Abstract (sommario):

Purpose Central venous catheters (CVC) are associated with increased infection rates, morbidity and mortality compared to other hemodialysis vascular access. Chlorhexidine-impregnated transparent (CHG-transparent) dressings allow for continuous antimicrobial exposure and easy visibility of the CVC insertion site. We conducted a quality improvement project to compare catheter-related infection (CRI) rates in two dressing regimens – CHG-transparent dressings and adhesive dry gauze dressing in hemodialysis patients with tunneled CVCs. Methods The study was conducted in two phases. In phase 1, CHG-transparent dressing was introduced to EDC hemodialysis unit, while EDG and EDN hemodialysis units, served as the control sites and maintained adhesive dry gauze dressing. Phase 2 of the study involved replacing the adhesive dry gauze dressing with CHG-transparent dressing at EDG and EDN and maintaining CHG-transparent dressing at EDC. CRI rates at each hemodialysis unit during the 12-month intervention were compared to CRI rates for the 12-month pre-intervention period for each study phase. CRI rates were also compared between all three hemodialysis units. Results In phase 1, CRI rates (per 1000 days) in EDC (intervention site) decreased by 52% (1.69 vs. 0.82, p<0.05) and increased by 12% (1.80 vs. 2.02, p = 0.75) at EDG, and 35% (0.91 vs. 1.23, p = 0.40) at EDN. In phase 2, CRI rates at EDG and EDN (intervention sites) decreased by 86% (1.86 vs. 0.26 p<0.05), and 53% (1.89 vs. 0.88, p<0.05), respectively, and decreased by 20% at EDC (0.73 vs. 0.58, p = 0.65). Conclusions Replacing adhesive dry gauze dressing with CHG-transparent dressing for hemodialysis patients with tunneled CVC was associated with decreased CRI rates.

Abstract (sommario):

Taman Eden 100 is one of the tourist parks located in Toba Samosir, North Sumatra, Indonesia that provides a wide range of biodiversity. The objective of this research was to obtain the potential fungal isolate with high laccase activity to decolorize synthetic dyes. The results show that six isolates of Perenniporia subtephropora EDN 050, Trametes hirsuta EDN 082, T. hirsuta EDN 084, T. hirsuta EDN 085, Deconica coprophila EDN 114, and T. pavonia EDN 134, which were confirmed by molecular identification using 5.8 rDNA/ITS analysis, exhibited decolorization activity. Six potential isolates showed the ability to decolorize textile dyes of Acid Blue 129, RBBR,Orange II, and Reactive Black 5. The highest decolorization rate of 100 ppm Acid Blue 129, RBBR, Orange II, and Reactive Black 5 dyes by using T. pavonia EDN 134 were 98.87%, 98.26%, 100%, and 98.11%, respectively after 96 h incubation. Almost all positive isolates also decolorized dyes at higher concentrations (1,000 ppm). This study offers the potential of Indonesian tropical fungal isolates for synthetic dyes waste treatment.

Background: Energy drinks are a food supplement consisting of multivitamins, macronutrients, taurine, and caffeine. Their excessive consumption is suspected to be a risk factor for chronic kidney failure. Objective: This study aimed to determine the effect of energy drinks on kidney function in rat models with diabetes mellitus (DM). Method: Thirty-two experimental male Wistar rats were injected with alloxan 150 mg/kg intraperitoneally to induce DM and then randomly divided into four groups. The positive control group was treated with drinking water. ED1, ED2, and ED3 groups were respectively treated with energy drinks coded EDK, EDH, and EDE, with a dose equivalent to caffeine 25 mg/kg twice a day for 15 days. All energy drinks used are distributed in the Asian region, especially in Southeast Asia. Urinalysis, haematology, and kidney histopathology evaluations were carried out. Result: Energy drinks affect BUN, serum creatinine, sodium, and potassium. In the histopathological observation of kidney tissue, there was a significant difference in the value of damage between the control group and the ED3 group. Conclusion: Energy drinks significantly increase the risk of impaired kidney function in diabetes mellitus rat models.

Abstract (sommario):

Resveratrol butyrate ester (RBE) complexes have demonstrated higher antioxidant capacity and anti-fat accumulation activity in previous studies. In this study, silica gel, high-performance liquid chromatography, and 1H nuclear magnetic resonance were used for separation and identification of RBE complex components. With the exception of resveratrol, five different structures of ester derivatives were separated from silica gel: 3,4′-di-O-butanoylresveratrol (ED2, 18.8%), 3-O-butanoylresveratrol (ED4, 35.7%), 4′-O-butanoylresveratrol (ED5, 4.4%), 3,5,4′-tri-O-butanoylresveratrol (ED6, 1.5%), and 3,5-di-O-butanoylresveratrol (ED7, 0.7%). Among the ester derivatives obtained, ED2 and ED4 were the main ester derivatives in the RBE complex. Thus, the cellular antioxidant activities of the RBE mixture, ED2, and ED4 were evaluated. Results showed that the antioxidant capacity of ED2 and ED4 was higher than that of the RBE mixture, demonstrating that the number and position of butyrate esterification sites are related to cell survival rate and antioxidant capacity. This study is the first to report the successful isolation, structural identification, and cellular biological antioxidant activity of RBE complex derivatives, which are key characteristics for the potential practical application of RBE complexes.

Abstract (sommario):

HIPOTESIS DE TRABAJO Y OBJETIVOS: La hipertensión arterial (HTA), además de la edad, es el factor de riesgo que más se asocia a la presencia de lesiones cerebrales de sustancia blanca (LSB). De hecho, para la mayoría de autores, estas lesiones son consideradas como un marcador precoz de lesión cerebral en la HTA y relacionadas con el proceso arterioscleroso. No obstante, su etiopatogenia es compleja y supone la implicación de factores genéticos. La relación entre las LSB y los diferentes parámetros clínicos y biológicos asociados a la HTA no está suficientemente definida. Por otra parte, la mayoría de estudios realizados hasta la fecha se han efectuado en población anciana, o en un amplio rango de edades, o se han incluido pacientes hipertensos con tratamiento antihipertensivo, factores todos ellos que han podido actuar como factores de confusión. El objetivo del presente trabajo es determinar los posibles factores clínicos y biológicos relacionados con la presencia de LSB en pacientes asintomáticos de mediana edad afectos de hipertensión arterial esencial nunca tratada.

METODOLOGIA: Se han incluido 66 pacientes hipertensos esenciales de ambos sexos, de edades comprendidas entre 50-60 años, nunca tratados, y sin evidencia de enfermedad cardiovascular. Los pacientes con diabetes mellitus y/o ingesta enólica >30 grs/día, y aquéllos con una estenosis carotídea >50% fueron excluidos del estudio. Se realizaron las siguientes exploraciones: MAPA de 24 horas, ecocardiograma, y determinación del genotipo del gen de la enzima de conversión de la angiotensina (ECA) mediante PCR. A todos los pacientes se les realizó una resonancia magnética cerebral (RM) y se les clasificó en dos grupos, en función de la presencia o ausencia de LSB.

RESULTADOS: Veintisiete (40.9%) pacientes hipertensos mostraban LSB en la RM. Los pacientes con LSB tenían unas cifras de presión arterial (PA) sistólica, diastólica, y presión de pulso, significativamente mayores que los pacientes hipertensos sin LSB, tanto en la clínica como en la MAPA. La presencia de hipertrofia del ventrículo izquierdo (HVI) en pacientes hipertensos con LSB era significativamente mayor que en hipertensos sin LSB (88.4% versus 58.3%; P=0.01). De la misma manera, la presencia de una HVI concéntrica era significativamente mayor (P=0.002) en pacientes con LSB (54%) que en hipertensos sin LSB (11%). El riesgo relativo de HVI concéntrica para la presencia de LSB, independiente de los valores de PA, fue de 8.22 (95% IC: 2.06-32.78). En referencia al estudio genético, se objetivó que los pacientes con LSB presentaban con mayor frecuencia, en comparación con los pacientes sin LSB, tanto el alelo D (74% versus 51.4%; P=0.014) como el genotipo DD (64% versus 28.6%; P=0.022) del gen de la ECA.

CONCLUSIONES: La presencia de LSB en pacientes hipertensos asintomáticos de mediana edad es un hallazgo frecuente y está relacionado con la severidad de la elevación de la PA. La existencia de estas lesiones está asociada a la presencia de una HVI concéntrica. De la misma manera, y de forma independiente de las cifras de PA, la presencia del genotipo DD del gen de la ECA podría ser un factor predisponente para el desarrollo de LSB en pacientes con hipertensión arterial esencial.

Abstract (sommario):

Contexte : Le SEIPA est une allergie alimentaire non IgE-médiée particulière, répondant à des critères diagnostiques précis. Le traitement est l'éviction des aliments en cause. Il n'existe pas de biomarqueur sérique confirmant le diagnostic, ou prédisant l'acquisition de la tolérance. Seul le test de provocation par voie orale (TPO) permet d'évaluer régulièrement la tolérance à l'allergène en cause. L'évolution est le plus souvent favorable chez l'enfant vers l'âge scolaire. Cependant, la sévérité des symptômes et l'absence d'IgE spécifiques justifient une recherche sur le SEIPA pour mieux comprendre la physiopathologie. Hypothèses : Le SEIPA serait associé à une dysbiose digestive, une dysimmunité locale au niveau digestif, une inflammation intestinale chronique, et une augmentation de la perméabilité intestinale. L'inflammation et la perméabilité seraient majorées lors d'une réaction allergique aiguë. Après acquisition de la tolérance, tous ces paramètres reviendraient à la normale. Objectifs : Décrire de manière longitudinale les microbiotes fécaux et salivaires des patients pédiatriques avec un SEIPA, pendant l'allergie et après la tolérance, ainsi que des biomarqueurs d'intérêt évaluant l'inflammation, la perméabilité et la dysimmunité digestive, comparativement à une cohorte de référence. Matériels et méthodes : Des prélèvements fécaux et salivaires ont été collectés chez les enfants avec un SEIPA suivi dans plusieurs hôpitaux de l'AP-HP, après le diagnostic sous régime d'éviction, et après acquisition de la tolérance, ainsi que chez des témoins appariés aux mêmes âges (ratio 1:1). Des prélèvements de selles supplémentaires ont également été récoltés avant et après les TPO. La composition du microbiote a été analysée par séquençage du gène de l'ARN r16s (V3-V4). La fonction du microbiote fécal a été estimée par dosage des acides gras à chaine courte (AGCC) (chromatographie en phase gazeuse). La calprotectine fécale, la neurotoxine dérivée des éosinophiles (EDN), les IgA sécrétoires fécales, et la zonuline fécale ont évalué l'inflammation, l'immunité et la perméabilité intestinale (ELISA).Résultats principaux : De décembre 2019 à mai 2023, 38 patients ont été inclus (âge moyen : 1,3 ans). A l'inclusion, les patients allergiques avaient une dysbiose fécale et salivaire, s'exprimant par une diminution de l'alpha-diversité, une béta-diversité différente, plusieurs genres bactériens différents, et une augmentation de l'abondance relative de l'acétate par rapport aux témoins appariés. Les patients allergiques n'avaient pas d'inflammation intestinale sous régime d'éviction (calprotectine, EDN), mais seulement de manière aiguë et transitoire après une réaction allergique. Les taux d'IgA sécrétoires fécales étaient plus élevés chez les patients allergiques que chez les témoins ou chez les patients tolérants. La zonuline était similaire entre les patients et les témoins. Après acquisition de la tolérance, chez les 22 patients devenus tolérants, les microbiotes fécal et salivaire se sont rapprochés partiellement de ceux des témoins. Les AGCC et les IgA sécrétoires se sont normalisés (augmentation de l'abondance relative du butyrate, baisse du taux d'IgA). Conclusions : D'après les résultats de cette cohorte, et selon les données antérieures de la littérature scientifique, les enfants avec un SEIPA ont une dysbiose intestinale avant le début de l'allergie, une dysbiose intestinale et salivaire pendant l'allergie jusqu'à l'acquisition de la tolérance, puis un retour du microbiote proche de celui d'enfants sains. La réponse immunitaire adaptative locale est exagérée au cours de la pathologie, avec une augmentation des IgA sécrétoires fécales chez les individus allergiques par rapport aux témoins, et une normalisation après la tolérance. Background: FPIES is a non-IgE-mediated food allergy, meeting precise diagnostic criteria. Treatment involves avoiding the offending food. There is no serum biomarker confirming the diagnosis or predicting the development of tolerance. Only the oral food challenge (OFC) regularly assesses allergenic tolerance. In most cases, children acquire spontaneous tolerance at school age. However, the severity of symptoms and the absence of specific IgE justify research into FPIES to gain a better understanding of the pathophysiology.Hypothesis: FPIES is associated with digestive dysbiosis, local digestive dysimmunity, chronic intestinal inflammation and increased intestinal permeability. Inflammation and permeability are exacerbated during an acute allergic reaction. Once tolerance is acquired, all these parameters return to normal. Objectives: To provide a longitudinal description of the faecal and salivary microbiota of paediatric patients with SEIPA, at the times of allergy and tolerance, as well as biomarkers of interest assessing inflammation, permeability and digestive dysimmunity, compared with a reference cohort. Materials and methods: Faecal and salivary samples were collected from children with FPIES followed in several AP-HP hospitals, after diagnosis on an elimination diet, and after acquiring tolerance, as well as from matched controls at the same ages (1:1 ratio). Additional stool samples were also collected before and after OFC. The composition of the microbiota was analysed by r16s RNA gene sequencing (V3-V4). The function of the faecal microbiota was estimated by measuring short-chain fatty acids (SCFA) (gas chromatography). Faecal calprotectin, eosinophil-derived neurotoxin (EDN), faecal secretory IgA, and faecal zonulin assessed inflammation, immunity and intestinal permeability (ELISA).Main results: From December 2019 to May 2023, 38 patients were included (mean age: 1.3 years). At inclusion, allergic patients had faecal and salivary dysbiosis, expressed as decreased alpha-diversity, different beta-diversity, several different bacterial genera, and increased relative abundance of acetate compared with matched controls. Allergic patients did not have intestinal inflammation on elimination diet (calprotectin, EDN), but only acutely and transiently after an allergic reaction. Levels of faecal secretory IgA were higher in allergic patients than in controls or tolerant patients. Zonulin was similar between patients and controls. After acquiring tolerance, in the 22 patients becoming tolerant, faecal and salivary microbiota partially approximated those of controls. Secretory AGCC and IgA levels normalised (increase in relative butyrate, decrease in IgA). Conclusions: According to the results of this cohort, and to previous data in the scientific literature, children with FPIES have a gut dysbiosis before the onset of allergy, gut and salivary dysbiosis during allergy until the acquisition of tolerance, and then a return of a microbiota close to that of healthy children. The local adaptive immune response is exaggerated during the disease, with an increase in faecal secretory IgA in allergic individuals compared with controls, and normalisation after tolerance. Ішемічний інсульт поряд з ішемічною хворобою серця та онкопатологією продовжує утримувати лідируючі позиції серед причин захворюваності, смертності та інвалідизації населення як в Україні так і в цілому світі. Важливий вплив на розвиток інсульту здійснюють фактори ризику, а саме артеріальна гіпертензія (АГ), стать, підвищений індекс маси тіла (ІМТ) та куріння. У патогенезі ішемічного атеротромботичного інсульту (ІАТІ) провідну роль відіграє ендотеліальна дисфункція (ЕД), в розвиток якої важливий внесок здійснює генетична схильність.

Abstract (sommario):

EDS is a tool for quantitative and qualitative analysis of the materials. In electron microscopy, the energy of the electrons determines the depth of the region where the X-rays come from. By varying the energy of the electrons, the depth of the region where the X-rays come from can be changed. If a thin film is used as a specimen, different quantitative ratios of the elements for different electron energies can be obtained. Unique thickness of a specific film on a specific substrate gives unique energy-ratio diagram so the thickness of a thin film can be calculated by analyzing the fingerprints of the energy-ratio diagram of the EDS data obtained from the film.

Abstract (sommario):

La matrice extracellulaire (MEC) constitue une plateforme fibrillaire intégrant l'action des signaux chimiques et mécaniques de l'environnement. La fibronectine (FN), un composant majeur de la MEC, est au centre de cette plateforme de biorégulation et module de nombreuses actions biologiques telles que l’adhésion et la motilité cellulaires, la prolifération et la différenciation, ainsi que la structure et le dépôt de la MEC. La FN se présente sous deux formes : la FN plasmatique (pFN) et la FN cellulaire (cFN), dite « oncofoetale ». Cette dernière est nommée ainsi pour son expression uniquement au cours du développement et dans certaines conditions physiopathologiques (réparation tissulaire, inflammation et cancer). La différence entre les deux est l’existence dans la cFN des domaines supplémentaires EDB et EDA, qui résultent d’un épissage alternatif. Comment la présence de ces « extra-domaines », EDA et EDB , régit l'assemblage des FN et comment les variantes assemblées régulent le comportement des cellules est en grande partie inconnu. Des études de délétion ciblées d’un seul domaine chez la souris ont révélé le rôle de l'EDA dans des phénomènes très variés dont la morphogenèse des valves lymphatiques, l'athérosclérose et la cicatrisation / fibrose. Au niveau moléculaire, l'inclusion de l'EDA élargit le répertoire des récepteurs cellulaires de la FN (intégrines α4β1, α7β1 et TLR4). A ce jour, aucun récepteur n'a été rapporté pour l’EDB. De ce fait il a été proposé que sa présence modifierait la conformation du site de liaison de FN aux cellules et faciliterait ainsi l’assemblage fibrillaire induit par les intégrines via des mécanismes qui restent à établir.Le but des travaux de cette thèse était de décoder les rôles de l'EDA et de l'EDB de la FN, et plus précisément de :1) étudier l'impact de la présence des domaines EDA et EDB sur l'assemblage fibrillaire de la FN à la surface de cellules compétentes pour l'assemblage, et2) déterminer comment la présence de l’EDB et de l’EDA influence les propriétés biochimiques, physiques et fonctionnelles de la matrice, qui à leur tour affectent le comportement des cellules qui y sont attachées.Dans un premier temps l’équipe a développé un ensemble d'outils biologiques composé de : i) vecteurs lentiviraux hébergeant la séquence codante complète du gène de la cFN humaine contenant l’un, les deux ou aucun des « extra-domaines » alternativement épissés, ii) des variants de FN recombinants purifiés, iii) des fibroblastes embryonnaires de souris FN -/- (Fn1 -/- MEF), et iv) des MEF exprimant des variantes de FN. Cette batterie outils uniques a été utilisée pour étudier l'assemblage spécifique de variantes par les fibroblastes et les effets de réseaux de cFN homogène sur le comportement cellulaire.Nos résultats ont montré que les « extra-domaines « de la FN sont responsables au niveau des fibroblastes du réglage fin de l’amplitude de plusieurs réponses dont l’assemblage de la matrice, la croissance et le métabolisme énergétique. En utilisant une approche informatique « non-biaisée », nous avons démontré que la présence des « extra-domaines » modifie la structure de la matrice de la FN déposée par les fibroblastes. Ceci témoigne d’événements de signalisation cellulaire différents, qui sont susceptibles de modifier aussi bien les réponses précoces que tardives induites par la FN. Les matrices variant-spécifiques que nous avons développées représentent des outils très puissants pour décrypter les fonctions des « extra-domaines » de la FN dans les multiples types de cellules impliquées non seulement dans des réponses physiologiques mais également dans des situations pathologiques
The Extracellular Matrix (ECM) constitutes a fibrillar platform that integrates the action of chemical and mechanical cues from the environment. Fibronectin (FN), a major component of the ECM, is at the center of this bioregulatory stage, modulating numerous biological procedures such as cell adhesion and motility, cell proliferation and differentiation, as well as ECM deposition and structure. FN is found in two forms: plasma FN (pFN) and cellular FN (cFN). cFN differs from pFN by the presence of alternatively spliced Extra Domains, namely EDB and EDA. Each of these alternatively spliced regions is encoded by a single exon, the regulation of which is strictly regulated and limited to embryonic tissues, as well as pathophysiological conditions such as wound healing, inflammation, and cancer. The term “oncofetal” was coined in order to describe FN isoforms harboring either of the Extra Domains, that are normally absent in adult tissues.How the presence of EDA and EDB regulates FN assembly and how the assembled variants regulate cell behavior is largely unknown. Single Extra Domain-targeted deletion studies in the mouse have revealed roles for EDA in the morphogenesis of lymphatic valves, atherosclerosis, and wound healing/fibrosis. Mechanistically, the inclusion of EDA expands the repertoire of cellular FN receptors (α4β1, α7β1 integrins and TLR4). To date, no ligand has been reported for EDB. Rather, its presence has been postulated to alter the conformation of the cell-binding domain of FN and to facilitate integrin-driven fibrillar assembly by mechanisms that remain to be established.The aim of this dissertation is to provide more insight regarding the roles of the FN Extra Domains EDA and EDB, and more specifically:1) To investigate how the presence of alternatively spliced EDA and EDB domains affect the fibrillar assembly of FN at the surface of assembly-competent cells, and2) To determining how the presence of EDB and EDA influences the biochemical, physical, and functional properties of the matrix that in turn affect the behavior of cells attached to it.Here we present a biological toolset composed of : i) lentiviral vectors harboring the full-length coding sequence of human cFN containing one, both, or none of the alternatively-spliced Extra Domains, ii) purified recombinant FN variants, iii) FN-null mouse embryo fibroblasts (MEFs) and iv) FN variant-expressing MEFs. These unique tools were used to study variant-specific assembly by fibroblasts and the effects of homogeneous cFN networks on cell behavior.Utilizing an unbiased computational approach, we document that the presence of the Extra Domains results in a distinct pattern of FN deposition and assembly that in turn influences both early and late signaling events that control cell proliferation as well as cell contractility. Finally, we present a series of novel data that point towards a FN-impacted control of cell energetics.We conclude that FN Extra Domains are responsible for the fine-tuning of the extent of several cellular processes that can reflect strict regulation in pathophysiological procedures such as tissue repair, fibrosis, and tumor progression

Le premier chapitre de ce mémoire porte sur l'étude des minimum forts pour des problèmes de contrôle optimal gouvernés par des EDP semi-linéaires elliptiques et paraboliques avec contraintes intégrales sur l'état final. Le second chapitre porte sur l'étude du problème de temps minimal pour un système de type chemostat en présence de points singuliers stationnaires. On y étudie également un problème de contrôle optimal pour un système chemostat avec deux espèces en compétition et qui comporte une courbe de non-contrôlabilité. Le troisième chapitre s'intéresse à la synthèse d'un contrôle optimal par retour d'état pour un problème de temps minimal issu d'un système fed-batch, notamment en présence d'un contrôle impulsionnel. Le quatrième chapitre étudie deux problèmes de contrôle optimal sous contraintes d'état périodiques. Enfin, le dernier chapitre traite de problèmes d'optimisation de forme géométriques sous contraintes de convexité. Cette dernière est formulée comme une contrainte semi-définie, ce qui permet ensuite d'utiliser la programmation SDP pour minimiser la fonction coût.
