RÉSUMÉ : Cet article propose que l’équivalence postulée entre la descente infinie et l’induction transfinie dans les fondements de l’arithmétique fait intervenir le principe du tiers exclu par la double négation sur l’ensemble infini des nombres naturels et ne saurait donc être admissible du point de vue de la logique et des mathématiques intuitionnistes. Si, par ailleurs, on adopte le point de vue de la logique classique, les principes de l’induction complète, de l’induction transfinie, du plus petit nombre et de la descente infinie sont tous équivalents; pourtant, la descente infinie en jeu dans l’arithmétique ensembliste de Dedekind-Peano ne correspond pas à la descente infinie de Fermat en théorie des nombres et en arithmétique classique de Gauss jusqu’à nos jours. C’est là le point d’ancrage d’une critique fondationnelle qui cherche à mieux définir les options philosophiques dans les fondements de la logique et des mathématiques.

Chez les Matsigenka de l’Amazonie péruvienne, l’opposition topologique fondamentale entre l’amont (katonko) et l’aval (kamatikya) des cours d’eau est à l’intersection d’un double dualisme. Amont et aval sont en effet des lieux associés, d’une part, à une distinction entre le masculin et le féminin, et, d’autre part, à deux types d’altérités sociales opposées. À partir de l’analyse du répertoire mythologique et de certaines pratiques rituelles, l’article montre que ces oppositions confrontent les Matsigenka aux bouleversements sociohistoriques actuels. En effet, portée par la force du courant, la descente inexorable des individus vers l’aval est le signe d’une cannibalisation des Matsigenka par les colons et les Amérindiens belliqueux situés dans ces mondes inférieurs. C’est là un destin funeste qui, dans une perspective vraisemblablement androcentrée, est décrit comme l’englobement du masculin par le féminin. Le sens de l’écoulement des eaux est également celui du sens tragique de l’histoire qui suscite le désir anxieux de remonter vers les espaces supérieurs occupés par les populations des Andes et leurs influentes divinités. Ce parcours idéalisé à contre-courant serait une manière de ralentir la décomposition accélérée du monde matsigenka, en restant éloignés des espaces du bas dominés par l’inquiétante étrangeté du monde présent.

Double-descent curves in neural networks describe the phenomenon that the generalisation error initially descends with increasing parameters, then grows after reaching an optimal number of parameters which is less than the number of data points, but then descends again in the overparameterized regime. In this paper, we use techniques from random matrix theory to characterize the spectral distribution of the empirical feature covariance matrix as a width-dependent perturbation of the spectrum of the neural network Gaussian process (NNGP) kernel, thus establishing a novel connection between the NNGP literature and the random matrix theory literature in the context of neural networks. Our analytical expressions allow us to explore the generalisation behavior of the corresponding kernel and GP regression. Furthermore, they offer a new interpretation of double-descent in terms of the discrepancy between the width-dependent empirical kernel and the width-independent NNGP kernel.

AbstractManifera talaris, a voltzian conifer from the late early to middle Permian (ca. 270 Ma) of Texas, is the earliest known conifer to produce winged seeds indicative of autorotating flight. In contrast to autorotating seeds and fruits of extant plants, the ones ofM. talarisare exceptional in that they have variable morphology. They bore two wings that produced a range of wing configurations, from seeds with two equal-sized wings to single-winged specimens, via various stages of underdevelopment of one of the wings. To examine the effects of various seed morphologies on aerodynamics and dispersal potential, we studied the flight performance of paper models of three morphotypes: symmetric double-winged, asymmetric double-winged, and single-winged. Using a high-speed camera we identified the mode of descent (plummeting, gliding, autorotation) and quantified descent speed, autorotation frequency, and other flight characteristics. To validate such modeling as an inferential tool, we compared descent of extant analogues (kauri;Agathis australis) with descent of similarly constructed seed models. All three seed morphotypes exhibited autorotating flight behavior. However, double-winged seeds, especially symmetric ones, failed to initiate slow autorotative descent more frequently than single-winged seeds. Even when autorotating, symmetric double-winged seeds descend faster than asymmetric double-winged ones, and descent is roughly twice as fast compared to single-winged seeds. Moreover, the relative advantage that (effectively) single-winged seeds have in slowing descent during autorotation becomes larger as seed weight increases. Hence, the range in seed wing configurations inM. talarisproduced a wide variation in potential dispersal capacity. Overall, our results indicate that the evolutionarily novel autorotating winged seeds must have improved conifer seed dispersal, in a time when animal vectors for dispersion were virtually absent. Because of the range in wing configuration, the early evolution of autorotative flight in conifers was a functionally imperfect one, which provides us insight into the evolutionary developmental biology of autorotative seeds in conifers.

Abstract. In situ upper-air measurements are often made with instruments attached to weather balloons launched at the surface and lifted into the stratosphere. Present-day balloon-borne sensors allow near-continuous measurements from the Earth's surface to about 35 km (3–5 hPa), where the balloons burst and their instrument payloads descend with parachutes. It has been demonstrated that ascending weather balloons can perturb the air measured by very sensitive humidity and temperature sensors trailing behind them, particularly in the upper troposphere and lower stratosphere (UTLS). The use of controlled balloon descent for such measurements has therefore been investigated and is described here. We distinguish between the single balloon technique that uses a simple automatic valve system to release helium from the balloon at a preset ambient pressure, and the double balloon technique that uses a carrier balloon to lift the payload and a parachute balloon to control the descent of instruments after the carrier balloon is released at preset altitude. The automatic valve technique has been used for several decades for water vapor soundings with frost point hygrometers, whereas the double balloon technique has recently been re-established and deployed to measure radiation and temperature profiles through the atmosphere. Double balloon soundings also strongly reduce pendulum motion of the payload, stabilizing radiation instruments during ascent. We present the flight characteristics of these two ballooning techniques and compare the quality of temperature and humidity measurements made during ascent and descent.

Cet article propose une réflexion sur la condition noire en interrogeant l’application du concept de « double conscience » duboisien dans le contexte français contemporain. Il participe d’une étude qualitative et inductive réalisée sur le modèle de l’enquête de terrain et qui s’appuie sur des entretiens non directifs réalisés auprès de personnes afro-descendantes françaises. S’inscrivant dans une démarche interdisciplinaire, l’analyse psychanalytique que nous proposons se veut à la croisée de regards socio-historiques et philosophiques sur la condition noire en contexte français. Cette analyse met en lumière la prépondérance de la ligne de partage des couleurs dans la structure de nos sociétés modernes. Partant de là, elle rend compte de l’implication de cette ligne dans les processus de subjectivations des personnes afro-descendantes, de l’identification à la blancheur à la révolte sociale en passant par la guerre contre soi-même.

ABSTRACT Purpose To report a case of double descemet membrane forming a double anterior chamber following ‘Big bubble’ DALK converted to PKP. Results A 52-year-old man with keratoconus underwent DALK operation converted to PKP due to macro perforation of the descemet membrane. On the postoperative evaluation a remnant of the host's descemet membrane formed a double anterior chamber. After a few months with no intervention, an attempt to deflect a thickened descemet membrane had failed, making entrance into the anterior chamber in order to excise the membrane inevitable. Two years following the second surgery a graft endothelial failure and cataract formation was documented, leading to a second corneal transplantation and lens implantation. Conclusion Double DM is an unusual condition following a DALK procedure which was converted into PKP. In the above case this complication resulted in multiple procedures and graft failure. We believe that early interventions such as gas injection or YAG laser could have resulted in a more favorable outcome. How to cite this article Graffi S, Naftali M. Eur J Endocrinol 1994;1 30:60–4. ISSN 0804–4643 The medical treatment of retentio testis remains controversial because of ineffectiveness and/or adverse events. Follicle-stimulating hormone (FSH) seems to influence the spontaneous descent of the testis; furthermore, it induces luteinizing hormone (LH) receptors. Therefore, we performed a double-blind placebo-controlled study to investigate the effect of FSH with human chorionic gonadotrophin (hCG) versus hCG alone in retentio testis patients. Twenty-two boys with retentio testis were investigated, excluding retractile testis. Group A (N= 14: four with bilateral and 10 with unilateral retentio testis; mean age 3.1 5 years) was treated with 150 IU of FSH twice a week for 2 weeks followed by 1 50 IU of FSH and 250 IU of hCG (half the recommended World Health Organization dose) twice a week for another 4 weeks. Group B (N = 8: two with bilateral and six with unilateral retentio testis: mean age 3.3 years) was treated with 250 IU of hCG twice a week for 6 weeks. Testicular position, volume and consistency as well as the appearance of the scrotum and the penile length were determined at the start of the treatment as well as at weeks 2.4.6 and 12 by two independent investigators. Blood investigation consisted of measurements of LH, FSH, testosterone and sex hormone-binding globulin. Successful descent was considered when the testis reached a mid- or low scrotal position. In group A, 6/18 testes descended successfully. In group B, 6/10 testes descended. Of the unsuccessfully treated patients, six of group A and three of group B underwent surgery. Of these patients, 6/8 testes of group A and all testes of group B showed anatomical abnormalities, which could explain the lack of hormonal response. There were no significant differences in hormonal parameters between the two groups. In both groups no serious adverse events were mentioned or observed. In conclusion, half the recommended WHO dose of hCG is sufficient to reach successful descent in 43% of treated patients, with no serious adverse events; this response rate is in agreement with the literature. Follicle-stimulating hormone does not seem to have an additional effect on the success rate, and most of the unsuccessfully treated patients showed anatomical abnormalities at operation. Objective. To evaluate the efficacy and safety of CHDI in the treatment of influenza infection.Method. A randomized double-blind, double dummy trial was conducted. Influenza patients with a positive influenza A rapid test diagnosis were randomized to receive CHDI or oseltamivir. Primary outcome was assessed by the median fever alleviation time and clearance time, and secondary outcome was total scores of influenza symptoms.Results. One hundred thirty-nine participants were screened and 34 had a RT-PCR laboratory confirmation of influenza virus infection. Fever alleviation time was 2.5 and 5 hours in CHDI and oseltamivir, respectively, and fever clearance time was 32.5 and 49 hours. The HR of fever alleviation and clearance time shows no significant difference between two groups. Total scores of influenza symptoms descended significantly in both groups after treatment and descended more in CHDI than oseltamivir on day 2. Similar to total symptoms severity score, fever severity score descend more significantly in CHDI than oseltamivir on day 2, and there were no differences on other symptoms.Conclusions. CHDI have a similar effect to oseltamivir in reducing the duration of influenza illness. CHDI was well tolerated, with no serious adverse events noted during the study period.

Abstract We show that a variety of modern deep learning tasks exhibit a ‘double-descent’ phenomenon where, as we increase model size, performance first gets worse and then gets better. Moreover, we show that double descent occurs not just as a function of model size, but also as a function of the number of training epochs. We unify the above phenomena by defining a new complexity measure we call the effective model complexity and conjecture a generalized double descent with respect to this measure. Furthermore, our notion of model complexity allows us to identify certain regimes where increasing (even quadrupling) the number of train samples actually hurts test performance.

Abstract. Spatiotemporal characteristics of the ozone quasi-biennial oscillation (QBO) over the tropical-subtropical stratosphere (40° S–40° N) have been examined by analyzing data from the Microwave Limb Sounder (MLS) aboard Upper Atmospheric Research Satellite (UARS) for the period 1992–1999. A combination of regression analysis and wavelet analysis combines to act as an accurate QBO filter. Wavelet analysis provides inter-annual variability of amplitude and phase of the ozone QBO in the vertical structure of tropical-subtropical stratosphere. It gives minute details of phase propagation and descend rates, which can be used as input to models. Latitude-height structure shows evidence of a secondary meridional circulation induced by the QBO as double peak structure at the equator with maximum amplitude at two pressure levels 30 hPa and 9 hPa and a node at 14 hPa. The equatorial maxima are out of phase with each other. The maximum amplitude (~1.4 ppmv) of the ozone QBO was observed near the equator at 10 hPa. Descent rate of the easterly phase is greater than westerly. The lag correlation of the ozone QBO with circulation and variation of descent rates in the vertical structure of the stratosphere are examined in detail. In the equatorial upper stratosphere ozone anomalies descent with the rate ~1.5 km/month but in tropics and subtropics (above 2 hPa) they propagate upward.

This research proposes and investigates some improvements in gradient descent iterations that can be applied for solving system of nonlinear equations (SNE). In the available literature, such methods are termed improved gradient descent methods. We use verified advantages of various accelerated double direction and double step size gradient methods in solving single scalar equations. Our strategy is to control the speed of the convergence of gradient methods through the step size value defined using more parameters. As a result, efficient minimization schemes for solving SNE are introduced. Linear global convergence of the proposed iterative method is confirmed by theoretical analysis under standard assumptions. Numerical experiments confirm the significant computational efficiency of proposed methods compared to traditional gradient descent methods for solving SNE.

A reduction of the originally double step size iteration into the single step length scheme is derived under the proposed condition that relates two step lengths in the accelerated double step size gradient descent scheme. The proposed transformation is numerically tested. Obtained results confirm the substantial progress in comparison with the single step size accelerated gradient descent method defined in a classical way regarding all analyzed characteristics: number of iterations, CPU time, and number of function evaluations. Linear convergence of derived method has been proved.

A result of André Weil allows one to describe rank $n$ vector bundles on a smooth complete algebraic curve up to isomorphism via a double quotient of the set $\text{GL}_{n}(\mathbb{A})$ of regular matrices over the ring of adèles (over algebraically closed fields, this result is also known to extend to $G$-torsors for a reductive algebraic group $G$). In the present paper we develop analogous adelic descriptions for vector and principal bundles on arbitrary Noetherian schemes, by proving an adelic descent theorem for perfect complexes. We show that for Beilinson’s co-simplicial ring of adèles $\mathbb{A}_{X}^{\bullet }$, we have an equivalence $\mathsf{Perf}(X)\simeq |\mathsf{Perf}(\mathbb{A}_{X}^{\bullet })|$ between perfect complexes on $X$ and cartesian perfect complexes for $\mathbb{A}_{X}^{\bullet }$. Using the Tannakian formalism for symmetric monoidal $\infty$-categories, we conclude that a Noetherian scheme can be reconstructed from the co-simplicial ring of adèles. We view this statement as a scheme-theoretic analogue of Gelfand–Naimark’s reconstruction theorem for locally compact topological spaces from their ring of continuous functions. Several results for categories of perfect complexes over (a strong form of) flasque sheaves of algebras are established, which might be of independent interest.

If they do, it is unknown if there is an overlap of clinical features or prognostic implications. There are a few reported cases of overlap between these antibodies, mostly reported in patients with Japanese descent. Our aim for this case report is to turn more attention and interest for future MSA profile studies in the Hispanic population, which may hopefully spur better therapies if we realize the prognostic implications of certain myositis subsets including double-positive autoantibody syndromes. <b>Case presentation:</b> A 27-year-old Hispanic female was admitted to the medical intensive care unit due to acute hypoxemic respiratory failure secondary to acute respiratory distress syndrome (ARDS). She had failed conventional mechanical ventilation and was cannulated for venovenous extracorporeal membrane oxygenation (VV-ECMO) to manage her respiratory failure. She had erythematous scaly plaques on bilateral 3rd metacarpophalangeal joints on examination. Her autoimmune workup revealed positivity for both anti-PL-7(threonyl) and anti-melanoma differentiation-associated gene 5 (MDA5) autoantibodies. After extensive evaluation, it was concluded that she had rapidly progressive interstitial lung disease (RPILD) due to amyopathic dermatomyositis. Despite maximal medical management, she was ultimately transitioned to comfort care measures and expired. <b>Conclusion:</b> We would like to highlight the rarity of double antibody positive amyopathic dermatomyositis. This unique clinical presentation has only been reported in persons of Japanese descent. Our case is likely to be the first reported to occur in a person of Hispanic descent in the United States. The rarity of our case could stimulate further study of overlapping MSA to understand its varied presentations and prognoses including possible tendency toward a rapidly progressive ILD phenotype. Earlier detection of these clinical syndromes can lead to better outcomes for patients with RPILD. This case report could also herald an increased recognition and understanding of MSA profile in the Hispanic population in the USA.

AbstractThe tumbling motion of freely falling plates with aspect ratio ranging from 2 to 10 is studied using both experimental measurement and simplified lifting line theory. The trajectories of plates are recorded by high-speed video camera and analysed to obtain instantaneous and average kinematic and aerodynamic parameters, including the descent angle, rotating speed, descent velocities, lift and drag coefficients, and their fluctuation spectrums. A double period rotation with period doubling is observed here for some range of aspect ratio and the double frequency is determined from a Fourier analysis. By adding a correction from the inducing effect of trailing vortices and wing-tip vortices to the corresponding two-dimensional force and torque expressions, a simplified kinematic model is obtained which successfully predicts the qualitative influence of aspect ratio on the averaged kinematics, that the descent angle and the vertical descent velocity component are decreasing functions of the aspect ratio, while the rotation speed and horizontal velocity component are increasing functions of the aspect ratio. For decreasing aspect ratio, the induced drag due to the trailing and tip vortices reduces the lift to drag ratio and thus increases the descent angle, while the dissipative torque due to translational induced drag decreases the rotation speed.

The optimization-based alignment (OBA) methods, which are implemented by the optimal attitude estimation using vector observations—also called double-vectors—have proven to be effective at solving the in-flight alignment (IFA) problem. However, the traditional OBA methods are not applicable for the low-cost strap-down inertial navigation system (SINS) since the error of double-vectors will be accumulated over time due to the substantial drift of micro-electronic- mechanical system (MEMS) gyroscope. Moreover, the existing optimal estimation method is subject to a large computation burden, which results in a low alignment speed. To address these issues, in this article we propose a new fast IFA method based on modified double-vectors construction and the gradient descent method. To be specific, the modified construction method is implemented by reducing the integration interval and identifying the gyroscope bias during the construction procedure, which improves the accuracy of double-vectors and IFA; the gradient descent scheme is adopted to estimate the optimal attitude of alignment without complex matrix operation, which results in the improvement of alignment speed. The effect of different sizes of mini-batch on the performance of the gradient descent method is also discussed. Extensive simulations and vehicle experiments demonstrate that the proposed method has better accuracy and faster alignment speed than the related traditional methods for the low-cost SINS/global positioning system (GPS) integrated navigation system

We study c<1 matter coupled to gravity in the Coulomb gas formalism using the double cohomology of the string BRST and Felder BRST charges. We find that states outside the primary conformal grid are related to the states of nonzero ghost number by means of descent equations given by the double cohomology. Some aspects of the Virasoro structure of the Liouville-Fock space are studied. As a consequence, states of nonzero ghost number are easily constructed by “solving” these descent equations. This enables us to map ghost number conserving correlation functions involving nonzero ghost number states into those involving states outside the primary conformal grid.

Nonnegative matrix factorization (NMF) has been used as a powerful date representation tool in real world, because the nonnegativity of matrices is usually required. In recent years, many new methods are available to solve NMF in addition to multiplicative update algorithm, such as gradient descent algorithms, the active set method, and alternating nonnegative least squares (ANLS). In this paper, we propose an inexact update method, with two parameters, which can ensure that the objective function is always descent before the optimal solution is found. Experiment results show that the proposed method is effective.

Matrilineal systems in sub-Saharan Africa tend to co-occur with horticulture and are rare among pastoralists, with the causal arrow pointing from the introduction of cattle to the loss of matriliny. However, most work on this topic stems from either phylogenetic analyses or historical data. To better understand the shift from matrilineal to patrilineal inheritance that occurred among Bantu populations after the adoption of pastoralism, data from societies that are currently in transition are needed. Himba pastoralists, who practice ‘double descent’, may represent one such society. Using multi-generational ethnography and structured survey data, we describe current norms and preferences about inheritance, as well as associated norms related to female autonomy. We find that preferences for patrilineal inheritance are strong, despite the current practice of matrilineal cattle inheritance. We also find that a preference for patriliny predicts greater acceptance of norm violating behaviour favouring sons over nephews. Finally, we show that there are important generational differences in how men view women's autonomy, which are probably attributable to both changing norms about inheritance and exposure to majority-culture views on women's roles. Our data shed light on how systemic change like the shifts in descent reckoning that occurred during the Bantu expansion can occur. Technical innovations in barley breeding have resulted in the ability to reduce generation times. Single seed descent and doubled haploidy have become practical propositions in barley improvement. These Fx samples are also powerful tools for genetical analyses and have been used in this paper together with conventional generations to investigate the linkage of genes controlling quantitative characters. Such investigations are of relevance to barley breeders since the detection of significant levels of repulsion linkages would indicate that the single seed descent (SSD) technique would be preferable to doubled-haploid (DH) methods. The present study indicated that estimates of D (the additive genetic variance) from DH and SSD sampled did not differ significantly. Thus linkage disequilibrium is not an important component of the genetic architecture of the five crosses investigated. Hence F1-derived doubled haploids or SSD will be equally effective in producing desirable recombinant inbred lines.Key words: Hordeum, doubled haploids, linkage disequilibrium.

Prostate cancer (CaP) is the leading cancer among men of African descent in the USA, Caribbean, and Sub-Saharan Africa (SSA). The estimated number of CaP deaths in SSA during 2008 was more than five times that among African Americans and is expected to double in Africa by 2030. We summarize publicly available CaP data and collected data from the men of African descent and Carcinoma of the Prostate (MADCaP) Consortium and the African Caribbean Cancer Consortium (AC3) to evaluate CaP incidence and mortality in men of African descent worldwide. CaP incidence and mortality are highest in men of African descent in the USA and the Caribbean. Tumor stage and grade were highest in SSA. We report a higher proportion of T1 stage prostate tumors in countries with greater percent gross domestic product spent on health care and physicians per 100,000 persons. We also observed that regions with a higher proportion of advanced tumors reported lower mortality rates. This finding suggests that CaP is underdiagnosed and/or underreported in SSA men. Nonetheless, CaP incidence and mortality represent a significant public health problem in men of African descent around the world.

Phenotypic correlation of seed yield and its component traits were studied in four selected irradiated F,M, population, two three way and one double cross populations of blackgram advanced through bulk, individual plant selection (IPS) and single pod descent (SPD) method. All the twenty one populations exhibited significant positive correlation of plant height, number of clusters and pods per plant with seed yield per plant except three populations of bulk and one population of IPS method for plant height. Irradiated population of single cross 4 for pod length, single cross 1, 3 and three way cross 1 for number of seeds per pod exhibited significant positive correlation with seed yield per plant irrespective of the methods of advancement. The nature of association between pod length in three way cross 1 and number of seeds per pod in double cross advanced through bulk method was changed from negative to positive in IPS and SPD methods. Among the three methods of selection, single pod descent method showed a stronger magnitude and higher number of significant correlation in all the seven populations under study as compared to bulk and IPS methods. Most of the traits under study including pod length and number of seeds per pod contributed towards seed yield per plant in single cross F, M,, three way and double cross populations.

Abstract: In this paper we analyze the social meanings associated with the new native vernacular (NNV) variety of Modern Hebrew as a complex positive stance, constructed via differentiation from its alternatives. NNV is reflexive, and it speaks for itself: for the authority of experience, as opposed to the traditional authority of the text. A speaker of NNV is necessarily an active agent in the propagation of the new collective and its values. We also explore the consolidation and dissemination of these values by cultural agents, focusing on a 1950s column by Dahn Ben Amotz, which presents snapshots of "everyday life" in multiple sites in Israel, as part of the modernist project of constructing a hegemonic folk identity. We show how variation in the use of spoken Hebrew, together with other tropes such as location and ethnic descent, are implicated in the construction of the new folk identity.

We present an error reconciliation method for Quantum Key Distribution (QKD) that corrects 100% of errors generated in regular binary frames transmitted over a noisy quantum channel regardless of the quantum channel error rate. In a previous investigation, we introduced a novel distillation QKD algorithm whose secret key rate descends linearly with respect to the channel error rate. Now, as the main achievement of this work, we demonstrate an improved algorithm capable of retaining almost all the secret information enclosed in the regular binary frames. Remarkably, this technique increases quadratically the secret key rate as a function of the double matching detection events and doubly quadratically in the number of the quantum pulses. Furthermore, this reconciliation method opens up the opportunity to use less attenuated quantum pulses, would allow greater QKD distances at drastically increased secret key rate. Since our method can be implemented as a software update, we hope that quantum key distribution technology would be fast deployed over global data networks in the quantum era.

Deep anterior lamellar keratoplasty (DALK) is a feasible option in children with advanced keratoconus with or without stromal scarring. Descemet membrane detachment (DMD) and formation of double anterior chamber is one of the rare but serious complication of DALK surgery. Various treatment methods like descemetopexy with air, perfluoropropane gas (C3F8), and SF6 (sulfur hexafluoride gas) can be tried. We report a case of spontaneous reattachment of DMD in a pediatric male patient with Descemet membrane microperforation during DALK.

Purpose. To evaluate femtosecond laser in DSAEK surgery as an improvement to manual DSAEK. Settings. Department of Ophthalmology, Cairo University. Design. A retrospective observational clinical study. Methods. 20 eyes with SBK and Fuchs’ dystrophy underwent a Femto-assisted DSAEK by laser cutting of two matching posterior stromal discs in the recipient and donor corneas and then fitting the donor disc in the posterior corneal defect of the recipient using Busin’s glide or Terry forceps. Results. Corneal thickness decreased significantly from a mean of 900-micron preoperative values (900.7 m) to 562 m postoperatively. Evidence of side healing was documented by OCT. One patient had a double AC, one patient had an air interface entrapment “Double Bubble,” one patient had a fungal infection and was treated by a therapeutic penetrating keratoplasty, and one patient had a CMO. Conclusion. Femtolaser-assisted DSAEK may be superior to manual techniques as it offers better centration, thinner graft/host complex, earlier corneal detergecense, and stronger healing.
