Abstract (sommario):

Alcohol use disorders are common and make a significant contribution to the burden of disease throughout the world. This is especially true among the younger age groups. Although these disorders are common, evidence suggests that those affected do not seek help for their disorders. In order to understand this, reviews of the treatment literature and the epidemiological data on prevalence and correlates of alcohol use disorders and treatment seeking are presented. These reviews confirm that effective treatments exist and that screening in primary care can be efficacious. The reviews also highlight deficits such as the need for more epidemiological evidence on the validity of DSM definitions of alcohol use disorders and for more Australian data on the prevalence and correlates of the disorders and related treatment seeking. This thesis sets out to address these deficits applying sophisticated statistical techniques to data from a large nationally representative Australian sample. A confirmatory factor analysis of the eleven criteria that specify alcohol dependence and abuse examined the validity of DSM-IV definitions of alcohol use disorders and the best solution was found to be a single factor, not two as currently defined. These findings question the bi-axial nature of alcohol use disorders that has underpinned their definition since the publication of DSM-III-R in 1987. Data from this national sample also confirm that, in line with research from other western countries, Australians have high levels of alcohol use disorders, especially amongst males and younger people. Also no association was found between alcohol dependence and treatment seeking, and young people were least likely to seek treatment. However, a relatively large proportion of young people who drink had been in contact with their GPs in the past year; demonstrating that there is ample opportunity for screening and referral for treatment for alcohol use disorders in this vulnerable group. This research has found that although alcohol disorders are not necessarily associated with disability, there are those who can benefit from treatment. It suggests that outcomes for such individuals may be improved by better specification of disorders as well as improved access to best treatments.

Sons of male alcoholics (SOMAs) are at increased risk for the development of alcoholism, and are apparently characterized by other abnormalities. It is possible that one or more of these abnormalities might serve as a marker for the alcoholic predisposition. Research described in this thesis, conducted in the hopes of identifying such a marker, was designed (1) to separate the relative pharmacological and psychological effects of acute alcohol intoxication upon neuropsychological functioning; (2) to investigate the neuropsychological function of SOMAs with a multigenerational family history of male alcoholism; (3) to examine the relationship between SOMAs' neuropsychological function and their cardiovascular hyper-reactivity to threat of and aversive stimuli; and (4) to investigate the relationship between a number of cardiovascular response patterns and voluntary weekly alcohol consumption. These studies are linked conceptually, within the context of a general theory of information-processing and action.

This study investigated demographic and personality variables, that counselors used in the selection of alcoholics for long term treatment and compared the completion rate for this group against completion rates reported in past studies, in order to determine if certain personality variables are associated with completion. The MMPI, Survey of Drinking Patterns and Effects, and a demographic questionnaire was administered to 355 lower class and "skid row" inpatient alcoholics from a North Tucson alcoholism treatment center. Results showed that counselors selection judgements approximated the characteristics of alcoholics who had completed long term treatment programs in past studies. Completion rates for this group studied were found to be moderately high compared to past studies. It was concluded that certain biographical and psychological variables can be used as selection criteria for determining alcoholics long term treatment completion. Implications for these findings are discussed.

Abstract (sommario):

A wealth of data supports the hypothesis that the neurotransmitter serotonin regulates the intake of ethanol, and is involved in the development of alcoholism in humans. Reduced functioning of the serotonergic system hypothetically increases alcohol intake in both animals and humans. In this thesis, it was proposed that the effect of lowered serotonergic function on alcohol intake is mediated by an increase in disinhibition. The hypothesis that lowered serotonin increases disinhibition was tested in separate groups of individuals at high risk for the development of psychopathology: nonalcoholic young men with a strong family history of paternal alcoholism, and adolescent men with previous histories of physically aggressive behavior. Lowered serotonergic synthesis (and thus presumably function) was experimentally induced through a transient dietary reduction in the availability of the amino add precursor of serotonin, tryptophan. Disinhibition was quantified using a go/no-go task previously shown to characterize psychopaths and children with attention deficit hyperactivity disorder as disinhibited. In the first study, acute tryptophan depletion had no effect on aggressive responding on a modified competitive reaction time aggression task, but increased disinhibition in young men at risk for alcoholism. This effect was independent of the tryptophan depletion-induced mood alterations. The effect tryptophan depletion on disinhibition was not replicated in the second study with previously aggressive adolescent men. A number of explanations for this were posited, including the presence of a ceiling effect. An association between disinhibition and executive functioning (cognitive abilities associated with proper functioning of the prefrontal cortex, such as working memory, planning abilities) was demonstrated in the second study. Whereas many studies of adult children of alcoholics (COAs) have utilized clinical subjects, subjects in this study (n = 229) were relatively well functioning college students. The Children ofAlcoholics Screening Test (CAST; Jones, 1983) was utilized to define parental alcoholism.Results of a multivariate analysis of variance indicated that COAs were more likely to exhibit symptoms related to poor psychological adjustment than were their non-COA peers. No significant psychological differences in male and female COAs were found. COAs scored significantly higher than non-COAs (p.< .01) on the Alienation scale of the Psychological Screening Inventory (PSI; Lanyon, 1978), indicating the greater similarity of COAs to those with psychiatric problems. Social Nonconformity, as a measure of psychopathic behavior, was higher for COAs than for non-COAs (p. < .001). COAs from lower income families demonstrated higher scores on the Discomfort scale (p < .01), which measures symptoms. Those subjects who reported parental greater symptomatology and anxiety than those who reported no parental depression (p. ( .001). Although parental divorce rates were higher for COAs (32.4%) as compared to non-COAs (10.9%), no significant association with any of the psychological variables was found for parental divorce.In comparison to non-COAs, both male and female COAs evidenced greater problem alcohol use (p < .001) measured by the Michigan Alcoholism Screening Test (MAST; Selzer, 1971). As demonstrated by a MAST score of 7 or more, 88.8% of male COAs and 30.5% of female COAs indicated serious alcohol-related problems. Parental depression was also significantly related to problem alcohol use in subjects. It was found that the MacAndrew scale (MAC; MacAndrew, 1965) was of little utility for identifying subjects with problem alcohol use.

In this study, we address two research questions: 1) Can we identify bear market episodes in Australia in the past 20 years? 2) How do investors’ moods change as stock market conditions enter into a bear phase. To address the first question, we use a pattern recognition algorithm, called the penalised LSE approach. By defining bear markets as those stock market regimes where the average returns are statistically significantly negative or below the risk free rate, we are able to detect two bear market periods in Australia in the past 20 years. These are the November 1987 to February 1988 and the April 2000 to May 2000 periods. To address the second question, we study the change in investors’ attitudes to varieties of systematic risk and the aggregate number and dollar value of shares traded in portfolios as a result of the regime switch from pre-bear to bear period. Out of the 7 categories of risk considered in this study, the transition from pre-bear to bear regime in both sample periods had a significant impact mainly on investors’ attitude toward the size risk factor. Investors systematically became more sensitive to firm size as stock market conditions entered into the 1987⁄1988 bear market. In the later sample period, investors’ reaction to firm size was more selective as it depended on the characteristics of the stocks that made up their portfolios. We also find that the regime switches resulted in lower portfolio trading volumes. Based on these results we infer that the November 1987-February 1988 bear market evoked a general sad mood, while the April 2000-May 2000 bear market stirred up both angry and sad feelings in market participants depending on the composition of stocks in their portfolios.

Much empirical evidence attests to a strong relationship between the panic-related disorders and alcoholism. Recent data suggest that anxiety sensitivity (fear of anxiety) may be one common underlying vailable contributing to the large degree of overlap between the panic-related disorders and alcoholism. In fact, some data indicate that the relationship between anxiety sensitivity and alcohol misuse may be particularly strong in women, a group which is generally underrepresented in the alcoholism etiology literature. Research described in this thesis was conducted with the aim of further elucidating the nature of the relationship between anxiety sensitivity and alcohol use/abuse in young adult women. The series of seven experiments included in this thesis demonstrated that: (1) high levels of anxiety sensitivity are characteristic of subjects meeting diagnostic criteria for panic disorder, an anxiety disorder frequently associated with alcohol abuse; (2) female university students demonstrate significantly higher average levels of anxiety sensitivity than male university students; (3) anxiety sensitivity is an important predictor of self-reported rates of alcohol consumption in university women; (4) high anxiety sensitive university students are both more likely to report drinking alcohol primarily to "cope" with negative emotional states, and less likely to report drinking alcohol primarily for social-affiliative motives, than are low anxiety sensitive university students; (5) high anxiety sensitive women display greater degrees of sober subjective-emotional arousal when anticipating aversive stimulation, greater degrees of sober electrodermal reactivity to the aversive stimulation, and greater sensitivity to the dampening effects of alcohol on these measures of reactivity, than low anxiety sensitive controls; (6) high anxiety sensitive women show a sober attentional bias favoring the processing of physically threatening information, which is dampened through th

This study is an examination of the relationships between alcoholics' and nonalcoholics' attributions and their emotions of anxiety and depression. The research design is an ex post facto/causal comparative analysis. Subjects were 150 white males. The treatment factor is separated into three levels: no treatment for alcoholism (50 nonalcoholics), two or three day treatment for alcoholism (50 alcoholics in detoxification units), and three to six months of treatment for alcoholism (50 alcoholics in a supportive living situation, halfway house, three-quarterway house). The Michigan Alcoholism Screening Test (MAST) was used to separate nonalcoholics from alcoholics. The variables measured for the three populations are: attributional style (as measured by the Attributional Style Questionnaire-revised for drinking practices) and emotional state (as measured by the Profile of Mood State).One-way MANOVA analysis was used to examine the data because there are three attributional areas measured by the ASQ (internality, stability, and globality) and two emotional areas measured by the POMS (tension-anxiety, depression-dejection). MANOVA analysis resulted in a significant F. The three groups were significantly different at the multivariate level in terms of attributional and emotional states.Discriminate analysis was used to determine if the groups were reliably different. Both synthetic variables analyzed resulted in significant results. The first synthetic variable was labeled "emotional stability," and the second was labeled "specific causal attribution."Univariate analysis showed: (1) no significant difference between groups on the internal/external dimension or the stable/unstable dimension, (2) significant difference at the .01 level on the global/specific dimension when comparing alcoholics in recovery and nonalcoholics, and (3) significant difference at the .01 level on anxiety and depression levels of each of the three group comparisons.Pearson Product Correlations were examined to understand the relationships between attributions and emotional states for alcoholics and nonalcoholics. The stable/unstable and global/specific dimensions are positively correlated with depression and the global/specific dimension is positively correlated with anxiety.All the hypotheses were supported:1. Alcoholics in detoxification make different attributions and have different mood states than alcoholics in recovery.2. Nonalcoholics make different attributions and have different mood states than alcoholics in treatment and in recovery.3. There are correlational relationships between the attributions and emotional states for alcoholics and nonalcoholics.Some alcoholic treatment recommendations were made.

Research into the comorbidity (or co-occurrence) of mental disorders is a relatively recent phenomenon. Much of this research has been conducted in clinical samples, which are prone to a range of biases that mean that the patterns observed are not representative of the general population. Although population-level research has previously been carried out, this thesis provides the first examination of comorbidity in the Australian population. This thesis examined the comorbidity of substance use and mental disorders among Australian adults. The major empirical work involved an examination of the patterns of homotypic comorbidity (other substance use disorders) and heterotypic comorbidity (mood disorders, anxiety disorders, and psychosis) of alcohol, tobacco and cannabis in the 1997 Australian National Survey of Mental Health and Well-Being (NSMHWB). These drugs were chosen as they are the most commonly used psychoactive substances in the Australian population. The NSMHWB involved a structured diagnostic interview of mental disorders with a representative sample of Australian adults. Three questions were addressed using this data: (1) What patterns of comorbidity exist between tobacco, alcohol and cannabis use, and other substance use and mental disorders?; (2) Are these patterns of comorbidity explained by common factors?; and (3) Does comorbidity affect the likelihood that mental health treatment has been sought? Similar patterns of homotypic comorbidity were observed for all three substances, and they were not explained by the other factors examined (gender, age, education, relationship status, employment and neuroticism). Cannabis dependence was the most strongly associated with other substance use disorders. Heterotypic comorbidity differed between alcohol, tobacco and cannabis use. Tobacco use predicted increased rates of all three groups of mental disorders (mood, anxiety and psychotic disorders). In the case of alcohol, only alcohol dependence was related to increased rates of all groups of mental disorders; alcohol use and abuse were not associated with heterotypic comorbidity. Any level of cannabis involvement was related to a similarly increased risk of mood and anxiety disorders. Cannabis use was linearly related to the risk of screening positively for psychosis. Common factors did not change the patterns of heterotypic comorbidity of tobacco and alcohol use. However, alcohol, tobacco and other drug use appeared to explain the higher rates of mood and anxiety disorders among cannabis users. Treatment seeking was much more likely among alcohol, tobacco and cannabis users when they had comorbid mental disorders. It was moderately increased when they had comorbid substance use disorders. The second piece of empirical work provided a more detailed examination of comorbid substance use problems among persons with psychosis. This topic was selected due to the limited epidemiological research on this issue, and the relatively large burden of disability that psychosis places upon the individual and the community. NSMHWB data were used to examine the prevalence of comorbid substance use disorders among persons who were likely to have met criteria for psychosis (as assessed by a screener used in the NSMHWB). Multiple regression analyses were used to test possible explanations for the higher rates of substance use disorders observed among persons reporting higher numbers of psychotic symptoms. The odds of alcohol dependence and regular tobacco use increased 1.5 times, and the odds of cannabis dependence increased twice, with each additional psychotic symptom reported, after adjusting for other substance use disorders, other mental disorders and demographic characteristics. Given the debate about the reasons for the association between cannabis use and psychosis, the final study used mathematical modelling to test four hypotheses about relationships between cannabis use and psychosis. Specifically, it examined trends in psychosis that would be predicted given the marked increases in the prevalence of cannabis use that have occurred in Australia over the past thirty years. The results suggested that a causal relationship - in which cannabis use caused psychosis among persons who would not otherwise have developed the disorder - is unlikely to explain the association. There was a better fit to the data provided by the other hypotheses examined, namely, that (a) cannabis use precipitates psychosis among vulnerable individuals; (b) cannabis use increases the risk of relapse among persons with psychosis; and (c) persons with psychosis are more likely to become regular cannabis users (without any effect upon the disorder). This thesis has demonstrated that in Australian adults there is significant comorbidity between alcohol, tobacco and cannabis use and other substance use and mental disorders. These patterns differ across the three substances. Some types of heterotypic comorbidity (e.g. between cannabis use and mood/anxiety disorders) are explained by common factors. The limited range of common factors tested here did not explain homotypic comorbidity. This thesis also suggested that mathematical modelling is a useful approach to consider when examining the plausibility of different relationships between risk factors and mental disorders. A number of hypotheses regarding comorbidity could not be tested using NSMHWB data, such as common genetic and other environmental factors. These can best be tested in research with samples of twins, and using longitudinal designs that assess a wide range of social and environmental factors. The findings of this thesis also have implications for treatment, because persons with comorbid disorders are more likely to seek treatment. There is an absence of validated treatments for persons with comorbid substance use and mental disorders, and more research is needed on this issue.

The influence of a primarily Public form of self-awareness and of Private and Public Self-Consciousness on drunken physical aggression and complex reaction-time were examined. Two forms of the balanced-placebo design were employed. Results indicated that each form of self-focus played a significant role in the determination of various aspects of drunken comportment. Consumption of alcohol did not eliminate self-aware behavior on the measures employed. Public Self-Consciousness acted to increase drunken impairment. A modified form of the balanced-placebo design proved superior to the standard version for use with moderately high doses (1.32 ml/kg) of alcohol on a measure of subjective intoxication. The implications for the literature on self-focus and drunken comportment are discussed.

This thesis encompasses studies that examine autonomic reactivity and neuropsychological function in preadolescent boys who are at-risk for developing alcoholism and conduct disorder. The literature suggests an association between cognitive impairments and behavioural undercontrol, and that autonomic hyperreactivity may facilitate the use of alcohol to dampen stress. Thus, either factor may contribute to early alcohol use. In the first study, preadolescent sons of male multigenerational alcoholics were found to be cardiovascularly reactive during cognitive stress, and impaired on tests of executive function. Furthermore, reactivity was correlated with anxiety, and executive function deficits with conduct problems. In the second study, reactivity during a laboratory aggression task was monitored and cognitive functions (attentional or memory processes and executive functions) were assessed among aggressive boys rated as disruptive, or anxious-disruptive. During the aggression task, anxious-disruptive boys, unlike disruptive boys or controls, moderated their aggression when their opponent engaged in retaliatory behaviour. Anxious-disruptive boys were more highly aroused throughout the task, and the neuropsychological assessment found they were impaired on tests of executive function, independent of attention and memory. In the third study, anxious-disruptive boys exhibited greater cardiovascular, electrodermal, and muscle tension reactivity than disruptive or control boys during cognitive stress, while disruptive boys were electrodermally underaroused. These studies suggest it is possible to delineate specific neuropsychological profiles among at-risk youth, using either a behavioural genetics model, or selecting for personality traits. There were consistent autonomic reactivity patterns across tasks among groups defined by similar behavioural profiles. Police culture often valorises 'hard' drinking, and in NSW police label their heavy drinkers 'heroes'. It is queried if there is some relationship between occupational culture and drinking style.It is found that much of the current theorising about the origins and nature of problem drinking, such as psychological theorising about stress, is inadequate to explain and address the extraordinary level of high-risk drinking among police.This thesis explores alternative views such as critical and feminist perspectives on police culture, constructions of masculinity, and mechanisms of 'enabling', to discover whether these might prove more applicable and more productive. The research also explores the matter of whether a case can be made for taking alternative ideas and theories into account in designing intervention programmes for specific occupation contexts, and whether they raise any policy and practical implications for addressing problem drinking within the NSW Police Service.

The present study examined cross-sectional and longitudinal developmental characteristics of the association between mental health and substance use. N=1182 adolescents aged between 11 and 20 years were recruited from schools in Australia. Participants completed the Youth Self Report (YSR) (Achenbach, 1991a) and reported on their use of alcohol, tobacco, and marijuana, their social ability, their motivation for using substances, their ability to regulate emotion, and the influences of their parents and peers on their substance use. N=561 of time one participants were retained for testing one year later to assess the potential to predict substance use at time two from mental health at time one. Results indicated a clear and consistent cross-sectional association between externalising scores on the YSR and use of the three substances, regardless of gender. The relation between internalising scores and substance use was non-significant. When the relation between externalising scores and substance use was assessed for mediation, only the influence of parents and peers was found to be significant. A cross-sectional structural model developed to account for this association was demonstrated to be invariant across the three substances of interest, and across gender, but not age category. There was no evidence that social skills, emotion regulation, or substance use motives, had roles in mediating the relationship between mental health and substance use. A model was developed to assess the potential to predict substance use at time two from externalising scores at time one. Results showed that externalising scores predicted increases in alcohol use via parent and peer attitudes. Thus, externalising disposition, in the context of a facilitative social environment, was predictive of an increase in alcohol use over time. Structural models developed to account for the predictive relation between externalising scores and use of cigarettes and marijuana proved unstable and could not be tested. Substance use at time one was not predictive of externalising scores at time two. The results of the present research are discussed in relation to their potential to inform the developmental substance use literature, and efforts directed against the development of substance use problems. Limitations of the present research are noted.

ENGLISH ABSTRACT: In the South African context, research on the impact of alcohol rehabilitation is particularly limited. This study aimed at describing the impact of the alcohol rehabilitation programme offered at Neuro Clinic D, Stikland Hospital. Outcome was assessed at 12-15 months with the objective of exploring the perceptions and experiences of the ex-patients to ascertain whether there has been an improvement in psychological well-being and positive lifestyle changes. The study was motivated by a request made by the staff at Neuro Clinic D; as such an investigation has never been conducted. Both quantitative and qualitative methodologies were used to gather data from 44 out of the 166 ex-patients admitted to Neuro Clinic D between January and June 2002. The relationships between the rate of relapse and age, gender, marital status, employment status, previous rehabilitation, health problems, police contact, aftercare attendance, participation in the 3- or 4- week programme and other medication/substance abuse were explored. While no statistically significant relationships were found between any of the variables, some evidence of interaction emerged with regards to the relationships between relapse status and participation in the 3- or 4-week programme, aftercare attendance, employment and marital status. The qualitative analysis revealed four central themes, namely coping mechanisms; responses to specific components of the programme; confounding factors and the impact of the programme. An integration of the quantitative and qualitative data supports the conclusion that while there was a low rate of consistent abstinence, the respondents reported an overall post-treatment improvement in psychological well-being and positive lifestyle changes.
AFRIKAANSE OPSOMMING: Navorsing op die impak van alcohol rehabilitasie is besonder beperk in die Suid-Afrikaanse konteks. Hierdie studie het ten doelom die impak van die alcohol rehabilitasie program, gebied deur Neuro Kliniek D, Stikland Hospitaal, te beskryf. Die uitkoms is bepaal na 12-15 maande deur die persepsies en envaringe van eks-pasiente te ondersoek om sodoende vas te stelof daar verbetering was in sielkundige welstand en of positiewe leefstyle veranderinge aangegaan is. Die betrokke studie is gemotiveer deur 'n versoek gerig deur die personeel van Neuro Kliniek D, aangesien so 'n ondersoek nog nooit vantevore gedoen is nie. Beide kwantitatiewe en kwalitatiewe metodologieë is gebruik om data in te samel van 44 die 166 eks-pasiente toegelaat tot Neuro Kliniek D tussen Januarie en Junie 2002. Die korrelasie/verhouding tussen die terugvalkoers en ouderdom, geslag, huwelikstatus, werkstatus, vonge rehabilitasie, gesondheidsprobleme, polisie kontak, nasorg bywoning, deelname in die 3- of 4-week program en ander medikasie/substans misbruik is ondersoek. Terwyl geen statisties beduidende verhoudinge gevind is tussen enige van die veranderlikes nie, is daar wel aanduidings van interaksie met betrekking tot die verhouding tussen terugvalstatus en deelname aan die 3- of 4- week program, nasorg bywoning, werkstatus en huwelikstatus. Die kwalitatiewe analise het 4 sentrale temas onthul, naamlik, hanteringsmeganismes; response tot spesifieke komponente van die program; verydelende faktore en die impak van die program. 'n Integrasie van die kwalitatiewe en kwantitatiewe data steun die gevolgtrekking dat alhoewel daar 'n lae voorkoms was van volgehoue onthouding, die respondente wel 'n algemene verbetering in sielkundige welstand en postiewe leefstyle veranderinge na behandeling gerapporteer het.

This study explores the health beliefs and preventive health behaviours of older Hong Kong Chinese people resident in Australia. Participation in physical activity was used as the case study. There were two frameworks used to shape the research. Because of their perceived influence on the health beliefs and practices of Hong Kong Chinese people, the traditional Chinese philosophies of Confucianism, Taoism, and Buddhism provided the philosophical framework. The Theory of Planned Behaviour provided a theoretical framework for understanding the target group's preventive health behaviour. Data was collected by means of in-depth interviews, participant observation and case study. Twenty-two informants were interviewed, their transcripts analysed, summarised and typologised, identifying six states of physical activity participation. Findings demonstrated that this target group possessed a holistic view of health, with food taking a special role in preventive care and self-treatment at times of illness. The Cantonese terms used to denote “physical activity” caused confusion among the target group. Most interpreted it as meaning deliberate planned body movement, strength-enhancing activities or exercise, although some did see it as including mundane daily activities and chores. Lack of time, no interest and laziness were reported as the main reasons for low participation in deliberate planned physical activity. Cultural, social and environmental determinants were the intrinsic and extrinsic factors influencing attitudes toward physical activity, as well as perceived social supports and perceived control over physical activity participation barriers. To a large extent, these interactive determinants of health were rooted in the three traditional Chinese philosophies mentioned above. The thesis concludes by arguing that rather than simply advocating activities designed for other populations, health promotion strategies and education need to create links to the traditions of this target group and also clarify their conception of physical activity.

The present study assessed 351 undergraduate subjects on dimensions of parental alcohol abuse, childhood family environment, childhood social support, current coping style, current negative life experiences, current psychological symptomatology, and current indices of drug and alcohol abuse. Results indicated that, while paternal alcohol abuse, in and of itself, was not related to any of the outcome measures, maternal alcoholism was correlated with one measure of alcohol abuse and three measures of drug abuse, and was independently predictive of a portion of the variance of one alcohol abuse measure among subjects. Early environmental factors (family environment and low levels of social support), as well as current coping style and current negative life experiences, were found to be related to psychological symptomatology and to indices of drug and alcohol abuse. coping style was also found to be related to early family environment and to childhood social support, suggesting that coping style may have its developmental roots in early environmental factors. An interaction between paternal alcohol abuse and disengagement coping style was shown to be predictive of a portion of the variance of subjects' alcohol abuse. Interactions between maternal alcoholism and low level of social support were predictive of a portion of the variance of subjects' drug abuse.

This is a study of workplace bullying policy in the public service. The research draws on interviews with policy actors from three groups located in four Australian states and one Australian territory. The groups are senior managers, policy implementors and employee advocates. The study is also informed by research and popular literature to examine how assumptions about what the problem is in workplace bullying dictates the direction taken in policy development. Unlike much of the research into workplace bullying that is based on psychological theorisations, this study is influenced by scholars who focus on the power imbalances that underpin workplace bullying. The key argument in this thesis is that the conceptual dominance of 'gender neutrality' operates to mask the gendered power imbalances which perpetuate bullying behaviour. Hence, to start to address workplace bullying, the effects of power must be acknowledged and addressed in the organisational policy responses to the growing phenomenon of workplace bullying. However, analysing the effects of power is insufficient if gender is not made visible in the analysis. The methodological touchstone for this is Carol Bacchi's 'whats the problem' approach (1999), which is taken further through feminist organisational theory, post modernist understandings of power realtions and a critique of New Public Management practices. The thesis shows how workplace bullying policies in Australian public service administrations have been carefully crafted as gender-neutral, and interweaves data and literature to develop a thesis for why such an approach is a deeply flawed outcome of gender politics. This thesis concludes with some modest suggestions about how organizations might more effectively develop more effective gender-sensitive approaches to workplace bullying.

The aim of this study was to explore and describe the intergenerational interactional patterns in a Coloured family where the mother has abused alcohol. Researchers have found that drinking behaviours of parents can have major effects on the children’s lives throughout generations. Substance abuse of a parent has major influences on the family, their interactions, and relationships, mostly between the parent and the child. There is a major gap in the study of the Coloured families and how substance abuse affects their families and children. The study employed Bowen’s constructs of differentiation of self, multigenerational transmission process, triangulation, emotional cut-off, nuclear family emotional system, sibling position, family projection process, and societal regression and utilized analytical generalization of the concepts to achieve its aim. The mother, an older sibling and maternal mother were the main sources of data although the perspective of the mother herself was privileged in the study. Semi-structured interviews were utilized to gather the data. This allowed participants to freely narrate their personal perceptions and experiences of interaction in both the family of origin and the current nuclear family. The research used a single case study of a purposively sampled family. The researcher is a Coloured female who had specific interest in this specific cultural group and their interactional patterns, which optimised cultural familiarity during the research process and reduced the likelihood of potential discriminatory racial bias of the participants. The findings demonstrate that perceptions of interactional patterns in the nuclear family and family of origin coincided with certain of Bowen’s Family Systems concepts, particularly, triangulation and differentiation. However, the need for further exploration of concepts such as the nuclear family emotional system and the family projection process in Coloured families are still needed in future studies. The findings provided insight into the functioning of the relationships and FAMILY INTERACTION IN MATERNAL ALCOHOL ABUSE interactions in both the nuclear family and the family of origin. Limitations of the current study are identified and recommendations for future studies in this field are also offered.

[Truncated abstract] Background: A review of the literature indicated there was a need for more long-term randomised controlled studies on the effects of BRCA counselling/testing on high risk women, including improved strategies for risk communication. Reviews have also shown women are confused about the significance of inconclusive or non informative results with a need for more research in this area. Aims: The general aim of this study was to evaluate the impact of breast cancer genetic counselling on psychological distress levels, perception of risk, genetic knowledge and understanding of BRCA testing/test results in a cohort of 207 women from high risk breast cancer families who were referred for genetic counselling in Perth during the period 1997 to 2001. Short- and long-term impact of BRCA genetic counselling/testing was determined in women with and without cancer in a randomised controlled trial as part of which women were randomised to either receive immediate versus delayed genetic counselling. This included family communication patterns before BRCA testing, anticipated outcomes of testing on oneself and family including intentions for result disclosure. Comprehension of index and predictive BRCA testing with possible results was assessed both in the short- and the long-term and understanding of individual or family BRCA test results was evaluated at long-term. The effect of genetic counselling on breast cancer risk perception in unaffected women was evaluated. This study considered a theoretical framework of educational learning theories to provide a basis for risk communication with possible relevance for future research. ... Only 25% of the original study population (52/207) reported BRCA results and women's understanding of results is concerning. Key findings were: 1. The majority of affected women received an inconclusive result. 2. Out of twelve unaffected women who reported results, seven were inconclusive which are not congruent with predictive testing. This implies that these women did not understand their test result. 3. A minority of untested relatives did not know whether a family mutation had or had not been found in their tested family member or what their actual test result was. This implies either a lack of disclosure or that woman did not understand the rationale for and significance of testing for a family mutation. 4. Three relatives did not understand a positive result was a mutation. Conclusion: The implication of this research for breast cancer counselling and testing services is that women who wait for counselling are no worse off in terms of short- or long-term general psychological distress than women who receive the intervention early. There is a suggestion that unaffected women without the disease found counselling more advantageous than affected women. The meaning of BRCA results as reported by women is concerning particularly women's understanding of negative and inconclusive results and further research is needed in this area. Too much information presented at counselling may affect women's comprehension of risk, BRCA testing and future test results and further research is required to evaluate the effects of information overload.

Australia is experiencing widespread ecosystem degradation, including dryland salinity, erosion and vegetation loss. Approximately 1 million hectares (5.5%) of the south-west agricultural zone of Western Australia is affected by dryland salinity and is predicted to rise to 5.4 million hectares by 2050. Such degradation is associated with many environmental outcomes that may impact on human health, including a decrease in primary productivity, an increase in the number of invasive species, a decrease in the number of large trees, overall decrease in biodiversity, and an increase in dust production. The resulting degradation affects not only farm production but also farm values. This study examines the effects of such severe and widespread environmental degradation on the physical and mental health of residents. Western Australia has an extensive medical record database which links individual health records for all hospital admissions, cancer cases, births and deaths. For the 15 diseases examined in this project, the study area of the south west of Western Australia (excluding the capital city of Perth) contained 1,570,985 morbidity records and 27,627 mortality records for the 15 diseases examined in a population of approximately 460,000. Environmental data were obtained from the Western Australian Department of Agriculture?s soil and landscape mapping database. A spatial Bayesian framework was used to examine associations between these disease and environmental variables. The Bayesian model detected the confounding variables of socio-economic status and proportion of the population identified as Aboriginal or Torres Strait Islander. With the inclusion of these confounders in the model, associations were found between environmental degradation (including dryland salinity) and several diseases with known environmentally-mediated triggers, including asthma, ischaemic heart disease, suicide and depression. However, once records of individuals who had been diagnosed with coexistent depression were removed from the analysis, the effect of dryland salinity was no longer statistically detectable for asthma, ischaemic heart disease or suicide, although the effects of socio-economic status and size of the Aboriginal population remained. The spatial component of this study showed an association between land degradation and human health. These results indicated that such processes are driving the degree of psychological ill-health in these populations, although it remains uncertain whether this 4 is secondary to overall coexisting rural poverty or some other environmental mechanism. To further investigate this complex issue an instrument designed to measure mental health problems in rural communities was developed. Components of the survey included possible triggers for mental health, including environmental factors. The interview was administered in a pilot study through a telephone survey of a small number of farmers in South-Western Australia. Using logistic regression a significant association between the mental health of male farmers and dryland salinity was detected. However, the sample size of the survey was too small to detect any statistically significant associations between dryland salinity and the mental health of women. The results of this study indicate that dryland salinity, as with other examples of ecosystem degradation, is associated with an increased burden of human disease.

Children in upper primary school who have not made progress along the Swimming and Water Safety Continuum may be at a greater risk in an aquatic environment because they have not developed the swimming competency, endurance and skills needed for survival in threatening aquatic situations. Three groups representing different socio-economic and geographical areas were selected to explore the relationships between psycho-social factors and the development of swimming ability among year seven students. Two groups from lower socio-economic areas were chosen. The first group was directly on the coast with easy access to the beach, while the second group was inland in the foothills of Perth. As higher socio-economic areas in Perth are generally not far from the coast only one group from a higher socio-economic coastal area was chosen. The participants were 540 year seven students, 282 of whom were males and 258 of whom were females. The primary variable of interest was the current swimming stage of year seven participants, and the differences between genders and/or locality groups. The primary research questions investigated differences between locality groups and/or genders for; a) perceived athletic competence and global self-worth, b) perceived swimming competency, confidence in deep water and importance placed on learning to swim well. c) perceived social support for sport and swimming activities and d) attendance at Interm, Vacswim and other swimming programs and aquatic venue experience. Relationships between swimming stage and the above variables were analysed. The secondary research questions investigated the most frequent reasons given by the students for not attending or discontinuing participation inVacswim, and whether there were differences between locality groups or genders. Findings indicated that the lower socio-economic groups had a significantly lower swimming stage and lower perceived self-worth than the higher socioeconomic group. Students from the lower socio-economic inland area had the lowest mean swimming stage as well as lower perceived social support for sport and swimming than either of the other two groups in the study. The two lower socioeconomic groups also attended less swimming instruction and placed less emphasis on the importance of learning to swim well than the higher socio-economic group. Despite these findings, the lower socio-economic groups did not view themselves as any less able in terms of athletic and swimming competence. However, as the two lower socio-economic groups have not progressed along the Swimming and Water Safety Continuum to the 'desirable standards' of the RLSSA (1999), these groups could be viewed as at-risk in an aquatic environment. At the same time, there was evidence that attendance at Interm along with attendance at another swimming program enabled participants to reach the 'desirable standards' of the RLSSA (1999). Girls generally had a higher swimming stage than boys in the lower socio-economic areas, attended year seven Interm and Vacswim more than boys, reported more social support for sport and swimming, and placed more emphasis on the importance of learning to swim well than boys. While many of these relationships between swimming stage and psycho-social factors have been intuitively accepted within the swimming teaching industry, we now have a better understanding of the strength and direction of these relationships.

Where We Belong is more than a memoir. It is a love story about the untimely death of the oldest of five daughters born to a prominent New Haven, Connecticut family. It is also a tale of hubris, rage and frustration, a Greek tragedy about a man's life as re-examined through the lens of the two weeks his wife spent dying, a tale in which chronic illness and good intentions ensure the death of a loving wife, artist and mother. The journey on which her husband takes the reader explores a health care system oblivious to her plight, her family's unwitting complicity and a 12-step mythology that unfolds while he, her six weeping children and her aging mother helplessly look on. The author endures an agony that dwarfs incentives to lie, learning that people lie out of fear, and genuine grief supplants fear with the stark reality of what we fear most: death. Where We Belong gives voice to the internal dialogue the author encounters when reexamining not just memories, but the accoutrements of memory, as well. It is a voice that addresses his own grandiosity, sentimentalism and self-pity in the face of his wife's death, in addition to those details, circumstances and impressions that speak to the arrogance he brought to the task of being all he thought she and her six children needed him to be. He concludes the task was well beyond him, a realization evoked by the gut wrenching decision to literally "pull the plug" on this heartbreaking tale of reconstituted hope and great promise reduced to rubble by chronic illness, alcoholism, drug addiction and death. Born is the lesson that when we grieve, we are free to be ourselves. When we are free to be ourselves, we are free to love again.

The notion of 'Care' has occupied a central place as foundation in nursing theory, ethics and practice. An examination of critical and authoritative documents reveals that the term is vague and ill defined. The literature on care does not theorise the concept as fully as expected in a profession where the concept is considered central. Moreover, even the best theories of care neglect justice. Applying Wittgenstein's concept of family resemblance allows the debate to move on from definitional issues to assess the applicability of 'care' to the practice of the profession. An examination of a range of assessments of nursing practice shows that although 'care' is frequently used in connection with nursing, it does not appear to be a major consideration in key areas of appraisal. This thesis explores the relationship between care and justice in nursing and proposes that they be integrated within nursing theory. A theory for nursing that does not exclude care, but that has justice as its basis is proposed. Care, when used in the context of justice provides nursing with a theory that is appropriate not only for nursing theory and ethics but also assists the profession meet the demands of modern practice.

The Western Australian (WA) mining industry directly employs approximately 56,000 people. Almost half work Fly-in/Fly-out commute arrangements (FIFO, e.g. employees living in a city are flown to a remote worksite where they live and work during their work roster) and approximately half work more than 50 hours per week, on average. There are many anecdotal claims that FIFO has negative impacts on WA mining employees, leading to an elevated risk of high stress levels, depression, binge drinking, recreational drug use and relationship break-ups. Previous studies found FIFO can be stressful, and have negative impacts on WA employees 'and partners' lifestyles and relationships. This project investigated the long-term (Study One) and short-term (Study Two) impacts of FIFO and extended working hours on a representative sample of WA FIFO mining employees and partners. In Study One, a total of 222 FIFO and Daily Commute (DC) mining employees and partners completed an anonymous questionnaire investigating long-term impacts on work satisfaction, lifestyle, relationships and health. A subgroup of 32 Study One FIFO employees and partners also participated in Study Two; a detailed study of the short-term impacts of FIFO and extended working hours and how these impacts fluctuate in intensity during the mining roster. Study Two participants completed a diary and provided saliva samples each day throughout a complete mining roster. The main findings of the study were that FIFO and extended working hours had negative impacts on employees work satisfaction and FIFO was frequently reported to be disruptive to employees 'and partners' lifestyle, in the long-term. However, FIFO and extended working hours did not lead to poor quality relationships, high stress levels or poor health, on average in the long-term; there were generally no significant differences in these characteristics between FIFO and DC employees, or between the FIFO sample and the wider community. There were minor differences between FIFO and DC employees in long-term health characteristics, and Study One employees had similar, or in some cases poorer health outcomes than other community samples.

Nous proposons que c’est le manque d’individualisation qui fait défaut aux méthodes actuelles destinées à traiter l’alcoolo-dépendance qui ne s’avèrent que modérément efficaces, si l’on en juge par la proportion importante de rechutes sous traitement. Les théories contemporaines dominantes postulent que les addictions et leur maintien pourraient être expliqués par le déséquilibre entre deux grands systèmes neuraux et les mécanismes cognitifs qui y sont associés: un système impulsif, dépendant des régions méso-cortico-limbiques, à la base des mécanismes de récompense, de renforcement et de la formation d’habitudes, et un système réflexif, dépendant du cortex préfrontal, indispensable aux comportements de prise de décision, à l’anticipation des conséquences des comportements et au contrôle inhibiteur. Nous avons dans ce travail développé des outils électrophysiologiques de diagnostic de deux troubles cognitifs clés associés au dysfonctionnement de ces deux systèmes :les biais attentionnels et les troubles de l’inhibition. Nous avons pré-testé ces tâches combinées à l’enregistrement des potentiels évoqués dans des populations d’étudiants binge drinkers et de gros consommateurs d’alcool. Nous avons ensuite testé leur valeur prédictive de la rechute dans une population de patients dépendants en fin de cure de désintoxication. Nous pensons avoir dégagé l’existence de deux marqueurs neurophysiologiques pouvant prédire à trois mois la rechute ou l’abstinence du patient alcoolo-dépendant :il s’agit (1) d’un facteur de rechute, indexé par une P3d plus ample suggérant un besoin accru de ressources neurales pour inhiber correctement un comportement, et (2) d’un facteur de protection, indexé par une composante P3 moins ample en réponses aux stimulations liées à l’alcool, suggérant qu’un investissement motivationnel moindre des stimuli « alcool » peut protéger d’une rechute. La réplication et la confirmation de nos résultats ainsi que l’amélioration de nos outils pourraient mener à l’utilisation des marqueurs mis en évidence en pratique clinique afin d’orienter de façon personnalisée la prise en charge des patients.

The use of quality-of-life as an outcome measure provides detailed information about the effectiveness of medical treatments than morbidity or mortality rates alone. The use of quality-of-life data in the clinical setting can inform patients regarding treatment options, treatment benefits and costs. In competing health care markets, outcome measurement is regarded as important as it is concerned with the impact of health care practice and affects health policy decisions. Doessel (1978) conducted the first Australian study on the cost-effectiveness analysis of renal replacement therapies. The study was based on Klarman, Francis & Rosenthal's (1968) the study, where the output was measured in terms of the number of life years gained from kidney transplantation, and a twenty-five percent weight was allocated in an attempt to capture quality-of-life from kidney transplantation. Doessel (1978) used two sources of data: Australian data (Disney 1974) and European data (Gurland et al. 1973; Shiel et al. 1974). The study measured life years gained, and agreed with the Klarman et al. (1974) findings that transplantation is the most effective way to increase life expectancy of persons with chronic renal disease (Butler & Doessel 1989). The outputs of the alternative treatments were not reported in monetary terms; the study focused on life years gained as the output measure. Hence the importance of this current study, which includes a cost-effectiveness analysis for cadaver liver, and living kidney transplantation for end-stage liver and kidney disease patients. Calls to respect patient autonomy and to produce patient-centered outcomes have recently brought the patients point of view back into the center of clinical medicine (Sullivan 2003). Survival rates indicate one measure of outcome however they do not reflect patients perceptions of health benefit or experiences. Noting that patients psychosocial effect on functioning is of more concern to them than their physical Thesis Preamble iii ability, that more accurate knowledge of patients conditions be measured prior to transplantation (Tarter et al. 1991). Recently researchers advocated investigating transplant patients' states of health to assess the social benefit of these expensive health care services from their perspective (Joralemon & Fujinaga 1997). The current study's mixed method, bridges the gaps in treatment outcome measurements, as the mixed method applied (Creswell 1994; Sim & Sharp 1998) prospectively measured quality-oflife, determined health utility, quality-adjusted life years (QALYs) and incremental cost-effectiveness ratios (ICERs). The study reported the living donors experience of the donation process, described their needs; expressed using a new psychosocial model supporting future living kidney donor's during the donation process.

This study attempted to identifj and explore the role the building plays in the organisation culture of nursing homes. To do this a research plan was formulated in which the central plank was a case-study of a seventy-five bed high care nursing home. As part of the case-study, interviews were conducted at the nursing home with ten members of staff, two residents and a daughter of a resident. The study was also informed by interviews with two architects, who specialise in the design of nursing homes and aged care facilities. A theoretical model entitled the 'Conceptual Framework' was developed prior to the case-study. It was tested by applying it to findings related to the physical context and the organisation culture of the case-study venue. The hypothesis that the building does influence the culture of the nursing home environment was explored by studying the manner in which the building influenced the lives of those who work in the nursing home and those who live there. This challenge was met with the use of theoretical contributions from organisation theory and psychodynamics, which together provided a vehicle for analysis of the culture and the building's role in it.

The teaching and learning field has been renowned for being a rapidly changing and multifaceted environment. Moreover, being both intensely personal and public, the process of cultivating, discovering and relaying knowledge has also been influenced from a wide range of participating individuals to the broader groups in society. Such numerous possibilities for interaction have highlighted the difficulty of defining ‘good’ teaching and learning, especially considering the growing objectivism of modern day value systems. An increasing number of educators have thereby responded to this confusion by returning to more fundamental and holistic views of ‘knowing’ the ‘other.’ Such rising concerns for holistic teaching and learning practices represent many exciting possibilities for developments towards authenticity and autonomy, as teachers become responsible explorers of their profession. The current study is an autoethnography of my own teaching experiences at a small Korean Christian University. It captures my desires to develop greater sensitivity and empathy as a critical teacher practitioner, and further documents efforts to acquire aesthetic and creative skills as a writer. Ultimately, through my experiences as a teacher researcher, I have sought to develop a deeper picture of the knowing process as a rich and mutual dialogue between the 'knower' and the 'other.’ To do this, I have constructed eight stories based on my teaching experiences. The first describes the reflections accompanying my experiences of writing, whilst the next three involve narrative portrayals of certain striking colleagues and students. The following two stories convey the ‘faith’ and ‘acceptance’ experienced through the study, and the last two act as a form of reflective closure to the overall teaching and researching experience Since I believed that the symbolic and holistic nature of story writing could convey the depth, complexity and open-endedness of the knowing process, I have chosen narratives and reflective writing to capture and depict my experiences (Van Manen, 1997). Interviews and journals writing of my students and my colleagues have also been included to further explore these ideas. Accordingly, this current study seeks to portray a view of 'knowing' that enables teachers and students to become co-researchers, who can cultivate sensitivity, creativity and empathy towards the 'other.’

Mental health reflects an array of causal influences that span biological, psychological, and social circumstances, with resultant underlying causal pathways to poor mental health outcomes in childhood that are complex. Key features of this complexity are reciprocal interactions between person and environment that take place over time. The core of this thesis seeks to attend to the complexity of development to move the field of developmental health forward toward greater explanation, and more successful prediction and prevention. The focal point of the thesis is the psychosocial determinants of childhood mental health, the resource domain of the developing child, and the interplay between characteristics of the individual child, the family, and the community. The eventual goal is to better understand why and how socioeconomic circumstances impact on developmental health. One component of this thesis focuses on the expansion of extant developmental theory. The other component focuses on the development of an analytic strategy that more appropriately reflects the intricacies of this theoretical expansion. In the process, data are analysed, principally as a heuristic strategy, to illustrate the analytical approach needed to support the theoretical framework. The specification of a bioecological conceptual framework suitable to guide research and policy in developmental health is the first principal objective of the thesis. A critical examination of the resource framework proposed by Brooks-Gunn, Brown, Duncan, and Anderson Moore (1995) reveals it to be centred on family and community resources, but otherwise silent with respect to the physical and psychological resources of the child. The quintessential point of this thesis is that theory in developmental health must be able to account for the contribution individuals make to their own development. A modified resource framework is proposed that acknowledges financial, physical, human, and social capital, within the domains of the individual child, the family, and the community. The second principal objective of the thesis, the development of analytical methods that focus on the individual child and the complexity of data generated by this theoretical approach, is then introduced. Theory and method are thus integrated when comprehensive measures of characteristics in multiple domains across developmental periods are modeled using longitudinal data from the Western Australian Pregnancy Cohort (Raine) Study (Newnham, Evans, Michael, Stanley, & Landau, 1993). The mothers of 2,860 children were enrolled at 18 weeks in pregnancy and the children have been followed at birth, one, two, three, five, and eight years of age. Eighty-nine per cent (2,537 /2,860) of families were available for follow-up at eight and 74 per cent (2,126/2,860) of families responded. Extensive demographic, psychological, and developmental data were available for the children and their families and a limited amount of data were available for the communities in which they reside. A measure of mental health morbidity, the Child Behaviour Checklist (Achenbach, 1991), was available for the children at two, five, and eight years of age. In the first instance, dichotomous summary variables are derived for the demographic, psychological, and developmental variables of interest. Variables are then selected for inclusion in one of several explanatory models. To create a mathematical representation of resource characteristics, the information for each child is concatenated as a series of binary strings. Frequency tabulation is then used to aggregate the data and odds ratios are calculated to determine the degree of risk associated with each string of code, or pattern of factors relative to a nominated mental health outcome. The results provided a scaffold from which this theoretical and analytical approach is compared and contrasted with the reviewed literature. Two principal themes of investigation are pursued. The first theme to be examined is the interplay between characteristics of the child, family, and community and the contribution children make to their own development. The specific approach models the interaction between selected characteristics of the child, family and community in each of four developmentally significant time periods. The theoretical position adopted in the present study suggests that the effect of any personal or contextual factor on later development, if a relationship does truly exist, is most likely to be differential. That is, it is a combination of influences that determines developmental outcomes for children, not any single factor acting independently. The modelling process demonstrates that, for the children involved, personal and contextual factors impact mental health differentially depending on various other individual, family and/or community characteristics. The modelling process identifies patterns of factors that impact relatively small, but significant, numbers of children because the models focus on the effect for individual children rather than the effect for the group. For example, one model suggests that the effect of intra-uterine growth restriction for the group as a whole may be minimal, but the impact for some children could be critical depending on the combination of family and community influences, such as the mothers level of education, the family’s experience of significant life stress, and residence in a relatively disadvantaged community. The second theme to be examined is the possibility that the accumulation of resource deficits or risk characteristics, over time, amplifies the likelihood of mental health problems in childhood. The approach models selected characteristics of the child in each of the four periods of development collectively, and it also models selected characteristics spanning each of the four time periods discretely. The results suggest that latency, pathway, and recency effects may operate simultaneously, and that timing and accumulated burden may both be important determinants of risk. For example, with regard to children whose family experienced life stress, these three effects operated in a systematic way to increase the degree of risk of a mental health problem. In summary, the aggregation of data at the individual level is a productive approach in seeking to explain population level social phenomena. While seemingly paradoxical, the identification of the joint, interactive effects between individual, family, and community characteristics, better allows for the quantification of family and community characteristics operating through multiple causal pathways.

'In Two Minds' is a novel of false beliefs. Set in contemporary Sydney, it deals with the relationship between two sisters in their late thirties, Kara and Linda Hille. Told in the second person singular from the point of view of the elder sister, Linda, it is based around the neurological delusion of the younger sibling, Kara. Kara wrongly believes that their mother, Stella, has been replaced by an impostor, 'Mrs. Whitegloves'. For the greater part, the narrative 'you' relates events in the sisters' lives and deals with issues such as the consequences of condoned child abuse, the dilemma of human cloning and the future of 'the brand' in the light of contemporary global marketing. Linda, an advertising executive, struggles with a formidable work-project, an account that is lost to a competitor, and the mistaken belief that she is responsible for her sister's plight. Shocking graffiti about herself, which appears at the same time as she wins an advertising award, proves to be the catalyst that brings beneficial change to her life. Through the tragedy of confronting her sister's devastation and her own challenges, Linda leaves her job, believing this will allow her to start again - differently. In the final chapter, the difference is registered in a shift from the second person to the consolidated first person method of narration. ABSTRACT EXEGESIS The dissertation 'A Singular Voice' documents aspects of authorial, psychoanalytical and literary significance in the creation of a fiction which draws on personal material confrontational to the writer. It also discusses some wider (non-fictional and other) uses of the narrative 'you' in order to establish the literary tradition in which the novel 'In Two Minds' may be situated. This disseration examines the use of the second-person singular pronoun 'you' as narrator, mainly in contemporary fiction. It concentrates on the ways in which the narrative 'you' was employed to achieve a 'cover', mask or persona for the 'I' behind the text in the novel 'In Two Minds', and explains why it was necessary to seek such subterfuge. It describes how certain grammatical and rhetorical resources were used to build and maintain 'cover', while at the same time allowing the narrative 'you' to express a particular aspect of the fictional protagonist, address the reader, and sustain the story of which it is the intradiegetic narratee. Related narrative elements include construction of the characters through the use of the narrative 'you', for example the narcissistic mother, Stella; the phantom double, 'Mrs. Whitegloves'; the sufferer of Capgras' delusion, Kara; and the ultimate bearer of the singular 'you' voice, the protagonist Linda.

Between 1960 and 1975 more than 38,000 mothers lost babies to adoption in New South Wales, Australia, a pattern which was replicated in other Western societies. Various theories were proposed for women's exnuptial pregnancies which resulted in their babies being taken for adoption, culminating in the discursive construction of the unmarried mother as 'mad, bad, or stupid'. Until the 1990s, the voices of women whose babies had been taken for adoption had been silenced by the social order which adoption practices served. It is through their voices, and through the voices of other women who remember the era of the adoption bounty, that another meaning for the loss of a baby to adoption, through the process of semanalysis, has been sought. This thesis is informed primarily by the writings of the French postmodern feminist, Julia Kristeva. In addition the works of the post-structuralist philosopher, Michel Foucault, the German socialist feminist, Frigga Haug, and the American feminist psychologist, Michelle Fine are used as an heuristic lens through which to examine the phenomenon of losing a baby to adoption. A qualitative research methodology, incorporating feminist praxis and feminist multiple methods, has been employed. The framework for this thesis is that of a double helix which depicts two orders, the symbolic and the semiotic, which intertwine and intersect. The symbolic order is analogous to the public social order which through its hegemonic discourses constructed the unmarried mother and adoption. The semiotic order refers to the personal space where the voices of women are heard through counterdiscourses. At the scission of the two helical strands sits the thetic phase, a point of rupture by the semiotic into the symbolic where the voices of mothers are expressed through their poetry and art. Through the process of semanalysis, the tensions which simultaneously resist and challenge the semiotic and symbolic orders are exposed: tensions between discourse, discipline and docility; power and knowledge; sexuality and silence; power of / as sexuality; power and resistance; and resistance and / as silence. Furthermore, I examine the manifestations of mothers' resistance to silence, and their resistance as activism. My concluding analysis involves the notion of abjection as it binds together the threads of the loss of a baby to adoption: abjection as entrapment; abjection as infertility; and abjection as / in reunion. For mothers who lost babies to adoption, their loss finds meaning in the ultimate horror: it is abjection.

Individual differences in subjective response to alcohol have been implicated as a risk factor for the development of alcohol use disorders. There are, however, a variety of ethical, legal, and practical considerations surrounding alcohol administration studies which limit the extent to which this marker can be used to identify those believed to be at greater risk. The current research contains two related laboratory studies with the overall goal of identifying valid and reliable correlates of individual differences in subjective response to alcohol that can be used to discern emerging adults at greater risk for problematic drinking. Study 1 evaluated the associations between the actual subjective experience of a moderate dose of alcohol (BAC .08%) and three domains of potential correlates: anticipated subjective response based on a hypothetical drinking scenario (targeted .08% BAC); response to other physiological and perceptual challenges (e.g., CO₂ challenge, spinning challenge); and indices of cognitive impairment implicated as risk factors for the development of alcohol use disorders. Study 2 examined each of these domains in relation to transitions in heavy drinking from high school to college to determine whether they were associated with changes in relative risk during this developmental period. Overall, the results provide support for the utility of examining individual differences in subjective response to alcohol based on a hypothetical drinking scenario. The evidence suggests that experienced drinkers are capable of reliably and accurately estimating their subjective response to alcohol, that these anticipated effects are distinct from general beliefs about the effects of alcohol on behavior (i.e., alcohol expectancies), and that they are associated with patterns of drinking in emerging adults. There was little evidence to suggest that individual differences in subjective response to alcohol were associated with subjective response to other physiological or perceptual challenges, or patterns of cognitive impairment previously shown to be related to an increased risk for alcohol dependence. The results of the current study support the utility of using measures of anticipated subjective response as a proxy for individual differences in subjective response to alcohol when the administration of alcohol is either not appropriate or feasible.

The primary objective of this study was the development and investigation of the psychometric properties of a measurement instrument based on the integrated model of behavior roles of offspring of alcoholic parents (Schneider, 1989,1995). The Behavior Role Scale was developed utilizing Crocker and Algina's (1986) 10 step instrument development process and deals directly with the concepts of the original models of behavior roles of offspring of alcoholic parents (Black, 1981; Booz-Allen & Hamilton, 1974; Deutsch, 1983; Kritzberg, 1985; Wegscheider, 1981). The first three steps of the instrument development were completed in an earlier investigation (Schneider, 1989). In this investigation, steps 4 - 8 were completed. The following activities were undertaken: step 4, the construction of an initial pool of items; step 5, the review and revision of the items with the assistance of a panel of experts; step 6, a preliminary tryout of the items in a pilot test; step 7, a field test of the instrument with a nonclinical sample of offspring of alcoholic parents and offspring of nonalcoholic parents; and, step 8, initial investigation of the psychometric properties of the behavior role scale. This investigation contributed the following findings. The field test yielded reliability estimates of .72 for the Caretaker, .67 for the Super Achiever, .86 for the Invisible One, .89 for the Jester, .78 for the Rebel, and an overall alpha of .88. Initial validity was indicated by the emergence of a five factor model as the most interpretable factor analysis solution. The performance of the Behavior Role Scale was also investigated using six validity issues identified in the literature. Offspring of alcoholic parents, offspring of nonalcoholic parents who had experienced a significant family disruption, and offspring of nonalcoholic parents who had not experienced a significant family disruption had significantly different scores with the Super Achiever, Invisible One, and Rebel roles. The three groups did not demonstrate significant differences with the Caretaker or Jester roles. Additional significant differences were discovered with the Jester and Rebel roles among offspring of alcoholic fathers when they were compared according to their fathers' pattern of drinking. The findings of this investigation add to the research evidence which suggests that refinements need to be made to improve the clinical and psycho-educational uses of the behavior roles of offspring of alcoholic parents. Recommendations for future research include submitting the Behavior Role Scale to construct validation studies in order to further refine its ability to measure behavior roles and to be of assistance to researchers seeking to unravel the complexities of life for more than 28 million offspring of alcoholic parents.
