Testo completoAim: This study aimed to improve knowledge about and illuminate the phenomenon of sexual orientation discrimination (SOD), particularly as perceived by homosexual men in a male-dominated workplace such as the Danish Armed Forces. We also sought to increase understanding of how such discrimination affects those individuals. Moreover, wesought to increase knowledge about and highlight the challenges and problems regarding SOD in the context of coping strategies. Method: We used transcribed semi-structured in-depth interviews as our survey tool. To analyze our results, we used open, axial, and selective coding, as described in grounded theory. Results: Data analysis allowed us to identify minority power (MP) as a core variable. Further, MP is sourced in the four main categories: stressors, coping, environment, and identity, which are elements of the core process. The MP model that emerged from our results illustrates the core process. Conclusion: The study shows that homosexual men use a complex range of coping strategies in different situations where SOD occurs. Despite the negative minority stressors experienced by homosexual men in the Danish Armed Forces, the subjects showed a unique sense of coherence—meaningfulness, comprehension, and manageability—that leads to MP. To benefit the entire population, this study emphasized the need to include questions about sexual orientation in public health research. The study shows that training about the special social conditions for the LGBT population, a diversity policy, and recruitment campaigns aimed at homosexuals result in greater competence among all individuals in the workplace. Preventing discrimination based on sexual orientation requires a salutogen perspective and multifactorial strategy on an intersectorial and interdisciplinary level without placing all responsibility on the individual

Testo completoThe length of stay at hospital after surgery has shortened. Little is known about the immediate post-surgery period. The aim of this study is to assess whether spouses have to take active care of their husbands and to assess if the burden of other part of the health service increases as a result of early discharge after prostatic surgery. Furthermore, the study intends to increase the knowledge about how patients experience the first postoperative weeks. Initially qualitative interviews were conducted with ten spouses. The Phenomological method described by Giorgi and Karlsson was used for analysing the interviews. Based on the experience of this, a questionnaire was prepared and sent to all spouses of patients who had undergone prostatic surgery between November 2004 and May 2005. The results of the questionnaires were put into an access database. The principal finding is that many patients during the first postoperative period experience various urinary problems such as incontinence, frequency, haematuria, and urinary retention. Problems which negatively impacted on the spouses. Furthermore, the spouses were uncertain whether it was common to experience these problems, and they did not know whom to contact when in need of support

Testo completoAim: This study explored and describedthe factors that may influence how patients react to source isolation from others during hospitalization. The study also sought to determine how background variables such as gender, age, and previous hospitalization affect source isolation. Based on individuals’ risk assessment, this study also focusedon how hospitalsplan and the organization of care and treatment. Method: This qualitative study used content analysisto reviewd ata collected from interviews with five patients. The conceptual framework describes antibiotic resistance and infection control from a public health perspective and exploredits prevention in Denmark. Thetheoretical framework describe show patients experiencean infection acquired by exposure to drug-resistant bacteria, as well assubsequent source isolation. Results: Thelimited space of an isolation room, including closed doors, lack of contact with people, and few sensory stimuli, resulted in patient boredom, monotony,and anxiety. Moreover, the data showed that background variables affected how patients experience source isolation. Compared with men, women showed greater concern about precautions against infection and greater awareness of staff behavior. Women also worried more about the risk of transmitting bacteria/disease to visitors and familymembers, and display more emotion during isolation. In contrast, men outwardly resigned themselves to the situation and didnot speculate about infection precautions. Men had more rational approach, and tended to cope better when isolated in a single room. Younger patients seemed to have a better coping strategy during isolation, and considered a single room an advantage compared to the ward. Elderly patients felt sad and lonely during source isolation. In addition, previous negative experiences from earlier hospitalization seemedto influence current isolation. Patients developed their own strategies for coping with source isolation and found themselves balanced between being stressed and coping. Conclusion: Hospitals need more alternatives (e.g., better training and improved treatment culture) to prevent negative psychological affects due to isolation without compromising infection prevention. Hospitals should update their personnel at all organizational levels, and focus on room facilities in the ward, contact time,and improved information and communication. Riskassessment should be individualizedfor each patient.

Testo completoThis study aimed to assess how parents of children with spastic Cerebral Palsy (CP) evaluated the efficacy of Botulinum Toxin A (BTX) treatment on their child. Method: Triangulation of measurement processes, i.e., qualitative assessment including two focus group interviews with experts and three interviews with parents; quantitative assessment involving questionnaires send to parents of 47 children treated with BTX at Hilleroed Hospital. The qualitative data were analysed using content analysis. Results: Focus group interviews indicated that questionnaires should seek detailed information about the daily activities of children undergoing BTX treatment, beginning treatment initiation and continuing until BTX effect declined. Parent interviews showed that two out of three would allow another BTX injection; waiting time on treatment day was too long; and hospital routines should be optimized to meet the needs of disabled children. Forty-one parents (12 fathers and 29 mothers) completed the questionnaire. The study included 30 children receiving BTX between 2004 and 2007. Parents described the best effect of BTX as reduced spasticity. Walking ability improved in 67% of children who could walk before treatment, and improved motor skills provided new ways of moving. Twenty-two parents believed that BTX treatment fulfilled doctors’ expectations. Maximum BTX effect occurred after three to six months. Effects differed with each treatment, mostly on a declining scale. Parents felt very welcome in the children’s ward and also felt that the staff listened to them; their overall assessment of treatment day experience was good. Major concerns included waiting time and a missing meal following treatment. Views of healthcare services for children with CP did not differ between fathers and mothers. The study indicates that development of national guidelines for BTX treatment will be highly recommended. Conclusion: The majority of parents agreed to repeat BTX injections, mainly because outcome was positive or met their expectations. A minority would refuse future BTX treatment, primarily due to lack of effect or because of significant problems for small benefit. Optimizing hospital routines on treatment day would help disabled children navigate BTX treatment more successfully

For society,the carrierembodiesa reservoir and a risk of spreading resistant bacteriafrom the community into hospitals, resulting in problems with the treatment of patients in intensive and acute care wards. Purpose: This qualitative study aimed to deepenknowledge of the impactof MRSAon the Sense of Coherenceand Healthfor carriers living at home. Method: Thisqualitative study used Grounded Theory to analyze interviews with twelve carriers and four relatives. Results: The carriers describedthe experience of colonization with MRSA as a strange,unclear,and confusing state between sickness and wellness. The studyidentifiesacore category(i.e., “Inadequate information”)that causes emotional reactions among carriers and family members, including anxiety, insecurity, confusion, inadequate compliance, worries aboutthe future,and stigmatization. Increased stress negatively influences carriers’ and family members’sense of coherence. This was especially true infemale carriers and relatives who work in health and social carebecause they risk becominga health risk to their patients or clients. Conclusion: Thisstudy reveals inadequacy in the Primary Health Care System regarding the quality of information provided to MRSA-carriers and their relatives. Such inadequacynegatively affects carriers’Sense of Coherenceand health. Lack of knowledge in the network and local area can stigmatize MRSA carriers, making them susceptible tosevere bullying

Testo completoBackground: Inpsychiatry, development is characterized by a desire for efficient treatment. Establishment of Assertive Community Treatment (ACT) was created in recognition that psychiatric hospitals have not adequately been able to restore health. Based on the chronic mentally ill, this study focused on the Outreach Psychosis Team by studying the working method ACT. Health and social services are increasingly willing to accept ACT as an organized and functional method for prevention and health promotion. Mentally ill people need a working model in which a competent assistant can support the patient in comprehending everyday life and form a meaningful basis for dealing with life. Aim: This study aimed to gain more knowledge about how ACT teams in Denmark operate in a hectic schedule, and determine whether teams adhere to the ACT model. Method: The study highlighted ACT intervention using a structured electronic survey and subsequent qualitative content analysis. However, most of the collected quantitative data was rejected due to a low response rate . Results:The study included 72 respondents, 31 working in Region Hovedstaden, 12 in Region Sjælland 11 in Region Syddanmark, 2 in Region Midtjylland, and 16 in Region Nordjylland. ACT teams in Denmark include 33 teams in Region Hovedstaden, 1 in Region Sjælland, 8 in Region Syddanmark, 11 in Region Midtjylland, and 4 in Region Nordjylland. The teams work differently and with a different composition and function than the original US model, but they have proven their functionality for more than 10 years. ACT includes salutogenesis in its working methods, and focuses on resources and on improving health. Moreover, ACT is proactive and assertive in dealing with clients. Conclusion: Psychiatry must treat clients wherever they are intheir own lives with offers that are available, consistent, and meaningful to each person on his way to a good life without or in spite of illness. ACT in Denmark has shown that it considers the needs of both clients (i.e., working psychiatry and society), and the working method efficiently reduces the complexity of health care and social work. Team members appreciate the working method. Members of Danish ACT teams are capable of working in teams, and they experience interdisciplinary collaboration as meaningful and manageable. Because working in an ACT team is attractive, it is possible to recruit competent members. Services provided by ACT teams include prevention, care, treatment, rehabilitation, an increased sense of accessibility, coordination and continuity, and decreased complexity. The Danish ACT teams do not strictly adhere to the original ACT model, but rather have adapted to Danish conditions and limitations. Abroad, ACT reduces complexity and ensures greater consistency in mental health work. They do not have complete authority. Currently, ACT is being developed and adjusted to a Danish context

Testo completoThe aim of this MPH thesis is to describe the experience of elderly people who have completed an eight-week intervention walking project. The insight and knowledge acquired in this study have been very useful with regard to the planning of walking for the elderly people who would not otherwise have ventured outside. A qualitative method has been used. The population consists of the eleven projectparticipants who completed the eight-week intervention period. The theme-based interviews have been transcribed in full and analyzed in accordance with a qualitative thematic analysis inspired by Giorgi’s guidelines and Husserl ́s philosophy. The project participants` comments demonstrate that walking as intervention gave them the experience of doing something, especially for their own wellbeing. Other important results were a sense of increasing mastery of their situation ,improved physical fitness, moreenergy, higher spirits a sense of fellowship, and some knowledge of the surroundings and nature. The project includes a new dimension: People who would not otherwise have ventured outside now do so because they get support either by walking alone and experiencing the "obligation" of walking or by walking with a companion. Offering walks to elderly people who would not otherwise have ventured outside seems to be a good decision in a public health care perspective because these elderly peoplepresumably live longer with a better health.
