Abstract (sommario):
In this article, the authors present an interdisciplinary discussion of the multiple dimensions of racism and formulate conceptions of its impact on the formation of healthy personalities. They describe how racism has both ideological and structural components and perpetuates itself recursively at the macro-(e.g., group, institution) and microlevels (e.g., interpersonal). As one consequence of its embedded, cyclical nature, efforts to treat client problems that involve issues of race and racism will necessarily entail piercing distortions in reality, encouraging self-moral development, and eliciting risk-taking behaviors. To take part in transforming current structures of racism, counseling psychologists are urged to extend these strategies beyond the therapeutic milieu. Implications for practice, training, and research are presented.

Abstract (sommario):

Abstract (sommario):
The phrase ‘psychology in action’ may automatically surface positive connotations to the effects of psychological interventions. However, the fact that something is ‘in action’ or moving, that someone is ‘doing something’, does not automatically indicate favourable outcomes. Misled or badly executed actions can be damaging. Psychiatric care facilities exist primarily to treat the psychological and psychiatric difficulties faced by individuals. Even before the appointment with the therapist and doctor, the mere existence of a psychiatric facility represents ‘psychology in action’. Everything from the appearance of the building to the attitude of the staff contributes to how the facility is putting psychology into action. Therefore, from the moment one is admitted to a psychiatric institution ‘psychology in action’ begins to influence the patient. Theory and therapeutic techniques are essential for effectively putting psychology into action. Indeed, a plethora of information exists regarding the academics of psychology. However, how often are mental health care practitioners made to look at the reality of their patients and clients’ genuine, lived experiences of psychology in action? It is this question which, in light of personal experience, will be kept in mind while reflecting on the subjective experience of both a damaging, and subsequently effective experience of two psychiatric institutions.

Abstract (sommario):
Cultural competence is increasingly recognized as an essential component of effective mental health care delivery to address diversity and equity issues. Drawing from the literature and our experience in providing cultural competence consultation and training, the paper will discuss our perspective on the foundational concepts of cultural competence and how it applies to a health care organization, including its programs and services. Based on a recent consultation project, we present a methodology for assessing cultural competence in health care organizations, involving mixed quantitative and qualitative methods. Key findings and recommendations from the resulting cultural competence plan are discussed, including core principles, change strategies, and an Organizational Cultural Competence Framework, which may be applicable to other health care institutions seeking such changes. This framework, consisting of eight domains, can be used for organizational assessment and cultural competence planning, ultimately aiming at enhancing mental health care service to the diverse patients, families, and communities.

Abstract (sommario):
The aim of this article is to analyze the mental health problems of children and adolescents from the perspective of an Institution entrusted by Law 4/2021, of 27 July, on Children and Adolescents of Andalusia, to be the guarantor of the defense of the rights and freedoms of this particularly vulnerable sector of society. It will address, first, main problems and challenges affecting these children with mental health problems, taking into account the experience of the Ombudsman’s Office in the complaints received from those affected, their families or associations in the sector. Claims that, on the other hand, have been increased after the pandemic caused by Covid-19. Protecting and defending the rights of children also involves formulating proposals for improvements to be taken into consideration by public administrations. Therefore, the document also contains a compendium of those suggestions and recommendations that the Ombudsman has addressed to the Administration of the Andalusian Regional Government to improve the quality of care offered to children suffering from any mental pathology.

ABSTRACTEthics is the Science of morals in human conduct. However, ethics and morals are not same. The ethics is based on certain principles, which include 'Respect for Person and Justice'. This principle is not in total conformity with Hippocratic tradition, where Physician is given a 'Position of Paternalism'. The basic idea of 'justice' is that all human beings are equally valuable. Similarly, the principle of liberty is the right to self determination but what is the use of this right that can not be fulfilled. These basic principles are evolving and are constantly posing problems in every health care institution and are a challenge to every Physician.

Testo completo
Abstract (sommario):
Thesis (M.Cur) --University of Limpopo, 2013
Adherence to antipsychotic medication is very important to patients with schizophrenia. Therefore, if patients with schizophrenia are non-adherent to treatment, they are at risk of relapse and being re-admitted at a mental health care institution in the Limpopo Province. Despite the proven benefits of antipsychotic medications, half of the patients with schizophrenia do not take their prescribed drugs. The researcher has observed the following occurrences during practice: • Lack of adherence to antipsychotic medications of schizophrenic patients results in symptoms not being relieved, poor drug effectiveness and patients developed other serious or costly consequences, such as being violent and damaging property; • High rate of relapse; and • High rate of re-admissions. This research questions has guided the study: • What are the factors affecting adherence of mentally stable schizophrenic patients to antipsychotic medications at a mental health institution in the Limpopo Province? • Do mentally stable schizophrenic patients adhere to prescribed treatment? • Are there any guidelines used to promote adherence to antipsychotic treatment? The aim of the study has been to determine the level of adherence of mentally stable schizophrenic patients to antipsychotic medication at a mental healthcare institution in the Limpopo Province. The objectives of the study have been to describe adherence of mentally stable schizophrenic patients to antipsychotic medication at a mental health institution in the Limpopo Province. It implies that participants have been given the opportunity to describe their experience while on medication. vi Their responses have led to the development of guidelines to promote adherence of mentally stable schizophrenic patients to antipsychotic medication. The study site has been the Thabamoopo Mental Healthcare Institution in the Capricorn District of the Limpopo Province. A descriptive, exploratory and contextual qualitative research design has been used in this study. The population has consisted of all mentally stable schizophrenic patients and all the carers of such patients. Non-probability purposive sampling has been used to select participants in this study. The researcher has used a semi- structured interview with two schedule guides for the patient and carers/relatives, which have specified the issues and questions covered. It has assisted the researcher with gathering information about the problem studied (De Vos et al. 2005).A total of twenty (n = 20) participants, consisting of fourteen (n = 14) mentally stable schizophrenic patients and six (n = 6) carers/relatives has been included voluntarily in the semi-structured interview sessions. The steps of data analysis as described by Tesch (1990) in Cresswell (1994) have been followed in this study. The findings of this study reveal a central storyline which indicates that participants share the same point of view in connection with aspects of adherence to antipsychotic treatment and also knowledge about the causes of mental illness and its prognosis. The following four themes and their sub-themes have emerged during data analysis: Theme 1: Participants share the same point of view related to aspects of adherence to antipsychotic treatment; Theme 2: Knowledge related to mental illness; Theme 3: Health seeking behaviours of mentally ill patients; and Theme 4: Experiences of relatives caring for mentally stable patients on treatment. Guidelines and recommendations based on the findings of this study are described in Chapter 4. The criteria for establishing the trustworthiness of qualitative data maintained in this study have been: Credibility, dependability, confirmability and transferability. The following ethical principles have been adhered to: The principle of beneficence, justice, the principle of human respect and dignity, permission to conduct the study, informed consent and confidentiality, privacy and anonymity.

Abstract (sommario):
Magister Curationis - MCur
Research reported that it is very challenging for mental health care users to quit smoking and the risk of relapsing after quitting is high. However, tobacco dependence treatment is possible and potentially lifesaving for people. The delivery of tobacco dependence treatment by nurses is influenced by a variety of factors, including lack of knowledge and skills, limited professional leadership, and smoking within the profession. There is a strong link between mental illness and smoking-related diseases, including cancer, respiratory diseases and heart diseases that are linked with depression. There is a high incidence of cancer in people with bipolar mood disorder and schizophrenia due to smoking. In spite of this, smoking is regarded as part of the culture of psychiatric institutions and tobacco is seen as “necessary self- medication for the mentally ill”.

Tanner, Carolyn A. "Perception of palliative care practice of health care professionals in a mental institution : a descriptive study". Thesis, University of British Columbia, 1991. http://hdl.handle.net/2429/29703.

Testo completo
Abstract (sommario):
The purpose of this descriptive study is to investigate the perception of practice of health care professionals at a mental institution, as it pertains to their work with dying patients, and also to identify areas of change. The conceptual framework is constructed of six factors important to palliative care that have been identified from the literature. These are personal death anxiety, organizational structure of the hospital system, role expectations of professionals, teamwork, education and training for care of the dying, and conveyance and exchange of diagnostic information. The sample surveyed by a written questionnaire included physicians, psychiatrists, health care workers, social workers and pastoral care workers from the Geriatric Division of Riverview Hospital, Port Coquitlam. The findings indicate that age, sex, marital status and length of working experience at Riverview Hospital had no significant association with personal death anxiety. Informal education such as workshops and in-services had a significant correlation with personal death anxiety, as did perception of being competent and/or confident about working with the dying. The study also raised concerns that not all was being done for the dying patients and their families at this institution. Suggestions such as education and training, support mechanisms, and less stereotyping of professional roles were offered to improve this situation. As well, findings indicated that there was a need for palliative care service either in the form of a team or separate unit, or simply the practicing of the palliative care philosophy.

Martin, Penelope Dawnette. "The development of a model of emotional support for undergraduate nursing students working in mental health care settings". Thesis, University of Western Cape, 2013. http://hdl.handle.net/11394/3246.

Testo completo
Abstract (sommario):
Philosophiae Doctor - PhD
The mental health care environment is a stressful environment because of the environment, perceived unpreparedness of students and the emotional demands placed on the students which are inherent in the nature of the work. The use of the self as a therapeutic tool also presents challenges for students. Whilst researchers have identified supportive interventions and strategies to address students support needs in mental health care settings, these interventions/strategies focus on meeting clinical learning objectives with the assumption that if the student learning needs are met, they will feel emotionally supported. Literature and experience indicates that it is imperative that in this field of study, students should be prepared to cope emotionally with the demands of mental health work. The aim of this research was to develop a model of emotional support for student nurses working in mental health care settings. A theory-generating design based on a qualitative, explorative and descriptive research approach was used to achieve the aim of the study. Purposive sampling was employed to select participants namely: students, educators and clinical staff who met the eligibility criteria. A sample of 40 students, nine educators and nine clinical staff who worked in the four psychiatric hospitals and community mental health clinics in the Western Cape participated in the study. Data collection was by means of focus group interviews (students) and indepth individual interviews (educators and clinical staff). Data was analysed by means of Tesch’s method of content analysis. The model was developed by means of the four steps of the theory generation process. Step one was concept development which consisted of two sub-steps namely concept identification and concept definition. A total of 22 concepts were identified which was further synthesised into six main concepts. The main concepts which were used to develop the model were: positive self-concept; positive work environment; academic and professional development; effective communication; formal and informal supportive interventions and collaboration between the Higher Education Institution and the mental health care setting. Step two of the theory generating process was model development. These main concepts were placed in relation with each other which formed an emotional support model for students working in mental health care settings. Step three was model description. The model was described using the three phases of interpersonal communication namely orientation phase, working phase and the termination phase. A visual application of the model which depicts the main concepts, the process and the context was shown. Step four dealt with the development of the guidelines for the implementation of the emotional support model. A critical reflection of the model was done using five criteria for model evaluation according to Chinn & Kramer. Trustworthiness of the data was ensured by means of applying Guba’ model of truth value, applicability, consistency and neutrality. Reflexivity was used by the researcher to further enhance trustworthiness. Permission to conduct the study was obtained from the relevant authorities. The ethical principles of respect for human dignity, beneficence and justice were applied throughout the study. Limitations were identified and ecommendations for nursing practice, education concluded the study.

Simar, Melinda. "“It’s All About You Being Successful as a Student”: Mental Health and Wellness at a Post-Secondary Institution in Ontario: A Governmentality Analysis". Thesis, Université d'Ottawa / University of Ottawa, 2020. http://hdl.handle.net/10393/40346.

Testo completo
Abstract (sommario):
A mental health crisis is happening on post-secondary campuses in Ontario today. Post-secondary institutions provide mental health services to students in an effort to respond to this crisis and manage students in distress. The management of students and the implementation of these mental health services is the main concern of this thesis, more specifically, the ways in which these services expose conscious and/or unconscious beliefs about student mental health. By extension, these beliefs constitute the ways in which we can think about, talk about, and know about patient safety. The object of study is the intersection of the neo-liberal university with the ‘good’ student and the resulting effects of this relationship on the development and implementation of mental health services. These intersections themselves create possibilities for acting on students in distress, but that also create unintended contradictions in the services themselves. An examination of this intersection can address a gap in the literature on post-secondary student mental health. The conceptual framework used in this study is primarily built from Michel Foucault’s concepts of subjectivity, and governmentality. The object of consideration is limited in this study to senior employees directly involved in student mental health at a university in Ontario. Documents are analysed to show how student mental health problems have been problematized nationally, provincially and locally and, thus, a behaviour to be regulated with governing practices. Data from interviews with senior University employees and observations of wellness events are analysed to examine the imbrication of advanced liberal rationalities and techniques in the implementation of mental health services on campus. The thesis argues that the development of these services is not an unproblematic process, whereby services and activities act simply as neutral tools to improve the mental health and well-being of students. Rather, these services aim to produce successful, enterprising students. Discourses of mental health and student success produce certain truths about practices and student subjectivities, obscuring and narrowing the definition of health and well-being and creating contradictions for students experiencing mental distress. In particular, this thesis shows how the University’s objectives for providing mental health services have implications for the development of mental health services and the governing of post-secondary in advanced liberal ways.

Abstract (sommario):
This thesis intends to study the pioneering approach of the psychiatrist/pedagogue G. Daumézon (1912-1979), concerning the relationships between both caring and education in psychiatric institutions, marked by the emergence and the development of institutional psychotherapy and pedagogy. The general hypothesis is that the use of a comprehensive and reflective approach of this singular work will enable underlining the pertinence and the importance of its contribution in accordance with educational sciences. Both through a sociohistorical, documentary and archival study of G. Daumézon’s work, and a pluralist and cross-disciplinary analysis, completed with researching interviews, it is shown how the asylum, later on the psychiatric hospital, has been questioned by education, pedagogy and training. In the end, it is underlined that the empirical and reflective legacy might be nowadays a contributory recourse to avoid development and standardization of terms and conditions of psychiatric care of mental illness

Abstract (sommario):
Modern psychiatry in the United States emerged at the same time as debate about slavery intensified and dominated public discourse, contributing to dramatic denominational schisms and to the greater visibility of women in the public sphere. as the only institution to accept slaves and free blacks as patients, and to employ slaves as attendants, The Eastern Lunatic Asylum of Williamsburg, Virginia, offers unique insights into the ways in which gender, race and religion transformed psychiatry from an obscure enterprise in the early nineteenth century to a medical specialty with wide-reaching cultural authority by the twentieth century.;Utilizing a variety of sources, including a collection of un-catalogued and largely unexamined papers, this dissertation employs interdisciplinary methods to explore the meaning of interracial medical encounters, and the role of the asylum in promoting rational religion and normalizing domestic violence.;The dissertation begins by examining the life and writings of asylum Superintendent John M. Galt, whose experience at the head of an interracial institution led him to reject proposals for separate institutions for whites and blacks and to promote the cottage system of outpatient care. The following chapter addresses the labor of enslaved attendants, without whom the asylum could not have functioned and for whom moral rectitude and spiritual equality appear to have been the ethical foundation of care-giving. Discussion of ethics and spirituality, in turn, prompts consideration of the role of religion in asylum care. The association of enthusiastic religion with slaves and with abolitionism contributed to the regulation of religious expression as a common feature of asylum medicine. Religious evangelism was viewed by hospital administrators as a symptom of insanity, while religious rationalism was enshrined as normative and, paradoxically, as secular.;Asylum medicine also normalized domestic violence by treating the social problem of violence, from wife beating to the rape of slave women, as the medical pathology of individuals. In so doing, the asylum undermined the religious authority from which many women derived comfort, meaning and purpose; and overemphasized the role of female sexual and reproductive organs as an alleged cause of insanity. Ultimately, the struggle over efforts to contain interracial alliances, women's autonomy and enthusiastic religious expression coalesced in the state's promotion of eugenics in the early twentieth century.

Abstract (sommario):
AbstractFreebody explores how a patient’s class, gender, age, physical health and mental condition influenced the allocation of occupation in French and English institutions. Class was an important consideration since manual labour was considered unsuitable for the middle classes, particularly for women, despite its alleged benefits as a therapy. Whether a patient’s condition was perceived as curable or incurable made a difference to the type of occupation prescribed in England, and to whether it was prescribed in France. The reasons why curable patients at the acute stage of their illness were more likely to be prescribed occupational therapy in England, or unoccupied and treated biologically in France, are examined. In both countries, incurable patients, and those whose condition had deteriorated into chronicity, were allocated work around the hospital, provided they were physically fit, for the benefit of both institution and patient. Freebody compares the material conditions of English and French, rural and metropolitan institutions, that provided the context for patient work, and the varied approaches towards offering incentives to work.

Abstract (sommario):
AbstractFocusing on the varied responses of French psychiatrists to new theories of occupational therapy emerging after World War I, this chapter argues that the voices of psychiatrists in favour of the new methods were “drowned out” by those who continued to interpret mental disorder in purely organic terms, by the desire of many French psychiatrists to enhance their “scientific” credentials through the use of biological treatments, by the reliance on the funds generated by traditional forms of patients work, and by a lack of skilled staff to supervise occupational therapy. As a result, patients in French asylums continued to remain idle or to be occupied with tasks designed to benefit the institution rather than the patient.

Abstract (sommario):
AbstractThe mental wellbeing of higher education students has become a salient issue facing higher education worldwide. This increased attention can be at least partially attributed to prevalence studies which indicate that the severity of mental health difficulties is growing across student populations, and a high proportion of students—higher than the general community—are experiencing moderate-to-severe levels of depression, anxiety, and stress. There has also been increasing recognition that universities have a critical role in providing supportive and health-promoting environments and developing whole-of-institution wellbeing policies. In addition to their moral imperative to ensure the safety of all students, supporting the mental health and wellbeing of students should be a priority for universities given that psychological distress is known to adversely affect students’ academic motivation, retention, and achievement. This is arguably even more important in the wake of COVID-19, which led to rapid changes in learning delivery, as well as a reduction in social connectedness and students’ perceptions of the quality of their university experience. This chapter examines the current state of play with regard to institutional mental health and wellbeing strategies and policies in the Australian higher education sector and suggests recommendations for future directions.

Abstract (sommario):
AbstractToday, citizens in many western countries are increasingly confronted with questions of mental health. In the mid-twentieth century, mental issues were not a subject commonly debated in public. Rather, the shame connected to so-called nervous diseases caused many families to conceal the mental distress of a family member. Drawing on and unpacking three oral history interviews, this chapter explores how the family as an institution has formed a changing site of illness. Following ecological approaches to mental suffering, it more specifically shows how the changing dynamics of secrecy and disclosure within families have shaped and determined how mental illness has been experienced. In doing so, the chapter offers a novel lens on how secrecy, as a kind of knowledge management, has intersected with broader societal and cultural developments and shaped the situated and historically contingent experiences of mental suffering.

Abstract (sommario):
Drug use among higher education students can cause harm to the individual, their institution, and the wider society. Academic performance, physical and mental health, institutional reputation, crime and unemployment can all be impacted by student drug use. Tackling this is a challenge, and is often compounded by limited student health and counselling capacity and the student’s reluctance or unwillingness to seek support. Digital brief interventions have shown promise in reducing harm from substance use, and provide an opportunity to meet students where they are, delivering always-on, confidential support and intervention. However, limited interventions for drug use are available for students, and many struggle with engagement and retention. Our team have developed a novel brief intervention, using best practices in digital intervention development, and behavioural change to overcome some of these challenges. We describe the development of our intervention and discuss how implementation could result in tangible benefits to the individual, institution, and society.

Abstract (sommario):
Sports technologies have been in continuous and uninterrupted 'change and development' from the day they first started to emerge until today. With each passing year, sports technologies are developing by taking reference from each other through new technologies. With the development of sports technologies, generations, encounter more 'new and advanced' technologies every day. Sports have an important place in protecting the physical and psychological health of individuals. In addition to physical health, mental health also has an important place in sports. In this respect, the aim of the current research is to examine different technology areas in sports in terms of mental health. For this purpose, as a data collection tool; DergiPark, Higher Education Institution National Thesis Center (YÖKTEZ) and Google Scholar were searched for Turkish sources and Web of Science and PubMed databases for English sources with the keywords "Sport Technology", "Sport Mental Health", "Sport Entertainment" and "Technology Addiction". As a result of the research; it is seen that technology in sports is used for different purposes such as 'entertainment, exercise and performance sports'. In some cases, the use of technology in more equipment or time than needed can create technology addiction in individuals and negatively affect their mental health. On the other hand, it is thought that conscious technology users can transform their existing sports/exercises into an efficient and fun way with sports technologies developed for different purposes. In this respect, it was concluded that, as with all new developing technologies, it is important to determine the needs well when using these technologies and to be a conscious user of sports technologies in terms of mental health.

Abstract (sommario):
Abstract: This study aims to investigate how applied well-being elements and geographic location of a company/institution influence the workplace satisfaction of different working groups in Hungary. In our research, 108 companies and institutions of different sizes were selected in Budapest and in the countryside, as well as organizations with national scope in Hungary. HR managers and professionals were asked for the help of a self-filling expert questionnaire. The study explores how physical, mental, and emotional support is provided in these different locations, and whether well-being-supportive technologies are utilized. Additionally, the study looks at the availability of self-development opportunities and the impact of location on the differences in real-time workforce monitoring, transparent communication, and the redefinition of HR missions. The provision of appreciation, positive feedback, contact with managers, career planning, mentoring, fringe benefits, and physical and mental health are highlighted in relation to location. Our results show that regarding the applied language of appreciation companies in the countryside are able to stay in the competition with companies located in Budapest (the capital) or the ones with nationwide scope. However, such well-being elements like real-time workforce monitoring, utilizing well-being supportive technologies, physical, mental, and emotional support of new and modern HR strategies are in seriously disadvantaged positions compared to companies located in Budapest or to those with nationwide scope. Independently of all this, we found only at companies in the countryside correlations between the satisfaction level of all three working groups and the implemented well-being dimensions and appreciation languages.

Abstract (sommario):
During the careers of military personnel of the Parachute Infantry Brigade, who, due to the needs of their service, are exposed to various stimuli in their activities considered high risk, the physical and mental health of these professionals is essential, as they can be affected by high levels of stress, suggesting impacts on their quality of life. Based on the encouragement of professor Janiara de Lima Medeiros provoked by concerns about the inclusion of the female body in the Agulhas Negras Military Academy, starting in 2018, this research was directed to questions about healthy living in the barracks. The objective of this research was to correlate the stress levels and quality of life of 113 military personnel of the Parachute Infantry Brigade, who are fit for airborne activities. Five instruments were used in this research: free and informed consent form; letter of consent; sociodemographic data; stress inventory (LIPP-2000) and quality of life assessment instrument of the World Health Organization (WHOQOL-100). The results highlighted positive correlations between quality of life, length of service and age, and there was no significant correlation between total stress and quality of life. The limitations of the research are discussed, since it did not include samples from other genders, thus restricting the scope of other possibilities inherent to the researched public, since in its active personnel, we have female representation. Another issue is the size of the sample in relation to the population, since it did not include a sufficient number of participants to achieve a smaller margin of error and a higher level of confidence in the results. We emphasize the importance of a more comprehensive study to verify whether the results found in this study will be maintained in the face of changes in the scenario of this military institution.

Abstract (sommario):
In the second half of the 20th century, the tendency to abolish the death penalty influenced to an increased use of life imprisonment. According to available data, about half a million people in the world today are serving a life sentences in prison. Out of 216 countries and territories, life imprisonment is imposed in 183. Between 2004 and 2015, there was an increase in the imposition of these sentences of about 84%. Life imprisonment, with the exception of countries where the death penalty is applied, is imposed as the maximum punishment for the perpetrators of the most serious crimes. In different countries, life imprisonment is imposed with different options: with or without the possibility of parole depending on the severity of the crime and the social risk of the crime, the minimum sentence served, etc. A particular problem is the execution of this criminal sanction from the aspect of: application of the treatment, preservation of security in the institution both personal and general, preservation of the minimum level of mental health of convicts, accommodation of these convicts - individually or in a group with other categories of convicts, etc. International organizations dealing with human rights of prisoners recommend that prison institutions take advantage of all the opportunities that treatment provides in order to, as far as possible, preserve health, moral and spiritual strength of every prisoner who is serving a life sentence in prison, as a human, moral and civilized act of society. Shaw, Kristi Lee, e Geoff Bridgman. Creating Appreciation and Community Support for Mothers Caring for a Child with an Anxiety Disorder. Unitec ePress, febbraio 2023. http://dx.doi.org/10.34074/mono.097.

Testo completo
Abstract (sommario):
This research examined a unique approach to anxiety disorder, one of the most prevalent and growing mental health concerns internationally. It uncovered the mostly invisible and challenging experiences of mothers caring for a child with an anxiety disorder and the value of their reciprocal relationships with their children for both their health and wellbeing. In addition, it explored social identity in making meaningful connection using a generative action-oriented social approach to address anxiety in the community. An appreciative inquiry, using social constructionist theory, and underpinned by elements of kaupapa Māori values, was utilised to explore the research questions. The data was collected via paired interviews, focus groups and small questionnaires with three to four mothers, after which thematic analysis was undertaken to identify important themes. There were four key themes discovered in the findings: (1) the mothers’ ongoing and challenging experiences of being silenced and isolated on the fringes, navigating the quagmire of social and institutional systems to help them help their children; (2) the mothers’ learning to cope by creating calm in the home, the child, and in themselves, often requiring them to ‘suspend’ their lives until their children become more independent; (3) the mothers employing a mother as advocate identity to face the challenges, and co-creating a mother as advocate group identity to continue to face those challenges to design a collective initiative;and (4) the value of freedom that the mothers experienced participating in the appreciative inquiry process with other mothers facing similar challenges and sharing their stories. This study demonstrates how appreciative inquiry is aligned with and supports the value of social identity theory and creating meaningful connections to help position and address anxiety disorder in the community. A key insight gained in this study is that our current social and institutional systems create disconnection in many facets of Western life, which contributes to the generation and perpetuation of stigmatisation, isolation and anxiety disorder. Within a Western capitalistic and individualistic culture, mental illness has become predominantly pathologised and medicated, positioning anxiety disorder within the child, and relegating the social dimension of the biopsychosocial approach as almost irrelevant. As mothers in this system spend valuable energy advocating for more support for their children, they put their own mental health at risk. There is no one solution; however, this study demonstrates that when mothers are supported through an appreciative inquiry process, strengthening their personal and social identities, there is the potential for health and wellbeing to increase for them, their children and the community.

Social and Structural Determinants of Maternal Morbidity and Mortality: An Evidence Map. Agency for Healthcare Research and Quality (AHRQ), dicembre 2023. http://dx.doi.org/10.23970/ahrqepccer264.

Testo completo
Abstract (sommario):
Objective. The purpose was to review available evidence of risk factors associated with maternal morbidity and mortality in the United States during the prenatal and postpartum periods to inform a National Institutes of Health Pathways to Prevention Workshop: Identifying Risks and Interventions to Optimize Postpartum Health, held November 29–December 1, 2022. Data sources. We searched MEDLINE®, CINAHL®, and the Social Sciences Citation Index through November 2022. Review methods. We searched for observational studies examining exposures related to social and structural determinants of health and at least one health or healthcare-related outcome for pregnant and birthing people. We extracted basic study information and grouped studies by social and structural determinants of health domains and maternal outcomes. We prioritized studies according to study design and rigor of analytic approaches to address selection bias based on the ROBINS-E. We summarize all included studies and provide additional descriptions of direction of association between potential risk exposures and outcomes. Results. We identified 8,378 unique references, with 118 included studies reporting social and structural determinants of health associated with maternal health outcomes. Studies covered risk factors broadly, including identity and discrimination, socioeconomic, violence, trauma, psychological stress, structural/institutional, rural/urban, environment, comorbidities, hospital, and healthcare use factors. However, the risk factors we identified represent only a subset of potential social and structural determinants of interest. We found an unexpectedly large volume of research on violence and trauma relative to other potential exposures of interest for pregnant people. Outcome domains included maternal mortality, severe maternal morbidity, hypertensive disorders, gestational diabetes, cardio/metabolic disorders, weathering (the physiological effect of premature aging caused by chronic stressful experiences), depression, other mental health or substance use disorders, and cost/healthcare use outcomes. Depression/other mental health outcomes represented a large proportion of medical outcomes captured. Risk of bias was high, and rarely did studies report the excess risk attributable to a specific exposure. Conclusions. Identifying risk factors pregnant and birthing people face is vitally important. Limited depth and quality of available research within each social and structural determinant of health impeded our ability to outline specific pathways, including risk factor interdependence. While more recently published literature showed a trend toward increased rigor, future research can emphasize techniques that estimate the causal impacts of risk factors. Improved reporting in studies, along with organized and curated catalogues of maternal health exposures and their presumed mechanisms, would make it easier to examine exposures in the future. In the longer term, the field could be advanced by datasets designed to more fully capture the data required to robustly examine racism and other social and structural determinants of health, in combination with their intersections and feedback loops with other biologic/medical risk factors.

Abstract (sommario):
Doctoral students have higher rates of mental ill health than comparable populations. Contributors include institutional stressors such as competitive fields, uncertain futures and liminal professional identity. This exploratory study drew on social psychology, taking a broad narrative approach, to explore what professional (academic) communities postgraduate researchers (PGRs) identify with, and how these identifications relate to wellbeing. Focus groups were conducted with social science and humanities PGRs in three UK Russell Group universities. PGRs experiences were diverse, but common themes related to ambiguity about their roles as students and researchers; the precariousness of academic careers; commitments to scholarly research; the importance of validation from supervisors and the wider academic community; and the particular challenges when other social roles (e.g. relating to ethnicity or parenthood) align poorly with academic roles. Key conclusions are the importance of validating and supportive research communities that did not necessarily map onto departments or disciplines; meaningful and practically empowering supervisory relationships, which can serve as a buffer against stress and uncertainty; and the relative paucity of ‘postgraduate/doctoral researcher/student’ as a social identity.

Abstract (sommario):
Background: Major depressive disorder is a common mental disorder. Many pressing questions regarding depression treatment and outcomes exist, and new, efficient research approaches are necessary to address them. The primary objective of this project is to demonstrate the feasibility and value of capturing the harmonized depression outcome measures in the clinical workflow and submitting these data to different registries. Secondary objectives include demonstrating the feasibility of using these data for patient-centered outcomes research and developing a toolkit to support registries interested in sharing data with external researchers. Methods: The harmonized outcome measures for depression were developed through a multi-stakeholder, consensus-based process supported by AHRQ. For this implementation effort, the PRIME Registry, sponsored by the American Board of Family Medicine, and PsychPRO, sponsored by the American Psychiatric Association, each recruited 10 pilot sites from existing registry sites, added the harmonized measures to the registry platform, and submitted the project for institutional review board review Results: The process of preparing each registry to calculate the harmonized measures produced three major findings. First, some clarifications were necessary to make the harmonized definitions operational. Second, some data necessary for the measures are not routinely captured in structured form (e.g., PHQ-9 item 9, adverse events, suicide ideation and behavior, and mortality data). Finally, capture of the PHQ-9 requires operational and technical modifications. The next phase of this project will focus collection of the baseline and follow-up PHQ-9s, as well as other supporting clinical documentation. In parallel to the data collection process, the project team will examine the feasibility of using natural language processing to extract information on PHQ-9 scores, adverse events, and suicidal behaviors from unstructured data. Conclusion: This pilot project represents the first practical implementation of the harmonized outcome measures for depression. Initial results indicate that it is feasible to calculate the measures within the two patient registries, although some challenges were encountered related to the harmonized definition specifications, the availability of the necessary data, and the clinical workflow for collecting the PHQ-9. The ongoing data collection period, combined with an evaluation of the utility of natural language processing for these measures, will produce more information about the practical challenges, value, and burden of using the harmonized measures in the primary care and mental health setting. These findings will be useful to inform future implementations of the harmonized depression outcome measures.

Murphy, Keire, e Amy Stapleton. Access to autonomous housing for beneficiaries of International Protection in Ireland. ESRI, giugno 2024. http://dx.doi.org/10.26504/rs184.

Abstract (sommario):
Housing plays a key role in the integration of beneficiaries of international protection (BIPs). However, past research has shown that BIPs often face significant barriers in accessing housing. Many countries provide supports to access autonomous housing as a result of this. These barriers pose a challenge for BIPs, who can find themselves with limited options to exit institutional or homeless accommodation, as well as for reception systems, which require outflow to accommodate new applicants. Understanding the challenges, barriers, and good practices is therefore a crucial element of the governance of accommodation of international protection applicants (IPA) in Ireland, as well as the integration of BIPs. However, there is limited recent research on the policies and supports in place for this group, their outcomes, and the challenges and barriers faced. This report considers the situation of those who arrived in Ireland, applied for international protection, and were granted this status. Applicants for international protection, beneficiaries of temporary protection (BOTP) fleeing the conflict in Ukraine, resettled refugees, and other third-country nationals residing in Ireland are outside of the scope of this study. The report is based on desk research, interviews, and stakeholder consultation. The research found that BIPs face significant challenges and barriers in trying to access autonomous housing, and that these have resulted in over 6,000 people with status remaining in International Protection Accommodation Services (IPAS) accommodation as of January 2024. One of its key findings is that some major challenges are actually mainstream issues not specific to this group. Examples include severe shortages in the supply of social housing and affordable rental housing, inadequacies in supports such as the Housing Assistance Payment (HAP), a lack of resources and high staff turnover among frontline workers in public services, and a lack of coordination or communication between different agencies or departments. Alongside these mainstream issues, and reflecting recent European comparative research (EMN, 2024), this study also found that BIPs experience additional challenges and barriers. In Ireland, these include language barriers and a lack of knowledge or information about the system, psychological issues arising from their background and the international protection system, and discrimination in the housing market. In addition, this study shows that many challenges in the Irish context arose from existing policies concerning this group, such as isolated reception centres, which impact on employment outcomes, mental health, access to services, and social connections. Insufficient resources for transition services, the recent IPAS policy of issuing transfers to different centres, and a lack of monitoring of the housing outcomes of BIPs once they leave IPAS accommodation also emerged as challenges. Other difficulties reported include a lack of knowledge or clarity within local authorities about the rights and entitlements of this group, as well as a lack of planning for families arriving through the process of family reunification.
