Abstract (sommario):
Introduction:Over the past 20 to 30 years, alternative dementia care models have been developed. Dementia villages challenge popular perceptions about life with dementia and contrast to the traditional model of long-term care facilities that are often seen as institutional, impersonal, and risk-averse. The first dementia village, De Hogeweyk, was developed in 2009 and is located in Weesp, Netherlands. Hogeweyk aims to create a safe environment, enabling the person with dementia to live an “ordinary life” with as much autonomy as possible and also maintaining integration with the local community. Other dementia villages have been established in several countries, following De Hogeweyk model. Objectives:The aim of this presentation is to describe the functioning of dementia villages and evaluate its benefits on dementia patients. Methods:A non-systematic review of the literature was performed on PubMed, PsycINFO and Web of science using selected keywords. We also consult the official websites of the institutions. Results:Dementia villages seem to improve functioning and reduce the need for medication. Anxiety, restlessness and homesickness can still persist, but are reduced by the homelike and hospitable setting in which residents live. In fact, antipsychotic medication use at the residence has decreased from approximately 50% of residents, before the dementia village was introduced, to approximately 12% in 2019. The staff also reported greater job satisfaction. Although dementia villages are growing throughout the Western world, this concept has also been criticized, arguing that this type of living is dishonest, misleading the residents to believe that they are still living in the ‘real community’.Conclusions:Dementia villages are guided by the principles “deinstitutionalize, transform and normalize” care for people with advanced dementia. Although its intuitive advantages, there is no research evidence to demonstrate that this environment has any beneficial effect in behaviour, functional ability or cognition. In future studies, clinical outcomes could be used as a measure of quality of care. Hogeweyk concept has made societies rethinking dementia care and has been inspiring the development of other innovative models of dementia care.

Abstract (sommario):
This non-synchronized design research tested the effects of dementia prevention program on cognitive and emotional functions, and activities of daily living of subjects in The Dementia Safety Village by National Responsibility System for Dementia. The subjects were 11 elders at local elderly center of The Dementia Safety Village. Outcome variables include cognition (MMSE-DS: Mini-Mental State Examination-Dementia Scale), emotion (SGDS-K: Short form Geriatric Depression Scale-Korean version), and physical function (IADL: Instrumental Activity of Daily Living). The program provided two hours per a session per week for eight weeks. After completing the program, the score of MMSE-DS, a tool for cognitive function evaluation, increased with a statistically significant figure (t = –3.35, p = 0.007). The score of SGDS-K, which evaluated emotional function, increased, though it did not reach in a statistically significant level (t = –0.482, p = 0.640). The score of IADL showed no a statistically significant figure (t = 0.680, p = 0.512), but the mean showed a slight decreased figure (pre 12.00 ± 4.49, post 11.00 ± 2.19). Based on these results, the dementia program may be used for dementia prevention to the patients of The Dementia Safety Village. This research suggests that an appropriate program for the prevention of dementia needed to be developed further in the future.

An 18-year-old girl from a remote village in Central India presented with rapidly worsening forgetfulness and poor academic performance. A magnetic resonance imaging revealed multiple neurocysticerci in the brain at different stages of development. She received antihelmintic therapy along with corticosteroids, resulting in significant improvement in her dementia symptoms. This case highlights the importance of early intervention in rapidly progressive dementia to promptly address treatable causes and reduce morbidity. It further emphasizes the diagnosis of neurocysticercosis as a cause of dementia.

Abstract (sommario):
Background: The prevalence of elderly people continues to increase every year. This is directly proportional to the problems that continue to occur. One of them is a decrease in cognitive abilities which results in dementia. One of the risk factors for dementia is the level of physical activity. Decreased physical activity is believed to be one of the causes of dementia in the elderly, along with other risk factors. Objective: To determine the relationship between physical activity levels and risk factors for dementia in elderly posyandu participants in RW 03, Dinoyo Village. Research Method: This research uses descriptive analytical methods with a cross-sectional design. The method with sampling of both variables is carried out in the same time period. The total sample was 51 elderly posyandu participants in RW 03 Dinoyo Village who met the inclusion criteria. The measurement instrument uses the IPAQ-SF questionnaire to measure physical activity levels and the MMSE questionnaire to measure cognitive abilities. The correlation test used the Spearman Rho test with IBM SPSS 25. Results: Based on the Spearman Rho correlation test, the result was p = 0.339 (p > 0.05). The results showed that there was no relationship between the level of physical activity and the risk of dementia. Conclusion: There is no significant relationship between the level of physical activity and the risk of dementia in respondents. The lack of results in this study was caused by several factors, namely the limited research design and other supporting factors that were not studied.

Abstract (sommario):
Dementia care organizations face a range of daunting environmental changes and challenges. Internationally, there is a long history of efforts to improve quality of care and life for individuals with dementia. In a time of particular attention to the images of older adults confined to a single or shared room in a care facility due to COVID-19, autonomy becomes the overarching problem, not only because in general institutions limit the freedom of frail elders, but because the existential conditions that create the need for long term care such as chronic disease, cognitive decline, and the need for general support with activities of daily living (ADLs) rail against the autonomy of independent self-sufficiency. Additionally, these environments are institutional in design and size, with little access to outdoor spaces and other amenities. This perspective manuscript addresses the ethics of past and future memory care models, looking specifically at the European Dementia Village (DV) model. This model allows for autonomy and continuation of patterns of daily living through housing integrated with exterior walks, gardens, restaurants, and amenities within familiar and normal surroundings. This pioneering health care experiment negotiates rivaling discourses of intimacy, professionalization, and medicalization. In order to get a deeper understanding of the culture and ethics of this integrated care model, the first two dementia village sites were visited which included meetings with care staff and administrators. Those conversations and observations led to a series of aligned themes relating to the ethics of the DV model which include: the strength of the social approach, clinical support, resident/staff collaborations, and advocating for ethical dementia care. Rethinking the ethics of dementia care entails individual perspectives and group discussions on what can keep individuals social connected within their care community, including focusing on strengths of the individual and normalized daily routines.

Abstract (sommario):
Long term care for older people is a highly regulated sector providing accommodation, health, and social care to vulnerable older adults. Older adults in New Zealand are among the highest users of long term care services globally. Traditionally those requiring specialist care for dementia are housed apart from other residents. In an example of organizational innovation, 1 provider relocated residents to a secure village where residents requiring specialist dementia care would be desegregated. We utilized a critical realist case study to explain the role of intersectoral collaboration among government agencies in supporting the transition while managing risk and ensuring regulatory compliance

Abstract (sommario):
One of the most difficult challenges associated with an ageing population will be a significant increase in the number of people living with dementia. In Australia, this number is estimated to triple by 2050; a situation that is reflected globally. This will place increased demands on health and long-term care providers but it should also force an examination of the ability of contemporary cities to facilitate or constrain inclusion. Globally, designers and students of this discipline are contributing their skills to the challenge of dementia but solutions are typically proposed at a product, institutional or suburban scale. This paper will present two propositional projects, created using a speculative design methodology within a design studio at The University of Melbourne, that provoke architects to more seriously interrogate what it means for a city to support social inclusion, independence and choice for those who are ageing in place. These projects illuminate new avenues for critical and necessary research. This paper will begin with a reflection on the limitations of the Hogeweyk Dementia Village (Amsterdam), considered the current gold standard in dementia design, to highlight the value of thinking speculatively within the context of dementia; to disrupt the limitations of contemporary design thinking and ask what role the architect can play in improving the lives of those living with dementia?

Abstract (sommario):
Behavioural and psychological symptoms of dementia include agitation, depression, apathy, repetitive questioning, psychosis, aggression, sleep problems, wandering, and a variety of inappropriate behaviors. One or more of these symptoms will affect nearly all people with dementia over the course of their illness. These symptoms are among the most complex, stressful, and costly aspects of care, and they lead to a myriad of poor patient health outcomes, healthcare problems, and income loss for family caregivers. The complexity of these symptoms means that there is no “one size fits all solution, and approaches tailored to the patient and the caregiver are needed”. Non-pharmacologic approaches should be used first line, although several exceptions are discussed. The current pilot study examined the effects of the Person-Centered Environment Program (Memory Care®, developed by Belmont Village Senior Living) on agitation, cognition, stress, pain, sleep, and activities of daily living for Mexican senior living residents with dementia. Thirty individuals participated in the study. Memory Care® included sensitive, cognitive and affective stimulation, based on participants’ preferences and needs. memory care sessions were held daily (7 days per week, 8 hours) and a total of 6 weeks were performed at the first Belmont Village Community in Mexico City. Findings showed that agitation and pain improved with the Memory Care® Program (t = 2.91, P < 0.02; t = 4.51, P < 0.002, respectively). Findings suggested that a better study design, repeated with a bigger sample size, must be considered, but promissory results are shown.Disclosure of interestThe author has not supplied his/her declaration of competing interest.

Abstract (sommario):
This symposium will review the latest data on the influence of environmental design and its attributes on the cognitive and psychological wellbeing of older adults living with dementia. The presenters will cover the myriad ways in which the physical environment of care can adapt to the changing demands of older adults with sensory, motor and cognitive deficits and foster optimal functioning and quality of life. The role of emerging technologies will also be reviewed as they complement the contribution of the design of the physical environment to the wellbeing of older adults with cognitive impairment. Information will be offered through a review of the existing research literature as well as case studies that illustrate the impact of environmental modification on fostering wellbeing and minimizing the emergence of the behavioral and psychological symptoms of dementia. The presenters will represent and integrate sensibilities that have emerged from the fields of architecture, cognitive neuroscience and psychology. How the Principles of the Culture Change Movement Inform Environmental Design and the Application of Technology in the Care of Older Adults Living with DementiaWilliam E. ReichmanThe culture change movement informs a number of principles that have been applied to more contemporary design concepts for the congregate care of older adults living with dementia. This talk will review the core tenets of the Culture Change Movement as exemplified by the Greenhouse, Dementia Village and other innovative models of congregate long-term care. Specific reference will be made to how these tenets have been operationalized around the world into the design of programming and the creation of residential care environments that foster a better quality of life for older adults and an enhanced work environment for care providers. This talk will also include the emerging role of technologies that complement innovative design of the environment and which foster optimized social and recreational functioning of older adults living with dementia.A Better Life Through a Better Nursing Home DesignL. Bradford PerkinsOver the last 20 years there has been extensive experimentation related to the role of the environment in the housing, care and treatment of persons with Alzheimer’s and other age related dementias. Prior to that time the typical housing and care environment was a locked unit in a skilled nursing or other restrictive senior living facility. In 1991 the Presbyterian Association on Aging in Western Pennsylvania opened Woodside Place on its Oakmont campus. This small 36 bed facility was designed to incorporate the latest research and care experience with persons suffering from these issues. This one small project, as well as the long post occupancy research led by Carnegie Mellon University, clearly demonstrated that individuals with Alzheimer’s and related forms of dementia could lead a healthier, happier, higher quality of life in a more residential, less restrictive environment. Not everything in this pioneering project worked, and five generations of living and care models have followed that have refined the ideas first demonstrated by Woodside Place. Bradford Perkins, whose firm designed Woodside Place and over 100 other related projects, will discuss what was learned from Woodside Place as well as the five generations of projects (and post occupancy research) that followed.Innovative dementia care environments as alternatives for traditional nursing homes: evidence and experiences from the NetherlandsHilde VerbeekKey goals of the dementia care environment focus on increasing autonomy, supporting independence and trying to enable one’s own lifestyle for as long as possible. To meet these goals, innovative, small-scale and homelike care environments have been developed that have radically changed the physical, social and organizational aspects of long-term care in the Netherlands. This presentation discusses various Dutch models that have implemented small-scale and homelike care environments, including green care farms, dementia village and citizen initiatives. The models reflect a common care concept, focusing on residents’ remaining strengths, providing opportunity for choice and aiming to sustain a sense of self and control. A small number of residents (usually 6 to 8) live together in a homelike environment and nursing staff are part of the household. Residents are encouraged to participate in daily household activities, emphasizing normalization of daily life with person-centred care. The physical environment resembles an archetypal home. This talk presents the scientific evidence on the impact and effects of these small-scale, homelike models on residents, their family caregivers and staff. Furthermore, the presentation will highlight working approaches and how these initiatives have positively influenced routine care across the long-term care spectrum.

Abstract (sommario):
Klampis Ngasem is one of the areas in East Surabaya, precisely in Sukolilo sub-district, Surabaya. The target group in the community partnership program (PKM) is the Elderly Posyandu "Barokah" in RW 3. Problems that occur in the Elderly Barokah Posyandu are less varied activities and innovation so that some elderly are less interested and bored to follow routinely as indicated by the presence of the elderly < 75%, the absence of efforts to prevent dementia that is carried out regularly and continue for the elderly, lack of elderly knowledge about changes in cognitive function and efforts to maintain cognitive function so as to prevent dementia, and the availability of leisure time for the elderly in addition to routine posyandu activities and those that have not been utilized to be useful and productive. The solution to overcome these problems is through the application of Art Therapy to make colet batik. The purpose of this community service is to help express themselves, maintain the cognitive function of the elderly and provide new skills in making batik as an activity in the Posyandu Elder Barokah Klampis Ngasem Village. The activities carried out are screening to see the cognitive function of the elderly using Mini mental state examination (MMSE) and clock drawing test (CDT), counseling about prevention of dementia which includes knowing dementia, prevention of dementia, and art therapy, batik colet training with demonstration methods, and simulation. The results of batik colet will be processed again with the help of cadres or the elderly who have sewing skills to be used as sofa pillowcases. From this process, a work or product will be produced from the batik colet method.

Abstract (sommario):
The current study investigated the results of a 6-week singing program targeted for older individuals with dementia and their caregivers. Participants were from: a) a residential care unit within a retirement village with dementia clients and their site-based professional caregivers (mainly occupational therapists, but some care assistants); and b) a drop-in program organized by a dementia trust for clients living in the community, in which both client and spouse were asked to participate. The program, already tried and tested, had been developed with elderly clients mainly without dementia (see Davidson & Faulkner, 2010 ) but was modified to suit the new cohort and evaluated using standardized measures of dementia, health, and wellbeing, plus measures designed to examine specific singing program outcomes. In addition, video footage of the sessions and checklists kept by both the singing group facilitator and some of the caregivers were drawn upon for qualitative analysis. Whilst little effect of the singing program was revealed using standard outcome measures, study-specific measures and the qualitative analyses indicated that many participants had positive gains including lucidity and improved social interaction within session, as well as enjoyment, singing engagement, and carry-over memory and recall from one week to the next. Thus, it seems that well-structured singing programs can encourage both caregiver and cared-for participant to develop better communication. The lack of outcomes from the standardized measures indicates that these types of measures are either not sufficiently fine-grained or appropriate to the context of the short-term singing program to account for the changes that are otherwise captured in the qualitative measures.

Abstract As countries around the world are entering the super aged societies, aging in place will need to be planned not only as an individual but also as a multi-disciplinary and age-friendly community. Many models are built based on the concept of "Aging in communities" in order to consider the multi-levels of needs and interests that older adults face when living in the same area. With Asia being the next fastest aging continent, innovative models and programs are also developed in Taiwan as it tries to promote healthy aging, combating dementia and supporting family caregivers. This symposium will include 4 papers by introducing a national program for frailty prevention, a community friendly model for people with dementia and a collaborative network to prevent family caregivers from being burned out. In addition, the village model developed in the U. S. will be compared to show how east meets west to demonstrate communities meeting the needs of older adults.

Stress is a significant part of daily life, and systemic social inequities, such as racism and discrimination, are well-established contributors of chronic stress for African Americans. Added exposure to the stress of caregiving may exacerbate adverse health outcomes. This secondary analysis describes subjective and objective stress in African American family caregivers, and relationships of subjective and objective stress to health outcomes. Baseline data from 142 African American dementia family caregivers from the “Great Village” study were described using means and frequencies; regression models and Pearson’s correlation were used to examine associations between demographics, social determinants of health, and health outcomes. Mixed models were used to examine change and change variation in cortisol. Most caregivers had moderate degrees of stress. Stress was associated with sleep disruption and depressive symptoms, and discrimination appeared to be an independent contributor to depressive symptoms. This work provides a foundation for interpreting subjective and objective indicators of stress to tailor existing multicomponent interventions.

Abstract (sommario):
Elderly people are the final stage of human life, characterized by deterioration of health balance and physiological conditions that increase the risk of organ disorders. Reduced brain function can lead to various neurological and psychological disorders, delirium, and dementia. Psychomotor function, including the ability to move, act, and coordinate, may also decline with age, which may reduce agility in older adults. Older adults also experience a decline in their ability to carry out daily life and become more vulnerable to various challenges. With the growing number of elderly people in Indonesia, this issue has a significant impact on government and society. Physical and mental decline in older adults, including reduced cognitive function, is a major concern that requires appropriate management and prevention efforts. To address these problems, the Community Service Program (PKM), specifically targeting the elderly, senior citizens and BKL Anggrek RT. 05 RW. 02 in the village of Cepokokuning, provides concrete solutions to overcome this problem through the provision of health and home education programs. This program aims to improve the optimization of cognitive functions and coordination in older adults

Abstract (sommario):
Studying behavioral disturbances of dementia across cultures allows us to identify commonalities and differences that may be useful in determining the best approach to managing these problems. However, what we tend to find in cross-cultural studies is that the best approach may not be the same approach, given the different prevalence of and levels of tolerance for various behavioral problems. These differences are apparent in the authors' studies of four populations—Jamaicans in Kingston; Cree in Northern Manitoba, Canada; Yoruba in Ibadan, Nigeria; and African Americans in the United States. The Jamaicans in this study live in a poor suburb of Kingston, the Cree live in two fairly small, isolated communities in Northern Manitoba, and the Yoruba live in Ibadan, a city of more than 1 million people. The Yoruba community the authors are studying, although concentrated in the city center, functions much like a village. The African-American population resides in Indianapolis, Indiana, a moderately sized city of approximately 1 million people.

Abstract (sommario):
Abstract “Il Paese Ritrovato” is an innovative long-term facility dedicated to subjects with mild-to-moderate Alzheimer’s disease or other dementias, located in Monza (Lombardia, Italy).By living in a village that resembles a community neighborhood, it aims at people’s wellbeing and represents a shift in the paradigm of traditional nursing home care, allowing the guests to visit the hair stylist, the supermarket, and various activities according to their personal habits, always supported by trained professionals. We performed a retrospective cohort study on 60 subjects, comparing their characteristics before and during the SARS-CoV-2 pandemic, from June 2018 to December 2020. We collected demographic information, cognition, comorbidity, functional status, engagement in the activities through Popularity Index (PI) and Engagement Social Index (ISE), and use of psychoactive medications. The follow-up is performed every 6 months from admission: T0, T6, T12, and T18 are the time-points. The assessment performed during the pandemic is called Tcovid.Compared to a relative stability in cognition before the pandemic, the assessment at Tcovid showed an accelerated worsening of MMSE and CDR scales. We observed an increase in the prescription of antipsychotics (+5%), antidepressants (+11.7%), and benzodiazepines (+1.7%). As expected, engagement in activities dropped (lower PI and ISE). Functional status gradually worsened during the follow-up, according to the natural progression of dementias, but Tcovid slightly accelerated this process and overall worsened the balance performances.Although we are currently unable to quantify the negative effect of this reorganization, future studies need to address the real impact of pandemic on the guests’ performances.
