ABSTRACT Background The number of graduating primary care physicians will not meet the demands of the growing and aging US population. In 2011, the Veterans Affairs (VA) Office of Academic Affiliations established 5 Centers of Excellence in Primary Care Education (CoEPCE) to transform primary care training. Objective We created an innovative training model with immersive primary care experiences to foster careers in primary care for residents. Methods As a CoEPCE, the Seattle VA partnered with the University of Washington internal medicine residency program to form a Center of Excellence (CoE) pathway with increased outpatient training time. The CoEPCE created a longitudinal curriculum of continuity clinic immersion and new thematically based rotations (eg, Homeless Health) for CoE residents. These rotations expanded primary care experiences and allowed for in-depth opportunities to care for the unique needs of veterans. Resident feedback was solicited through program evaluations, and career choices were tracked. Results Eighty-five of 102 (83%) possible rotation evaluations from 2014 to 2017 were reviewed. Residents reported that CoEPCE rotations had a positive effect on their care of patients and career choice, and provided opportunities to interface with faculty role models. Seventy-five percent of Seattle VA CoE residents selected primary care careers compared to 36% of historical controls. Conclusions The CoEPCE rotation curriculum offers in-depth primary care training and may contribute to trainees maintaining interest in primary care careers.

Abstract (sommario):
Homelessness is a social phenomenon that manifests itself equally in Poland and all over the world. Despite the continuous development of civilization, homelessness does not decrease, but rather increases. When raising the issue of homelessness, one should keep in mind its complexity and the impossibility of unambiguous wording. No less difficult is the task of revealing the main cause that would help slow down the development of such a serious social problem. The article draws attention to the social phenomenon of homelessness not only from the standpoint of lack of permanent residence, but also considers the individual characteristics of the homeless, taking into consideration such issues as: individuality of emotions and personal tendencies, specific attitude to self-perception, attitude to the family, attitude to the life stabilization, the ability to solve existential problems, the specifics of cultural behavior. The author points out the social causes of homelessness, such as: certain factors related to addictions, personality disorders, disability, as well as serious diseases that require expensive treatment, social factors arising from family conflicts, educational institutions or rehabilitation centers, structural factors related to unemployment, poverty and housing problems. The author of the article admits that the percentage of education among homeless people will increase if society pays enough attention to the homeless, and provides the special assistance to this category of people. It is also emphasized that assistance to the homeless should be offered in the following areas: psychological assistance, which can clarify the reasons that led a person to a difficult life situation, social care — in order to return homeless people to normal functioning in the society; career guidance assistance: cooperation with the institutions that will find jobs for the homeless people.

The need for counseling and career/educational services for homeless veterans has captured political and economic venues for more than 25 years. Veterans are three times more likely to become homeless than the general population if veterans live in poverty or are minority veterans. This mixed methods study emphasized a life-span perspective approach for exploring factors influencing normative aging and life-quality of 39 homeless veterans in Alabama and Florida. Seven descriptive quantitative and qualitative research questions framed the investigation. Study participants completed a quantitative survey reflecting their preferences and needs with a subset of the sample (N=12) also participating in individual qualitative interview sessions. Thirty-two service providers and stakeholders completed quantitative surveys. Empirical and qualitative data with appropriate triangulation procedures provided interpretive information relative to a life-span development perspective. Study findings provide evidence of the need for future research efforts to address strategies that focus on the health and economic challenges of veterans before they are threatened with the possibility of homelessness. Implications of the study findings provide important information associated with the premise that human development occurs throughout life with specific characteristics influencing the individual’s passage. Implications for aging/homelessness research are grounded in late-life transitioning and human development intervention considerations.

This welled up into community activism and advocacy for the homeless together with Berkeley Catholic Workers, eventually resulting in a café inside of Berkeley’s People’s Park in 1989, providing rationale for Scheper-Hughes’s own well-known applied anthropology and activism, which has made her famous as one of today’s leading anthropologists.

In the Summer of 2001, on The Heels of a Four-Year Recession, a Number of Ostensible Want Ads Appeared on Large Billboards in downtown Zurich. “Sought for our sales department: team member with an alcohol problem,” read one of them (fig.1); “Wanted: coworker with a drug career,” read another. Each billboard also featured the slogan “Everyone deserves a second chance.” The billboards were one of the first public campaigns mounted by the editors of Surprise, the Swiss street magazine that has supported the homeless and poor by employing them as vendors since 1998. The magazine's declared aim was to attract salespeople from the community, but the billboards did much more than recruit vendors: they intervened in Zurich's upscale business district and challenged the divide separating the haves from the have-nots.

Abstract: Poverty is one of the factors of urbanization. Unfortunately, there are so many people who urbanize to big cities without adequate education, experience and skills so that many of them fail to make a career and make a living in the city and end up being homeless and beggars. The government through the government regulation of the Republic of Indonesia Number 31 of 1980 seeks to overcome the phenomenon of homeless people and beggars by establishing rehabilitation centers and holding mental and spiritual training in them. Therefore, the purpose of this study is to 1) explain the process of spiritual mental development carried out by religious mentors to foster sprawl independence at BRSEGP Pangudi Luhur Bekasi 2) analyze the supporting and inhibiting factors faced in the process of mental spiritual development at BRSEGP Pangudi Luhur Bekasi. This research uses a qualitative approach with a case study method. The research informants were taken by purposive sampling technique as many as 8 informants; 1 (one) religious advisor, 3 (three) social workers, 4 (four) homeless people and beggars. The results showed that 1) the process of implementing mental spiritual development at BRSEGP Pangudi Luhur Bekasi was carried out for two hours every Monday, Wednesday and Friday night and the methods used were lectures, question and answer methods, and demonstration methods, 2) supporting factors in the process. spiritual guidance is a mentor and coach who is experienced and understands the material, the material is easy to understand, the friendliness of the coach and mentor and the willingness to change in the homeless and beggars. The inhibiting factors include the indiscipline of homeless people and beggars, activities can only be followed by homeless people and beggars who don't work, lack of facilities and differences in education levels of homeless people and beggars.Abstrak: Kemiskinan merupakan salah satu faktor terjadinya urbanisasi. Sayangnya banyak sekali penduduk yang melakukan urbanisasi ke kota besar tanpa persiapan pendidikan, pengalaman dan keterampilan yang memadai sehingga banyak dari mereka yang gagal untuk berkarir dan mencari penghidupan di kota dan berakhir menjadi gelandangan dan pengemis (gepeng). Pemerintah melalui peraturan pemerintah Republik Indonesia Nomor 31 Tahun 1980 berupaya menanggulangi fenomena gepeng dengan mendirikan panti-panti rehabilitasi dan mengadakan peminaan mental spiritual di dalamnya. Maka dari itu tujuan penelitian ini untuk 1) menjelaskan proses pembinaan mental spiritual yang dilakukan pembimbing agama untuk menumbuhkan kemandirian gepeng di BRSEGP Pangudi Luhur Bekasi 2) menganalisis faktor pendukung dan penghambat yang dihadapi pada proses pembinaan mental spiritual di BRSEGP Pangudi Luhur Bekasi. Penelitian ini menggunakan pendekatan kualitatif dengan metode studi kasus. Adapun informan penelitian diambil dengan teknik purposive sampling sebanyak 8 orang informan; 1 (satu) orang pembimbing agama, 3 (tiga) orang pekerja sosial, 4 (empat) orang gepeng. Hasil penelitian menunjukkan bahwa 1) proses pelaksanaan pembinaan mental spiritual di BRSEGP Pangudi Luhur Bekasi di laksanakan selama dua jam setiap hari Senin, Rabu dan malam Jumat dan metode yang digunakan adalah ceramah, metode Tanya jawab, dan metode demonstrasi, 2) faktor pendukung dalam proses pembinaan spiritual adalah pembimbing dan pembina yang berpengalaman dan memahami materi, materi mudah dipahami, keramahan pembina dan pembimbing dan adanya kemauan untuk berubah pada diri gepeng. Adapun faktor penghambat meliputi ketidak disiplinan gepeng, kegiatan hanya bisa diikuti gepeng yang tidak bekerja, kurangnya fasilitas dan perbedaan tingakat pendidikan gepeng.

Contemporary academic literature has controversial opinions about selflessness of motives of volunteers. Some authors state that volunteers are primarily motivated either by altruistic causes or egoistic factors; others declare the impossibility of the existence of pure altruists or egoists in the activity. Part of scholars report on the extreme importance of age regarding volunteers’ motives. Conducted studies mostly consider volunteers with people. Papers investigating motives of volunteers with homeless animals are limited. A still unsolved question is whether it is reasonable to apply received results from those who volunteer with people to volunteers with non-human recipients. In the paper, we investigate the particularities and structures of motives among volunteers with diverse ages and recipients (N=220), utilizing VFI (Volunteer Function Inventory). The research sample consisted of volunteers serving people and/or homeless animals. It has been found that volunteers are mostly motivated by values, understanding, and enhancement functions. The social, career and protective functions were not common among the volunteers. Younger volunteers tend to be more multi-motivated and actuated by egoistic factors. Starting at age 25, the older the volunteer was, the more likely their motives were selfless and distinct. Studies such as ours can be valuable in recruitment and retention of volunteers according to their needs and motives. The obtained data can be used by organizations working with differently aged volunteers and diverse recipients of assistance (e.g., people, animals).

In North America, the key performance indicator of success in community strategies to address homelessness is whether a homeless person is housed or not. In this article, we argue that for young people experiencing homelessness, we need to advance a broader consideration of outcomes to include a range of well-being indicators designed to understand the needs of developing adolescents and young adults and contribute to housing stability. We articulate that the positive outcomes of young people across life domains that include housing stability as well as their safety and security, health and well-being, social connections to peers, family and meaningful adults, connections to groups/neighbourhoods/communities, interests and recreation and leisure, and school and career/work aspirations and goals must be at the centre of these efforts. The Making the Shift project is designed to test this outcomes framework in order to enhance service and measurement capacity and ultimately improve outcomes for youth.

Ce travail porte sur la catégorie d’action publique « jeunes en errance ». Il vise à comprendre son impact sur la mise en œuvre de l’action publique sur le terrain au travers de l’analyse des usages qu’en font les acteurs professionnels et les jeunes eux-mêmes. Après avoir retracé les étapes de la constitution du phénomène de l’errance des jeunes en problème public, nous avons analysé le rôle de l’Etat social dans la prise en charge de ce problème, au prisme de la norme instituée de l’insertion pour les jeunes. Ceci nous a permis d’en pointer les apories et notamment la bascule rapide des jeunes cumulant des difficultés depuis l’enfance vers le circuit de l’infra-assistance. A l’aune de ces constats, nous montrons en quoi la relation d’aide devient un lieu de renégociation des objectifs de l’action publique, les artisans de la relation d’aide considérés étant les professionnels de terrain et les jeunes usagers des services d’aide. L’analyse des pratiques professionnelles nous a révélé leur caractère « bricolé ». Nous avons ici pu démontrer qu’elles permettaient de redéfinir les cadres de l’action publique en direction des « jeunes en errance ». Grâce à la mise en place d’une éthique de l’intervention centrée sur la prise en compte des spécificités des jeunes accueillis, les professionnels fabriquent des ponts leur permettant d’aller au-delà des manques de l’aide publique. Nous avons également analysé les comportements des jeunes fréquentant régulièrement les structures de l’urgence sociale. Leur mode de recours à l’aide leur permet d’asseoir une identité légitime de « jeunes en errance », qu’ils valorisent au sein de la relation d’aide. Ils mettent en place des stratégies identitaires les autorisant à renégocier les cadres de l’attribution de l’aide, en y ayant recours de manière détournée ou partielle. Ils ignorent ainsi les objectifs d’insertion qui lui sont attachés, en en faisant un usage routinier, les entretenant alors dans une carrière de « jeunes en errance »
This research investigates the French public action category “jeunes en errance” (which will be translated by “homeless youth”). It aims at understanding the impact of this category on the implementation of the public action, through the analysis of the usage that both the professional actors and the youth make of it. After having redrawn the stages of the construction of the homeless youth phenomenon as a public problem, the analysis focused on the role of the Welfare state based on the instituted norm of the youth’s integration. Paradoxes of public action have thus been identified, such as the quick turnaround towards the infra-assistance system of young people who have accumulated difficulties since their childhood. In the light of these findings, the research then shows how this “support relationship” transforms itself in an arena for the renegotiation of the public action's objectives between the professionals on the one hand, and the youth recipients of the services on the other hand. The analysis of the professional's practices puts the light on their unstable dimension. The research has shown that these practices facilitated the redefinition of the frameworks of the public action toward “homeless youth”. Through the setting of an intervention’s ethics based on the specificities of each young person, the professionals manage to compensate the shortages of public assistance. The behavior of young people who frequently resort to the structures of social emergency have also been investigated. The analysis has identified the process of construction of a legitimate identity of “homeless youth”, which young people put forward within the assistance relationship. They set up identity strategies which enable them to renegotiate the conditions of the allocation of support, by resorting to it but only in a partial or in an indirect way. Thus, they ignore the insertion objectives which are attached to these public measures and make a routinized usage of them, which contributes to maintain them in a “homeless youth” career

Cette thèse en sociologie se fixe comme objectif d’appréhender l’éventail de facteurs structurels et de raisons individuelles étant à l’origine du développement du nombre de personnes sans-domicile en France. Au terme de ce travail, les différents rouages de la fabrique sociale du sans-abrisme seront mis à jour et il nous deviendra alors possible de comprendre comment un phénomène social, qui fait de nouveau partie des préoccupations majeures de nos institutions depuis une quarantaine d’années, n’ait toujours pas été résolu. …/…Je suis en mesure d’apporter une réponse détaillée à ce questionnement grâce au point de vue inédit que j’ai pu avoir sur les institutions chargées de prendre en charge le sans-abrisme. En effet, de 2015 à 2018, j’ai été embauché en tant que doctorant-chercheur CIFRE au sein du Service Intégré d’Accueil et d’Orientation (SIAO) du Val-d’Oise. Ainsi, j’ai été intégré au cœur du secteur de l’Accueil, de l’Hébergement et de l’Insertion (secteur AHI) de ce département, ce qui m’a permis d’avoir un regard de l’intérieur du fonctionnement de ces dispositifs. J’y ai mené une étude ethnographique approfondie du travail des acteurs AHI et du quotidien des personnes sans-domicile. Aussi, cette thèse a été l’occasion de décrire les principales étapes et dynamiques du processus de transformation de soi expérimenté par les personnes sans-domicile. À cette fin, j’ai recouru au concept de carrière de sans-domicile, dont la modélisation constitue l’un des apports de cette thèse. Dans cette perspective, un suivi de plusieurs années de 80 ménages sans-domicile a été réalisé
This thesis in sociology aims to understand the range of structural factors and individual reasons behind the growth of the number of homeless people in France. At the end of this work, the various working of the social factory of homelessness will be updated and it will then become possible for us to understand why a social phenomenon, which has been a major preoccupation of our institutions for the last 40 years, has not been resolved. …/…I am able to provide a detailed answer to this question thanks to the new point of view I have had on the institutions responsible for taking charge of homelessness. Indeed, from 2015 to 2018, I was hired as a CIFRE doctoral researcher in the Service Intégré d'Accueil et d'Orientation (SIAO) in Val-d'Oise. Thus, I was integrated into the heart of the Reception, Accommodation and Insertion (AHI) sector of this department, which allowed me to have an inside look at the operation of these devices. I conducted an ethnographic of the work of AHI actors and the daily life of homeless people. Thereby, this thesis was an opportunity to describe the main stages and dynamics of the process of self-transformation experienced by homeless people. To this end, I used the concept of a career of homeless, whose modelling constitutes one of the contributions of this thesis. In this perspective, a follow-up of several years of 80 trajectories of homeless households was carried out

Resumo: A pesquisa trata-se de um levantamento epidemiológico no Município de São Paulo ¿ SP, Brasil, em 137 crianças em situação de rua, de ambos os sexos, para avaliar as condições de saúde bucal. As inspeções foram realizadas segundo critério da OMS, utilizando o índice CPO-D. O termo ¿em situação de rua¿ foi utilizado para definir jovens que vivem perambulando pelas ruas e praças públicas, ainda, para aqueles que pela baixa condição econômica vivem de pequenos trabalhos ou esmolando pelos faróis da cidade, que podem vir a ser um morador de rua. Os últimos trinta e cinco anos da odontologia brasileira sofreram profundas transformações técnicas e científicas, que resultou na queda do índice de CPO-D nos diversos grupos populacionais. Em 1986, o CPO-D aos doze anos de idade, a média nacional era de 6,67, reduzindo em 2004 para a marca de 2.8, atingindo as metas da OMS para o ano 2000, por outro lado o índice encontrado para cidade de São Paulo foi de 2,3. Os jovens foram divididos em grupos: No grupo I ¿ crianças que trabalhavam em semáforos, o índice CPO-D aos 12 anos foi de 4,4; o grupo II ¿ jovens moradores de rua, CPO-D, de 6,5; e finalmente o grupo III ¿ adolescentes institucionalizados, CPO-D obtido 4,7, desviando da média estadual de 2,30. A faixa etária de 5 à 6 anos, encontramos 100% das crianças acometidas pela cárie dental, apresentando CPO médio de 1.45. O atendimento odontológico, no município de São Paulo, principalmente na periferia, é limitada em recursos humanos, na grande maioria das unidades de saúde o cirurgião dentista não faz parte do quadro funcional, e onde estão presentes, praticam exclusivamente uma odontologia mutiladora, sem a valorização da prevenção, e não utilizam em larga escala os modernos materiais odontológicos, principalmente os liberadores de flúor. Pressupomos que as condições socioeconômicas desses menores sejam condicionantes às cáries dentarias, geralmente habitam espaços reduzidos, com lugares impróprios a higiene bucal, convivendo diariamente com a falta do imprescindível à qualidade de vida, sobrevivem com uma média de duzentos e quarenta reais, por família. Abstract: The aim of this study was to realize and epidemiologic survey in São Paulo ¿ SP, Brazil, in 137 street young of both sexus, to evoluate the conditions of oral health, the oral inspections, were done following who¿s criterion. The Young, had been divided in group I ¿ they work in lighthouses, the index DMFT of 12 years old was 4,4. In the group II ¿ they live in the street, DMFT, was 6,5, and group III ¿ they are in an institution, DMFT, was 4,7. Deviating from the state average of 2,30. Of this children, 36,5% did not possessed dental brush and 58% do not utilize the dental creme relugalarly. Ken Loach is one of Britain's most distinguished, and respected, film-makers. His career embraces both film and television. His landmark TV production of Cathy Come Home caused such an outcry over the plight of the homeless that Shelter was established in response. His film work is as remarkable as his television work. He makes tough, uncompromising films about a beleaguered working class – but with a poetry (as in Kes) and with a humanity soaked in humour (as in Riff Raff and Raining Stones). His work has been feted, especially on the Continent where Riff Raff received the Felix award (Europe's equivalent of the Oscar). Raining Stones won the Jury Prize at Cannes and the Best British Film of the Year award, and Land and Freedom, his film about the Spanish Civil War, won the International Critics prize at Cannes. Loach on Loach is an exploration of Ken Loach's cinema of social conscience, making much use of interviews and conversations with the man himself.

In the Netherlands, the city of Utrecht is leading in providing adequate accommodation through mixed housing projects for ‘regular’ tenants and people previously living in homeless services or protected housing facilities. ‘New’ homeless persons also obtain the possibility for making a new start, instead of having to depend on shelters first. The concept of mixed housing, not to be confused with mixed income housing, relates in our definition to small and medium-sized (up to 500 residents) housing projects that are home to different groups of people who intentionally live next to each other, connect and engage in joint activities.In this paper, we examine three projects, with mainly self-contained dwellings: ‘Groene Sticht’ (since 2003), a small neighbourhood with 69 regular tenants and home-owners, and 35 ex-homeless persons; ‘Parana’ (2014), a purpose build complex with 24 regular and 44 (ex-)homeless individuals/families; ‘Majella Wonen’ (2016), older basic, post-war dwellings with 39 regular tenants and 35 homeless persons/families. These price-winning projects, co-created by a homeless service, social integration services and a social housing provider are built on an innovative concept of social management, with a high level of self-organisation. All residents are fully eligible members of the residents-committees and take responsibility for activities such as festivities, gardening, and the selection of new tenants. If necessary, ex-homeless inhabitants receive individualised support.We discuss structures and mechanisms that help homeless people feel at home amidst their (new) neighbours and foster their social integration. In addition, we identify several tensions that hamper integration and analyse the ways in which both residents and professionals try to tackle these obstacles.We collected data (2016 -2018) through the participatory meetings of a Community of Practice on Mixed housing, the study of documents, in-depth interviews with inhabitants of the housing complexes, focus group-sessions with professionals and interviews with local stakeholders.Based on this research we will apply for follow up funding. Through national funds and/or European funds.

Background: The COVID-19 pandemic has disproportionately impacted homeless populations and service workers, especially within homeless shelter/hostel settings. To date, there have been few evidence syntheses examining outbreaks of COVID-19 among both homeless shelters residents and service workers, nor has there been a critically engaged summary of relevant infection control and prevention (IPAC) measures. This scoping review offers a timely and much-needed synthesis of COVID-19 prevalence within homeless shelters and a review of current and pertinent IPAC measures. Methods: We conducted a scoping review in June 2021 that synthesized academic and grey literature published from March 2020 to July 2021 pertaining to 1) the prevalence of COVID-19 among both residents and staff in homeless shelters and hostels in high-income countries, and 2) COVID-19 IPAC strategies applied in these settings. Two reviewers independently screened the results of the literature search of several databases that included MEDLINE, PsycInfo, and the WHO’s COVID-19 Global Health Portal. All the extracted data was mapped, categorized, and thematically discussed. Results: Thematic analysis of included literature revealed five key themes: 1) the demographics of COVID-19 in homeless shelters, 2) asymptomatic spread, 3) pre-existing vulnerability of the homeless and shelters, 4) the limited application of IPAC, and 5) IPAC effectiveness. Conclusion: This review offers a useful glimpse into the landscape of COVID-19 outbreaks in homeless shelters/hostels and the major contributing factors to these events. The scoping review revealed that there is no clear indication of generally accepted IPAC standards for homeless populations and shelter care workers. This review also illustrated a great need for future research to establish IPAC best practices as well as additional resources for shelter systems to protect residents and staff at homeless shelters/hostels in high-income countries. Finally, the findings from this review reaffirm that homelessness prevention is a key to limiting disease outbreaks, and the associated negative health outcomes in shelter populations.

Community members who evacuate to shelters may represent the most socially and economically vulnerable group within a hurricane’s affected geographic area. Disaster research has established associations between socioeconomic conditions and adverse effects, but data are overwhelmingly collected retrospectively on large populations and lack further explication. As Hurricane Florence approached North Carolina in September 2018, RTI International developed a pilot survey for American Red Cross evacuation shelter clients. Two instruments, an interviewer-led paper questionnaire and a short message service (SMS text) questionnaire, were tested. A total of 200 evacuees completed the paper survey, but only 34 participated in the SMS text portion of the study. Data confirmed that the sample represented very marginalized coastline residents: 60 percent were unemployed, 70 percent had no family or friends to stay with during evacuation, 65 percent could not afford to evacuate to another location, 36 percent needed medicine/medical care, and 11 percent were homeless. Although 19 percent of participants had a history of evacuating for prior hurricanes/disasters and 14 percent had previously utilized shelters, we observed few associations between previous experiences and current evacuation resources, behaviors, or opinions about safety. This study demonstrates that, for vulnerable populations exposed to storms of increasing intensity and frequency, traditional survey research methods are best employed to learn about their experiences and needs.
