Background: The placebo effect and the placebo response is a misunderstood phenomenon and their potential is wildly underestimated. Most people, including health care professionals, consider the placebo effect to be a “make believe” effect which only affects gullible people. It is argued that the nurse is in an unusually advantageous position to observe and facilitate the placebo effect and make use of the benefits for the patients. Observation of said response is made by many authors but there is little to no research on the placebo effect from a nursing standpoint. Some of the research results can however be translated to the nurse profession and used in clinical settings. Aim: The purpose is to describe areas where the placebo effect may affect nursing of the patient. Method: A literature review format was chosen to comprise existing knowledge on the placebo effect, which can be applied for nursing. Results: Themes that arose from examination of chosen literature included: Emotional state, Expectation and Conditioning, Conscious and Social learning and Patient-caregiver relationship. Many of the observed interventions could be applied using the nursing philosophy of Hildegard Peplau. Conclusion: The placebo effect can be used by the nurse to improve the result of the patient´s treatment. If placebo becomes a focus in nursing research the results could be more applicable to benefit patients than if researched by other professional groups.

Stroke is a leading cause of serious long-term disability in the United States. The neurological insult following a stroke may leave the survivor with a chronic illness encompassing a lifetime of recovery. Recovery for the stroke survivor entails more than the return of function. A synopsis of the literature indicates that there are three domains of stroke recovery: physical, psychological, and social. There are six categories that comprise the three domains: cognition, function, health perception, self-concept, relationships, and role change. Stroke is a multifaceted and complex disease. Individual aspects of stroke recovery do not occur in isolation and cannot be separated from one another. In the future, studies involving the integration of the domains of stroke recovery are needed to understand the interactive processes that support recovery.

Death is an inevitable experience for each individual. Although death is a natural human experience, the avoidance of death is an issue in the culture at large. This isolation of death and dying also is evident in the health professions of nursing and medicine. Despite the fact that death and dying has received considerably more scholarly attention over time, relatively little attention has been given to the topic of death education. This study explores the literature on death education within nursing and medicine from the 1970s onwards. Using an integrative literature review, scholarly articles were reviewed to determine how death education is enacted or made real in the health care environment, particularly in the nursing and medical curricula. This study examines how educators have taught the topic of death and dying over time and how an analysis of these past experiences may inform current education on death related concepts within health professional fields. The lack of attention to death and dying in nursing and medical curricula affects the confidence and competence that health care professionals have in managing these situations. Although death is a common occurrence in the health care environment, there remains a gap in how educators are supported to teach these concepts to students. The analysis of scholarly literature from the 1970s until the present reveals three themes. The avoidance of death in the culture at large, the importance of psychosocial aspects of care, and the lack of support for educators are three over-arching themes. The main recommendations for educators teaching death related topics appearing in the literature reviewed center on ensuring student contact with terminally ill and dying patients in the clinical setting as a way to learn about the dying process, use of simulation based learning, encouraging collaboration amongst the interdisciplinary team to meet patient and family needs, and the use of explicit competencies related to end-of-life care to ensure consistency amongst all students. The findings from this review are relevant for student learning, educator preparation, and may also influence how educators in the health professions incorporate death and dying concepts into their curricula.

The use of antipsychotic medications in older adults with dementia increases risk of mortality; therefore, it is critical that nurses use nonpharmacological interventions in dementia care. The nurses' role is integral to implementation of treatment strategies to dementia patients and efforts to improve care in patients with dementia using a nonpharmacological approach are necessary. Therefore, guidelines outlining nonpharmacologic dementia care management will enable nurses to provide a wider spectrum of care to dementia patients. The purpose of this project was to make recommendations for the development and implementation of interdisciplinary practice guidelines to standardize care in the organizational setting. An integrative literature review was conducted using the Fineout-Overholt, Melnyk, Stillwell, and Williamson's analytical approach to reviewing evidence using 7 levels for evaluating the hierarchy of evidence. Inclusion criteria were limited to studies from January 2010 to October 2017 in English with full text. A total of 16 studies were reviewed and categorized according to 1 of the 7 hierarchical levels. Findings were summarized at each appropriate level and included systematic review or meta-analysis, randomized controlled trials, controlled trial without randomization, case-control or cohort studies, qualitative or descriptive studies and expert opinion or consensus. Non-pharmacological approaches including music therapy, cognitive-behavioral therapy, distraction, horticultural therapy, ear acupressure, and cognitive stimulation were all shown to be effective in dementia care. Social change based on this project targets improving nurses' knowledge of nonpharmacological interventions in dementia care.

Bakgrund: Sexuell hälsa är en mänsklig rättighet, även för den äldre populationen trots fördomar och tabun som motsäger detta. Sexualitet och intimitet kan yttra sig på fler sätt än penetrativt sex, som handhållning och komplimanger. Vårdpersonal har som arbetsuppgift att hjälpa sina patienter att uppnå bästa möjliga hälsa och sexuell hälsa, samt att dokumentera detta, speciellt för sjuksköterskor. Syfte: Examensarbetets syfte var att belysa vårdpersonals erfarenheter vid samtal om sexualitet och intimitet med äldre som bor på särskilt boende. Metod: En litteraturstudie med kvalitativ ansats skrevs. Databaserna PubMed och CINAHL användes. Elva artiklar valdes och kvalitetsgranskades. Resultat: Ansvarsdiffusion rådde då en del av vårdpersonalen ansåg att det var en del av deras profession att ta upp sexualitet och intimitet, medan andra av vårdpersonalen ansåg att det var patientens ansvar att ta upp samtal om ämnet. Vårdpersonal upplevde samtalen som obekväma på grund av normer, rådande tabun kring ämnet och kunskapsbrist. Vårdpersonals bemötande varierade beroende på faktorer som uppväxt och religion. De handlade också olika beroende på sin bekvämlighetsnivå. Barriärer, kommunikationssvårigheter och normer hindrade öppen kommunikation om sexualitet mellan vårdpersonal och äldre. Konklusion: Det framkom att vårdpersonal kände sig obekväma och dåligt förberedda inför samtal med äldre om sexualitet och intimitet. Bristande stöd från arbetslaget och bristande eller icke-existerande rutiner till hur en ska gå tillväga spädde på tystnaden kring ämnet. Vidare utbildning inom ämnet behövs för att rutiner ska implementeras och för att vårdpersonal ska kunna utvecklas i sina professioner.
Background: Sexual health is a human right for everyone, including elderly people. Even though taboos and prejudices may contradict this. Sexuality and intimacy may show itself as more than penetrative sex, such as hand holding or compliments. It is included in nursing staffs’ duties to help their patient achieve the best possible health, sexual health, and to document this, especially for registered nurses. Aim: The aim of this literature review was to explore nursing staffs’ experiences in conversations about sexuality and intimacy with elderly people living in nursing homes. Method: A literature study with qualitative approach was written. The databases PubMed and CINAHL were used. Eleven articles were selected and quality-reviewed. Result: Diffusion of responsibility prevailed when some of the nursing staff felt that it was part of their profession to address sexuality and intimacy, while others of nursing staff thought that it was the patient's responsibility to raise conversations about the subject. Nursing staff experienced the conversations as uncomfortable due to norms, the current taboo about the subject and the lack of knowledge. Nursing staffs’ approach varied depending on factors such as upbringing and religion. They also acted differently depending on their level of comfort. Barriers, communication difficulties and norms prevented open communication about sexuality between nursing staff and elderly people. Conclusion: It was found that nursing staff felt uncomfortable and poorly prepared for conversations about sexuality and intimacy with elderly people. Lacking support from the work team and inadequate or non-existent routines on how to proceed increased the silence about the subject. Further education about the subject is needed to implement routines and to enable nursing staff to develop within their professions.

Each year, over 50 million surgical and nonsurgical inpatient procedures are performed and yet, shared decision making between patients and health care providers is not achieved. Obtaining patients' informed consent is part of a nurse's daily routine during admissions and before a procedure. The purpose of this project was to evaluate evidence to answer the practice-focused question regarding support for a policy change to implement a nurse-driven informed consent protocol. The systematic literature review was conducted using the adapted literature review by Souz, Silva, and Carvalho, which consisted of 6 levels for evaluating evidence. A total of 15 articles were graded using the updated Johns Hopkins nursing evidence-based practice model. Evidence from the literature review showed that nurses had several roles in the informed consent process: advocate, communicator, and witness. A modified Real Time Delphi 2 round survey was used to measure an expert panel's reaction to the systematic review and to evaluate a nurse-driven informed consent protocol. The results showed consensus from the expert panel (n=16; 81% agreement) for implementing a nurse-driven informed consent protocol, with Cronbach's Alpha, α = .70 for internal consistency and reliability, and Fischer's exact test yielded p = 1.0, showing no differences between staff nurses and managers in advocating for a policy change. Implications for positive social change include improving a nursing process, and impacting patient outcomes, and encouraging collaborative decision-making in health care.

 Bakgrund: Förebyggande av höftfraktur hos äldre är ett angeläget ämne på grund av att världens befolkning ständigt blir äldre. Äldre på särskilda boenden har en särskilt hög incidens av höftfrakturer. Höftskyddsbyxor är en relativt ny produkt för att förebygga höftfrakturer orsakade av fallolyckor. Syftet med denna litteraturstudie var att utvärdera användandet av höftskyddsbyxor och om det är en effektiv sjuksköterskeåtgärd för att förebygga höftfrakturer på särskilda boenden. Metod: Studien gjordes som en litteraturöversikt och manifest innehållsanalys användes. Resultat: I elva artiklar ansågs höftskyddsbyxor vara effektiva i förebyggandet av höftfrakturer. I fyra av artiklarna visade resultatet att de var ineffektiva. Endast en sorts höftskyddsbyxor användes i fler än en studie. Målgruppen var bland annat äldre med hög fallrisk, sköra äldre och kvinnor med lågt BMI. Diskussion: I alla studier fanns problem med följsamheten och användningen av höftskyddsbyxor som förmodligen drog ner effekten. Höftskyddsbyxor skyddade inte mot alla höftfrakturer även när de användes korrekt. Det var omöjligt att se någon skillnad i effektivitet mellan olika sorts höftskyddsbyxor. Slutsats: Sjuksköterskor behöver noggrant bedöma vilka äldre som behöver höftskyddsbyxor och fortlöpande kontrollera förändringar i deras behov. Background: Hip fracture prevention in the elderly is of major importance with an ageing world-wide population. Elderly living in nursing homes have a particularly high rate of hip fractures. Hip protectors are a recent development in prevention of hip fracture caused by falling.                                                                                                                                          The aim of this literature review was to examine if use of hip protectors are an effective nursing intervention in the prevention of hip fractures among the elderly living in nursing homes. Method: Literature review and manifest content analysis was used.                                Result: In eleven studies were hip protectors considered to be effective in the prevention of hip fractures. In four of the studies they were considered ineffective. The target group was those at greatest risk of hip fracture including high-risk of falling, the frail and women with low BMI.                                                                                                                      Discussion: All of the studies had difficulties with compliance of hip protector use which probably decreased their effectiveness. Hip protectors did not prevent all hip fractures even when worn correctly. Differences in the effectiveness of hip protector types were not able to be assessed as only one type was used in one than one study.                                                                                                          Conclusion: Nurses need to carefully assess which elderly need hip protectors and to continue assessing as needs change. Nurses must educate caregivers and the elderly on the correct use of hip protectors and their limitations.

Background World Health Organization reports that there are no exact statistics of how many older adults worldwide suffer from malnutrition, but available data suggest that about 15–60 percent of older adults cared for in hospitals, nursing homes and home-care programmes are suffering from malnutrition. It is important that the registered nurse is aware of different interventions and how they can be implemented in order to improve the nutritional status of the older adult. Aim The aim of this study was to examine nursing interventions used to treat and prevent malnutrition in older adults at hospitals and nursing homes. Method The study design was a literature review with 15 scientific articles included. The data collection was done in the databases PubMed and CINAHL. The result articles were a mix of both qualitative- and quantitative studies. Intergraded data analysis was used to compile the 15 articles. Results The interventions had an overall positive effect on the nutritional status of the older adults. Oral nutritional supplements were the most common intervention. Both environmental and social factors had effect on the mealtime experience. The older adult was not often involved in the nutritional care and the evaluation of the outcome of the interventions was scarce. The teamwork between health professionals, relatives and the older adult was inadequate. Conclusions Malnutrition in older adults is a prevalent problem and is often de-prioritized in nursing care. The most common intervention used was oral nutritional supplements but there are other interventions that also provide positive results and provide more individual care and are often disregarded by the RN’s. The older adults’ involvement in the nutritional care can be increased and improved as well as involvement of relatives. The importance of treating and preventing malnutrition is a topic that could be emphasized more within the geriatric care.
Bakgrund Världshälsoorganisationen rapporterar att det inte finns någon exakt statistik över hur många äldre personer över hela världen som lider av undernäring, men tillgängliga data tyder på att cirka 15–60 procent av de äldre personer som vårdas på sjukhus, särskilt boende och inom hemsjukvården lider av undernäring. Det är viktigt att den legitimerade sjuksköterskan är medveten om olika omvårdnadsåtgärder och hur de kan genomföras för att förbättra den äldre personens näringsstatus. Syfte Syftet med denna studie var att undersöka tillämpade omvårdnadsåtgärder för att förebygga och behandla undernäring hos äldre personer på sjukhus och särskilt boende. Metod Studiedesignen var en litteraturöversikt med 15 vetenskapliga artiklar inkluderade. Datainsamlingen gjordes i databaserna PubMed och CINAHL. Resultatartiklarna var en blandning av både kvalitativa och kvantitativa artiklar. En integrerad analys utfördes för att sammanställa de 15 artiklarna. Resultat Omvårdnadsåtgärderna hade en övergripande positiv effekt på de äldre personernas näringsstatus. Näringsdrycker var den vanligaste åtgärden. Både miljömässiga och sociala faktorer påverkade matupplevelsen. Den äldre personen var sällan involverad i sin nutritionsvård och utvärdering av omvårdnadsåtgärderna var sällsynt. Samarbetet mellan vårdpersonal, anhöriga och den äldre personen var otillräckligt. Slutsats Undernäring hos äldre personer är ett vanligt förekommande problem som ofta bortprioriteras inom vården. Den vanligaste åtgärden var insättande av näringsdryck men det finns andra åtgärder som ger goda resultat och är mer individanpassade som ofta förbises av sjuksköterskan. Den äldre personens medverkan i sin vård gällande nutrition kan ökas och förbättras, såväl som medverkan av anhöriga. Vikten av att behandla och förebygga undernäring är ett ämne som skulle kunna betonas mer inom geriatrisk vård.

Technology and the complexity of the patient care can take the nurse's attention away from caring for the patient to caring for the technology. The purpose of this systematic review was to gather evidence from the literature on tools for measuring caring and the outcomes in order to make that body of knowledge easily accessible to the direct care nurse for implementation. Jean Watson's theory of human caring 'the theoretical framework that guided and informed this DNP project' focuses on human caring processes and experiences. It assumes that effective caring promotes health and outcomes of the nurse and patient. By using such a theory, a systematic review of caring behaviors can illustrate how using caring behaviors with intention can improve patient outcomes as well as nurse satisfaction outcomes. Inclusion and exclusion criteria were identified and articles were appraised using the Johns Hopkins Evidence-based Practice Model and process that nurses at all levels have used to appraise research and non-research for practice changes. Articles were narrowed to10 that met criteria and caring behaviors were recognized. The chosen articles used 5 different tools to measure caring behaviors. Although the individual caring behaviors were identified, the diversity of the tools used to measure and assess caring behaviors did not provide an easy way to assess for commonalities. This review identified areas for future research including, how one tool can be used to measure caring at different levels of care, in an acute care setting, an whether there exists an opportunity to develop common terminology for describing caring behaviors? Future research on using caring behaviors with intention could change nursing practice by changing how nurses perceive their skills and tasks.

After the 2001 attacks on the World Trade Center, public health garnered more attention. This event forced public health nurses to acquire new roles needed to respond to disasters. The purpose of this project was to synthesize the current evidence related to the role of nurses during disaster preparedness and apply that evidence to public health nurses. The practice question focused on the role of nurses during disasters which were not a part of their normal daily activity. The Fineout-Overholt, Melnyk, Stillwell, and Williamson hierarchy for grading evidence was the theoretical framework used. A systematic literature review was conducted to synthesize the current evidence and answer the proposed practice questions. After removing exclusion criteria, there were 3 articles evaluated and graded for levels of evidence. Much of the current literature offered recommendations for competencies, role identification for nurses during disaster preparedness by their employers, curriculum development, and training needed for nurses to identify their role during disasters. A suggested recommendation based on this project is to conduct a mixed methods study to help identify the specific role of the public health nurse during disasters. Obtaining a broader perspective of the role for the public health nurse during disasters can provide a better understanding of the new roles and duties they can assume. The social implication of these findings is that local health departments can identify the role of public health nurses during disaster preparedness and improve the nurse's response to disasters, thereby protecting the health of populations.

Nurses may share a commonality of issues which can affect their willingness and ability to respond as post-disaster emergency care providers. Guided by expectancy, locus of control, and chaos theory, a systematic literature review was conducted to identify the barriers which affect nurses’ willingness and ability to report to their unit after a disaster occurs. Briggs methodology guided this systematic review, and Fineout-Overholt’s and Melnyk levels of evidence were used to evaluate the reliability of information and effectiveness of their interventions. Fifteen articles meeting the inclusion criteria (addressed nurses’ willingness to report to their unit or to contact the incident command center for mobilization, published in 2005 or after, and written in English) were reviewed. Twelve were systemic reviews of descriptive and qualitative studies (Level 5), one was a cohort study (Level 4), one was a report of expert committees (Level 7), and one reported findings from a pilot study. Five articles reported personal barriers related to the nurses’ home caregiver responsibilities and four articles reported personal barriers related to nurses’ concern for personal and family safety. Three articles reported institutional barriers related to unsure availability of necessary safety equipment and two articles reported lack of disaster preparedness. Developing a disaster plan that includes emergency phone numbers, a prepared backpack of basic survival gear, and a plan for emergency child and elder care arrangements, as well as providing disaster training for nurses was recommended. Understanding health provider needs and willingness to respond to emergency situations contributes to positive social change by contributing to disaster risk reduction and ensuring safer and more resilient communities.

Amputation affects men and women of all ages, classes, races and religions. The patients’ reactions are, however, entirely unique. This is a literature study comprising fourteen scientific articles which make up the results. The psychosocial effects of amputation can be daunting and considering the mountain of literature on the subject, it is therefore necessary to start by explaining some of the research on stress, crisis, and coping. Brief descriptions of Lazarus’s and Cullberg’s work have been selected to represent a sample of existing theories which have grappled with these psychosocial concepts. The purpose of this study is to describe the various coping strategies utilized by patients who have undergone amputation. The results consist of seventeen coping strategies which are divided up into five theme groups (positive coping, avoidance coping, social support, maladaptive coping, and religion). Evidence is provided which describes how the various coping strategies can actually manifest themselves in reality as witnessed by amputee patients. In conclusion, amputees use various coping strategies at different stages of recovery to deal with the trauma of their loss; these strategies can be observed and identified in both clinical and out-patient settings.

Purpose: To describe how the role of the nurse practitioner has evolved as described in the literature from 2000–2012 using Bronfenbrenner's (1979) Socio-Ecological Model (SEM) of behavior change as adapted by McLeroy, Bibeau, Steckler, and Glanz (1988).

Data Collection: This study is based on the review of five large databases for literature related to nurse practitioners role evolution between the years 2000-2012. Databases used for the search were Academic Search Complete, PubMed (Medical Publications), Cumulative Index to Nursing and Allied Health Literature (CINAHL), Medline (US National Library of Medicine), SCOPUS, and Cochrane Database of Systematic Review. The search terms were based on the five levels of the Socio-Ecological Model: Intrapersonal, Interpersonal, Organizational, Community, and Public Policy. Included in the search were the terms, role and utilization of nurse practitioners.

Results: Using the Socio-Ecological Model demonstrated how the role and utilization of nurse practitioners has changed from 2000-2012. Nurse practitioners, once serving rural, underserved, and uninsured populations now have opportunities to provide health care in many settings.

Implications: Nurse practitioners are important primary care providers. An increase in patient acuity and the complexity of health care have created a need for additional health care providers in primary care. Nurse practitioners are well suited to play a significant role in the present and future delivery of health care. Nurse practitioners as primary care providers need to be at the table when policy is made that relates to health care delivery.

Heart failure (HF) hospital readmission reductions are linked to nursing interventions that include scheduling a hospital follow-up appointment with the patient's health care provider within a week of discharge. Yet, patients often leave the hospital without an appointment scheduled. The focus of this integrative literature review was on analyzing data that associated follow-up within 7 days with reduced 30-day readmissions. A search of articles using CINAHL, MEDLINE, Cochrane Database of Systematic Reviews, and ProQuest databases resulted in 4,813 articles retrieved using the following search terms: heart failure, readmissions, follow-up appointments, and heart failure guidelines. Scholarly articles selected for inclusion were published between January 1, 2007, and June 30, 2017, in the English language, regarding studies completed in the United States, available online in full text, and specific to patients with HF. The Melnyk Critical Appraisal Guide was used for the appraisal, evaluation, and synthesis of the evidence. The transitional care model served as the theoretical framework for the project. A key finding of the review was that follow-up appointment scheduling within 7 days was associated with a modest reduction in readmissions; more research is needed to produce additional evidence on this topic. Project dissemination may result in positive social change by raising awareness of health disparities and empowering patients and staff to work collaboratively. Through improved communication and follow-up between patients and the interdisciplinary team, patients with HF may be able to experience improved disease management and a reduced number of hospitalizations.
