Community Health Centers (CHCs), designed to provide accessible and affordable health care services to low-income families, were first funded by the Federal Government as part of the War on Poverty in the mid-1960s. Improving healthcare organizational performance efficiency is paramount. It is an especially pressing need for CHCs' because they carry a disproportionate burden of caring for the uninsured within limited budgets. Prior studies suffer from conceptual and methodological limitations. A longitudinal multivariate analysis of factors influencing the performance of CHCs is needed. The purpose of this study is to benchmark CHC performance in terms of technical and cost efficiency, and examine factors that affect its variation. A theoretically grounded non-experimental study design is used, with five waves of panel data from 493 CHCs for the years 2000 through 2004. This study found that data mining and predictor tree analysis of factors influencing the variation in CHCs' technical and cost efficiency yielded inconsistent results. A declining trend in technical efficiency scores over the five-year study period was observed. Based on growth curve modeling, the three factors that influenced technical efficiency at the initial period of the study are: the percentages of Medicare, Medicaid, and Hispanic population being served by the CHCs. The five factors that positively influenced the variation in cost efficiency at the initial period were: the initial score of technical efficiency, the percentage of Hispanic patient population, staffing mix (ratio of providers to total staff), pay mix (ratio of federal grant dollars to total revenue), and percentage of Medicare-eligible. The initial cost-efficiency score and the initial technical efficiency score are negatively associated with the growth trend of technical efficiency. The initial level of technical efficiency is not statistically significantly associated with the growth trend of cost efficiency. The two factors influencing the growth trend of cost efficiency are the growth trend of technical efficiency (with a positive influence) and the initial level of cost efficiency (with a negative influence). Analysis of the effects of contextual and organizational-structural variables on the technical efficiency and cost efficiency of community health centers found that the explanatory power of the predictors is much greater for cost efficiency than for technical efficiency. The study lends support to contingency theory and confirms the independent and additive influences of contextual and organizational predictors on efficiency. Irrespective of the efficiency measures, contextual factors have much more influence on CHCs' efficiency than design (organizational structural) factors do. The three study hypotheses supported by multivariate analysis are: technical efficiency is associated with contextual factors and organizational factors; cost efficiency is associated with contextual factors and organizational factors; and technical efficiency positively affects cost efficiency.

The purpose of this thesis was to redesign a commercial center in Miami, Florida in a manner that incorporates the needs of pedestrians as well as the automobile. In my research, I studied projects that had been successful at integrating cars in retail design. I applied the strategies learned from this research to the design of a center that creates a positive interaction of pedestrian and car traffic, addressing the needs of the surrounding community. I designed a master plan that includes a mix of residential, retail, commercial and parking space. The parking is designed so that the retail center is not dominated by surface parking. Rather, the automobile is introduced into the different layers of the proposed buildings. The design focused on connecting pedestrian plazas and parking areas beneath them through the introduction of light and greenery. The findings show how a shopping center might transform the area around it by including spaces for residential, civic, cultural and social functions, as well as for the automotive infrastructure that make those functions possible.

The problem addressed in this study is the lack of formal strategic planning in community college learning resource centers. Without guidelines, a process, and plans, libraries cannot achieve their fullest potential nor can they be effective in accomplishing goals. The purpose of this study is to develop guidelines for the planning process and design a plan for a community college learning resource center. The documents developed for the plan are based on Florida Community y College’s environment. Planning in libraries as expressed in the literature most often refers to building and facility design. Discussion of program planning has been extremely limited. This study focuses on programs' and services' planning instead of facility planning. The planning guidelines assist practitioners in coping with risk and uncertainty in order to ensure growth and viability in their learning resource centers. They show how to look at library trends, educational conditions, technological changes, competition, strengths, and weaknesses. Discussed throughout the document are how planning affects the whole organization and how the effects must be anticipated and managed. Clarification of objectives, mission, philosophy, and purposes of institutions are key components illustrated in the guidelines. How to ensure that goals are being met and corrective action initiated if needed are provided. The guidelines suggest that learning resource center staffs examine all facets of the organization to determine which elements should be maintained or phased out. A continuous exchange of information between the centers and their environments is recommended. That information should be accurate, timely, and comprehensive. Strategic planning is very topical in today’s uncertain environment. The guidelines were developed to enhance library management and achievement of goals. The project began with a literature review of planning in libraries and businesses. The origin of strategic planning, who utilizes it, and what results have been achieved were highlighted. Attention was focused next on determining whether strategic planning takes place in libraries. Information is extensive on how to plan, components of a plan, suggested planning frameworks, environments conducive to planning and how successful companies plan. Many sources are cited for suggestions on planning procedures. In establishing guidelines for planning, it was essential to search for trends, terminology, and authorities. Cleland, King, Steiner, Drucker, Rowe and Riggs are reviewed and discussed as major planning researchers throughout the document. The data gathering consisted of developing a plan-to plan, examining the structure of a plan, and developing planning guidelines. The plan-to-plan is a discussion and examination of nineteen items related to an organization's planning process. These items range from statements by the CEO, definition of key terms, and financial data to strengths and weaknesses. The definitions of strategic terms such as WOTS-UP and mission are given. Examples are shown on how the level of specificity becomes greater as one moves from the mission statement to the goals. It is illustrated how a learning resource center's mission must be congruent with its parent organization, the community college. Directions are given on the need for an explicit mission. Several items influencing the mission statement are discussed: the organizations. Philosophy, self-concept, public image, management style, and work environment. The environmental scan is reviewed, readers are given six key areas to consider in determining the status of the organization's environment: economic, political, social, technological, geographical, and competitive forces. How to conduct a WOTS-UP analysis is also included. The WOTS-UP is next in the sequence of understanding the environmental factors, opportunities, and threats in order to effectively achieve a niche for formulating strategy. The second portion of the investigation examines the structure of planning. The motivation for this aspect centered on the lack · of planners understanding how to plan, what is contained in a plan, how to organize the responsible person, and evaluative monitoring of the written plan. These facets range from background information, resources, target dates and contingency plans to evaluation and control activities. Shown are Gannt and PERT charts to be used in placing planning in a time frame to indicate project completion. Numerous planning charts are provided to monitor the progress of goal achievement. Evaluative tools are provided to determine appropriate action for redirecting activities when performance is unsatisfactory. The final segment of the investigation presents planning guidelines. Specific directions, suggestions, procedures, strategies, ideas and methods of organizing and implementing the written plan are given role of leadership, these guidelines also discuss the mission establishing, strategy formulation, and evaluation. Implementation seems always to present problems for organizations. Many of these problems center around the fact that plans are expected to be carried out by people who were not involved in the original formulation. A checklist is included that summarizes many of the points planners should provide adequate responses to for effective plan implementation. Evaluation principles and checklists by Steiner, King and Cleland are illustrated. A list of recommended sources for use in strategic planning is provided in the references.

The aim of this thesis is to develop a vocabulary of principles to be used in the design of future community centers in Jordan. Community centers provide the stage for bringing members of the community together to meet and discuss community issues, and for increasing their sense of community spirit which advances social and economic development. This thesis rethinks the present idea of a community center taking into account the changing social structure and culture, the regional climate and availability of local materials. The principles that evolved may be divided into two categories; spatial vocabulary, which focuses on the relationship and design of spaces that encourage interaction amongst members of the community; and character vocabulary, which focuses on the character reflected by the community center as being the heart of the community. This thesis was investigated through the process of redesigning a community center in one of the housing projects built by the government to house low-income families.

Within the past ten years, there has been increasing interest in bringing retail back into inner-city neighborhoods as a commercial revitalization and economic development strategy. In this thesis, a large data set of all US shopping centers is analyzed to examine general trends in shopping center development, as well as trends in inner-city shopping center development over time. This research showed that inner-city centers are fairly similar to those in other parts of the MSA, although they are smaller, denser, and house a slightly different profile of retailers. During the 1980s, shopping center development in the inner city increased along with development in the rest of the metro area. As the overall market for shopping center development declined in the 1990s, inner-city development has declined even more sharply. And, while there is a major trend in the rest of the metropolitan area toward increasing renovation, expansion, and redevelopment of older centers, shopping centers in the inner city are not experiencing this surge of re-investment.

This thesis also examines the case for economic development through shopping center development, and explores the local community impacts of shopping center development through brief case studies of three inner-city shopping center projects in Houston, Boston, and Baltimore, with the goal of presenting how successful projects came about, why they were attractive to potential tenants, and what their impact was on the neighborhood.

This study investigated the use of storytelling in order to support childrenâ s emergent literacy in Malawiâ s resource deprived Community Based Childcare Centers (CBCCs). It focused on the professional development of four caregivers from four CBCCs following a Formative and Design Experiment model, and using qualitative methods of inquiry. The professional development on storytelling was designed following an informative two-week observation period. Following the observation, a daylong professional development was organized to train the caregivers. In the seven-week intervention period that followed the professional development, the main focus of the study was on the perceptions of the caregivers about their participation in a professional development on storytelling in CBCCs, their responses to using storytelling, and their perceptions about childrenâ s responses to using storytelling in CBCCs. Data that informed the study comprised caregiversâ reflective notes in their journals, individual caregiver weekly interviews, weekly focus group discussions, and research reflective field notes that were collected over seven weeks after the professional development. Findings suggested that the four caregivers found the professional development beneficial to them all. However, out of the four caregivers, three of them and their respective children demonstrated benefit from storytelling, growth in knowledge and development of storytelling skills. The three caregivers reported becoming more connected with the children, understood them better, found storytelling to be a teaching approach, and felt that their teaching was made easier and enjoyable. The children taught by these three caregivers enjoyed their learning and even resourced stories from their communities. They too, became storytellers. In the end, the caregivers felt that they were ready to share their experiences with other caregivers in Zomba District in Malawi. These findings suggest that storytelling could be used in support of emergent literacy at a larger scale, as well as serve as springboard for pedagogical training of the caregivers culminating in the development of locally available teaching and learning resources in the Malawian CBCC.

Class of 2017 Abstract
Objectives: A chart will be used to assess the need for the asthma education program. The chart review aims to: analyze the refill history of asthma medications along with verifying adherence via electronic health record and assess if patients are getting appropriate asthma therapy per 2007 NHLBI asthma guidelines. Methods: The patient’s MHC Healthcare electronic record profile and pharmacy refill record will be accessed. The information that will be obtained from the electronic health record will be: ethnicity, age, sex, allergic rhinitis diagnosis, flu vaccine status, pneumonia vaccine status, GERD diagnosis, spirometry measure if available, use of prednisone for exacerbation, prescribed asthma therapy, and provider’s (MD, NP, PA, etc)’s notes that might indicate nonadherence. The information that will be obtained from the pharmacy record are controller and rescue medication refill history. The information will be recorded in paper data collection forms and electronic versions will be saved to the secure college of pharmacy workgroup. The raw physical information described above will be stored in a locked cabinet at MHC healthcare pharmacy inside the pharmacy director’s office and the de-identified information will be stored in the secure workgroup. A compreensão do funcionamento cognitivo no envelhecimento torna-se fundamental na atualidade diante do crescimento desta população. A presente dissertação, elaborada a partir de dois artigos, teve como objetivo investigar o perfil cognitivo, funcional e sintomas de depressão em idosos residentes na comunidade. O protocolo neuropsicológico utilizado em ambos os estudos foi composto por instrumentos de rastreio validados para a população brasileira. O estudo foi realizado com 298 idosos inseridos nas atividades oferecidas pelas Casas de Convivência da Prefeitura do Rio de Janeiro. O primeiro estudo investigou o perfil cognitivo e funcional destes idosos e identificou um padrão de normalidade na amostra, apesar de um desempenho abaixo do esperado no Teste do Desenho do Relógio. Entretanto, 20,9 por cento dos idosos apresentaram sintomas leves de depressão. Desta forma, o segundo artigo avaliou estes idosos com sintomas de depressão, comparando-os com um grupo controle pareado por idade e escolaridade. Os resultados evidenciaram que os sintomas depressivos interferiram nas atividades instrumentais de vida diária, na memória episódica anterógrada e na fluência verbal. Os estudos apontaram a importância da investigação neuropsicológica em idosos para uma maior compreensão da senescência, bem como os fatores que interferem neste processo e que representam um risco potencial para uma evolução para transtornos cognitivos leves ou estágios pré-clínicos de quadros demenciais.
Understanding how the aging process affects cognitive functioning is crucial nowadays, given the growth of the elderly population in the world. This dissertation, comprised of two research papers, aimed to investigate the cognitive and functional profile, as well as symptoms of depression, in community-dwelling elderly adults. The neuropsychological protocol used in both studies consisted of screening tools validated for the Brazilian population. The studies were conducted with 298 elderly adults who participate in the activities offered by the government in the city of Rio de Janeiro. The first study investigated the cognitive and functional profile of these subjects and identified a pattern of normality in the sample, despite a below average performance in the Clock Drawing Test. However, 20,9 percent of the elderly had symptoms of mild depression. Thus, the second study specifically assessed these subjects, comparing them to a control group matched for age and education. The results showed that depressive symptoms significantly interfered with instrumental activities of daily living, anterograde episodic memory, and verbal fluency. The studies pointed out the importance of neuropsychological research of elderly adults for a greater understanding of senescence, as well as factors that interfere with this process and represent a potential risk for progression to mild cognitive impairment or preclinical stages of dementia.

The purpose of this paper is to investigate retail site selection criteria in community centers and ascertain the implications of these criteria for the retail real estate developer. Historically, real estate developers contemplating the development of a community center will acquire a site based on hunches, experience, and a limited amount of data. Real estate academics, on the other hand, have produced numerous mathematical models and methods for retail site selection. However, to the real estate developer the retail community shopping center has remained elusive. Not only will developers oftentimes face the prospect of site selection without a complete picture of who their tenants will be, but they will be left to their own experience to understand in depth the needs and preferences of typical community center tenants. This paper will explore current methods and site selection criteria used by leading community center tenants for site selection and will investigate how that information can be implemented by the developer to improve the ir approach to retail site selection.

In the United States, Mosques and Islamic centers have existed for many decades to meet the social and religious needs of Muslims. However, since Muslims are a minority in this country, they still face some problems since the practice of various Islamic laws and prohibitions sometimes are inappropriate with the Western way of life. Assessing Muslims in the American context, a key issue to consider is the degree to which Muslims may become socially integrated into the American culture. Here the role of the Islamic center, as an institution, is significant in helping Muslims to meet each other and to adjust to the realities of life in America.In general, an Islamic center involves a concentration of facilities for activities which have the characteristic of Islam. Therefore, there is no basic difference in function between a Mosque and an Islamic Center, since the Mosque traditionally is not only a house of worship, but is also the center of Islamic society and culture. The term Islamic center is used in this study to emphasize the interest in the social side of Mosque activities, in addition to religious functions, that might be more appropriate with Muslims life in the American context.The purpose of the Islamic center is to promote a better understanding of Islam and greater acceptance and appreciation of its truth, culture, and contribution to human civilization. As we know, the ethos of Islam lies not only in the connection of individual with God, but also with human relations in the social order.
