Abstract (sommario):
Introduction: Extramarital affairs disrupt family functioning and have serious negative consequences for the family members. Aim: The present study aimed to compare the effectiveness of combined cognitive-behavioral couple therapy and integrated behavioral couple therapy in improving family functioning in couples with extramarital affairs. Method: In this quasi-experimental study, with a pretest-posttest control group design, the statistical population consisted of all couples, referred to the counseling centers of Mashhad, Iran in 2021. Thirty nine couples were selected using the convenience sampling method among people with extramarital relationships and randomly assigned to three groups: control group (13 couples), combined cognitive-behavioral couple therapy (13 couples), and integrative behavioral couple therapy (13 couples). The experimental and control groups completed the Family Functioning Scale by Epstein et al. (1983) before and after the interventions. In order to data analysis covariance analysis was performed in SPSS version 20. Results: Results indicated that the scores of all family functioning components significantly decreased in the intervention groups compared to the control group (P<0.05). Combined cognitive-behavioral couple therapy was more effective in the dimensions of problem-solving and communication, while integrated behavioral couple therapy was more effective in emotional companionship (P<0.05). Conclusion: According to the present results, both combined cognitive-behavioral therapy and integrated behavioral couple therapy had positive effects on improving family functioning. Therefore, depending on the conditions and type of marital problems, therapists are suggested to use a combination of these two methods to improve family functioning in couples with extramarital relationships.

Abstract (sommario):
Objective: This study aimed to compare the effectiveness of Aceptance and Commitment Therapy (ACT) and schema therapy on forgiveness and fear of intimacy in conflicting couples. Methods: This quasi-experimental study was conducted using a pre-test, post-test design with a control group. The participants were selected from the conflicting couples who were referred to counseling and psychological service centers in districts 1 and 5 of Tehran in 2019. Of the couples who scored high on the Fear-of-Intimacy Scale and obtained a low score on the Interpersonal Forgiveness Inventory, 30 couples (60 people) were selected using convenience sampling and randomly assigned to the schema therapy group (10 couples), the ACT group (10 couples), and the control group (10 couples). Then, the participants in each of the intervention groups attended eight 90-minute therapy sessions based on ACT and Schema therapy techniques. After the interventions, the participants in the three groups completed the questionnaires again and were followed up after three months. Results: The results showed that both ACT and schema therapy had a significant effect on fear of intimacy and forgiveness. Upon controlling the pre-test score, significant differences were found between the two ACT and schema therapy groups in terms of reconnection and revenge control (P<0.01), but the two groups were not significantly different in terms of resentment control, realistic perceptions, and fear of intimacy and schema couple therapy was more effective than ACT in controlling conflicting couples’ revenge and improving reconnection and this greater effectiveness was still retained in the follow-up phase. Conclusion: ACT and schema-based couple therapies can be used by psychologists and counselors as effective approaches to increase forgiveness and reduce fear of intimacy in couples. However, schema couple therapy is more effective than ACT in controlling negative emotions and treating them in couples.

Friis, Kirsten Leigh. "The influence of the therapist's activities on clients' subject positioning in relation to gender in narrative couples therapy". Thesis, Rhodes University, 2014. http://hdl.handle.net/10962/d1013050.

Testo completo
Abstract (sommario):
Narrative Therapy draws on an understanding of how discourse acts to construct, reproduce and deconstruct power relationships. Therapy is focussed on collaborating with clients in a process of re-authoring their self-stories by critically evaluating the positions made available to them in relation to dominant discourses. Whilst there is a large body of theoretical knowledge on post-structuralism and psychotherapy, very few discourse analyses of psychotherapy sessions have been published. Thus, though post-structuralist therapeutic approaches are theoretically well supported, there is an identified gap in the literature on the therapeutic processes that occur within practice. This research aims to further explore these therapeutic processes, focussing specifically on subject positioning with relation to gender in Narrative Couple Therapy, and the influence of the therapist thereupon. It is based on the analysis of a video of a Michael White Narrative Couple Therapy session, "The Best of Friends." Using discourse analysis as a methodological approach, the analysis seeks first to identify talk that signifies discourses of gender difference, and then to explore how the subjects (therapist and clients) are positioned in relation to these discourses. The analysis focuses on the therapist's activities to investigate the influence of his interactions with the partners on how they are positioned or position themselves within stories of gender difference. The analysis shows that, by working reflexively, transparently and collaboratively with the partners, the therapist assists to render power, restrictive discourses, and alternate positions visible to the clients. The therapist resists being drawn into dominant therapy discourses of expert and patient, and instead continuously recognises the knowledges and expertise that the clients bring to the therapy context. He then works closely with the clients to thicken alternative representations of self.

Turner, Erica R. "Exploring a Psycholinguistic Method of Assessing Attachment Orientation in Couple Therapy". Thesis, Virginia Tech, 2013. http://hdl.handle.net/10919/51552.

Testo completo
Abstract (sommario):
Research conducted with the Adult Attachment Interview (AAI) has provided a wealth of information regarding how adults behave in and respond to relationships. Unfortunately, despite the AAI\'s usefulness in assessing attachment orientation, administering the AAI requires intensive training, cost, and time. Because of this, a limited number of researchers are able to use the AAI despite the measure\'s demonstrated psychometric properties and utility for research related to attachment theory. The present study aimed to utilize the Linguistic Inquiry and Word Count software to assess attachment orientation in the context of an attachment-based couple therapy, with the broad goal of potentially expanding our ability to measure attachment orientation in research. The results indicate that attachment orientation is a meaningful metric for analyzing individuals\' speech regarding their relationships in an attachment-based couple therapy.

Abstract (sommario):
Few therapists test their assumptions about how they do therapy against what they actually do. The purpose of this study was to test how well integration is practiced within the proposed theoretical framework of one therapist. Qualitative and quantitative data were designed to answer four research questions: fidelity to the integrated model, client change, how working with the integrated model influenced ongoing sessions or cases, and change in the integrated model through the course of the study. Three couples were used as the sample; 17 sessions were coded and analyzed to answer the research questions. Results indicate that the therapist maintained fidelity to the integrated model that resulted in positive change for each couple. Using the integrated model was found to influence ongoing sessions and cases in a number of ways that also resulted in changes of the integrated model. Other findings, limitations, and clinical implications are discussed.

Abstract (sommario):
Stent thrombosis is a major complication that occurs after the placement of stents in the coronary artery through balloon angioplasty. The common treatment for stent thrombosis is to provide patients with anticoagulant and antiplatelet therapy through the bloodstream. This study uses numerical modeling to compare two delivery methods of heparin anticoagulant to the arterial wall to reduce thrombus formation: through the flow and via a drug-eluting stent. A unique computational fluid dynamics model is developed that couples an incompressible flow solver with a convection-diffusion-reaction equation solver. The flow solver uses a sharp-interface immersed boundary method on a Cartesian grid to characterize pulsatile flow over the curved wires of the stent. Concurrently, the convection-diffusion-reaction equations are solved for the 19 coupled reactions that make up the coagulation cascade and heparin interactions, as well as reaction and transport equations for both active and inactive platelet species. The simulation is run with input boundary conditions of steady flow, pulsatile Poiseuille flow, and a Womersley flow profile. Results are collected for the bare metal stent case, anticoagulant delivered through the bloodstream, and anticoagulant delivered through a drug-eluting stent. The results generally find that the drug-eluting stent delivery of anticoagulant is more effective in reducing platelet activation and clotting, while also providing a more localized anticoagulant distribution.

Abstract (sommario):
The potential of nonlinear directional couplers (NLDCs) as the basis for all-optical switching devices has been studied extensively [1]. The mode of operation of an NLDC as an all-optical switch is based on the optical intensity dependence of the nonlinear refractive indices of the two waveguiding regions of the coupler, where the modal propagation contants (β) of the waveguides are matched at low powers and mismatched at high powers. When the waveguides are in close proximity, the light in the input waveguide will couple to the other waveguide in a coupling distance Lc at low powers, but at high powers the waveguides will decouple and the light will remain in the input waveguide. The power at which the light is equally split between the waveguides is defined as the critical power, Pc [2]. Proposals to reduce Pc in fibers NLDCs have included asymmetric designs, matched gain in the input waveguide to loss in the other [3], and non-identical nonlinear refractive indices in the two waveguides [4], However, for true optoelectronic integration it is desirable to design NLDCs using semiconductor materials. Our previous studies of semiconductor NLDCs for the first time have focussed on the enhancement of asymmetry and realistic geometries for matching gain to loss [5]. This has been demonstrated by the use of vertical design configurations with a quasi- planar-waveguide coupled to a rib-waveguide (see figure 1). Such designs have a high degree of geometric asymmetry, and because the waveguides are in separate planes the doping and biasing requirements needed for inducing gain in only one waveguide are experimentally realisable. Furthermore, the separation of the waveguides is only limited by the epitaxial fabrication process. This indicates that the waveguide separation in a vertical NLDC can be much smaller than that for a conventional two-rib horizontal design, thereby further enhancing the predicted characteristics of the design by minimizing Lc. In this paper, we extend these studies to the design of vertical NLDC geometries with nonlinear refractive index mismatch between the two waveguides. This is achieved by having the two waveguiding layers made of different semiconductor materials. The structures have been designed and optimized using two computational modelling techniques, the Spectral Index method and Coupled Mode Theory. In this paper we report a fesibility study of the predicted advantages of such a design and the possible reductions in Pc.

Abstract (sommario):
Photothermal therapy involving nanoparticles is evolving as a promising targeted treatment for cancer. This paper presents the results for the effect of nanoparticle concentration, within a tumor, to control the thermal damage during nanoparticle assisted thermal therapy. A surface tumor embedded with gold nanoparticles (distributed uniformly) is considered. The thermal damage is evaluated for various nanoparticle concentrations (within the tumor) to identify an optimal concentration of the nanoparticles so as to achieve spatial confinement of the damage to the tumor region. Optical interaction is coupled to the biological heat transfer through Pennes’ bioheat model and Beer’s law. Spatiotemporal thermal damage is simulated through the Arrhenius method. The finite difference implicit method is used to solve the coupled phenomenon. Results show that there is a specific value of nanoparticle concentration at which it is possible to confine thermal damage to the tumor within a spatial scale of less than 1 mm. This way the healthy tissues surrounding a tumor are safe. This optimum value of nanoparticle concentration (irrespective of tumor diameters) is 0.00001%. This concentration along with irradiation intensity of 1 W/cm2 for irradiation duration of 110 seconds is sufficient to thermally ablate the considered tumors. Novelty of this study is that it presents a combination of the controlling parameters for achieving a high (<1 mm) spatial confinement of the thermal damage. This finding is very much significant from clinical point of view. Clinically it is always desired to attain the therapeutic efficacy with minimal delivery of external agents (nanoparticles in this case) to a patient.

Abstract (sommario):
Laser interstitial thermal therapy (LITT) is a neurosurgical procedure that involves using heat treatment to ablate glioblastomas in the brain. Current methods for placing probes in LITT involve straight trajectory pathways. This limitation often requires surgeons to make multiple trajectories or leave undesired margins. There has been extensive work in steerable needles, concentric tube cannulas, and flexible surgical tools. In this work, we present an approach which focuses on providing steerability to tools that aren’t inherently steerable. To do this, we developed a curved port delivery system that leverages an active cannula for the deployment of a plastic, flexible port that delivers existing surgical tools. We present an initial prototype coupled with feasibility results illustrating that the port can be placed to steer probes to a desired location.

Abstract (sommario):
This paper describes the impact of structural deformations on interstage cavity flow dynamics by adopting thermal fluid-structure interaction methods. These coupled numerical approaches solve the fluid-solid heat transfer in conjunction with the geometrical deformation due to mismatched centrifugal and thermal expansion of rotating and stationary turbine discs. Especially the changing clearances at the interstage labyrinth seal, at the rotor blade tips and at the rotor stator rim seals can be captured to calculate the correct flow physics at these locations. A manual explicit coupling approach in ANSYS is utilized that couples the CFX CHT solver with the FE solver Mechanical. The validation of a 3D sector model with experimental data shows improvements in predicting the metal temperature of the rotating walls but also disclose problems with the overheated stationary parts, mainly due to the utilization of steady state mixing planes. Additionally, a surrogate 2D model of the 3D model is introduced to compare the explicit coupling approach with an implicit approach exploiting the ANSYS MFX interface between the fluid and the solid domain. Thereby, the manual coupling approach reveals to be much more efficient for the examined thermal fluid-structure interaction.

Abstract (sommario):
Abstract Remanufacturing is a crucial component for our societies to move toward a circular economy. Compared to new manufacturing, the distinctive nature of remanufacturing is found to have high variability, high uncertainty and, thereby, complexity. Therefore, remanufacturers need to enhance their ability to adjust their systems flexibly. Especially, the ability to reconfigure the production planning and control is crucial for reacting to the high variability and uncertainty. However, few practical methods to do that are available so far. Therefore, to solve this problem, this paper aims to propose a method for designing processes of production planning and control in remanufacturing based on the concept of loosely coupled systems. In the proposed method, Design Structure Matrix (DSM) is applied to identify loosely coupled subsystems that enable to localize impacts of changes within themselves. These subsystems are also utilized to appropriately determine leverage points, as well as allocate human resource. Through the application to a real case of remanufacturing, the proposed method was found to be effective for reconfiguring teams and processes for production planning and control depending on given uncertainties, as well as performing production planning and control activities efficiently.

Abstract Recent advances in the methods of preparing transmission electron microscope (TEM) samples using the focused ion beam (FIB) are presented. In particular, automation of TEM sample preparation is described. These automation methods, coupled with advances in ion column design, make it possible to prepare samples more efficiently and with less user involvement, thereby increasing FIB system utilization. Issues relating to sample handling and process-induced artifacts are also discussed.

Abstract (sommario):
The laser spark has been used extensively in the laboratory to provide an elemental analysis of metals.1 This method, called Laser-Induced Breakdown Spectroscopy (LIBS)2, uses a laser spark to vaporize and excite a material. The spark light is then spectrally and temporally resolved, as in other methods of atomic emission spectroscopy, to determine the elemental composition. The method has many potential advantages over more common laboratory-based methods like atomic absorption spectroscopy and the inductively coupled plasma. These advantages include simplicity, minimal sample preparation, possibility of non-invasive and in-situ analysis, and rapid response. On the other hand, the accuracy and precision of LIBS measurements are generally inferior to the laboratory methods thereby cancelling somewhat the advantages of LIBS for routine analyses. In some field-based applications, however, a high degree of analytical performance may not be required. These include prospecting, industrial process control monitoring, and metal identification. The latter area, which includes alloy sorting and identification of stockpiled metals, offers the best chance for immediate application and so has been examined in detail.

Abstract (sommario):
Review question / Objective: We provide a protocol to evaluate the efficacy of integrated Shoutai Pill and Western medicine to update the evaluation for the best available and security treatment for recurrent pregnancy loss(RPL). Condition being studied: Recurrent pregnancy loss (RPL) is a distinct disorder defined by two or more consecutive pregnancy failures before 20 gestational weeks infertile couples. The incidence of this disease accounts for about 1%-5% of women of reproductive age and seriously affects their physical and psychological health. At present, the known etiology of this disease mainly includes abnormal anatomic structures, genetic abnormality, endocrine disorders, prethrombotic status, abnormal immune function, infection, etc. Excluding the above factors, approximately 40-50% of RPL remain unexplained, known as unexplained recurrent pregnancy loss (URPL). At present, the main therapeutic methods of RPL are surgical therapy, preimplantation genetic diagnosis (PGD), hormone therapy, anti-infection therapy, anticoagulation, and immunoregulatory therapy, etc. However, there is no effective treatment has been identified for URPL. Therefore, we still need to investigate effective treatments to reduce pregnancy losses and maintain successful pregnancy preservation in these patients.
