Abstract (sommario):
Child Cerebral Palsy (CCP) is a non-progressive, chronic brain injury that can occur during pregnancy - prenatal period, during childbirth - perinatal period or postnatal period. It is characterized by a disorder of muscle tone, disturbance of balance, coordination, speech, hearing, etc. Also, some children may experience some degree of cognitive and intellectual deficiency. Damage mainly occurs at the upper motor neuron affecting the cerebellum. This disorder of the central nervous system leads to disruption of the normal development of motor functions, muscle tone, and coordination. Children's cerebral palsy is a disease that very often leads to severe disability, and to successfully battle with the consequences and serious disabilities, early detection, and the onset of treatment is needed from the first days of the child's life. The treatment of CCP is complex in which, besides medical, pedagogical and social rehabilitation, kinesitherapy is included. The aim of the study is to evaluate the effect of the kinesitherapeutic approach and the specialized kinesitherapeutic method in children with CP. Material and Methods: The study was conducted with 8 children with CP due to premature birth and complications at birth and with spastic diplexia after periventricular leucomalation, the leg being more affected than the hand and with no signs of cortical damage and spastic hemiplegia.The study excludes patients with severe and frequent epileptic seizures and serious cognitive problems. For the purpose of the study, a set of diagnostic methods was applied, and the results were scored at the beginning, 3rd month, and 6th month of treatment initiation and were applied to a working datasheet. Were Evaluated: the functionality of a modified test Chedoke-McMaster (Chedoke-McMaster Stroke Assessment), muscle tone with a modified scale of Ashworth and the balanace capabilities with test Berg (Berg Balanace Ssale-BBS). Results: The specialized kinesitherapeutic methodology permanently stabilizes the functional opportunities for rehabilitation and balanced opportunities for children with CP. Conclusion: The complete and in-depth analysis of the tangible results gives us reason to assert that the applied kinesitherapy has both an early and a late therapeutic effect that is related to the duration of administration, the structure of performance and the purpose of the included kinesitherapeutic means.

Abstract (sommario):
Hip arthrosis is the main cause of disability of all degenerative joint diseases and therefore it is considered to be of social significance. The presented clinical case aims to confirm the efficacy of the applied kinesitherapeutic program in conservative treatment of 62-year-old patient with right-sided coxarthrosis. The kinesitherapeutic methods included right lower limb massage, suspension therapy, tractions and manual movements of the right hip joint, positional treatment, active exercisesinan openkinetic chain, postisometric relaxation and stretching for shortened muscles.The pain symptoms were reduced, the volume of movement in the right hip increased, the strength of the right hip joint stabilizer musclespartially improved.

The purpose of the paper is to prove the efficacy of a kinesitherapeutic program intherecovery of a patient with trimalleolar fractureofthe left lower limb.We present a 42-year-old patient with a trimalleolar fracture and paresis of n. peroneus communis. Kinesitherapy was performed after thirteen weeks of surgery for ten consecutive days. The kinesitherapeuticmeans included: cryotherapy, lymphatic drainage massage, isometric contractions for the femur muscles, active exercises with and without resistance, active exercises with and on gymnastic apparatuses, post-isometric relaxation for the sciatic musculature and m. triceps surai, training in walking.As a result of the kinesitherapeutic exercises, the ankle swelling decreased.The volume of movement increased, pain was reduced as well as the muscle spasm in the ankle joint. The gait improved.

The individual kinesitherapeutic approach is increasingly required in rehabilitation practice due to the presence of concomitant diseases, social problems and psycho-emotional characteristics of the patients. Its effectiveness can be assessed by examining the level of satisfaction of patients with total hip arthroplasty with the work of the kinesitherapist. Purpose: to study and evaluate the effectiveness of the individual kinesitherapeutic approach on patients with total hip arthroplasty. Material and methods: Material: 60 patients who have undergone operative intervention for total hip replacement at the Orthopaedics and Traumatology Clinic at the University Hospital St. Marina, Varna for the period 2012–2019. The patients are divided into experimental (30 patients with a mean age of 56.17) and control groups (30 patients with a mean age of 55.53). Methods: Sociological method (questionnaire survey): questionnaire for satisfaction with the kinesitherapist's work on the Likert scale (α-Cronbach 0.823); Statistical method: statistical data grouping, Cronbach's test, descriptive method, testing of statistical hypotheses, correlation analysis. The data is processed with SPSS v.19.0 for Windows. Results and discussion: 83,3% of patients with total hip replacement answered that they have received answers to their questions from the kinesitherapist treating them. The patients in the experimental and control groups were equally aware of the need for kinesitherapy in their postoperative recovery period. 100% of the respondents in the control group believed that the kinesitherapist was very interested and inquired about concomitant diseases and problems that arose during the procedures. Patients with custom hip replacement report greater freedom "to a very large extent" and 26,7% to a large extent. Conclusion: the application of the individual kinesitherapeutic approach to patients with total hip arthroplasty leads to a faster and fuller functional recovery and to a higher level of satisfaction.

Disc hernias are usually the result of disc wear, especially in the exercise of heavy physical occupations, weight lifting, forced posture, overwork. The clinical case described is a 69-year-old patient, a journalist by profession. At the end of May, he went to the Mount Athos peninsula in Greece to visit and describe the Monasteries in Mount Athos. On 30th of May he made a long and difficult transition into the mountain with a heavy backpack. In the morning of the same day, he suffered sharp spontaneous motion pains. He hoped that the pain would subsist and he would continue the journey. The day after, the inflammatory process encompassed the left buttocks with numbness on the outer surface of the left thigh and lower leg with a very limited movement. The patient was very worried about his health and how he would be transported to the nearest hospital. He was transported on a stretcher to Bulgaria and on 06.06. 2016г. to the hospital MHALNP "St. Naum" in Sofia for research and treatment. The Computer tomography was performed on 07.06.2018. upon acceptance of the patient, the results from it were: Disorders of the intervertebral discs in the lumbar spine and the other parts of the spine with radiculopathy at the level L4-L5 - left-side foramen disk hernia with compression of the left L4 nerve root in intra- and extraforaminal disc herniation.This example shows that in case of problems with vertebrogenic etiology it should not be awaiting and the patient should take precautions in time to avoid complications.

AIM: The study aims to trace the influence of specialized kinesitherapeutic methodology (SKTM) on balance reactions in patients with ischemic stroke in the chronic period (ISChP).MATERIAL AND METHODS: A prospective, multicenter study with 56 patients with ISChP. Evaluation of balance reactions using Berg Balance Scale - BBS, includes implementation of 14 tasks with increasing difficulty reflecting the usual activities of everyday life. The first 5 assignments are used to assess the main balance potential and the remaining 9 (6th to 14th task) include more sophisticated balance tasks.RESULTS: The patients were found with a significant improvement in balance opportunities, according to the scale of Berg. Compared to initial data there is a significant increase in the number of points in the measured indicators for functional and static balance. In absolute terms, positive change is most pronounced during the 1st month with a level of significance of p <0.001. CONCLUSION: The applied specialized kinesitherapeutic methodology continued later as adapted exercise program at home, and significantly improved equilibrium reactions in patients with postural disorders because of ischemic stroke and is with a supportive prolonged exposure.

AIM: The study aims to trace the influence of specialized kinesitherapeutic methodology (SKTM) on gait in patients with ischemic stroke in the chronic period (ISChP).MATERIAL AND METHODS: The study was conducted with 56 patients with ISChP (duration of the disease up to 1 year). For determining changes in gait before and after the treatment a cadence of gait and maximum movement speed were taken into consideration. To determine the cadence, steps are counted for covering 6 meters and 10 meters respectively. The maximum speed of the gait is determined in m/min by dividing undergone distance (m) and time (min).RESULTS: Patients were found to significantly normalize the parameters of gait. Compared to the initial data, there is a significant reduction in the number of steps on 6 and 10 meters and a tendency to increase the speed of gait, with the significant change during the 1st month with a level of significance of p <0.001.CONCLUSION: The applied specialized kinesitherapeutic methodology continued later as exercise program at home, which significantly improved gait cadence and speed of movement in patients with ischemic stroke in the chronic period and is with a supportive prolonged exposure.

The orthostatic autoregulation is evaluated four times - at the beginning of the study, on the 10th day, on the 1st month and three months after the start of the KT. The classification by Thulesius was used to separate the patients into two groups depending on the type of their orthostatic reactivity.RESULTS: At the beginning of the study of infringements symptomatic type orthostatic reactivity (SOR) was observed in 24 patients and hypertensive type orthostatic reactivity (HOR) was observed in the remaining 32 patients. Once applied SKTM establish improvement of orthostatic autoregulation for the groups SOR and HOR at the 10th day and the 1st month with a level of significance p <0.05.CONCLUSION: The applied specialized kinesitherapeutic methodology continued later as an adapted exercise program at home, has significantly improved the orthostatic reactivity in patients with orthostatic dysregulation due to the ISChP.

Abstract (sommario):
The aim of this study was to approbate own kinesitherapeutic methodology in lumbosacral pain syndrome and to study its effectiveness. Material and Methods: The study includes 36 participants, mean age (X±SD) 51.67±4.93 years, selected according to the indications for the use of experimental methods. The methodology includes the following functional tests: anamnesis, somatoscopy, Shober Test, Ott Test, Lasseg Test, VAS (Visual Analog Scale). The examination of the pain was done through experimentally induced palpation pain (applying the same pressure, by the same person) in trigger point (TP) and evaluated with VAS. Values of pain were measured before therapy and tree months after kinesitherapy treatment which includes: soft-tissue mobilization, analytical exercises, post-isometric relaxation, hydrotherapy (tangentor). Results: 36 people (12 women and 24 men) with lumbosacral pain syndrome were included. The mean (± SD) of the pain threshold measured before and three months after the kinesitherapy program was 9.06 ± 0.79mm and 1.69 ± 0.82mm, respectively. Differences in mean pain thresholds in both measurement period were statistically significant (p <0.05). Conclusions: Data presented demonstrate the effectiveness and positive effects of the applied kinesitherapeutic methodology in patients with lumbosacral pain syndrome. The strong reduction in pain symptom and muscle spasm in the course of three months treatment improves the quality of life of patients with this pathology.

Abstract (sommario):
Aim: The aim of the study is to evaluate the effect of the specialized kinesitherapeutic (KT) methodology (SKTM) on functional independence in patients with supratentorial unilateral stroke in the chronic period (SUSChP). Material and Methods: The study was conducted with 67 patients with SUSChP (56 patients included in the experimental group - 32 men and 24 women, with duration of the disease 7.8 ± 2.0 months, and 11 patients in the control group - 9 men and 2 women, with duration of the disease 7.3 ± 1.5 months). To evaluate the changes in functional independence, test for Functional Independence Measure (FIM) was used. Patients in the experimental group were treated with a specialized 10-day KT, which later continued to be performed as an adapted exercise program at home for a period of one month. Control patients perform a regular 10-day KT. This was done using SKTM principles of modern neurorehabilitation and motor learning as opposed to usual kinesitherapy. Results: After applying SKTM, the highest trend towards the improvement of functional independence was established after the 1st month with a level of significance during treatment p<0.001. Conclusion: In conclusion, the enclosed SKTM in the experimental group continued later as an adapted exercise program at home. It significantly improves the functional independence in patients with supratentorial unilateral stroke in the chronic period (SUSChP) compared with the usual kinesitherapeutic methodology applied in the control group.

Child Cerebral Palsy (CP) is the most common cause of disability in children. Quality treatment requires active therapy from an early age. In 2004, at the International Workshop on Definition and Classification of CP in Bethesda, Maryland, the following definition was adopted: "A group of diseases of the development of movement and posture causing limitations in activities which belong to the non-progressive disabilities observed in the developing brain of the fetus and the baby. Increasingly, in addition to routine kinesitherapy, innovative neurodevelopmental methods such as Doman-Delacato and Perfetti are included in the work with such children. These methods are based on the neuroplastic properties of the brain. The research on the topic suggests the need to apply more innovative methods and assess their effectiveness. This motivates us to do the following research. Aim. The aim of the present study is to investigate the effect of the application of new specialized kinesitherapeutic methods in order to optimize the treatment of children and improve their motor activity. Methods. 30 children aged 3-7 years old with a spastic hemiplegic CP were randomized and split equally in two groups - a control group (CG) recieving routine kinesitherapeutic method and an experimental group (EG) including, besides the routine methodology and specialized neurodevelopmental methods of Doman-Delacato and Perfetti. For children in both groups, kinesitherapy was applied three times a week with a duration of each procedure of 50 minutes. All participants were evaluated with Ashworth Modified Scale, the Holt and Hoskins - Squires Test, modified for children by Chavdarov (1985), and Global Motor Measurement, through the Gross Motor Function Measurement Test - GMFM - 66. The study period ran from February 2017 to February 2018. The evaluation were done before kinesitherapy, 6 months after kinesitherapy and 1 year after kinesitherapy. Friedmann ANOVA Test and post Bonferroni multiple comparison test were used to evaluate within and between-group differences respectively. The level of significance was accepted as p<0.05. Results and analysis. The dynamics of the results in the two research groups was followed. Muscle spasticity in EG children decreases as early as the sixth month of the kinesitherapy with a statistically significant difference p <0.01, and after one year it decreases even further with a difference p <0.001. In KG children, spasticity decreases much later with a statistically less significant difference p <0.05, 1 year after kinesitherapy. The results of Holt and Hoskins - Squires test we obtained were with statistically significant differences in EG p <0.05, 1 year after treatment, while in KG there were no statistically significant differences between the period before kinesitherapy and 1 year after. In terms of global motor function measured by GMFM-66, both groups reached the results 1 year after the treatment, but the difference was statistically more significant in EG p <0.01. Conclusion. In conclusion, we can argue that innovative neurodevelopmental kinesitherapeutic methodologies, such as Doman-Delacato and Perfetti in combination with routine kinesitherapy, lead to statistically significant differences in the group of children with CP they were applied. This demonstrates the effectiveness of the applied complex of exercises in which neurodevelopmental methods have been intentionally added in parallel with standard approaches. Therefore, we can assert that our methodology optimizes the treatment of children with CP and improves their motor activity.

The complexity of the movements of the bones of the knee joint requires an anatomical study to construct a knee prosthesis. The reasons for knee replacement are related to wear of the articular cartilage, which leads to pain, deformation, and limitation of movements. The consequences of the surgical intervention allow functional restoration of the knee joint and the supporting function of the lower limb. Kinesitherapy begins the day after surgery and aims to reduce pain, restore mobility of the knee joint, actively lock the knee during extension, increase and maintain muscle strength, and ensure the functional independence of patients. To a large extent, the volume of movement is restored, and the pain symptoms are reduced. Patients have the comfort of everyday movements and the possibility of participating in certain sports activities—swimming, cycling, and golf. The paper deals with pharmacological prophylaxis, the kinesitherapeutic program, and total knee arthroprosthesis.

This study analyzed the pathophysiological impacts of Chronic Obstructive Pulmonary Disease (COPD) on the respiratory system and the benefits of kinesiotherapeutic interventions on diaphragmatic mobility in patients affected by COPD. Systematic literature review carried out in the Scielo, Medline, VHL, Lilacs and PEDro databases, using the following keywords: Chronic Obstructive Pulmonary Disease, Respiratory Rehabilitation, Kinesitherapeutic Interventions and Diaphragmatic Dysfunction. Only original articles published between 2009 and 2019, about the benefits of different respiratory physical therapy intervention programs were included. Two examiners analyzed the qualities and evidence of the articles using the PEDro scale. Twelve manuscripts make up the discussion of this work. The present study confirms the beneficial effects of different Kinesiotherapy Intervention programs on physical health and quality of life, reduction of respiratory symptoms and risk of mortality, increased mobility of the rib cage, diaphragmatic functionality, functional capacity and respiratory muscle strength of patients affected by COPD.Keywords: chronic obstructive pulmonary disease, respiratory rehabilitation, kinesiotherapeutic interventions, diaphragmatic dysfunction.

The treatment of elbow joint fractures is a serious medical and social problem due to frequent complications and impairment of the ability of self-service and performance of various household and labour activities. We needed evidence as to which exercises were appropriate in the treatment of patients with fracture elbow-Monteggia. The aim of the presented clinical case is to examine the effectiveness of the applied kinesitherapeutic methodology in Monteggia fracture-dislocation treated surgically by internal fixation. Kinesitherapy was carried out in two phases (maximally and moderately protective), and as a result of the applied means of functional recovery, there was achieved a good elbow reintegration in the motor circuit of upper limb. There was observed a small deficit in terms of flexion and extension in the right elbow joint, which should be ascribed to the lack of artromot, as well as to insufficient patient motivation. Global functions reported by MEPS were 84/100.

The restoration of the knee joint function in osteoarthritis is currently a serious problem, despite the list of biomechanical and pharmacotherapeutic methods used. In old age, there is a decrease in compensatory capabilities. When compiling a complex of kinesitherapeutic measures with elderly women, concomitant diseases and the degree of physical fitness were necessarily taken into account. The level of loads was determined taking into account individual characteristics. An obligatory condition before the beginning of the exercise in women was measured heart rate and blood pressure. The methods included classes on specialized simulators, when performing the exercises, the basic principles should be followed: the weight of the burden was determined based on the calculation of 15–20 repetitions; performing exercises with the same burden for a long time (if the weight is chosen incorrectly) in the health aspect is impractical, when performing strength exercises, the emphasis should be on correct breathing and accuracy of movements. In the process of conducting classes, it is necessary to monitor the dynamics of the growth of weights and monitor the physical condition of women.

AIM: The aim of the study is to evaluate the changes of the kinetic parameters of gait in patients with supratentorial unilateral stroke in the chronic period (SUSChP).MATERIAL AND METHODS: The study was conducted with 67 patients with SUSChP (56 patients included in the experimental group - 32 men and 24 women, with duration of disease 7.8 ± 2.0 months, and 11 patients in the control group - 9 men and 2 women, with duration of disease 7.3 ± 1.5 months). To evaluate the changes in the gait were followed cadence of 6m and 10m and the speed of movement which are the most informative kinetic parameters. Patients in the experimental group were treated with a specialised 10-day KT, which later continued to be performed as an adapted exercise program at home for one month.RESULTS: After applying specialised kinesitherapeutic methodology (SKTM), the highest trend towards improvement in the kinetic parameters of gait was established after the 1st month with a level of significance during treatment p < 0.001.CONCLUSION: The enclosed SKTM in the experimental group continued later as an adapted exercise program at home, significantly improving the kinetic parameters of gait in patients with SUSChP, compared with the usual kinesi-therapeutic methodology applied in the control group.

The aim of the study was to investigate and compare different kinesitherapy techniques and their impact on the recovery of patients with ischemic stroke. Also, to investigate the effect of kinesiotaping on thrombolized patients. Materials and Methods. The study was conducted in 2014-2015 at the General Hospital for Treatment in Blagoevgrad - Neurology Department. The study contingent consisted of 20 patients, 10 of whom had an ischemic stroke and underwent venous thrombolysis (experimental group). In the other ten patients, no innovation was applied (control group). Results and analysis: In patients with a brain accident, the muscles in the affected limbs are hypotonic. From a kinesitherapeutic point of view, it is very important in the hospital setting not to allow a pathological increase in muscle tone without a precondition for spasticity in the muscles and subsequent contracture. In our study, we examined the change in muscle tone using the Ashworth test. All patients were tested on the day of hospitalization, on the 5th day and on the 7th day (the day of hospitalization), on the 30th day. Discussion. Kinesio tape provides maintenance of the affected muscles and causes significant proprioceptive irritation, which helps to restore the existing motor deficit. Conclusion. We believe that the choice of methodology is extremely difficult due to the colorful clinical picture and the general condition of the patient. The combination of different methods could help the patient recover faster.

Abstract. The purpose of the work is to develop and substantiate a technique for physical rehabilitation in knee arthrosis. We assumed that the technique we developed will contribute to the speedy restoration of knee joint functions, the removal of pain syndrome and an increase in the physical activity of patients. Of greatest importance in kinesitherapy is the determination of women's motor capabilities, abilities for household and labor skills, for which muscle testing on multifunctional simulators is used. For subjective assessment of pain, we proposed a verbal descriptive scale. This method is designed to determine the intensity of pain and allows you to evaluate the subjective pain sensations that the patient experiences at the time of the study. Hydroxynesitherapy is in the physical rehabilitation program. When performing gymnastics in water, different depth of immersion is used, dynamic exercises, exercises with elastic (rubber) bandage, rubber espanders, shovels (to increase rowing resistance), flippers and special cuffs on knee joints are used. Duration of classes 15–20 min 2–3 times a week, course 30–45 days and subsequent application of teips. The rehabilitation included treatment baths, which women conducted independently at home. When conducting kinesitherapeutic measures, we took into account the patient's complaints, we carried out medical supervision, control of heart rate and blood pressure. We presented the results of the study of influence of the proposed technique on morphofunctional state of women 45–50 years old. We noted positive dynamics of influence of kinesitherapy on motor functions of knee joint.

Aim: To monitor the rehabilitation progress of a child with fracturaolecrani with the participation of a student volunteer in the multidisciplinary team. Methods: K.J. is an 11-year-old child admitted for rehabilitation in the Clinic of Orthopaedics and Traumatology at University Hospital "St. Marina"- Varna. In 2011 K.J. suffered a fracture in the area of the right elbow joint and underwent surgery with the placement of three Kirschner needles. Four years later, the patient suffered the same fracture and underwent surgery with the placement of two Kirschner needles. Oedema, joint deformation, severely limited range of motion, contractures, muscle hypotrophy and psychotrauma, were found after examination by an orthopaedist and a kinesitherapist. The individual kinesitherapeutic approach is focused on reducing oedema; improving activity; preventing complications; stimulating the patient's participation in social activities. Results: With the help of functional examinations (centimetre measurements, anglemetry and MMT), we established significant restoration of the upper extremity's function and reduction of the oedema. The minimal differences result from the rough operative cicatrix and slight joint deformation. The range of motion is restored to a large extent, as the deficit of 5-10 degrees is not essential in daily activities and could be further restored. Conclusions: the application of an individual approach in the multidisciplinary team's work, with the participation of a student volunteer, helps build a trustworthy relationship with the child patient. The inclusion of a student volunteer in the multidisciplinary team improves communication with vulnerable population groups and contributes to a faster physical and mental recovery.

To separate groups of related diseases3. Develop appropriate kinesitherapeutic programs that students can perform in sports classes.The object of study was 200 students from first year of both sexes.The survey was done through 2018. For the purposes of some empirical sociological survey questionnaire was designed, which is direct, standardized in writing. The results show that the average age of students in the first year of 2018/2019. at the Medical College is 23.9 years old. A positive fact is that 50% of the students are up to 20 years old, they did as soon as they finished their education. 23% apply after a few years, and young mothers are also included in this group. As a result of the dynamic political, economic and social changes leading to market economy processes, the number of potential students over the age of thirty (24%) has increased sharply as a result of unemployment and job cuts. This trend is the reason to increase the number of students with healthy ones, which are already 17% of newly admitted students.

The proportion of injuries to the upper limbs, accompanied by nerve damage, ranges from 1.5 to 10%. Damage of the peripheralnerves of the upper limb in children is associated with fractures of the long bones in 10-12% of cases. The high incidence of damageto the peripheral nerves of the upper limb, the risk of impairment or loss of function, disability of the patient often requires not onlyactive conservative treatment, but also often surgical intervention. Aim. To analyze literature data and generalize the results of our own research to determine the optimal technologies for medical rehabilitationof children with post-traumatic neuropathies of the upper extremities. The success of both conservative and surgical treatmentof nerve injuries is largely determined by the volume and quality of the diagnostics performed. The goal of surgical or conservativetreatment of peripheral nerve damage is to restore the conduction of the nerve trunks with the highest possible functional result.Physical factors occupy one of the leading places in the system of medical rehabilitation of children with post-traumatic neuropathies.The medical rehabilitation program includes the use of apparatus physiotherapy, robotic mechanotherapy, applied kinesitherapy,massage, soft manual techniques, orthosis, and kinesiotaping. The leading technologies are the methods of motor rehabilitation. The effects of kinesitherapeutic influences are potentiated by neuromodulatory methods of hardware physiotherapy (magnetostimulation,electrical stimulation). Conclusion. Injuries to the peripheral nerves of the upper extremities in children requires long-term staged medical rehabilitation inmost cases. At the same time, to date, there are no clear recommendations for rehabilitation of children with damage to peripheralnerves in the postoperative period, there is no unified algorithm for rehabilitation measures. Motor rehabilitation technologies are keyin medical rehabilitation programs for children with post-traumatic neuropathy. The timely start of rehabilitation measures and differentiatedprescription of physical factors adjusted to the period of injury ensure the preservation of the result of surgical treatment,contribute to the early restoration of the function of the injured limb.

The current systematic review is focused on a recent question aroused during the pandemic’s successive waves: are there post-SARS-CoV-2 immune-mediated reactions responsible for promoting neurodegeneration? Does the host’s dysregulated immune counter-offensive contribute to the pathogenesis of neurodegenerative diseases, emerging as Parkinson’s disease, in a complex interrelation between genetic and epigenetic risk factors? A synthetic and systematic literature review was accomplished based on the ”Preferred Reporting Items for Systematic Principles Reviews and Meta-Analyses” (PRISMA) methodology, including registration on the specific online platform: International prospective register of systematic reviews—PROSPERO, no. 312183. Initially, 1894 articles were detected. After fulfilling the five steps of the selection methodology, 104 papers were selected for this synthetic review. Documentation was enhanced with a supplementary 47 bibliographic resources identified in the literature within a non-standardized search connected to the subject. As a final step of the PRISMA method, we have fulfilled a Population-Intervention-Comparison-Outcome-Time (PICOT)/Population-Intervention-Comparison-Outcome-Study type (PICOS)—based metanalysis of clinical trials identified as connected to our search, targeting the outcomes of rehabilitative kinesitherapeutic interventions compared to clinical approaches lacking such kind of treatment. Accordingly, we identified 10 clinical trials related to our article. The multi/interdisciplinary conventional therapy of Parkinson’s disease and non-conventional multitarget approach to an integrative treatment was briefly analyzed. This article synthesizes the current findings on the pathogenic interference between the dysregulated complex mechanisms involved in aging, neuroinflammation, and neurodegeneration, focusing on Parkinson’s disease and the acute and chronic repercussions of COVID-19. Time will tell whether COVID-19 neuroinflammatory events could trigger long-term neurodegenerative effects and contribute to the worsening and/or explosion of new cases of PD. The extent of the interrelated neuropathogenic phenomenon remains obscure, so further clinical observations and prospective longitudinal cohort studies are needed.

Abstract (sommario):
A significant place in the treatment of osteoarthritis (OA) is taken by physical therapy. Its modalities may be grouped into the following three categories, which could be used alone or in combination: 1) physiotherapeutic (preformed); 2) kinesitherapeutic; 3) resort-climatic (natural) means. The purpose of this review is to outline the place of physical therapy in the treatment of OA. The choice of one or another preformed physical factor depends on the severity of the disease process, the sensitivity of the patient and the physiotherapist's experience. Among the most commonly used physiotherapeutic agents for pain and stiffness and joint function improvement are ultrasound, laser, magnetic therapy, and transcutaneous electrical nerve stimulation. Kinesitherapy aims to soothe pain, strengthen, relax and stretch the muscles, increase the volume of movements, improve gait, aid joints, and more. Balneotherapy has an important place in the complex therapeutic program for diseases of the musculoskeletal system.

Abstract (sommario):
The study presents the current problems concerning the principles of physiotherapeutic treatment of shoulder pain syndromes in the course of rheumatic diseases. The principles of the treatment in the most common rheumatic diseases such as RA, OA and SpA are discussed. Particular attention has been paid to the importance of kinesitherapeutic treatment, while indicating the role of treatment with physical methods in the field of thermotherapy, electrotherapy, laser therapy, the use of ultrasounds and magnetic field. Balneological treatment with myorelaxing, analgesic and anti-inflammatory effects is of complementary importance. The summary emphasizes that the goal of rehabilitation in shoulder pain syndromes in the course of rheumatic diseases is not only to achieve analgesic effect and improve the functional status, but, above all, an individual and comprehensive therapy aimed at improving the efficiency of the patients, enabling return to daily activities. The implementation of an effective and complex therapeutic program requires the simultaneous undertaking of comprehensive activities: educational, developing an individual strategy of physiotherapeutic and psychotherapeutic treatment, the use of individually selected orthopedic supplies, as well as offering social assistance. In order to perform above procedures, a multidisciplinary team is necessary. Only the most immediate possible implementation of interdisciplinary and complex procedures makes enables patients to return to full family, social and professional activity.

Abstract (sommario):
INTRODUCTION: Zudek syndrome (neuroreflex algodystrophy, accompanied by osteoporosis, limited range of motion in the wrist and interphalangeal joints, impaired blood flow and associated trophic alterations) accounts for up to 62% of complications of fracture of the distal metaepiphysis of the radius. It may be a consequence of primary action of pathological factors (I type), or may result from compression of peripheral nerves (II type). The article describes a clinical case illustrating occurrence of almost all critical errors in the treatment process (non-performance of local anesthesia of the fracture and of reposition of fragments; fixation of the hand in a vicious (for this type of fracture) flexion position for up to 3 weeks; lack of local physiotherapy, anti-inflammatory and kinesitherapeutic treatment; use of “aggressive” mechanotherapy in passive restoration of the volume of movements and of “hard” massage against the background pain syndrome) that contributed to the development of the given II type complication. As a result, a surgical correction was required — corrective osteotomy of the radius to eliminate the cause of compression of the ulnar and median nerves and restore the interrelations between the articular surfaces in the wrist joint. CONCLUSION: Non-observance of the principles of treatment of a fracture of the radius in a “typical place” (emergency care; adequate anesthesia; immediate sparing reposition; subsequent clinical and radiological control, functional management; sufficient period of immobilization; early physiotherapy and kinesitherapy; etc.) may provoke such a threatening complication as Zudek syndrome characterized by a prolonged disability and inflicting physical and moral suffering. Besides, it is associated with a high cost of treatment and restoration of the function of the damaged limb, significantly exceeding the economic standard.
