A sedação para a realização de cateterismo cardíaco, apesar de pouco estudada, tem sido alvo de preocupação de alguns anestesiologistas. Os benzodiazepínicos, os agonistas alfa-2 adrenérgicos e os opióides são comumente utilizados para este fim, entretanto, cada um destes medicamentos possui vantagens e desvantagens. O objetivo deste trabalho é avaliar a eficácia do sufentanil e da clonidina como medicação sedativa em pacientes submetidos a cateterismo cardíaco, comparando o impacto dos mesmos sobre os parâmetros hemodinâmicos e respiratórios apresentados, observando a presença de efeitos colaterais, além da satisfação do paciente e do hemodinamicista durante o exame. Trata-se de um ensaio clínico prospectivo, duplo-cego, randomizado e controlado, que envolveu sessenta pacientes divididos igualmente em dois grupos: GS e GC que receberam respectivamente, 0,1 mcg/kg de sufentanil e 0,5 mcg/kg de clonidina. Estes fármacos foram administrados antes da realização do cateterismo cardíaco. O escore de sedação segundo a escala de Ramsay, a necessidade de utilização de midazolam, os efeitos colaterais, a PAS, a PAD, a FC, a FR e a SpO2 foram registrados a cada cinco minutos, sendo os dados analisados em 06 diferentes momentos. Os grupos foram homogêneos em relação aos dados demográficos e avaliação clínica inicial. O comportamento da PAS, da PAD, da FC e da FR foi semelhante nos dois grupos, entretanto os pacientes do GS apresentaram menor escore de sedação segundo a escala de Ramsay no momento 2 e SpO2 menor que o GC no momento 6. Os pacientes do GS apresentaram maior incidência de NVPO que os pacientes do GC. A satisfação dos pacientes foi maior no GC. Os hemodinamicistas mostraram-se igualmente satisfeitos nos dois grupos. O sufentanil e a clonidina se mostraram efetivos como fármacos sedativos em pacientes submetidos a cateterismo cardíaco.

Not only benzodiazepinic medicine, but also alpha- 2 adrenergic agonist and opioids are commonly used drugs to this end. Each one of these medicaments presents a series of advantages as well as disadvantages. The objective of this paper is to evaluate the effectiveness of sufentanil and clonidine as a sedative medicament to patients submitted to cardiac catheterization, comparing their impact over the hemodynamic and respiratory parameters witnessed, observing the occurrence of side effects, besides patient’s and hemodynamicist’s satisfaction during the examination. This consists of a prospective, double-blinded, randomized and controlled clinical essay, which involved sixty patients equally divided in two different groups: GS and GC, who, respectively, received 0,1mcg/kg of sufentanil and 0,5 mcg/kg of clonidine. Both administered before cardiac catheterization. The sedation score, according to Ramsay’s scale, the necessity of utilizing midazolam, side effects, SAP, DAP, CF and SpO2 were registered every five minutes and data analyzed in six different moments. The studied groups were homogeneous regarding demographic data and initial clinical evaluation. The behavior of SAP, DAP, CF and RF was similar in both groups. However, a lower sedation score regarding Ramsay’s scale was observed in GS patients at moment 2 and at moment 6 SpO2 was found to be lower than GC. All GS patients seemed to have had higher incidence of PONV compared to GC patients. Considering patients’ satisfaction, it was higher in GC. Hemodynamicists seemed equally satisfied in both groups. Sufentanil and clonidine seemed to have been more effective as sedative medicaments in patients submitted to cardiac catheterization.

Testo completoNeste estudo retrospetivo pretendeu-se avaliar os resultados a curto (24 horas) e a longo prazo (quatro meses) da oclusão da persistência do canal arterial (PCA) por cateterismo cardíaco, recorrendo a dispositivos cardíacos como, a espiral de embolização (n=7), o Amplatzer® Vascular Plug (n=2) e o Amplatz® Canine Duct Occluder (ACDO; n=16) em 25 cães. Outro objetivo foi comparar as medidas do diâmetro mínimo do canal obtidas em ecocardiografia transesofágica bidimensional (ETE 2D) com as obtidas em ecocardiografia transtorácica bidimensional (ETT 2D), em ETT Doppler de cor e em ETE Doppler de cor. Confirmou-se a oclusão completa em 20 pacientes e a presença de um fluxo residual mínimo em cinco pacientes através da angiografia realizada durante o procedimento da oclusão. A curto prazo, através da ETT Doppler de cor foi possível observar a oclusão completa em todos os pacientes. O procedimento foi bem sucedido hemodinamicamente, evidenciando-se uma redução significativa do diâmetro diastólico final do ventrículo esquerdo indexado ao peso corporal (DDVE/Peso1/3; P<0,01), da fração de encurtamento (P<0,01) e do rácio diâmetro do átrio esquerdo/diâmetro da aorta (AE:Ao; P<0,001) nas primeiras 24 horas após o procedimento de oclusão. Quatro meses após o procedimento, o DDVE/Peso1/3 sofreu uma significativa redução (P=0,03) e o AE:Ao manteve-se constante. As complicações secundárias incluíram embolização pulmonar de um ACDO e rotação tardia de um Amplatzer® Vascular Plug resultando na presença de fluxo através do canal arterial. O diâmetro mínimo do canal obtido por ETT 2D foi o que mais se aproximou do obtido por ETE 2D. A oclusão da PCA utilizando um ACDO em cães com mais de 3 kg e uma espiral de embolização transarterial em cães com menos de 3 kg teve um elevado índice de oclusão completa e imediata. O cateterismo cardíaco com este tipo de dispositivos provou ser um método de tratamento seguro e eficaz em canídeos com PCA.
ABSTRACT - Retrospective study of patent ductus arteriosus correction by cardiac catheterization in dogs - This retrospective study aimed to evaluate the results in the short (24 hours) and long term (four months) patent ductus arteriosus (PDA) occlusion by cardiac catheterization, using cardiac devices as embolization coil (n=7), the Amplatzer® Vascular Plug (n=2) and Amplatz® Canine Duct Occluder (ACDO; n=16) in 25 dogs. Another objective was to compare the measures of the minimal ductal diameter obtained from two-dimensional transesophageal echocardiography (2D TEE) with those obtained in two-dimensional transthoracic echocardiography (2D TTE), color Doppler TTE and color Doppler TEE. Angiography was performed during the occlusion procedure and it confirmed complete occlusion in 20 dogs and a trivial residual flow in five dogs. In the short term, color Doppler TTE revealed that complete ductal closure was achieved in all dogs. The procedure was hemodynamically successful, as evidenced, by a reduction in indexed left ventricular internal diameter in diastole (LVIDd; P<0.01), fractional shortening (P<0.01) and left atrial to aortic ratio (LA:Ao; P<0.001) within 24 hours after occlusion procedure. Four months after procedure, indexed LVIDd was significantly reduced (P=0.03) and LA:Ao remained constant. Secondary complications included pulmonary arterial embolization of an ACDO and a late rotation of an Amplatzer® Vascular Plug resulting in an increased flow through the PDA. Minimal ductal diameter obtained with 2D TTE was the closest to the 2D TEE. PDA occlusion using an ACDO for dogs with more than 3 kg and a transarterial coil embolization for dogs with less than 3 kg had a high rate of immediate complete occlusion. Transcatheter occlusion using those devices proved to be a safe and effective therapeutic method for PDA in dogs.

Testo completoIntroduction: Lymphangioleiomyomatosis (LAM) is associated with pulmonary hypertension (PH) and is included in group 5 of the current classification (unknown multifactorial mechanisms). However, data regarding the occurrence of PH in LAM are scarce. The objectives of the study were to evaluate the prevalence and characteristics of PH in patients with LAM at different stages of evolution, as well as to compare the clinical and functional characteristics of the 6-minute walk test (6MWT) and the quality of life of patients with and without PH. Methodology: One hundred and five patients with LAM underwent echocardiogram, pulmonary function test (PFT) and 6MWT. Patients with suspected PH on the echocardiogram, defined as the presence of estimated systolic pulmonary arterial pressure (PsAP) above 35 mmHg, or PFT showing carbon monoxide diffusion (DLco) below 40% of the predicted value, were submitted to right cardiac catheterization to confirm the diagnosis of PH. Results: Eight patients (7.6%) had PH confirmed in right cardiac catheterization, six patients (5.7%) had a pre-capillary pattern and two patients (1.9%) had a post capillary pattern. Only one patient (1%) presented mean pulmonary artery pressure (PAPm) above 35 mmHg. Patients with PH had lower FEV1 and DLco in PFP and greater oxygen desaturation and dyspnea intensity during the 6MWT compared to those without PH. In 63% of patients with confirmed PH, right heart catheterization was performed because of the DLco result. Conclusions: The prevalence of PH is low in patients with LAM. Pulmonary hypertension is commonly mild and is significantly associated with pulmonary parenchymal involvement. The measure DLco has improved the identification of PH in patients with LAM

Resumo: O cateterismo cardíaco é um teste diagnóstico invasivo utilizado para detecção de alterações cardíacas dos pacientes com queixas cardiovasculares como anginas e infartos previamente detectados. Este ocorre por meio de um cateter arterial percutâneo, podendo ser escolhida a via radial ou femoral. Objetivo Obter por meio da revisão integrativa e validação de conteúdo, os principais cuidados para a retirada do introdutor arterial pelo enfermeiro com segurança. Metodologia. Estudo de abordagem quantitativa realizado em duas etapas: primeiro a realização de uma revisão integrativa para elaboração de dois Procedimentos Operacionais Padrão (POP's) preliminar, utilizando as Bases de Dados BVS (Biblioteca Virtual da Saúde-BVS), Scopus, Embase, Web of Science e Pubmed e numa segunda etapa a submissão para apreciação de 8 peritos para obter um parecer sobre o conteúdo apresentado, utilizando a Técnica Delphi. E a partir destes, em posse das observações feitas pelos peritos, obter os POP's definitivos. Resultados e Discussão. Estudos mostram que o enfermeiro tem papel fundamental na equipe multidisciplinar, garantindo a segurança na técnica de retirada do introdutor arterial com segurança garantida ao paciente. Importante, porém, protocolizar e dispor a informação para a equipe de enfermagem contribuindo para autonomia do enfermeiro e segurança do paciente. Produtos elaborados. Abstract: The cardiac catheterization is an invasive diagnostic test used for the detection of changes of cardiac patients with cardiovascular complaints as angina and heart attacks previously detected. This occurs through a percutaneous arterial catheter, can be chosen via the femoral or radial. Objective. Obtain through integrative review and validation of content, the main introducer removal care nurse blood safely. Methodology. Quantitative approach study carried out in two steps: first the realization of a integrative review for development of two standard operating procedures (POP's), using the VHL (Virtual Health Library-BVS), Scopus, Embase, Web of Science and Pubmed and in a second step the submission for consideration of 8 experts for an opinion on the content presented, using the Delphi Technique. And from these, in possession of the observations made by the experts, get the POP's definitive. Results and Discussion. Studies show that the nurse has a fundamental role in the multidisciplinary team, ensuring the technical security of withdrawal of arterial introducer with guaranteed safety to the patient. Importantly, however, Docketing and disposal information for nursing staff contributing to autonomy of nurse and patient safety. Products produced. Standard operating procedure (POP) on withdrawal of Arterial Introducer mechanically and manually by the nurse and an Ebook that will be available in the Virtual Library of the Hospital das Clinicas of Botucatu. Conclusion. It is the responsibility of the nurse to prevent vascular complications such as hematoma and bleeding sites in withdrawal of blood via femoral introducer, mechanically or manually. The Protocol is a technical resource for the nurse to develop their practice with higher quality and patient safety.

Testo completoCardiac catheterization has for a long time been a valuable way to evaluate the hemodynamics of a patient. One of the benefits is that the entire blood pressure waveform can be recorded and visualized to the cardiologist. These measurements are however disturbed by different phenomenon, such as respiration and the dynamics of the fluid filled catheter, which introduces artifacts in the blood pressure waveform. If these disturbances could be removed, the measurement would be more accurate. This report focuses on the effects of respiratory artifacts in blood pressure signals during cardiac catheterization.
Four methods, a standard bandpass filter, two adaptive filters and one wavelet based method are considered. The difference between respiratory artifacts in systolic and diastolic pressure is studied and dealt with during compensation. All investigated methods are implemented in Matlab and validated against blood pressure signals from catheterized patients.
The results are algorithms that try to correct for respiratory artifacts. The rate of success is hard to determine since only a few measured blood pressure signals have been available and since the size and appearance of the actual artifacts are unknown.

Testo completoThe value of non-invasive methods in adding useful information to establish the prognosis in survivors of myocardial infarction is still under debate. Therefore we undertook the present investigation to analyze if non-invasive tests and cardiac catheterization added independent prognostic information to simple clinical variables. In 288 survivors of a myocardial infarction (MI) a cardiac catheterization was performed the 10th day after admission. An M-mode and bidimensional echocardiogram was obtained in 231 patients, left ventricular technetium ventriculography in 120 patients and Holter recording in 204 patients. One month after discharge, patients underwent a symptom limited exercise test with ergometric bicycle and an exercise and rest thallium gammagraphy. The first end-point was cardiac death. The variables that showed significant differences in the univariate analysis were entered in successive stepwise logistic regression analysis including the clinical variables and the results of one different test each time. Among the clinical variables, the presence of diabetes mellitus, the existence of bifascicular bundle branch block or previous myocardial infarction showed independent prognostic value for death. Isotopic ejection fraction, ventricular arrhythmias in the Holter, the systolic pressure during exercise test, the angiographic ejection fraction and the number of diseased vessels also showed independent prognostic value. When the clinical variables and all the non-invasive tests were analyzed together, the presence of diabetes mellitus and the ejection fraction were the only variables that showed independent prognostic value. When the variables "number of diseased vessels" was included in the analysis, it also showed independent value. The second end-point was the presence of severe non-fatal ischemia during the follow-up. Among the clinical variables, the early post-myocardial infarction angina and the existence of a previous myocardial infarction were the only ones that showed independent predictive value of severe ischemia. From the tests performed, on1y the number of diseased vessels added independent prognostic value to the equation obtained from the clinical variables. In conclusion the practice of a technetium ventriculography, exercise test or Holter can add some prognostic information for cardiac death to the one obtained from the clinical variables, although the practice of several tests did not improve the prediction. On the other hand non-invasive tests do not seem useful in predicting new several non-fatal ischemic events.

Trata-se de um estudo de abordagem qualitativa, do tipo descritivo, comparativo, baseado no referencial da hermenêutica dialética, cujo objetivo foi analisar as repercussões imediatas da ação educativa do enfermeiro, através da sala de espera na hemodinâmica, junto aos pacientes e acompanhantes antes de um procedimento de intervenção cardiovascular. A educação é um instrumento que contribui para a formação das pessoas e do mundo. A educação em saúde é entendida como uma prática que trabalha junto ao indivíduo estratégias de mudança em seu benefício, para seu próprio bem-estar, desenvolvendo um pensamento reflexivo e posteriormente crítico sobre os próprios atos e cultura, possibilitando melhoras no estado de saúde. O cateterismo cardíaco é um exame de diagnóstico por imagem, amplamente utilizado na investigação das coronariopatias e valvulopatias cardíacas estudadas no setor de hemodinâmica. A enfermagem que atua neste serviço tem grande importância no que tange às orientações em saúde com vistas ao bem-estar dos pacientes acometidos por doenças cardiovasculares e que são encaminhados para diagnóstico ou tratamento. Realizou-se entrevista semiestruturada com cinco pacientes e cinco acompanhantes antes da e após a ação educativa de enfermagem. Os dados encontrados foram submetidos à análise proposta por Minayo composta pelas etapas de ordenação, classificação e análise final. Os resultados possibilitaram a construção das seguintes categorias: desvendando o desconhecido; em busca de um protagonista; orientações de enfermagem como forma de esclarecimento e alívio da ansiedade. Através destas, identificamos a existência de lacunas nas informações sobre o procedimento; ausência de um mediador do conhecimento técnico-científico que auxilie na construção de saberes e troca de experiências; um cenário onde pacientes e acompanhantes se sentem ansiosos e temerosos pela espera por algo que não conhecem, mas também demonstraram a importância das orientações de enfermagem para melhora da ansiedade e busca de outros saberes. A prática educativa implementada demonstrou ser de grande relevância para atuação do enfermeiro no serviço de hemodinâmica, corroborando para melhor compreensão do exame, maior qualidade de informação e controle do nível de ansiedade.
It is a study of qualitative approach, descriptive, comparative, based on the dialectic hermeneutics reference whose aim was to analyze the immediate repercussions of educational action of the nurses, through the waiting room in hemodynamics, with patients and companions before of an interventional cardiovascular procedure. Education is a tool that contributes to the formation of people and of the world. Health education is understood as an educational practice that works with the individual to critical and liberating vision of living conditions, aiming to change strategies for their benefit, for your own well-being, developing a critical reflective thinking and later on the own actions, culture, enabling improvements in health status. Cardiac catheterization is a test of diagnostic imaging, high resolution, widely used in the investigation of coronary artery disease and cardiac valve disorders studied in the hemodynamic sector. The nursing acting in this service is of great importance when it comes to health orientation with a view to the welfare of patients suffering from cardiovascular disease and who are referred for diagnosis or treatment. Was held semi-structured interview with five patients and five companions before and after the nursing education action. Data were subjected to analysis proposed by Minayo comprised by ordination of data, data classification and final analysis. The results allowed the construction of the following categories: uncovering the unknown; looking for a protagonist; nursing's orientation as a way to enlightenment and relief from anxiety. Through these, we identified the gaps in information about the procedure; absence of a mediator of technical and scientific knowledge to assist in the construction of knowledge and exchange of experiences; a scenario where patients and companions feel anxious and fearful waiting for they do not know, but also the importance of nursing guidelines for improvement in anxiety and search for other knowledge. The implemented educational practice proved to be of great relevance for nursing work in the hemodynamics service corroborating to better understanding the exam, better information and relief of the level of anxiety.

Introdução: A cardiologia intervencionista requer necessariamente um acesso vascular invasivo, sendo esta via de acesso uma escolha do médico operador. Nesta escolha interfere o tipo de procedimento, impacto ao paciente, risco ao profissional e experiência técnica. O acesso via artéria femoral é o mais difundido globalmente e a técnica de acesso pela artéria radial tem sido progressivamente adotada devido ao maior conforto para o paciente, menores taxas de complicações e em alguns cenários, de mortalidade. Um dos focos de interesse crescente é a exposição à radiação ionizante e medidas para minimizar o risco ao paciente e ao profissional. Escassa é a literatura disponível na área e conflitantes são seus resultados. Objetivo: Avaliar os níveis de radiação recebidos pelo médico operador de acordo com as diferentes técnicas de acesso, bem como mapear áreas de escape de radiação. Após essa etapa, o estudo visou desenvolver e validar dispositivo de radioproteção dedicado à técnica radial (TRIPTable) voltado à redução da radiação ao médico operador, comparando o seu impacto à técnica femoral e radial padrão no cenário de pacientes portadores de síndrome coronária aguda randomicamente alocados para cateterismo com intenção de tratamento percutâneo. Métodos: A fase inicial pré-clínica do estudo foi constituída de avaliação em ambiente controlado dos níveis de radiação em pontos pré-especificados de um operador simulado com phantom de água. A fase clínica do estudo foi um ensaio clínico prospectivo, randomizado, unicêntrico, comparativo entre três grupos denominados técnica femoral, técnica radial e técnica radial com dispositivo radioprotetor TRIPTable, recrutados no Hospital das Clínicas de Marília (Marília/SP) e randomizados na proporção 1:1:1 (108 pacientes - 36 por grupo) avaliando a radiação recebida pelo operador em três diferentes pontos (gônadas, tireoide e olhos). Os resultados foram comparados com o grupo controle de 108 pacientes provenientes de coorte randomizada externa entre técnica radial e femoral 1:1, utilizando critérios similares, porém com operadores cegos aos objetivos de avaliação de radiação visando detecção de vieses de técnica (efeito Hawthorne) e validação de resultados. Resultados: Não houve diferença entre o estudo e a coorte externa ou entre os grupos do estudo quanto a características clínicas, desfechos dos procedimentos ou parâmetros de exposição radiológica ao paciente. Os resultados indicam maior radiação recebida pelo operador com a técnica radial (12,5 mSv), seguida da femoral (10,1 mSv) e TRIPTable (6,8 mSv). Em relação aos locais de exposição, o território de gônadas teve maior sensibilidade radiológica com uso da técnica radial (p=0,001). Com uso da técnica femoral não houve diferença entre os três territórios (p=0,398), porém na análise ad hoc o território de gônadas foi mais sensível quando comparado à exposição aos olhos (p=0,016) e limítrofe em comparação à tireoide (p=0,056). No grupo do dispositivo TRIPTable não houve diferença significativa entre qualquer um dos territórios analisados (p=0,180). Conclusões: O estudo indica equivalência entre os resultados do procedimento entre os grupos e quanto à exposição radiológica ao paciente. Contudo, demonstrou maior impacto radiológico para o operador que utilizou a técnica radial, em comparação à técnica femoral e à técnica radial com uso do dispositivo TRIPTable. O uso do dispositivo reduziu o impacto radiológico comparado ainda à técnica femoral. Tais diferenças derivaram primordialmente da variação em território de gônadas. Os achados demonstram um campo de exposição radiológica heterogêneo ao corpo do operador, benefício do dispositivo testado e oportunidade de novas formas de desenvolvimento de medidas de radioproteção. Background: Interventional cardiology requires an invasive vascular access, a choice of medical operator. This choice is affected by type of procedure, patient impact, professional risk and technical experience. Interventional procedures via radial technique have progressively increased due to improved patient comfort, lower complication rates, and reduced mortality in some scenarios. One area of interest is radiation exposure and ways to minimizing it. Most studies focusing on patient radiation risk demonstrated conflicting results, and there is no consensus for increased exposure with any technique. Objective: The aim of this study was to evaluate radiological exposure under controlled radial and femoral access simulation tests, mapping radiation paths. After this stage, the study developed a radiological protection device for the transradial technique (TRIPTable), comparing its impact as compared to standard femoral and radial techniques in the setting of patients with acute coronary syndrome randomly assigned for catheterization with intent to percutaneous treatment. Methods: Radiation exposure was simulated under controlled conditions for femoral and radial techniques using a pressurized ionization chamber and water phantom. Different measurement points were defined according to standard positions to simulate radiation received by the operator in the gonads, thyroid, and eyes at different angles during real procedures. The clinical phase of the study is a prospective clinical trial, single-center, randomized, and comparing three groups (femoral technique, radial technique, and radial technique with radioprotective device), admitted in the Emergency Department of Hospital das Clinicas de Marilia (Marilia / SP – Brazil). Patients were randomized in a 1:1:1 proportion (108 patients - 36 per group) evaluating the radiation received by the operator measured by dosimetry at three different points (gonads, thyroid and eyes). As it is impossible to blind operators to study techniques, the results were compared to an external control cohort of patients including 108 individuals with similar inclusion and exclusion criteria, randomized for radial and femoral technique, but with blinded operators to radiation objectives, aiming technical bias detection (Hawthorne effect) and validation of results. Results: There was no difference between groups regarding clinical characteristics, procedures or patient radiation exposure outcomes. The results indicate higher radiation received by the operator with the radial technique (12.5 mSv) as compared to femoral technique (10.1 mSv) and TRIPTable technique (6.8 mSv). Regarding exposure locations, the gonad region had a higher radiation sensitivity with radial technique (p=0.001). With femoral technique there was no difference between the three territories (p=0.398) but the ad hoc analysis indicated higher radiosensibility in gonads when compared to eyes exposure (p=0.016) and borderline compared to thyroid exposure (p=0.056). In the TRIPTable device group there was no significant difference between any of the territories analyzed (p=0.180). Conclusions: The study indicates no difference of radiation exposure to the patients between the groups. However, there was a greater radiologic impact to the operator who used the radial technique, compared to the femoral technique and TRIPTable device technique. The device reduced the radiological impact even compared to the femoral technique. Such differences derived primarily from variation in gonads exposure. The findings demonstrate a heterogeneous radiation exposure to the operator body, device benefit and an opportunity to develop new ways to improve radiation protection. A expressividade desses valores tem elevado os investimentos das técnicas e tecnologias utilizadas na elucidação diagnóstica e terapêuticas eficazes com intenção do controle dessas doenças. Esses avanços contribuem para o aprimoramento de estudos realizados em laboratórios de hemodinâmica como o cateterismo cardíaco. Esse exame, considerado padrão ouro no diagnóstico das obstruções coronárias, tem cerca de 80% dos procedimentos ambulatoriais, com a vantagem da redução do tempo de permanência no hospital e a desvantagem de diminuir o tempo de contato com a equipe, contribuindo para falhas na comunicação e interferindo na percepção dos profissionais no que se refere as necessidades e sentimentos do usuário em relação ao procedimento. Diante do exposto, este estudo tem por objetivo compreender o significado e descrever as informações recebidas sobre o cateterismo cardíaco aos usuários de um serviço de hemodinâmica, com vistas a elaborar orientações de enfermagem no pré-procedimento. É um estudo descritivo com abordagem qualitativa, que foi realizado no setor de hemodinâmica, de um Hospital de Ensino do Interior do Estado de São Paulo. Participaram do estudo 31 pacientes adultos que, pela primeira vez, estavam em procedimento de cateterismo cardíaco eletivo os quais aceitaram participar, assinando o Termo de Consentimento Livre e Esclarecido. Os dados foram coletados por meio de entrevista semiestruturada, realizada pela pesquisadora e a análise dos dados foi realizada pelo método de Análise de Conteúdo. Dos discursos dos entrevistados emergiram as seguintes categorias: os usuários conseguem relatar os sintomas e a trajetória que os levaram à realização do cateterismo cardíaco; as informações para o procedimento de cateterismo cardíaco são fornecidas na forma escrita, porém parcialmente assimiladas pelos usuários; os usuários desconhecem o procedimento de cateterismo cardíaco; a realização do cateterismo cardíaco mobiliza sentimentos de ansiedade e os usuários têm esperança de que o cateterismo cardíaco promova mudanças que melhorem a qualidade de vida. Os resultados do estudo permitem concluir que os usuários demonstram conhecimento insatisfatório acerca do cateterismo cardíaco e que as informações recebidas são parcialmente assimiladas, evidenciando a importância da equipe investir em capacitação comunicativa e no planejamento de orientações que fortaleçam os indivíduos na promoção da saúde, esperança e autonomia, além de estimular o reconhecimento das ações de Enfermagem.
The World Health Organization ranked cardiovascular diseases as the leading cause of death worldwide, being responsible for high rates of hospitalization and high hospital costs. These remarkable values have led to high investments in techniques and technology used in laboratory diagnosis and effective treatment in order to control these diseases. These advances contribute to the improvement of studies in hemodynamic laboratories, such as cardiac catheterization. This exam, considered gold standard in the diagnosis of coronary obstructions, amounts to about 80 % of outpatient procedures, with the advantage of reducing the patient's stay in hospital and the disadvantage of reducing the contact time with the team, contributing to failures in communication, and thus interfering with the staff’s perception of the user’s needs and feelings about the procedure. Given the above, this study aims to understand the meaning and describe the information received by the users of a hemodynamic service about the cardiac catheterization in order to prepare pre-procedure nursing guidelines. It is a descriptive qualitative study, which was conducted in the hemodynamic sector of a school hospital in São Paulo State. The study included 31 adult first-time patients who were under elective cardiac catheterization procedure and agreed to participate by signing the Instrument of Consent. Data were collected through a semi-structured interview, conducted by the researcher and data analysis was performed by the content analysis method. From the interviews the following categories emerged: users can report symptoms and the path that led to cardiac catheterization; the information for the cardiac catheterization procedure is provided in written form, but partially assimilated by users; users are unaware of the cardiac catheterization procedure; the cardiac catheterization mobilizes feelings of anxiety and users hope that the cardiac catheterization may promote changes which will improve their quality of life. The study results support the conclusion that the patients have poor knowledge about cardiac catheterization and the received information is partially assimilated, showing the importance for the staff to invest in communication training and planning guidelines which can reinforce individuals in health promotion, hope and autonomy in addition to stimulate the recognition of Nursing actions.

Testo completoFavorable evidence increase the use of the transradial technique in recent years. Original studies that address this approach suggest low risk of local complications, greater comfort for patients, the possibility of early mobilization and ambulation after the procedure, a shorter hospital stay and reduced hospital costs. Among the possible complications, the most feared is radial artery occlusion (RAO). Although RAO is of concern, there is still no consensus regarding the routine assessment of radial artery patency before discharge and its relationship to the hemostatic device used. In order to fill this knowledge gap, this study was planned to compare the effect of two hemostasis devices after cardiac catheterization performed by transradial access on the appearance of RAO. A two-group Randomized Clinical Trial (RCT) was conducted at the Hemodynamic Laboratory (HL) of the Hospital de Clínicas of Porto Alegre (HCPA), a public university hospital in the metropolitan region of Porto Alegre, Rio Grande do Sul. The collection period was from November 2015 to October 2016. Adult patients from the outpatient clinic and hospitalized patients undergoing cardiac diagnostic and / or therapeutic catheterization due to transradial access, in an elective and / or emergency manner, were included. Participants were randomized into two groups: intervention group (IG) - hemostasis with TR Band device; Control Group (CG) - hemostasis with gaze device and adhesive elastic bandage. The primary outcome was immediate RAO to the procedure and in 30 days; secondary outcomes were migration of the curvature patterns after immediate device removal and in 30 days, the need for extra time to reach hemostasis other vascular complications related to the puncture site (bleeding and hematoma), and the presence and intensity of pain. A total of 600 patients were included: IG (n=301) and CG (n=299), predominantly males with a mean age of 63 ± 10 years; RAO immediately following device removal occurred in 24 (8%) and 19 (6%), in the IG and CG groups, respectively; in 30 days for a third of the sample was 5 (5%) and 7 (6%) in IG and CG, respectively. For these two periods no statistical difference was demonstrated. The additional hemostasis time, as well as the time required for another type of compression, was significantly higher in IG, p=0.006 and p<0.001, respectively; patients from both groups maintained a pattern of migration of similar curves in the pre-procedure assessment, immediately after the device withdrawal and in 30 days; The lowest bleeding was significantly higher in the IG, compared to the GC, 67 (22%) vs 40 (13%), respectively, p = 0.006; the hematoma was similar between the two groups. The reported pain was similar between the groups, and the reported intensity was moderate. The results of this study allow us to conclude that the incidence of RAO was similar between TR Band groups and adhesive elastic bandage. The standards of the Barbeau test curve were similar in all assessments. Patients who used TR Band required more extra hemostasis time as well as more time with another type of compression. Minor bleeding was more incident in the TR Band group, while the hematoma was similar between the groups. The occurrence of moderate pain was similar between the two groups.

A via radial é objeto de interesse crescente de cardiologistas intervencionistas, por oferecer menores taxas de complicações vasculares e redução de sangramento maior, associado ao risco de morte e eventos isquêmicos. Este estudo objetiva comparar a eficácia da hemostasia com compressão mecânica em duas e três horas, e a ocorrência de complicações vasculares, por avaliação clínica e por USG com Doppler. Realizou-se uma revisão de literatura intitulada "Oclusão da artéria radial relacionada ao cateterismo cardíaco transradial", no qual se revisaram 19 artigos de periódicos publicados entre 2005 a 2015, os quais relacionaram os principais fatores predisponentes à oclusão da artéria radial após cateterismo cardíaco. No presente estudo 206 pacientes submetidos a cateterismo transradial foram randomizados em dois grupos com tempo de compressão em duas horas (T2) e três horas (T3), contendo 103 pacientes em cada grupo. Foi realizado exame clínico no local de punção, antes e depois da compressão no dia do procedimento, e após o 7º dia reavaliado através de exame clínico e USG com Doppler. No artigo intitulado "Tempo de compressão da artéria radial pós cateterismo cardíaco e complicações vasculares", descreveu-se o estudo em detalhes. Em seus resultados observou-se que a avaliação clínica realizada no dia do cateterismo cardíaco não demonstrou diferenças relevantes entre os grupos T2 e T3. Equimose foi encontrada em apenas dois pacientes (0,9%); Espasmo arterial durante a retirada do introdutor ocorreu em 56 pacientes (27,2%); Hematoma pós compressão foi encontrado em 34 pacientes (16,5%); O tamanho do hematoma encontrado na maioria dos pacientes (97,1%) foi de pequena dimensão (<5cm) , apenas um paciente e pertencente ao grupo T2 (2,9%) apresentou hematoma extenso (≥10cm); Em 24 pacientes (11,7%) ocorreu sangramento após remoção do curativo compressivo no grupo T3; Quanto ao exame ultrassonográfico com Doppler evidenciou-se redução do fluxo sanguíneo em dois pacientes de cada grupo (2,0%), presença de oclusão da artéria radial no grupo T3 (11,0%), hematoma subcutâneo em dois pacientes do grupo T2 (2,0%), pseudoaneurisma em um paciente no grupo T3 (1,0%), Edema subcutâneo em um paciente do grupo T3 (1,0%). Dissecção arterial em um paciente de cada grupo (1,0%). Não foi evidenciado presença de estenose ou fístula arteriovenosa nos grupos. Conclui-se que na amostra estudada a utilização da via transradial para cateterismo cardíaco foi segura e eficaz, não apresentando diferenças estatísticas significantes na ocorrência de sangramento e complicações vasculares, com a utilização do curativo compressivo para hemostasia em duas e três horas. E que o tempo de duas horas deve ser adotado pois se mostrou seguro e eficaz na hemostasia sanguínea, apresentando baixas taxas de complicações vasculares, como a oclusão da artéria radial.
The radial routeis object of growing interest of interventional cardiologists, by offering lower rates of vascular complications and bleeding reduction, associated to the risk of death and ischemic events. This study aims to compare the efficacy of hemostasis with mechanical compression of two and three hours, and the occurrence of vascular complications, by clinical assessment and USG with Doppler. We carried out a revision in the literature entitled "occlusion of the radial artery related to transradial cardiac catheterization," in which it was reviewed 19 journal articles published from 2005 to 2015, which related the main factors predisposing to occlusion of the radial artery after cardiac catheterization. In this study 206 patients undergoing to the transradial catheterization were randomized into two groups with compression time in two hours (T2) and three hours (T3) containing 103 patients in each group. Clinical examination was performed at the puncture site, before and after the compression on the day of the procedure, and after the 7th day reassessed by clinical examination and USG Doppler In the article entitled "radial artery compression time after cardiac catheterization and vascular complications," was described the study in detail. In its results it was observed that the clinical evaluation performed on the day of cardiac catheterization showed no significant differences between T2 and T3 groups. Bruising was found in only two patients (0.9%); arterial spasm during with draw al of introducer occurred in 56 patients (27.2%); post compression hematoma was found in 34 patients (16.5%); The size of the hematoma found in most patients (97.1%) was by the small dimension (<5cm), only one patients belonging to the group and T2 (2.9%) showed extensive hematoma (≥ 10cm);In 24 patients (11.7%) occurred bleeding after removal of the pressure dressing in T3 group; as to the ultrasound Doppler examination showed a reduction of blood flow in two patients in each group (2.0%), presence of radial artery occlusion in the T3 group (11.0%), subcutaneous hematoma in two patients in the T2 group (2.0%), pseudo aneurysm in 01 patients in the T3 group (1.0%), subcutaneous edema in on patient T3 group (1.0%). Arterial dissection in one patient in each group (1.0%).It was not evidenced the presence of stenosis or arteriovenous fistula in the groups. it was concluded that in the sample studied the utilization of the transradial route for the cardiac catheterization was safe and effective, without statistically significant differences in the occurrence of bleeding and vascular complications such as occlusion of the radial artery, in the use of the compressive dressing to hemostasis of two and three hours. And the time of two hours should be adopted because it proved to be safe and effective hemostasis in the blood , resulting in low rates of vascular complications , such as the radial artery occlusion.

Testo completoINTRODUCTION: The ostium secundum atrial septal defect is a congenital heart disease characterized by partial or total deficiency of the fossa ovalis, also known as the septum primum. It corresponds to 10-12% of all congenital heart defects, being the most frequently found in adulthood. Currently, percutaneous closure is the therapeutic method of choice for defects with suitable anatomic features for device implantation in most centers in the world. Bi-dimensional transesophageal echocardiography with color flow mapping is considered the gold-standard tool for anatomic assessment and procedural monitoring, being crucial for optimal device selection. In this regard, a sizing balloon is introduced and inflated across the defect with temporary occlusion. The waist measurement seen on the balloon (stretched diameter) is used as a reference for selection of device size. Recently, real time three-dimensional transesophageal echocardiography has been utilized in this type of percutaneous intervention. In this study we assessed the role of this modality in optimal device selection taking into consideration the dimensions and the geometry of the defect and the thickness of the interatrial septum rims. METHODS: Observational, prospective, non-randomized, single-arm study of a cohort of 33 adults with atrial septal defects submitted to percutaneous closure using a self-centered nitinol device (Cera (TM), Lifetech Scientific, Shenzhen, China). The largest and the smallest diameter of the defect, its area and the measurements of the stretched diameter of the sizing balloon were assessed by both echocardiographic modalities. The defects were considered as elliptical or circular according to their geometry; the rims surrounding the defect were considered thick (> 2 mm) or thin. The selected device was equal to or 2 mm larger than the stretched diameter on bi-dimensional transesophageal echocardiography (gold-standard). In an attempt to identify a variable that could replace the stretched balloon diameter, a series of linear correlations were performed. RESULTS: The mean age and weight were 42.1 ± 14.9 years and 66.0 ± 9.4 kgs, respectively; being 22 of the female gender. There were no statistical differences between the largest and smallest diameters of the defects and the stretched diameters determined by both echocardiographic modalities. The correlation between the measurements obtained by both methods was excellent (r > 0.90). The largest defect diameter obtained by three-dimensional transesophageal echocardiography was the variable that showed the best correlation with the selected device size in the entire group (r= 0.89), especially in the subgroups with elliptical geometry (r= 0.96) and with thick rims surrounding the defect (r=0.96). CONCLUSIONS: In this study in adults with ostium secundum atrial septal defects submitted to percutaneous occlusion with the Cera (TM) device, optimal device selection could be performed using solely the largest diameter of the defect obtained by real time three-dimensional transesophageal echocardiography, especially in patients with elliptical defects and thick rims.

cateterismo cardíaco é um procedimento invasivo que tem por objetivo avaliar a anatomia e fisiopatologia cardiovascular. Na pediatria, ele permite diagnosticar cardiopatias congênitas, avaliar sua repercussão, bem como, tratá-las de forma paliativa ou definitiva. A assistência de enfermagem à criança deve ultrapassar a prestação de cuidados físicos fundamentada apenas numa visão biológica. Devem-se considerar também suas necessidades emocionais, sociais e utilizar técnicas de comunicação e de relacionamento que sejam adequadas, dentre elas, destaca-se o brinquedo terapêutico. Esta dissertação objetivou validar um brinquedo e história como tecnologia educativa para o preparo de crianças que serão submetidas ao cateterismo cardíaco em sessão de brinquedo terapêutico. Foram desenvolvidos dois artigos científicos que constituem a seção dos resultados. O artigo de revisão integrativa objetivou investigar as tecnologias educativas utilizadas em ações de educação em saúde na promoção da saúde da criança. O levantamento bibliográfico foi realizado nas bases de dados LILACS, MEDLINE, BDENF, PUBMED, CINAHL e na biblioteca virtual Cochrane Library e SciELO a partir do cruzamento dos descritores “educação em saúde”, “tecnologia educacional”, “criança”, “promoção da saúde” e “saúde da criança”. Foram selecionados 13 artigos os quais evidenciaram que as tecnologias educativas têm sido empregadas a fim de promover ações para promoção da saúde da criança, contribuindo com o alcance de resultados duradouros. São utilizadas tecnologias leve, leve-duras e duras. A associação da tecnologia leve e dura obteve melhores resultados. O artigo original objetivou validar um brinquedo e história como tecnologia educativa para o preparo de crianças que serão submetidas ao cateterismo cardíaco em sessão de brinquedo terapêutico. Foi realizado um estudo metodológico, desenvolvido em duas etapas - construção do brinquedo e história que ocorreu mediante compreensão da assistência prestada à criança submetida a cateterismo cardíaco e entrevista com profissionais da equipe, e - validação de conteúdo da tecnologia educativa realizada por 23 juízes (contadores de história, educadoras infantis, enfermeiros, médicos, psicóloga, psicopedagogas e terapeutas ocupacionais). O brinquedo construído neste estudo é composto por nove bonecos de pano, um protótipo do angiógrafo e do aparelho de anestesia e objetos de uso hospitalar. A história elaborada aborda de forma lúdica a estrutura física da sala de 10 Hemodinâmica e todas as etapas que compreendem o pré, trans e pós-cateterismo cardíaco. Foi considerado o índice de validade de conteúdo para validação de 0,8. Resultados: a análise foi realizada em relação ao objetivo, à estrutura e apresentação e à relevância. O índice de validade de conteúdo obtido quanto ao objetivo, estrutura e apresentação foi 0,95 e quanto à relevância 0,96. Os juízes contribuíram para o aprimoramento da tecnologia mediante maior dinamismo, objetividade, ampliação da interação com a criança, adequação da linguagem e ordenação dos fatos. O índice de validade de conteúdo global obtido foi de 0,95 o que demonstra adequação em relação às dimensões avaliadas. O brinquedo e história validados neste estudo ao serem utilizados em sessão de brinquedo terapêutico poderão constituir um diálogo entre saúde, educação e arte, capaz de possibilitar uma interação entre o profissional da saúde, a criança e familiares.
Cardiac catheterization is an invasive procedure that aims to assess the anatomy and cardiovascular pathophysiology. In pediatrics, it allows diagnose congenital heart disease, assess their impact as well, treat them palliative or definitive form. The child nursing care must go beyond the provision of physical care based only on a biological view. Also should consider their emotional, social and use communication techniques and relationships that are appropriate, among which stands out the therapeutic toy. This work aims to validate a toy and history as an educational technology to prepare children to be submitted to cardiac catheterization in therapeutic play session. Two papers that make up the section of the results have been developed. The integrative review article aimed to investigate the educational technologies used in health education activities in the children's health. The bibliographic research was conducted in the databases LILACS, MEDLINE, BDENF, PubMed, CINAHL and the Cochrane Library and virtual library SciELO from crossing the descriptors "health education", "educational technology", "child", "promotion of health "and" health of the child. " We selected 13 articles which showed that educational technologies have been employed to promote actions for child health promotion, contributing to the achievement of lasting results. Light technologies, light-hard and hard are used. The association of light and hard technology produced better results. Was considered the content validity index for validation 0.8. Results: The analysis was performed in relation to the purpose, structure and presentation and relevance. The content validity index obtained about the purpose, structure and presentation was 0.95 and the relevance 0.96. The judges contributed to the improvement of technology through greater dynamism, objectivity, expansion of interaction with the child, and family.

OBJETIVO: A troca valvar aórtica é procedimento rotineiro, envolve substituição da valva nativa/prótese. Na maioria destes pacientes o risco é aceitável, porém, em alguns casos, o risco predito pode justificar contra-indicação. O implante de valva aórtica minimamente invasivo transcateter tem se mostrado viável e com menor morbi-mortalidade. O objetivo deste trabalho foi desenvolver bioprótese aórtica nacional, montada em cateter, para implante em posição aórtica. MÉTODO: Após desenvolvimento em animais, 14 pacientes com EuroSCORE elevado foram submetidos ao implante. A idade média dos pacientes foi 75,5 anos. O Euroscore logístico médio foi 43,7 %. Quatro pacientes apresentavam disfunção de biopróteses e o restante, estenose aórtica grave com gradiente de pico médio de 79,7 mmHg. Após minitoracotomia e sob controle ecocardiográfico e fluoroscópico, cateter-balão foi posicionado sobre posição aórtica e insuflado. Após, segundo cateter-balão, com endoprótese valvada, foi posicionado e liberado sob alta frequência ventricular ou hipotensão controlada por drenagem sanguínea. Controles angiográficos e ecocardiográficos foram realizados e pacientes encaminhados para UTI. RESULTADOS: O implante foi possível em 13 casos. Não ocorreram óbitos intraoperatórios. A mortalidade geral foi 42%. O gradiente valvar aórtico reduziu de forma significativa para 25,0 mmHg em média, a função ventricular esquerda apresentou melhora significativa nos primeiros 7 dias de evolução. A regurgitação aórtica perivalvar esteve presente na maioria dos casos, porém foi predominantemente leve. A contagem plaquetária reduziu de forma significativa após o procedimento, recuperando seu valor inicial apenas após 30 dias. CONCLUSÃO: O implante de bioprótese transapical montada em cateter mostrou ser procedimento factível. Detalhes técnicos e a curva de aprendizado demandam discussão.
OBJECTIVE: The aortic valve replacement is a routine procedure, and involves replacement of the native valve/prosthesis. In most of the patients who undergo such procedure the risk is acceptable, but in some cases, such risk can justify contraindication. The minimally invasive transcatheter aortic valve implantation has been viable, with lower morbidity and mortality. The aim of this study was to develop a national catheter-mounted aortic bioprosthesis for aortic position implant. METHOD: After animal studies, 14 patients with high EuroSCORE underwent transcatheter aortic valve implantation. Median age was 75,5 years. Median Logistic EuroSCORE was 43,7%. Four patients presented with disfunctional bioprosthesis, remainig ones presented calcified aortic stenosis (peak transvalvular gradient 79,7 mmHg). After minithoracotomy and under echocardiograph and fluoroscopic control, a balloon catheter was placed on aortic position and inflated. After, a second balloon with valved endoprosthesis was positioned and released under high ventricular rate. Echocardiograph and angiograph controls were performed and the patients were referred to ICU. RESULTS: Implant was feasible in 13 cases. There were no intra-operative deaths. Mortality was 42%. Median peak transvalvular aortic gradient reduced to 25,0 mmHg, and left ventricular function improved in the first 7 post operative days. Paravalvular aortic regurgitation was mild and present in most cases. Platelet count significantly decreased after procedure, but returned to normal values after 30 days. CONCLUSION: The transapical implantation of catheter-mounted bioprosthesis was a feasible procedure. Technical details and learning curve require further discussion.

A Hipertensão Arterial Pulmonar (HAP) é caracterizada pela elevação sustentada da pressão arterial nos capilares pulmonares, com vasoconstrição e remodelação vascular local. A HAP é definida hemodinamicamente através de valores da pressão arterial pulmonar média em repouso ≥ 25 mmHg, com pressão arterial capilar ≤ 15mmHg e resistência vascular pulmonar > 3 unidades de wood. A HAP é considerada como doença rara, cujos estudos epidemiológicos europeus descrevem a incidência de 1 a 2 casos e prevalência de 15 casos de HAP por milhão de habitantes, com predomínio do sexo feminino, na razão de 1,7: 1,0 e média de idade de 37 anos. Objetivo: Determinar a prevalência de Hipertensão Arterial Pulmonar e o possível impacto clínico em pacientes com a forma crônica indeterminada da Doença de Chagas. Estudo transversal e observacional, onde foram incluídos 99 pacientes com a Forma Crônica Indeterminada da Doença de Chagas e 08 controles com ausência da doença. Houve exclusão de doenças prévias que pudessem ocasionar HP. Três grupos foram formados: G1 (controle), G2 (Chagas) e G3 (Chagas com suspeita de HP), através de critérios clínicos e da realização de ecocardiograma.Os indivíduos foram classificados de acordo com a Classe Funcional(NYHA) modificada para HP e submetidos à Espirometria, Teste de Caminhada de Seis Minutos, Dosagem de Óxido Nítrico (NO) .Predominaram gênero masculino, com idade de 55,62 ± 8,71 anos, Classe Funcional de NYHA :I-II (93,45%) e Espirometrias normais: 88 (82,24%). A dosagem de NO foi estatisticamente significante p<0,05 ao compararmos os indivíduos com Chagas ao Grupo Controle. Pacientes com suspeita de Hipertensão Pulmonar (HP) detectada no ecocardiograma com Pressão Sistólica Artéria Pulmonar (PSAP)>40mmHg, foram submetidos ao Cateterismo Cardíaco Direito e resultou em prevalência de HAP: 1,01% . O indivíduo com HAP tem 48 anos e sexo feminino, compatível com a literatura. A biodisponibilidade elevada dos níveis de NO esteve presente nos pacientes com Chagas com ou sem sintomas de HP. Os dados presentes no estudo reforçam a hipótese de que pacientes com a Forma Crônica Indeterminada da Doença de Chagas sofrem com sobrecarga de volume intravascular associada a processo inflamatório intenso com provável evolução para comprometimento cardíaco.

After realization of echocardiographic exams three groups were formed: G1: control, G2: Chagas and G3: Chagas with suspicion of PH. The individuals were classified according to the Functional Class of the New York Heart Association (NYHA), and submitted to Spirometry, Six-Minute Walking Test and Serum Dosing of Nitric Oxide (NO). This was a predominantly male (54.20%), middle-aged (mean 55.62 ± 8.71 years) cohort of NYHA Functional Class I-II (93.45%) and normal Spirometries (82.24%). During echocardiograph, patients that presented Pulmonary Artery Systolic Pressure (PASP)>40mmHg were considered to have suspicion of PH and submitted to right cardiac catheterization, resulting in PAH prevalence of 1.01%. The patient with PAH is female, aged 48 years, and presents studied variables compatible with the literature. Elevated NO bioavailability was present in patients with Chagas with or without PH symptoms. The findings of the present study reinforce the hypothesis that patients with the indeterminate chronic form of CD suffer from an overload of intravascular volume associated with an intense inflammatory process with probable evolution to cardiac impairment.

Testo completoAdvances in knowledge, techniques, devices, and therapeutic approaches, combined with more powerful systems for anticoagulation, have generated impact on the occurrence of vascular complications in patients undergoing invasive procedures in Hemodynamics Laboratory. Despite mortality scores, major bleeding, and risk of cardiovascular events are available in the literature, it is not of our knowledge that there is a risk score to assess vascular complications in patients undergoing cardiac invasive procedures in Hemodynamics Laboratories. Seeking to fill this gap, the aim of this study is to develop and to validate a predictive score of vascular complications related to arterial puncture in patients undergoing cardiac procedures. It was designed a cohort study, multicenter, developed in three institutions with Hemodynamics Laboratory of Rio Grande do Sul, from October 2012 to March 2014. We included adult patients who underwent arterial puncture procedure classified as elective or urgent. The data collection was preceded by the training of the research team. Endpoints were defined as the presence of vascular complications: 1) hematoma at the arterial puncture site; 2) major and minor bleeding; and 3) need for surgical correction of retroperitoneal hemorrhage, pseudo aneurysm, or arteriovenous fistula formation. In order to derive and validate the score after the entire data collection, 2/3 of the patients were randomly divided to compose the derivation cohort, while the remaining patients composed the validation cohort. To establish the derived score of the evaluation tool of the patients, the identified variables from the univariate analysis with p<0.25 were included in a logistic regression model. The final model included variables with p<0.05. Using the rounded values of OR, the score was built (derivation cohort). Subsequently, the predictive performance of the score was evaluated in the derivation cohort, when it was identified the best balance between sensitivity and specificity, plotted ROC curve, and evaluated using C-statistics. The predictive confirmation was tested in the validation cohort using the chi-square test. The project was ethically and methodologically approved across the three centers. Of the 2696 patients included, 237 had some type of vascular complication in the arterial puncture site (8.8%). The derived predictive score for complication is the following equation: (Introductor > 6F x 4.0) + (Therapeutic Procedure x 2.5) + (prior Vascular Hemodynamics Complication x 2.0) + (previous use of anticoagulants Warfarina® (Marevan ®) or Fenprocumon® (Marcoumar®) x 2.0) + (Females x 1.5) + (age ≥ 60 years x 1.5). After establishment of the cohort point, the score was dichotomized with the value of ≥3, with sensitivity of 0.66 (95% CI 0.59 to .73), and specificity of 0.59 (95% CI 0.56 to .61). When comparing the risk for complications in all patients (n = 2696), it was identified a higher risk for vascular complications in patients with a score ≥3 (OR: 2.95; 95% CI: 2.22 to 3.91). Results of this study show a predictive score of vascular complications, and of easy application in clinical practice by professionals involved in the patient care in the Hemodynamics Laboratory.
El avance en el conocimiento, técnicas, artificios, arsenal terapéutico, coligados a regímenes más potentes de anticoagulación, ha generado impacto en la ocurrencia de complicaciones vasculares en pacientes sometidos a procedimientos invasivos en el Laboratorio de Hemodinámica. Aunque disponibles en la literatura del área resultados sobre mortalidad, sangramiento mayor y riesgo de eventos cardiovasculares, no es de nuestro conocimiento que exista un resultado de riesgo para evaluar complicaciones vasculares en pacientes sometidos a procedimientos invasivos cardiológicos en Laboratorios de Hemodinámica. Con el objetivo de rellenar este vacío del conocimiento, este estudio busca desarrollar y validar un resultado de predicción de complicaciones vasculares relacionadas a la punción arterial en pacientes sometidos a procedimientos cardiológicos. Se ha hecho un estudio de Coorte, multicéntrico, ejecutado en tres instituciones con Laboratorios de Hemodinámica de Rio Grande del Sur, en el periodo de octubre de 2012 a marzo de 2014. Fueron inclusos pacientes adultos, que hicieron procedimientos por punción arterial en carácter electivo o urgente. La recolección fue antecedida por una capacitación del equipo de investigación. Fueron considerados como resultado final la presencia de complicaciones vasculares: 1) hematoma en el local de la punción arterial; 2) sangramiento mayor y menor; 3) solicitud de corrección quirúrgica para hemorragia retroperitoneal, pseudoaneurisma o formación de fístula arteriovenosa. Con la finalidad de derivar y validar el resultado, tras la recolección de la totalidad de los datos, de modo randómico, 2/3 de los pacientes fueron elegidos para constituir la Coorte de derivación, mientras los demás constituyeron la Coorte de validación. Para establecer el resultado derivado del instrumento de evaluación de los pacientes, las variables identificadas en el análisis univariado con valor de p<0,25 fueron incluidas en un modelo de Regresión Logística. Permanecieron en el modelo final las variables con valor de p<0,05. Usándose el valor arredondado de OR encontrado, fue dispuesto el resultado (Coorte de derivación). Tras el desempeño predictivo de este resultado fue evaluado en la Coorte de derivación, cuando fue identificado el mejor punto de equilibrio entre sensibilidad y especificidad, ploteada curva ROC y evaluada estadística-C. La confirmación de la predicción fue sometida a un control en la Coorte de validación a través del Test Qui-cuadrado. El proyecto fue aprobado con relación a sus aspectos éticos y metodológicos en los tres centros. De los 2696 pacientes incluídos, 237 presentaron algun tipo de complicación vascular en el lugar de la punción arterial (8,8%). El resultado derivado para predicción de complicación constituye la siguiente equación:(Introductor >6F x 4,0) + (Procedimiento Terapéutico x 2,5) + (Complicación Hemodinámica Vascular previa x 2,0) + Uso previo de Anticoagulantes Warfarina® (Marevan®) o Fenprocumon® (Marcoumar®) x 2,0) + (Sexo Femenino x 1,5) + (Edad ≥ 60 años x 1,5). Luego del estabelecimiento del punto de coorte, el resultado fue dividido en el valor ≥3, siendo sensibilidad de 0,66 (IC 95% 0,59-0,73) y especificidad 0,59 (IC 95% 0,56-0,61). Al compararse el riesgo para complicaciones, en la totalidad los pacientes (n=2696) se identificó mayor riesgo para complicaciones vasculares en aquellos con resultado ≥3 (OR: 2,95; IC95%: 2,22-3,91). Los resultados de ese estudio mostraron un riesgo capaz de anticipar complicaciones vasculares, de fácil aplicación en la práctica clínica diaria, por profesionales que realizan el cuidado en el Laboratorio de Hemodinámica.

Testo completoIntrodução: As doenças cardiovasculares são as principais causas de morte no mundo. Dentro desse grupo, as doenças isquêmicas do coração (DIC) são as mais frequentes e responsáveis por altos números de morbimortalidade. Nas últimas duas décadas ocorreram grandes avanços no reconhecimento, manejo e tratamento de pacientes com DIC, o que determinou significativa redução das taxas de óbito, infartos e isquemia recorrente. Apesar do crescente avanço tecnológico e da utilização de técnicas contemporâneas, a dor e ansiedade ocasionam uma série de repercussões fisiológicas que podem prejudicar a qualidade dos desfechos clínicos. Objetivos: Este estudo tem como objetivo identificar na literatura o efeito da música sob a dor, ansiedade e parâmetros hemodinâmicos antes e após procedimentos cardíacos percutâneos. Método: Trata-se de uma revisão sistemática com metanálise que seguiu todas as recomendações da Colaboração Cochrane e seguindo os itens de verificação preconizados pela ferramenta Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA), bem como se baseou na ferramenta Assessing the Methodological Quality of Systematic Reviews (AMSTAR). Resultados: Foram encontrados 40 artigos na busca inicial dos quais 11 preencheram os critérios de inclusão. Estes demonstraram que a música é efetiva na redução da ansiedade e dos parâmetros hemodinâmicos, sendo ainda confirmada na metanálise que identificou uma estimativa metanalítica de diferença nas médias padronizadas de 4.02 (IC 95%: 2.48 a 6.68), mesmo sendo escolhida pelo pesquisador. Contudo, seus efeitos sobre a dor foram inconclusivos, o que também foi mostrado na metanálise que identificou uma estimativa metanalítica não significativa de -0,01 (IC 95%: -0,36 a 0,32) quando se utilizou a EVA, sendo necessário a realização de estudos futuros. Devido a variabilidade dos estilos e tempo de exposição musical, não se pôde determinar qual a música e o tempo de exposição ideais para alcançar resultados mais positivos sobre os desfechos de interesse, mas infere-se que aquelas de batidas suaves, sem letras e lentas presentem melhores efeitos. Quanto à forma de aplicação, a utilização de fones de ouvido é recomendada, pois permite que o paciente foque sua atenção no estimulo musical. A maior parte dos estudos obteve boa qualidade metodológica com baixo risco de viés. Já a metanálise identificou risco de viés de publicação para os estudos que abordaram ansiedade. Conclusão: Esta revisão sistemática identificou que a música é efetiva na redução da ansiedade e dos parâmetros hemodinâmicos. Entretanto, os resultados foram inconclusivos para determinar associação na redução da dor. Estes resultados podem ser úteis no desenvolvimento de recomendações para utilização da música na cardiologia intervencionista.

Testo completoDespite evidence that point out that reduction of rest in bed time after diagnostic cardiac catheterization using transfemoral approach does not contribute to complications of arterial puncture, such practice was still not incorporated into several hemodynamic laboratories (HL), especially in Latin American centers. In order to fill this knowledge gap, this study presents a test for the reduction of rest in bed time of three hours (IG) compared to rest in bed time of five hours (CG), aiming at verifying if the time does not increase complications of arterial puncture after diagnostic cardiac catheterization using 6 French introducer and the transfemoral approach. A randomized clinical trial (RCT) was conducted at a HL in a public university hospital located in the metropolitan area of Rio Grande do Sul/Brazil, between January 2011 and September 2013. Adult outpatients were included in this trial. The IG remained three hours after the withdrawal of the 6 French introducer and the CG remained five hours in the ambulatory. All patients remained five hours in the observation room, being checked every one hour by the nursing staff and being contacted by telephone at 24, 48 and 72 hours after the discharge. The outcomes hematoma, bleeding, retroperitoneal hemorrhage, pseudoaneurysm, arteriovenous fistula and vasovagal response were assessed. For the study, 730 patients were included and separated into GI (n=367) and GC (n=363), mean age of 62 ± 11. While patients remained in the observation room in the HL, the hematoma was the most common complication observed in both groups, 12(3%) in IG and 13(4%) in CG (P=0.87); in IG, 11(3%) patients presented small hematoma and 1(0.3%) presented large hematoma; in CG, 11(3%) patients presented small hematoma and 2(0.6%) presented large hematoma; bleeding occurred in 4(1%) patients in IG and 6(2%) in CG (P=0.51); both IG and CG presented minor bleeding occurrences. The vasovagal response occurred in 5(1.4%) patients in IG and 4(1.1%) patients in CG (P=0.75). During telephone contacts at 24, 48 and 72 hours, ecchymosis was the most prevalent complication for the three periods in both groups, followed by pain at the puncture site. Statistical significance was not observed in any of the comparisons. Only 1(0.3%) patient in the CG had pseudoaneurysm within 48 hours after discharge and had to return to the hospital for treatment. No other complications were observed during the study period. The results of this study demonstrate that reducing the time of rest in bed for three hours after diagnostic cardiac catheterization proved to be a safe procedure, with no increase in complications when compared to patients who remained resting in bed for five hours.
Con relación a las evidencias de que la reducción del reposo en el lecho tras cateterismo cardíaco diagnóstico bajo abordaje transfemoral no aumenta las complicaciones decurrentes de la punción arterial, esta práctica aún no está incorporada en muchos laboratorios de hemodinámica (LH), principalmente en centros latinoamericanos. En la tentativa de rellenar este hueco del conocimiento se hizo un test en este estudio para saber si la reducción del tiempo de reposo en el leco para tres horas (GI), comparada a cinco horas (GC), no aumenta las complicaciones decurrentes de la punción arterial tras cateterismo cardíaco diagnóstico con introductor 6 French y vía transfemoral. Fue dirigido un Ensayo clínico randomizado (ECR) en LH de un hospital público y universitario, región metropolitana, de Río Grande del Sur en el periodo de enero de 2011 a septiembre de 2013. Fueron incluidos pacientes adultos ambulatoriales. GI deambuló tres horas luego de la retirada del introductor, y GC tras cinco horas. Todos los pacientes permanecieron cinco horas en la sala de observación donde fueron observados a cada hora, por el equipo de enfermería, y contactados por teléfono en 24, 48 y 72 horas tras el alta hospitalario. Fueron evaluados los siguientes resultados: hematoma, hemorragia, hematoma retroperitoneal, pseudoaneurisma, formación de fístula arteriovenosa, reacción vaso y vagal. Se incluyeron 730 pacientes en la sala de observación de LH, el hematoma fue la complicación más observada en ambos grupos, 12 (3%) GI y 13 (4%) GC (P=0,87); en GI 11(3%) pacientes presentaron hematoma clasificado como pequeño y 1(0,3%) presentó hematoma clasificado como grande; mientras que en GC 11(3%) pacientes presentaron hematoma clasificado como pequeño y 2 (0,6%) presentaron hematoma clasificado como grande; la hemorragia ocurrió en 4 (1%) de los pacientes en GI y 6 (2%) en GC (P=0,51), tanto en GI como en GC la ocurrencia de hemorragia fue considerada menor. La reacción vaso vagal ocurrió en 5 (1,4%) pacientes en GI y 4 (1,1%) pacientes en GC (P=0,75). En los contactos en 24, 48 y 72 horas la equimosis fue la complicación que más prevaleció en los tres periodos, para ambos grupos, seguida por el relato de dolor en el local de la punción, para ninguna de las comparaciones fue observada significancia estadística. 1 (0,3%) paciente de GC presentó pseudoaneurisma en el contacto en 48 horas, necesitando volver al hospital para tratamiento. No se observó ninguna otra complicación durante todo el periodo del estudio. Los resultados de este estudio permiten concluir que la intervención en la reducción del tiempo de reposo para tres horas luego de cateterismo cardíaco diagnóstico electivo es posible y segura, sin aumento de complicaciones cuando comparada a los pacientes que permanecieron en reposo de cinco horas.

Testo completoA Persistência do Canal Arterial (PCA) é uma das malformações cardíacas congénitas mais comuns em cães, caracterizada pela falha no encerramento fisiológico da comunicação vascular entre a aorta (Ao) descendente e a artéria pulmonar (AP) no período neonatal. Apresenta predisposição rácica e uma maior incidência em fêmeas. Tipicamente apresenta shunt esquerdo-direito (PCA E-D), caracterizado por um sopro cardíaco contínuo e pulso hipercinético, que leva a sobrecarga de volume da circulação pulmonar e do átrio e ventrículo esquerdos. Esta sobrecarga pode levar ao aumento da resistência pulmonar e consequentemente a inversão do shunt para direito-esquerdo (PCA D-E), caracterizado pela presença de cianose diferencial das mucosas, letargia e síncope. É importante o diagnóstico precoce da PCA para uma correção cirúrgica atempada, antes que ocorra inversão do shunt. A ecocardiografia Doppler é importante no seu diagnóstico definitivo, através da identificação de um fluxo turbulento contínuo ao nível da Ao e da AP. O encerramento cirúrgico, tanto por cirurgia clássica como por cateterismo, apresenta elevadas taxas de sucesso e é atualmente o tratamento recomendado para a correção de PCA. Embora o prognóstico seja reservado na ausência de tratamento, trata-se de uma doença que poderá apresentar bom prognóstico após o encerramento atempado do canal arterial (CA). Este trabalho foi baseado em 11 casos com PCA E-D submetidos a correção cirúrgica, 5 dos quais assistidos durante o período de estágio entre Setembro de 2017 e Fevereiro de 2018. Ocorreu uma maior prevalência de PCA em fêmeas e em raças predispostas. O exame físico revelou sopro cardíaco contínuo de alta intensidade e pulso hipercinético. No ECG destacaram-se ondas Q profundas e na ecocardiografia transtorácica verificou-se dilatação ventricular esquerda e fluxo turbulento contínuo de alta velocidade através do modo Doppler. Para o encerramento da PCA, os animais foram submetidos a correção cirúrgica pela técnica clássica adaptada ou pela técnica de oclusão por cateterismo cardíaco com auxílio de um dispositivo Amplatz® Canine Duct Occluder (ACDO). Ambas as técnicas revelaram ser eficazes no tratamento da PCA, tendo a ecocardiografia transtorácica pós-cirúrgica evidenciado melhoria nos vários parâmetros cardíacos, independentemente da técnica utilizada.
ABSTRACT - Patent Ductus Arteriosus (PCA) is one of the most common congenital cardiac malformations in dogs, characterized by failure to physiologically close the vascular communication between the descending aorta (Ao) and the pulmonary artery (PA) during the neonatal period. It has breed predisposition and a higher incidence in females. Typically, it presents a left to right shunt (PCA E-D), characterized by a continuous heart murmur and hyperkinetic pulse, which leads to volume overload of the pulmonary circulation and of the left atrium and ventricle. This overload may lead to an increase in the pulmonary resistance and consequently the reversal of the shunt into a right to left shunt (PCA D-E), characterized by the presence of differential cyanosis of the mucosa, lethargy and syncope. Early diagnosis of PCA is important for timely surgical correction before reversal of the shunt occurs. Doppler echocardiography is important in providing a definitive diagnosis, through the identification of a continuous turbulent flow near the Ao and the AP. Surgical closure, both for classic and catheter surgery, has high success rates and is currently the recommended treatment for PCA correction. Although the prognosis is reserved in the absence of treatment, it is a disease that may present a good prognosis after a timely closure of the ductus arteriosus. This work was based on 11 cases with PCA E-D undergoing surgical correction, 5 of which were assisted during the period of the internship between September 2017 and February 2018. There was a higher prevalence of PCA in females and predisposed breeds. Physical examination revealed continuous high-intensity cardiac murmur and hyperkinetic pulse. The ECG revealed deep Q waves and transthoracic echocardiography revealed left ventricular dilatation and high velocity continuous turbulent flow through the Doppler mode. For the closure of the PCA, the animals were submitted to surgical correction by either an adaptation of the classic ligation technique or by occlusion through cardiac catheterization with the aid of an Amplatz® Canine Duct Occluder (ACDO) device. Both techniques proved to be effective in the treatment of PCA, and post-surgical transthoracic echocardiography revealed improvement in various cardiac parameters, regardless of the technique used.

A presente pesquisa abordou a prática educativa do enfermeiro na assistência de enfermagem prestada no Serviço de Hemodinâmica aos usuários submetidos ao exame de cineangiocoronariografia, tendo como objeto as representações sociais desses usuários acerca da cineangiocoronariografia (cateterismo cardíaco). Objetivos: caracterizar o perfil dos usuários do Serviço de Hemodinâmica submetidos à cineangiocoronariografia; descrever as representações sociais dos usuários acerca do exame de cineangiocoronariografia e desenvolver uma cartilha educativa acerca do exame de cineangiocoronariografia, de modo a contribuir para assistência qualificada, com base nas necessidades de informações dos usuários, resultando em real adesão ao preparo do exame. O referencial teórico foi pautado na Teoria das Representações Sociais. Método: estudo exploratório e descritivo, com abordagem qualitativa, tendo como cenário o serviço de hemodinâmica de um hospital universitário no estado do Rio de Janeiro, no período de agosto de 2012 a julho de 2014. Os sujeitos foram 20 usuários que procuraram o serviço de hemodinâmica para marcação e realização de procedimentos de cineangiocoronariografia. As etapas de desenvolvimento da pesquisa consistiram em: assinatura do Termo de Consentimento Livre e Esclarecido (TCLE); a coleta de dados foi através da técnica da entrevista semiestruturada, com perguntas abertas e fechadas, nas quais se identificou o perfil sociocultural dos sujeitos do estudo, e a construção da tecnologia educativa elegida, no caso cartilha. Os dados foram analisados e interpretados a partir da triangulação dos achados, à luz da análise de conteúdo temática. Resultados e discussão: os dados foram agrupados em três categorias: 1-Finalidade da cineangiocoronariografia: diagnóstico ou tratamento; 2 -Cineangiocoronariografia como um risco sentido/percebido; 3 - A enfermeira e as necessidades de informação sobre a cineangiocoronariografia. Os resultados revelaram que os usuários desconhecem a finalidade do exame e para que ele serve; mas possuem alguma informação sobre este. Apesar de não revelarem claramente, fazem associação do exame com a morte. Os resultados mostraram ainda que esta associação nem sempre está voltada para a finitude da vida, mas também para as mudanças que poderão advir a partir do resultado do exame. Conclusão: As tecnologias educativas são ferramentas que, usadas pelo enfermeiro, poderão complementar a sua práxis. As orientações podem ser realizadas pelo enfermeiro através de estratégias que promovam a adesão, a redução do medo, do estresse e da ansiedade, de maneira a favorecer a interação usuário-enfermeiro. É necessário favorecer um espaço para o processo terapêutico do cuidar, onde os profissionais de saúde possam contribuir de forma eficaz no processo do cuidado, de maneira que não utilizem apenas as suas habilidades e conhecimentos científicos
This research addressed the educational practice of nurses in the nursing care provided in the Hemodynamics Service to users submitted to the coronary cineangiography examination, taking as object the social representations of these users about coronary cineangiography (cardiac catheterization). Objectives: to characterize the profile of the users of the Hemodynamics Service submitted to coronary cineangiography; to describe the social representations of users about the coronary cineangiography examination and develop a educational leaflet about the aforementioned procedure, in such a way as to contribute to quality care, with basis on the needs for information on the part of users, resulting in actual adherence to the preparation of the examination. Theoretical benchmark grounded on the Social Representations Theory. Method: Exploratory and descriptive study with a qualitative approach, taking as scenario the Hemodynamics Service of a university hospital in the state of Rio de Janeiro, during the period between August 2012 and July 2014. The subjects were 20 users who sought the Hemodynamics Service for scheduling and accomplishment of procedures of coronary cineangiography. The stages of development of the research consisted of: signature of the Free and Informed Consent Form (FICF); the data collection was performed through the technique of semi-structured interview, with open and closed questions, where the socio-cultural profile of the study subjects was identified, as well there was the construction of the elected educational technology, in which case it is leaflet. The data were analyzed and interpreted from the triangulation of findings, in the light of the thematic content analysis. Results and discussion: The data were grouped into three categories: 1- Purpose of coronary cineangiography: diagnosis or treatment; 2- Coronary cineangiography as a felt/perceived risk; 3 - The nurse and needs for information about coronary cineangiography. The results have revealed that users are unaware of the purpose of the examination and its usability; but they have some information about it. Although they clearly do not reveal, they make association between such examination and death. Furthermore, the results denotes that this association is not always linked to the finiteness of life, but to changes that may arise from the outcome of the examination. Conclusion: The educational technologies are tools that, when used by nurses, will be able to complement their praxis. The guidelines can be performed by nurses through strategies that promote adherence, reduction of fear, stress and anxiety, in order to encourage the user-nurse interaction. One should provide a space to promote the therapeutic process of care in which health professionals contribute effectively in the process of care, in such a way as to prevent that they only make use of their scientific knowledge and skills

Testo completoIn the early 90’s a new class of agents blocking the GPIIb/IIIa-receptors on the platelet surface was developed capable of speeding up pharmacological reopening of infarct related arteries. The optimal time of administration remains subject of discussion. Despite these advantages hospital mortality in great registries is still more than 15% and hospital stay 12 days. By January 1st, 1999 a hotline between ambulance car and the interventional cardiologist was instituted. In each case management with either immediate PCI under GPIIb/IIIa-inhibition or half dose thrombolytics and full dose GPIIb/IIIa-inhibitors, if possible, started out of hospital. The approach was to see if the mortality was lower and the hospital stay shorter than in comparable registries. During September 1999 – December 2001 347 patients aged 26 to 92 years (60 plus minus 13) were treated. Median age was 62 years. There were 93 female (27%) and 254 male patients (73%). 57 (16%) patients received out of hospital thrombolysis, 81 (20%) were treated inhospital and 218 (63%) patients underwent primary cardiac catheterization. Overall inhospital median stay was 6.0 days. The median ICU-time for all thrombolysed patient was 1,0 days (prehospital 0,8 vs inhospital 1,3 days) and in the interventionally treated group 0,9 days. In all patients .8 MACE events occurred. Overall hospital mortality was 6,3 %, in the group of thrombolysed patients 9,3% (prehospital 12,3%, inhospital 6,9%). Shock excluded mortality was below 3%. 346/347 patients reached 180 days of follow-up. 180 days mortality overall was 10,8%, for the thrombolysed group 11,7% (prehospital 14,0% vs. inhospital 9,9%) and 9,2% in the interventionally treated group. There was no statistical significant difference. The time optimized individualized treatment assignment to pharmacological therapy followed by routine facilitated PCI within 24 hours is an approach that is more aggressive than currently recommended in the guidelines, but seems to offer further improvements of outcome with no further riscs for patients with AMI in comparison to other registries. Testo completoBACKGROUND: Pulmonary arterial hypertension (PAH) is a severe disease of the pulmonary circulation, which still relies on the right heart catheterization (RHC) for its accurate characterization. Electrical impedance tomography (EIT) is a non-invasive image tool that allows the bedside estimation of lung perfusion through the measurement of impedance variation during systole (deltaZQ). Although the search for non-invasive methods for evaluation of PAH is developing, data concerning application of EIT in PAH patients remain scant. OBJECTIVES: To evaluate the relationship between deltaZQ and the hemodynamic profile, severity, and prognosis of PAH patients. METHODS: Patients followed at the Pulmonary Circulation Unit (InCor-HCFMUSP) and submitted to RHC (due to the suspicion of PAH or clinical worsening) were simultaneously evaluated by means of EIT. It was possible to measure deltaZQ by producing ECG-gated EIT images. The impedance variation related to ventilation (deltaZV) and the deltaZV/deltaZQ ratio were also analyzed. Based on the results of RHC, patients were discriminated into 2 groups: PAH and those with normal hemodynamics, referred as normo-pressoric (NP) group. deltaZQ, deltaZVdeltaZQ, and deltaZQ corrected by the anthropometric parameter weight (deltaZQ*w) were compared between groups, correlated to hemodynamic parameters, and analyzed as predictors of mortality. RESULTS: After RHC, 35 patients composed the PAH group, and 8 patients, the NP one. PAH patients showed a significant reduction of deltaZQ as compared to NP, as well as an increase of deltaZV/deltaZQ. A significant correlation between deltaZQ and hemodynamic parameters was found, particularly with stroke volume (SV) (r = 0.66, R2 = 0.43; p < 0.001). Weight correction increased the correlation between deltaZQ and SV (r = 0.77, R2 = 0.59; p < 0.001). During the study period, 7 patients died; they presented a worse hemodynamic profile, reduction of deltaZQ compared to PAH survivors, and an even higher deltaZV/deltaZQ ratio. The overall survival at 36 months was 65%. Patients with deltaZQ*w < 154.6 Kg.% or deltaZV/deltaZQ >12 presented worse survival. CONCLUSIONS: deltaZQ, particularly corrected by weight, is associated with hemodynamic status of PAH patients and its reduction is associated with disease severity and worse prognosis

Testo completoPulmonary hypertension (PH) is a disease with many different etiologies contributing to an increased pressure in the pulmonary circulation. PH is defined as a mean arterial pressure in the pulmonary artery ≥ 25mm Hg at rest measured by right heart catheterization (RHC). The different causes of PH are divided into five groups. This study focuses on group I, which consists of pulmonary arterial hypertension (PAH). PAH contributes to a pressure overload of the right ventricle. The right ventricle must work at a higher pressure, which leads to a reduced right ventricular function. Currently, right ventricular stroke work index (RVSWI) by right heart catheterization (RHC) is a measure of right ventricular workload. The purpose of the study was to compare RVSWI calculated with echocardiography to RVSWI by RHC. And to find an additional measure for assessing right ventricular function by echocardiography in patients with PAH. RVSWI was evaluated with two echocardiographic methods (RVSWIEKO1= 90 - (0,62 x pulmonary acceleration time (PAT) x stroke volume index (SVIEKO)), RVSWIEKO2= ((0,61 x tricuspid regurgitant maximum pressure gradient (TRmaxPG)) + mean right atrial pressure (mRAP) + 2) x SVIEKO). Both the echocardiographic methods showed a statistically significant difference (p <0.001) in absolute values compared to RVSWI by RHC. One of the echocardiographic methods (RVSWI EKO2) showed a moderate correlation with RHC, while RVSWIEKO1 showed a negligible with RHC. The study has shown that RVSWI evaluated with echocardiography can be used as an additional measure, when assessing right ventricular function. However, more studies are needed until the parameter can be used clinically.

Testo completoCoronary artery disease is the first cause of death in our environment. Contrast angiography and coronariography was performed in more than 140.000 cases in Spain last year, in more than 70.000 cases an interventional coronary procedure was also done to treat coronary flow disturbances. The initial arterial access to the coronary ostia was historically through femoral or brachial access but recent material development and improvement in quality Rx image has helped to switch to radial artery access in more than 90% of the cases done in our hospital. Rapid and progressive adoption of radial access has been boosted in the medical community due to the better clinical results published in patients in whom radial access is used. The study that we have conducted to analyze the incidence of radial artery occlusion after transradial catheterization aimed to detect the influence of the compression method used to achieve homeostasis in artery occlusion and also studied the potential functional impairment of the patient’s hand affected. We know from previous studies that type of compression influences thrombosis and artery occlusion after catheterization. We also know that percutaneous closure devices contribute to faster patient mobilization but do not avoid arterial access typical complications. We designed a study with three different methods to explore radial artery permeability (Allen Clinical Test, Pulsioximetric Test and Eco Doppler exploration) at basal, 1 week after and 3 month after catheterization and the compression method being randomized to pneumatic force (Terumo®) or elastic compression (Tensoplast®) between all the patients treated consecutively in our center during a 18 month period time. We did a functional test of hand strength following two different protocols: one to test maintained strenght and the other to test Rapid Exchange repetitions with two interval specified tests. We analized 274 patients, 124 submitted finally to the elastic compression method Tensoplast® and 150 patients treated with a pneumatic compression method Terumo®. Despite there were not statistically significant differences in basal characteristics between both groups our study showed higher radial artery occlusion rate at one week in the Terumo ® group. This was consistently confirmed with the three different methods of radial artery occlusion used. The difference in the occlusion rate lost statistical significance at the analysis performed at 3 month due to a higher rate of recanalization of the radial artery occluded in the pneumatic group, being more than double of the one raised with the elastic method. Incidence of late occlusion detected only at 3 month period was equivalent in both groups while persistent occlusion was clearly higher in the Terumo® patients. Functional strength analysis with all the prespecified tests did not show any differences between methods of compression nor between patients that suffered radial occlusion against those whose radial was kept permeable. By the other hand anatomic analysis showed variation between arterial diameters related to radial occlusion: if these was present ulnar diameter was bigger at 1 week and also at 3 month time. Arterial blood flow increased at one week if radial was occluded specifically at its systolic phase, not at all the other possible situations. In summary pneumatic compression is associated with a higher incidence of radial artery occlusion at 1 week. This phenomenon has not functional impact in the patient’s hand strenght but an adaptive anatomic change is found in the contralateral vessel: the ulnar artery.
