Abstract Background Compassionate caring and dignity are key priorities in current policy and research agendas and are central to the quality of experience for patients, families and staff. Developing relationships has been identified as a key component in enabling excellence in caring to be realised in practice. However there is little evidence that identifies the processes involved in delivering compassionate relationship-centred care. This study sought to address this gap in the knowledge base. Aims and research questions The study's aim was to examine and evaluate processes that enhance compassionate relationship-centred care within an older people care setting in an acute hospital. Key objectives were: to develop an understanding of the concept of compassionate relationship-centred care within the practice setting through exploring the views, perceptions and experiences of staff, patients and their families; to develop, implement and evaluate strategies that promote this concept; to examine the processes that need to be put in place to enable sustainability of these strategies; and to identify the lessons learnt to inform practice, education, policy and research. This study was part of a larger programme of work that aimed to integrate compassionate care across practice and education. Approach and methods My role as a senior nurse and practitioner researcher meant that I was in a unique position to be able to capture not just theoretical views of compassion but how this was enacted in practice. To do this the study used the approach of appreciative inquiry. A range of methods was used: participant observation, stories using emotional touchpoints, photo-elicitation, and group discussions to explore beliefs and values. Data were continually fedback to staff participants to involve them in analysis. An iterative and inductive process of immersion crystallization was used to analyse data. Findings A key finding was the development of a practice model to support practitioners to deliver compassionate relationship-centred care. This model suggests that, in order to deliver such care, people need to engage in the process of appreciative caring conversations in order to understand a) who people are and what matters to them; and b) how people feel about their experience. This in turn enables a process of working together to shape the way things are done. The findings support the notion that during these caring conversations the practitioner needs to connect emotionally, be curious, collaborative, able to compromise, considerate of others perspectives, courageous and actively celebrate when practices have worked well in order to promote compassionate relationship-centred care. This framework comprises the 7 ‘C's of caring conversations and makes a unique contribution to the body of knowledge in providing practical guidance as to the ‘how' of compassionate relationship-centred care. Key outcomes of implementing this model were that people felt comfortable to express emotions, developed stronger relationships, were more consistent in delivering compassionate care practice across the team, and had a sense of learned hopefulness in the face of complex and competing demands. Conclusions and implications Implementation of activities in practice to support this way of working revealed that these processes are complex, often requiring the practitioner to takes risks and therefore the provision of appropriate support, facilitation and strong leadership are important factors in helping to sustain such practices. The outcomes of this research build upon the existing knowledge base by providing a practice model that specifies how to deliver compassionate relationship-centred care, and they demonstrate the impact of using appreciative approaches to facilitate improvement within health care contexts.

Research has evidenced variously the impact on those caring for traumatised people, such as wives of distressed police officers, partners of war veterans and trauma counsellors. However, there is a lack of research exploring the impact on foster parents (FPs) caring for traumatised looked-after children (LAC). This study aimed to explore FPs’ experiences of caring for traumatised LAC, including their understanding of the impact the trauma has on the young person and on themselves and their biological family. Eleven FPs were interviewed to elicit their personal experiences of caring for traumatised LAC and its impact on them, the young people and the FPs’ families. Transcripts were analysed using Interpretative Phenomenological Analysis (IPA). Six master-themes emerged from the data: emotional impact on foster parents, cognitive impact on foster parents, impact from the wider fostering system, impact on foster parents’ family and friends, foster parent coping and perceived understanding of the impact of trauma on LAC. These themes were linked to previous research and existing theoretical constructs, such as secondary trauma (ST), vicarious trauma (VT) and compassion satisfaction. Whilst all related experiences within each of the themes, participants differed between which themes dominated. FPs experience a variety of complex negative impacts from caring for traumatised LAC, that warrants further investigation and development of screening tools to measure potential ST/VT symptoms. However, several FPs reported a wealth of positive experiences from their role, which may or may not counteract these symptoms.

Siblings can be seen as a huge resource in caring for adults with a diagnosis of schizophrenia: Many expect to take over the caring role of caring when their parents no longer can, and positive sibling relationships are related to better outcomes for adults with a diagnosis of schizophrenia (Smith, Greenberg and Seltzer, 2007). Despite these facts research into family carers has tended to neglect exploring sibling relationships until recently. Previous research has focused on the negative aspects of caring, and the coping strategies of the siblings. Very little has considered the impact of caring on the sibling relationship over the life-cycle, and why some siblings may offer support for their diagnosed siblings, while others do not. This study aimed to explore the sibling relationship over the life-cycle. Six adults with a sibling diagnosed with schizophrenia were interviewed about their experiences growing up with and caring for their sibling over their life-cycle. Data was analysed using existentialist-informed hermeneutic phenomenology. Following this analysis three superordinate themes were identified, these were: the ‘terrible weight’ of having a sibling with a diagnosis of schizophrenia, evolution of the caring role over time, and expectations for their life-cycle. The findings from this study support previous findings showing that caring for a sibling with a diagnosis of schizophrenia is a difficult experience. This study also showed that the participants’ ability to care for their sibling increased over time, related to access to have some emotional distance, learning to care for themselves, gaining knowledge about schizophrenia and their sibling’s idiosyncrasies and being able to negotiate a caring vs a sibling relationship. The findings suggest that the participants felt a sense of obligation to care for their diagnosed siblings and that taking on this role might impact on their life choices, such as to have children. Limitations of the study, the implications for clinical psychology and recommendations for future research are discussed.

Corporate sustainability comes in a variety of shapes and the boundaries defining what can be called sustainability are disputed. This thesis explores the making of sustainability at a medium-sized company in Sweden, in this study called HygieneTech, that actively works with sustainability. Inspired by theories from science and technology studies (STS), the author discusses how different versions of sustainability are created in practices. The study is based on a theoretical approach, actor network theory (ANT), which understands reality as performed rather than observed, and since reality and its objects are enacted differently in diverse situations, reality and objects are considered multiple. Moreover, inspired by Maria Puig de la Bellacasa, “thinking with care” enables the study to tell new stories about how companies work with sustainability. This view of the actors as doing sustainability through care practice in a corporate setting paves the way for a caring analysis, exploring the different versions of sustainability as different matters of care. The enactment of sustainability is studied through participant observation, interviews and document studies. The most frequently enacted versions of sustainability are sustainability as saving resources, sustainability through standards/labels, sustainability as caring for people, and sustainability as a conscious choice. The study shows how some versions of sustainability in one setting can clash, such as when the employees at HygieneTech have to decide between caring for saving resources or caring for labelled products. Even so, in other settings versions of sustainability can be added together and thus enable the employees to care for sustainability both as a marketing device and as a conscious choice. Furthermore, the thesis shows that sustainability is sometimes made absent in relation to other matters of care, such as cleanliness and economy, while in other settings economy and sustainability can be added together. Finally, the thesis shows how sustainability, while enacted in several versions, still can cohere though professional tinkering.
Hur företag arbetar med hållbarhetsfrågor kan se väldigt olika ut och många gånger uppstår stridigheter kring vad som får kallas hållbart. Denna avhandling studerar hur hållbarhet iscensätts i ett medelstort företag i Sverige som arbetar aktivt med hållbarhetsfrågor. Företaget är i denna studie anonymiserat och kallas HygieneTech. Genom att använda teorier från studier av teknik och vetenskap (STS) studeras hur olika versioner av hållbarhet skapas i olika praktiker. Studien utgår från ett teoretiskt angreppssätt, aktör-nätverksteori (ANT), som genom en symmetrisk analys studerar såväl materiella som mänskliga aktörer. ANT anser att verkligheten och dess objekt görs snarare än upplevs och eftersom verkligheten iscensätts på olika sätt i olika situationer, är verkligheten och objekten multipla. Genom att studera hållbarhet som omsorgspraktiker, inspirerad av Maria Puig de la Bellacasa, skriv nya typer av historier kring hållarbetsarbete på företag. Att tänka med omsorg (”thinking with care”) ger forskaren möjlighet att istället för att återberätta den traditionellt återkommande historien om företaget som en vinstmaximerande rationell agent, lägga fokus inte bara på rationalitet, utan även affektion. Hur hållbarhet görs i praktiken studeras genom deltagande observationer, intervjuer samt dokumentstudier. Denna avhandling visar på att det finns ett flertal olika versioner av hållbarhet som iscensätts vilka ofta är sammanknutna i varandra. Några av de viktigaste versionerna av hållbarhet i HygieneTech handlar om hållbarhet genom att spara resurser, hållbarhet genom märkningar/standarder, hållbarhet genom omsorg om människor och hållbarhet som ett medvetet val. Vissa av dessa versioner kan adderas samman, men i andra situationer ställs de mot varandra. Studien diskuterar även exempel på när hållbarhet görs osynligt på grund av att andra värden såsom renlighet och ekonomi istället sätts i fokus, samt hur hållbarhet trots sina olika versioner hålls samman.

The purpose of this study was to examine registered nurses' attitudes toward caring. The instrument used to measure nurses' attitudes was the Nyberg Caring Assessment Scale. The tool consists of a set of twenty statements asked four times to establish: a) ideal caring; b) actual caring; c) supervisor caring; and d) caring five years ago. A convenience sample of sixty three registered nurses from one midwestern hospital was used for this study. The surveyed registered nurses exhibited positive attitudes toward caring in today's cost containment healthcare system. There were significant correlation coefficients among the four sub-scales of caring. Results of this study have implications for nursing practice and nursing education.

The majority of literature concerning carers of people with acquired brain injury (ABI) concentrates on the negative consequences. Knowledge of post-traumatic growth (PTG) and positive gain from caring is expanding in the literature. The aim of this study was to explore the experience and report of PTG in carers of people with ABI. Chapter 2 looked at the self-report of PTG, positive and negative changes in relation to coping and mental health symptoms. The results of thirty-six carers suggest that carers will report at least one positive change as a result of being a carer. However, a stronger relationship between negative than positive changes for mental health symptoms and maladaptive coping. The experiences of being a wife of someone with ABI were further explored in Chapter 3. Thematic analysis using Interpretive Phenomenological Analysis (IPA) was used to analyse interviews of wives’ experiences and report of PTG. The themes identified relate to adjustment, coping and the life domains associated with PTG, in particular to interpersonal relationships and philosophy of life. Consideration of informal carers led to speculation of the motivations of professional carers, including psychological therapists. A literature review (Chapter 1) was conducted of the empirical evidence describing the influence of personality and family of origin on choice of career as a psychological therapist. The results are discussed in relation to methodological limitations, clinical implications and future directions for research. Chapter 4 offers reflections on the process of carrying out research and the impact, particularly of qualitative research, on the researcher. Some clinical implications from Chapters 2 and 3 are expanded to that contained within the chapters.

Narratives are used to explore personal beliefs and assumptions about caring in oneâ s personal and professional life. This dissertation recognizes the process of caring is interpretative and evolves from personal experience. I address issues of caring within the practice of nursing and nursing education from a feminist perspective. I begin with my own personal narrative in which I seek to uncover my own caring essence as a basis for inquiring into issues of caring and feminism in nursing education. Theoretical constructs from educational nursing and feminist literature are explored to develop a personal model of caring within nursing education. Nursing students must be educated within a caring learning environment so they can develop a caring stance with patients. Dialogue within teacher-student interaction is at the center of such an environment. This dialogue encourages authentic presence with students that leads to an intuitive knowing. Nursing students need to learn to let their intuitive sense guide the use of technology. Of course, that means educating their intuitions. Autobiographical vignettes are used to reflect on the experience of a nurse educator as caring guides teaching-learning activities in a nursing curriculum. Caring within clinical teaching encourages reflection and increases self-awareness. Clinical teaching is seen as an opportunity to unite theory and practice. It encourages students to be receptive to patients and places value on contextual experiences. An examination of contextual experience shows that care is relational and encourages connections with others. Care is sustained through relationships that give voice to nursing students and patients. This process of giving voice makes caring more visible to others. This visibility allows nurses to celebrate caring occasions and validates worth of caring in nursing. Caring practices within nursing education help students recognize the power within themselves to institute change. Nurse educators must encourage students to care for themselves. This process will help them stay in touch with what they need as individuals as they provide care to others.

Hospital care is changing, - dominated by the dollar and technology and the patient is taking second place. Nursing, traditionally known as being a caring profession, is not exempt from the changes and the traditional entrance interview to find caring students has been replaced by a computerised system. Graduates from university programs are being branded as non-caring. This research sought to find practical ways in which nurses showed caring to patients, and to develop a framework which could be used to teach and cultivate caring attributes in undergraduate students. A broad sweep of the past was taken to show how, dominated by disease and the necessity to take care of the suffering and infirm, nursing has been inextricably linked to the inability of science to cure. The discussion on the impact of the Therapeutic Revolution and a health care system, dominated by the dollar, places nursing and caring in perspective. Some philosophical, religious and psychological notions of caring were briefly explored. A participative paradigm underpins this research which uses a collaborative inquiry cyclical and reflective process of three phases. In Phase I a group of Graduate Registered Nurses reflected on the enacted/strategies/skills that were used to show caring to patients. Six key enactments were elicited from the total of 64 reflections that were submitted. Phase II constructed a framework that could be used to teach caring, which was then critiqued by an expert group of educators. Phase III returned to Phases I and II and used a process of reflection-on-reflection and a new emergent meaning of caring in nursing ensued. As a result of this reflection-on-reflection, a reconfiguration of the framework resulted in the development of the Care Connections model.

Teachers who care for their students are considered to be essential to a productive learning environment for students. The presence of such teachers in classrooms is therefore important if students are to experience the best possibilities for learning. This thesis examines the nature of caring teachers and explores both personal and contextual factors that influence teachers' caring practices, providing insight into how these teachers' practices may be sustained. Specifically, the thesis addresses three research questions: 1) How do caring teachers demonstrate care? 2) What are the personal factors that contribute to a caring teacher's approach? 3) What are the contextual factors that hinder and/or support a caring teacher's approach? The thesis presents two models: the first hypothesises that the demonstration of care includes both a personal and educational dimension; the second presents factors that may influence a caring teacher's approach. These models were explored using a mixed method approach within three secondary, co-educational Catholic schools in Sydney, Australia. Teachers within these schools were surveyed (N = 178), 10 peer-nominated caring teachers were interviewed and observed (n = 10), students were interviewed in groups (N = 33) and colleagues of the 10 teachers were surveyed (n = 13). Analysis of the data shows that there are a number of distinct caring practices utilised by caring teachers. These include (a) a focus on relationships with students, (b) attentiveness, (c) flexibility, (d) compassion, (e) recognition of limitations, and (f) an approachable manner. The practices of caring teachers were found to be motivated by 10 mindsets underpinned by an optimistic belief that change is possible and that help given will improve a situation. The study also reveals that the caring process involves three phases.;Mindsets are found to be the first phase of the educational caring process and provide both the rationale and motivation for caring. The second phase, called the inner response, consists of two key elements in a caring teacher's approach. The first element is the ability to notice and recognise the need for care, while the second is concerned with an emotional response to that need, usually in the form of compassion or concern. The final phase of the caring process involves three key aspects of care demonstrated through personal qualities, commitment and caring acts. The caring acts demonstrated by the teachers in this study confirm the first model based upon the literature review which placed educational and personal care as the two key dimensions of care. However, as a result of the data analysis, the model is modified to show that relationship is at the core of these two types of care and facilitates the caring acts. Investigation of the third research question provided much needed empirical findings with regard to the factors that affect caring. Results confirm many of the theoretical perspectives but also show that spending time with students is important to caring teachers in the support and maintenance of caring practices. Additionally, factors that hinder caring teachers include (a) tiredness and feeling drained, (b) students failing to respond to care, (c) lack of time, and (d) staff with different mindsets. Hindering factors are fewer in number than those that support. Both the hindering and supportive factors identified by the nominated caring teachers are somewhat different to those factors identified by other teachers. Based upon the results, the thesis presents a modified model of care for future research, along with recommendations related to the selection, training, induction, and sustaining of caring teachers.

Technology and the complexity of the patient care can take the nurse's attention away from caring for the patient to caring for the technology. The purpose of this systematic review was to gather evidence from the literature on tools for measuring caring and the outcomes in order to make that body of knowledge easily accessible to the direct care nurse for implementation. Jean Watson's theory of human caring 'the theoretical framework that guided and informed this DNP project' focuses on human caring processes and experiences. It assumes that effective caring promotes health and outcomes of the nurse and patient. By using such a theory, a systematic review of caring behaviors can illustrate how using caring behaviors with intention can improve patient outcomes as well as nurse satisfaction outcomes. Inclusion and exclusion criteria were identified and articles were appraised using the Johns Hopkins Evidence-based Practice Model and process that nurses at all levels have used to appraise research and non-research for practice changes. Articles were narrowed to10 that met criteria and caring behaviors were recognized. The chosen articles used 5 different tools to measure caring behaviors. Although the individual caring behaviors were identified, the diversity of the tools used to measure and assess caring behaviors did not provide an easy way to assess for commonalities. This review identified areas for future research including, how one tool can be used to measure caring at different levels of care, in an acute care setting, an whether there exists an opportunity to develop common terminology for describing caring behaviors? Future research on using caring behaviors with intention could change nursing practice by changing how nurses perceive their skills and tasks.

This study scrutinizes the mothering experiences of professional women living in istanbul. It is about class based mothering experiences of professional women, who are being influenced from both mothering and professionalisation ideology. It analyses many different aspects of mothering experience starting from the pregnancy period to the decision making about the socialization of children through their relation with the changing understanding of control on mothering experience. It is argued that professional understanding of mothering necessitates being successful and this necessity is supported by the mothering ideology assigning women as the primary responsible parent from child caring. Professional women, who have been considered as advantageous because of both their class positioning and professional role in the labor market, continue to experience the burden of the gendered structure of parenting. On the other hand, they have an active role in the reproduction of gender and class based structure.

This study begins with a premise that ‘caring’ is no longer as evident in nursing practice, which in turn has repercussions for understanding nursing as a ‘vocation’. The study, therefore, sets out to problematise both ‘caring’ and ‘vocation’ and in so doing identifies that whilst both are elusive ‘caring’ is particularly difficult to ‘pin down’ - in terms of abstracting a definitive definition, and importantly, how it is articulated in practice. By situating the study within social constructionism (Berger and Luckmann, 1991) and communities of practice (Lave and Wenger, 1991) and by additionally drawing on narrative inquiry, the study pays attention to the stories of six final year student nurses. By closely reading these accounts, through a number of theoretical frames, including Sheldon Stryker (1980), it becomes possible to glimpse some of the interactions and oscillations where an individual’s nursing ideals situates them in what MacLure describes as a ‘moral universe (2003: 9). It is by disentangling this ‘universe’ that I am able to catch some of the meanings that circulate around ‘care’ and which reverberate with the notion of nursing as a ‘vocation’. Further disentanglements, especially those relating to discourses and discursive power occur when Foucault is brought into the picture. Whilst this study is unable to provide a definitive account of what it means to care or provide guidelines for nursing as a vocation it does nevertheless raise a number of pressing and critical questions; questions that highlight the political, social, emotional and ethical work that student nurses have to undertake where hopes, beliefs and ideals in relation to ‘care’ have to find ‘some sort of place’. Moreover, by working with Judith Butler’s theoretical ideas relating to ‘performance’ I am obliged to (re)turn again to the data so as to radically (re)consider the means by which the participants are con(script)ed to perform care in some ways and not others. The study also illustrates the reflexive journey that I have undertaken, where my own ideological longings in relation to ‘care’ have been sorely tampered. It is argued that such tampering is a necessary irritant and component within the context of the university classroom where it obliges both me and the students to work together so that we can, together, (re)think and (re)configure what it means to ‘care’.

The thesis explores the meaning of caring in the lives of a group of people who are labelled as dependants, adults with learning difficulties. Through biographical interviews and documentary research the author examines how care for people with learning difficulties has developed over time in one local area; and the understandings people on the receiving end have of the care offered them by families and staff. The findings suggest that dependency is not acknowledged by the majority of people interviewed who present themselves as givers of care as much as recipients of care. The research identifies gender as an important variable in the way care and caring are understood and experienced. The research makes a contribution to the literatures on gender and caring; family; peer and staff relationships of adults with learning difficulties; the history of learning disability; and qualitative research with marginalised groups.

This research is investigating ways of promoting social change towards more sustainable practices around the consumption of decorative home textiles. Knowledge is built and a prototype developed by using transdisciplinary design theory and participatory design methods.  Motivators for this work are the sustainability goals defined by the United Nations, specifically reducing waste through prevention and building awareness for sustainable lifestyles (UN Sustainability Initiative, 2020); and a combined interest of becoming more accessible to more people and interacting with them there where they are from the point of view of product or service design, as well as working towards sustainability goals of IKEA (Ingka Group, 2020).  The context of home textiles has been chosen because textile waste is a growing problem and with the main emphasis being on issues relating to clothing items and fast fashion, problems with home textiles can be easily overlooked. However, with giant fast fashion companies like H&M and Zara entering the home decor market, there is a reason to focus on preventing the same kind of issues rising with home fashion textiles as have with fast fashion clothes.  The aim is to understand the behaviours and meanings surrounding home fashion textiles and to reduce the amount of textile waste going to landfills through prevention and mindful consumption. The purpose is to develop a vehicle of interaction between IKEA and consumers that emphasizes inclusion and the merits of co-creating to solve a problem in a sustainable way and benefits IKEA in their pursuits of cultivating a stronger relationship with the consumers while working towards their circularity goals.  The prototype proposed as a result is an evidence-based tool for communication which enables IKEA to access more people while offering both parties a chance to learn from each other while developing sustainable initiatives and mindsets.

There is presently an increasing trend towards deinstitutionalization of several different populations, including physically disabled adults. Although a variety of staffed resources provide for alternative care, 'home' and 'family' remain the primary resource in the community based care system. Given that the family of the 1980's has a decreased capacity to care due to such factors as increased mobility and loss of extended family networks we must be concerned with the continued viability of this resource. And, given that 'family’ is often a euphemism for 'nearest female relative' and that women's role in society has changed markedly in recent years the issue of women as caregivers is one of particularly urgent concern. This study is based on extensive interviews with eight women caring for disabled husbands. The interview schedule and methodology facilitated descriptive data regarding caregiving tasks as well as a comprehensive exploration of what was problematic about the day to day reality of caregiving for these women. The data analysis identified struggling with the 'role' of wife-caregiver, isolation and a changed marital dynamic as primary themes in the process of caregiving. This study provides some insight into what is problematic about caregiving for these wives and some suggestions for improving social work practice and providing more effective services in relation to this population.

Obwohl die lokale Ebene in der Pflegepolitik eine zentrale Rolle spielt, nehmen international vergleichende Studien dieses Feld bisher kaum in den Blick. Das Ziel von "Caring Cities" besteht darin, am Beispiel von Bratislava, Budapest, Prag und Wien, die Rolle zentraleuropäischer Städte im Bereich der Pflege zu untersuchen. In der "pflegenden Stadt" treffen die nationale und die lokale Ebene auf verschiedene Weise aufeinander. Einerseits manifestieren sich nationale Pflegeregime in einem spezifisch-urbanen Kontext. Andererseits sind lokale AkteurInnen selbst in die Regulierung, Organisation und Erbringung von Pflege involviert und verfügen so über einen eigenen gestalterischen Spielraum. Um dies zu analysieren, wurden quantitative Daten erhoben und Interviews mit ExpertInnen in den jeweiligen Städten geführt. Die Untersuchung bestätigt für alle vier Hauptstädte die große Bedeutung regionaler bzw. lokaler Akteure in der Pflegepolitik. Während Wien auf eine lange und relativ kontinuierliche Geschichte lokaler Sozialpolitik aufbaut, wurde in Tschechien, der Slowakei und in Ungarn Pflegepolitik mit der Transformation in den frühen 1990er Jahren verstärkt an die regionale und lokale Ebene übertragen. In diesen Ländern ist der Sektor aber auch durch ein hohes Maß an Fragmentierung und teilweise überlappende Verantwortlichkeiten charakterisiert. Dies wiederum ist Konsequenz einer vielfach fehlenden umfassenden und kohärenten Pflegepolitik. Angespannte öffentliche Budgets, vor allem in den jüngsten Krisenjahren, haben den Ausbau einer adäquaten Dienstleistungsinfrastruktur zusätzlich verschärft.

Through the theoretical lens of Foucault's archaeological method, this thesis undertakes a discourse analysis to examine how old age and ageing are strategically positioned as forms of governmentality in New Labour social care policy documents. It is argued that these discourses are not directed purely at the older generation, but at everyone, at all stages of life, encompassing all aspects of everyday living. Old age thus becomes a strategy of governing the population through individual everyday lives. This hints at the way ageing is prefigured, anticipated and lived in advance. An analytical method is developed by weaving together Foucault's notions of archaeology and governmentality; the latter is utilised both as an analytical perspective and to provide an understanding of how people primarily act and interact in contemporary Western societies. This analytical perspective is initially applied to an exploration of how the form and function of social policy enable ordinary practices of life to become targets of political government, making both possible and desirable the government of everyday living: governing how we ought to live in every aspect of life from work and finances to health, to personal relationships and leisure activities. The thesis progresses to explore this in more detail through a practical application of governmentality and focused discourse analysis of eight New Labour social care policy texts. The aim of the analysis is to explore what subjectivities and forms of life are possible within these discourses and therefore what these policies actually do, as distinguished from what they claim to be doing. It is argued that the discourses that emerge in these policies act to limit and subjectify, by attempting to contain and stabilise the multitude of possibilities for practices of living. By ostensibly aiming to create social inclusion the policies make possible vast areas of exclusion that become prime spaces of government. Thus many ways of living, ageing, and being old become untenable due to their inherent contradiction with the social values and rationalities upon which these discourses are based. Whilst governmentality analyses have been brought to many other policy areas, this thesis makes an original contribution by: developing a governmental analysis of social policy as a form of biopolitics; by applying this analysis to the social care field; and by using policy discourses of old age and ageing to draw out significant aspects of a governmental society. In particular it explores the dispersion of many traditional boundaries, leading to the rearrangement of relations, responsibilities and subjectivities.

Begreppet tillit är något sjuksköterskan och patienten kommer i kontakt med varje dag omedvetet. Begreppet tillit definieras som människans spontana livsyttringar. Det finns brist på vetenskaplig kunskap om begreppet tillit inom vårdvetenskapen, nursing och caring, kunskap om begreppet kan bidra till möjligheten att stärka hälsan i det goda vårdandet. Syftet var att belysa hur begreppet tillit uttrycks inom vårdvetenskapen, nursing samt caring. Studien är byggd på en litteratursökning med en datainsamlingsmetod av vetenskapliga artiklar. Resultatet visar på hur tillit upplevs inom nursing och caring. Patienten upplevde högre tillit till sjuksköterskan när tydlig och relevant information gavs, när patienten kände sig värderad och bekräftad av sjuksköterskan, när sjuksköterskan visar god attityd. Brist på tillit gav en känsla av osäkerhet och var svår att återuppbygga. Begreppet tillit är relativt outforskat men har stor betydelse hos både patient och sjuksköterska, det stärker relationen vilket gör att sjuksköterskan känner förtroende för patienten och patienten känner högre trygghet för sjuksköterskan. Resultatet visar ett samband mellan sjuksköterskans attityd och den upplevda tilliten hos patienten. Begreppet tillit finns ej utvecklad som oberoende omvårdnadsåtgärd i litteratur och därför kan utökad forskning vara relevant för att utveckla nursing och caring.
The concept of trust is something nurse and patient come into contact with on a daily basis, without thinking about it. The concept of trust is defined as human’s spontaneous manifestations of life. There is a lack of scientific knowledge within care science, nursing and caring. Increasing the knowledge about the concept of trust can contribute to the possibility of enhancing health in a caring environment. The aim of the study was to illuminate how the concept of trust is expressed within care science, nursing and caring. The study is built on a literature research were data collection was based on scientific articles. The result shows how trust is experienced within nursing and caring. The patient felt valued and confirmed by the nurse and strengthened by the nurse’s good and respectful attitude. A lack of trust gave a feeling of insecurity. The concept of trust is relatively unexplored but is of huge importance to the nurse and patient. Trust strengthens the nurse-patient relation which creates safety and confidence. The result shows the connection between the nurse’s attitudes and the trust experienced by the patient. Trust is not developed as a nursing-care intervention in literature and further research is relevant.

Summary Background: Caring forms the core of nursing and midwifery. Despite caring being an important emotional aspect of midwifery and nursing, there are general public complaints about uncaring behaviour in midwifery. Therefore, there is a need to explore caring from midwives’ point of view with the hope of identifying solutions and recommendations for midwifery practice. Furthermore, the study aimed to stimulate debate and discussion about the caring behaviour of midwives. Objective: To explore caring during clinical practice as perceived and experienced by midwives. Method: The study was contextual, exploratory and qualitative. The participants were midwives working in state and private hospitals in Tshwane, South Africa where BTech II and III midwifery learners were allocated for work integrated learning (WIL). Data collection was carried out through self-report using a questionnaire and focus group. Questionnaires were distributed to 40 midwives at private and state hospitals in Tshwane. This was followed by two focus group sessions to ensure that data is enriched. The hermeneutic interpretive approach was used to analyse data, and analysis continued until saturation. Results: Themes of caring and uncaring related to patient care and midwives emerged. The findings illustrated that the midwives had excellent theoretical knowledge of caring, but some of them did not display caring behaviour during clinical practice. Conclusion: Some of the midwives did not display caring behaviour. Implication for practice was provided based on the research findings. Recommendations included measures of improving caring behaviours during midwifery practice.

Throughout the life course, all family members can expect to find themselves in some type of caregiving role. Individuals may find themselves providing care for a child or for an older family member who needs assistance. With the increase in life expectancy and the shrinking family size, individuals may find themselves spending time providing care to older family members. Despite the fact that caregiving related issues are well researched in the field of gerontology, the developmental experiences of grandchildren caregivers and the meanings of their caregiving experiences have not been explored in previous empirical work. Influenced by symbolic interactionism theory and the life course and life-span perspectives, the research questions that guide this study are: What is the nature of caregiving from the perspectives of grandchildren in the grandparent-grandchild relationship? What meanings do grandchildren give to the caregiving role? A qualitative study was conducted to examine the experiences of adult grandchildren (21-29 years old) who were currently providing some type of care-related activity for at least one grandparent. Face-to-face semi-structured interviews, lasting 30-80 minutes, with 17 grandchildren caregivers were conducted one time to examine the participantsâ caregiving experience. All interviews were completed at a location of the participantsâ choice. Interviews were tape-recorded and tapes were transcribed verbatim to aid in data analysis. Grandchildren caregiversâ experiences illustrate variation with the reasons for providing care and the amount of time engaged in care related activities. Grandchildren were assisting with instrumental activities of daily living and activities of daily living. Reasons for providing care included grandparentsâ chronic illness or gradual aging, a crisis or event that left grandparents needing assistance, and because they had been providing care since they were young children. The amount of time grandchildren engaged in care related activities ranged from daily to several hours a week during summer and winter breaks. Grandchildren caregivers reported that family values, making grandparents happy, and preparing for the future were how they made sense of their role. Grandchildren experienced benefits and drawbacks from assisting grandparents and discussed how parents served as mediators and distracters to their caregiving role. Grandchildren caregivers exhibited the ability to adapt to caregiving situations and develop coping mechanisms that allowed them to be successful caregivers. Service professionals may want to include grandchildren caregivers in established support groups, caregiver programs, and enhance caregiver resources to support the generational needs of grandchildren caregivers in their 20s.

Due to the high incidence of ill health and disability amongst their contemporaries, older people are likely to be involved in informal caring relationships. Due to the limited nature of post-retirement incomes, such carers are also likely to be relatively poor. In spite of this, little attention has been given to the role of older carers or to the influence of material deprivation upon this role. Drawing on fieldwork interviews with a sample of older co-resident carers and the secondary analysis of official statistics, this research aims to redress this ‘ageless’ and ‘classless’ analysis of informal care. While the original intention of the research was to focus solely on the way in which material resources went on to affect caring in older age, subsequent analysis revealed a complex interaction between material circumstances, physical pathology, culture and attitude. The concept of resources was therefore broadened to incorporate these issues and the study’s materialist focus has been supplemented by a consideration of the age-based specificity in the way in which respondents manage and experience their financial situation and caring role. The thesis concludes with a consideration of the implications of these findings for future policy, literature and research. Thus, it is suggested that in view of the high degree of interdependence in caring relationships, intervention should focus less on the conflicting needs of care providers and care receivers and more on their commonality and their shared need for adequate and appropriate support. Similar synthesis is required in the exploration of poverty, older age and caring and also in the three generations of thought that have engaged in this exploration. In the light of this synthesis, it is argued that the ‘relative autonomy’ of the age dynamic should be recognised and responded to by researchers, practitioners and policy makers alike.

The aim of this study was to describe, and to further understand the lived experience of intensive care nurses, who care for patients for whom life sustaining treatment is being withdrawn. It is known that a large percentage of patient deaths that occur in the Intensive Care Unit (ICU) follow the withdrawal of life sustaining treatment. Nurses are the primary care givers of patients in the ICU and are directly involved in end-of-life care and the initiation of withdrawal of life sustaining treatment. Interpretive phenomenology served as the methodology by which the researcher explored and interpreted the lived experience of these ICU nurses. Unstructured face-to-face interviews were conducted with six intensive care nurses employed in the ICU of a large tertiary care hospital. The interviews were transcribed verbatim and analyzed using approaches outlined by Colaizzi (1978) and van Manen (1990). The essence of this experience was described by intensive care nurses as "trying to do the right thing". Three major themes emerged from the data: A Journey -- Creating Comfort Along the Way; Working in Professional Angst and Providing Memories. The findings have given a voice to intensive care nurses in their role of providing care to patients and families during this experience.

Caring, according to Jean Watson (1979), is the “core” and “essence” of nursing. The link between wisdom (knowledge) and caring can be achieved for the faculty member, according to Berkowitz (2011), if more attention is paid to the learning environment, role modeling, interactional practices, and professional development of the teaching staff. To do so, it will be critical for faculty to model caring as the core value and precept of nursing (Li et al., 2013) as well as create a caring environment (milieu) for students to replicate when they become professional nurses.

A gap was identified in the literature review regarding the experience and perceptions identified by senior associate degree nursing (ADN) students of faculty caring. Overall, there is little written about AD senior nursing students’ perceptions of faculty caring and the caring milieu in an ADN program. This research investigated students’ perceptions of the caring behaviors and actions of the faculty, students’ perceptions of the overall caring milieu of the program as well as the impact these behaviors have on the students’ caring abilities.

The findings in this study identified students’ perception of faculty caring and the impact on their own caring behaviors. The participants expressed both joy and alienation. They shared stories of positive role modeling behavior of faculty such as listening, respecting, and “being there” for the participant. The narratives also shared students’ perceptions that some faculty and staff nurses were uncaring, disrespectful, or not helpful. The implementation of the 4 Rs (Resources, Responsivity, Reachability, and Role modeling) for faculty and nurse administrators may help to change the caring perception of students and nurses.

Geller’s Actively Caring Survey (ACS) was theorized to measure person states deemed necessary to “Actively Care” or act altruistically toward others. Empirical research of the ACS has been limited, and this researcher sought to evaluate its reliability, validity, and factorial consistency. Undergraduate students (n = 1,095) completed the measure online. Hypotheses were partially supported. Unrotated primary component analysis found the ACS to be a unitary measure with 73.3% of the items loading onto the first factor. The ACS showed excellent internal consistency. Convergent and divergent validity with existing measures (i.e., the Big 5 Personality, Marlowe-Crowne Social Desirability, Cognitive Failures Questionnaire, Barratt Impulsiveness, and Eysenck Personality Questionnaire Venturesomeness scales) was found in 88.9% of the predicted relationships; the ACS was negatively correlated with social desirability. An abbreviated ACS revision produced similar findings. Future studies should evaluate the measure in nonstudent populations, use clinical and industrial settings, and explore predictive validity.

Abstract (sommario):

Background: When a person suddenly pass away, the relatives often arrive to the Emergency Department under chaotic circumstances. In this critical situation the nurse has an important role for the care. Many nurses lack of knowledge about how to take care of the bereaved relatives. Aim: The aim of the literature study was to describe aspects of caring for suddenly bereaved relatives. Method: The data for the study were scientific articles found in the databases PubMed and Cinahl and from a manual search. The study included ten articles which were analysed using a content analysis. The content was organized in meaning units, subcategories and main categories. Result: The study resulted in three main categories: Factors that affect nurses care, Relatives needs and Conditions in the working area. Nurses with high competence and a high level of self knowledge seemed to have the best opportunities to take care of bereaved relatives. The relatives valued emotional support, confirmation, justification of their grief reactions and honest information. Lack of facilities such as private rooms, stressful situations and unclearly defined responsibility areas were factors that affected nurses care about bereaved relatives .Conclusion: Insufficient knowledge concerning the care of bereaved relatives is a fact among nurses today. Further science focused on relatives experiences are necessary to give nurses and other health care personnel increased knowledge. It is our hope that this study may inspire such science.

Research on carers has shown that, whilst there are often significant differences between men and women in terms of activity and intensity within filial and sibling caring relationships, the respective contributions ofmale and female spousal carers are very similar. Nevertheless, despite feminist considerations of the informal caring that men do, the qualitative experience ofmale carers has not received the same in-depth examination as that offemale carers. This thesis explores the way in which thirty men, identified as carers for their spouse, construct their understanding ofcaring and masculinity. The influence ofageing and retirement in this process are also considered. Fifty-nine qualitative interviews were gathered and analysed using feminist approaches -premised on the co-procluction of knowledge. The theoretical framework of the study prioritised the agency ofindividual male carers, albeit constrained by material realities of an ageing body, and retirement. The thesis offers a new framework for understanding the way men undertake informal caring activities for their spouse and the emotional as well as practical significance ofinformal caring for them. An important aspect ofthe framework is that it describes how men, who begin to care for their spouse informally, often do so without an awareness ofneed and the practical caring skills they require to care holistically: Older men, however, particularly when they have experienced their own health problems, show greater responsiveness to their wife, enabling a more holistic caring relationship to develop. The framework also shows how this transition is influenced by male carers' interaction with health care professionals, support groups, family, and friends. Finally, the study points to potential topics for future research, particularly the relationship between masculinity, ageing and life course. It also confirms the conclusions ofrecent theoretical research on men and masculinity that suggests the importance oflooking at men's everyday practices, and not just focussing on men's hegemonic behaviours, because after all, men do not always behave in hegemonic ways, but men are always gendered.

Using a Wittgensteinian approach to understanding, this thesis extends and challenges recent feminist discussions of the ethic of care as a gender-sensitive corrective to traditional moral theory. It elaborates a more complex understanding of the diversity and ambiguity of the ethical possibilities of caring than has been presented in earlier analyses. A brief introduction to the contemporary debate is followed by accounts of six different examples of caring practices, viz: caring attention, taking care of oneself, mothering, friendship, nursing and citizenship. The aim of this survey is to show that caring constitutes an intricate labyrinth of ethical possibilities, the understanding of which involves approaching it from numerous directions. Through concern for the similarities and differences between these examples, their insights and their oversights, the thesis displays the limitations of theories which presume a unified, non-contexted ethic of care. At the same time the detailed descriptions of caring practices affirm the ethical significance of a range of activities that are frequently overlooked in conventional accounts of ethics.

In this qualitative descriptive study seven homeless young adults in a northern town in Alaska were interviewed for the purpose of finding out how they cared for their health. The common barriers experienced by the participants in caring for their health were identified as hunger, absence of shelter, inattention to health, and the act of asking for help. Common facilitators described by the participants were access to food, resources, support system, and goals. Based on the findings nurses understanding the effects of hunger, lack of sleep, lack of support, and the stress of being homeless will be better able to assess the needs of homeless young adults. Findings support increased knowledge of resources in the community of practice and the culture associated with homelessness to facilitate the young homeless adults' health care and support them in their endeavors to secure a better future. Building a relationship with resource agencies within the nurse's community will enable the nurse to seek the assistance homeless young adults require for meeting their basic needs.

This thesis will consider UK labour law’s role in promoting fairness for carers. Building upon Fineman’s work, I will argue that caring relationships are of vital importance to society and should be supported by the state. The principle of justice as fairness, substantiated by the capabilities approach, will underpin this argument. I will focus upon modifying the workplace through care centric labour laws to achieve fairness for carers. Care centric legislation, developed by Busby, focuses upon promoting carers’ rights to work, rather than workers’ rights to care. Much of the analysis will focus upon reconciliation legislation, which aims to support people providing care within the paid workplace. This is because it has been the main way successive UK governments have aimed to help people reconcile these competing commitments. Although this body of legislation has gone some way towards achieving this, I will show that it could have done more. To make labour law care centric, something more radical is required. In this regard, I will analyse a right to care. To conclude, I will highlight the need for more empirical work in this context to further understand how fairness for all carers could be achieved.

Compassion fatigue is of significant concern in critical care settings. Nurses experiencing compassion fatigue suffer physical, emotional, and behavioral symptoms that impact them professionally and personally. When nurses suffer from compassion fatigue, patient care is negatively impacted as nurses lose their sense of caring, compassion and ability to relate to patients or meet their needs resulting in reduced patient outcomes and satisfaction scores. Compassion fatigue prevention programs must be implemented to educate nurses and nurse leaders regarding risk factors, symptoms, and interventions to prevent and treat compassion fatigue.

Development of a compassion fatigue training (CFT) module occurred to address these concerns at a local healthcare organization. Great care was taken to develop an effective training module. Partnerships were formed. The education department director, staff, and unit managers were valuable resources, assisting with the development and implementation of the CFT module. The plan consisted of:

• Administration of a Professional Quality of Life (ProQOL) Scale to critical care and highrisk area nurses • Data analysis to determine specific education needs and address deficits • Creation of a CFT Module based on the ProQOL Scale results • Assignment of CFT and deployment of the module in the HealthStream system • Nurse completion of CFT • Post training evaluation of learning • Analysis of evaluation data and summary of learning

After completion of the CFT module, nurses concluded with a post quiz. Passing scores of 80% were required for successful accomplishment of CFT. All participants achieved the required score and demonstrated meaningful understanding of compassion fatigue, risk factors and interventions through successful completion of a post training quiz. A learning evaluation was available to the nurses to appraise effectiveness of the training and achievement of module objectives. Nurses reported knowledge acquisition regarding compassion fatigue, symptom recognition, coping, and resources. Nurses were able to enumerate coping mechanisms and resources for assistance. They responded that CFT provided valuable information and reported they would use the knowledge and skills gained to secure work-life balance. CFT promoted improvements in career satisfaction, burnout, and secondary traumatic stress levels. CFT must become part of an ongoing measure to ensure staff are receiving instruction necessary to manage the effects of compassion fatigue.

The current review aims to unravel what is known regarding the neural substrates of empathicdistress and compassion fatigue. Empathic distress is a self-oriented feeling of anxietyexperienced in response to the suffering of another. It has been related to, and theorized to bea precursor of, compassion fatigue. This is a form of caregiver burnout received by secondaryexposure to trauma. In the current thesis, Scopus, Web of Science and PsycINFO weresearched identifying 301 articles that were subsequently screened. In the end, five studieswere included that measured either empathic distress or compassion fatigue in relation tobrain structure or function. Findings are largely inconsistent but areas involved in theory ofmind and that are important for the self-other distinction are discussed. A need for moreresearch is identified, together with a desire for conceptual clarification between compassionfatigue and burnout.

The concept of caring is reported to be an important factor in the teacher-student relationship. However, this concept has not been widely explored in the context of the teacher-student relationship in physical education. The purpose of this study was to gain insights into students' perceptions of physical education teachers' caring. The researcher sought to describe the perceptions students had in regard to caring behaviors exhibited by their physical education teachers. Twelve eighth-grade students from a local middle school in Southwest Virginia participated in this study. The research questions were investigated using a qualitative research design that included semi-formal interviews and field observations. The results indicated that multiple opportunities existed for the physical education teachers to demonstrate caring behaviors toward their students. The students described their perceptions of physical education teachers' caring along dimensions of content and pedagogy and interpersonal relationships. The researcher determined that factors existed that facilitated and presented barriers to the caring process. Facilitators of caring in physical education included the nature of the class, flexibility in teacher expectations, and class activities. Barriers to caring were revealed as length of time with the teacher, student personalities, role of the teacher/coach, and class size. Furthermore, physical education teachers' caring was determined to be a positive factor in students' attitudes toward physical education and their participation in physical education class. The results did not indicate a clear pattern of responses based on the skill level of each participant. Directions for future research on the concept of caring as it relates to physical education are discussed.
