Four men with HIV infection who were referred to liaison psychiatry for assessment of eating disorders are described. In all four cases the eating disorder had implications for the clinical management of their HIV infection. Investigations of weight loss, dietary intervention, and compliance may all be affected by the presence of an eating disorder. The development of HIV disease may exacerbate the symptoms of an eating disorder.

Introduction and purpose: Eating disorders belong to the group of mental diseases characterized by significant somatic complications and high mortality. There is a common opinion that these disorders mainly affect women. Such assumptions may result in limited knowledge of the diagnosis and treatment of an eating disorder in the male population. The aim of this study is to summarize the current knowledge about eating disorders among men, including binge eating disorder (BED), anorexia nervosa (AN) and bulimia nervosa (BN). Description of the state of knowledge: It turns out that the problem of eating disorders among the male sex is not so rare- it is estimated that approximately 10 million US men will experience an eating disorder at some point in their lives. The most common eating disorder among men appears to be binge eating disorder (BED). Men are also more likely to report binge eating than women. AN and BN occur much less frequently than BED in the male population. The symptoms of anorexia nervosa and bulimia nervosa may differ between men and women. More and more often we observe a development of a certain type of muscle dysmorphia among men, the so-called “reverse anorexia". Eating disorders carry a number of medical complications such as cardiac disorders, electrolyte disturbances, digestive problems and skeletal disturbances. Therapeutic interventions in the treatment of male eating disorders should take into account gender-specific problems. Conclusions: The real number of men suffering from eating disorders may be underestimated due to the neglect of the problem in the context of the male gender. It also results in poorly developed diagnostic and support schemes for men struggling with this problem. More research is needed on the topic of eating disorders in this group of patients as it will help to develop better diagnostic and therapeutic regimens adapted to the male gender.

The onset of an eating disorder in middle-age men is poorly researched as are eating disorders in men generally. Therefore, life events that influence eating disorders in men, including delayed onset of an eating disorder remains unknown. Given the limited understanding of males with eating disorders and limited access to large samples of men with eating disorders, an in-depth analysis of a single case of a male in middle age with an eating disorder was chosen to gain insight and understanding into this phenomenon. A Life History approach explored the case of Joseph (pseudonym), who was diagnosed at age 44 years with an Eating Disorder Not Otherwise Specified. Data were collected through (a) life course open-ended questioning through interviews, (b) written statements, and (c) comments on transcripts. Three themes emerged, loss and unworthiness, becoming bigger, and wanting to change reflecting eating behaviors associated with attachment disruption, loss and trauma, body dissatisfaction, and negative affect. Later in life, an emotional “tipping point” precipitated an eating disorder. Results indicate traumatic loss leading to early attachment disruption as influential in Joseph’s delayed onset of an eating disorder. The value of thorough narrative life histories during therapy when eating disorders occur late in life is discussed as well as the significance for men.

This study aimed to explore experiences of men currently using eating disorder services. Eight men from two eating disorder services were interviewed about their experiences of seeking and receiving treatment. Two superordinate themes emerged from Interpretative Phenomenological Analysis: (1) difficulty seeing self as having an eating disorder; and (2) experiences of treatment: how important is gender? The underlying themes varied in their specificity to men, with some echoing findings from the female eating disorder literature. Difficulty admitting the eating disorder may link with eating disorder psychopathology as well as gender-specific issues. Implications for clinical practice and future research are discussed.

Eating disorders are distressing and sometimes life-threatening disorders. As many as half of the people who seek treatment fail to benefit from current first-line psychological interventions. The emerging evidence base for mindfulness-based interventions for eating disorders was reviewed to establish the effectiveness of these interventions. Group mindfulness-based interventions were effective for eating disorders, with or without eo- morbid disorders, but maintenance of treatment effects was limited. Mindfulness-based interventions provide additional treatment options for eating disorders, especially for more complex cases. However, there is currently a lack of high-quality empirical studies. Eating disorders in men are on the increase. The empirical study investigated relationships between parental bonding, maladaptive schemas, core beliefs, and eating disorder symptoms in a non-clinical sample of men. Two hundred and forty six men completed online versions of standardised questionnaires. Parental bonding was not a significant predictor of eating disorder symptoms; while the schemas Vulnerability to harm, Unrelenting standards, Enmeshment, Emotional Deprivation and Insufficient self- control, and the core beliefs High standards for self, and Abandonment/Isolation were significant predictors of eating disorder symptoms in the sample. Schemas and core beliefs are important cognitive variables in the development of eating disorders in men.

Eating disorders is considered as a well researched area. Although, an increasing number of mental health clinicians are becoming better equipped in recognising eating disorders in individuals, specialist eating disorder services still underrepresent various groups. This doctoral thesis examines two of those groups specifically, men and ethnic minorities, in particular for the latter, South Asian women and eating disorders. The first paper reviews published research and examines the link between eating disorders in men and gender differences. It specifically focuses on the factors, which are argued to have a causal link to the development and maintenance of eating disorders in men. It further examines the validity and reliability of eating disorder research in this area and explores the implications for clinical practice. The second paper presents an empirical study exploring the development and experiences of eating disorders in South Asian women including a comparative analysis with Caucasian women. The final paper provides a reflective account of my journey in carrying out this research.

There is an overrepresentation of gay men seeking treatment for eating disorders. This study investigated several factors that were thought to possibly impact the prevalence rates of gay men seeking treatment for eating disorders. The current study investigated the influence that gender role conflict, attitudes towards help seeking, symptom recognition, and media influence have on the prevalence of eating disorders. Nationwide participant recruitment was utilized to gather a sample that consisted of 86 heterosexual men and 75 gay men. Multivariate and univariate analyses of variance were utilized to examine the differences between gay and heterosexual men on the factors of interest. A significant difference was not found between gay and heterosexual men related to gender role conflict or media influence. However, a significant difference was found between heterosexual and gay men on measures of attitudes towards help seeking and symptom recognition. The results support that gender role conflict may have a limited role in the development of eating disorders in gay men and that mental health prevention and awareness within the gay community may be having a positive impact. Implications for the prevention and treatment of eating disorders are discussed as well as directions for future research.

Ätstörningar är en sjukdomsgrupp som blir allt vanligare i samhället. En av orsakerna till att sjukdomen ökar är det orealistiska idealet som presenters i media. Mediatrycket blir även mer påtagligt och lättåtkomligt. Idealet medför en ökad missnöjdhet med den egna kroppen och med utseendet som bidrar till ett driv för förändring. Syftet med denna studie är att undersöka om män påverkats av media i samma utsträckning som kvinnor i insjuknandet av en ätstörning. Samt om könet spelar en central roll för vem som drabbas. Av de som drabbas av en ätstörning och söker vård utgörs enbart 10% av män. Kan idealet ha en påverkan till att färre män drabbas? Eller medför de olika föreställningarna kring könen och sjukdomen att män inte inser att se är sjuka, vågar de inte söka hjälp eftersom det anses vara en kvinnosjukdom? Resultatet tyder på att sjukdomen är komplex till sin natur, där inte enbart en mediafaktorn bidrar till att en person drabbas.
Eating disorders are a disease group that is becoming more common in society. One of the reasons that the disease is increasing is the unrealistic ideal that is presented in media. The media-pressure is substantial and accessible. The beauty ideal involve a dissatisfaction of a persons own body and his or hers appearance that will contribute to the drive of change. The purpose of this study is to investigate if men, as well as women, are influenced by the media-factor in the sicken of an eating disorder. Also if the sex matters and plays a certain role for those who suffer. Only 10 percent of those who are suffering from an eating disorder and that seeks help are men. Can the ideal have an direct affect that less men suffer from this disease? Or can the different expectations of the sexes and of the disease implicate that men don´t realize that they are sick, or that they feel shameful to seek help because eating disorders are known to be a women’s disease? The results implicate that eating disorders is a complex disease and that the media-factor alone does not contribute to sicken or the cause of sickness.

This study investigates the impact of sociocultural mediators in relation to eating disorders among male undergraduates. Literature on eating disorders has demonstrated that a thin body shape ideal depicted in the media directly contributes to eating pathology among females, but little research has investigated the direct effects of ideal body shape images among men. The focus of the present investigation was to assess the direct effects of exposure to the ideal male body shape on men’s affect, self esteem, body satisfaction, and endorsement of U. S. societal ideals of attractiveness. In addition, the relation of these variables to bulimic symptomatology was examined. Modeling a study conducted on women (Stice & Shaw, 1994), male undergraduates between the ages of 18 to 25 participated in premeasure (N = 169) and post measure (N = 95) conditions. Participants in the post measure were randomly exposed to pictures from magazines containing either male models depicting the ideal body shape, an average body or pictures of clothing without models. Results from repeated mulitvariate analysis indicated that exposure to the ideal body shape condition did not demonstrate significant negative changes in men’s affect, self esteem, body satisfaction or endorsement of U. S. societal ideals of attractiveness. Indirect support for the sociocultural theory of eating disorders was provided by multiple regression analyses which demonstrated that increased body mass, self esteem, stress and anxiety predicted bulimic symptomatology in men. Future research should direct itself toward investigating possible sociocultural influences of eating disorders on certain male subenvironments, such as athletes or homosexual males that place a greater emphasis on maintaining lower body mass and an ideal body shape.

Eating disorders (ED) are maladaptive eating patterns that can have social, biological, health, and occupational consequences. The purpose of this study was to explore and compare binge eating episodes, locus of control, and body dissatisfaction between African-American men (n = 66; 53.70%) and Caucasian-American men (n = 57; 46.30%). There is a current gap in the existing literature regarding the study of men who BE and a sampling bias with regard to ethnic minorities. Based on Bandura's social learning theory model and Rotter's locus of control, the purpose of this research was to determine and compare the relationship between BE, locus of control, and body dissatisfaction among African-American and Caucasian-American men. The participants answered a demographic questionnaire, Eating Attitudes Test (EAT-26), Internality, Powerful Others and Chance Scale (IPC), and Body Satisfaction Questionnaire (BSQ). A quantitative research design was used and the chi-square was performed to evaluate the variables of the research questions. The sample population came from the Walden University participants pool and men who are self-described binge eaters from the African-American and Caucasian-American ethnicity in the community. Key results showed that African- American men believed they had less power in their lives, lower levels of body dissatisfaction, and increased feelings of chance in their lives. Recommendations for further research can be to replicate this study using other ethnicities. Implications for social change can include increased knowledge of men that BE which can improve their overall health and quality of life while reducing medical costs.

A plethora of research has linked negative affect with binge eating in people with binge eating disorder (BED). Cognitive behavioural therapy (CBT) and interpersonal therapy (IPT) have not traditionally addressed emotional regulation deficits. Failure to address emotional aspects of binge eating may explain why some individuals do not respond to CBT or IPT, and why many of those who do respond relapse shortly after finishing treatment. Dialectical behaviour therapy (DBT) specifically targets the inability to accurately recognise and regulate affect. Preliminary investigations have shown that DBT may be efficacious in treating BED. However, it can take up to 10 months, and a shorter intervention targeting affect regulation and recognition may produce similar effects. Four studies evaluating a ten session emotion-focussed group psychoeducational intervention for BED were conducted. In the first, 25 women diagnosed with subthreshold or full syndrome BED (using DSM-IV-TR criteria) were treated. The second study tested whether the specific components of the intervention, or the treatment as a whole, was required to produce positive outcomes in women. Study three, examined the efficacy of the treatment programme with three men diagnosed with subthreshold or full syndrome BED (also using DSM-IV-TR criteria). The fourth study compared the women's and men's response to treatment. Following the intervention with women, binge abstinence rates, comparable to those of CBT and IPT, and various other positive changes to eating and general pathology, were observed. These effects were well-maintained up to one-year later. Overall, it was concluded that the whole treatment programme was necessary to produce the optimum outcome for BED in women. A positive outcome was measured in the men, although the effects were not as dramatic as those found in the women. Suggestions for improvements, and suggestions for further research, are discussed. The results provide support for the Affect Regulation Model of BED in women and men.

This quantitative study examined body image and disordered eating in older males. Using a series of questionnaires and demographic questions, two research questions were explored: a) What are the characteristics of older males in terms of eating and body image? and b) Are disordered eating behaviors among older males related to dissatisfaction with body image, specifically physical appearance or physical functioning? Paired samples t-tests revealed that older males rated their ideal body figure as significantly smaller than their current figure, t(35) = -5.53, p < .01, which indicates the presence of body dissatisfaction. Twenty percent of participants were found to be at risk for disordered eating attitudes and behaviors. Furthermore, a correlation was found between disordered eating attitudes and body dissatisfaction particularly as it relates to physical appearance, (r(33) = -.486, p < .01).

Abstract (sommario):
Introduction: Eating disorders are mental illnesses characterised by abnormal eating habits that have a negative impact on a person's physical or mental health. In the last decade hospitalizations which included eating disorders increased among all age groups. The assessment of eating disorders associated with affective disorders has important clinical implications, but the standard psychiatric classification DSM-5 (American Psychiatry Association, 2013) and ICD-10 (World Health Organization, 1993) are limited. Objectives: The current study aims to broaden the evaluation of this association and better understand its clinical implications. In addition, the study's goal is to comprehend the implications of eating disorders in Galaţi County. Method: We conducted a retrospective study on 147 patients with eating disorders and mood [affective] disorders who were admitted to the Psychiatry Hospital "Elisabeta Doamna" Galati between January 1 and February 1, 2019.We used ICD-10 (Classification of Mental and Behavioral Disorders) and DSM-5 criteria for diagnosis (Diagnostic and Statistical Manual of Mental Disorders). Results: In the period from 1 January 2019 - 1 February 2019 a total of 1131 patients was admitted in the Psychiatry Hospital, Of these, 147 were diagnosed with mood (affective) disorders, of whom 17 patients (12%) associated disorder and food as well as the independent disorder. Among these patients, the percentage of women with eating disorders associated with the affective disorder was 82 % and the percentage of men was 18 %. Conclusions: According to the findings, women are more likely to associate eating disorders with mood [affective] disorders. We also found a poor relationship between eating disorders and affective disorders, with eating disorders being associated with a high percentage of other psychiatric disorders, which is represented by alcohol and substance use, but also by high-impact diseases like Alzheimer's disease and schizophrenia.

Introduction: University stage imply some changes and challenges that turn it into a crucial and tricky period for mental health and substance use. Material and methods: We present a study based on a sample of 49 people of the university community, 37 women and 12 men, evaluated in a seven months period. The assessment consisted in an interview carried out by a psychiatrist. A database was designed, providing the clinical information obtained from the interview and entry sheets during the first visit. Following descriptive and analytic studies were performed using the hypothesis contrast “Chi-Square” test. The reference statistical significance level was α = 0.05. Aims: To study the impact of substance use in the mental health of our sample in order to implement new prevention and treatment strategies. Results: 53,1% of the sample abuse substances: 30,8% men and 69,2% women (p 0,277). Alcohol is the most used substance in the sample (80,8%), followed by tobacco (57,7%) and cannabis (30,8%). Concerning psychopathology, insomnia (53,8%) is the most frequent symptom (p 0,260); thoughts of death were present in 46,2% of the substance users (p 0,419); while self-harm (p 0,365) and suicidal attempts (p 0,113) were described by the 19,2% of that group of the sample. Eating disorders and psychosis were observed in 23,1% (p 0,560) and 11,5% (p 0,743) respectively. Conclusions: Substance use is a very prevalent practice in our sample. We have observed some clinical symptoms are frequent in this part of the sample and we should pay special attention to their prevention and early treatment, as well as that of the substance use. This way we could minimize and tackle Mental Health problems in our sample.

The study involved 137 women with various values of body mass index, aged 17 to 72 years (mean age 35 years). The first group included 81 women with insufficient and normal body weight. The second group was formed by women with overweight and obesity in the amount of 56 people. Analysis of data collected using the Thomas Cash Multimodal Body Attitude Questionnaire (MBSRQ, Thomas F. Cash) showed differences corresponding to the following scales: Overweight Preoccupation, Self-Classified Weight, and Body Areas Satisfaction. Women from the second group are characterized by a more accurate idea of their own weight and a higher concern about being overweight. Satisfaction with body parameters was more pronounced in the group of women with insufficient and normal body weight, which suggests that satisfaction with body parameters in modern society is very often accepted in combination with a slender body. To study the phenomenon of an unhealthy lifestyle of a person and the tendency to neglect one’s own body, we used the questionnaire “Healthy Behavior Disorders” by E.L. Lutsenko, O.E. Gabelkova. Differences were found on the Self-Injurious Behavior scale. For women with insufficient and normal body weight, conscious or unconscious self-harm is more common. Other scales did not differ significantly in the two samples. It was also found that in the total sample (n=137), the actual weight has only one correlation out of 9 scales of the “Healthy Behavior Disorders” methodology – a direct correlation with eating disorders. At the same time, in overweight preoccupation we found significant associations with four out of nine scales: eating disorders, craving for alcohol, chasing a fashionable image, and a general indicator of propensity to violate healthy behavior. This suggests that the subjective rejection of one's weight can bring more negative consequences than actual excess weight. В исследовании участвовали 137 женщин с различными значениями индекса массы тела в возрасте от 17 до 72 лет (средний возраст 35 лет). В первую группу вошли 81 женщина с недостаточной и нормальной массой тела. Вторую группу сформировали женщины с избыточной массой тела и ожирением в количестве 56 человек. Анализ данных, собранных с помощью мультимодального опросника отношения к собственному телу Томаса Кэша (MBSRQ, Thomas F.Cash) показал различия, соответствующие шкалам: озабоченность лишним весом, самооценка веса и удовлетворенность параметрами тела. Для женщин из второй группы характерно более точное представление о собственном весе и более высокая озабоченность лишним весом. Удовлетворенность параметрами тела оказалась более выражена в выборке женщин с недостаточной и нормальной массой тела, что говорит о том, что удовлетворенность телесными параметрами в современном обществе очень часто принимается в сочетании со стройным телом. Для изучения феномена нездорового образа жизни человека и тенденции к пренебрежению собственного тела мы использовали опросник «Нарушения здорового поведения» Е.Л. Луценко, О.Е. Габелковой. Были выявлены различия по шкале Самоповреждающее поведение. Для женщин с недостаточной и нормальной массой тела более свойственно осознанное или бессознательное нанесение себе повреждений. Другие шкалы значимо не различались в двух выборках. Также было выявлено, что в общей выборке (n=137) фактический вес имеет лишь одну корреляцию из 9 шкал методики НЗП – прямая с нарушениями питания. При этом по показателю озабоченность лишним весом мы выявили значимые связи с четырьмя из девяти шкал: нарушения питания, тяга к алкоголю, погоня за модным имиджем, и общий показатель склонности к нарушениям здорового поведения. Это свидетельствует о том, что субъективное неприятие своего веса может принести больше негативных последствий, чем фактический лишний вес.

Balanced diet and regular physical activities are essential for support health status, physical fitness and endurance, and decrease the risk of health disorders and morbidity. Balanced diet that cover physiological requirements and needs is a very important for support physical fitness and improve the quality of recovering processes after physical load. Knowledge and skills of individuals allow making good and right choice of daily essential nutrients and keeping optimal health status and improve physical endurance. The aim of the study to analyse cadets` daily dietary habits and knowledge in nutritional education on the base of questionnaire and elaborate adapted to respondents. Cadets` daily dietary habits and knowledge in nutritional education analysed on the base of questionnaire that elaborated and adapted to respondents of study group. The study group included 73 persons of both gender (male N= 65, and female N= 7) in aged from 20 years until 35 years. Questionnaire included 22 questions about daily dietary habits, menu planning, nutrients levels, food products consumption, and composition. Only 30.5 % of respondents interested into eating adapted dietary patterns and follow to dietary recommendations. Supervision of dietary habits, water consumption, dietary patterns are essential for health capacity support done by 73.6% of respondents. Quality and quantity characteristics of dietary patterns are essential for support physical and mental activity. Balanced and moderate diet is essential for improving working capacities, diminished fatigue, improve concentration capacities and support mental activity as a result provide wellbeing and welfare of individuals.

Bakgrunn: WIRUS er et nasjonalt forskningsprosjekt om alkohol, arbeidsliv, sykefravær, sykenærvær og arbeidsplassbaserte intervensjoner. Prosjektet består av flere delstudier. Én av disse er en screeningstudie der ansatte i ulike virksomheter, på tvers av sektorer og bransjer, svarer på et spørreskjema som handler om bl.a. alkoholbruk, holdninger til arbeidsrelatert alkoholbruk og forventninger til virkninger av alkoholbruk. Én av virksomhetene som deltar i WIRUS er et forvaltningsorgan i Rogaland. I denne rapporten presenteres resultater fra spørreundersøkelsen blant ansatte i denne virksomheten. Målet med denne rapporten er å beskrive alkoholbruk, holdninger til arbeidsrelatert alkoholbruk og forventninger til virkninger av alkoholbruk blant ansatte i virksomheten. Alkoholbruk er kartlagt ved hjelp av Alcohol Use Disorders Identification Test (AUDIT), som er utviklet av Verdens helseorganisasjon (WHO). Virksomhetens resultater sammenliknes med de øvrige virksomhetene i WIRUS, og også resultater fra en tidligere utført befolkningsstudie i Norge. Rapporten kan brukes som et kunnskapsgrunnlag for arbeid med alkoholpolicy på arbeidsplassen, og for situasjoner i gråsonen mellom jobb og fritid. Resultatene for alkoholbruk viser at virksomhetens ansatte i gjennomsnitt rapporterte et noe høyere alkoholbruk enn ansatte i de øvrige virksomhetene i WIRUS (4,31 mot 4,00 av 40 mulige poeng på AUDIT-skalaen). Forskjellen var statistisk signifikant. Forekomsten av risikofylt alkoholbruk blant de ansatte var også noe høyere i virksomheten (12,5 %) enn hva som er funnet i de øvrige WIRUS-virksomhetene (11,4 %), men likevel lavere enn hva tidligere forskning har funnet i den generelle norske befolkningen (16,9 %). Forskjellen i andel risikodrikkere mellom virksomheten og de øvrige WIRUS-virksomhetene er imidlertid ikke statistisk signifikant. Virksomhetens ansatte rapporterte å ha noe mer positive (liberale) holdninger til arbeidsrelatert alkoholbruk enn ansatte i de øvrige WIRUS-virksomhetene (2,26 mot 2,14 av 4 poeng). Forskjellen var statistisk signifikant. Virksomhetens ansatte rapporterte også om marginalt mer positive forventninger til virkninger av alkoholbruk enn ansatte i de øvrige WIRUS-virksomhetene (1,80 mot 1,74 av 4 poeng), men denne forskjellen var ikke statistisk signifikant. Konklusjon og anbefalinger: Resultatene fra denne undersøkelsen tyder på at virksomhetens ansatte hadde noe høyere alkoholforbruk og noe mer positive (liberale) holdninger til jobbrelatert alkoholbruk enn alle ansatte i virksomhetene som deltar i WIRUS-prosjektet. Forekomsten av risikofylt alkoholbruk og forventninger til virkninger av alkoholbruk var imidlertid ikke signifikant forskjellig. Det var også svært få ansatte med høy risiko i virksomheten, og ingen som skåret ut med trolig avhengighet. Denne undersøkelsen gir i så måte ikke grunnlag for å anbefale særskilte tiltak for denne virksomheten, utover det som generelt kan betraktes som hensiktsmessig alkoholforebyggende arbeid i arbeidslivet.
