Testo completoNon-adherence to oral chemotherapies can lead to lowered efficacy and increased risk of adverse events. The objective of this PhD work was twofold : i) to identify dis-adherence risk factors ii) to develop and test the feasibility of a validated, tailored therapeutic educational program with the aim of improving adherence to oral endocrine adjuvant chemotherapy in breast cancer. A survey was carried out to collect information on drug prescription, administration and surveillance, in order to identify non- adherence risk factors related to health professional behaviors : the majority of prescribers followed no standards in prescription writing, safety monitoring, toxicity prevention and patient education. A cohort study was carried out to identify adherence profiles among patients treated with capecitabine, using a mixed method. A profile of low adherence appeared (highly educated patients, with an irregular active life, with occupied relatives) and absolutely all patients showed an over-adherence profile (with a high risk of toxicity). The pilot study assessing the development and the feasibility of an educational program tailored to patients’ needs led to the improvement of the program : an extra session dealing with anxiety was built, and a new recruitment method was developed. Otherwise, the program succeeded in improving knowledge and trust in the treatment. This PhD work succeeded in identifying new dis-adherence risk factors, thanks to qualitative-quantitative methods. Those risk factors were incorporate in the development process of an educational program, in order to tailor it to the targeted population. This method should guarantee the efficacy of the program on patient’s adherence

Testo completoClinical manifestations of oral mucosal pathologies exhibit significant polymorphism. This includes benign dermatoses, cancerous or even pre-cancerous lesions. Early diagnosis remains a key element of management, especially for oral cavity cancers. However, this diagnosis is currently often delayed, whether due to the inherent difficulty in clinical presentation or insufficient availability of specialist consultations, leading to extended waiting times. Therefore, it appears useful to propose reliable diagnostic support tools for oral healthcare professionals. While artificial intelligence in dermatological diagnosis is advancing rapidly, its application to oral cavity pathology is still relatively limited. We adapted and evaluated various artificial intelligence approaches and developed a tool intended for healthcare professionals

Testo completoThe hypothesis of a hormonal dependence of cutaneous melanoma has been debated in the literature over past decades. Within this context, it is critical to increase our knowledge on the influence of exogenous hormone use on the risk of this cancer.The aim of this project was to explore the influence of hormonal treatments (oral contraceptives, fertility drugs, premenopausal use of progestogens, and menopause hormone therapy) on the risk of cutaneous melanoma in women.The project was mainly based on the large E3N (Étude épidémiologique auprès de femmes de l’Éducation Nationale) prospective cohort, which included about 100,000 women aged 40-65 years at inclusion in 1990. The cohort collected data on pigmentary phenotype, sun exposure, hormonal treatment use, and medical data including cutaneous melanomas, which were ascertained through pathology reports (761 incident cases between 1990 and 2011). Additional information on sun exposure, available for a portion of the E3N cohort, was used. These detailed data were collected as part of the E3N-SunExp case-control study launched in 2008.Analyses were extended to the EPIC consortium (European Prospective Investigation into Cancer and Nutrition), which includes about 334 500 women and 1,734 incident melanoma cases.The results arising from this project show modest associations between exogenous hormone use and melanoma risk and suggest no strong direct influence of hormones on melanoma development. However, results suggest intentional UV exposure behaviors in exogenous hormone users, which opens new perspectives for future investigations in this field

Testo completoThe transfer of chemotherapy from the hospital towards the home patient and the increase of oral anticancer drugs prescriptions has radically transformed the therapeutic pattern for cancer patients. It is unprecedented in the oncology field and it increase the patients’ own responsibility for the effectiveness of the protocol from the dispensing to the management of side effects. Being away from healthcare professionals and supportive care leads patients to also manage the psychological consequences of the illness. The first study is transversal with a comparative purpose between the representations of disease and treatments, self-efficacy and the quality of life of patients treated with oral cancer drugs and those treated with intravenous chemotherapy. Regressions analysis indicate the role of self-efficacy and representations as mediating variable in the relationship administration route – quality of life. The second study aim to develop and evaluate a therapeutic patient education. A preliminary feasibility study with test patients is carried out. Then, the second part of the study is exploratory. The aim is to show the immediate and/or long term effects of therapeutic education. The mixed methodology explores the psychological impact of the disease announcement and the initiation of the oral treatment, the educational patients needs and whether the are consistent with the recommendations of the literature. In addition, statistical analysis assess the evolution of disease and treatment representations, the self-efficacy and the quality of life for cancer patients. Finally, comparing the results of participants in therapeutic education with tose of a control group shows the effectiveness of the therapeutic education
