An exploratory study was conducted to examine the effectiveness of a group gratitude intervention with 29 permanent residents at a long term care/ skilled nursing facility in improving elder mood, behavior, and well- being over a 3 week time period. The sample included individuals diagnosed with dementia, other cognitive impairment, major depressive disorder, insomnia, and generalized anxiety disorder. The gratitude intervention consisted of asking elders to share what they are thankful for at the dinner table each day. Measures included the Elder Well Being Scale and The Dinner Rating Scale. On both measures, higher scores indicated better functioning. To test the hypothesis that post treatment elder well-being will be significantly higher than pretreatment elder well-being ratings, a one-way ANOVA was conducted. Post-hoc tests revealed a statistically significant increase in Elder Well Being Scale scores. An ANOVA of comparing Dinner Ratings demonstrated a nonsignificant increase over the 3 week experiment. Implications are discussed.

Abstract (sommario):

Ainsworth and Bowlby’s Attachment Theory suggest that young children experience lasting effects of disconnection if separated from their primary caretaker. Foster children are legally removed from their primary caretakers, yet the effects of foster care on later social relationships of foster children is unknown.

The purpose of this exploratory study was to investigate the perceptions of adult foster children, ages 18 years and over, who were placed into foster care under 5 years of age. The major challenge was to obtain qualifying participants, first, because this is an invisible population in the American culture and, secondly, these individuals are hesitant to share their stories. The initial population included 5 persons from a local shelter and, using the snowball method, the researcher secured an additional 15 adult foster children that met the criteria.

Applying the phenomenological approach, these long interviews included 6 questions that addressed the research question: What do adults who have been in foster care placements prior to age 5 and have experienced more than 5 years in foster care placements from age 0-18 perceive are the long-term effects of foster care on their adult social relationships? The interviews occurred throughout Southern California and were taped and lasted 40 minutes to 2 hours, often filled with tears and high emotion. The participants included 3 with prior jail time, 6 who were homeless, 14 who were employed, 10 with an addiction, and 2 who were married. All had obtained a high school degree. All shared repeated unsuccessful friendship or romantic relationships.

The transcribed interviews were reviewed by 4 trained coders in a doctoral program and produced 8 themes, leading to the 8 conclusions. The primary conclusions are adult foster children express that abandonment is a deep core aspect of their psychological profile (95%); share the mental health issues of low self-esteem, lack of trust, and putting up walls in their social relationships (100%); act out their generational cycles of various addictive behavior relating to abandonment (100%); spirituality helped to stabilize more than half of these adult foster children through challenging times; and those without spiritual connections described the support of mentors in their lives.

The purpose of this study was to investigate depression and psychotherapy for adults who are in long-term care facilities. Depression is a serious problem for the elderly in general and for residents of nursing homes in particular. The current study drew on the dynamic stress vulnerability approach to explain how illness occurs in older people, using evidence obtained from the biological, social, and psychological domains with respect to depression. The research question addressed the difference in posttreatment depressive symptoms among 6 types of psychotherapy as measured by the Hamilton rating scale for depression (HRSD)? The 6 therapy techniques were (a) cognitive behavioral therapy, (b) supportive psychotherapy, (c) life review therapy, (d) reality oriented therapy, (e) mindfulness training, and (f) affect regulation therapy. A 1-group pretest-posttest research design was used with archival data from de-identified medical records. The analysis of this study controlled for pretreatment depressive symptoms as measured by the HRSD. A statistically significant main effect of psychotherapy was found, revealing a difference in posttreatment depressive symptoms as measured by the HRSD between at least 1 pair of the 6 types of psychotherapy after controlling for pretreatment HRSD. The covariate, pretreatment HRSD, was also statistically significant, indicating a relationship between pretreatment HRSD and posttreatment HRSD when controlling for psychotherapy group. This research study contributes to the breadth of information concerning efficacious treatments for depression among the elderly in nursing homes and can assist researchers, nursing homes, and doctors to promote positive social change by better treating the depressive symptoms in a pretreatment environment.

Through advancements in modern medicine, human beings worldwide are living longer. The increase in life expectancy creates a need for a more qualified and knowledgeable nursing workforce for the delivery of quality geriatric health care. Although nurses are the primary care providers for vulnerable older adults, they are generally not well trained or prepared in geriatric care. Therefore, the purpose of the project was to develop an education program aimed at improving nursing knowledge of geriatric care in the long-term care (LTC) setting. The goal of the project was to answer the research question: What impact would attendance in the program have on improving nursing geriatric knowledge, retention, and job satisfaction by project end? Guided by the advancing research and clinical practice through close collaboration (ARCC) model for evidence-based practice (EBP), the learning needs of nurses in the LTC setting were assessed. The 6-week education program was designed to provide education on the basics of geriatric care, pharmacology and aging, symptom evaluation, dementia care, and geriatric physical assessment. Project participants consisted of 8 nurses who volunteered to take part in the program. The program was evaluated using a pre-test and post-test method to examine nurse’s knowledge before and after the education program. Using a paired t test, the results indicated a significant increase (p < .05) between the mean pre-test (3.75) and mean post-test (4.25) scores of nurse knowledge of geriatric care. The education program improved participant knowledge of geriatric care. Positive social change will be achieved with this scholarly project as nurses with greater knowledge gain confidence, strengthen decision making and clinical skills, and improve patient outcomes in the LTC setting.

The aim of this research has been the examination of the long term psychological consequences of admission to the Intensive Care Unit (ICU) for critical illness. The major objectives were first, psychometric assessment at specified intervals post-discharge, using the General Health Questionnaire, Rosenberg Self Esteem scale, and the Impact of Event Scale, and secondly identification of ICU related variables which influence psychological wellbeing and recovery. The design was prospective and used survey methods. Seventy-two patients were recruited from the ICU at St James's University Hospital in Leeds. Data were collected at six weeks, six months and twelve months post-discharge. The initial analysis produced evidence of discrimination between subgroups, in particular age, length of stay in ICU, admission severity, indication for admission, communication, pre-existence of cancer, and the use of muscle relaxant drugs. Further analysis by way of a logistic regression identified four factors which may have predictive properties - age, admission severity, trauma and pre-existence of cancer. Patients from younger age groups reported more post traumatic stress symptoms than older patients; patients with pre-existing cancer also reported fewer post traumatic stress symptoms. Patients admitted following trauma reported poorer psychological outcome. Admission severity was negatively associated with psychological dysfunction, with those who were sickest on admission reporting fewer problems. Symptoms of post traumatic stress disorder were found in a number of patients; at final follow-up 27% of the surviving sample had medium levels, and 27% high levels of post traumatic stress symptoms. ICU patients form a fragile group to study and sample attrition was considerable. Nevertheless the findings are of interest to this developing research area and suggestions are made regarding their utilisation.

A stress, appraisal, and coping framework was used to explore the sources of stress for adult children of elderly parents residing in long-term care facilities and their perceived self-efficacy in managing the stressful events of caregiving. Sixty-four daughters and sons of parents in three long-term care facilities completed interviews and questionnaires. Events related to the parents' behaviour and cognitive status were more frequently appraised by their children as stressful and were appraised as more stressful than those related to assisting parents with activities of daily living or to events associated with the long-term care facility itself. Characteristics of the parent were more significantly related to the appraisal of caregiving events as stressful than were those of the adult child or the caregiving situation. In general, adult children were most confident in managing the stress associated with assisting their parents with activities of daily living and least confident in dealing with the stress related to interactions between their parents and the staff and between themselves and the staff. Practice implications for nurses working in long-term care facilities are discussed.

This study involved 58 persons with dementia living in three rural Canadian long-term care (LTC) facilities. In an attempt to find the relationship between these person's possible pain and levels of agitation, data on five proxy indicators of pain were collected and correlated with scores from the Pittsburgh Agitation Scale (PAS). Results indicated that three of the resident pain measures were significantly correlated with PAS scores. In particular, the palliative consultant pain ratings and the DS-DAT were strongly correlated with total PAS scores, and the five PAS sub-factors. Importantly, the PAS sub-factor of resistance to care was strongly correlated with three of the pain variables. Major study implications include the need for increased use of palliative pain consultants in LTC, and the need for nursing staff to realize that when demented residents resist care, it may be a potential clue that the resident is experiencing untreated pain.

Abstract (sommario):

The purpose of this study was to examine the phenomena of burnout and self-care as experienced by long-term anti-war activists. The current investigation of burnout and self-care was carried out using a qualitative transcendental phenomenological approach developed by Clark Moustakas. Interviews were conducted with eight adult men and women who had been active in the anti-war movement for at least ten years. The data collection consisted of in-depth interviews with open-ended questions regarding participants' experience of self-care and burnout in the process of anti-war activism.

Findings reveal that participants experienced burnout and self-care in much the same way as other professions. Participants experienced burnout within the following themes: stress, frustration and exhaustion; inner and outer pressure and responsibility; body shut-down; betrayal and isolation; and depression and grief. These activists experienced self-care within the following themes: enrichment of soul; satisfaction and celebration; great love and healing; awareness and keeping balance; deep understanding; and belief, enthusiasm and inspiration. In addition, the research uncovered the irony that while anti-war activism causes burnout at times, this same activism plays a critical role in self-care for the activist. Furthermore, findings lend support to previous studies of the experience of anti-war activists. Future research on this topic is suggested in several areas.

Moving into an institutional environment may cause feelings of anxiety, depression, grief or loss in the increasing number of elderly residents in long term care facilities. The lived experience of pet visitation was explored as a possible remedy. This study is significant because findings provide information about intervention strategies to assist residents in comfort and adjustment.Five themes, identified in ten interviews of residents in three midwest long term care facilities, illustrated that pet interaction: (a) contributed a sense of responsibility, environmental control, (b) afforded unconditional caring, companionship, (c) provided a connection with family and home. (d) furnished a sense of identity in the facility community, and (e) provided a sense of being protected within the facility. Evidence supports that pet visitation is a beneficial experience to the residents in long term care facilities. Study results are available for facility administrators as encouragement and justification to establish pet visitation programs.

The high turnover rate of graduate nurses is a challenge in the United States' hospitals because of high job dissatisfaction rates. The premature disaffiliation of the graduate nurses is costly for organizations and can significantly affect the quality and safety of patient care due to the inadequate supply of adequately prepared staff nurses, particularly in the long-term acute care hospitals. The purpose of the project was to decrease the turnover rate of graduate nurses in a long-term acute care setting from 40% to 20% through modification of the nurse residency program by applying an intervention based on Bauer and Erdogan's theory of organizational socialization. The intervention included provision of psychosocial and educational support to the graduate nurses in addition to the formal classroom learning and a preceptorship with experienced registered nurses. The project used a prospective descriptive design to examine whether the provision of psychosocial and educational support intervention to all newly hired graduate nurses for four weeks beyond the existing residency program would improve the nurses' perceptions on items of the Casey-Fink Graduate Nurse Experience Survey and decrease the turnover rate from 40% to 20%. Eighteen nurses completed the survey at eight weeks (the end of the existing residency program) and at 12 weeks (when four additional weeks of psychosocial and education support ended). The pretest and posttest survey results were compared using descriptive statistics. The graduate nurses' self-reported confidence level, job satisfaction level, and role transition experiences all improved after the intervention. The social change resulting from the project was positive residency learning experience for the graduate nurses that may result in better patient care, commitment to the organization, and retention of nurses.

Research studies show that work engagement is credited with having a positive effect on business outcomes. As health care leaders and administrators continue to make efforts to improve work engagement, the global nursing shortage has become a major problem that can affect registered nurses (RNs') work engagement and turnover intentions. RNs are important to the nation's well-being as everyone's health care needs involve the role of a nurse, but RNs currently in practice experience higher demands in an environment already constrained. This qualitative case study explored the perceptions of eight RNs in a long-term health care facility located in a Northeastern region of the United States utilizing semi-structured open-ended interviews. The study illustrated how RNs' work engagement could be improved and strengthened, turnover intentions minimized to make nurses' practice environment more attractive to motivate and retain nurses in long-term care setting. Study results suggest that nurses are highly engaged in their work though they encounter several issues including understaffing that creates work overload leading to burnout and job-related stress that challenge and undermine their engagement. Results also indicate that in order to improve RNs' practice environment and minimize turnover intentions and turnover, concerns regarding understaffing, job role stress, and nurse-management conflicts need to be addressed. Study results imply that to be effective as a nurse one has to be dedicated and committed to his or her work and the profession. Additionally, organizational leaders should make concerted effort to identify and address nurses' issues to create practice environments that attract and retain nurses.

Abstract (sommario):

The primary purpose of this clinical scholarly research project is to determine whether there are undiagnosed symptoms of depression among the residents in a small population Long Term Care facility (LTC). Caregivers at the LTC receive specific knowledge regarding older adult late-life depression and then apply said knowledge by surveying the LTC residents (LTCR’s) using the Geriatric Depression Scale instrument (GDS-15). The secondary purpose is to determine whether the LTC caregivers perceive this training as beneficial. Using two-sample t-test statistical analysis, the results of this study indicate a positive correlation with the alternative hypothesis; the number of LTCRs with previously undetected depressive symptoms increased. Ha: μ LTCR with depression or delirium ≠ μ LTCR with depression or delirium. In this study, the number of LTCRs with symptoms of depression or delirium increased by 60.4 percent. The P (T<=t) two-tail value is less than 0.001. In the literature, this difference is extremely statistically significant. The mean of the medical records examined minus the mean of the number of medical records with an indication of depression or delirium is 0.60. For a 95% confidence interval, this difference must fall between the ranges of 0.45 to 0.76. These data confirm the alternative hypothesis.

Additionally, the caregiver participant’s satisfaction outcome survey results reported the training was substantially beneficial.

The purpose of this study was to determine whether providing residents of the James H. Quillen Veterans Affairs Medical Center Community Living Center with a choice of nourishment options between meals would improve their satisfaction with the nourishment administration process. As opposed to being given the same nourishment item each day, residents were offered choices from a nourishment cart based upon their diet order. Sixteen residents completed the pre-survey, participated in the revised nourishment cart system for a 3-week period, and completed the postsurvey. Level of satisfaction was determined by analyzing the responses given by the residents on the pre- and post-surveys. There were increases in the overall level of satisfaction the residents felt with the incorporation of the new nourishment cart system.

The purpose of this study was to further theoretical and empirical understanding of care recipients' experiences within care relationships. Specifically, this study was concerned with identifying the strategies elders use to seek care and how the processes and characteristics of individual, interpersonal, and structural levels interact to affect elders' care seeking. A synthesized symbolic interactionist and life span developmental framework informed the research. Multiple qualitative field work methods (semi-structured interviews, participant observation, focus group) were utilized to collect information about care recipients who were representative of the variety of care relationships within a life care retirement community. The care seeking process emerged as a sequence of activities engaged in as elders elicited care from others and negotiated care with others. Of most conceptual interest, however, was elders' dependency work: The work elders engaged in as they not only sought care for a specific need but also situated each care occasion within past experiences of care and future possibilities for care. Dependency work was agency in action, serving to define the everyday lives of elders involved in care relationships. The findings of this study suggest the need for viewing care recipients as agents, actively being with others in their delicate dance of dependency.

Health professionals as well as families are being confronted with long-term care and caregiver issues that accompany the increasing incidence of individuals surviving traumatic brain injury. A sample of parents and brain-injured offspring from 20 families served as informants. The purpose of this study was to identify a qualitatively generated theory describing the parent's experience following a brain-injured child's return to the home setting. An exploratory qualitative design using grounded theory methodology was used during data collection and analysis. All the brain-injured offspring had survived a moderate-to-severe traumatic brain injury; were living with at least one parent; and were ages 17 to 34. A three-phase theory, Investing in the Comeback, was generated using grounded theory methodology. The theory's three stages, centering on fostering independence and seeking stability, describe the work of the parent living with a brain-injured offspring. The first phase, Centering On, involves the parent's focusing attention and behavior primarily on the brain-injured offspring. During Fostering Independence, the second phase, the parent initiates and maintains efforts to promote the offspring's resumption of independent functioning. The final phase, Seeking Stability, consists of the parent working to establish a regime that maintains the brain-injured offspring's optimal performance, while minimizing the strain on other family members. Theoretical sampling guided the identification of categories, properties, conditions, and consequences of each phase. Four quantitative measures supplied descriptions of sample characteristics and included demographics, cognitive deficit ratings of the child by the parent and the investigator, and the parent's perception of the family's functioning.

Background: This thesis examines the impact of dream or wish fulfilment on seriously ill children and their families. Dream or wish fulfilment is operationalised as the actualisation of a seriously ill child’s wish by a charity that provides desired experiences. Anecdotal reports suggest that the experience of having a dream or a wish fulfilled can provide seriously ill children and their families with a sense of hope and time away from illness. However, little empirical research has been conducted in this area. This thesis reports the impact of dream fulfilment on the psychosocial well-being of ill children and their families. The research questions are: what is the experience of having a dream fulfilled for the child? What is the impact of dream fulfilment on the family? Methods: A constructivist grounded theory methodology was adopted, using theoretical sampling to recruit families from across the UK. Twenty-one families were interviewed, including 15 dream recipients, 8 siblings, and 24 parents. Analysis followed the grounded theory methodology of simultaneous data collection and development of theory, resulting in analytic interpretations of participants’ worlds. Results and Conclusions: This thesis reports for the first time a theory and accompanying theoretical model, that explain the impact of dream fulfilment on families’ lives. The generated theory suggests that dream fulfilment was conceptualised as an alternative milestone in seriously ill children and their families’ lives. Additionally, the dream experience shifted perceptions of illness by providing instances and experiences where illness did not underscore family life. Findings additionally suggest that the dream fulfilment process provided families with ill children, who often felt excluded and stigmatised from services, with a period of much needed support. Findings also highlight the unintended negative consequences of dream fulfilment. Implications for Dreams Come True, and other dream and wish fulfilment organisations are discussed.

The goal of this thesis was to investigate the hypothesis whether symptomatic remission for patents with schizophrenia spectrum syndrome would provide practical and qualitative benefits for patients and for society. The first work (Paper I) examines the relationship between cross-sectional remission and the patients’ practical resources. The patients’ adaptations to society as well as their employments, living situations and social networks were studied. Their activity of daily living resources are described in terms of the extent to which they could pursue various undertakings autonomously. Their need for care was also studied. The results showed that the patient group that had achieved remission scored better on all of the variables studied, had better resources and less need for medical care and sheltered housing. The second work (Paper II) analysed the relationship between cross-sectional remission and quality of life, burden placed on patients by the illness, and the patients’ satisfaction with care received and with their medication, and also their capacity for understanding their symptoms and their illness. Even the results of this study showed that the patients in the remission group had a better quality of life, bore a lesser burden, were more satisfied with their care and had better insight into their illness. The third work (Paper III) studied the patients’ neurocognitive capacity to see if a higher capacity resulted in a higher percentage of patients achieving remission. The results showed unequivocally that the patients in remission consistently performed better in the cognitive tests. They also showed that they even had a larger vocabulary indicating a higher premorbid capacity. The fourth work (Paper IV) followed the patients over an average time of 65 months. In this study, the patients’ needs for medical care and for housing were investigated. Patients who were not in cross-sectional remission at baseline had for all the measured variables a higher consumption than those in remission even if the differences were not significant, except for patients who lived in sheltered housing and who had greater need for medical care. Taking all these findings together, the result of this thesis shows that the attainment of remission provides practical and qualitative advantages to the patients and to society.

This descriptive correlational study was designed to examine the relationship between situational control, and both psychological and physical well-being, in the institutionalized elderly. A convenience sample of 52 elderly institutionalized subjects was selected from two intermediate care facilities. The subjects completed the Perceived Weil-Being Scale, the Situational Control Of Daily Activities Scale, and the Subject Information Sheet. All residents were found to have an overall perception of situational control. However, residents perceived a lack of control for the daily activities of eating and grooming. The majority of residents obtained a moderately high score on the psychological well-being and physical well-being scales. No significant relationship was found between situational control and psychological well-being, or situational control and physical well-being. A significant positive relationship was found between psychological well-being and physical well-being.

The long-term care institutions for the elderly are becoming one of the house source and support to the elderly in need of long-term care. Be admitted in these places has a great impact in the life of these people, which in addition to adapt to the routine, need also to establish new relationships and state their spaces. In these situations, conflicts between elderly residents or between them and professionals working at the institution may arise, which facilitates the occurrence of disharmonious conditions. Thus, situations that triggers conflicts and creates possible implications in daily life and health of institutionalized elderly are identified. This study has the main aim: to analyze the perception of the nursing team about the occurrence of relational conflicts of eldery in a long-term care institution. It is a qualitative study with 15 nursing professionals, that was conducted in March and July 2015 in an institution located in the central region of Rio Grande do Sul State. Data collecting was made using semi-structured interviews, recorded, transcribed and analyzed according to thematic content analysis of Minayo operative proposal. The ethical aspects of research involving human beings were respected following the Resolution 466/12 of the National Health Council. Conflict situations experienced by nurses and elderly of the institution, the aspects that led to these conflicts, and the management of nursing professionals at these situations were identified. Among the situations identified in this study, conflicts emerges in moments when the emotional and inclusion needs of elderly women were not met; when them try to preserve their identity and autonomy within the institution; and in situations where their behaviors are modified by the presence of psychological/psychiatric symptoms and have their attitudes contradicted by others. Factors that motivated the emergence of conflicts included attitudes from habits and the individuality created before the institutionalization and changes in behavior caused by neurological and / or psychiatric diseases. The performance of some professionals in an attempt to stop the conflicts are focused on authority, intimidation, punishment attitudes and the administration of medications. Other professionals have chosen to use conversation as an alternative for solving the conflicts. The results show that nursing professionals have difficulties for the management of relational conflicts of eldery that often come from the lack of skills of interpersonal relationships and knowledge about the specificities of fragile elderly population.
As Instituições de Longa Permanência para Idosos vem tornando-se uma das fontes de residência e amparo à população idosa que necessita de cuidados de longo prazo. Ser admitido nesses locais é um fator de grande impacto na vida do idoso, que além de adequar-se às regras, necessita estabelecer novos relacionamentos e demarcar seus espaços. Frente a essas situações, podem surgir conflitos entre os idosos residentes ou entre eles e profissionais que atuam no local, o que favorece a ocorrência de condições desarmônicas. A partir disso, identifica-se situações propulsoras de conflitos e que geram possíveis implicações no cotidiano e na saúde dos idosos institucionalizados. Este estudo tem como objetivo geral: analisar a percepção da equipe de enfermagem acerca da ocorrência de conflitos interpessoais de idosas em Instituição de Longa Permanência. Trata-se de uma pesquisa qualitativa com 15 profissionais de enfermagem, realizada nos meses de março a julho de 2015, em uma instituição localizada na região central do Estado do Rio Grande do Sul. A coleta dos dados ocorreu por meio de entrevista semiestruturada, gravada, transcrita e, após, os dados foram analisados conforme a análise de conteúdo temática da proposta operativa de Minayo. Os aspectos éticos das pesquisas com seres humanos foram respeitados seguindo a Resolução 466/12 do Conselho Nacional de Saúde. Foram identificadas as situações de conflito vivenciadas por profissionais de enfermagem e idosas da instituição, os fatores que ocasionaram esses conflitos e as condutas dos profissionais de enfermagem nessas ocasiões. Dentre as situações de conflitos identificadas destacam-se momentos em que as necessidades afetivas e de inclusão das idosas não foram satisfeitas, quando as idosas tentam preservar a identidade e autonomia no âmbito da instituição e em ocasiões que apresentam comportamentos alterados pela presença de sintomas psíquicos/ psiquiátricos e tem suas atitudes contrariadas pelos demais. Quanto aos fatores que motivaram o surgimento de conflitos foram evidenciadas as atitudes provenientes de costumes e da individualidade construídos anterior a institucionalização e de alterações no comportamento ocasionadas por doenças neurológicas e/ou psiquiátricas. As condutas de alguns profissionais no intuito de cessar os conflitos centraram-se em atitudes como autoridade, intimidação, punição e administração de medicações. Outros profissionais optaram por utilizar a conversa como alternativa de grande potencial para resolutividade frente aos conflitos. Os resultados apontam que existem dificuldades dos profissionais de enfermagem para o manejo dos conflitos interpessoais das idosas que, muitas vezes, são provenientes da falta de habilidades com os relacionamentos interpessoais e do conhecimento frágil às especificidades da população idosa.

O objetivo principal dessa pesquisa foi estudar a afetividade de idosos residentes em InstituiÃÃes de Longa PermanÃncia, na cidade de Fortaleza â CE, sendo estas consideradas lugares de moradia para os que aà vivem. Buscou-se verificar questÃes como as polÃticas pÃblicas existentes, relacionar as condiÃÃes de moradia com o nÃvel de satisfaÃÃo dos idosos residentes, assim como avaliar seus processos de apropriaÃÃo do espaÃo e a relaÃÃo desta com o envelhecimento. Com o intuito de se ter um panorama da distribuiÃÃo das InstituiÃÃes de Longa PermanÃncia para Idosos â ILPI - existente na cidade de Fortaleza, fizeram parte do nosso campo de pesquisa um total de 21 idosos distribuÃdos em seis instituiÃÃes, sendo uma de cada Secretaria Executiva Regional â SER â que a cidade està dividida. Utilizou-se como referencial teÃrico a Psicologia Ambiental e a Psicologia Social e seus mÃtodos e tÃcnicas, como os grupos focais, as entrevistas individuais, o diÃrio de campo e o instrumento gerador dos mapas afetivos, este como mÃtodo de apreensÃo dos afetos. Por meio das imagens de agradabilidade e de pertencimento geradas foi possÃvel visualizar sentimentos e qualidades positivos dos residentes, o que revela uma estima positiva com relaÃÃo ao lugar. Jà nas categorias de pertencimento como sobrevivÃncia e pertencimento ao passado foram registrados sentimentos e qualidades despotencializadores do sujeito, o que deflagra uma estima negativa dos residentes com relaÃÃo Ãs ILPIs. Na busca de contemplar esses objetivos percebeu-se que, apesar da populaÃÃo brasileira estar em pleno processo de envelhecimento, a sociedade atual està despreparada para lidar com esse tema. As polÃticas pÃblicas existentes sÃo muito recentes e ainda estÃo em processo de implantaÃÃo. Os ÃrgÃos pÃblicos que deveriam contribuir e apoiar o desenvolvimento de instituiÃÃes como as de longa permanÃncia para idosos (ILPI) se restringem Ãs funÃÃes de cobranÃa, julgamento e multa. Percebe-se que esses idosos residentes em ILPIs estÃo desassistidos tanto no que concerne Ãs polÃticas pÃblicas, como ao apoio da famÃlia e da sociedade em geral.
The main purpose of this study was to investigate the affection of the elderly at Long Term Institutions in the city of Fortaleza - CE, which are considered dwelling places for those who live there. We verified issues such as the current public policies; housing conditions relate to the level of satisfaction of elderly residents and assess their processes of space appropriation and its relation to the aging. In order to have an overview of the Long Term Institutions distribution, in Fortaleza, a total of 21 elderly people distributed in six institutions were part of our field research, one from each Regional Executive Secretary â RES, where the city is divided. As a theoretical framework we had Environmental Psychology and Social Psychology, its methods and techniques such as focus groups, personal interviews, field diary and the instrument to generate affective maps as a method to seize emotions. Through the images of pleasantness and belonging generated we could visualize positive feelings and qualities of residents, revealing a positive esteem about the place. Regarding the categories of belonging as survival and belonging to the past we recorded feelings and qualities that weakened the person, which triggers a negative esteem of residents in relation to LTCF. Seeking to contemplate these goals we realize that, despite the Brazilian population being in full process of aging, the current society is unprepared to deal with this issue. The existing public policies are very recent and still under implementation. Governmental entities which should help and support the development of institutions such as Long Term Care Facilities for the elderly (LTCF) are only with the role of charge, prosecute and fine. We realize that these elderly residents are disadvantaged in public policy and, moreover, they have no support from family and even society in general.

Policies at assisted living facilities should be designed to develop high quality social relationships among older persons that could increase the contentment of the residents. Despite the broad consensus on this mission, the role of social support in the perceived contentment of assisted living facility residents has not been adequately explored. Using social network theory as the framework for this study, the purpose of this quantitative study was to determine whether (a) perceived level of social support was related to perceived level of contentment with life among assisted living facility residents, (b) the length of time spent in the facility was related to perceived levels of contentment, and (c) perceived social support moderated the relationship between the length of time in the facility and perceived levels of contentment with life. The sample included 100 residents from 2 assisted living facilities in North Carolina. The Multidimensional Scale of Perceived Social Support and the Generalized Contentment Scale were used in this study. Linear regression analyses were employed to answer the research questions. Participants with higher levels of perceived social support tended to have higher levels of perceived contentment with life, and the length of time residents had spent in the facility was not related to their perceived contentment with life. In addition, levels of social support did not moderate the relationship between the length of time respondents had been in the facility and contentment; age, gender, ethnicity, and marital status were not related to perceived contentment with life. This study leads to positive social change by providing long-term care providers with information on social support systems and how staff can create conditions for them to enjoy better social relationships and experience greater support, thereby facilitating their contentment with life.

Americans over age 65 constitute a larger percentage of the population each year: from 14% in 2010 (40 million elderly) to possibly 20% in 2030 (70 million elderly). In 2015, an estimated 66 million people provided care to the ill, disabled, and elderly in the United States. In 2000, according to the Centers for Disease Control and Prevention (CDC), 15 million Americans used some form of long-term care: adult day care, home health, nursing home, or hospice. In all, 13% of people over 85 years old, compared with 1% of those ages 65 to 74, live in nursing homes in the United States. Transitions of care, among these various levels of care, are common: Nursing home to hospital transfer, one of the best-studied transitions, occurs in more than 25% of nursing home residents per year. This article follows one patient through several levels of care.

This thesis analyses the effects of expanding public Long-Term Care (LTC) benefits in Spain, using three different approaches. In the first chapter, I estimate the impact of public LTC allowances on the mortality of the beneficiaries. My results suggest that providing care prevents a deterioration in health, such that death is postponed when the level of needs is low or moderate. In the second chapter –with Garc´ıa-G´omez, L´opez-Casasnovas and Vidiella-Martin–, we assess the equity of the access of public LTC. We show that the system is particularly inequitable regarding the form of provision of benefits. This translates into a pro-poor concentration of longer waiting time to access care. Finally, in the last chapter –with L´opez-Casasnovas and Nicodemo–, we investigate the unintended consequences of a non-linear scheme of benefits. We identify that around 3% of the claimants are upgraded to the next level of benefits, increasing the cost for the system. Instead, the proposed linear system of benefits could make the system more egalitarian and minimise the unintended incentives.
Aquesta tesi analitza els efectes de l’expansió de les prestacions públiques en dependència a l’estat espanyol, des de tres perspectives diferents. En el primer capítol, estimo l’impacte de les prestacions públiques en la mortalitat dels beneficiaris. Els resultats suggereixen que la provisió d’atenció prevé el deteriorament de la salut, fins al punt de posposar la mort quan el nivell de dependència és baix o moderat. En el segon capítol –amb García-Gómez, López-Casasnovas i Vidiella- Martin–, avaluem l’equitat en l’accés als diversos serveis públics per a l’atenció a la dependència. Mostrem que el sistema públic de dependència és especialment inequitatiu en la forma de provisió de beneficis. Això es tradueix en major concentració entre els més pobres del temps d’espera per accedir als recursos públics. Al darrer capítol –amb López-Casasnovas i Nicodemo–, investiguem les conseqüèencies inintencionades d’un sistema de beneficis no lineal. Identifiquem que entorn el 3% de la gent que sol.licita les prestacions són classificats en nivells de necessitat per sobre dels que els pertoca, fet que incrementa el cost del sistema. És per això, que proposem un sistema lineal de prestacions, que esdevingui més igualitari i minimitzi els incentius perversos.

Long-term care expenditures represent one of the largest uninsured financial risks facing the elderly. Medicaid provides incomplete insurance against these costs: unlimited nursing home benefits with a deductible equal to the savings and income above the means-testing limits. While private insurance is available, fewer than 10 percent of the elderly are currently covered. This thesis explores how the elderly prepare for future nursing home use and the interactions between the private and public insurance systems. Chapter one exploits the state-variation in Medicaid generosity to study the financial response of the elderly to perceived future nursing home needs. I find that the elderly shift their consumption and savings decisions in response to Medicaid. Single households have lower net worth through the median of the distribution due to Medicaid policy. On the other hand, I find that married households do not lower total net worth, but they change their relative holdings of protected and non-protected assets. Chapter two explores the crowd-out effect of the public Medicaid program on demand for private long-term care insurance coverage. We estimate the impact of Medicaid program rules on private long-term care insurance coverage for the elderly. We find small but statistically significant marginal crowd-out effects.
(cont.) Our estimates imply that even a $67,000 decrease in the asset disregard for couples would only increase private long-term care insurance ownership among the elderly by 1.9 percentage points. These findings underscore that marginal reforms to the existing Medicaid program are unlikely to be an effective way of increasing private long-term care insurance coverage among the elderly. Chapter three explores individuals' expectations for future nursing home use. I compare self-reported probabilities to the statistical probability computed with a state-of-the-art model used by the long-term care insurance industry. I find that respondents tend to overestimate unlikely outcomes and underestimate likely outcomes. On average, though, the expectations are very accurate. I find that expectations for nursing home use evolve with health conditions in similar ways as the statistical probability. While I find that expectations include private information, they do not account for all information available to the individual, especially the individual's demographic characteristics.

Family relationships are a significant part of a person’s life. Unfortunately, moving a relative from a family home into a long-term care (LTC) home requires the readjustment of family relationships from a familiar to a foreign environment and puts these relationships and associated family identity at risk. With the shift in focus to person-centred care and its need for the maintenance of family connection, the fostering of family relationships in LTC homes becomes an important part of properly enacting this approach to care. This critical ethnography explored the ways in which family relationships are fostered in LTC and identified the efforts made and barriers that exist in the broader organizational and social context of LTC that influence how these relationships are fostered. Data from a larger critical ethnography was collected through semi-structured interviews with and participant observation of 12 family members, seven staff members, and three residents from a LTC home in an urban city in British Columbia, Canada. Family members came into the LTC home to spend time with their relative and maintain a connection with them, but a lack of opportunity to take part in activities that recognized the importance of these connections prevented family relationships from being fostered. Task-focused care overlooked the importance of relational care and further prevented family relationships from being maintained. Physical and cognitive decline also made it difficult for family members to engage and connect with their relative, resulting in a significant impact on the fostering of family relationships. Driving distances and a lack of opportunity to maintain connections with relatives outside of the LTC home added to the challenge of fostering family relationships. Family members want to maintain family identities and associated relationships but require support in doing so. The findings from this study have implications for nursing practice, administration, education, and research.

This research examines the physical operations of a health care sector called long term care facilities. Long term care refers to organizations that provide onsite nursing care for people with long-term illnesses, elderly people who need continuous medical supervision, or for those in long-term rehabilitation programs. Most specifically, this research looks at the performance of the physical buildings (i.e. facilities) and how this performance impacts the external environment. Using secondary data from the organization Practice Greenhealth, this study addresses the following two research questions.

1. What is the combined current state of environmental performance at the long term care facilities that have applied for a Practice Greenhealth award?

2. What are the proposed environmental key performance indicators for the long term care sector?

Based on this research, a model is suggested for future researchers interested in the environmental performance of long term care facilities. The model suggests six drivers for improving environmental performance in long term care; (1) maintain compliance with environmental regulations, (2) reduce costs, (3) meet expectations of parent organization, (4) increase market competitiveness, (5) engage stakeholders, and (6) improve patient outcomes. Four environmental improvement categories are then established; (1) waste, (2) energy, (3) water, and (4) engagement initiatives, along with each categories’ associated indicators. Finally, this study develops a concise environmental performance survey that could be used by any long term care facility looking to make improvements to its environmental performance over time.

This research used a Delphi methodology to solicit challenges that significantly impact the operational success of pediatric long-term care facilities. Further, this study sought to understand the ability of pediatrics administrators to impact, affect, overcome, or resolve these challenges. Round 1 accomplished the desired goal of eliciting a substantial amount of new qualitative information regarding administrative challenges in pediatric long-term care, which previously had been extant in the literature. In Round 2, data were distilled into categories and unique exemplars which in turn were validated by participants. In Round 3, administrators ranked and rated categories and individual challenges in terms of significance and impact.

Findings revealed that the most important challenges for administrators are those of working with an inadequate model that is designed for a geriatric population, inadequate Medicaid funding, and the lack of clinical and administrative indicators in pediatric long-term care. The most important challenges that could be affected by the administrators are related to inadequate model, clinical practice, and the need for diversification. Administrators believe they can strongly impact the rules, regulations, and protocols that are currently geriatric focused, develop pediatric long-term care indicators and evidence-based research, and impact their financial security by diversification.

The Delphi research accomplishes the desired goal of eliciting a substantial amount of new information regarding administrative challenges in pediatric long- term care and contributes to the broader body of knowledge in health services administration. The significant research findings suggest the need for changes in the regulatory and financial models in pediatric long-term care and the need for enhanced clinical practice though evidence-based practice and outcomes.

This research was jointly funded by the Economic and Social Research Council (ESRC) and the Engineering and Physical Sciences Research Council (EPSRC). As such, its underpinning and innovative aim was to explore the use of Operational Research (OR) techniques, a research area traditionally associated with the EPSRC, to address key societal problems traditionally associated with the ESRC. The ageing population presents many significant challenges for social care services at both a national and local level, one of which is to meet the demand for long-term care. The population of people aged over 65 will continue to grow for some time as the ―baby boom‖ generation ages. The concern for policy planners is whether there will be enough resources in place to handle the expected strain on the system in the future. The research presented in this thesis addresses this key issue, and was carried out in collaboration with the Adult Services Department of Hampshire County Council (HCC). The overarching aim of this thesis was to develop computer models (using data local to Hampshire) which would be of practical use in estimating the future demand and planning the supply of long-term care in Hampshire. A cell-based model was built to forecast the demand for long-term care in Hampshire from people aged 65 and over for the period 2009 to 2026. An important part of this research was to understand the main drivers of future demand for long-term care and to predict the future number of people with a disability. Hampshire County Council has already tried to address these issues of demographic change through a modernisation programme. Part of this has been the establishment of a contact centre called Hantsdirect. A discrete-event simulation model of the contact centre was developed. The two models were combined to explore the short- and long-term performance of the contact centre in the light of demographic change. This hybrid model has enabled HCC to explore the short- and long-term performance of the contact centre. This study combines OR with Gerontology, Demography and Social Policy. This research is novel as it iteratively combines a compartmental population model with a discrete-event simulation model. From an OR perspective, the aim was not only to explore the use of modelling in social care (where, unlike healthcare, there has not hitherto been a lot of research), but also to investigate the potential for combining different modelling approaches in order to obtain additional value from the modelling. This novel approach in a social care setting is one of the main contributions of this thesis.

An evidence-based clinical practice guideline was developed to prevent and manage constipation in the long-term care (LTC) resident. Constipation is a prevalent condition in the LTC resident. Guidelines do not address this vulnerable population. Constipation may result in poor nutritional status, quality of life along with increased health care costs and hospitalizations. A literature review was completed and evidence was evaluated and included into the initial draft recommendations. The guideline was reviewed for content validity using a Delphi Committee of clinical experts in gastroenterology, geriatrics, and pharmacy. The updated guidelines were presented to an interdisciplinary team of long-term care residents. Participants were asked to review and complete a survey regarding clinical applicability of the guideline. Thirty-one interdisciplinary members participated in the education session and 30 surveys were received. Overall, the interdisciplinary team members agreed or strongly agreed the guideline was clinically applicable. Finally, the guideline was evaluated by a group of doctoral prepared practicing nurse practitioners using the AGREE II instrument. The scores were >80% in every domain, with an overall score of 91%, indicating the guideline is high in quality. Development of this guideline signifies an initial step in the management and prevention of constipation in the LTC resident.
