Abstract (sommario):
Introduction: Extramarital affairs disrupt family functioning and have serious negative consequences for the family members. Aim: The present study aimed to compare the effectiveness of combined cognitive-behavioral couple therapy and integrated behavioral couple therapy in improving family functioning in couples with extramarital affairs. Method: In this quasi-experimental study, with a pretest-posttest control group design, the statistical population consisted of all couples, referred to the counseling centers of Mashhad, Iran in 2021. Thirty nine couples were selected using the convenience sampling method among people with extramarital relationships and randomly assigned to three groups: control group (13 couples), combined cognitive-behavioral couple therapy (13 couples), and integrative behavioral couple therapy (13 couples). The experimental and control groups completed the Family Functioning Scale by Epstein et al. (1983) before and after the interventions. In order to data analysis covariance analysis was performed in SPSS version 20. Results: Results indicated that the scores of all family functioning components significantly decreased in the intervention groups compared to the control group (P<0.05). Combined cognitive-behavioral couple therapy was more effective in the dimensions of problem-solving and communication, while integrated behavioral couple therapy was more effective in emotional companionship (P<0.05). Conclusion: According to the present results, both combined cognitive-behavioral therapy and integrated behavioral couple therapy had positive effects on improving family functioning. Therefore, depending on the conditions and type of marital problems, therapists are suggested to use a combination of these two methods to improve family functioning in couples with extramarital relationships.

Abstract (sommario):
Objective: This study aimed to compare the effectiveness of Aceptance and Commitment Therapy (ACT) and schema therapy on forgiveness and fear of intimacy in conflicting couples. Methods: This quasi-experimental study was conducted using a pre-test, post-test design with a control group. The participants were selected from the conflicting couples who were referred to counseling and psychological service centers in districts 1 and 5 of Tehran in 2019. Of the couples who scored high on the Fear-of-Intimacy Scale and obtained a low score on the Interpersonal Forgiveness Inventory, 30 couples (60 people) were selected using convenience sampling and randomly assigned to the schema therapy group (10 couples), the ACT group (10 couples), and the control group (10 couples). Then, the participants in each of the intervention groups attended eight 90-minute therapy sessions based on ACT and Schema therapy techniques. After the interventions, the participants in the three groups completed the questionnaires again and were followed up after three months. Results: The results showed that both ACT and schema therapy had a significant effect on fear of intimacy and forgiveness. Upon controlling the pre-test score, significant differences were found between the two ACT and schema therapy groups in terms of reconnection and revenge control (P<0.01), but the two groups were not significantly different in terms of resentment control, realistic perceptions, and fear of intimacy and schema couple therapy was more effective than ACT in controlling conflicting couples’ revenge and improving reconnection and this greater effectiveness was still retained in the follow-up phase. Conclusion: ACT and schema-based couple therapies can be used by psychologists and counselors as effective approaches to increase forgiveness and reduce fear of intimacy in couples. However, schema couple therapy is more effective than ACT in controlling negative emotions and treating them in couples.

Abstract (sommario):
Background: Divorce rates have increased during the last decade, leading to a greater focus of marital scholars on the importance of understanding couple-maintaining strategies within marital life. Distresses in couples are attributable to difficulties controlling felt, experienced, and expressed emotions; thus, emotion dysregulation is a core stressor in couples with maladaptive responses. Objective: The aim of the study was to evaluate the effect of Dialectical Behaviour Therapy (DBT) on outpatient couples to treat emotion dysregulation. Methods: We recruited 20 couples with marital distress in which partners presented emotion dysregulation. We offered the couples the opportunity to join a couple DBT group at their convenience and based on the immediate availability of treatment slots. We measured the treatment efficacy using psychometric tools (the Difficulties in Emotion Regulation Scale (DERS) and the Dyadic Adjustment Ccale (DAS) at baseline and after DBT therapy. Results: Both male and female partners presented significant improvements in marital adjustment DAS and emotion regulation scores. Female partners showed significantly greater amplitude changes in both scales. Female partners showed significant improvement in most DERS subscales (except the GOALS subscale); on the other hand, male partners showed significant improvements in impulse, awareness, strategies, and clarity subscales. We found significant improvements in most DAS subscales in both sexes; only affectional expression remained unchanged before and after therapy. Conclusion: DBT for couples is an effective approach to treat emotion dysregulation.

Abstract (sommario):
Abstract Purpose Infertility represents a peculiar social burden affecting more than 15% of couples, provoking it a real threat to the general quality of life and to the sexual health. The medicalization (diagnosis, therapy and follow up) of the lack of fertility is frequently a challenge in term of personal and couple’s involvement. In particular, while the Assisted Reproductive Technology (ART) has allowed many infertile couples to achieve pregnancy, the therapeutic process faced by the couple bears a strong psychological stress that can affect the couple's quality of life, relationship and sexuality. Despite infertility affects both female and male sexual health, only recently the interest in the effects of ART on the couple's sexuality has grown, especially for women. Methods A literature research on the sexual dysfunction in fertility care and particularly in ART setting was performed. Results Literature largely found that intimacy and sexuality appear specifically impaired by intrusiveness of treatments and medical prescriptions. Moreover, there is a close relationship between emotional, psychological and sexual aspects, which can be integrated in the new concept of Inferto-Sex Syndrome (ISS) that can impair the ART treatment outcomes. Evidence demonstrates that the assessment of sexual function is necessary in couples undergoing diagnosis of infertility and ART. Conclusion A close relationship between infertility and sexuality, both in the female and male partners, was detected. ART treatments may heavily impact on the couple's psychosexual health. A couple-centred program for the integrated management of psychological and sexual dysfunction should be considered in the context of ART programs.

Abstract (sommario):
Objectives Purpose This study attempted to investigate the effect of art therapy programs to promote marital relationships and communication, marital conflict resolution, and happiness. The purpose of the art therapy program was to naturally express one's inner feelings, understand and accept oneself and others, and to verify the effect on enhancing marital relations and communication by making positive changes. Methods Data were collected from January 1 to June 30, 2021 for the couple of members of the Cultural Center in N city. After confirming the research purpose and program composition, 16 couples who expressed their willingness to participate voluntarily were selected, and 8 pairs were wirelessly composed of experimental groups and 8 pairs were composed of control groups. For this study, a couple interaction scale, a couple communication scale, a couple conflict scale, and a happiness scale were used, and a dynamic house-tree-person picture test was conducted. The independent sample t-test and the corresponding sample t-test were statistically analyzed using SPSSWIN 25.0, and 22-verification and covariance verification (ANCOVA) were performed. In addition, the qualitative research method was introduced, and the interviews with participants and activity memo information were used to analyze them with Colaizzi's phenomenological research method. Results As a result of the study, it was verified that the couple art therapy program was effective in enhancing marital relations and communication. In the process of couples exploring themselves, expressing their thoughts, and expressing their suppressed feelings through art activities, their understanding of themselves and their spouses increased, and sincere communication served as an opportunity to resolve conflicts between couples and realize their willingness to change for each other. As a result, it was found that the improvement of the relationship and satisfaction between couples can be the basis for determining the happiness of couples. Conclusions This study is a study that introduced qualitative research after focusing on quantitative research. For this study, the reconstructed art therapy program was operated, and the marital interaction scale, marital communication scale, marital conflict scale, and happiness scale were conducted before and after the marital art therapy program. As a result, it was found to be significant in the effectiveness test of the couple art therapy program, and it was statistically significant in marital interaction, marital communication, marital conflict, and marital happiness scale. In this regard, it was also found to be high satisfaction in program satisfaction. These significant results showed that even in the analysis of the introduced qualitative research, sincere communication between couples was the willingness to increase understanding between couples, resolve conflicts, and change between couples.

Abstract (sommario):
Emotionally focused therapy was created as a couples therapy approach and has become a frequently used therapeutic approach these days. This approach, theoretically based on the understanding that the attachment relationship observed between mother and child characterized by proximity seeking and separation anxiety resembles the relationship observed between couples, aims to turn unhealthy attachment relationships between couples into healthy ones. This process is realized by revealing the implicit and hidden emotions that cause problem situations to occur and gaining awareness about these emotions. Different studies have shown spirituality to be a major factor both in forming and solving problem situations in therapy for couples who care about spiritual values. In particular, the meanings couples who attach importance to religious values attribute to marriage and the quality of the relations developed within this meaning can be handled within the framework of emotionally focused couples therapy. The purpose of this article is to explain how the therapy process proceeds when emotionally focused couples therapy is combined with spiritual values and to show which methods and techniques are used in the light of sample cases. In addition, the article aims to raise awareness on how to use this therapy approach, as it can be combined with Islamic elements, and investigates how to set an example in this regard. This study, which briefly introduces emotionally focused couples therapy, provides short examples of how to perform interventions while working with couples who seek counseling using religious or spiritual matters.

Abstract (sommario):
Infertility is the inability to conceive after one year of unprotected intercourse, and about one in six couples deal with this issue. Nearly half of the factors contributing to a couple’s infertility are female-related, and the underlying cause is variable. Many treatment options can be used depending on the underlying cause of infertility. The available treatments often include in vitro fertilization, intrauterine insemination, and fertility-inducing drugs. Numerous couples favor complementary medicine alongside conventional treatments to enhance treatment efficacy and reduce side effects. Manual therapy and massage are among these complementary treatment approaches. Manual therapy is defined in various ways. One of them is manipulating and mobilizing soft tissue structures and related joints using the hands. Manual therapy can alleviate pain, reduce inflammation, prevent contracture formation, manage existing contractures, increase range of motion, aid mobilization, and improve overall health. It contains a variety of massage treatments, stretching, and mobilization applications. Manual therapy and massage can be administered to manage various disorders, from pediatric to geriatric populations. These are safe interventions. Manual therapy and massage have beneficial effects on the management of female infertility. These methods reduce adhesions and mobilize organs in the pelvic region. They have positive effects on circulatory system disorders, including lymphatic drainage abnormalities. Overcoming reproductive system lymphatic drainage and circulation difficulties can result in balanced hormone levels, regular menstrual cycles, and pregnancies. In addition, manual therapy and massage improve the psychological state of individuals in a way that will benefit the management of female infertility.

Abstract (sommario):
Relationship quality is important for well-being and quality of life in couples living with dementia. Home-based music therapy interventions may be conducted with the aim of enhancing relationship quality. However, the effects or influences of such interventions are only briefly investigated in previous studies. This study’s aim was to identify how a 12-week home-based music therapy intervention may influence relationship quality in couples living with dementia, through an adapted convergent mixed methods design. In this case, 68 participating couples from the HOMESIDE RCT study, and four individually recruited couples, received the music therapy intervention. Relationship quality for all participants was measured by the standardized Quality of Caregiver-Patient Relationship scale, and qualitative interviews were conducted with the four individually recruited participants at baseline and post intervention. Quantitative analysis indicated no statistically significant intervention effect. However, relationship quality remained stable over the intervention period. The qualitative analysis identified that the music therapy interventions primarily led to positive emotions, closeness, intimacy, and communication between the persons with dementia and their care partners. Intervention influences could also be ambiguous, as sharing music experiences might involve a risk of evoking vulnerabilities or negative emotional responses.

Abstract (sommario):
For many years, the negative therapeutic reaction (NTR) was regarded as a form of resistance arising solely in individual psychoanalytic treatment. The possibility, however, that the NTR may also appear in couples has recently emerged as worthy of exploration. After presenting a case in which NTR dynamics predominate, and reviewing analytic literature about the NTR, the authors propose a conceptualisation of a couple-based form of the NTR that relies upon both paradoxical motivations and paradoxical uses of projective and introjective identification. The countertransference reactions these couples tend to induce in therapists will also be explored, and methods for their management will be suggested.

Cognitive, affective, and behavioral individual and couple risk factors for violence are reviewed, as are therapeutic concerns regarding the use of conjoint treatment. Current conjoint treatments that are intended to reduce the incidence of abusive behavior among couples in which one or both partners have engaged in forms of psychological and/or mild to moderate physical aggression, do not engage in battering or severe violence, and desire to improve their relationships and stay together are described. We focus on our Couples Abuse Prevention Program (CAPP) that compares the efficacy of cognitive-behavioral couple therapy procedures and treatment as usual at a university-based couple and family therapy clinic. Outcomes from the CAPP project and evaluations of the other programs demonstrate the potential of judiciously applied conjoint interventions for aggressive behavior in couple relationships.

Abstract (sommario):
Aim: The current research was conducted with the aim of the effectiveness of the couple therapy package of de-jealousy with an Islamic approach on family efficiency and jealousy in incompatible couples. Methods: The research method was experimental and within the framework of a quasi-experimental design of pre-test-post-test type with a control group with follow-up after two months. The statistical population of the present study included all couples in Tehran in 1400 who announced their readiness to participate in the training course for improving marital relations. After conducting the tests, the researcher selected the couples in which the women had a score lower than the average score in Safurai Parisi's Family Efficiency Questionnaire (2008) and a score higher than the average score in the researcher's Marital Jealousy Questionnaire and randomly placed them in the groups. 32 couples were selected from among the statistical population of the study by purposeful sampling and randomly replaced in two experimental and control groups. The participants of the educational group were subjected to the intervention of the couple therapy model from the perspective of the Qur'an and hadiths in 9 sessions of 90 minutes (Jaafari, Fayaz, Pourshahriari, and Abdulahi, 2021) and the control group did not receive any treatment. For data analysis, repeated measurement variance analysis was used with SPSS-25 statistical software. Results: The results of the analysis showed that the average scores of family efficiency in the experimental group compared to the control group increased (F=553.05, P=0.001) and the average scores of jealousy in the experimental group compared to the control group decreased (F=337/ 29, P=0.001). Conclusion: As a result, it can be said that the package of couples therapy to eliminate jealousy with an Islamic approach has been effective on family efficiency and jealousy in incompatible couples.

Abstract (sommario):
Background: Obstructive Sleep Apnoea (OSA) is a common chronic sleep-related breathing disorder. Drug therapy is a recent approach to treating OSA, and no data is available regarding its effects on a couple’s life. The aim of this study was to investigate the impact on couples’ lives of a novel drug treatment for OSA. Methods: Participants of a trial on new drug therapy (reboxetine plus oxybutynin) for OSA were interviewed. The study was conducted using a descriptive phenomenological approach by Sundler et al. Results: Ten patients and six of their partners were recruited through a purposive sampling between February and April 2021. The results indicate that drug treatment of OSA had a favourable impact on the couples’ lives. Pharmacotherapy improved self-reported sleep quality, as the absence of CPAP allows people to sleep closer to the bed partner and because the patient does not have to sleep in a forced position. Moreover, the couples developed teamwork from the beginning which appeared to be crucial during the trial, especially when some difficulties and adverse events occurred. Conclusions: This study presents the presence of a positive closed loop that could be considered a predictor of adherence to drug therapy.

Abstract (sommario):
Aim: The present study aimed to determine the effectiveness of emotionally focused therapy (EFT) in comparison with acceptance and commitment therapy (ACT) in improving emotional self-regulation, psychological well-being, and resilience of couples. Methods: The research was quasi-experimental and applied with a pretest-posttest design, a control group, and quarterly follow-up. The statistical population consisted of all couples with marital non-adjustment living in District 4 of Qom who visited counseling and psychotherapy centers in the second half of 2020. The convenience sampling method was performed. After the random assignment of experimental and control groups, the first experimental group received the emotionally focused therapy by Johnson (2012), and the second experimental group received the Acceptance and Commitment Therapy by Hayes and Strosahl (2010) in eight one-hour sessions. The data collection tool included difficulties in emotion regulation scale (DERS) by Gratz and Roemer (2004), and Ryff's scale of the psychological well-being-short form (RSPWB) at three stages, pre-test, post-test, and follow-up. Data were analyzed using repeated-measures analysis of variance. Results: The results indicated the effects of emotionally focused therapy and acceptance and commitment therapy on emotional self-regulation (F=10.90, P=0.009) and psychological well-being (F=31.10, P=0.001), and stability of the effect at the follow-up stage. The results of Tukey's post hoc test indicated that the difference between the two methods of couple therapy was not significant (P=0.31). Conclusion: The results suggest evidence that emotionally focused couple therapy and acceptance and commitment therapy were appropriate for improving and increasing emotional self-regulation and psychological well-being.

Abstract (sommario):
Safer conception services are needed to minimize HIV transmission among HIV sero-different couples desiring pregnancy. Few studies have evaluated the choices couples make when offered multiple safer conception methods or real-world method acceptability and effectiveness. We piloted a comprehensive safer conception program (Clintrials.gov identifier: NCT03049176) for HIV sero-different couples planning pregnancy in Zimbabwe to measure feasibility, method uptake, acceptability, pregnancy outcome, and HIV transmission. This study was not designed to compare rates of HIV transmission by safer conception method choice but rather to understand choices couples make when seeking to minimize risk of HIV transmission and maximize likelihood of pregnancy. Couples in this prospective, non-randomized study were given a choice of one or more currently available safer conception methods: antiretroviral therapy (ART) with monthly viral load (VL) monitoring for the HIV-positive partner (ART/VL), pre-exposure prophylaxis (PrEP) for the HIV-negative partner, vaginal insemination (VI) for couples with an HIV-positive woman, and semen washing (SW) for couples with an HIV-positive man. Couples were followed monthly for up to 12 months of pregnancy attempts, quarterly during pregnancy, and 12 weeks post-partum. At each visit, data on method use, urine for pregnancy testing, and blood for HIV antibody testing, or viral load if HIV-positive, were obtained. Infants born to HIV-positive women were tested for HIV at 6 and 12 weeks. Between March 2017 and June 2019, 46 individuals from 23 HIV sero-different partnerships were enrolled and followed. At enrollment, all couples chose ART/VL, and all couples chose at least one additional method; 74% chose PrEP, 36% chose SW, and 25% chose VI. During pre-pregnancy follow-up visits, three couples discontinued SW, and one couple discontinued VI; all four of these couples opted for ART/VL plus PrEP. Satisfaction with safer conception methods was high among those who chose ART/VL and PrEP. Twelve couples achieved pregnancy. There were no cases of HIV transmission to partners, and no infants tested positive for HIV. This safer conception program is feasible and acceptable, allowing sero-different couples to safely achieve pregnancy. Sero-different couples in Zimbabwe seek a combination of HIV prevention methods, particularly ART/VL plus PrEP. Trial Registration: Clintrials.gov, NCT03049176.
