ABSTRACT Transposon mutant strain 3G6 of Pseudomonas fluorescensATCC 17400 which was deficient in pyoverdine production, was found to produce another iron-chelating molecule; this molecule was identified as 8-hydroxy-4-methoxy-quinaldic acid (designated quinolobactin). The pyoverdine-deficient mutant produced a supplementary 75-kDa iron-repressed outer membrane protein (IROMP) in addition to the 85-kDa IROMP present in the wild type. The mutant was also characterized by substantially increased uptake of 59Fe-quinolobactin. The 75-kDa IROMP was produced by the wild type after induction by quinolobactin-containing culture supernatants obtained from the pyoverdine-negative mutant or by purified quinolobactin. Conversely, adding purified wild-type pyoverdine to the growth medium resulted in suppression of the 75-kDa IROMP in the pyoverdine-deficient mutant; however, suppression was not observed when Pseudomonas aeruginosa PAO1 pyoverdine, a siderophore utilized by strain 3G6, was added to the culture. Therefore, we assume that the quinolobactin receptor is the 75-kDa IROMP and that the quinolobactin-mediated iron uptake system is repressed by the cognate pyoverdine.

Vi polysaccharide and iron-regulated outer-membrane proteins (IROMPs) were extracted and purified from the standard strain of Salmonella typhi, Ty2. These were then conjugated by chemical coupling using the carbodimide method. Vi–IROMP conjugate was tested for its ability to protect against colonization by S. typhi in different organs. Mice immunized with 2.5 μg Vi–IROMP conjugate showed the most protection, as the least bacterial colonization of spleen, liver and Peyer's patches was observed. Peritoneal macrophages obtained from conjugate-treated mice phagocytosed bacteria efficiently. Circulating antibodies and the delayed type hypersensitivity response elucidated by mouse foot-pad swelling was significantly higher in conjugate-treated animals. These results clearly demonstrate that an IROMP and polysaccharide conjugate of S. typhi prepared from the same strain has the potential to protect animals against challenge.

Almost 30 years after the first endoscopic mitral valve repair, Minimally Invasive Mitral Valve Surgery (MIMVS) has become the standard at many institutions due to optimal clinical results and fast recovery. The question that arises is can already good results be further improved by an Institutional Risk Management Performance (IRMP) system in decreasing risks in minimally invasive mitral valve surgery (MIMVS)? As of yet, there are no reports on IRMP and learning systems in the literature. (2) Methods: We described and appraised our five-year single institutional experience with MIMVS in isolated valve surgery included in the Netherlands Heart Registry (NHR) and investigated root causes of high-impact complications. (3) Results: The 120-day and 12-month mortality were 1.1% and 1.9%, respectively, compared to the average of 4.3% and 5.3% reported in the NHR. The regurgitation rate was 1.4% compared to 5.2% nationwide. The few high-impact complications appeared not to be preventable. (4) Discussion: In MIMVS, freedom from major and minor complications is a strong indicator of an effective IRMP but remains concealed from physicians and patients, despite its relevance to shared decision making. Innovation adds to the complexity of MIMVS and challenges surgical competence. An IRMP system may detect and control new risks earlier. (5) Conclusion: An IRMP system contributes to an effective reduction of risks, pain and discomfort; provides relevant input for shared decision making; and warrants the safe introduction of new technology. Crossroads conclusions: investment in machine learning and AI for an effective IRMP system is recommended and the roles for commanding and operating surgeons should be considered.

Abstract The progenitor systems and explosion mechanisms responsible for the thermonuclear events observationally classified as Type Ia supernovae are uncertain and difficult to uniquely constrain using traditional observations of Type Ia supernova host galaxies, progenitors, light curves, and remnants. For the subset of thermonuclear events that are prolific producers of iron, we use published theoretical nucleosynthetic yields to identify a set of elemental abundance ratios infrequently observed in metal-poor stars but shared across a range of progenitor systems and explosion mechanisms: [Na, Mg, Co/Fe] < 0. We label stars with this abundance signature “iron-rich metal-poor,” or IRMP stars. We suggest that IRMP stars formed in environments dominated by thermonuclear nucleosynthesis and consequently that their elemental abundances can be used to constrain both the progenitor systems and explosion mechanisms responsible for thermonuclear explosions. We identify three IRMP stars in the literature and homogeneously infer their elemental abundances. We find that the elemental abundances of BD +80 245, HE 0533–5340, and SMSS J034249.53–284216.0 are best explained by the (double) detonations of sub-Chandrasekhar-mass CO white dwarfs. If our interpretation of IRMP stars is accurate, then they should be very rare in globular clusters and more common in the Magellanic Clouds and dwarf spheroidal galaxies than in the Milky Way’s halo. We propose that future studies of IRMP stars will quantify the relative occurrences of different thermonuclear event progenitor systems and explosion mechanisms.

Abstract It has been suggested that a class of chemically peculiar metal-poor stars called iron-rich metal-poor (IRMP) stars formed from molecular cores with metal contents dominated by thermonuclear supernova nucleosynthesis. If this interpretation is accurate, then IRMP stars should be more common in environments where thermonuclear supernovae were important contributors to chemical evolution. Conversely, IRMP stars should be less common in environments where thermonuclear supernovae were not important contributors to chemical evolution. At constant [Fe/H] ≲ −1, the Milky Way’s satellite classical dwarf spheroidal (dSph) galaxies and the Magellanic Clouds have lower [α/Fe] than the Milky Way field and globular cluster populations. This difference is thought to demonstrate the importance of thermonuclear supernova nucleosynthesis for the chemical evolution of the Milky Way’s satellite classical dSph galaxies and the Magellanic Clouds. We use data from the Sloan Digital Sky Survey Apache Point Observatory Galactic Evolution Experiment and Gaia to infer the occurrence of IRMP stars in the Milky Way’s satellite classical dSph galaxies η dSph and the Magellanic Clouds η Mag, as well as in the Milky Way field η MWF and globular cluster populations η MWGC. In order of decreasing occurrence, we find η dSph = 0.07 − 0.02 + 0.02 , η Mag = 0.037 − 0.006 + 0.007 , η MWF = 0.0013 − 0.0005 + 0.0006 , and a 1σ upper limit η MWGC < 0.00057. These occurrences support the inference that IRMP stars formed in environments dominated by thermonuclear supernova nucleosynthesis and that the time lag between the formation of the first and second stellar generations in globular clusters was longer than the thermonuclear supernova delay time.

Le traitement conventionnel du cancer localisé de la prostate, le plus fréquent chez l'homme, repose sur des approches de traitement de la glande entière et entraîne un risque important d'effets indésirables en termes de continence et de sexualité. A l'heure actuelle, les différentes stratégies de traitements peuvent être une chirurgie, une radiothérapie externe ou une curiethérapie interstitielle. Cependant, des échantillons chirurgicaux ont montré que chez une proportion significative de patients, le risque de progression du cancer était lié à un seul foyer de cancer indifférencié. De ce fait, le traitement de la lésion index primitive permettrait à la fois de lutter contre le cancer et de préserver la qualité de vie des patients. Bien que les développements récents des technologies d'imagerie et de biopsie puissent décrire l'étendue du cancer, le défi de focaliser avec précision le traitement sur la cible unique reste une tâche ardue, et les traitements focaux proposés entre la surveillance active du cancer de la prostate à faible risque et le traitement définitif des formes agressives sont encore des options en cours d'évaluation. En effet, les traitements focaux soulèvent donc des questions encore imparfaitement résolues pour un montage clinique optimal : Comment définir précisément le volume cible? Comment focaliser l'agent thérapeutique sur cette cible? Et enfin, comment surveiller le reste de la glande après le traitement ? Ce projet de thèse a pour but de développer et d'évaluer la faisabilité dosimétrique d'un traitement expérimental ultra-focal d'un cancer de la prostate par une technique de curiethérapie de grains liés d'iode 125 (I-125) et de développer une méthode de quantification dans la définition précise du foyer primitif tumoral à l'aide d'imagerie IRM multiparamétrique (IRMmp). Pour cela, le nombre et l'activité des grains d'I-125 utilisées pour la curiethérapie conventionnelle de la prostate sont définis sur la base du volume de la glande entière (environ 2 grains/cc). Par définition, la curiethérapie ultra-focale s'adresse à des cibles de formes complexes et de plus petits volumes, ce qui nous a poussé à développer une méthode dosimétrique d'implantation de multiples grains de faible activité. Deuxièmement, l'objectif de précision dans la délivrance des grains nous a conduit à mettre en œuvre l'utilisation séquentielle de trois systèmes d'imagerie distincts: l'IRMmp pour informer sur la position et le volume de la cible, l'échographie transrectale 3D (TRUS) pour délivrer un marqueur fiduciaire dans la cible tirant parti de la fusion non rigide avec l'IRMmp et une sonde échographique 2D biplanaire requis pour la planification du traitement et la pose des grains. Enfin, l'évaluation de la qualité du traitement - c'est-à-dire de la dose délivrée à la cible unique, par opposition à la glande entière en curiethérapie conventionnelle - a été rendu possible grâce au développement d'une méthode de quantification basée sur la définition précise de la position du volume cible tumoral à traiter à partir des données des imageries IRMmp et en comparaison par rapport aux données dosimétriques obtenues pour chaque patient de l'étude
The conventional treatment of localized prostate cancer, the most common cancer in men, is based on whole-gland treatment approaches and drives a significant risk of adverse effects in terms of continence and sexuality. Currently, various treatment strategies include surgery, external beam radiotherapy, or brachytherapy. However, surgical specimens have shown that in a significant proportion of patients, the risk of cancer progression is associated with a single focus of undifferentiated cancer. Therefore, treating the primary index lesion would achieve both cancer control and preservation of quality of life. Although recent developments in imaging and biopsy technologies can outline the extent of the cancer, the challenge of focusing the required precision with the treatment agent remains a daunting task, and focal treatments proposed between active surveillance of low-risk Prostate Cancer and definitive treatment of aggressive forms are still options under assessment Indeed, focal treatments raise questions that are still imperfectly resolved for an optimal clinical implementation: How to precisely define the target volume? How to focus the therapeutic agent on this target? And finally, how to monitor the rest of the gland after treatment? The aim of this thesis project is to develop and evaluate the dosimetric feasibility of an experimental ultra-focal treatment for prostate cancer using iodine-125 seeds brachytherapy and to develop a quantification method for the precise definition of the primary tumor focus using multiparametric MRI (mpMRI) imaging. To achieve this, the number and activity of I-125 seeds used for conventional prostate brachytherapy are defined based on the volume of the entire gland (approximately 2 seeds/cc). By definition, ultra-focal brachytherapy targets complex-shaped and smaller volumes, prompting us to develop a dosimetric method for implanting multiple seeds with low activity. Secondly, the precision objective in seed delivery led us to implement the sequential use of three distinct imaging systems: mpMRI to inform on the position and volume of the target, 3D transrectal ultrasound to deliver a fiducial marker into the target leveraging non-rigid fusion with mpMRI, and a 2D biplanar ultrasound probe required for treatment planning and seed placement. Finally, the evaluation of treatment quality - that is, the dose delivered to the single target as opposed to the entire gland in conventional brachytherapy - was made possible by developing a quantification method based on the precise definition of the position of the target tumor volume to be treated from mpMRI data and compared to dosimetric data obtained for each patient in the study

Pour réduire le temps d'acquisition ou bien améliorer la résolution spatio-temporelle dans certaines application en IRM, de puissantes techniques parallèles utilisant plusieurs antennes réceptrices sont apparues depuis les années 90. Dans ce contexte, les images d'IRM doivent être reconstruites à partir des données sous-échantillonnées acquises dans le " k-space ". Plusieurs approches de reconstruction ont donc été proposées dont la méthode SENSitivity Encoding (SENSE). Cependant, les images reconstruites sont souvent entâchées par des artéfacts dus au bruit affectant les données observées, ou bien à des erreurs d'estimation des profils de sensibilité des antennes. Dans ce travail, nous présentons de nouvelles méthodes de reconstruction basées sur l'algorithme SENSE, qui introduisent une régularisation dans le domaine transformé en ondelettes afin de promouvoir la parcimonie de la solution. Sous des conditions expérimentales dégradées, ces méthodes donnent une bonne qualité de reconstruction contrairement à la méthode SENSE et aux autres techniques de régularisation classique (e.g. Tikhonov). Les méthodes proposées reposent sur des algorithmes parallèles d'optimisation permettant de traiter des critères convexes, mais non nécessairement différentiables contenant des a priori parcimonieux. Contrairement à la plupart des méthodes de reconstruction qui opèrent coupe par coupe, l'une des méthodes proposées permet une reconstruction 4D (3D + temps) en exploitant les corrélations spatiales et temporelles. Le problème d'estimation d'hyperparamètres sous-jacent au processus de régularisation a aussi été traité dans un cadre bayésien en utilisant des techniques MCMC. Une validation sur des données réelles anatomiques et fonctionnelles montre que les méthodes proposées réduisent les artéfacts de reconstruction et améliorent la sensibilité/spécificité statistique en IRM fonctionnelle

L’IRM apporte des informations morphologiques concernant la tumeur, mais également des informations concernant sa micro-vascularisation. En TEP/TDM, l’accumulation de la 18F-FDopa dans les cellules tumorales résulte de l’activité métabolique plus importante que celle des tissus sains. Nous avons étudié 28 gliomes pour lesquels nous avons analysé les données provenant d’IRM et de TEP/TDM. Une méthode de recalage a été développée afin de combiner les informations issues des deux modalités TEP et IRM et d’extraire des volumes d’intérêt sur la base des données conjointes TEP et IRM. L’analyse du contenu de ces volumes d’intérêt par un modèle de mélange gaussien a permis de différencier, dans ces volumes, les tissus tumoraux et les tissus sains, et d’obtenir ainsi des volumes tumoraux et de référence communs pour les modalités TEP et IRM. Des paramètres issus de la TEP ou de l’IRM ont ensuite été calculés dans ces volumes communs aux deux modalités, pour caractériser les tumeurs et les tissus sains. L’analyse discriminante linéaire (ADL) des données TEP/TDM et d’IRM combinées permet de discriminer les différentes classes tissulaires. Les courbes Receiver Operating Characteristic ROC combinées à l’ADL permettent d’évaluer les critères multiples [SUVmax , rCBV] et [rk1 , rCBV] et conduisent à des AUC respectives de 0,88 et 0,92. En considérant les informations combinées [SUVmax , rCBV], nous avons obtenu une sensibilité de détection des tumeurs de haut grade de 95% pour une spécificité correspondante de 60% ainsi qu’une valeur prédictive négative de 52% pour une valeur prédictive positive de 95%. De même, avec le critère [rk1 , rCBV], nous avons obtenu une spécificité de 60% pour 95% de sensibilité de détection des tumeurs de haut grade ainsi qu’une valeur prédictive négative de 60% pour une valeur prédictive positive de 95%. Nos travaux montrent que la fusion des informations microvasculaires et métaboliques est possible. Dans le cas du diagnostic différentiel des gliomes, l’information microvasculaire n’apporte cependant pas d’information plus discriminante que l’information métabolique seule
MRI provides morphological information about tumour, but also provides information regarding the micro-vascularization of the tumour. In PET/CT, the accumulation of 18F-FDopa in tumour cells results from the metabolic activity greater than that of healthy tissues. We studied 28 gliomas for which we analysed data from MRI and PET/CT. A registration method has been developed to combine information from both PET and MRI and to extract volumes of interest consistent with the information included in the two modalities. In these volumes, the tumour compartment and normal tissue compartment were identified using a Gaussian mixture model. Parameters from PET or MRI data were then calculated in these compartments. ROC analyses combined with linear discriminant analyses were used to assess whether joint observation of standardized uptake value (SUVmax ) and relative Cerebral Blood Volume (rCBV) or of relative rk1 and rCBV could distinguish between low grade and high grade tumours. We found that using this joint analysis, 82.4% of high-grade tumors and 70.0% of low-grade tumors were correctly classified (AUC of 0.88 for [SUVmax , rCBV] and of 0.92 for [rk1 , rCBV]). Considering the [SUVmax , rCBV] combined information, the sensitivity for detecting high-grade tumors was 95% with a specificity of 60%. The negative predictive value was 52% for a positive predictive value of 95%. Similarly, considering the [rk1 , rCBV] combined information, we also a specificity of 60% associated with a 95% sensitivity for detecting high-grade tumors, with a negative predictive value of 60% and positive predictive value of 95%. Our work shows that joint analysis of microvascular and metabolic information is possible by combining PET and MR imaging data. However, we found that, in our patient population, the microvascular information given by MR did not bring information more discriminating than the metabolic information derived from PET only

Antimicrobial proteins (AMPs) are effector molecules and an important part of the innate immune system. AMPs have a broad antimicrobial spectrum and lyse microbial cells by interaction with biomembranes. Besides direct impact in host defence, AMPs are mediators of inflammation with impact on epithelial and inflammatory cells influencing diverse processes such as cytokine release, cell proliferation, angiogenesis, wound healing, chemotaxis, immune induction, and protease-antiprotease balance. AMPs could replace antibiotics which efficiency has decreased due to extensive clinical use. Therefore knowledge of mechanism of action of antimicrobial peptides, their properties and possible usage is essential for their further use as therapeutics. Ticks are blood-feeding ectoparisites that serve as extremely effective vectors of pathogens. Analysis of the ticks molecules that are involved in immune response to the pathogens invasion represent one of the strategies in searching for new compounds that might be used in future as theurapeutic agents. This study represents analysis of newly identified antimicrobial peptide form in the hard tick Ixodes ricinus (IRAMP).
