Perinatal drug abuse is becoming a profound issue facing the health and wellbeing of neonates. The community serviced by the project site, which lies within the boundaries of an Indian Reservation, suffers from perinatal drug abuse at a higher rate than state and federal averages. The purpose of this project was to provide the project site with a policy to consistently screen for perinatal drug abuse. Lave's theory of situational learning and the Sanford Way model for quality improvement framed this project. To guide policy development, data were compiled through a systematic review of current literature, national and state guidelines, state law, local tribal government, and community stakeholders. Data included: (a) studies completed in the past 10 years specifically targeting drug abuse in child-bearing aged women, with intentional exclusion of tobacco and alcohol studies; (b) prevalence of illicit drug abuse in child bearing aged women at a local, state, and national levels; and (c) local, state, and national guidelines, as well as state law, for perinatal drug abuse intervention and screening. In addition, interviews and meetings with local stakeholders were completed and their feedback was incorporated into the development of the perinatal drug abuse screening and intervention policy. To evaluate policy effectiveness, it is proposed that perinatal drug screens ordered at the project site be monitored for six months prior to and after implementation of the new policy. The desired outcome will be that providers consistently intervene with perinatal drug abuse in a non-biased fashion. This quality improvement project will create a positive social change by allowing non-biased intervention with perinatal drug abuse using evidence-based practice and by promoting nursing-driven policy development.

Abstract (sommario):
The study explores drug abuse by learners in selected Grahamstown high schools. The study`s main concern is that drug abuse by learners is on the rise but yet there is less research on the drugs which are being used and the factors which motivate the learners to use such drugs. There are several drug abuse prevention strategies which are being implemented but yet they seem not to be very effective. It is in this frame of reference that this study saw it vital to focus on drug abuse by high school learners in Grahamstown. The study aims to identify the most commonly abused drugs by learners in the high schools of Grahamstown and the reasons why they use these drugs. The study made use of the mixed method research that is making use of both qualitative and quantitative research. The questionnaire was the instrument of data collection in quantitative data and interviews for the qualitative data. The packages which were used for the analysis of data include the Statistical Analysis System (SAS) V8 which was used to provide descriptive analysis and correlations. The findings of the study establish that the commonly abused drugs by learners are: alcohol with 58%, followed by cigarettes 22%, hookah-pipe 9%, dagga 7%. The main reasons why learners use drugs are: peer pressure, role models, availability, environment, and curiosity. Differences in drug use between male and female learners were noted. Twenty-five percent of the male learners and 16% of the female learners reported to be using drugs. In the view of these results, the researcher recommends that a comprehensive drug abuse prevention framework be formulated which focuses on preventing drug abuse at individual, family and community levels.

ENGLISH ABSTRACT : In this study we look at the syndemic of substance abuse and drug-related crime in the Western Cape province of South Africa. The intent of this study is to provoke critical thinking about the possibilities systems thinking and system dynamics posses for social and health challenges in a diverse and complex environment like that of South Africa, especially the Western Cape. This study ventures into cross-discipline work between Epidemiology, Biomathematics and System Dynamics, with the hope of encouraging researchers from different fields to collaborate in order to curb the scourge of substance abuse and drug-related crime in South Africa. Substance abuse and the associated health and social hazards such as drug-related crime is a major problem in the Western Cape. Drug-related crime cases reported by the South African Police Services (SAPS) for the Western Cape exhibited a 311.5% growth in the past decade. This highlights how the reduction of substance abuse and drug-related crime within theWestern Cape province, will be an elixir for the safety and development of the communities. The fight against substance abuse has been driven by a multi-sectorial approach involving several government departments, non-governmental organisations and communities. With systems thinking the assumption is that the world is systemic, which means that phenomena is understood to be an emergent property of the interrelated whole. Firstly, using non-linear ordinary differential equations, we formulate a deterministic mathematical model for the substance abuse and drug-related crime syndemic, evaluate the threshold number and use sensitivity analysis to analyze the model. Secondly, a dynamic system, called the Substance Abuse and Drug-related Crime in theWestern Cape (SADC-WC) system is constructed using the STELLA in order to explore and classify the underlying relationships and structures within the substance abuse and drug-related crime system. Both the sensitivity analysis, and the simulations of the SADC-WC system indicate that an increase of successful convictions will have a significant influence on the syndemic, and promise to reduce drug-related crime cases.
AFRIKAANSE OPSOMMING : In hierdie studie ondersoek on die syndemie (‘syndemic’) van dwelmmisbruik en dwelmverwante misdaad in die Wes-Kaap provinsie, in Suid-Afrika. Die moontlikhede wat sistemiese denke en dinamiese sisteme inhou vir sosiale en gesondheid kwale in ’n diverse en komplekse omgewing soos Suid-Afrika, word ondersoek. Hierdie studie waag interdisiplinêre werk tussen Epidemiologie, Biowiskunde en Dinamiese sisteme, met die hoop om navorsers van verskillende velde aan te moedig om saam te werk om die plaag van dwelmmisbruik en dwelm-verwante misdaad in Suid-Afrika te bekamp. Dwelmmisbruik en die gepaardgaande gesondheid en maatskaplike gevare soos dwelmverwante misdaad is ’n groot probleem in dieWes-Kaap. Die SAPD se vermelde dwelmverwante midaad het ’n groei van 311,5% ondergaan in die afgelope dekade, en is aanduidend vir hoe die beheer en beperking van dwelmmisbruik en dwelm-verwante misdaad in die Wes-Kaap provinsie bevordering van beide die veiligheid en ontwikkeling van die gemeenskap sal verseker. Dit beklemtoon hoe die vermindering van dwelmmisbruik en dwelm-verwante misdaad in dieWes-Kaapland, sal ’n elikser vir die veiligheid en ontwikkeling van die gemeenskappe. Tweedens, is ’n dinamiese sisteem genaamd die Middelmisbruik en dwelmverwante misdaad in dieWes-Kaap (SADC-WC) stelsel gebou met behulp van die STELLA platform om te verken en klassifiseer die onderliggende verhoudings en strukture binne die dwelmmisbruik en dwelm-verwante misdaad stelsel. Beide die sensitiwiteitsanalise, en die simulasies van die SADC-WC stelsel dui aan dat ’n toename in suksesvolle vonisse ’n beduidende invloed op die epidemies sal hê; en beloof om sake van dwelmverwante misdaad te verminder.

Northeast Tennessee has been disproportionately burdened by a high prevalence of opioid prescribing, prescription drug abuse, addiction, overdose, and Neonatal Abstinence Syndrome. The East Tennessee State University Prescription Drug Abuse and Misuse Working Group (PDAMWG) formed upon recognizing an urgent need for a collaborative, multi-faceted response to reduce prescription drug abuse. Composed of over 100 members from various agencies, organizations, and institutions, members of this highly interprofessional, university-sponsored working group include academic scholars, healthcare providers, pharmacists, elected officials, students, community members, and more. This collaboration has successfully generated multiple funded research projects and numerous evidence- and community-based initiatives targeting prescription drug abuse. This seminar will focus on substance abuse prevention, specifically the prevention of prescription drug abuse. In addition to providing a data supported summary of the epidemic in the region, this seminar will outline the historical development of the PDAMWG and describe past and present research and community-based initiatives. An emphasis will be on the continuous commitment of the PDAMWG to the development, dissemination, and implementation of evidence-based practices to reduce prescription drug abuse and improve population health in the region. This seminar will also document research as well as community-based outcomes of PDAMWG efforts. Consequently, participants will recognize the effectiveness of strong, cross-sector partnerships for population health improvement. Relatedly, participants will develop a concrete understanding of a collaborative approach that bridges academic research and community-based practice. Perhaps most importantly, participants will discover its potential for replication in other communities to support the achievement of maximum, evidence-based outcomes for various health concerns. Lastly, participants will be introduced to the underlying model of the PDAMWG, which visually depicts evidence-based strategies along the disease continuum. As a result, participants will appreciate the complexity of public health problems and the subsequent importance of a multi-pronged, evidence-based response to addressing them

The problem of drugs in Oman has assumed serious proportions in recent years. It leads to creation of an underground economy and is associated with loss of skilled manpower. However, the problem has several dimensions. It has direct and indirect adverse effects. In response to increases in both worldwide drug production and drug demand in Oman, law makers in Oman instituted legal measures designed to protect the health, welfare and finance of people from the effects of drug use. In addition, law makers in Oman have recognized that trade in illicit drugs is a global activity and that drug-related activities are associated with other criminal behaviour; therefore they have tried to honour the spirit of international conventions related to drug control. Thus, the Drug Act No. 17 was passed in March 1999 and came into force on 6th April 1999, to regulate the procedures regarding the trafficking and trade of drugs in Oman. The most striking feature of the new law is a 'death penalty. The death penalty would be applied to those charged with drug trafficking and smuggling, as laid down in article No 43. Therefore, the question is can the death penalty deter criminals from committing trafficking and smuggling in drugs? No research has been done to examine the effect of the new drug law in Oman. This thesis aims to fill that gap and investigate the impact of the new drugs law in Oman. Data collection for the study was carried out using three methods: questionnaire, semi-structured interview and documentary data from police files before and after the introduction of the death penalty for the period from 1st April 1996 to 31st March 2002. The total period covered was 72 months. Statistical reports and other research papers carried out in Oman and other countries were also reviewed. The results of testing a number of hypotheses indicated that since the introduction of the death penalty for drug offences, the numbers of drug arrests and the incidence of violence related to drug offences have increased. In addition, the smuggling of drugs by sea has increased. Moreover, heroin trafficking has increased since the introduction of the death penalty. The study found that there is a lack of knowledge about drugs (the types of drugs, punishment, addictions treatment and belief about drug effects) among both groups of participants, drug offenders and non-drug offenders. Finally, the findings of this study indicate that the introduction of the death penalty in Oman does not deter drug offenders from involvement in drug related offences in general and drug trafficking offences in particular.

This study addressed the efficacy of a multimodal substance abuse prevention program with elite-level athletes. By utilizing components of substance abuse information and education, coping skills training, and self-esteem strategies, the study examined the effect of these variables on the subsequent alcohol and drug use behaviors and attitudes of the participants.In addition, measures of self-esteem and stress were examined to determine the effect of prevention on these variables. Gender differences were examined regarding substance use behaviors and attitudes, and change score measures were utilized to observe for behavioral changes from pretest to follow-up testing.Results indicated no differences on any of the dependent measures. Utilizing a repeated measures multivariate analysis of variance (MANOVA), no significant differences in substance use behaviors and attitudes between athletes in the treatment and control conditions were found. There were also no gender differences reported on the dependent measures. In addition, chisquare (X2) analyses demonstrated no behavioral or attitudinal differences. A significant finding indicated that control group subjects had more substance abuse changes (both positive and negative) than the treatment condition. The hypotheses of the study were unconfirmed as a result of the statistical analyses.Limitations of the present study include the small sample size (although representative of the population), the small amount of actual reported use behaviors, and the length of the program (not sufficient time). It is recommended that future research in this area maintain the multimodal approach, while lengthening the time of implementation. Future dependent measures must be able to accurately detect small fluctuations in reported substance abuse behavioral changes.The survey data suggest that athletes at the elite level do demonstrate alcohol and drug use behaviors that may be detrimental to their personal and professional potential. Substance abuse education programs are necessary components of holistic prevention for athletes at all levels of training and competition. Future programs must consider the present study in the development of more successful and practical substance abuse education for athletes.

Three questions concerning drug use and its treatment in Cape Town are posed in an attempt to guide planning of the treatment system. Firstly, is drug use in Cape Town of a problematic nature and magnitude? An affirmative conclusion is reached here by analysis of relevant local studies but is rendered tentative by inadequacies of the data. Secondly, are the relevant professional medical-psychological treatment facilities able to meet the treatment needs of all drug abusers in Cape Town? Analysis of the treatment system by means of a questionnaire produced a negative conclusion which was necessarily tentative. Thirdly, what directions should future expansion or modification take in order that cost-effectiveness be maximised? Three areas, were investigated by review of the literature - should psychotherapy be included in drug abuse treatment? Can drug abusers be effectively treated alongside other psychiatric patients? Can drug abusers be effectively treated as outpatients? Affirmative conclusions were reached in each case. Suggestions are made for accurate matching of patients to in- or outpatient treatment situations.

School drug use has been attributed to the annual suspension of thousands of secondary adolescents, subsequently contributing to poor academics, low graduation rates, and continued school drug use. The purpose of this qualitative study was to examine the experiences of former adolescents who engaged in school drug use. Social learning theory suggests that behaviors observed from within the environment may later be modeled. Using Bandura's social learning theory, 10 participants, 18 years old and older responded to open-ended questions about how they processed school drug use. Using interpretative phenomenological analysis, the responses to the open-ended questions were coded and analyzed. Bandura's social learning theory supports the key findings. Findings indicated that former students reported being influenced by their environment and others around them, such as friends. Additional findings from the study suggest that boredom was another reason students engaged in school drug use. While a few students enrolled in a new school after being suspended for school drug use, the findings suggest that out-of-school suspensions are not meaningful to students. After returning to school from their suspension, students felt behind in their school work. Implications for social change include understanding potential influences of school drug use on students and their environment. This understanding can benefit schools, parents, and the community through interventions and proactive measures that target school drug use among adolescents.

Background: There is a widespread and increasing use of psychoactive prescription drugs, such as opioid analgesics, anxiolytics, hypnotics and anti-epileptics, but their use is associated with a risk of drug use disorder, misuse and abuse. Today, these are globally recognized and emerging public health concerns. Aim: The aim of this thesis is to estimate the prevalence of psychoactive prescription drug (PPD) use disorders, misuse and abuse, and to investigate the association with some potential risk factors. Methods: A study using register data from forensic cause of death investigations investigated and described cases of fatal unintentional intoxication with tramadol (Study I). Based on register data on spontaneously reported adverse drug reactions (ADRs) reported cases of tramadol dependence were investigated and summarised (Study II). In a study in suspected drug-impaired drivers with a toxicology analysis confirming the intake of one out of five pre-specified PPDs, the prevalence of non-prescribed use was assessed and associated factors were investigated (Study III). From a cohort of patients initiating prescribed treatment with pregabalin, using data on prescription fills, a study investigated longitudinal utilisation patterns during five years with regards to use of the drug above the maximum approved daily dose (MAD), and factors associated with the utilisation patterns (Study IV). Results: In the first study, 17 cases of unintentional intoxications were identified, of which more concerned men, the median age was 44 years and the majority used multiple psychoactive substances (alcohol, illicit drugs and prescription drugs). The second study identified 104 spontaneously reported cases of tramadol dependence, in which more concerned women, the median age was 45 years, and a third reported a history of substance abuse and 40% of past psychoactive medication use. In the third study, more than half of the individuals suspected of drug-impaired driving used the drug without a recent prescription. Non prescribed use was most frequent in users of benzodiazepines and tramadol, and was more likely in younger individuals and in multiple-substance users. In the last paper five longitudinal utilisation patterns were found in pregabalin users, with two patterns associated with a particularly high risk of doses above the maximum approved dosing recommendation. This pattern of use was associated with male sex, younger age, non-urban residency and a recent prescribed treatment with an antiepileptic or opioid analgesic drug. Conclusions: This thesis shows that psychoactive prescription drug use disorders, misuse and abuse occur and may have serious and even fatal consequences. The prevalence varies between different drugs and populations. Abuse and misuse seem to be more common in young people. Fatal intoxications and misuse of prescribed drugs may be more common in men, while drug use disorders following prescribed treatment may be more common in women and non-prescribed use equally distributed between women and men. Individuals with a history of mental illness, substance use disorder or abuse, or of past use of psychoactive medications are likely important risk groups. In summary, the findings suggest a potential for improvements in the utilisation of psychoactive prescription drugs. The results may be useful in the planning of clinical and regulatory preventive interventions to promote the rational, individualised and safe use of such drugs.

Currently, many methods are available for the analysis of drugs of abuse in urine but they all have their drawbacks. Thus, the purpose of this research was to overcome some of these drawbacks by developing multi-analyte detection systems based on sequential or spatial techniques and immunoassays. The first system was based on a spatial technique and involved a simple indirect competitive ELISA format. This produced relatively rapid multi-analyte dip-strip ELISAs for benzoylecgonine (BE), methadone (MET) and morphine (MOR). Various enzyme-labelled antibodies, substrates and filters were investigated. A multi-analyte dip-strip assay was developed based on cellulose nitrate filters, alkaline phosphatase labelled anti-mouse second antibody and nitro blue tetrazolium / 5-bromo-4-chloro-3- indoyl phosphate (NBT /BCIP) substrate. The resulting assays gave a simple 'yes/no' result when drug was present or absent from a sample at concentrations of 1.45 f,lg ml-I , 1.55 f,lg ml-I and 1.43 f,lg ml-I for BE, MET and MOR respectively. Limitations however were encountered that caused the concentrations to be above the accepted cutoff levels for these three drugs of abuse. The second system was based on a sequential technique and involved a flow-injection nnmunoassay (FIlA). Various monoclonal antibodies, fluorotracers and immobilisation methods were investigated. For morphine, a novel simple FIlA was developed which is based on competition between a mixture of a fluorescein derivative of the drug and morphine in flow over low affinity monoclonal morphine antibodies immobilised on a N-hydroxysuccinimidyl chloroformate activated agarose immunoreactor. With this system, a split peak profile (unbound and retarded fractions) was observed under isocratic conditions with the retarded peak disappearing and the unbound peak increasing in peak height/area as the concentration of morphine increased. Using a flow-rate of 0.5 ml min-I and a fluorescein derivative dilution of 1: 100, this assay had a sample throughput of 4 samples h-I and a detection limit of 14.1 f,lg ml- I . For a flow-rate of 1.6 ml min-I and a fluorescein derivative dilution of 1: 1 00,the assay had a sample throughput of 6 samples h-I and a detection limit of 10.9 J.!g mri. The origin of the phenomenon was investigated and revealed to be due to the low association rate of the drug tracer with the morphine antibody used and the near equivalence of the monoclonal antibody affinity for its respective tracer and drug. It was found that when these values are exceeded, the "split peak" phenomenon was not observed but the reagents could be used in conventional displacement flow injection fluoroimmunoassays as was demonstrated for benzoylecgonine and methadone.

The thesis investigates the dynamics that surround participants' responses to questions about illicit drug activities. By examining the attributional and discursive literatures, the opening chapters (Chapters 1 and 2) outline the difficulties associated with assuming veridicality in question-answer dyads. Emphasis is placed on the essentially social and intentional foundations of the applied research procedure. The existing research on methodological effects in substance research is outlined at the start of Chapter 3. These form the foundation for the empirical investigations that constitute the remainder of the thesis. The studies carried out attempt to examine methodological issues in the context of applied research procedures that combine quantitative outcomes with qualitative considerations such as reflexive consideration of the role of the researcher and the status of the participant. The first investigation demonstrates the influence of treatment status on the discourse provided by adult substance users. Drug users in contact with treatment services provide drug-related explanations distinct from those given by users who are not in treatment. This distinction is assessed in terms of a theoretical model of addiction based on discursive criteria and contextual influence (Chapter 4). These contextual influences are further examined in the empirical studies presented in Chapters 4 and 5 in which the subjects are young people whose drug experiences are assessed in the context of drug education (Chapter 5) and treatment and service needs (Chapter 6). Each of these investigations attempts to demonstrate the sophistication of discourse that respondents exhibit in their drug-related conversations and the ways in which their attitudes and understandings of these topics are shaped by the context of the experiences they have had.

Cocaine is a widely abused drug without an FDA-approved medication. It has been recognized as an ideal anti-cocaine medication to accelerate cocaine metabolism producing biologically inactive metabolites via a route similar to the primary cocaine-metabolizing pathway, i.e. butyrylcholinesterase (BChE)-catalyzed hydrolysis. However, the native BChE has a low catalytic activity against cocaine. We recently designed and discovered a set of BChE mutants with a high catalytic activity specifically for cocaine. An ideal, therapeutically valuable mutant of human BChE should have not only a significantly improved catalytic activity against cocaine, but also certain selectivity for cocaine over neurotransmitter acetylcholine (ACh) such that one would not expect systemic administration of the BChE mutant to interrupt cholinergic transmission. Through integrated computational-experimental studies, several BChE mutants were identified to have not only a considerably improved catalytic efficiency against cocaine, but also the desirable selectivity for cocaine over ACh. Representative BChE mutants have been confirmed to be potent in actual protection of mice from acute toxicity (convulsion and lethality) of a lethal dose of cocaine (180 mg/kg, LD100). Pretreatment with the BChE mutant (i.e. 1 min prior to cocaine administration) dose-dependently protected mice against cocaine-induced convulsions and lethality. The in vivo data reveal the primary factor, i.e. the relative catalytic efficiency, determining the efficacy in practical protection of mice from the acute cocaine toxicity and future direction for further improving the efficacy of the enzyme in the cocaine overdose treatment. For further characterization in animal models, we successfully developed high-efficiency stable cell lines efficiently expressing the BChE mutants by using a lentivirus-based repeated-transduction method. The large-scale protein production enabled us to further characterize the in vivo profiles of the BChE mutant concerning the biological half-life and potency in accelerating cocaine clearance. In particular, it has been demonstrated that the BChE mutant can rapidly metabolize cocaine and completely eliminate cocaine-induced hyperactivity in rodents, implying that the BChE mutant may be developed as a promising therapeutic agent for cocaine abuse treatment.

Background This paper provides an updated review of the literature on drug prevention programmes (from 2001 to July2011) and reports the findings from these previous studies on the effectiveness and nature of present drug prevention practices. Relevant data on the effectiveness of drug prevention intervention among adolescents has been summarised and examined. In addition, this paper identifies various essential elements that have the potential for creating and providing effective drug prevention strategies, whether to prevent substance misuse or to minimise the harm caused. Different prevention strategies will also be discussed, including: social influence approach, refusal skill training, and motivational interviewing. Design The paper will conduct a review of the previous literature. Aims This literature review proposes to: 1. Review all of the published evidence from research which was conducted between 2001and 2011 on the effectiveness of drug prevention programmes for adolescents. 2. Identify findings and recommendations regarding the content, approaches, format, theoretical bases, and methods associated with an effective drug prevention program. 3. Discuss any potential or proved effective components on a drug prevention program in tackling such drug abuse problems. Methods Two searching engines (i.e. PubMed and Medline) were used to find the relevant papers and journals which have been published within the past ten years (i.e. 2001 to 2011). Studies about the evaluation of the effectiveness of drug education and prevention strategies, targeting the most vulnerable group (i.e. aged between 12 and 25) were included. Results Twenty four studies met the criteria and were reviewed in depth. Thirteen of them evaluated the effectiveness of school-based interventions, including the school-based drug testing program. The remaining papers evaluated the effectiveness of non-school based prevention interventions in different approaches such as social influence, parental cooperation, refusal skills education, and life skills training. Conclusions Social influence approaches were evidenced as the most effective intervention to prevent drug abuse. Interventions that are conducted interactively (e.g. simulated scenario and role-playing) are more effective than providing normative information. Gender differences were found in some of the interventions. However, it is recommended that further research should be conducted to evaluate these results.

Purpose: To evaluate the effectiveness of the West Virginia Prescription Drug Abuse Quitline (WVPDAQ). Methods: Descriptive data and inferential analyses are provided for the period of operations from 9/11/2008 to 3/1/2010. Chi-square tests for Independence compared differences between callers completing at least one follow-up survey against callers completing only the baseline intake survey were observed. A Wilcoxon signed rank sum test tested differences between the scores of intake callers and follow-up callers at the one-week (n=177) and one-month (n=89) intervals. Results: A total of 1,056 calls were received, including 670 intakes, 177 1st follow-ups, 89 2nd follow-ups, and 36 3rd follow ups, and 84 caller satisfaction surveys. Chi-square analyses determined that callers who only completed the intake survey tended to have initiated drug use at a younger age ([less than or equal to] 39 years of age, [chi square]=7.63, p=.02). Longitudinal findings indicated significant self-reported declines in daily drug use (p<.0001), increased intentions to quit in the next 30 days (p<.0001), and declines in requesting a referral for treatment (p<.0001) at the one-month follow-up. Finally, approximately 19% (n=17) of callers reported obtaining a counseling/ treatment appointment as a result of calling the WVPDAQ at the one-month follow up. Conclusions: Preliminary results suggest the WVPDAQ has established itself as a meaningful resource to combat prescription drug abuse in West Virginia. Sustainable funding and greater integration of the WVPDAQ into existing and developing treatment infrastructure could provide more affordable options for the state of West Virginia and its citizens.
