Abstract (sommario):

Historians of pragmatism have long overlooked Kenneth Burke and Richard McKeon. This has not been without good reason. At first glance, the two read more like critics than adherents of the tradition. Yet placing Burke and McKeon's writings from the 1920s to the late 1950s in the context of their development reveals a shared project aimed at reforming pragmatism. While pragmatists such as John Dewey and Sidney Hook alleged a conceptual fidelity between the scientific method and democratic processes such as public debate, Burke and McKeon questioned this link. Metaphors drawn from science, they believed, blinded pragmatists to the nature of communication. Due to this oversight, pragmatists ignored the ideological ambiguity that surrounded terms like “science” and “democracy” during the mid-twentieth century. Burke and McKeon sought to fix this omission. Pragmatists, they argued, needed to trade the language of science for a terminology drawn from a source more attuned to the power of communication: the arts. By advancing this case, Burke and McKeon crafted an aesthetic form of pragmatism—a variant of the philosophy that, ultimately, contemporaries would barely recognize as such.

This essay builds on M. McKeon’s premises in The Origins of the English Novel (1987) in order to explore how Samuel Richardson’s Pamela, Or Virtue Rewarded and Henry Fielding’s Joseph Andrews struggle to diminish social and cultural instabilities that persisted in the fluctuating attitudes, namely; the aristocratic, progressive and conservative, towards the individual’s honor in their respective social orders. The novels supposedly, according to McKeon, set to resolve these instabilities; however, the essay underscores the novels’ inability to effectively achieve this objective. The essay demonstrates that both Pamela and Joseph Andrews instead internalized the ideological instability as they failed to uphold distinct reformative perspective at the expense of the other. The novels are usually read as culprits of progressive ideals; that is, they advocate for assessing individuals’ worth according to their actions rather than their lineage, which aristocracy historically instilled as a criterion. However, the essay illustrates how the novels simultaneously exhibit the persistence of that conservative model, recycling of aristocracy, which defines people’s honor based on their nurture.

Abstract (sommario):

Abstract Background: CCTG MA38 (NCT02630693) was a randomized phase II clinical trial that evaluated the efficacy of 2 dose schedules of palbociclib [100mg PO on continuous daily dose (CDD)] compared to 125mg PO daily for 3 weeks on/1 week off (STD) along with physician-choice of endocrine therapy in patients with ER+, HER- metastatic breast cancer (MBC) in the second-line setting. The final analysis indicated that palbociclib had comparable efficacy, safety and QOL for both treatment arms (SABCS 2018, abstr PD1-10). Our lab has reported that IL-8 was highly expressed by primary human breast cancers, and that higher pretreatment plasma IL-8 was significantly correlated with elevated bone resorption in HR+MBC patients (Kamalakar A et al. Bone 61:176-85, 2014). Most recently, our lab has reported that elevated serum IL-8 predicts for significantly reduced OS in 3 large metastatic cohorts of pancreatic (ASCO 2019, abstr 4131), prostate (ASCO 2020, abstr e17565) and breast cancer patients (ASCO 2020, abstr 1067). In this retrospective study, we investigated the prognostic value of pretreatment plasma IL-8 in CCTG MA38. Methods: 123 patients enrolled in MA38 were analyzed in this retrospective biomarker study. Serum IL-8 levels were measured using the ELLA immunoassay platform (ProteinSimple, San Jose, CA). Kaplan-Meier analysis and log-rank test were used to correlate plasma IL-8 levels with OS and progression-free survival (PFS). Results: In the total study cohort, pretreatment plasma IL-8 concentration had a median of 11.10 pg/ml, and 25% and 75% interquartiles of 7.36 and 15.00 pg/ml, respectively. In univariate analysis higher plasma IL-8 was a significant adverse biomarker for reduced PFS as a continuous variable (p = 0.01), at the median cutpoint (HR= 1.55, p=0.042), and in quartile cutpoints (HR=2.28, p=0.03, Q4 vs Q1). For OS, higher plasma IL-8 also trended significant for reduced OS at the median cutpoint (HR= 1.66, p=0.10). In multivariate analysis, higher plasma IL-8 also trended significant for reduced PFS (HR= 1.47, p=0.094). Conclusions: In the CCTG MA.38 trial, higher pre-treatment plasma IL-8 level was associated with reduced PFS, and is therefore an adverse prognostic biomarker for reduced outcome to the CDK 4/6 inhibitor palbociclib. Anti-IL-8 therapy combined with CDK 4/6 inhibitors should be evaluated in future trials to improve outcome in patients with higher circulating IL-8. Abstract OBJECTIVES To compare the long-term overall survival and outcomes of patients with oesophageal squamous cell cancer treated with minimally invasive McKeown or Ivor Lewis oesophagectomy. METHODS A multicentre, non-interventional, retrospective, observational study was performed in oesophageal squamous cell cancer patients pathologically confirmed with stage IA–IIIB middle or lower thoracic tumours who underwent minimally invasive oesophagectomy between 1 January 2010 and 30 June 2017 in 7 hospitals in China. Cox proportional hazards models assessed factors associated with overall survival and disease recurrence. The primary outcome was overall survival and cancer recurrence; the secondary outcomes included number of lymph nodes resected, 30-day mortality and postoperative complications. RESULTS A total of 1540 patients were included (950 McKeown, 590 Ivor Lewis). The mean age was 61.6 years, and 1204 were male. The mean number of lymph nodes removed during the McKeown procedure was 21.2 ± 11.4 compared with 14.8 ± 8.9 in Ivor Lewis patients (P < 0.001). The 5-year overall survival rates were 67.9% (McKeown) and 55.0% (Ivor Lewis). McKeown oesophagectomy was associated with improved overall survival (Ivor Lewis versus McKeown hazard ratio 1.36, 95% confidence interval 1.11–1.66; P = 0.003), particularly in patients with stage T3 tumours (middle thoracic oesophagus). However, postoperative complications occurred more frequently following McKeown oesophagectomy (42.2% vs 17.6% Ivor Lewis; P < 0.001). CONCLUSIONS Minimally invasive McKeown oesophagectomy was associated with improved overall survival and a decreased risk of disease recurrence, while Ivor Lewis patients had fewer postoperative complications. McKeown oesophagectomy may represent the optimal technique for patients with stage T3 tumours. Clinical trial registration: clinicaltrial.gov NCT03428074

4509 Background: Robust evidence is lacking whether Ivor Lewis minimally invasive esophagectomy (MIE) or McKeown MIE should be preferred for patients with mid to distal esophageal or gastro-esophageal junction Siewert I-II (GEJ) cancer. Methods: In this multicenter randomized controlled trial, patients with esophageal (below the level of the carina) or GEJ cancer planned for curative resection were recruited. Eligible patients were randomly assigned (1:1) to either Ivor Lewis MIE or McKeown MIE. The primary endpoint was anastomotic leakage (AL) requiring endoscopic, radiologic or surgical intervention. Secondary outcome parameters were overall AL rate, postoperative complications, length of stay and mortality. Results: A total of 262 patients were randomly assigned to Ivor Lewis MIE (n = 130) or McKeown MIE (n = 132). Seventeen patients were excluded from the trial due to not meeting inclusion criteria (n = 2), physical unfitness for surgery (n = 3), patients’ choice (n = 3), interval metastases (n = 5) or peroperative metastases (n = 4). AL necessitating reintervention occurred in 15 (12.3%) of 122 patients after Ivor Lewis MIE and in 39 (31.7%) of 123 patients after McKeown MIE (relative risk 0.39, 95% CI 0.22-0.65; risk difference 19.4%, 95% CI 7.9%-31.8%). Overall AL rate was 12.3% after Ivor Lewis MIE and 34.1% after McKeown MIE. Severe complications (Clavien-Dindo ≥ 3b) were observed in 10.7% after Ivor Lewis MIE and in 22.0% after McKeown MIE. Pleural effusion requiring drainage occurred in 9.8% of patients after Ivor Lewis MIE and 21.1% of patients after McKeown MIE. RLN palsy rate was 0% after Ivor Lewis MIE and 7.3% after McKeown MIE. Median length of hospital stay was 10 days (IQR 8 – 15 days) after Ivor Lewis MIE and 12 days (IQR 9 – 18 days) after McKeown MIE. ICU length of stay and mortality rates were comparable between groups. Conclusions: These findings provide evidence for a lower rate of AL requiring reintervention after Ivor Lewis MIE compared to McKeown MIE for patients with mid to distal esophageal or GEJ cancer. Aims: This study aimed to determine the degree of reflux esophagitis after either intrathoracic or cervical esophagogastrostomy in patients with esophagogastric junction carcinoma. Patients and Methods: The study population consisted of 10 and 15 consecutive patients who underwent esophagectomy with gastric conduit reconstruction via intrathoracic (Ivor Lewis) or cervical (McKeown) esophagogastrostomy, respectively. Reflux esophagitis was evaluated annually after surgery and scored on a 0- to 4-point scale corresponding to grades N/M, A, B, C, and D, respectively. The reflux esophagitis score of each patient, defined as the average of scores at 1, 2, and 3 years after surgery, was compared between the groups. Results: Of the 30 planned annual endoscopic follow-ups (3 years in 10 patients) in the Ivor Lewis group and 45 planned follow-ups (3 years in 15 patients) in the McKeown group, 24 and 29 such follow-ups were performed in the Ivor Lewis and McKeown groups, respectively. The reflux esophagitis score was significantly better in the McKeown group than in the Ivor Lewis group (0.51 ± 0.24 versus 1.46 ± 0.29, P = 0.019). Overall survival did not significantly differ between the Ivor Lewis and McKeown groups (respective 5-year survival rates, 64% versus 57%, P = 0.75). Conclusions: The degree of reflux esophagitis may be greater in patients with esophagogastric junction cancer treated by Ivor Lewis esophagectomy than in those treated by McKeown esophagectomy. McKeown esophagectomy might be a more suitable method for the treatment of esophagogastric junction cancer with extended esophageal invasion.

Abstract (sommario):

The aim of this study was to compare the perioperative outcomes and long-term survival rates of the McKeown and Sweet procedures in patients with esophageal cancer younger than 70 years or older than 70 years. A total of 1432 consecutive patients with esophageal squamous cell carcinoma (ESCC) who received surgery at Sun Yat-sen University Cancer Center from January 2009 to October 2012 were analyzed. Propensity score matching was used to balance the clinical characteristics of the patients who underwent different surgical approaches, and 275 and 71 paired cases were matched among those younger and older than 70 years, respectively. The prognosis and postoperative outcomes were compared between the McKeown and the Sweet esophagectomy. For patients younger than 70 years, those who underwent the McKeown procedure had better overall survival (OS) than those in the Sweet group (log rank = 4.467; P = .035). However, no significant difference in disease-free survival and OS was observed between two approaches for the elderly patients (log rank = 1.562; P = .211 and log rank = 0.668; P = .414, respectively). Cox regression analysis revealed that McKeown approach was a positive prognostic factor compared to the Sweet approach for patients younger than 70 years in univariable analysis (HR = 0.790; 95% CI, 0.625-0.997; P = .047), whereas the surgical approach was not significantly related to the prognosis in the elderly patients. For patients older than 70 years, the occurrence of anastomotic fistula increased in those who underwent the McKeown procedure (23.9% vs 11.3%, P = .038, for the McKeown and Sweet esophagectomy, respectively). The McKeown approach increases the OS in younger patients with ESCC. However, for patients older than 70 years, the Sweet approach was proven to be an effective therapy, given the better perioperative outcomes and similar long-term survival compared with patients in the McKeown group.

Two Australian genera, Tessaromma Newman and Phlyctaenodes Newman of the tribe Phlyctaenodini Lacordaire, 1868 (=Tessarommatini Lacordaire, 1868) syn. nov. are revised. All known species are redescribed and illustrated. Keys to the genera of Australian Phlyctaenodini and species of Tessaromma Newman and Phlyctaenodes are provided. One new genus Escalonia gen. nov. (type species: Tessaromma loxleyae McKeown, 1942) and three new species Phlyctaenodes queenslandicus sp. nov., Escalonia carolinae sp. nov. and Escalonia surprise sp. nov. are described. Tessaromma truncatispina McKeown, 1940 is regarded as a junior synonym of Tessaromma sordida McKeown, 1940; Tessaromma nigroapicale Aurivillius, 1917 is synonymized with Tessaromma nanum Blackburn, 1899; and Tessaromma sericans (Erichson, 1842) is synonymized with Tessaromma triste (Hope, 1841). Tessaromma setosa McKeown, 1942 is moved to Ectinope Pascoe, and Zoedia intricata Gressitt, 1959 is moved to Escalonia gen. nov.

Abstract (sommario):

One admires and applauds the enterprise of anyone who uses Gauss’s 1801 Disquisitiones arithmeticae as the starting point for mathematical exploration. I enjoyed McKeon and Sherry’s description of their journey [1] and the challenge of their conjectures. They drew attention to a class of polynomials that satisfy what they called the double angle condition ((1) below). Unfortunately, their failure to work with an appropriate definition of cyclotomic polynomials seriously handicapped their computer-aided attempt to classify double angle polynomials. Once this is remedied, a pleasant classification emerges, at least for polynomials with rational coefficients, without recourse to a computer. The main aim of this article is to present this classification. A brief final section considers McKeon and Sherry’s conjectures about irreducible double angle polynomials.

The composition of the cerambycine tribe Strongylurini is investigated. Results from cladistic analyses indicate two clades that may together constitute a single higher clade that represents the tribe Strongylurini excluding Bebius Pascoe. The first clade comprises Phantissus McKeown, Exaeretiformis McKeown and Lygesis Pascoe and the other Opsidota Pascoe, Strongylurus Hope, Piesarthrius Hope and Aprosictus Pascoe. ‘Piesarthrius’ gearyi McKeown is clearly misplaced in Piesarthrius. The endophalli of Piesarthrius marginellus Hope and P. gearyi are illustrated for the first time and provide some useful characters for assessing phylogenetic relationships.

Abstract Background This study aims to investigate the feasibility of single-port video-assisted thoracoscope (SPVATS) in treating thoracic esophageal squamous cell carcinoma (TESCC) using McKeown approach. Methods METHODS: 25 McKeown approach-based SPVATS surgeries (19 males and 6 females, aged 42–70years) were carried out from January 2015 to December 2017 to treat TESCC. Results RESULTS: All the patients were successfully completed SPVATS, with average thoracic surgery time as 150 min, intraoperative blood loss as 30–260 ml (average 90 ml), and postoperative hospital stay as 9–16d (average 12d). Conclusion SPVATS was technically feasible and safe in treating TESCC using McKeown approach, with less trauma and rapid postoperative recovery, so it could be used as a new surgical option for McKeown approach-based TESCC treatment. This article discusses the relationship between inner space (the mind/consciousness) and perceptions of outer space (the extraterrestrial) in Western psychedelic cultures. In particular, it analyses the writings and lectures of Terence McKenna, the most influential psychedelic thinker since the 1960s. Assimilating a broad range of ideas taken from esotericism, shamanism, and science fiction, McKenna became the principal architect of an occult theory of psychedelic experiences referred to here as psychedelic ufology. The article further argues that McKenna was formatively influenced by the ideas of Carl Jung and that, as such, much subsequent psychedelic ufology tends to be Jungian.

Background. Jejunostomy is the main form of enteral nutritional support after McKeown-type esophagectomy. However, this requires the jejunum to be secured to the abdominal wall, which can lead to catheter-related complications. Here, we present a new type of jejunostomy, ultra-proximal jejunostomy, which does not require fixation of the jejunum to the abdominal wall. Methods. Patients who underwent McKeown-type esophagectomy between January 2021 and March 2022 were included in this study. Postoperative outcomes of patients who underwent ultra-proximal jejunostomy are also presented. Results. Forty-three patients were able to receive enteral nutritional support via an ultra-proximal jejunostomy after McKeown-type esophagectomy, and no cases of enteral fistulas were observed. The pain in the left lower abdomen largely disappeared after the removal of the jejunostomy tube in all patients, and there was no difficulty in removing the tube. To date, none of these patients have experienced bowel obstruction or jejunal torsion. Conclusion. An ultra-proximal jejunostomy is a safe and feasible method and a better option for enteral nutrition support after McKeown-type esophagectomy.

The McKenna reaction is a well-known and popular method for the efficient and mild synthesis of organophosphorus acids. Bromotrimethylsilane (BTMS) is the main reagent in this reaction, which transforms dialkyl phosphonate esters into bis(trimethylsilyl)esters, which are then easily converted into the target acids. However, the versatile character of the McKenna reaction is not always used to its full extent, due to formation of side products. Herein, demonstrated by using model examples we have not only analyzed the typical side processes accompanying the McKenna reaction, but also uncovered new ones. Further, we discovered that some commonly recommended precautions did not always circumvent the side reactions. The proposed results and recommendations may facilitate the synthesis of phosphonic acids.

Abstract Background Both Ivor Lewis (anastomosis in the chest) and McKeown (anastomosis in the neck) esophagectomy has been used to treat patients with esophageal cancer. It is unclear in literature about the survival difference performed by these two methods. Methods A prospective randomized trial enrolling 100 patients with esophageal cancer in the middle or lower esophagus was done to compare the survival outcome treated with minimally invasive esophagectomy by Ivor Lewis and McKeown approaches. Analysis for the retrospective patient cohort (n = 253) including Ivor Lewis (n = 115) and McKeown (n = 138) MIE in the same hospital was also performed. Results There is no significant difference in the overall and disease progression-free survival duration between the two groups of patients (n = 50 for each group) (Figure 1 for overall survival). Similar results were found when the comparison was done for the retrospective and whole patients cohort. Multivariate analysis demonstrates the TNM staging of the tumor to be the single significant factor for prognosis in terms of overall and disease progression-free survival. There was no significant difference in overall and disease progression- free survival between the patients with Ivor Lewis and McKeown MIE both in prospective and retrospective study cohort. Conclusion Ivor Lewis and McKeown MIE provide a similar survival results for the patients with esophageal cancer in the middle and lower thoracic esophagus. In her well-known piece on Walter Benjamin, Hannah Arendt wrote that “posthumous fame… seems to be the lot of the unclassifiable ones.” It is achieved by “those whose work neither fits the existing order nor introduces a new genre that lends itself to future classification.” If she is right, then there may be some hope that Richard McKeon will one day have his moment. For McKeon (who was, in fact, a friend of Arendt) is eminently unclassifiable. Born in 1900, he studied philosophy both with the great French medievalist Étienne Gilson and with John Dewey. He was a twentieth-century American pragmatist who sought to revolutionize philosophy so that it could deal with the novel challenges of a technological age. Yet he was also a brilliant scholar of classical and medieval thought, who wrote such articles as “Poetry and Philosophy in the Twelfth Century,” “Rhetoric in the Middle Ages,” and “The Hellenistic and Roman Foundations of the Tradition of Aristotle in the West.”

The emergence, growth and development of The Church of Pentecost (CoP), Ghana is a demonstration of a new era of African Christianity. The Church of Pentecost was listed as the largest Protestant Christian denomination in Ghana by the last empirical church survey of the Ghana Evangelism Committee. In 193 7, James and Sophia McKeown were sent by the Apostolic church in the UK to the then Gold Coast (now Ghana) as missionaries. Working in partnership with the indigenous converts and personnel, McKeown worked tirelessly in Ghana to bring the Gospel of Jesus Christ to the country through his emphases on the word of God, the transforming power of the Holy Spirit, and holy living. In 1953 McKeown was compelled to come out of the Apostolic Church of the UK and get along with the indigenous people under the name 'Ghana Apostolic Church' which was renamed 'The Church of Pentecost' in 1962.
