Abstract (sommario):
We aimed to investigate the effects of melatonin and resveratrol on diabetes-related papillary muscle dysfunction and structural heart disorders. The protective effect of resveratrol and melatonin supplementation on cardiac functions was investigated in a diabetic elderly female rat model. 16-month-old rats (n=48) were allocated into 8 groups. Group1: Control, Group2: Resveratrol Control, Group3: Melatonin Control, Group4: Resveratrol and Melatonin Control, Group5: Diabetes, Group6: Diabetes Resveratrol, Group7: Diabetes Melatonin, Group8: Diabetes Resveratrol and Melatonin. Streptozotocin was injected intraperitoneally to the rats for experimental diabetes induction. Thereafter, resveratrol (intraperitoneal) and melatonin (subcutaneous) were administered for 4 weeks. Resveratrol and melatonin had a protective effect on the contractile parameters and structural properties of the papillary muscle, which was impaired by diabetes. it has been presented that diabetes impairs the contractile function of the papillary muscle for each stimulus frequency tested and the responses obtained as a result of Ca+2 uptake and release mechanisms from the Sarcoplasmic reticulum, and it has been observed that these effects are improved with resveratrol and melatonin injection. The decrease in myocardial papillary muscle strength in the diabetic elderly female rat can be reversed with the combination of resveratrol, melatonin and resveratrol+melatonin. Melatonin+resveratrol supplementation is no different from melatonin and/or resveratrol supplementation. Resveratrol and melatonin supplementation may have a protective effect on cardiac functions in a diabetic elderly female rat model.

Objective: Aspirin is a non-steroidal anti-inflammatory drug with antioxidative properties. It is recommended to use different doses and durations according to the characteristics of the patient and the type of disease. Therefore, in this study, we aimed to investigate the effect of using aspirin at different doses and for different durations on oxidative stress in ovarian tissue. Material and Methods: Female Wistar albino rats were divided into five groups. Group 1: control group, no special treatment was applied to the rats in this group. Group 2: 1 mg/kg aspirin was administered orally to the rats in this group every day for 28 days. Group 3: 3 mg/kg aspirin was administered orally to rats in this group every three days. Ggroup 4: 5 mg/kg aspirin was administered orally to rats in this group every five days. Group 5: 7 mg/kg aspirin was administered orally to the rats in this group once a week. After fasting overnight following the last application, the rats were sacrificed, and their ovarian tissues were collected. Malondialdehyde, catalase, total thiol group, and AOPP levels were studied from ovarian tissue. Results: Group4 and group5 ovarian tissue MDA levels were found to be significantly higher than the other groups (p<0.05). There was no significant difference between group1, group2 and group3 ovarian tissue MDA levels (p>0.05). Group1 (control group) ovarian tissue AOPP level was found to be significantly lower than all aspirin-administered groups (p<0.05). Group2 ovarian tissue AOPP level was found to be significantly lower than group3, group4 and group5 (p<0.05). TSG level was found to be significantly higher in group 5 when compared to other groups (p0<0.05). Group4 ovarian tissue TSG level was found to be significantly higher when compared to group1, group2 and group3 (p<0.05). Group3 and group4 ovarian tissue CAT activity was found to be significantly higher than group1, group2 and group5 (p<0.05). When group1, group2 and group5 ovarian tissue CAT activities were compared, no significant difference was found (p>0.05). Conclusion: The application of aspirin at certain intervals rather than daily application may have more positive effects on the antioxidant system. especially taking aspirin at intervals of 3 or 5 days may be more effective

Introduction: Anxiety related to dentistryis not uncommon in paediatric patients. Poor pain control alongside the fear and anxiety might interfere with appropriate dental management. In this study a modified mucosal vibration device is used to modify the pain perception during LA administration. Aim: To compare the effectiveness of topical anaesthetic gel and mucosal vibration during local anaestheticadministration in children in the age group8-11 years using both subjective and objective scalescales for measuring anxiety and pain. Methodology: Thirty children who required inferior alveolar nerve blockanaesthesia for routine dental procedures were included and divided equally into 3 groups Group I- control Group II- topical anaesthetic gel Group III - Mucosal vibrator Pain perception was compared, assessed and analysed during injection using Sound, Eye, Motor (SEM) scale, Doremon cartoon modified Wong Baker Faces Pain Rating Scale, MotuPatlu cartoon modified Venham scale Results: The scores of SEM scale and Doremon pain scale when subjected to statistical analysis gave a p value <0.001. Addiction is related to aggression and quality of life. This study examined the relationship between these three factors according to occupation group in a mixed urban/rural area to better understand adult addiction problems. This study was a secondary data analysis of cross-sectional data collected by a 2017 regional survey of adults living in Gunsan City, South Korea. The survey included 500 people split into the unemployed (Group1), full-time homemakers (Group2), and primary (Group3), secondary (Group4), and tertiary (Group5) industry workers. Addiction problems and aggression were positively correlated (p < 0.01). Aggression and alcohol use disorder were correlated in Group3 (r = 0.31), Group4 (r = 0.34), and Group5 (r = 0.32), and aggression and smartphone addiction were correlated in Group2 (r = 0.39) and Group4 (r = 0.31). Problem gambling was correlated with aggression in Group5 (r = 0.39). A negative relationship between quality of life and alcohol use disorder occurred in Group1 (r = −0.36). According to the occupation group, the relationships between addiction problems, aggression, and quality of life were different. These findings suggest that addiction management for adults should be implemented in consideration of occupation groups.

ABSTRACTObjective: To investigate the effect of a prophylaxis paste on surface roughness of different composites, enamel, dentin and porcelain surfaces.Methods: Three different composites (FiltekZ250/Group1, Filtek Supreme XT/Group2, Premise/ Group3), enamel/Group4, dentin/Group5 and porcelain/Group6 samples were used in this study. All specimens were prepared flat by SiC discs and polished with a diamond polishing paste. The surface roughness measurements were determined with a profilometer after polishing (initial surface roughness). Prophylaxis paste was applied to the samples for 12 seconds, renewing every 6 seconds. After cleaning the samples, roughness values were measured again. Data were analyzed by Kruskal Wallis and Dunn’s multiple comparison test. Wilcoxon test was performed for the comparison of the initial and final surface roughness values (P<.05). The results were evaluated within the P<.05 confidence level.Results: The mean %SMHC was:C=77.27%, CL= 72.45%, SL=78.43%, G=66.65% and P=67.95%. Comparing the %SMHC promoted by 5 soft drinks, SL = C > CL > P = G (P<.05). There was not significant correlation between %SMHC and the other variables tested for the five drinks (P>>.05)Results: The initial and final surface roughness values (μm) were determined as follows: Group1: 0.039±0.009 and 0.157±0.018, Group2: 0.023±0.005 and 0.145±0.027, Group3: 0.028±0.008 and 0.109±0.012, Group4: 0.024±0.006 and 0.071±0.015, Group5: 0.030±0.007 and 0.143±0.029, Group6: 0.024±0.006 and 0.064±0.014. Significant difference was determined between the initial and final values for all groups.Conclusions: Composite and dentin surfaces were more affected by the application of prophylaxis paste than enamel and porcelain surfaces. The prophylaxis paste increased the surface roughness of all groups, but did not reach the bacterial retention roughness rate of 0.2μm. (Eur J Dent 2012;6:1-8)

Helical plates are preferably used for proximal humeral shaft fracture fixation and potentially avoid radial nerve irritation as compared to straight plates. Aims:(1) to investigate the safety of applying different long plate designs (straight, 45°-, 90°-helical and ALPS) in MIPO-technique to the humerus. (2) to assess and compare their distances to adjacent anatomical structures at risk.MIPO was performed in 16 human cadaveric humeri using either a straight plate (group1), a 45°-helical (group2), a 90°-helical (group3) or an ALPS (group4). Using CT-angiography, distances between brachial arteries and plates were evaluated. Following, all specimens were dissected, and distances to the axillary, radial and musculocutaneous nerve were evaluated.None of the specimens demonstrated injuries of the anatomical structures at risk after MIPO with all investigated plate designs. Closest overall distance (mm(range)) between each plate and the radial nerve was 1(1-3) in group1, 7(2-11) in group2, 14(7-25) in group3 and 6(3-8) in group4. It was significantly longer in group3 and significantly shorter in group1 as compared to all other groups, p<0.001. Closest overall distance (mm(range)) between each plate and the musculocutaneous nerve was 16(8-28) in group1, 11(7-18) in group2, 3(2-4) in group3 and 6(3-8) in group4. It was significantly longer in group1 and significantly shorter in group3 as compared to all other groups, p<0.001. Closest overall distance (mm(range)) between each plate and the brachial artery was 21(18-23) in group1, 7(6-7) in group2, 4(3-5) in group3 and 7(6-7) in group4. It was significantly longer in group1 and significantly shorter in group3 as compared to all other groups, p<0.021.MIPO with 45°- and 90°-helical plates as well as ALPS is safely feasible and showed a significant greater distance to the radial nerve compared to straight plates. However, distances remain low, and attention must be paid to the musculocutaneous nerve and the brachial artery when MIPO is used with ALPS, 45°- and 90°-helical implants. Anterior parts of the deltoid insertion will be detached using 90°-helical and ALPS implants in MIPO-technique.

Background. From 2005 to 2015 routine calcitonin (CT) screening was performed in our department in all patients with multinodular goiter (MNG) using the same assay.Aim. We investigated possible associations between unstimulated serum CT levels and the presence of either thyroid autoimmunity (AITD) or thyroid neoplasia.Methods. This is a retrospective study of 648 patients (559 female [F] 86.3%, 89 male [M] 13.7%, age range 18-89, median 58 years,). CT≤4.6 pg/ml [F] and ≤11.5 pg/ml [M] was defined as normal. Patients were stratified into 4 groups according to CT. Group1: CT<0.05 (undetectable), Group2: CT [F&M] within normal range, Group3: CT:4.7-10 [F] & 11.6-20 [M], Group4: CT>10 [F] & >20 [M]. Furthermore patients were subcategorized in those with Autoimmune Thyroid Disease (AITD) and those without (non-AITD).Results. The distribution of patients was: Group1: n=186 (28.7%), Group2: n=422 (65.1%), Group3: n=29 (4.5%), Group4: n=11 (1.7%). Of the patients with AITD history 23.4% belonged to Group1, 68.6% to Group2, 6.4% to Group3 and 1.6% to Group4 (x2, p=0.037). Forty seven patients (7.3%) underwent total thyroidectomy. Histopathological examination revealed: Medullary Thyroid Carcinoma (MTC) n=3 (3/3 Group4), C-Cell Hyperplasia (CCH) n=5 (3/5 Group3, 2/5 Group4), Papillary Thyroid Carcinoma (PTC) n=17 (7/17 Group1, 10/17 Group2), MNG n=22 (8/22 Group1, 10/22 Group2, 2/22 Group3, 2/22 Group4). 2/5 patients with CCH had PTC. 1/17 PTC patient had mixed PTC-MTC. Patients with MTC had remarkably higher CT levels (253-1222 pg/ml) compared to those with CCH (5.8-16.1 pg/ml).Conclusions. This study reaffirms the positive correlation between CT levels and the presence of MTC or CCH, clearly and conspicuously distinguished by the range of CT levels, albeit in a small number of patients with these diagnoses. Patients with AITD have more frequently detectable or slightly increased CT levels.

Proximal humeral shaft fractures are commonly treated with long straight plates or intramedullary nails. Helical plates might overcome the downsides of these techniques as they are able to avoid the radial nerve distally. The aim of this study was to investigate in an artificial bone model: (1) the biomechanical competence of different plate designs and (2) to compare them against the alternative treatment option of intramedullary nails.Twenty-four artificial humeri were assigned in 4 groups and instrumented as follows: group1 (straight 10-hole-PHILOS), group2 (MULTILOCK-nail), group3 (45°-helical-PHILOS) and group4 (90°-helical-PHILOS). An unstable proximal humeral shaft fracture was simulated. Specimens were tested under quasi-static loading in axial compression, internal/external rotation and bending in 4 directions monitored by optical motion tracking.Axial displacement (mm) was significantly lower in group2 (0.1±0.1) compared to all other groups (1: 3.7±0.6; 3: 3.8±0.8; 4: 3.5±0.4), p<0.001. Varus stiffness in group2 (0.8±0.1) was significantly higher compared to groups1+3, p≤0.013 (1: 0.7±0.1; 3: 0.7±0.1; 4: 0.8±0.1). Varus bending (°) was significantly lower in group2 compared to all other groups (p<0.001) and group4 to group1, p=0.022. Flexion stiffness in group1 was significantly higher compared to groups2+4 (p≤0,03) and group4 to group1, p≤0,029 (1: 0.8±0.1; 2: 0.7±0.1; 3: 0.7±0.1; 4: 0.6±0.1). Flexion bending (°) in group4 was higher compared to all other groups (p≤0.024) and lower in group2 compared to groups1+4, p≤0.024. Torsional stiffness remained non significantly different, p≥0.086. Torsional deformation in group2 was significantly higher compared to all other groups, p≤0.017. Shear displacement remained non significantly different, p≥0.112.From a biomechanical perspective, helical plating with 45° and 90° may be considered as a valid alternative fixation technique to standard straight plating of proximal third humeral fractures. Intramedullary nails demonstrated higher axial and bending stiffness as well as lower fracture gap movements during axial loading compared to all plate designs. However, despite similar torsional stiffness they were associated with higher torsional movements during internal/external rotation as compared to all investigated plate designs.

Abstract Somatic genomic alterations acquired during GBM growth enhance adaptation of tumor cells to their microenvironment and give rise to molecular heterogeneity. Radiogenomics could facilitate exploration of the underlying pathobiology of tumor growth in specific microenvironments and thereby, promote precision medicine for the patients. We derived radiogenomic signatures of key driver genes and evaluated molecular compositions of tumor groups with predisposition to specific brain regions. Pre-operative multiparametric conventional MRI scans of 357 IDH-wildtype GBM patients with available targeted NGS data were jointly segmented and registered into a common template. We constructed spatial distribution atlases for tumors harboring mutations in driver genes and identified four distinct groups of tumor locations with predilection to the left frontal cingulate region (Group1), right temporal (Group2), right parietal (Group3), and occipital pole (Group4). Evaluation of the differences in molecular features of the tumor groups included: (1) exploring similarities of genomic profiles across all four groups by evaluating cosine similarity metric (CSM) between mutational signatures; (2) quantification of molecular heterogeneity based on Mutant Allele Tumor Heterogeneity (MATH) scores; and (3) inference of the evolutionary trajectories. Groups 1 and 4 were the most different, and Groups 2 and 3 were the most similar tumors, molecularly. The mutational signatures between Groups 1 and 4 revealed a CSM of 0.35. Group1 showed significantly lower MATH score (less heterogeneity) compared to Group4 (p&lt; 0.05). Evaluation of evolutionary patterns suggested NF1 mutation as an early event in Group1, without subsequent gain of function or mutation in EGFR. In contrast, in Group4, EGFR mutations were early events triggering PTEN mutations later in the evolutionary trajectory. Radiogenomic signatures revealed distinct molecular underpinnings for the tumors with predilection towards specific brain regions that may suggest existence of different tumor microenvironments in different brain regions that cause intra- and inter-patient heterogeneity in the molecular tumor composition.

Category: Midfoot/Forefoot Introduction/Purpose: The purpose of this retrospective study is to clarify the difference in plantar pressure distribution during walking and related patient-based outcomes between forefoot joint-preserving arthroplasty and resection-replacement arthroplasty in patients with rheumatoid arthritis (RA). Methods: Four groups of patients were recruited. Group1 included 22 feet of 11 healthy controls (age 48.6 years), Group2 included 36 feet of 28 RA patients with deformed non-operated feet (age 64.8 years, Disease activity score assessing 28 joints with CRP [DAS28-CRP] 2.3), Group3 included 27 feet of 20 RA patients with metatarsal head resection-replacement arthroplasty (age 60.7 years, post-operative duration 5.6 years, DAS28-CRP 2.4), and Group4 included 34 feet of 29 RA patients with metatarsophalangeal (MTP) joint-preserving arthroplasty (age 64.6 years, post-operative duration 3.2 years, DAS28-CRP 2.3). Patients were cross-sectionally examined by F-SCAN II® to evaluate walking plantar pressure, and the self-administered foot evaluation questionnaire (SAFE-Q). Twenty joint-preserving arthroplasty feet were longitudinally examined at both pre- and post-operation. Results: In the 1st MTP joint, Group4 showed higher pressure distribution (13.7%) than Group2 (8.0%) and Group3 (6.7%) (P<0.001). In the 2nd-3 rd MTP joint, Group4 showed lower pressure distribution (9.0%) than Group2 (14.5%) (P<0.001) and Group3 (11.5%) (P<0.05). On longitudinal analysis, Group4 showed increased 1st MTP joint pressure (8.5% vs. 14.7%; P<0.001) and decreased 2nd-3 rd MTP joint pressure (15.2% vs. 10.7%; P<0.01) distribution. In the SAFE-Q subscale scores, Group4 showed higher scores than Group3 in pain and pain-related scores (84.1 vs. 71.7; P<0.01) and in shoe-related scores (62.5 vs. 43.1; P<0.01). Conclusion: Joint-preserving arthroplasty resulted in higher 1st MTP joint and lower 2nd-3 rd MTP joint pressures than resection-replacement arthroplasty, which were associated with better patient-based outcomes.

Noise, the most commonly encountered stressor in our daily lives, affects the body in different ways depending on its frequency, intensity, and duration. Thus, this study aimed to demonstrate the effect of noise stress exposure in different durations on the immunological status of femaleand maleadult mice. Sixty femaleand male adult mice (Balb/C)were used in this. Fifty mice were exposed to noise stress at 90degreesfor four hours per day,and 10mice (5males and 5females) were left without exposure to stress which was used as a control group. Exposure stress mice were divided into five groups (n =10, 5males and 5females); Group1 was exposed to noise stress for only 1day, Group2 for 8days, Group3 for 14days, Group4 for 21days, and Group5 for 30days. The blood sample was collected and used for immunoglobulins, glucocorticoids and WBCs indices tests. Results showed that there was a highly significant increase in glucocorticoids for all periods after noise exposure. WBCcount, the level of IgG and IgM were highly significantin mice that were exposed to noise stress during1,8,14, and 21 days, while there was no significant difference in the 28-days stress group compared to control. In conclusion, the levels of immunoglobulins levels after 28 day of noise stress exposure could completely differ from the short-term exposure.

This paper is a sequel to [2]. A polycyclic-by-finite group G was there called dihedral free if G contains no subgroup isomorphic to 〈b, a:ba = b-1 a2 = 1〉 whose normalizer has finite index in G. It was shown in [2, Theorem F] that, if R is a commutative Noetherian domain, the group ring RG is a prime Noetherian maximal order if and only if R is integrally closed, G is dihedral free, and G has no non-trivial finite normal subgroups. Throughout, R and G will be assumed to satisfy these hypotheses. The main aim of the paper is to study the class group of the maximal order RG.

To determine the serum concentration of certain inflammatory markers including TNF-α and hs-CRPβ-thalassemic patients on different types of treatmentand to investigate the possible correlation between these inflammatory markers and iron overload referred to by serum ferritin concentration, moreoverto assesswhether inflammation in β-thalassemia could be controlled by deferasirox or deferoxamine as compared to transfusion dependent patients without iron chelator. Ninety transfusion dependent β-thalassemic children with age range 13–75 months were included in this study, and 30 age and sex matched healthy subjects served as control,thepatients were divided into three groups according to the type of the treatment,each group included 30 patients Group1 comprised β-thalassemic patients without iron chelator, Group2 comprisedβ-thalassemic patients on deferasirox iron chelator while Group3 comprisedβ-thalassemic patients on deferoxamine iron chelator. Serum ferritin, hs-CRP, and TNF-α was measured for all participants by ELISA method. Compared with the control, the serum level of ferritin and hs-CRP were significantly elevated in all patients groups while the serum level of TNF-α were only significantly elevated in Group1 and Group3. Further analysis of the results revealed positive correlation between serum ferritin withTNF-α and with hs-CRP.A comparison among the patients groups show that patients on deferasirox (Group2) had significantly low level of TNF-α as compared to Group1 and Group3. On the other hand serum ferritin and hs-CRP concentration in Group3 were significantly higher than the other two groups, furthermore when Group3 patients are subdivided according to their compliance on deferoxaminethe result show that the serum level of ferritin, hs-CRP and TNF-α are significantly higher in poor compliance subgroup as compared to good compliancepatients and when good compliance subgroup are compared with Group2 no significant deference in the measured parameter was found. Chronic inflammatory state is present in these patients withincreased levels of inflammatory markers such as TNF-α andCRP. The observed correlations of ferritin with inflammatory markers imply that iron overload may play a key role in release of these markers. Chelation Therapy with deferasirox can attenuate inflammation and reduces inflammatory markers level while deferoxamine appear to be less effective as most patients suffer from poor compliance on this type of iron chelator.

Se propone la Clínica Grupal Operativa (CGO) como alternativa, pues el modelo biomédico sobremedicaliza el sufrimiento de la población. La CGO se basa en el paradigma biopsicosocial, incluye la subjetividad e intersubjetividad en la psicopatología, propone una psicopatología vincular y grupal, y plantea estrategias psicoterapéuticas de mayor complejidad (pareja, grupo, familia y multifamiliar). La CGO, que tiene como núcleo central la teoría psicoanalítica, es un nuevo modelo grupal para la comprensión de los procesos de salud-enfermedad mental, fundamental, a nuestro entender, en la formación de los profesionales para el buen desarrollo de las tareas de asistencia, docencia e investigación en los SSM.

Background:IL-6 contributes significantly to the chronic inflammatory process of rheumatoid arthritis (RA). Sarilumab (SRL), a human anti-human IL-6 receptor alpha monoclonal antibody that blocks the signaling originated by the IL-6/IL-6R complex like tocilizumab,is an effective treatment. However, predictors of the response to sarilumab are still required.Objectives:We aimed to combine IL-6, soluble IL-6R (sIL-6R) and gp130 (sgp130) levels to identify groups of sarilumab responses.Methods:This research is a retrospective study. a total of 32 RA patients with SRL therapy in our department from February 1 in 2018 to December 31 in 2019 were included. Serum and clinical data from 32 RA patients were collected before treatment and until the last visit. Follow-up period was up to one year after starting SRL treatment. Serum were tested for IL-6 (Human IL-6 Quantikine ELISA Kit, R&D systems), sIL-6R (Human soluble IL-6R alpha Quantikine ELISA Kit, R&D systems) and sgp130 (Human soluble gp130 Quantikine ELISA Kit, R&D systems), using specific ELISAs according to the manufacturer’s instructions. Hierarchical cluster analysis (JMP14.3.0) was used to establish the relationship between IL-6, sIL-6R and sgp130. We evaluated the efficacy of SRL treatment on the last visit using European League Against Rheumatism (EULAR) response criteria in the groups of patients. The other statistical analyses were performed with EZR 1.41, and p Values less than 0.05 were considered significant.Results:The median age of patients was 70.5 (IQR: 66.5-74.3) years and the median of disease duration was 7.3 (1.7-15.3) years. Nine (28.1%) patients were biologics and Jakinibs naive. the median follow-up periods were 24 (12-26) weeks. The baseline DAS28 was median 4.39 (3.77 - 5.43), and CDAI was 21.1 (11.7-29.5). When comparing responders and non-responders, there were no significant differences in any of the baseline parameters and cytokines. Four statistical significant clusters of RA patients (i.e., Group1, Group2, Group3 and tocilizumab use group before SRL) were defined by serum concentrations of IL-6, sIL-6R and spg130 at baseline. The levels of IL-6 expressed as median in Group1 patients were 25.6 (14.4–72.2) pg/ml, in Group2 5.9 (3.3–11.3) pg/ml, and in Group3 70.2 (45.4–86.1) pg/ml (p < 0.002, significant difference only between Group2 and Group3). The levels of sIL-6R expressed as median in Group1 patients were 38.7 (34.7-45.1) ng/ml, in Group2 35.1 (24.8-41.9) ng/ml, and in Group3 35.7 (34.2-39.8) ng/ml (p = 0.5477). The levels of sgp130 expressed as median in Group1 patients were 272.6 (263.0-277.2) ng/ml, in Group2 223.1 (221.0-228.0) ng/ml, and in Group3 204.6 (192.0-207.6) ng/ml (p < 0.00003, significant difference between the three groups respectively). There were no significant differences in any of the baseline clinical features and laboratory findings between the three groups. Out of the 8 patients in Group1 had a good or moderate response to SRL. Conversely, the percentage of patients with no response to SRL was higher in Group3 than in Group1 and Group2.Conclusion:RA patients could be easily stratified prior to the rapeutic intervention with sgp130 related to the IL-6 signal reguration. Abstract Abstract 3083 Allogeneic hematopoietic stem cell transplantation (HSCT) is indicated for patients (pts) with acute leukemia (AL) with poor-risk cytogenetics or refractoriness to chemotherapy. For adults requiring HSCT urgently, such as pts in first complete remission (CR1), a single (s) or double unit (d) UCBT is a valid stem cells source. In the sUCBT setting, type of conditioning regimen seems to be associated with better outcome (Sanz BMT 2012). With the aim to compare single vs double UCBT after myeloablative conditioning regimen (MAC) in a homogeneous series of pts, we analyzed 239 adults (>18years) with AL in CR1. Pts were transplanted with sUCBT (n=156) or dUCBT (n=83) from 2005–2011 in EBMT centers for ALL (n=101) and AML (n=138). Type of MAC was statistically associated with outcomes therefore pts were analyzed in 3 different groups: Group 1: pts receiving sUCBT with TBI-based+Cy (+Flu) (n=68) (performed in 42 transplant centers (TC)), Group 2: pts receiving sUCBT with Bu+Flu+Thiotepa (n=88) (performed in 23 TC) and Group 3: pts receiving dUCBT with Cy+TBI+Flu (n=83) (performed in 47 TC). No statistical differences were found among the 3 groups for pts and disease characteristics (diagnosis, risk, gender, weight, CMV status, year of UCBT and time from diagnosis to UCBT) however pts in group2 were older than in group1 and 3 (median age 38 vs 33 vs 31 years) (p=0.03). Cytogenetic at diagnosis was available for 176 pts, 39% of pts were classified in the intermediate risk and 56% in unfavorable risk group. Forty-two pts had t(9;22) and 26 FLT3/ITD mutation. No differences on cytogenetic were found among the 3 groups. Thirty one percent of CB units were identical to recipient or had 1 HLA disparity (antigen level typing for HLA-A and B and allelic level for DRB1) while 69% had 2–3 HLA disparities. There was no difference on HLA disparities among the 3 groups. Median infused TNC was 2.9×107/kg for group1, 3×107/kg for group2, and 3.7×107/kg for group3 (p=0.01) and median CD34 was 1.2×105/kg, 1.6×105/kg and 1.5×105/kg, respectively (p=0.32). ATG was part of conditioning regimen in 73% of pts. The use of ATG was different in the 3 groups (70%, 90% and 40% for group1, 2 and 3, respectively p<0.001). GVHD prophylaxis consisted either of CSA±MMF or CSA±steroids in 46% and 22% of pts, respectively. All groups had the same median follow-up time: 24 (range 3–74) months. For group1, group2 and group3, the cumulative incidence (CI) of 60 days neutrophil recovery was 82%, 89% and 87% (p=0.15), with median time of 27, 21 and 24 days, respectively (p<0.001). Chimerism analysis performed at day 100 showed full donor chimerism in 87% of pts (data available for 80% of pts who engrafted). No differences in chimerism status were found between the 3 groups (p=0.47). At day 100, CI of acute GVHD (grade II-IV) was 30% vs 20% vs 45% for group1, group2 and group3, respectively (p=0.001). Pts receiving a dUCBT who developed aGvHD (n=38), experienced mainly grade II aGvHD with skin involvement (grade II (n=25), grade III (n=10), grade IV (n=3)). CI of chronic GvHD at 1 year was 29%, with no differences in the incidence among the groups. At 1 year, CI of TRM was 44% for group1, 33% for group2 and 36% for group3 (p=0.46). In multivariate analysis, two factors were associated with higher TRM: diagnosis of ALL (p=0.048) and age>35 years (p=0.049). One-Hundred-six pts died and the causes of death were infection (n=38), GvHD (n=18), other transplant-related events (n=31) or relapse (n=18). CI of 2y relapse was 25% for group1, 18% for group2 and 16% for group3 (p=0.22). No factors were found to be associated with increase relapse incidence in multivariate analysis. The 2y probability of leukemia-free-survival (LFS) was 31% for group1 (sUCBT-TBI based), 48% for group2 (sUCBT-BuFluTT), and 47% for group3 (dUCBT) (p=0.03). No center effect was found for LFS. In multivariate analysis, use of sUCBT using TBI based MAC (HR=0.9, p=0.003), diagnosis of ALL (HR=0.69, p=0.04) and age>35years (HR=1.4, p=0.04) were independently associated with decreased LFS. In this retrospective based registry analysis, in the myeloablative setting for adults with AL in CR1, outcomes (TRM, RI and LFS) after dUCBT were not statistically different from sUCBT using iv-BuFluTT. However, compared to sUCBT using TBI-based MAC, dUCBT was associated with lower RI and better LFS rates. In the MAC setting, the combination of conditioning regimens and type of graft (single vs. double) may have different impact UCBT outcomes. Disclosures: No relevant conflicts of interest to declare.

IntroductionAlthough there are several empirical studies exploring the relationship between problematic Internet use and personality traits, few had considered perfectionism.ObjectivesTo explore the association between generalized problematic Internet use and perfectionism.MethodsA sample of 433 Portuguese university students (Mage = 20.15 years, SD = 1.77, range = 18-25 years) completed the Generalized Problematic Internet Use Scale 2/GPIU and the Multidimensional Perfectionism Composite Scale – short version.ResultsGPIU total score (rs=.16**), Mood Regulation (rs=.22**), and Deficient Self-Regulation (rs=.13**) were correlated with Positive Striving factor. GPIU total score (rs=.38**), Preference for Online Social Interaction (rs=.16**), Mood Regulation (rs=.28**), Deficient Self-Regulation (rs = .33**), and Negative Consequences (rs=.41**) were significantly correlated with Evaluative Concerns factor. A one-way between groups analysis of variance was conducted to explore the relation between GPIU and perfectionism. Subjects were divided into three groups according to their GPIU risk levels (Group1:low-risk; Group2:medium-risk; Group3:high-risk). There was a statistically difference at p >.05 level in Positive Striving scores for the three risk level groups: F (2,430)=4.39, p=.013, and in Evaluative Concerns scores, F (2,430)=28.83, p=<.001. Post-hoc comparisons using the Tukey USD test, for Positive Striving, indicated that the mean score for Group1 (M=39.21, SD=8.56) was significantly different from Group3 (M=43.69, SD=9.74). Considering Evaluative Concerns, the mean score for Group1 (M=39.86, SD=11.31) was significantly different from Group2 (M=46.91, SD=11.42) and from Group3 (M=51.75, SD=8.54).ConclusionsGPIU is consistently related to maladaptive perfectionism. Future longitudinal studies are needed to clarify the bidirectional association between GPIU and perfectionism traits.

Background: Early detection and/or characterization of cancer associated biomarkers have revolutionized the personalized treatment approach of cancer to date. Therefore this study was design to accessed the prevalence of suspected cancer biomarkers from northeastern part of Bangladesh. Materials and Methods: A total of 892 patients’ data were analyzed in a cross-sectional manner and the collection period was 2017 to 2019. An ELISA-based chemiluminescent microparticle immune assay was used to detect serum cancer biomarkers. An unpaired t-test was used to compare the independent variables. Results: Data were clustered into four categories; group1 (10-17 yr, adolescent), group2 (18 to 35 yr, young adult), group3 (36 to 55 yr, middle), and group4 (>55 yr, older adult). Serum PSA level was higher in group4 than group3 (mean ± sem, 8.422 ± 1.423 vs 3.884 ±1.15 ng/ml). CA-125 is significantly higher in females with age > 35 yr (older and middle) than at age <35 yr (young adult and adolescent). No significant difference in AFP was found among the four age groups while sexual disparity has shown a statistically significant difference. Both males and females from group4 were showing a higher expression level of CEA (96.94 ± 54.02 and 48.23 ± 25.45 ng/ml respectively) though gender-wise test frequency was same. CA19-9 was high in group4 whereas no correlation was found between the gender variation. Conclusion: Our analysis have shown that level of PSA, CA-125, CEA and CA19-9 were upregulated in older peoples, while AFP was higher in male than female.

Abstract Background Screening is an important tool in staging esophageal cancer as only patients without suspicion of metastases are considered for surgery-based treatment. Nevertheless unexpected metastatic disease is still found in some patients during surgery. In these cases should esophagectomy be aborted, or is there a place for palliative resection? Methods Between 2002 and 2015, 681 patients with locally advanced potentially resectable esophageal cancer were scheduled for neoadjuvant therapy and subsequent esophagectomy. In 552 patients potentially curative esophagectomy was performed. In twelve patients, unexpected disease was discovered during surgery but esophagectomy was performed with synchronous resection of the metastases. Ten of them were oligometastatic (≤ 4 single organ metastases). In 117 patients esophagectomy was not performed with among them 50 patients because of disease progression. Fourteen of these patients were also single organ oligometastatic and ten of them received systemic treatment. 10 single organ oligometastatic patients that underwent esophageal resection (group1) were compared to 10 non-resected—but treated—counterparts (group2) and to 228 patients that underwent a potentially curative esophagectomy with persistent pathological lymph nodes (group3). Clinicopathological data were retrospectively reviewed and survival of the three groups was compared from date of pathological diagnosis. Results In the oligometastatic esophagectomy patients, 5 had lung metastases, 1 peritoneal, 2 adrenal, 1 pleural, and 1 pancreatic. In the oligometastatic non-resected patients, 2 had lung metastases, 5 liver and 3 brain metastases. Median overall survival was 21.4, 12.1 and 20.2 months in the respective groups. (group1 vs group2: P = 0.042; group2 vs group3: P = 0.002; group1 vs group3: P = 0.88). Conclusion Survival is prolonged in patients undergoing palliative esophagectomy in case of unexpected single organ oligometastatic disease during surgery and is comparable to survival of patients with persistent pathological lymph nodes. Palliative resection in unexpected oligometastatic disease seems to be justified. Disclosure All authors have declared no conflicts of interest.

Aim of study: To evaluate demographical dynamics of yew populations in the Iberian Mediterranean mountains in order to assess population trends and a hypothetical natural regeneration of the species.Area of study: Ayllón Mountain Range, Eastern end of Central Mountain Range, Spain.Material and methods: Afield survey was conducted on three yew groves, taking data of altitude, orientation, companion woody species, UTM coordinates, height, perimeter (at breast height, 120cm) and sex. Individuals were classified in four age groups, considering height and trunk perimeter: AgeGroup1, Seedling; AgeGroup2, Juvenile; AgeGroup3, Adult and AgeGroup4, Old. Finally Recruitment rate was calculated for each population.Main results: Surveyed yew populations include more than 1600 trees, showing elevated Recruitment rates, which indicates that there are more seedling and juvenile individuals (Group1 and Group2) than adult and old ones (Group3 and Group4).Research highlights: Surveyed yew populations show vigorous regeneration. These results lead us to reconsider the relictic character assigned to the species in the Iberian Mediterranean mountains.Key words: Ayllón; microclimatic; regeneration rate; relictic; survey.

Abstract Introduction: Success of endodontic treatment relies on minimizing microbial load by chemo-mechanical preparation and intra-canal medication(ICM). Calcium hydroxide based ICMs have known disadvantages. Calcium silicate-based cements(CSC) exhibit antibacterial activity, thus promoting researchers to experiment with their formulations to use them as ICMs. Aim: Evaluation and comparison of the antimicrobial efficacy of two experimental CSC (MTA & Biodentine + 2%chlorhexidine) and Bio-C Temp against E.faecalis. Methods and Material: Test materials were divided into four groups namely Group1-Bio-C Temp, Group2-UltraCAL XS, Group3-Biodentine+2%CHX and Group4-MTA+2%CHX. Direct contact test was done by placing a standardized suspension of E.faecalis on test materials and bacterial growth was assessed spectrophotometrically using ELISA at one, three and seven days. Statistical Analysis: Data was analysed using one-way ANOVA, Tukey’s multiple post hoc test and paired-t test. Results: Intragroup comparison revealed decreased mean optical density(OD) in groups 1, 2, and 4; no significant difference in group 3. Intergroup comparison showed statistical differences in mean OD values between groups (3 and 4); groups (1 and 2) at days one(p-0.018) and three(p-0.035), but no difference individually. Group 4 showed the highest antimicrobial efficacy on day seven. Conclusion: MTA+2%CHX & Biodentine+2%CHX showed better antimicrobial efficacy and hence could be used as potential ICMs.

Objective: To assess the influence of operator experience on: shaping and centering ability, mean preparation time and presence of canal aberrations of ProTaper Universal and WaveOne systems on simulated root canals. Materials and Methods: Sixty S-shaped canals in resin blocks were assigned to four groups (n=15 for each group). Group1 (Experienced operator, ProTaper), Group2 (Experienced operator, WaveOne), Group3 (Inexperienced operator, ProTaper), Group4 (Inexperienced operator, WaveOne). Photographic method was used to record pre- and post-instrumentations images. After superimposition, it has been evaluated presence of canal aberrations and differences in shaping and centering ability between groups. Results: WaveOne system produced a lower amount of canal aberrations both in the hand of expert than inexpert operators. However, a WaveOne instrument breakage occurred in the hands of an inexperienced operator. No differences have been found in the evaluation of shaping ability with both systematics. Operator’s experience doesn't influence the shaping ability of ProTaper and WaveOne systems. Conclusion: Experience factor could influence the centering ability in the use of both the systematics. However, WaveOne Primary reduce the mean preparation time and the presence of canal aberrations.

Abstract Understanding the immune microenvironment in pediatric low-grade glioma (pLGG) patients may help in identification of the patients who benefit from anti-tumor immunotherapies. However, surgical resection is not feasible for many pLGG tumors in certain anatomical locations. Therefore, developing non-invasive tools that characterize the tumor microenvironment prior to therapeutic interventions could contribute to stratification and enrollment of the patients into relevant clinical trials. In this work, we derived radiomic signatures of immune profiles (radioimmunomics) based on machine learning (ML) analysis of readily available conventional MRI scans. Transcriptomic data for a cohort of 197 subjects was retrospectively collected from Open Pediatric Brain Tumor Atlas (OpenPBTA). The patients were categorized into three groups (Group1-3) based on their immunological profiles using consensus clustering algorithm. This analysis revealed greater immune cell infiltration in non-BRAF mutated pLGGs. Group1 showed more enrichment in M1 macrophages, and microenvironment and immune scores compared to Group2 and Group3. Elevated tumor inflammation score (TIS), as a predictor of clinical response to anti-PD-1 blockade, was observed in Group1 compared to Group2 (p= 1.4e-7) and Group3 (p= 0.0054). Radiomic features, including volumetric, morphologic, histogram, and texture descriptors, were extracted from the segmented tumor regions on multiparametric MRI (mpMRI) scans of 71 (of 197) patients. Multivariate ML models were trained to predict the three immunological groups based on radiomic features using cross-validated random forest classifier along with recursive feature elimination, which yielded AUC of 0.72 for this multi-class classification problem. Our findings indicate the presence of distinct immunological groups in pLGG tumors, with possibly more favorable response to immunotherapies in Group1 tumors. Furthermore, we developed radioimmunomic signatures based on pre-operative conventional mpMRI that can potentially stratify the patients based on their immune tumor microenvironment. Based on these initial promising results, we are exploring additional features to increase the accuracy of radioimmunomics model.

We construct a $p$-group $P$ of nilpotency class two, rank seven and exponent $p$, such that $\text{Aut}(P)$ induces $N_{\text{GL}(7,p)}(G_{2}(p))=Z(\text{GL}(7,p))G_{2}(p)$ on the Frattini quotient $P/\unicode[STIX]{x1D6F7}(P)$. The constructed group $P$ is the smallest $p$-group with these properties, having order $p^{14}$, and when $p=3$ our construction gives two nonisomorphic $p$-groups. To show that $P$ satisfies the specified properties, we study the action of $G_{2}(q)$ on the octonion algebra over $\mathbb{F}_{q}$, for each power $q$ of $p$, and explore the reducibility of the exterior square of each irreducible seven-dimensional $\mathbb{F}_{q}[G_{2}(q)]$-module.
