Abstract (sommario):
Improving farm productivity either through introduction of modern technologies or by increasing agricultural extension services is vital. Agricultural extension is the primary mechanism that increases agricultural production. Therefore, reviewing the role of agricultural extension on increasing farm productivity has been interesting issue. Cognizant of this fact, reviewing the role of agricultural extension in increasing farm productivity is an important issue for policy makers in order to improve productivity. Consequently, this seminar was aimed to provide information on the role of agricultural extension in increasing farm productivity and to review agricultural extension constraints in Ethiopia. The review shows that agricultural extension service had positive and significant role on increasing farm productivity by increasing the farmer’s knowhow on agronomic practices such as pest and disease control and adoption of improved seed varieties as well as soil and water conservation technologies. On the other hand, the review reveals that weak interaction; low participation, lack of technical skills, missed use of services, the weak link of research extension, a lack of incentives, and a lack of suitable adaptation to technologies constrain the extension system across the country. Thus, based on the review the following recommendations are forwarded. Extension services should be increase to farmers by the government agents especially by district agriculture development unit, and NGO’s to assist farmers to have easy access to extension so as to increase farm productivity. In addition, to increase the current working conditions of extension agents, including transportation, housing, and proper budget allocation, the government should adopt a very responsible and practical approach.

Abstract (sommario):
The Ethiopian poultry population (chicken) is projected to be around 56.06 million, with indigenous, hybrid, and foreign breeds accounting for 88.19 percent, 6.45 percent, and 5.36 percent of the total poultry, respectively. Ethiopian poultry farming methods are typically low-productivity subsistence systems. This paper will now collect data on chicken kinds used for meat and egg production, as well as their use, difficulties, and potential in Ethiopian poultry productivity. It also discusses Ethiopian poultry production methods, their contributions, and challenges such as feed shortages, predators, disease, veterinary service, health management, marketing, genetic quality (breed), and Extension service issues. However, information covers both the past and the present; production options include training and extension services, veterinary services, market access, financial services, and the requirement for limited space and inputs. As a result, certain actions must be taken immediately to alleviate these limits and boost the poultry’s production potential.

Abstract (sommario):
This scientific paper presents an in-depth analysis of the integration of mobile technology in agricultural extension services, illustrating its pivotal role in revolutionizing information dissemination and management in agriculture. It commences with a detailed overview of various mobile technologies such as smartphones, SMS, voice messaging, and mobile internet, and their profound impact on the agricultural sector. The study progresses to examine diverse modes of technology delivery, including direct communication methods like voice calls and SMS, multimedia approaches like videos and podcasts, and interactive services such as chatbots and forums. This is followed by an evaluation of the impact of these technologies on extension service delivery, focusing on their reach, speed, cost-effectiveness, and resource utilization, and how they compare to traditional extension methods. The paper provides empirical insights through case studies and real-world examples, including successful implementations like Kenya's M-Kilimo service and more challenging projects like Nigeria's AgroMobile, offering a nuanced understanding of the factors influencing success and failure. A comparative analysis is conducted to contextualize the use of mobile technology in agricultural extension services globally, considering different socio-economic and technological environments. The study concludes by exploring emerging trends and future innovations in mobile technology, such as artificial intelligence (AI) and the Internet of Things (IoT), and proposes strategic recommendations for enhancing its application in extension services. These recommendations include advancing digital literacy, tailoring technology to local contexts, and fostering supportive policy environments. This comprehensive examination underscores mobile technology's critical role in advancing agricultural practices, enhancing farmer engagement, and contributing to sustainable agricultural development.

Abstract (sommario):
A detail study on the experience of agricultural extension in Nepal was done by discussion with experts, academicians and involved agricultural officers of Nepal along with review of different documents, books and articles on the subject matter. Since from the first effort of extension service, Training and visit, Integrated Rural Development Approach, Tuki Approach, Farming System Research and Extension Approach, Block Production Program were the approaches used in the past. Conventional Educational Approach, Pocket Package Approach, Projectization Approach, Farmers Group Approach, Farmers Field School Approach, Partnership Approach are the approaches being followed presently in agricultural extension in Nepal. The extension efforts in the country are guided by the National Agricultural Extension Strategy. Department of agriculture under ministry of agricultural development is responsible for providing public extension service via District Agriculture Development Office (DADO), Agriculture Service Centre, Contact Centre, Model Agriculture Service Centre and Community Agriculture Service Centre at the farmers level. Farmers Group and cooperatives, International and National Nongovernmental organizations, Community Based Organizations and few private entities are providing the private extension services. major issues found in public extension systems are lack of motivation among the rural youths, farmers; natural resource degradation and climate change and sustainability issues; inadequate number of the extension workers and their qualification and skills; inadequate infrastructure and capacity for use of ICTs among the ground level extension workers; lack of monitoring and assessment of impact of extension activities in rural farmers; low level of need based extension coverage particularly for small farmers; ineffective and weak linkages between stakeholders at different levels; low level of education of farmers; insufficient budget and investment for extension activities; domination of supply driven approaches rather than demand driven; inadequate extension services in parts of value addition and market exposure.

Abstract (sommario):
The authors investigate the conceptualization and measurement of service quality and the relationships between service quality, consumer satisfaction, and purchase intentions. A literature review suggests that the current operationalization of service quality confounds satisfaction and attitude. Hence, the authors test (1) an alternative method of operationalizing perceived service quality and (2) the significance of the relationships between service quality, consumer satisfaction, and purchase intentions. The results suggest that (1) a performance-based measure of service quality may be an improved means of measuring the service quality construct, (2) service quality is an antecedent of consumer satisfaction, (3) consumer satisfaction has a significant effect on purchase intentions, and (4) service quality has less effect on purchase intentions than does consumer satisfaction. Implications for managers and future research are discussed.

Abstract (sommario):
The political changes and reformation initiated the agriculture extension devolution to a lower level in the 2000s in Nepal. The federal system devolved the agriculture extension to local government to bring changes in the delivery system, institutional role, and farmers' access to knowledge development and financial resources. The research has analysed the impact of the agriculture extension devolution to the local level in the last twenty years through a structured literature review. Specifically, it has been identified that the increase in government institutions at the local level allowed farmers to receive extension services from field-level institutions. However, lack of accountability towards farmers, limited human resources, and low coordination between government institutions have impacted service delivery. Furthermore, the study shows that the devolution of agriculture extension has provided the private sector, development partners, and field-level extension actors to support government institutions by sharing the role of the extension service provider.

Abstract (sommario):
The quality of rural extension and advisory services is a crucial element in fostering innovation and rural development. This article aims to clarify the concept of quality of rural extension and to develop a preliminary theoretical framework. An ample literature review was conducted in search of articles on service quality and quality of rural extension and advisory services. The first part presents the main results of the literature searchon quality of extension services. The definition of quality is not universal. Quality cannot be conceptualized only as farmers’ satisfaction or as extension results. It has different dimensions or components and stakeholders have different points of view about it. The second part of this article discusses the definition of service quality and the concept of Total Quality Management and underlines that the concept of quality variesaccording to industry types or contexts and is the result of complex negotiation among different stakeholders. Finally, a comprehensive theoretical framework for addressing quality of rural extension and advisory services is presented that differentiates among enablers that limit or facilitate the delivery of quality rural extension and advisory services, the production and delivery processes, and results obtained. Here, the key roleplayed by quality self-assessment and organizational learning is highlighted.

The paper focused on a review of COVID-19 pandemic, its implication on the Nigeria agriculture and the role of extension. The purpose was to review the impact of the pandemic on the Nigeria agriculture and narrate the implication for agricultural extension service. Being a review, data was drawn from verified websites, research papers, journals, statistical bulletins and other online materials. The review uncovered a subtle but undeniable negative impact on all the value chain of the Nigeria agriculture. Also, with enough evidence, there is likelihood of possible food catastrophe if concerted action is not taken to save the sector. One of the most important factors that may avert this negative impact is agricultural extension service. But in this time of global lockdown, travel ban and social distancing, the way out becomes technology. Thus, from the review, it was concluded that government and institutions at all levels should intentionally deploy technology tools to aid effective agricultural extension service transmission to farmers.

AbstractA very important task to improve the sustainability of existing structures is to extend their service life especially in terms of resource conservation and sustainability. Therefore, the assessment and the required safety level of an existing structure are of particular importance (condition monitoring). This contribution focusses on how data from established inspection and monitoring approaches are essential and examines the more advanced, additional approaches currently under review. Both the load‐bearing capacity and the serviceability must be guaranteed over a certain period of time at or above the required safety level. The serviceability of a structure can be measured by performance indicators. It is examined how the definition and measurement of performance indicators in conjunction with a well‐designed life cycle management action can be used to extend the service life while ensuring safety levels.

Abstract (sommario):
Study was conducted to explore the role of extension advisory services in relation to smart agriculture to mitigate the effect of climate. Agriculture is a very prominent sector of the Indian economy accounts for (49%) of the employment and one-sixth of the GDP. The agricultural industry is supported by 500 million small holder farms. Agriculture is extremely vulnerable for climate change and water as well. Climate change affects agriculture in the Asia and Pacific region particularly in those areas that are vulnerable to natural disasters. Higher temperature reduce yield of desirable crops while encouraging weeds and pests proliferation. Changes in precipitation pattern and floods affect agriculture causing crop failures. In India, significant negative impacts occurred due to climate change reasoned to reducing the yields. In addition to advisory services advanced information on likelihood of weather help to carry out to proper management of resources for agricultural operations to minimize the risk and facilitate growth and realizes the optimum crop yield. Further, decentralization of extension services tracing the different state or district-level funding is needing exercise not always known at national level in various states or provinces spending on extension service. It plays a prominent role by creating awareness regarding adaptation of best practices regarding climate change, deforestation and planting of trees also potential to transform the vision of India’s famers about food security, poverty alleviation and sudden climatic threats. Thus, the advisory services help taking the advantage of benevolent weather and minimize adverse impact of malevolent weather.

Abstract (sommario):
Abstract: The effects of COVID-19 have tested and crushed the earlier achievement in sustaining the agricultural and extension delivery system globally. COVID-19 has pressed a reset panel on the extension services more especially among the smallholder farmers who are already underprivileged. Specifically, this review paper aims to analyse studies on the sustainability of agricultural extension delivery systems in face of the COVID-19 crisis. The various theories and factors advanced in discussing sustainable agricultural extension delivery systems, and how they contribute to improved production and productivity to end poverty have been addressed. The COVID-19 mitigation strategies and their effect on agricultural extension sustainability have been laid bare. No single sustainability theory can explain how COVID-19 has disrupted the extension delivery systems. The service providers and farmers have had their effects differently, and all players have followed mitigation to the best of their knowledge. Extension delivery has undergone numerous modifications over the years to better meet the demands of farmers and the changing nature of the markets in which they operate. In order to provide effective service delivery and outcomes to farmers, collective involvement of various stakeholders is necessary since no single player can achieve effective extension sustainability alone due to the diverse nature of the challenges, which are mainly influenced by factors such as geographical location, poverty, and social status among others. Therefore, appropriate approaches should be selected based on the needs of farmers and the market dynamics of a specific economic orientation in a given area. We conclude that effective and sustainable extension delivery systems require many combined theories, support functions, formal and informal rules, and regulations involving all extension-based stakeholders. Abstract (sommario):
The knowledge on health service use, systematic follow-up, and support for families bereaved by suicide remains scarce. This scoping review includes studies from 2010 to March 2022 that investigate the follow-up and support offered by health services, peer support services, and other resources available (e.g., internet-based resources) for families bereaved by suicide. We followed the scoping review framework provided by the Johanna Briggs Institute and performed a double-blinded screening process using Covidence. Data were extracted by four researchers and a thematic analysis was performed to summarize the results. The PRISMA Extension for Scoping reviews was used for reporting results. Of 2385 studies screened by title, 190 by abstract, and 93 by full-text reading, we included 63 original articles of which 24, 29 and 10 were quantitative, qualitative, or mixed-methods studies, respectively. The review shows that we have some knowledge about the need for, and experiences with, health services and support resources for immediate family members bereaved by suicide, but a lack of knowledge about their help-seeking behaviour, patient pathways, systematic follow-up, coordination between services, and long-term outcomes. We need more longitudinal observational studies of health service use and patient trajectories for people bereaved by suicide.

Abstract (sommario):
Artificial insemination (AI) was introduced in Botswana in 1966 to provide smallholder cattle farmers with access to superior bulls for cross-breeding to improve their herd quality and performance. This review endeavours to highlight farmers’ uptake of AI service, challenges and the opportunities in the use of AI technology in Botswana. The 15 AI centres have been underutilised for the past 11 years while the number of inseminated cows has declined over time. Similarly, conception rates declined as these are a function of inseminated cows. Some major challenges limiting AI utilisation are persistent drought, outbreak of livestock diseases and parasites, climate change, inadequate extension service, high transportation costs, shortage of qualified ranch managers, inadequate transport, inadequate staff accommodation, lack of maintenance of fire breaks and perimeter fence at the AI centres, and trekking cows over very long distances to the AI centres for insemination. The underutilisation of AI centres presents an opportunity for farmers to send their cattle for insemination. In order to optimally use the AI service, extension efforts must be directed towards encouraging more farmers to use the AI centres.

Abstract (sommario):
ABSTRACTThis study presents a proposal for a model for execution of Technical Assistance and Rural Extension (TARE) services in Brazil, presenting a flow of actions through methodological tools. For this purpose, the study used a mixed methodological approach, that is, it used the principles of data triangulation, which comprised a survey of qualitative primary data and secondary quantitative data to prove the evidence. In other words, this study raised, from semi-structured interviews with specialists in the execution of TARE, the main methodological tools used in the execution of that service. Based on the terms most frequently cited by the interviewees, a search syntax developed to carry out a systematic review of the literature to theoretically prove the tools raised. As a result of the verification, it was possible to identify a model based on six main TARE execution tools: awareness, diagnosis, individual service, collective service, reference unit and program. This article has as its main contribution the offer of a model validated in the literature. Proposes to standardize a model for the design of TARE notices, and execution plans to contribute with the maximum efficiency in offering the service to the beneficiaries.

Abstract (sommario):
Under the new constitution adopted in 2015, Nepal embraced the federal structure of government comprising seven provincial and 753 local governments, each with their own legislative, judicial, and executive powers. Nepal’s agriculture sector provides livelihoods to about 60% of the population. However, its bottlenecks are rooted in poor implementation of agricultural policies and plans, low levels of investment, uncertain political commitment and weak governance, especially a lack of an effective service delivery mechanism to farmers. This study analyzed the impacts of federalism on the institutional arrangements and governance of the agriculture sector through both review of literature and field-level information gathering, particularly focusing on extension service delivery to farmers in Province 2. The findings highlight the impacts of federalism on agricultural governance mainly in functional overlapping, resource allocation, priority setting, coordination, human resource management, and extension service delivery. The lack of coordination and collaboration between the three tiers of government and the line agencies results in less-effective extension service delivery, especially in providing integrated, specialized technical services to farmers which is the main responsibility of local governments. Lack of poor understanding of governance, institutionalization, and human resources management is found to be one of the most serious problems with the provincial and local governments. The consequences are that despite a huge potential to improve service delivery leading to increased production and a market surplus, the province remains food-deficient and lacks food and nutrition security. The study recommends a strong political commitment, better policy and institutional coordination and coherence, and good governance in all tiers of government by providing demand-driven agricultural services leading to higher cropping intensity and productivity potential for which it is well recognized.

Abstract (sommario):
Comprehensive medication management (CMM) is the service offered within the clinical practice of pharmaceutical care, which has the objective to optimize pharmacotherapeutic outcomes. Patient satisfaction is a multidimensional construct that points to the quality of the health services offered and the degree to which the patients’ expectations and needs are met. The evaluation of the level of patient satisfaction is a key indicator to support decisions and to improve the quality of the service provided. This study aims to describe the protocol for a scoping review to map the instruments to measure patient satisfaction with CMM services and compare them according to their development characteristics and the applicability of patient-reported outcome measures. The literature search will be conducted using the scoping review methodology, proposed by the Joanna Briggs Institute and the PRISMA Extension for Scoping Reviews (PRISMA-ScR) method. The results will be presented in two sessions: (1) description of the search strategy; and (2) the characteristics of the satisfaction instruments, number of items and questions related to the conceptual model, content validity, construct validity, reliability, score/interpretation, and respondent burden. This review will shed light on the available satisfaction measurement instruments, allowing existing gaps to be identified for future research.

Abstract (sommario):
Background: The Coronavirus disease 2019 (COVID-19) pandemic had a significant and on-going impact on the delivery of health and social care services across the globe due to imposed restrictions required to control the spread of the virus. This, undoubtedly, impacted population health through disruption to healthcare services and delayed delivery of care. The impact of these restrictions and precautions were especially pertinent to those who were deemed at high risk of developing complications from COVID-19, for example, those living with chronic disease and significant comorbidities including diabetes mellitus (DM) and diabetic foot ulceration (DFU). Objectives: The objectives of this scoping review are to conduct a systematic search of the literature to identify the nature and extent of the research evidence to which the COVID-19 pandemic impacted and disrupted the care of DFU and the subsequent impact on patient outcomes. Secondly, to identify transformation in models of care and service delivery, adoption of new technologies and new models of service innovations including modalities such as telehealth that were adopted during the pandemic. Methods: This systematic scoping review will be conducted in accordance with the Joanna Briggs Institute methodology for scoping reviews and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping reviews (PRISMA-ScR). Discussion: It is well established that DFU are a devastating complication of DM that cause significant morbidity and mortality, however, there is a need to establish the impact of the COVID-19 pandemic on those living with DFU during the pandemic. We will synthesise and appraise the current literature to provide an in depth understanding of the impact of COVID-19 and the global response on the care of DFU including radical transformation in models of care and service delivery, adoption of new technologies and new models of service innovations, including modalities such as telehealth.

Abstract (sommario):
Access to appropriate and timely agricultural extension services has remained an enduring input in the alleviation of rural poverty. A pluralistic approach to agricultural extension services by government, farmer-based organizations, non-governmental organizations (NGOs) and active participants in the agricultural value chain will help facilitate the needed advisory services in order to achieve improvement in livelihoods and reduce vulnerability. The exponential growth rate resulting in a population explosion, fast-rising vagaries in technology use, heightened increase in livelihood vulnerabilities and the impact of climate change; have further mandated the need for a holistic and urgent review of the method of extension service delivery. The training and visit system, on-farm visits, village meetings, pilot projects, farmer's field schools, and model villages are among the long-standing public extension approaches. In view of the foregoing, there is now an urgent need to appraise both the existing traditional and emerging modern methods. This article, therefore, reviewed and compared the two methods to see the possibility of blending the two so as to attain an improvement in livelihoods and national development. Therefore, the study recommends a decentralization of the mode of operation of agricultural extension according to farmers' needs for development in order to ensure sustainability and attain national development.

Abstract (sommario):
Abstract Background Business intelligence can give businesses the ability to understand their strengths, weaknesses, and opportunities for improvement and can help reduce uncertainty in the decision-making process. With the increasing use of electronic dental records creating more and more dental data each day, it is an opportune time to determine if the data can be coupled with business intelligence systems to improve the management decision-making process in dental organizations to result in service improvement. Methods A scoping review was performed to map the research on the use of business intelligence in dental organizations and to identify any gaps in the existing literature. This scoping review was conducted following the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-Scr) framework. The following databases were searched: Medline, Embase, Emcare, Cinahl, Informit, Web of Science, and Scopus. Data extracted from the articles included the organization type, purpose/aims, the software utilized, data sources utilized, outcomes measured, decision-makers involved, service benefit type, and service improvements. Results In all, 945 articles were found during the search strategy, with 25 articles selected for full-text review. Of these 25 articles, only 3 met the final inclusion in this review. All three included articles were centered around dental school organizations and all situated in the United States. All three articles demonstrated a benefit from management decision-makers utilizing business intelligence systems for improving service efficiency. Conclusion There is limited evidence to show that managers utilizing business intelligence systems in dental school organizations can lead to improvements in the organization's services. There was no evidence to support the use of a business intelligence system in other types of dental organizations. More research is required in this area.

IntroductionIn Aotearoa New Zealand, Māori and Pacific People experience worse health outcomes compared with other New Zealanders. No population-based eye health survey has been conducted, and eye health services do not generate routine monitoring reports, so the extent of eye health inequality is unknown. This information is required to plan equitable eye health services. In this scoping review, we aimed to summarise the nature and extent of the evidence reporting vision impairment, its main causes and access to eye health services by ethnicity in New Zealand.MethodsThis scoping review was reported according to Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. An information specialist conducted a search on MEDLINE and Embase databases in October 2022. Included studies reported outcomes among any population group resident in New Zealand or attendees at New Zealand health facilities. Data screening, full-text review and data extraction were performed independently by two authors. We summarised the characteristics of studies and outcomes, and the results were synthesised narratively.ResultsOur search identified 2711 reports, of which 53 (from 47 studies) were included. We mapped 72 outcomes, many of which were access-related (n=32), published since 2000 (n=28) and largely focused on diabetic retinopathy (n=21) or cataract (n=13) in facility-based settings (n=18). Over two-thirds of reported outcomes were disaggregated by at least two ethnicities. When outcomes were disaggregated by ethnicity, Māori and Pacific People were consistently included, and experienced worse access and outcomes compared with other New Zealanders.ConclusionThe findings of this review highlight the presence of ethnic disparity in access to diabetic retinopathy and cataract services. Closing the evidence gap identified for refractive error, glaucoma and macular degeneration service coverage could be a priority for future research. Furthermore, future research can be strengthened to enable consistent monitoring of eye health service coverage over time.

Abstract (sommario):
Online incremental learning is one of the emerging research interests among the researchers in the recent years. The sentiment classification through the online incremental learning faces many challenges due to the limitations in the memory and the computing resources available for processing the online reviews. This work has introduced an online incremental learning algorithm for classifying the train reviews. The sentiments available in the reviews provided for the public services are necessary for improving the quality of the service. This work proposes the online kernel optimization-based support vector machine (OKO-SVM) classifier for the sentiment classification of the train reviews. This paper is the extension of the previous work kernel optimization-based support vector machine (KO-SVM). The OKO-SVM classifier uses the proposed fuzzy bound for modifying the weight for each incoming review database for the particular time duration. The simulation uses the standard train review and the movie review database for the classification. From the simulation results, it is evident that the proposed model has achieved a better performance with the values of 84.42%, 93.86%, and 74.56% regarding the accuracy, sensitivity, and specificity while classifying the train review database.

Each contract is allowed to include a clause regarding contract extension, when the contract ends. The clause in the contract is that even though both parties have reached an agreement, the extension of the contract is not an obligation or achievement in the contract, for this reason if the construction and handover cooperation contract has ended and the service recipient hands over management to the government as the service user , refusing a request to extend the contract cannot be said to be a breach of promise or default. However, even though refusing to extend the construction and handover cooperation contract is the right of the service user, there needs to be a review if the contract period which is supposed to be long-term only stipulates a period of 7 (seven) years if it results in the contractor or pemodal as the service recipient being harmed.

Abstract (sommario):
This review paper analyses the challenge and opportunities of information and communication technologies for dissemination of agricultural information in Ethiopia. Understanding the challenges and opportunities of information and communication technologies enables to solve the problem of information problem to smallholder farmers. The data was collected, assessed and reviewed from recent secondary data sources from Central statistical agency, and scholarly articles published on the issue of information and communication technology in agriculture (ICT 4 Ag).Compared to the other sector like governance, health and education sectors, ICT-adoption in the agricultural sector has started relatively late in Ethiopia. Even though it is in infant stage, there are many agricultural ICT related service and application in Ethiopia. The current radical penetration of ICTs like mobile phone, website, social media and other internet/online services brings an immense prospect to farmers to create, share, store, process, interpret and preserve agricultural information and knowledge. ICT related agricultural extension system is the solely choice for dissemination of agricultural knowledge and information. Ethiopia is the country which has more than 65000 extension personnel for agricultural extension. But this extension system is not sustainable since it is very costly and inefficient. Thus, agricultural extension policy and intervention should focus on ICT based extension system.

Abstract (sommario):
Abstract The aim of this pilot was to assess the value of a hospital-based teledermatology hub for urgent suspected skin cancer referrals in a semirural integrated care system. The hub uses a new medical photography-led teledermoscopy service to assess both teledermatology and artificial intelligence (AI) compared with face-to-face outpatient review in clinic. The deployment and this work were funded as part of the National Health Service AI in Health and Care Award. In parallel, the existing advice and guidance (A&G) service using images taken by general practice teams was promoted for all referrals other than urgent suspected skin cancer, including low-risk basal cell carcinomas and precancerous lesions. Analysis of the first 100 patients through the teledermoscopy–AI service showed that 18% of patients were diagnosed with benign lesions using the AI (and second-read dermatologist), 25% of patients required face-to-face review for diagnosis, 45% were triaged directly for a surgical procedure with a preoperative telephone call, 6% were prescribed topical therapy virtually, and 6% were discharged using images and telephone consultation without requiring treatment. Subsequent expansion of the dermatology medical photography team using both medical photographers and a trained healthcare assistant has supported extension of the teledermoscopy–AI service, with the pilot running to August 2024. Expansion to community hospital teledermoscopy hubs and extension of the urgent skin cancer service to the community diagnostic centre are changes supported by the pilot, to develop more streamlined pathways for patients with urgent suspected skin cancer. Through 2023 the community A&G service also expanded from around 250 requests per month to over 400 requests per month, with maintenance of conversion rates to referral of 30%, and redirection of patients with low-risk basal cell carcinomas to community skin surgery services. Development of the new urgent suspected skin cancer virtual teledermatology–AI pathway has required administrative adaptations within the hospital electronic patient record to support patient tracking, resourcing and job planning for image review. Training of photographers has been supported by online teledermoscopy resources developed by the Institute of Medical Illustrators. Selection of appropriate patients for the teledermoscopy–AI pathway while maintaining patient choice and safety has remained central to the teledermoscopy–AI pilot.

Abstract (sommario):
Purpose To date, community pharmacy research has largely focused on the impact of service quality elements on patronage behavior. Investigation into the influence of cost and value is limited. The purpose of this study is to explore what is known about customers’ perceptions of cost and value, and how these influence patronage patterns in community pharmacy. Design/methodology/approach A scoping review framework was used to conduct a systematic search of four databases with the addition of articles sourced from reference lists. The database search was reported using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis: Extension for Scoping Reviews protocol. Studies were analyzed in terms of author name, date of publication, study location, study population, methods and key findings. Findings The 26 studies retrieved were qualitative or quantitative in nature and included a broad sample population. Both cost and value were found to be key factors influencing pharmacy choice. Pharmacy customers were found to perceive costs in terms of monetary, psychological, emotional and convenience-related sacrifices. Value was either perceived as relating to the worth or utility of a good or service, or in terms of a trade-off relationship between what was received and given up by the consumer. Research limitations/implications A comprehensive interrogation into the true meaning of “value” to consumers is warranted to improve quantitative measurement instruments. Practical implications Pharmacies may attempt to influence customer behavior by minimizing unfixed costs to the consumer such as the price of goods and services and time costs. Originality/value This review highlights the need for academic enquiry into how consumers trade-off perceived costs for service in community pharmacy.

Abstract (sommario):
The traditional approach to agricultural extension was top-down. The scientist, at the top, was the one who created the technology and this technology was disseminated through the extension service to the farmers who had to apply this technology. The process of dissemination of innovation adopts various methods. As a means, ICTs play a key role. Over the last couple of decades ICTs have been playing a pivotal role. Hence it is important to study the e- readiness of various extension service providers of diff erent agricultural research and development institutes as they act as channels between lab to land. Upon review of existing tools to measure e-Readiness, it was felt that there is need for a reliable scale to measure the same. For the construction of the scale, summated rating method suggested by Likert (1932) was followed. Based on a review of literature 55 statements were enlisted initially and 39 statements were fi nalized for the fi nal construction of the scale. The reliability and validity of the scale were calculated to fi nd out the precision and consistency of the results.
