This concluding chapter discusses the departure of the Chinese from their involvement in lumbering, years after making a significant contribution to the building of the American West. In the 1880s Chinese immigrants constituted the majority of the men employed in the lumber trade in the Sierra Nevada. They undertook a variety of jobs, from wood transportation and mill operation to digging ditches, grading roads, cooking and cleaning, and caring for the animals. The wages they earned were determined by the type of job they did. This chapter shows that Chinese laborers in the logging industry had moved either into other occupations or to work in other forests—some even returned to China—by 1920 due to a variety of factors, such as the emerging forest conservation movement, technological improvements in logging, decreased demand for lumber, and the rise of large corporations that drove the smaller lumber firms out of business.

Lumbar spondylolisthesis typically presents as low back and/or lower extremity pain. Spondylolisthesis is most commonly observed in female patients and the elderly. Lumbar spondylolisthesis may result from congenital, isthmic, trauma-related, degenerative, and iatrogenic causes. Diagnostic imaging (radiographs, magnetic resonance imaging) and physical examination are needed to differentiate the categories of lumbar spondylolisthesis, which will assist in selecting the appropriate treatment. A customized interdisciplinary treatment plan tailored to the patient’s presentation and goals will produce clinical improvement. Surgical remediation is typically reserved for recalcitrant cases but can be effective.

Lumbar neurogenic claudication, sometimes referred to as pseudoclaudication, is the clinical syndrome of back pain radiating down one or both legs during ambulation. Classically, the symptoms abate with forward flexion of the lumbar spine and worsen with extension. The condition arises from lumbar spinal stenosis, which is common in the elderly population. Many asymptomatic individuals have lumbar spinal stenosis seen on magnetic resonance imaging (MRI), so this syndrome is a clinical diagnosis. The majority of patients have favorable responses with conservative treatment, which includes physical therapy, bracing, medications, and epidural steroid injections. Patients who do not improve may go on to have surgery. Spinal stenosis has become one of the most common reasons to undergo lumbar spinal surgery in patients older than 65 years of age.

A careful history will aid in the appropriate diagnosis of LSS, although the physical examination findings may be nonspecific. While exact diagnostic criteria for LSS have not been agreed upon, radiologic and electrodiagnostic studies can help confirm the diagnosis. Treatment generally progresses from conservative to aggressive. High-quality evidence regarding definitive treatment options is lacking, so therapeutic options should be discussed on a case-by-case basis.

Lumbar epidural injection is most often performed for patients experiencing low back pain with radicular symptoms. The radicular symptoms can be precipitated by disc herniation or foraminal stenosis. In addition, spinal stenosis with associated neurogenic claudication is another common indication for this injection. These procedures may be effective in treatment of other syndromes that are associated with radiculopathic low back pain, including intervertebral disc degeneration without disc herniation, central spinal stenosis, spondylothesis, and failed lumbar back surgery syndrome. Lumbar epidural steroid injection (LESI) is the most commonly performed intervention. Fluoroscopically guided lumbar epidural injections led to a lower rate of complications than that reported for all lumbar epidural injections.

Discogenic lumbar pain is a frequent cause of prolonged disability. Currently, there are few effective minimally invasive therapeutic options to treat diseased discs and provide a long-term pain relief. Intradiscal biacuplasty improves functional capacity and affords pain relief in properly selected patients. Provocative discography is a relatively invasive intradiscal technique that has been used as a diagnostic tool to help to detect painful discs and associated morphological changes. One of the effective therapeutic approaches to control discogenic pain is to use an ablative radiofrequency intradiscal procedure, like biacuplasty. Intradiscal electrothermal therapy (IDET) is currently in limited use. Serious complications of intradiscal procedures are rather rare.

Lumbar sympathetic blocks (LSB) result in the interruption of the sympathetic efferent fibers to the lower extremities with sparing of the somatic nerves, thus providing a diagnostic value as to the relative sympathetic contribution to the patient’s pain syndrome. In those patients with significant sympathetically maintained pain, repeated blocks may provide a therapeutic value and help facilitate physical therapy. The original described technique is the paramedian or “classic” approach described by Mandl in 1926. A more lateral approach was later developed by Reid and colleagues. The incidence of complications related to lumbar sympathetic blockade is minimal. The complications result either from insertion and manipulation of the needle or as a direct result of the injected solution.

Cervical and lumbar spine pain are major causes of disability and distress. Careful assessment is needed of the nature and extent of the problem, for diagnosis and exclusion of important (treatable) differential diagnoses, and for the formulation and engagement of the patient in an appropriate treatment plan. Acute spinal pain frequently does not indicate underlying joint pathology. Chronic spinal pain is often associated with intervertebral disc disease or which is often classified together with facet joint osteoarthritis as spondylosis. Sciatica, brachalgia, or spinal claudication may each be a consequence of either spondylosis or intervertebral disc prolapse. Simple mechanical low back and neck pain may respond well to conservative management with analgesics and physiotherapy. Specific spinal problems, such as neuronal compromise, may require additional treatments. The roles of injections and surgery in the management of spinal pain continue to evolve. Although ongoing management is largely determined by the individual's clinical response, comprehensive health economic analyses inform healthcare policies which may limit treatment availability. Many people with spinal problems suffer long-term or recurrent pain and disability, with significant psychological and social impact. Multidisciplinary approaches are needed to facilitate pain management and enable people with spinal pain to lead fulfilling lives when the underlying condition cannot be cured.

This chapter describes the relevant anatomy and sonoanatomy and the ultrasound-guided technique for lumbar medial branch blocks. The ultrasound-guided lumbar medial branch block is an intermediate level block. Prior to performing this block, it is important to have a detailed understanding of lumbar sonoanatomy in order to be able to target the correct level, the lumbar medial branch and the L5 dorsal ramus zones. In those individuals with body mass indexes in the ideal range, current studies suggest the L3 and L4 medial branches can be successfully targeted. The L5 dorsal ramus may be challenging secondary to the iliac crest, which may limit the ultrasound views needed for the target zone. Further technical and equipment advancements are needed to improve and reduce the existing limitations associated with the ultrasound-guided lumbar medial branch block technique.

Lumbar radicular pain is a significant burden for patients and is one of the most common complaints presented to spine practitioners. In the young, it is most often due to acute disc herniation, but in older patients, the onset is usually multifactorial. Most cases resolve on their own, but many patients experience recurrences. This chapter reviews a continuum of treatment options, from comprehensive physical rehabilitation to surgery. Understanding the pathophysiologic process may allow a more meaningful understanding of the development of chronic neuropathic pain of the extremity. A comprehensive history and physical examination, with findings confirmed by radiologic studies as warranted, may raise red flags early in the process and will facilitate efficient treatment of the underlying issues. An interdisciplinary approach may allow for earlier improvement and suitable treatment as needed.

For patients with multilevel lumbar spondylosis and back pain without leg symptoms, few surgical procedures exist to provide relief of pain. Axial back pain has been attributed in part to pain generated at the facet joints. The lumbar rhizotomy has emerged as an effective method for treatment of axial lumbar back pain in these patients. By performing a limited surgical approach and using an endoscopic method, patients report quick recovery without the pain that typically follows a larger surgical approach. This method provides effective means of ablating the medial branch of the dorsal root, with limited side effects and complications. The history of selective root rhizotomy is discussed, with focus on medial branch neurotomy for treatment of back pain. An endoscopically assisted rhizotomy technique using a minimally invasive approach to the lumbar spine is described. This technique may be expanded for use in other parts of the spine.
