In this article, which is an edited version of my inaugural lecture at Te Herenga Waka – Victoria University of Wellington, I argue that the concept of sovereignty is crucial to understanding one of the main questions of constitutional theory: how can constitutions facilitate self-government and, at the same time, function as mechanisms for the limitation of political power? I do so by re-examining four different ways in which I have encountered the concept of sovereignty through my academic work.

ABSTRACTStars formed in clusters can encounter other stars at close distances. In typical open clusters in the Solar neighbourhood containing hundreds or thousands of member stars, 10–20 per cent of Solar-mass member stars are expected to encounter another star at distances closer than 100 au. These close encounters strongly perturb the planetary systems, directly causing ejection of planets or their capture by the intruding star, as well as exciting the orbits. Using extensive N-body simulations, we study such fly-by encounters between two Solar system analogues, each with four giant planets from Jupiter to Neptune. We quantify the rates of loss and capture immediately after the encounter, e.g. the Neptune analogue is lost in one in four encounters within 100 au, and captured by the flying-by star in 1 in 12 encounters. We then perform long-term (up to 1 Gyr) simulations investigating the ensuing post-encounter evolution. We show that large numbers of planets are removed from systems due to planet–planet interactions and that captured planets further enhance the system instability. While encounters can initially leave a planetary system containing more planets by inserting additional ones, the long-term instability causes a net reduction in planet number. A captured planet ends up on a retrograde orbit in half of the runs in which it survives for 1Gyr; also, a planet bound to its original host star but flipped during the encounter may survive. Thus, encounters between planetary systems are a channel to create counter-rotating planets, This would happen in around 1 per cent of systems, and such planets are potentially detectable through astrometry or direct imaging.

Background: In the U.S., 20% of medical hospital discharges are readmitted within 30 days. Non-medical intervention strategies, such as teaching four specific self-management skills, have demonstrated reduction in readmission risk by a third or more. Objective: To investigate whether hospitalized patients are taught or learn four basic self-management skills (“four skills”) during routine clinical encounters.Methods: Design: Observational study from May to October 2012 in an academic teaching hospital. Participants: Consenting medical patients (aged 18 or over), their visitors or caregivers and their providers. Unit managers identified patients who would be discharged home and capable of learning the four skills. Interventions: A trained observer consented patients and monitored every provider encounter for mentioning and/or teaching of the four specific self-management skills’ content for intervals of up to 6 hours during daytime shifts. After each encounter, the observer queried the patient and their caregiver to see what information relating to the four skills they retained and understood. Additionally, observers recorded patient demographics, native language, provider type, and major medical conditions present at the time of the encounter. Five control activities expected to occur routinely for each encounter were monitored for comparison. Main measures: Frequency of one of four self-management skill education events during patient-provider encounters.Results: We observed 56 patients over 326 encounters involving 117 physicians or medical students, 134 nurses or nursing students, and 163 hospital staff. Among 189 encounters with clinical staff (physicians or nurses), the four basic self-management skills were mentioned in 54 encounters (28.6%) but taught only in 12 encounters (6.3%). The comparison with control behaviors show a much higher proportion (hand washing 35%-41.3%, identified role 29.5%, asking for more question 19.1%). Physicians are more likely than nurses to discuss clinical conditions, medications, or any of the four skills’ topics. Specific self-management teaching of patients led to better understanding of the these skills compared to just mentioning them (p = .03).Conclusions: Hospital providers rarely teach four basic self-management skills as part of regular care. Patients can learn this content while hospitalized which could potentially reduce 30-day readmission risk.

Purpose – The purpose of this paper is to determine whether there were differences in clinical encounter time between patients who speak English and those who require an interpretation service in an ethnically diverse pediatric clinic. Design/methodology/approach – Encounter time with patients requiring interpretation was compared to encounter time with patients who spoke English. The sample consisted of 310 encounters at a pediatric orthopedic clinic where patients spoke over 18 primary languages. Data were analyzed using ANOVA to compare four types of encounters. Findings – Approximately 12 percent (n=38) required interpretation and encounters requiring interpretation took 30 percent (nine minutes) longer than those that did not, p < 0.01(25 vs 16 minutes). Furthermore, this difference was mainly among new patients: Approximately, 53 percent increase in time for new patient encounters requiring interpretation (36 vs 23 minutes) while only 25 percent increase in encounter time for established patients (20 vs 16 minutes) was detected. Research limitations/implications – Preventing problems due to language barriers requires time for interpretation which places demands on staff resources and presents clinical challenges. However, long-term benefits of quality health care outweigh the costs associated with interpretation service. Originality/value – To the knowledge, this is the first study to investigate actual encounter time differences in a pediatric clinical setting. The authors found that clinical encounters requiring interpretation took approximately nine minutes longer in general and four minutes longer for established patients. These findings could give much needed information for hospital administrators to allocate appropriate amounts of time and resources to care for those who need interpretation services. However, they also indicate a broader concern of the reduction of clinical encounter time for overall health care system in the country that might need further investigation.

BackgroundEmergency department (ED) physicians and nurses frequently interact with emotionally evocative patients, which can impact clinical decision-making and behaviour. This study introduces well-established methods from social psychology to investigate ED providers’ reported emotional experiences and engagement in their own recent patient encounters, as well as perceived effects of emotion on patient care.MethodsNinety-four experienced ED providers (50 physicians and 44 nurses) vividly recalled and wrote about three recent patient encounters (qualitative data): one that elicited anger/frustration/irritation (angry encounter), one that elicited happiness/satisfaction/appreciation (positive encounter), and one with a patient with a mental health condition (mental health encounter). Providers rated their emotions and engagement in each encounter (quantitative data), and reported their perception of whether and how their emotions impacted their clinical decision-making and behaviour (qualitative data).ResultsProviders generated 282 encounter descriptions. Emotions reported in angry and mental health encounters were remarkably similar, highly negative, and associated with reports of low provider engagement compared with positive encounters. Providers reported their emotions influenced their clinical decision-making and behaviour most frequently in angry encounters, followed by mental health and then positive encounters. Emotions in angry and mental health encounters were associated with increased perceptions of patient safety risks; emotions in positive encounters were associated with perceptions of higher quality care.ConclusionsPositive and negative emotions can influence clinical decision-making and impact patient safety. Findings underscore the need for (1) education and training initiatives to promote awareness of emotional influences and to consider strategies for managing these influences, and (2) a comprehensive research agenda to facilitate discovery of evidence-based interventions to mitigate emotion-induced patient safety risks. The current work lays the foundation for testing novel interventions.

I remember hesitating at the door and then joining my four partners at Mr. C.'s side, trepidation giving way to curiosity, the moment imprinted forever in my memory by the smell of formalin.

Tourists are in contact with two types of services at destinations: enterprise services offered by tourism enterprises (e.g., hotels, shops, restaurants, etc.) and public services (public transportation, public information, public safety, etc.) provided by the local government. Following Churchill’s (1979) procedures, a model of destination service encounters (DSEs), including four dimensions (i.e.,enterprise personal interaction encounters (EPIEs), enterprise physical environment encounters (EPEEs), public personal interaction encounters (PPIEs), public physical environment encounters (PPEEs), and 10 subdimensions, was constructed. Then, the effects of DSEs on tourist satisfaction (TS) were tested with data collected in Shaoshan Township, China. The results revealed that EPIEs, PPIEs, and PPEEs had significant positive effects on TS. This model promotes the application of service encounter (SE) theory in destination management, and it offers implications for the synergy management of public and private sectors at destinations to improve tourist experiences.

Objective To investigate the feasibility of pre- and post-encounter patient-reported outcome (PRO) measure collection within an outpatient integrative health and medicine (IHM) clinic and to characterize factors associated with successful completion. Methods We conducted a retrospective review of 27,464 outpatient IHM encounters including 9,520 chiropractic; 8,237 acupuncture; 5,847 massage; 2,345 IHM consultation; and 1,515 osteopathic manipulation treatment encounters at four clinics offering IHM over 18 months. Patients were asked to complete paper questionnaires rating pain, anxiety, and stress from 0–10 immediately pre- and post-encounter. Generalized linear mixed effect regression models were used to examine the relationship between demographic, clinical, and operational covariates and completing (1) pre-encounter and (2) paired (i.e., pre and post) PROs. Results Patients (N = 5587, mean age 49 years, 74% white, 77% female) generally presented for musculoskeletal conditions (81.7%), with a chief complaint of pain (55.1%). 21,852 (79.6%) encounters were among patients who completed pre-encounter PROs; 11,709/21,852 (53.6%) completed subsequent post-encounter PROs. Odds of PRO completion were more impacted by provider, operational, and clinical-level factors than patient factors. Covariates associated with increased odds of pre-encounter PRO completion included being female, having additional IHM encounters, and having a pain or anxiety complaint. Covariates associated with increased odds of paired PRO completion included being aged 31–40 vs. 51–60 years and having additional IHM encounters. Conclusion Implementing a paper-based PRO collection system in outpatient IHM is feasible; however, collecting post-encounter PROs was challenging. Future endeavors should leverage the electronic health record and patient portals to optimize PRO collection and engage patients and clinical providers.

This article explores the multiple and multifarious encounters of and with Globe, a 1-metre-diameter copper spherical sculpture hosting four cameras that has been rolled by the artist Janetka Platun and others in London, Shrewsbury and Delhi. Situating Globe in relation to Janetka’s art practice and the wider ‘art of rolling’, and extending broader debates about globality, encounter and relational aesthetics, the article argues that Globe’s journeys generated ‘meaningful content’ beyond an aesthetic moment of interaction by inspiring people to share stories, ideas and reflections on home, migration and belonging through their encounters with her. Globe’s encounters were inspired by curiosity, often sparked by her materiality, mobility and ‘globe-ness’. Rather than merely act as a prompt for people to reflect on home, migration and belonging, Globe has also been marked by her own journeys and encounters, reflecting their unpredictable and often transformative nature.

Abstract A real-time model of a wake vortex has been developed for an engineering flight simulator, which enables studies to be undertaken into the effects of wake vortex encounters with civil transport aircraft. A brief summary of the methods used to create a vortex model and integrate it with a real-time flight simulator is presented. Tools have been developed to determine the structural loads on an aircraft during a simulated wake vortex encounter and to visualise the airframe loads. Four wake vortex encounters were flown with different pilot recovery strategies and automatic flight controls. It is clear from analysis of these results that it is difficult for a pilot to appreciate the margin of tail loading during an encounter and a prototype training display is outlined to enable pilots to fully understand the effects of rudder inputs in recovery from these events.

Most reptiles exhibit no parental care and aggressive behaviour towards heterospecific predators has rarely been recorded in the natural environment. Several species of the subfamily Egerniinae are amongst the most highly social of all squamate reptiles, exhibiting stable social aggregations and high levels of long-term social and genetic monogamy. We have examined Cunningham’s skinks, Egernia cunninghami, over a three-year period during late January and early February (total 32 days) in the alpine region of New South Wales using video and thermal imaging. Four birthing sessions were witnessed during our field studies of social aggregations of skinks. Our observations monitored skink encounters, in the presence of offspring, with an eastern brown snake, Pseudonaja textilis (two separate encounters, one recorded by video/imaging) and 12 encounters with the Australian magpie, Gymnorhina tibicen. All events were associated with aggressive chasing and/or attack by adult skinks. The first snake encounter involved the active targeting of a recently born juvenile with the mother of the juvenile attacking the snake (running towards the snake, biting and remaining attached for several seconds). The second encounter (the following year) comprised two adult skinks attacking and biting a snake, Pseudonaja textilis. All magpie encounters resulted in chases by adult skinks.

Against the backdrop of the Bauhaus centenary in 2019, Encounters with Southeast Asian Modernism examined the history, significance, and future of postcolonial modernism in this region, with partners in four cities – Jakarta, Phnom Penh, Singapore, and Yangon. The project provided a historical perspective on the societal and political upheaval that accompanied the transition to independence after the colonial period in these countries. It also showcased current initiatives in the fields of art, architecture, and science that are committed to the preservation and use of Modernist buildings. In 2020, the project will continue with an exhibition and accompanying program in Berlin.

AbstractThis a contribution to the cultural semiotics of African cultural encounters seen through the eyes of Swedish naturalists at the end of the eighteenth century. European travellers faced severe problems in understanding the alien African cultures they encountered; they even had difficulty understanding the other culture as a culture. They were not just other cultures that they could relate to, but often something completely different, belonging to the natural history of the human species. The Khoikhoi and other groups were believed by Europeans to be, from their perspective, the most distant culture. The Linnaean disciple Anders Sparrman and others, however, tried to transcend this cultural gap, and used their cognitive resources, such as empathy and intersubjectivity, in order to understand the alien culture they encountered.The aim of this paper is to unearth the cultural semiosis of African encounters and the intersubjective challenges that human interactions provoke. These encounters not only changed the view the travellers had of the Other, but also changed themselves and their self-perception. The encounter between the Ego and the Other is, however, not static, something predestined by the differences in their cultures, but dynamic, changing according to individual encounters and the actual intersubjective interplay that transform and change the perception of the Other. There are in particular four meaning-making processes and challenges within cultural encounters that are in focus: recognizing cultural complexity; invoking intersubjectivity; determining similarities and dissimilarities; and identifying the Other as a mirror of oneself.The triad of cultures – Ego, Alter, and Alius – can be understood as gradual and changing aspects depending on the actual situation of the encounter and the personal perspectives, interpretations, and behaviour of the thinking subjects involved. Using concrete examples from Southern and Western Africa in the 1770s and 1780s, this study aims to explore this dynamic semiosis. One of the conclusions is that the relation between the Ego and the Alter/Alius is not something only predetermined by the cultures involved and their ideologies, but also depends on the individual thinking subjects and how they use their specific cognitive and semiotic resources, not least their intersubjective abilities, within specific temporal and spatial contexts.

Introduction: Patients with incurable cancer have poor prognostic awareness. We present a detailed analysis of the dialogue between oncologists and patients in conversations with prognostic implications. Methods: A total of 128 audio-recorded encounters from a large multisite trial were obtained, and 64 involved scan results. We used conversation analysis, a qualitative method for studying human interaction, to analyze typical patterns and conversational devices. Results: Four components consistently occurred in sequential order: symptom-talk, scan-talk, treatment-talk, and logistic-talk. Six of the encounters (19%) were identified as good news, 15 (45%) as stable news, and 12 (36%) as bad news. The visit duration varied by the type of news: good, 15 minutes (07:00-29:00); stable, 17 minutes (07:00-41:00); and bad, 20 minutes (07:00-28:00). Conversational devices were common, appearing in half of recordings. Treatment-talk occupied 50% of bad-news encounters, 31% of good-news encounters, and 19% of stable-news encounters. Scan-talk occupied less than 10% of all conversations. There were only four instances of frank prognosis discussion. Conclusion: Oncologists and patients are complicit in constructing the typical encounter. Oncologists spend little time discussing scan results and the prognostic implications in favor of treatment-related talk. Conversational devices routinely help transition from scan-talk to detailed discussions about treatment options. We observed an opportunity to create prognosis-talk after scan-talk with a new conversational device, the question “Would you like to talk about what this means?” as the oncologist seeks permission to disclose prognostic information while ceding control to the patient.

Interactions between humans and animals benefit human health and well-being. There has been little study on the effects of bird encounters on people. This study addresses this conceptual gap by analyzing the results of a between-subjects experiment with 136 undergraduate students who were randomly assigned to experience one of four 360 stereo panorama virtual environments representing four encounters, namely "bird depicted in images," "Birds in a Cage," "Watching Birds in Nature inside a path," and "Birds in nature." Following their encounter in a virtual environment, participants assessed their experience in terms of spatial awareness, emotions, psychological well-being, and connection to nature through the use of an online survey. Standard descriptive statistics, correlation, Kruskal–Wallis, and post hoc Bonferroni analysis are used in data analysis. The findings suggested that experiences with birds in open surroundings were more likely to affect participants' spatial perception, emotions, psychological health potentials, and connection to nature than encounters with birds in confined places. This study is critical for environmental awareness since maintaining biodiversity and wildlife is inextricably linked to the potential well-being and quality of life of the human population who are part of the ecosystem.

Purpose – The purpose of this paper is to investigate the role of serendipity in legal information seeking behavior of family law advocates, whom act in a challenging information environment that lacks published court rulings. Design/methodology/approach – A quantitative research using a web-based structured questionnaire, among Israeli family law advocates. Single stage systematic sampling, with random starting point and no recurring pattern of each sixth family law advocate on the Israel Bar Advocates List, was applied. Data from 135 Israeli family law advocates were used for analysis. Findings – Electronic information sources were found as most serendipitous; family law advocates were identified as super encounterers; four types of professional background concerns and seven legal professional contributions of the unexpected encounters with court rulings, were identified. Furthermore, findings support several frameworks presented on earlier information encounter literature. Research limitations/implications – Data absence on demographic and professional variables distributions of Israeli family law advocates was a limiting factor, compensated by the systematic sampling method used, thus can be regarded to reflect the views of the entire study population. Surveys’ reliance on self-reporting recalls of serendipitous events is also a limiting factor, though predicted and acceptable in this matter since chance encounters occur unexpectedly and are complex to capture. Practical implications – Chance encounters may expose lawyers to meaningful information it is unlikely they were able to find because its limited publication, and assist them keep up with current law for better serves their clients. Originality/value – The study augments the current empirically based knowledge on serendipity and provides insights into legal information chance encounters among a little-studied group of knowledge workers: family law advocates.

Abstract Background Nosocomial infections pose significant risk to patients and hygienic standards may be easily overlooked, especially in fast-paced emergency department settings. Studies have shown that stethoscopes can harbor pathogens, which can be transferred to patients when proper sanitary measures are not taken. Survey-based studies have mostly assessed stethoscope hygiene, but they may not accurately represent cleaning practice. This study aimed to accurately assess cleaning practice through observation of stethoscope cleaning and hand hygiene among medical providers in an emergency department setting. Methods Four hundred twenty-six provider–patient encounters were observed in the emergency department of the VA San Diego Healthcare System. The frequency and methods of stethoscope and hand hygiene practices were anonymously observed and recorded. Stethoscope hygiene was recorded during and after each encounter if cleaning took place for at least 15 seconds. Hand hygiene data were also gathered before and after each encounter. Data analysis was performed to determine the frequency of these practices. Results Of 426 encounters, 115 involved the use of a personal stethoscope. In 15 of 115 encounters (13.0%), the provider placed a glove over the stethoscope before patient contact. Following patient interaction, 13 of 115 encounters (11.3%) involved stethoscope hygiene with an alcohol swab. Stethoscope hygiene with water and hand towel before patient interaction was observed in 5 of 115 encounters (4.3%). Hand sanitizer use or hand washing was observed in 213 of 426 encounters (50.0%) before patient interaction. Gloves were also used prior to patient interaction in 206 of 426 (48.4%) encounters, some overlapping with those who used hand sanitizer or washed their hands. Hand sanitizer or hand washing was used in 332 of 426 encounters (77.9%) after patient interaction. Conclusion Only 11.3% of encounters involving the use of a personal stethoscope had stethoscope sanitation with an alcohol swab. In 4.3% post-encounters, there was an attempt to clean stethoscopes with water and towel. Rates of appropriate hand hygiene were also lower than expected. Stethoscope and hand hygiene standards need to be raised to reduce infection risk and further studies need to be conducted to set guidelines for adequate stethoscope sanitation. Purpose The purpose of this paper is to develop and test a series of hypotheses regarding the use of procedurally just policing during suspect encounters. Design/methodology/approach Systematic social observation data from police encounters with suspects are used (N=939). Ordinary least-squares regression models are estimated to evaluate the effects of four variable clusters (i.e. suspect self-presentation, situational factors, suspect social characteristics, and officer characteristics) on procedurally just policing practices. Findings Results from the regression models show that the most salient predictors of police officers exercising authority in a procedurally just manner include the level of self-control displayed by suspects, the number of citizen onlookers, whether the encounter involved a traffic problem, the race/ethnicity of suspects, and suspects’ social status. Research limitations/implications This study focused only on police-suspects encounters where compliance requests were made. While the size of the sample is relatively large, the results from this study do not generalize to all types of police encounters with members of the public. Originality/value This research adds to an emerging body of research focused on predicting procedurally just practices in police encounters. The findings support increased attention to theories that explain police-citizens interactions, and also indicate that further consideration to the measurement of police behavior is warranted.

Abstract A key element of wilderness management planning is prescribing standards for acceptable levels of impact or change to wilderness conditions based on input from the public. Specifically, to obtain standards for social impacts, wilderness users are surveyed to determine acceptable levels of encounters with other parties. Using a variance components model and generalizability analysis, this study examines four sources of variation in user-based social impact standards: occasion (on-site vs. mailback surveys), wilderness area, encounter context (type and location of encounter), and respondent. Data come from a survey of visitors to four wilderness areas. Results indicate that social impact standards are generalizable across wilderness areas and to a lesser degree across measurement occasions. As expected, social impact standards vary depending on the context of contact. Contrary to normative theory in sociology, standards are also highly variable across respondents. These results suggest that respondents appear to share a high level of sensitivity to encounters, but that the task of assigning a numerical standard may be too abstract or hypothetical to be meaningful. Results from numerical integrations of random binary-binary encounters have been used to obtain various cross-sections and outcome distributions for the four-body scattering. The initial orbital elements were chosen randomly except the Kepler-energies for which various selected values were used. Rough estimates for mass effects were obtained by simulating encounters of binaries with unequal component masses.We developed a semi-analytical theory for obtaining the types and energies of the outcome configurations. The theory contains some adjustable parameters, the values of which we deduced by comparing the theory and experiments.The energy transfer rate by collisions (=outcome is not two binaries) dominates over that due to fly-by's by an order of magnitude, provided that the binaries are hard. The formation of a hierarchical three-body system is fairly common. In a collision of energetically similar very hard binaries the probability is about 20 percent, while it is greater than 50 percent if the binding energies differ by a factor of more than four.

This study explores the self-reported perception of moods, emotions, and engagement of Brazilian undergraduate students enrolled in a music ensemble course. A mixed method approach was used through a convergent design. Sixteen Brazilian undergraduate students enrolled in a music ensemble course participated in this research. The Present Mood States List (PMSL) was used as a quantitative data collection instrument before and after 15 encounters (14 classes and a final concert). Analysis of quantitative data showed that 15 differences in the mood state values were statistically significant when considering all encounters and 22 differences were statistically significant in specific encounters. The qualitative data collection procedures were (1) the application of a questionnaire on emotions and musical engagement after each encounter and (2) a semi-structured group interview carried out after the fourteenth class. The qualitative data were grouped into four categories: didactics, music repertoire, moods, and data collection. According to the results, participants presented mostly positive mood states and emotions both before and after the encounters. In general, the students’ mood states and emotions seemed to influence their musical engagement, and, at the same time, their sense of musical engagement seemed to influence the presence of positive experiences.

Objective: To estimate how reliably and validly can medical students encode reasons for encounter and diagnoses using the International Classification of Primary Care, revised 2nd edition (ICPC-2-R). Methods: For every encounter they supervised during an entire semester, three family and community physician teachers entered the reasons for encounter and diagnoses in free text into a form. Two of four medical students and one teacher encoded each reason for encounter or diagnosis using the ICPC-2-R. In the beginning of the study, two three-hour workshops were held, until the teachers were confident the students were ready for the encoding. After all the reasons for encounter and the diagnoses had been independently encoded, the seven encoders resolved the definitive codes by consensus. We defined reliability as agreement between students and validity as their agreement with the definitive codes, and used Gwet’s AC1 to estimate this agreement. Results: After exclusion of encounters encoded before the last workshop, the sample consisted of 149 consecutive encounters, comprising 262 reasons for encounter and 226 diagnoses. The encoding had moderate to substantial reliability (AC1, 0.805; 95% CI, 0.767–0.843) and substantial validity (AC1, 0.864; 95% CI, 0.833–0.891). Conclusion: Medical students can encode reasons for encounter and diagnoses with the ICPC-2-R if they are adequately trained.

Background Internet sex-seeking has been associated at the person level with sexual risk. However, the most robust method of encounter-level inference to determine associations between internet sex-seeking and sexual risk is to compare encounters against each other. We systematically reviewed within-subjects comparisons of sexual encounters that tested associations between internet sex-seeking and sexual risk in men who have sex with men. Methods: We systematically searched databases on 9 July 2013, then screened records and full-text articles in duplicate and independently. Studies were synthesised narratively. Results: Four studies were included. Although studies were generally of high quality, the findings were inconsistent and did not show clear evidence of a relationship between internet sex-seeking and sexual risk. Conclusions: Further research in internet sex-seeking among men who have sex with men is required, particularly as internet-enabled sexual sociality continues to evolve. Internet-based health promotion may wish to target person-level features instead of encounter-specific characteristics.

Aims. The purpose of this study was to describe the challenge of cross-cultural care encounters from perspective of imported nurses in Lhasa, Tibet, as well as investigate the relationship of cross-cultural care encounters and its influencing factors. Methods. A cross-sectional survey was designed among 300 imported nurses and 255 patients selected from four comprehensive hospitals (including two Grade III Class A hospitals and two Grade III Class B hospitals) in Lhasa. The average number, standard deviations, constituent ratios, T-tests, rank-sum tests, one-way ANOVAs, multiple stepwise regression analyses, and Pearson correlation analysis were used to analyze cross-cultural care encounters and its influencing factors. P<0.05 was considered statistically significant. Results. The cross-cultural care encounter of nurses was 61.73±11.86, mainly relating to age, technical titles, Tibetan language ability, and participation in humanistic training. Age, gender, educational level, technical titles, Tibetan language ability, years working in Tibet, and participation in language and humanities training were the influencing factors (P<0.05). The average total score of culturally competent care of imported nurses in Lhasa was 218±31.09. Cross-cultural care encounters of nurses were positively correlated with culturally competent care (r=0.126, P<0.01) and the needs of patients’ cultural care (r=0.183). Conclusion. The scores of culturally competent care and cross-cultural care encounter of imported nurses were at a high level, and their culturally competent care was in the second stage of “conscious and incapable” status. The cross-cultural care encounter of nurses is positively related to culturally competent care and the needs of patients’ cultural care. Abilities of language communication, understanding of Tibetan culture, and enhancement of the cultural ability needed optimization.

PurposeDespite recognition of the importance of emotions and emotion regulation in service encounters, emotion regulation has been generally studied from an employee perspective. This study investigated customer emotion regulation behaviours (CEREBs) in face-to-face service encounters; arguing for a more nuanced approach through an emotion regulation matrix representing the playing up and downplaying of positive or negative emotions. Motivational factors and service-related situational conditions that influence the likelihood of emotion regulation were also examined.Design/methodology/approachFour focus groups and the critical incident technique method were used to obtain data from people who had interacted with service employees within the previous six months.FindingsThere was support for emotion regulation in the four facets of the emotion regulation matrix. Five CEREB dimensions, including verbal behaviours and facial expressions, were evident. Motivational factors and situational conditions that impacted on customer emotion regulation in service encounters were also identified.Research limitations/implicationsThe findings were based on two qualitative methods. A quantitative approach should be used to further validate the suggested framework.Originality/valueMost research on emotion regulation has focused on employees. We examined the phenomenon from a customer viewpoint and in a service encounter context. As customers are not bound by employment rules and conventions, a wider range of emotion regulation behaviours were found. The study used the four-faceted emotion regulation matrix to investigate this, developing a conceptual framework that provides a foundation for future research.

Abstract: This article revisits four well-known accounts of the first European encounters with Amerindians in the Amazon. The sporadic character of these encounters make the impact on Amerindian societies irregular and uneven. My analysis is directed to the present condition as encountered, especially the variety of contacts. This approach obliges the text be treated as a whole, rather than being read selectively. Maintaining the integrity of the text allows us to see the different kinds of relations in their contexts. My intention is to use these reports to search for the bridges across cultural separations. Each drew the other towards them, in their own ways. These steps opened the way for the “refounding” of indigenous riverine societies in the seventeenth century.

This field study examined competition for space among encrusting bryozoans on artificial panel substrata used to simulate the habitat found on the undersides of boulders. In all 1761 interactions were analysed, involving 16 species of cheilostomes (genera Callopora, Celleporella, Cribrilina, Electra, Escharella, Escharoides, Haplopoma, Membraniporella, Microporella, Phaeostachys, Schizomavella, Schizoporella and Umbonula) and two ctenostome genera (Alcyonidium and Flustrellidra). Neither absolutely transitive nor intransitive arrangements of competitive outcome occurred among the species. Instead, the high incidence of ‘ties’ and reversals of outcome produced a network-like arrangement. In terms of outcome alone Escharoides coccinea (Abildgaard), Membraniporella nitida (Johnston) and Schizoporella unicornis (Johnston in Wood) were apparently ‘overgrowth dominants’, Alcyonidium spp. and Cribrilina cryptooecium Norman were ‘intermediate dominants’ and Callopora lineata (L.), Callopora craticula (Alder), Celleporella hyalina (L.) and Electra pilosa (L.) were ‘inferior competitors’ for space. The effects of four variables on the outcome of specific competitive encounters were assessed in detail and included (i) the identity of the colony encountered, (ii) the angle of contact between the competing colonies, (iii) the location (two sites, on both the east and west coasts of Scotland) and (iv) the year (1983, 1984). Models incorporating these variables were developed, using the Generalized Linear Interactive Modelling System (GLIM), to examine their influence on the outcome of bryozoan encounters. All of these factors contributed to explaining the considerable variability in the outcome of competitive interactions. The species were not, however, affected equally by all the variables, although encounter angle and year were significant for dominants and encounter angle, site and year all were significant for intermediate species.

ObjectivesTo explore children’s foot, ankle and leg consultation patterns and management practices in Australian primary care.DesignCross-sectional, retrospective study.SettingAustralia Bettering the Evaluation and Care of Health program dataset.ParticipantsData were extracted for general practitioners (GPs) and patients <18 years from April 2000 to March 2016 inclusive.Main outcome measuresDemographic characteristics: sex, GP age groups (ie, <45, 45–54, 55+ years), GP country of training, patient age grouping (0–4, 5–9, 10–14, 15–18 years), postcode, concession card status, indigenous status, up to three patient encounter reasons, up to four encounter problems/diagnoses and the clinical management actioned by the GP.ResultsChildren’s foot, ankle or leg problems were managed at a rate of 2.05 (95% CI 1.99 to 2.11) per 100 encounters during 229 137 GP encounters with children. There was a significant increase in the rate of foot, ankle and leg problems managed per 100 children in the population, from 6.1 (95% CI 5.3 to 6.8) in 2005–2006 to 9.0 (95% CI 7.9 to 10.1) in 2015–2016. Management of children’s foot, ankle and leg problems were independently associated with male patients (30% more than female), older children (15–18 years were 7.1 times more than <1 years), male GPs (13% more) and younger GPs (<45 years of age 13% more than 55+). The top four most frequently managed problems were injuries (755.9 per 100 000 encounters), infections (458.2), dermatological conditions (299.4) and unspecified pain (176.3). The most frequently managed problems differed according to age grouping.ConclusionsChildren commonly present to GPs for foot, ankle and leg problems. Presentation frequencies varied according to age. Unexpectedly, conditions presenting commonly in adults, but rarely in children, were also frequently recorded. Wet–dry encounters between basins and regions have an important impact on the allocation of water resources. This study proposes a multi-objective allocation model for basin water resources under full probability scenarios considering wet–dry encounters (FPS-MOWAM) to solve the problem of basin water resource allocation. In the FPS-MOWAM model, the sub-regions were merged by precipitation correlation analysis. Next, the joint probability distribution of basin runoff and region precipitation was constructed using copula functions. The possible wet–dry encounter scenarios and their probabilities were then acquired. Finally, the multi-objective allocation model of water resources was constructed using the full probability scenario for wet–dry encounters in each region. The FPS-MOWAM is calculated by the NSGA-II algorithm and the optimal water resource allocation scheme was selected using the fuzzy comprehensive evaluation method. Using the Yellow River Basin as an example, the following conclusions were obtained: (1) the Yellow River Basin can be divided into four sub-regions based on precipitation correlations: Qh-Sc (Qinghai, Sichuan), Sg-Nx-Nmg (Gansu, Ningxia, Inner Mongolia), Sxq-Sxj (Shaanxi, Shanxi), and Hn-Sd (Henan, Shandong), (2) the inconsistencies in synchronous–asynchronous encounter probabilities in the Yellow River Basin were significant (the asynchronous probabilities were 0.763), whereas the asynchronous probabilities among the four regions were 0.632, 0.932, and 0.763 under the high, medium, and low flow conditions in the Yellow River Basin respectively, and (3) the allocation of water resources tends to increase with time, allocating the most during dry years. In 2035, the expected economic benefits are between 11,982.7 billion CNY and 12,499.6 billion CNY, while the expected water shortage rate is between 2.02% and 3.43%. In 2050, the expected economic benefits are between 21,291.4 billion CNY and 21,781.3 billion CNY, while the expected water shortage rate is between 1.28% and 6.05%.

Purpose Research has long focused on the notion of access and the trajectory towards a healthcare encounter but has neglected what happens to patients after these initial encounters. This paper focuses attention on what happens after an initial healthcare encounter leading to a more nuanced understanding of how patients from a diverse range of backgrounds make sense of medical advice, how they mix this knowledge with other forms of information and how they make decisions about what to do next. Design/methodology/approach Drawing on 160 in-depth interviews across four European countries the paper problematizes the notion of access; expands the definition of “decision partners”; and reframes the medical encounter as a journey, where one encounter leads to and informs the next. Findings This approach reveals the significant unseen, unrecognised and unacknowledged work that patients undertake to solve their health concerns. Originality/value De-centring the professional from the healthcare encounter allows us to understand why patients take particular pathways to care and how resources might be more appropriately leveraged to support both patients and professionals along this journey.

AbstractObjectiveThis study evaluated how Tactical Emergency Casualty Care (TECC) training prepared law enforcement officers (LEOs) with the tools necessary to provide immediate, on-scene medical care to successfully stabilize victims of trauma.MethodsThis was a retrospective, de-identified study using a seven-item Fairfax County (Virginia USA) TECC After-Action Questionnaire and Arlington County (Virginia USA) police reports.ResultsForty-six encounters were collected from 2015 through 2016. Eighty-four percent (n=39) of the encounters were from TECC After-Action Questionnaires and 15% (n=7) were from police reports. The main injuries included 13% (n=6) arterial bleeds, 46% (n=21) mild/moderate bleeds, 37% (n=17) large wounds, 20% (n=9) penetrating chest wounds, and 13% (n=6) open abdominal wounds. One-hundred percent of officers reported success in stabilizing victim injuries. Seventy-four percent of officers (n=26) did not encounter problems caring for a patient while 26% (n=9) encountered a problem. Ninety-seven percent (n=37/38) answered Yes, the training was sufficient, and three percent (n=1) indicated it was OK.ConclusionThis is the most comprehensive study of TECC use among LEOs to date that supports the importance of TECC training for all LEOs in prehospital trauma care. Results of this study showed TECC training prepared LEOs with the operational tools necessary to provide immediate, on-scene medical care to successfully stabilize victims of trauma. Despite guidelines, many individuals are not routinely tested for HIV within healthcare settings. Our objective was to quantify and characterize preceding clinical encounters by newly-diagnosed persons living with HIV in southern Alberta, Canada. We discuss the clinical impact of missed HIV testing, and options for remediation. Clinical encounters prior to HIV diagnosis including the discharge diagnosis were collected between 1 April 2011 and 1 April 2016. We followed the HIV Indicator Diseases across Europe Study criteria to identify HIV Clinical Indicator Conditions (HCICs) present at clinical encounters. Patients accessing prior care were compared to those who had not previously accessed care. Of 393 individuals, 231 (58.7%) had ≥1 encounter prior to diagnosis; 224 (57%) of encounters occurred in outpatient clinics, 130 (33.1%) in emergency departments, and 39 (9.9%) in urgent care clinics. Approximately 25% (n = 57) of patients who engaged healthcare had ≥ 1 recognized HCIC but did not receive HIV testing. The most frequent HCICs were infection (n = 34; 60%) and hematological disorders (n = 12; 21%). The median CD4 cell count at HIV diagnosis for patients with an HCIC was 127 cells/mm3. In this population, three of five patients had accessed healthcare prior to diagnosis with one of four presenting with HCICs but were not offered HIV testing. Protocols beyond the current recommendations are urgently required to address missed HIV diagnostic opportunities who engaged healthcare.
