This chapter provides contemporary perspectives on the issues of recovery assessment and monitoring, as well as an overview of the criteria-related discharge following anesthesia outside the operating room. The need for appropriate monitoring in the recovery period is argued based on data from an outcomes and claims registry. Discharge scoring systems are also discussed together with pertinent postanesthesia issues that may delay discharge from the postanesthesia care unit, such as nausea and vomiting, significant pain, as well as the need for mandatory patient escort. The standards and guidelines mentioned in this chapter apply not only to the anesthesiologists, but also to other physicians who supervise the recovery of patients in different units.

For the past 13,000 years Indians in the North American Great Plains hunted bison (Bison bison and B. antiquus) in large communally organized drive operations. This chapter briefly describes the taxonomy of fossil and living bison, the behaviour of modern bison, and what is known from ethnohistoric and archaeological sources about the ways that Indians conducted these drives, including the use of foot surrounds, cliff jumps, arroyo traps, and pounds (corrals). The chapter concludes by considering whether such drives were conducted annually in the late fall and/or early winter as a means of winter provisioning; or instead were conducted periodically, but not necessarily annually, and at many different times of year, as a socio-political mechanism for integrating otherwise widely dispersed and highly mobile hunting bands.

Continuing research on the Early Agricultural period has bolstered our understanding of early farming technology, community structure, and chronology. Environmental factors clearly affected the pace of agriculture’s spread, as did social interaction and population movement. Recent projects have contributed new information on how irrigation communities were organized, and how early farmers worked on the landscape. Much of the research conducted over the past thirty years has been conducted under the aegis of Section 106 of the National Historic Preservation Act of 1966. Methodological and theoretical advances have broadened the scope of study, but international and interdisciplinary cooperation remain goals to further this research.

This chapter illustrates how the Philharmonic had completely transformed itself in just eight years. It had silenced concerns about its artistic integrity, withstood challenges to its financial viability, and broadened its listener base. Judson played an integral role in the makeover. His managerial skills provided a steady hand, as he helped to direct Clarence Mackay's enthusiasm into shoring up the Philharmonic's foundation. But the 1930s had a new batch of challenges, particularly at the Philharmonic, which came to be increasingly reliant on guest conductors. Even though Arturo Toscanini was the face of the Philharmonic in the early 1930s and served as its principal conductor from 1930–35, the Philharmonic was far from being a single-conductor orchestra under his reign.

This chapter focuses on the activities of the Mormon Tabernacle Choir, first under Jay Welch and later Jerold Ottley. When Harold Lee ascended to the Mormon Presidency, he ramped up the musical forces of the Church of Jesus Christ of Latter-day Saints. He called dozens of new personnel, most of them academics, to a new Church Music Department that replaced the old Church Music Committee. Church President Spencer Kimball decided to replace Richard Condie with Welch, the conductor of the Mormon Youth Symphony and Chorus. Welch agreed to direct both the Tabernacle Choir and the Mormon Youth Symphony and Chorus, but he would soon be replaced by Ottley. This chapter first considers Welch's vision of Choir programming before discussing Ottley's initiatives as Choir conductor, along with the Choir's duties and achievements such as recordings, broadcasts, concerts, funerals, conferences, and international tours.

Kyle Smith, Jack Keating, and Ezra Stotland proposed the empathic–joy hypothesis, which claims that people feeling empathic concern help to get the pleasure of sharing vicariously in the joy that the target of empathy feels when his or her need is removed. They predicted that if those feeling high empathy for a person in need don’t anticipate feeling empathic joy, they will help no more than those feeling low empathy. Smith and colleagues conducted an experiment to test this prediction and claimed support. However, this claim relied on turning away from their experimental manipulation of empathy to a (probably inappropriate) assessment of self-reported empathic concern minus distress. Three additional experiments conducted to test the empathic–joy hypothesis found no support. Results instead consistently patterned as predicted by the empathy–altruism hypothesis. Again, it seemed time to accept the altruism hypothesis, but again acceptance was premature. Others researchers suggested three new possibilities.

With Franklin Roosevelt in the White House, the Democratic Party gained a firm grip on the American electorate. This finding is based on an examination of close to 200 polls that probed party identifications, beginning in 1937; all but a few were conducted by the Gallup Organization, albeit without any resonance in the press or the halls of academe. In a manner of speaking, FDR made America safe for Democrats. At the same time, according to polls conducted in real time, key groups of the fabled New Deal coalition were less conspicuous than has been widely believed. In the big cities, in the working class, in union households, and among Jewish as well as black Americans, Republicans and Independents combined outnumbered Democrats. Other than among Americans on relief, the Democratic Party enjoyed overwhelming support in only two sizeable groups in real-time polls in the Roosevelt era. They happened to be the same groups that Democrats had been counting on long before the New Deal and the Depression: the white South and Catholics.

In 1899, the Royal Baking Powder Trust bribed the Missouri state senate to pass a law that made alum baking powder poison. People were arrested for selling baking powder. Royal introduced this legislation across the U.S. but the American Baking Powder Association fought successfully. Through repeated bribery, the law stood until 1905, after Lincoln Steffens exposed it in a seminal article against lobbying called “Business as Treason.” Missouri indicted Royal’s William Ziegler but the governor of New York refused to extradite. Finally, charges were dropped or the “boodlers” were acquitted. At the same time, the U.S. Congress conducted hearings on the Pure Food law, passed in 1906.

Most accounts of corruption in Africa and the Global South are generally overly simplistic and macro-oriented, and commonly disconnect everyday (petty) corruption from political (grand) corruption. Contrary to standard approaches, Daniel Agbiboa offers a fresh and engaging look at the corruption complex in Africa through a micro analysis of its informal transport sector, where collusion between state and nonstate actors is most rife. Focusing on Lagos, Nigeria’s commercial capital and Africa’s largest city, They Eat Our Sweat investigates the workaday world of road transport operators as refracted through the extortion racket and violence of transport unions in complicity with the state. Steeped in an embodied knowledge of Lagos and backed by two years of thorough ethnographic fieldwork, including working as an informal bus conductor, Agbiboa provides an emic perspective on precarious labor, popular agency, and the daily pursuit of survival under the shadow of the modern world system. Corruption, Agbiboa argues, is not rooted in Nigerian “culture” but is shaped by the struggle to get by and get ahead on the fast and slow lanes of Lagos. The pursuit of economic survival compels transport operators to participate in the reproduction of the very transgressive system they denounce. They Eat Our Sweat is not just a book about corruption but also about transportation, politics, and governance in urban Africa.

In 1976, Israel conducted a successful, but highly controversial military rescue operation in Entebbe, Uganda, to save its nationals taken hostage on Ugandan territory by members of the ‘Popular Front of Liberation of Palestine’. From an international legal perspective, this case revolves around the existence of a right of a state to take military action to protect its nationals abroad in mortal danger. Following an extensive legal debate in the Security Council on the incident, it appears safe to conclude that a rescue operation such as conducted in Entebbe passes the threshold for a use of force within the meaning of Article 2(4) UN Charter. In light of the ambiguous justification, however, it appears that an Entebbe-type situation falls within a grey area of the prohibition of the use of force. Still, the incident suggests that if states are willing to support the legality of a military rescue operation only the right of self-defence can conceivably justify such a use of force, and only in a case where the local state does not itself deal with the threat in good faith, and under strict conditions of proportionality.

Depression in palliative care is common, under-recognised and has significant impacts for sufferers. There are effective treatments but often a shortage of staff to provide them. This chapter sets out a number of key issues to consider when assessing and treating individual patients and considers the way in which palliative care services can innovate to provide a population level response to depression. Palliative care staff can be trained to deliver basic depression care and follow simple protocols to initiate, monitor and adjust antidepressant treatment. These approaches have been tested in trials in cancer care but the challenge is to take these approaches from research trials conducted in centres of excellence with good resources, to other settings.

The differential diagnosis and approach to patients with high or low serum potassium are described. Patients with either abnormality have an increased mortality in large population-based studies. Most have significant renal, cardiovascular, endocrine, liver, or gastrointestinal disease. They are frequently taking prescription or other drugs and the evaluation of their electrolyte disorder should not be conducted in isolation, but within the context of their disease or diseases. The presence of isolated hypokalaemia or hyperkalaemia in the absence of these other diseases or any apparent drug administration should prompt the clinician to re-consider the clinical history and the reported laboratory values.

This chapter draws principally on the authors’ research in Panama (Nasua narica) and Argentina (Nasua nasua), but also discuss insights gained from work conducted in the United States, Mexico, Guatemala, Costa Rica, and Brazil. Preparing large ensembles for performance involves musical, social, logistical and financial challenges of a kind seldom encountered in other forms of collective music-making. The conventional approach to meeting the challenges that arise during rehearsal is to appoint a single musical overseer, usually a conductor, whose ostensible role in musical preparation is to directly influence the musicians while working towards the creation of a musical product to be delivered in later performances. Rehearsal leadership, viewed from this perspective, moves predominantly in one direction, from conductor to ensemble. But such a perspective oversimplifies the conductor’s relationship with the ensemble, the relationships between the musicians, and the strategies that the latter must employ when working in large ensembles. Conceptualizing the ensemble as a complex system of interrelated components, where leadership and creative agency are distributed and developed through rehearsal to achieve what audiences assume to be a unified whole, yields new understanding of the work of large ensembles. This chapter examines these components of the creative process in orchestral and choral rehearsal and performance, the internal and external forces shaping and constraining that process, and the approaches that individual musicians and conductors could adopt in response to the changing contexts in which such creativity might be manifested.

This chapter introduces the organization of the Lower Austrian Estates including the main assemblies and officeholders. The Estates’ presiding officer was the ruler-appointed Landmarschall, who was always drawn from the Estate of lords. The Estate of knights provided the Landuntermarschall. The Estates met and conducted business in differing combinations in a variety of assembles: the annual diet; the “three upper Estates” of prelates, lords, and knights; and three standing committees. One of these committees, the college of Deputies, constituted the Estates’ directorial panel, which was also a key administrative conduit between central power and the localities. Aristocrats and great abbots dominated, but did not monopolize, these bodies.

Abstract (sommario):
This chapter offers a critical discussion of the origins, aims, and main contributions of moral philosophies of international law. Then, in moving beyond mere exposition, the chapter turns to a meta-theoretical discussion of international law, and in particular to how international legal theory should best be conceived and conducted. The chapter argues for the development of normative legal philosophies of international law that take the normativity of law and hence its legality more seriously than international legal theorists have so far, but also than moral philosophers of international law have themselves, thus breaking away from the sterile oppositions between ‘realist’ and so-called ‘moralist’ approaches to international law.

This chapter records Michigan Anishinaabe women's long history of occupational mobility and creative adaptation against the impositions of federal policies, from women's earliest involvement in the global fur trade of the seventeenth century to waged and entrepreneurial service in tourism of the Upper Peninsula. Enriched by interviews conducted in the early 1990s with women of the Saginaw Chippewa, the chapter focuses on the postwar-era generations of women and their efforts to gain entry to postsecondary education and subsequently to white-collar and professional labor. It shows how they secured opportunities unavailable to their mothers but only because foremothers were so resourceful and persevering.

The first selective service registration on 5 June forced Schumann-Heink’s sons, the members of the ODJB, and millions of other American men between twenty-one and thirty to register for the draft. George M. Cohan’s song “Over There” was introduced to the public by singer Nora Bayes. James Reese Europe played a concert before seven thousand on 22 June but underwent thyroid surgery days later. The ODJB took a two-week vacation, during which Dunbar’s Tennessee Ten took their place on the Reisenweber’s bandstand. The board of the Oratorio Society of New York replaced popular conductor Louis Koemmenich with Walter Damrosch against the wishes of the membership.
