Testo completoBackground In the dialysis nurses and patients meet regularly every week, sometimes over several years. This makes it possible for relations to occur. Aim Describing dialysis nurses perceptions and experiences of long-term care relations with persons with chronic renal disease who requires hemodialysis. Method Twelve dialysis nurses at hospitals in the middle of Sweden were interviewed. Qualitative content analysis was performed. Results The satisfaction with long-term care relations with patients in hemodialysis enrich the dialysis nurse both professional and as a person. The encounter with patients during a long time gives many positive emotions and it is joyful to be able to follow the patient development. The experience of the Complexity in long term relations, to meet and be rubbed against the same patients over time, sometimes several years could be a challenge experience in the profession to deal with in a good way. Strategies to manage the boundaries of the profession are to have tools to handle the boundaries between the professional role and the patient. Examples of tools are reflection and coaching session with colleagues since this can make it easier to avoid to be too private in the relation and to feel professional. Colleagues' role in the work is an important part with exchange of experiences and support to deal with the relations and the situations that may occur, in these relations. Concluding comment It is enriching and positive to work with patients in long-term relations. The complex situations that occur can affect the dialysis nurses and it seems important to have access to forum where questions can be discussed and to have guidelines that is put together by the workgroup to rely on.

Testo completoBakgrund: I det postoperativa skedet är patientens förmåga att kommunicera nedsatt. Den postoperativa patienten är därför i stort behov av att informationen som ges är individanpassad. Syfte: Syftet var att undersöka kommunikativa barriärer och underlättande faktorer mellan sjuksköterskan och patienten i postoperativ vård samt beskriva förbättringsområden. Metod: Litteraturstudie bestående av 12 stycken kvalitativa vetenskapliga originalartiklar med fokus på kommunikation mellan sjuksköterskan och patienten i postoperativ vård samt patientens kommunikativa behov och sjuksköterskans förhållningssätt till detta. Resultat: Litteraturstudien visade på att barriärer likt stressiga arbetsförhållanden ledde till kort eller ingen kommunikation mellan sjuksköterskan och patienten samt att den postoperativa vårdtiden därigenom blev förlängd. Olika modersmål, dialekt, kön, kulturell övertygelse och socioekonomisk bakgrund gjorde att informationen från sjuksköterskorna ej uppfattades- eller tolkades korrekt samt att tiden lagd på att få fram budskapet ökade arbetsbördan. Vad som agerade underlättande var patienter som själva var framåt och frågade om sin behandling. Detta resulterade i att dessa patienter fick hjälp samt ökade sannolikheten för kommunikation med sjuksköterskorna. Slutsats: Resultatet av litteraturstudien visade att ett etablerat tydligt kommunikativt samspel mellan sjuksköterskan och patienten, med individanpassad vård, kunde av patienten upplevas som lika viktigt och önskvärt som ett snabbt tillfrisknande från sjukdom.

Testo completoBackground: Today's research shows that mental illness is increasing globally. In society, we can see a stigmatizing and discriminatory attitude towards people with mental illness. It appears that this patient group has a greater need for care, but that they feel unfairly treated and cared for in the health and medical care. Aim: The literature review aims to describe attitudes of nurses in somatic care towards people with mental illness. Method: This literature review has an integrative method which means that the result is based on both qualitative (9) and quantitative articles (2) from databases MEDLINE, CINAHL and PsycINFO. All articles have been read thoroughly to correlate to the studys’ aim and their quality has been assessed with guidelines from the School of Health and Welfare in Jönköping. Result: Three main themes emerged: Having a judgmental attitude towards the patient, avoiding attitudes and empathetic attitudes. Conclusion: Discriminatory and positive attitudes can be found in nurses. In between, there are nurses who prioritize caring for patients with mental illness related to external and internal circumstances such as perception of responsibility, care culture, lack of assets, powerlessness and hopelessness. Education and experience can, according to the results of the literature review, give positive attitudes to the nurse under the right conditions, but society's view of persons with mental illness seems to be a strong factor that influences nurses' attitude. Testo completoPatients with substance abuse problems occupy much of the bedsand resources of the health care. Therefore, it is important toensure that these patients receive proper care and highlight thefactors that may affect that care. The purpose of this literaturestudy was to illuminate nurses’ attitudes towards patients withsubstance abuse problems and describe the character of themeetings between nurses and these patients. As the basis for thispaper, 17 research articles were used, of them 11 articles werequantitative and six were qualitative. It was found that nurses oftenexhibited negative attitudes towards patients with substance abuseproblems and that this had an impact on the nursing care of thesepatients. Stigmatization, stereotyping and moralization of thesepatients were also detected and this often led to substandard care.Nursing care is characterized by an ethical approach and ahumanistic view that is overlooked if the negative attitudes ofnurses towards patients with substance abuse problems are allowedto affect care. More qualitative research on nurses’ attitudestowards patients with substance abuse problems is prompted.Furthermore, it is required more education in the undergraduatenursing schools and more occupational education at nurses’workplace about nursing care of patients with substance abuseproblems.

Testo completoBackground: From a nursing perspective communication is an integral part of nursing. Nurses need to establish a positive nurse-patient relationship to ensure good nursing practice. Effective communication leads to higher patient satisfaction, higher patient participation and a better health outcome. The communication between nurse and patient can be negatively impacted by communications barriers. The nurse’s role is to minimize the negative consequences of existing communication barriers by using methods best adapted for the specific situation. There is a need to identify the barriers for effective communication that nurses experience and methods to overcome these communication barriers. Aim: The aim of this study was to investigate how nurses experience the communication between patients and nurses while communication barriers exist as well as to investigate how nurses experience efforts to overcome communication barriers. Method: Literature review with a descriptive design. The literature search encompassed three databases: Pubmed, Psycinfo and CINAHL. The results were based on ten original articles using a qualitative approach, published between 2012 and 2019. Results: During the data analysis two themes were identified: Communication barriers and Measures and methods to overcome communication barriers. These two subject categories were subsequently divided into categories and/or sub-categories. This study has identified twelve communication barriers faced by nurses in their daily work and the nurse’s perceptions in this context. Five different categories relating to strategies and methods to overcome communication barriers, as perceived by the nurses, were identified. Conclusion: This study provides knowledge and tools for nurses to counteract ineffective communication. Further research is recommended to further strengthen clinical practice.

Testo completoI det dagliga arbetet och som en del av omvårdnaden måste sjuksköterskan samtala med patienterna. Samtal kan vara väldigt svåra, vissa samtal är svårare än andra. För att oerfarna sjuksköterskor lättare skall kunna relatera och hantera dessa samtal i yrkeslivet var syftet med studien att beskriva vad sjuksköterskor anser vara det svåraste i svåra samtal. Ett fåtal studier beskriver vad sjuksköterskor anser vara svåra samtal och ingen studie har funnits som beskriver vad de anser vara det svåraste i dessa samtal. En kvalitativ metod valdes och innehållsanalys användes som metodanalys. Studien innefattar intervjuer med sex sjuksköterskor från ett sjukhus i västra Sverige. Resultatet av analysen visar sex huvudkategorier som innehåller sex sjuksköterskors beskrivning av vad det svåraste är i svåra samtal. Kategorierna är: samtal när personkemin inte stämmer, hoppfulla samtal vid negativa besked, samtal med arga patienter, känsliga samtal, försonande samtal med anhöriga och närvara i samtal under tidsbrist. Alla sjuksköterskorna upplevde att det svåraste i alla samtalen var att vara närvarande i samtalet för att behålla patientens hopp och få patienten att försonas med sin situation.
In the daily work of a nurse, talking to patients is part of the nursing care. Communication can be very difficult, and some conversations are harder than others. To make it easier for inexperienced nurses to relate and handle these conversations it’s important to establish what conversations nurses consider difficult and what the most challenging aspects are, and this was the aim of this study. Only a few previous studies describe what nurses consider difficult communication, and none of them focus on what the nurses perceive as the most challenging aspects. A qualitative method was chosen and content analysis was used as an analyze method. The study comprises interviews with six nurses in one hospital in the west of Sweden. The results of the analysis show six main categories which contain six nurses’ descriptions of what they consider to be the most difficult communication. The categories are: communication when the personal chemistry don’t match, hopeful communication when delivering negative notification, communicating with angry patients, sensitive communication, conciliating communication with relatives, and being presence in communication under stress. What all nurses found most challenging was, to be present in the conversation to keep the patients hope and help the patient accept their situation.

Testo completoDepression is a public health problem, as nurses often come across. The approach towards patients is important in the care and based in the relationship between nurse and patient. The purpose of this study was to illuminate nursing care interactions with patients with depression. The review is built upon 14 scientific articles that has been selected and reviewed. The result shows three categories: Creating relationship, communicating and bringing hope.The results illustrate that patients with depression appreciate when the nurse takes the time, being present and listening actively in the meeting with the patients. Furthermore, the result illustrate the importance of nurses involve the patient, making the patient involved in the care and the nurse has the ability to instill hope, this leads to that the patient has easier to make decisions about their depression. Further research is needed regarding whether nurses need further education about depression for the nursing care to promote and strengthen the relationship between nurse and patient with depression.

Testo completoMed de ökade behov och krav som ställs på primärvården i västvärlden och den resursbrist på personal som råder är det en ekvation som är svår att lösa. Ett sätt att möta dessa behov är att uppgiftsväxla vården och använda sjuksköterskor i stället för läkare i större utsträckning då det i tidigare studier visat på lika bra eller förbättrad tillgänglighet, effektivitet och kvalitet. Denna litteraturbaserade studie belyser patienters upplevelser av mötet med sjuksköterskor som vårdgivare inom primärvården. I resultatet har tio vetenskapliga artiklar, både kvalitativa och kvantitativa analyserats. I resultatet framkommer vikten av att utveckla en förtroendefull relation genom kommunikation och delaktighet med sjuksköterskan. En förutsättning för att det skall kunna ske är att det finns tillräckligt med tid en annan aspekt är möjligheten till kontinuitet, att träffa samma sjuksköterska vid upprepade tillfällen. Kan vården tillgodose dessa behov finns stora förutsättningar för att patienten skall känna trygghet och tillit till vården. I dagens sjukvård med ökad efterfrågan på vård, ekonomiska utmaningar och personalbrist finns risk för att dessa behov inte tillgodoses. Med en ökad kunskap hos sjuksköterskorna och vårdorganisationen om patienternas behov kan detta bidra till en mer individanpassad och tillfredsställande vård för patienten. Det visar sig också att många patienter har en oklar uppfattning om vad sjuksköterskans roll egentligen innebär.

Testo completoBackground: Sexuality still includes a taboo in almost all societies, which may have contributed that the topic is relatively unexplored, despite the knowledge that sexuality is important for health and happiness. It is found that illnesses and medical conditions can have a negative impact on sexuality. In literature it is described how patients experience a lack of conversation and information about sexuality in the context of disease and medical conditions. Despite the acknowledged importance of sexuality for quality of life nurses often avoid or leave out a sexual anamnesis. Purpose: The purpose was to examine which factors that may facilitate or inhibit communication with patients about sexuality from the nurse's perspective. Method: A literature review which is based on ten scientific studies, both qualitative and quantitative, searched through the databases CINAHL, MEDLINE and PubMed. Data analysis has been done in agreement with Friberg and inspired by Granheim and Lundman. Results: It was found that a good nurse-patient relation and good communication skills encourage conversation about sexuality. However, it emerged mainly inhibitory factors that affect communication with patients about sexuality. The main barrier proved to be that the nurses did not take the time to discuss sexuality. The second major barrier was that nurses have a preunderstanding that patients do not expect nurses to approach sexuality. Other barriers were related to culture, lack of knowledge, the complexity of communicating about sexuality and the nurse's perception of responsibility. Discussion: A major barrier that might form the basis for nurses' attitudes is lack of education. A key aspect for the provision of professional nursing is a good nurse-patient relation, which requires knowledge and experience. Through training and education in how to approach sexuality the nurse can obtain the knowledge and tools that can promote communication. A prominent barrier was the comfort level, related to sexuality seen as taboo and too private to discuss. In order to address sexuality nurses themselves need to believe that sexuality is an important part of nursing care, and then are able to communicate that sexuality is an appropriate and permissible subject to discuss.

Testo completoThe aim of this study was to describe experienced registrered nurses (RNs) reflections regarding creating a good interaction with patients i.e how the RNs are/behave towards patients, how the RNs come across. The study had a descriptive design and was conducted as six semi structured interviews with experienced RNs, all working on different adult somatic wards. The results were analysed using qualitative content analysis. It highlighted the importance of creating an open atmosphere by being available and the key aspects of communication such as, actively listening and being perceptive to the unspoken word. Consciously mirroring their own spoken word with their body language and using body contact also conveyed positive communication to patients. Conveying open mindness and respect motivated patients to be involved in their own care. The RNs had to be aware of past patient hospital experiences and being self aware, in particular of their own limitations, benefitted both patients and RNs. Positive effects were shown when patients responded with signs of well-being and faith in the RNs. When RNs came across well generated this too personal satisfaction for the RNs.

Testo completoABSTRACT Background It is a growing global problem that nurses are exposed to threats and violence in health care. The consequences can be multidimensional. Nurses and patients are both at risk of negative effects as a result of threats and violence in health care. Aim The aim of this literature review was to describe how threats and violence in health care may affect the care nurses provide. Method The applied method was a non-systematic literature review. The database search was conducted in CINAHL, PubMed and PsycInfo. The included articles were quality examined in accordance with Sophiahemmet University´s matrix. The data analysis was performed with inspiration from Whittemore and Knafl's (2005) analysis model. Results The result was based on eight quantitative and eight qualitative studies. The result was presented based on the main theme with associated subthemes. Nurses with previous experience of threats and violence could become more aware of patients' behavior during the meeting. The relationship between patients and nurses could be adversely affected, i.e. feelings of distrust were described because of unpredictable patient behavior. As a result of threat and violence, nurses could distance themselves from patients and merely provide care that they were obligated to give. Conclusions Nurses exposed to threats and violence could experience negative stress, impaired self-dignity as well as well-being, which could negatively impact nursing care. Reduced patient safety and quality of care could occur due to reduced work motivation and productivity of the nurse, as a result of patient threats and violence. Such threats and violence could negatively affect the threatening/violent patient, however also other surrounding patients.

Patients with substance use disorders are a group that can be viewed as a group with a large demand of care. They are exposed to stigmatization and therefor they need specialized care. The meaning of patient centered care is to strive to optimize every individual’s well-being. Nurses may increase patient’s empowerment through a motivating approach. Aim: To explore patients’ perceptions of the nurse-patient relation of patients with substance use disorder. Method: A literature review, including both qualitative and quantitative studies. Result: Three categories were identified: To see the whole person, caring encounter and stigma. Both positive and negative aspects of every category were discovered. Discussion: A patient centered approach can be understood as an emphatic, caring, committed and helpful encounter. Many patients had experiences of discrimination and stigma during care. This could partly be understood as an unbalanced power-control relationship between caregiver and patient. Conclusion: The nurses actions can increase as well as decrease patients suffering. A caring encounter is important to promote health and to optimize the results of care. There is room for development of nursing skills in care of patients with substance use disorders.

Testo completoBackground Patients with alcohol abuse are found in all care settings. In emergency care, 30 percent of admissions are related to alcohol. Although alcoholism is now viewed as a widespread disease, there is a stigmatizing view of people with alcohol abuse. This stigmatization of the disease is believed to be a major factor in the fact that many patients do not seek care for their problems, but also that as a patient in alcohol screening, you underestimate the amount of alcohol consumed. Diminishing attitudes and preconceived opinions about this patient group entails a risk that the Health Care Act will not be followed, and in the end that patient safety is compromised. Aim The aim of this literature review was to describe nurses' attitudes about alcohol abuse and their attitudes to patients with alcohol abuse. Method A literature review was made in the hope of reviewing and summarizing the current state of knowledge. The result was based on 15 scientific original articles that were analyzed according to an integrated three-step analysis. The two main categories that emerged in connection with the analysis were Knowledge and Attitudes. The subcategories that emerged were Education & Knowledge Level, Nurse in Comparison with Other Professionals, Positive & Negative Attitudes, Attitude's Impact on Nursing, Perspective on Alcoholics and Feelings. Results The result showed knowledge gaps in the nurse regarding alcohol abuse and patients with alcohol abuse. The knowledge provided during nursing education is not sufficient for the nurse to have sufficient self-confidence to provide good nursing care to these patients. However, the nurse expressed a willingness to help and support these patients. The nurse also expressed a view of patients with alcohol abuse as persons in need of care. Conclusion There are patronizing attitudes in health care towards patients suffering from alcohol abuse. The results also showed insufficient knowledge among nurses regarding alcohol abuse and alcohol dependence, as well as care and nursing care for these patients.

Testo completoBackground Substance-use disorder is common in society and involves harmful use of different substances, for example. alcohol and drugs. The use is in progress despite adverse consequences for the person's life situation and increases the risk of unstable health. Which leads to an increased need for healthcare. Research has shown that people with substance-use disorder experience exclusion, stigma and shame in contact with healthcare Aim To describe nursing experience of meeting people with substance-use disorder and the nurse's attitudes in the healthcare meeting, as well as describing the survey articles of the included articles. Method A descriptive literature study. Ten items are included in the result. Article search was made in the PubMed and Cinahl databases. Discussion was conducted between the authors to compile an objective result. Results Nurses had different opinions about if substance-use disorder were a disease or not. Persons with substance-use disorder were considered irresponsible for their health status according to nurses. Nurses described how they experienced the patient group as erratic and manipulative and that caused increased workload. Furthermore, a sense of frustration arose over their own missile and the uncertainty surrounding the patient group. Requests for education were expressed. According to the nurses, patience and support were important parts of care and to not reflect their own opinion. Conclusion Depending on whether nurses saw the substance-use disorder as a state of illness or as self-inflicted, positive or negative attitudes and experiences have emerged. Knowledge and education in substance-use disorder is necessary to maintain an ethical approach within the nurse's profession.

Testo completoDementia is a complex disease state involving impairments in social, psychological and physical abilities, which are often exacerbated as the disease advances. A common symptom in dementia is a reduced communication capability. How a person is affected by the disease largely depends on how other people react. When professionals see beyond the symptoms it makes meetings, approaches and interactions easier in nursing. The aim of this study was to describe nurses’ experiences of interacting with people with dementia. The method of the work was a literature review, with purpose to make a concluded view of the chosen subject area. Included articles where searched in the databases CINAHL Complete and Pubmed. Totally 15 scientific articles were included in the study after quality review. The results shows four categories to describe nurses experiences of interacting with people with dementia: ”To be able to decipher and understand the interaction with people with dementia”, ”Methods and strategies for interaction and communication with people with dementia”, ”Possibilities and challenges for a good interaction with people with dementia” and ”The interactions consecutions and impacts”. In the results appears that nurses experience the interaction with people with dementia as complex, the interaction is described rewarding as well as challenging. Positive experiences are based on understanding, to understand these people as well as to make yourself understood as a nurse. The conclusion was that nurses have as well positive as negative experiences of interacting with people with dementia. These experiences also cause consequences depending on the experience. Areas of improvement and competence to ensure a good interaction between nurses and people with dementia where shown based on nurses’ experiences. Understanding for each other’s expressions can contribute to additional experienced quality of life and participation among people with dementia. Whilst this understanding can contribute to the feeling of qualitative nursing and comfort within work for the nurse. That can make the profession attractive as we have future challenges ahead in the care for people with dementia, where the number of people with dementia is expected to rise drastically globally.

Testo completoA therapeutic relationship is important in nursing care of patients with eating disorders and it is significant that the nurse is aware of the importance in the nursing care. The purpose was to investigate how a relationship is created and its significance in the care between the nurse and the patient with an eating disorder. It can be challenging for nurses to establish a therapeutic relationship with patients with eating disorders, because it is common that these patients do not have illness insight. The nurse can make use of different strategies and tools to develop a therapeutic relationship and a caring environment. The study was conducted as a litterature study and 12 scientific articles were analyzed. The result of the study shows that if a therapeutic relationship is going to be established it demands knowledge, time, respect and continuous information from the nurse to the patient. More scientific research about the therapeutic significance in the nursing care around patients with eating disorders is requested, since there was not much research on the subject. Even more education in the nursing programme for nursing care in mental health is enquired.

It´s important for patients that health care is built on a relationship where they feel valued and confirmed by the nurse. When patients are not given the opportunity to participate, it can lead to that they feel a lack of trust and they do not feel respected. Problem: Patients experience that there are not always conditions created for them to participate in their care. They feel neglected and ignored when the nurses uses their power and control. Objective/Aim: To describe nurses experience of patient participation. Method: A literature review with qualitative approach. Ten nursing science research articles were selected for analysis according to Evans. Results: Nurses strive to promote patient participation, they have also seen the importance of communication, information and the relationship between the patient and the nurse. Furthermore, they have identified circumstances that they feel are limitations to create conditions for patient participation. Conclusion: Patient participation is something that can easily be taken for granted by nurses, and something that they believe that they apply. By illustrating nurses experience can Health care hopefully create better conditions for professional and future nurses to apply patient participation.

The nurse needs to handle both her own and the patient's feelings in order for the care relationship to work. The nurse needs to support the patient as he or she finds a healthy lifestyle without substance use. A functioning care relationship between nurse and patient with substance use disorder is important for the care to work successfully. Aim: The aim of the study was to describe the care relationship between nurse and patient with substance use disorder from a nursing perspective. Method: A literature review was chosen as method. The article search was conducted in the databases PubMed and Cinahl. In total, 18 original scientific articles were included in the results. Results: The results showed that attitude and perspective as well as negative experience affect the relationship between nurse and patient with substance use disorder. The nurse experiences difficulties related to lack of knowledge and patients' manipulative behavior. Conclusions: The care relationship between the nurse and patient with substance use disorder can be improved by providing support and education for nurses in their professional role.

Testo completoBackground: There are few studies focusing on the patients’ perspective of psychiatric care. However, this perspective is important, as it offers insight in the care received from the patient’s view point. Aim: The aim of this study was to describe how the patients in a psychiatric ward perceive the caregiving behavior of the ward’s nurses. Method: Semi-structured interviews analyzed through qualitative content analysis. Result: Analysis of the data resulted in four main categories describing aspects of good and less good caregiving behavior as perceived by the patients. The categories were: The importance of being seen and validated by the staff, The staff’s attitudes and behavior towards the patients, Wishes and expectations regarding the staff and How non-verbal communication affects the way the care giving behavior is perceived. Conclusion: The most prominent aspect of good caregiving behavior was described as the nurse’s ability to take the time to listen to the patient, and the most prominent aspect of a less good caregiving behavior was described as insufficiency in this ability.

Testo completoBackground: Emergency departments are over-represented in the incidence of violence in health care. Violence affects nurses' ability to provide good care in a negative way and increases the risks to both patient safety and workplace environment. Purpose: To investigate the risk factors that can increase the risk of a patient exhibiting violent behavior in the emergency department and how the staff treats these patients. Method: The thesis is designed as an integrative literature review in which 12 articles were included. Results: Several factors were identified as important to the risk of patients exhibiting violent behavior when visiting the emergency department. Four factors amongst them were identified as especially important: waiting times under “structural factors”, patients under the influence of alcohol or narcotics under “factors related to substances”, as well as patients with psychiatric disorders under “medicinal factors”. The participants in the different studies often identified the same factors, even though they had differing opinions on the importance of the factors. The emergency department staff handled the contact with patients exhibiting violent behavior differently based on several factors such as experience. Better communication, information and an empathic treatment of could lessen the risk of violent behavior and help stave off violent behavior. Conclusion: The most common risk factors for violence in the emergency department are waiting times, alcohol and / or drug affected patients and patients with psychiatric illness. Good communication, information and empathy can reduce the risk of violence or averting violence.

Testo completoBackground: The number of migrants worldwide has increased in recent years. For migrants, adapting to the new country is a process and learning the language of the country is demanding and takes time. Migrants who have not yet learned the language experience conversations that occur in health care as particularly challenging. Patients with a migration background feels that communication is lacking and that the nurses do not always take the time to talk, instead they only choose to perform their working tasks. Aim: To elucidate nurses' experiences of communicating with patients who speak a different native language. Method: A literature review was made based on eleven scientific articles collected from the databases CINAHL Complete and PubMed. Nine articles were qualitative, one quantitative and one with mixed methods where only the qualitative part was used in the result. The articles were analyzed with Friberg’s method and thematised using colour coding. From this, four themes were formed. Results: The result showed that good communication is important for creating a good care relationship. The results indicated that there are shared opinions about experiences of using body language or interpreter to more easily communicate with patients who speak a language other than the nurses. It was also found in the result that communication deficiencies can have consequences in care. Discussion: In the method discussion the authors discuss the method strengths and weaknesses in the authors’ approach. The authors discuss how communication difficulties contributes to equal care. Further the authors discussed the result related to Leiningers transcultural nursing theory.
