the ISO index) directly correlated with the level of quality of life (r = 0.62; p <0.05)

Введение. Литературные данные показывают, что ортодонтические пациенты в 95% случаев страдают от различных видов боли. Боли у ортодонтического пациента имеют различную природу (эмоциональную, физическую, социальную и пр.) и проявляются в разных сферах – когнитивной, мотивационной и пр. У ортодонтических пациентов присутствие тревоги увеличивает степень проявления боли и понижает ее порог. Приблизительно 30% пациентов отказываются от последующего лечения из-за боли. В последние годы изучается и генетический аспект боли. Установлен ген восприимчивости к челюстно-лицевой боли. Также были выявлены три геногруппы с высокой, средней и низкой восприимчивостью к боли.Материалы и методы. В данную научную работу были включены 58 детей в возрасте 9–12 лет, которые были разделены на 2 группы, статистически одинаковые по возрасту, генам, лицевому развитию, сбалансированному психоэмоциональному состоянию. Пациенты из обеих групп были изучены по методологии традиционного клинического обследования, которое было дополнено нейрофизиологическим обследованием. Был осуществлен сравнительный анализ уровня стоматологической тревоги, установленной Corah Dental Anxiety Scale.Результаты. На основании проведенных исследований можно констатировать важность и необходимость изучения и применения методов диагностики функционального состояния тройничной системы и тройничных рефлексов у детей с патологическим прикусом III класса Энгля. Аргументы исходят из клинико-нейрофизиологических особенностей, выявленных в нашей научной работе.Последующие исследования детей с патологическим прикусом III класса по Энглю должны быть сосредоточены на выявлении соотношения степени проявления тройничных рефлексов с функциональным нарушением челюстно-лицевой и смежных систем (вестибулярные функции, болевые синдромы). Оптимизация диагностики и ортодонтического лечения невозможна без углубленного изучения взаимосвязи челюстно-лицевой системы с тройничной системой и мозговыми структурами. Показательным примером последних лет может послужить применение инновационного препарата, способного воздействовать на нейронном уровне на структуры мозга с целью снижения челюстно-лицевой боли и лечения воспалительных процессов. Introduction. Scientific sources show that orthodontic patients suffer from various types of pain in 95% of cases. Pain in an orthodontic patient has different origin (emotional, physical, social, etc.) and manifests itself in different spheres – cognitive, motivational, etc. In orthodontic patients, the presence of anxiety increases the degree of pain and lowers its threshold. Approximately 30% of patients refuse follow-up treatment because of pain. Recently, the genetic aspect of pain has also been studied. The gene of susceptibility to oral and maxillofacial pain is established. Three genetic groups with high, medium and low susceptibility to pain were also identified.Materials and methods. 58 children aged 9–12 were included in this scientific work. Were formed 2 groups, statistically identical in age, genes, facial development, balanced mental-emotional condition. Patients from both groups were examined by the methodology of a traditional clinical examination, which was supplemented by a neurophysiologic examination. A comparative analysis of the level of dental anxiety established by the Corah Dental Anxiety Scale was performed.Results. As a result, it is possible to underline the importance and necessity of studying and applying methods of the functional condition diagnostics of the trigeminal system and trigeminal reflexes in children with Angle class III malocclusion. The arguments come from clinical and neurophysiologic features identified in our scientific work.Subsequent studies of children with Angle class III malocclusion should focus on identifying the ratio of the trigeminal reflexes manifestation to functional disorder of maxillofacial and related systems (vestibular functions, algic syndrome). Optimization of diagnostics and orthodontic treatment is impossible without in-depth study of the relationship of the maxillofacial system with the trigeminal system and brain structures. A successful example of recent year’s investigations is the use of an innovative drug capable of affecting brain structures at the neural level in order to reduce maxillofacial pain and treat inflammatory processes.

ABSTRACT Anterior crown fractures are common form of traumatic dental injuries that mainly affect the maxillary anterior teeth in children, teenagers as well as adult population. A trauma with accompanying fracture of anterior teeth is an agonizing experience for an individual requiring immediate attention, not only because of the physical disfigurement but also because of the psychological impact on the patient. Over time numerous techniques and materials have evolved for the restoration of the injured teeth, such as, resin composite with or without pin, orthodontic bands, resin crown, and ceramic crowns. Reattachment of fractured tooth fragments can provide good and long-lasting esthetics and function. Patient cooperation and understanding of the limitations of the treatment is important for good prognosis. This article reports management of complicated coronal tooth fracture treated by reattachment on tooth fragments on the palatal aspect and single visit nonsurgical endodontic treatment. IntroductionAccording to Angle's classification, a correct bite should have, inter alia, the following regularities: the median line of the face should coincide with the line between the central incisors of the upper and lower arches, the lower incisors should be overlapped by the incisal edges of the upper incisors, the top of the canine in the upper arch is inserted between the lower canine and the tangent premolar, each tooth is in contact with two opposing teeth, adjacent teeth both in the upper and lower arch are in contact with each other. Any deviation from these rules may indicate the presence of a malocclusion. We can distinguish here, for example: posteroclusion, open bite, retrusive occlusion, protrusive occlusion, supraclusion bite. They seem to be important not only in terms of functionality, but also psychology. Research shows that the correction of mandibular prognathism increases the level of self-confidence and self-acceptance, which may improve the quality of life of patients.Purpose of researchThe aim of the study was to check the quality of life of orthodontic and surgical patients as well as what factors motivate these people to start treatment in the area of the masticatory system.Material and methods208 people aged 18 to 55 participated in the survey. They were both people with malocclusion and no malocclusion, who constituted the control group. The questionnaire was created on the basis of standardized OQLQ and OHIP-14 questionnaires. It also included questions about the factors that motivate to start treatment in the masticatory system.ResultsThe most common malocclusion among the respondents was progenia (58.8%), followed by retrogenia (34.3%), then open bite (4%) and posteroclusion (2.9%). According to the OHIP-14 form, statistically significant (p <.0001) higher quality of life was seen in people without a masticatory organ defect, compared to people with the malocclusion. A statistically significant difference (p <.0001) between people with present or recent malocclusion (M = 54.29, SD = 16.71) and people without malocclusion (M = 31.93, SD = 18.31) was also visible in the study with using the OQLQ questionnaire.ConclusionsA malocclusion worsens the comfort and quality of life.Incorrect bite, face and smile aesthetics as well as psychological aspects are the most important factors motivating to undertake surgical and orthodontic treatment.Bruxism and facial pain are not factors prompting the initiation of treatment of malocclusion.

Врожденные расщелины лица относятся к наиболее тяжелой по своим последствиям патологии челюстно-лицевой области. В работе приведены данные о возрастающей актуальности реабилитации детей с расщелинами губы и неба. Целью исследований было изучение влияния фактора кооперации на эффективность реабилитационных мероприятий. Разработана анкета, позволяющая оценить влияние указанного фактора на эффективность реабилитационных мероприятий. В результате анализа данных опросов выявлено противоречие между стремлением как можно раньше завершить процесс реабилитации и желанием как можно дольше пользоваться льготами, предоставляемыми государством семьям с больным ребенком. Анкета показала достаточную валидность для определения взглядов родителей на медицинские, психолого-педагогические и социально-экономические аспекты реализации индивидуальных программ реабилитации. Большинство родителей считают, что ортодонтическое лечение должно быть закончено в более раннем возрасте (до школы). При этом они отмечают определяющую роль родителей в выработке у ребенка устойчивого желания лечиться. Процесс полной реабилитации детей родители считают возможным продлить до поступления в профессиональные учебные заведения. Сделан вывод о необходимости повышения эффективности работы психолого-педагогической службы в системе комплексной реабилитации детей с расщелинами неба и губы Congenital clefts of the face are among the most severe pathology of the maxillofacial region in terms of their consequences. The paper provides data on the increasing relevance of the rehabilitation of children with cleft lip and palate. The aim of the research was to study the influence of the factor of cooperation on the effectiveness of rehabilitation measures. A questionnaire has been developed to assess the influence of this factor on the effectiveness of rehabilitation measures. The analysis of survey data revealed a contradiction between the desire to complete the rehabilitation process as early as possible and the desire to use the benefits provided by the state to families with a sick child for as long as possible. The questionnaire showed sufficient validity to determine the views of parents on medical, psychological, pedagogical and socio-economic aspects of the implementation of individual rehabilitation programs. Most parents believe that orthodontic treatment should be completed at an earlier age (before school). At the same time, they note the decisive role of parents in developing a stable desire for treatment in a child. The parents consider it possible to extend the process of full rehabilitation of their children until they enter vocational educational institutions. It is concluded that it is necessary to increase the efficiency of the psychological and pedagogical service in the system of complex rehabilitation of children with cleft palate and lip

ABSTRACT Objective: To assess the impact of facial profile on young adults’ oral health-related quality of life (OHRQoL) item levels. Methods: A cross-sectional study was carried out with a population-based sample of 205 young adults, with a mean age of 23.1 years. The individuals answered questions about OHRQoL (OHIP-14) and self-esteem (Global Negative Self-Evaluation). The Dental Health Component (DHC) of the Index of Orthodontic Treatment Need (IOTN) was used to evaluate normative orthodontic treatment needs and define dental malocclusion clinically. Facial profile was analyzed using photographs and dichotomized into two levels: normal (straight) and altered facial profile (convex or concave). A calibrated researcher performed the clinical examination. Association between the independent variables and the outcome (OHRQoL) was established by hierarchical multiple linear regression analysis for each item level. Considering the variable of interest (facial profile), the psychological incapacity domain was the most affected item. Results: Individuals with changed facial profile had 2.47 (1.04-5.85) times higher chances of reporting impacts on psychological incapacity than those with a normal profile (p> 0.05). The association was modulated by dental malocclusion and self-esteem. Conclusions: The convex and concave facial profile showed a negative impact on the psychological aspects of young adults’ quality of life.

Abstract Background COVID-19 has impacted the care of patients undergoing orthodontic treatment. We aimed to provide an overall view of patients’ perspectives, concerns, and expectations towards their treatment throughout the clinic lockdown during the pandemic; and to assess patients’ levels of mental distress and its association with their confidence in resuming care. Methods An anonymous, validated, in-person paper questionnaire was distributed to adult orthodontic patients’ post-lockdown at an academic institution. The survey focused on the clinical aspects and patients’ perspectives regarding orthodontic treatment during the pandemic. The Kessler Mental Distress Scale (K10) was used to evaluate their psychological status. Survey responses were descriptively summarized and confidence in resuming care was compared between normal patients and patients with mental distress using Mann-Whitney tests. Results One hundred fifty-four patients were surveyed from July to October 2020. Mean age of the participants was 29.30 (SD = 12.01) years and 62% were females. Emergencies during the closure (21%, 32/154) involved primarily irritation with protruding wires. Patients were neutral regarding tele-dentistry and preferred their current fixed appliances over clear aligners. Upon resuming care, 80.51% were extremely pleased with the restrictive protocols and with high level of confidence in resuming treatment. The average level of anxiety was low, and a modest association was found between mental distress and reduced confidence in resuming treatment. Conclusions Few numbers of minor emergencies occurred during the clinic closure. Despite the rising interest in tele-dentistry, patients were neutral on considering this option to monitor treatment and were content with fixed appliances. Patients had high confidence levels to resume their care based on the protocols established upon reopening. The association of mental distress and confidence in resuming care is suggestive and needs further investigation.

ABSTRACT Objective: The objective of this article was to evaluate the effect of alpha binaural beat music on pain level after initial placement of a maxillary fixed appliance, compared to music without binaural beats (placebo) and no music (control). Methods: 60 patients undergoing maxillary fixed orthodontic appliance and initial archwire placement were randomly allocated into the three aforementioned groups. The pain level experienced was monitored for the following seven days, using the short-form McGill pain questionnaire (SF-MPQ). Results / Descriptors: Intensity of both sensory and psychological aspects of pain reduced significantly in the binaural beat music (BBM) group, compared to the control, after the 5th day. Statistically significant lower affective and total pain scores were also found on day 6 for the placebo group, compared to the control. Present Pain Intensity (PPI): Statistically significant lower scores were found between the BBM group and the control group from days 3 to 7. Statistically significant lower scores were also found between the placebo and the control groups on days 4, 5 and 6. Visual Analog Scale (VAS): Compared to the control group, the placebo group had a lower VAS score on day 4, and the BBM group had lower scores on days 6 and 7. Conclusions: There was a significant reduction of pain demonstrated in the BBM group, compared to the control, toward the end of the first week of treatment. There was no difference in reported pain between the BBM and placebo groups for any of the scores.

Higher parents’ satisfaction was found in the unilateral cleft lip + alveolus (UCL + alveolus) group, compared with the unilateral cleft lip and palate (UCLP), regarding cleft-associated features, and compared with cleft palate (CP), regarding non-cleft-associated features ( P < .05). Greater parents and orthodontist satisfaction were found when parents’ education was >12 years. Parents of Jewish ethnicity showed higher satisfaction, compared with parents of non-Jewish ethnicity. AbstractObjectiveTo test the validity and reliability of the Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ)-Albanian version in Kosovar adolescents and assess gender-based differences of malocclusion on PIDAQ subscales.MethodsA sample of 200 adolescents aged 12–15 years were evaluated for orthodontic treatment need by a previous calibrated dental examiner, using the aesthetic component (AC) of the Index of Orthodontic Treatment Need-Aesthetic Concern (IOTN-AC) and the dental aesthetic index (DAI). Internal consistency, test-retest reliability, discriminant validity and gender-based impact of malocclusion on psychosocial impact was assessed.ResultsThe Albanian version of the PIDAQ shows satisfactory internal consistency (α ranging from 0.73 to 0.90) with high test-retest reliability (r > 0.83). The discriminant validity revealed that subjects with minor and severe malocclusion according to dichotomous DAI categorization had different PIDAQ scores. No significant gender-based differences were noted on the impact of malocclusion on psychological and social aspect or Dental Self-confidence (DSC); however, significant gender-based difference was noted on the impact of malocclusion on AC.ConclusionThe Albanian version of the PIDAQ demonstrated good psychometric properties and applicability to this age group in Kosova.

AbstractIndividuals with dentofacial deformities often display a low quality of life (QoL) through biological mechanisms that remain unraveled. In this case–control study, the salivary levels of cytokines, glutamate, and kynurenine metabolites were assessed in patients undergoing orthognathic surgery (OS), while correlating these parameters with QoL and psychological symptoms. Thirty-six patients were enrolled in control (under orthodontic treatment) and test (undergoing OS) groups, matched by age and sex. The QoL was assessed through the World Health Organization Quality of Life BREF (WHOQOL-BREF) and the Orthognathic Quality of Life Questionnaire (OQLQ). The psychological symptoms were evaluated by the Satisfaction with Life Scale, the Rosenberg Self-Esteem Scale (RSES), and the Depression, Anxiety, and Stress Scale-21 (DASS-21). The salivary levels of IL-1β, IL-6, IL-10, glutamate, and kynurenine metabolites were evaluated. The OQLQ demonstrated increased QoL scores in the test group, regarding social aspects, facial esthetics, and function domains, without significant differences in respect to the other surveys. These patients displayed higher IL-1β and glutamate levels; conversely, the kynurenine metabolites were unaltered. The glutamate levels positively correlated with the OQLQ function scores. The data brings novel evidence about the psychobiological features of patients with dentofacial deformities, showing salivary variations of inflammatory biomarkers in these individuals.

Dental malocclusion can affect the psychological and social aspects of adolescents. However, the knowledge of this condition has been limited in the Malaysian population. This study aims to explore the psychosocial impacts of dental malocclusion among adolescents in Kedah state, Malaysia. A qualitative study was conducted at two public specialized dental clinics. Focus group discussion (FGD) and in-depth interview (IDI) were conducted among adolescents aged between 11 and 19 years, who received orthodontic treatment for moderate to severe dental malocclusion. All the FGD and IDI sessions were video-recorded, transcribed verbatim, and translated into English. The thematic analysis method was used for the data analysis. A total of 32 adolescents participated in six sessions of FGD and five sessions of IDI. Four themes emerged from the data analysis: (i) pressure to seek treatment, (ii) negative feelings about one’s dentofacial image, (iii) negative influences on interpersonal relationship, and (iv) negative impacts on school performance. The findings indicate that adolescents with dental malocclusion had been receiving pressure from different parties and generally had low self-esteem, which eventually affected their relationship with family and friends and their involvement in school activities. Therefore, public education and interdisciplinary collaboration to address their psychosocial needs are required.
