Railroad, aviation, and shipping transport accidents tend to be handled as organizational incidents. The objective of accident investigation is to effectively decrease and prevent accident recurrence, rather than to ascribe blame to any person. Such investigations clarify technological, system-based, and managerial shortcomings. By eliminating factors that cause accidents, we may be able to prevent similar recurrences. In this article, we first review the global status of accident, then, look at current situations and issues as they apply to accident investigation in Japan.

Accident investigation is critical to the success of an effective safety program. The causes of the accidents have to be determined. A proper accident investigation report will also lead to a proper determination of whether an accident was preventable or not. The purpose of this study is to adopt the advantages from aircraft accident investigation report format. The prudent report format of accident is to enhance system and system safety. This study emphasizes the importance and necessity of element through studying an accidental investigation report. The results indicate that the conclusions to be raised and the forms of safety recommendations are best in all situations.

Abstract Accident models and theoretical foundations underpinning safety investigations are key to understanding how investigators construct causality and make recommendations. Safety science has devoted large efforts to investigating and theorizing about accidents. Why doesn’t healthcare pay more interest to these theories when investigating healthcare accidents? We use established accident theories to suggest how these can support safety investigations in healthcare and provide new lenses to investigatory bodies. We reflect on examples from research and practice in healthcare systems and other high-risk industries. Investigation processes and reports serve multiple purposes. We argue there is an untapped improvement potential for healthcare safety investigations and suggest new ways of integrating different accident theoretical reflections with investigatory practice.

Accidents provide important learning points to prevent future mishaps. In its approach to accident prevention one technique that Shell uses is the Tripod accident analysis tree.In the past there has been a tendency in all organisations to blame accidents on the people who suffer them and to see unsafe acts as arising from the stupidity, carelessness or recklessness of particular individuals. Accident investigations frequently cite 'human error' as the cause and many people still see such errors as the beginning and end of the accident sequence. The three more tangible components of such accident investigations are unsafe acts, failures of defences and the accidents themselves. Such unsafe acts or 'active failures' are often seen as the most important part of the accident story. In reality the story has its beginnings much earlier in areas such as Design, Procedures, Training or Maintenance Management, areas known as 'latent failures',To better understand the underlying causes of accidents, in 1988 Shell commissioned a behavioural research program called Tripod. Tripod does not address unsafe acts and hazards directly, but places them in context in a model of an accident which seeks to define the underlying organisational factors that provoke unsafe acts, and that allow errors to turn into accidents. The research has established that latent failures may be categorised into a small number of disruptive processes. These latent failures are incorporated into an accident diagram in the form of a 'tree', and improve our understanding of the causes of accidents, and direct our recommendations for improvement into areas that are likely to be the most effective.

The sole objective of an air accident investigation is the prevention of future accidents; its purpose is not to apportion blame or liability. This mantra of air accident investigators combined with requirements to ensure the independence of investigations – all contained in ICAO Annex 13 to the Chicago Convention – affords investigators the opportunity to review crucial evidence and reach conclusions on the cause of an accident without fear of recrimination or the presence of undue pressure. Ultimately, these provisions allow the proper and independent investigation of accidents and encourage aviation safety to be furthered. Recent high profile accidents have laid bare the pressures faced by investigators from within the investigation itself and external sources including the media, victims’ representatives, manufacturers, operators, and the spectre of blame proceedings. This article will study these stress points in the context of recent accidents and seek to illustrate why it is of fundamental importance to the independence of future air accident investigations, and the quality of the resultant safety outcomes, that such pressures are recognized and addressed by the International Civil Aviation Organization (ICAO). It will also consider whether the recently issued 11th edition of Annex 13 incorporating Amendment 15 achieves these aims.

Wamena airport experienced accidents in 2002, 2008, 2009, 2013, 2015, and 2016. All accidents were cargo flights and in approach and landing flight phases. As the Swiss Cheese concept, accident happened when errors penetrated safety defenses’ layers in straight line. Structuring NTSC’s investigations, under HFACS framework to understand the human factor failures type and HFIX strategy to close the failures by applying the recommendations, need to be done in air accident investigation. Eleven aviation experts and practitioners were interviewed in this study, to validate the framework. There were layers without any failures in accident 2008, 2013, and 2016. Accident in 2016 has no recommendation due operators’ safety actions were considered relevant to block failures. Accidents in 2002, 2009, 2013, and 2015 have failure in a layer which intervened by two or more recommendations. There were failures remain open in accident 2002, 2009, 2013, and 2016. Repetitive failure, error or violation of repetitive accidents in 2002, 2009, 2013, 2015, and 2016 is un-stabilized approach and has not been blocked with effective interventions. HFACS and HFIX are useful to framework the accident investigation, preventing similar accident happened in the future.

The collapse of engineering structures can cause significant casualties and have negative social effects. Collapse accident investigation can elucidate the potential causes and mechanisms of the collapse accident, thus remediating future structural collapse and enhancing the resilience. However, there are some obstacles to investigating complicated collapse accidents using conventional methods. For example, the out-syncs between on-site investigation and simulation analysis are intractable and can make discovering the cause of collapse accidents difficult. Hence, a digital twin-based investigation method for collapse accidents was proposed. First, basic virtual digital building models are established using real-world information. Then, after mapping the data from the real world into the virtual space, the corresponding highly realistic multistage models before and after the building collapse accident are constructed and synchronized. Using the digital twin method, investigators with multidisciplinary knowledge can efficiently integrate, update, and check the models. Finally, the potential collapse mechanism was revealed with the assistance of the corresponding models. To demonstrate the effectiveness of the proposed digital twin-based investigation method, a real collapse accident investigation is utilized as an example. These results validated our method.

One of the most important aims of incident management is the clearance of the incident scene as fast as possible. The accident Investigation provides physical evidence at the accident site for the investigators. This physical evidence is much more reliable than the witness's statements and they are very crucial for the incident reconstruction. The cars accidents investigation is a dangerous activity, so it should be undertaken with suitable, accurate, and fast equipment. Many law enforcement agencies in the world have used different surveying techniques for accident investigations including the coordinate method, total station, photogrammetry, laser scanner, etc. Therefore, this research has been carried out in order to introduce the benefits of using surveying techniques in traffic accident investigations, and show their impacts on evidence documentation and scene clearance. This is done by focusing on the advantages and the disadvantages of each method based on the relevant works of literature and compares between them. Although comparison result shows that the traditional method(coordinate method) is simpler and cheaper than other methods, surveying techniques methods are safer, and faster in clearing the accident scene, fewer investigators are needed, the scale can be provided directly, high accuracy measurements can be obtained, and three dimensions models can be produced. So it's worth using the surveying equipment in cars accidents investigations.

The investigation of traffic accidents within the framework of the crime under Art. 264 of the Criminal Code of the Russian Federation is, in most cases, a complicated and labor-intensive process primarily connected with the establishment and further investigation of the traffic accident’s mechanism. The investigation of a “classic” type of accident, in spite of the availability of investigation techniques, is a rather complicated process, while the investigation of “non-classic” types of traffic accidents poses legal challenges for the investigator acting as a law enforcer. This is explained by the absence of the investigation-court practice and investigation techniques of other types of traffic accidents, such as “no-contact” traffic accidents. The technique of investigating specific types of crimes has a great importance for a complete and comprehensive investigation of the analyzed type of accidents. By determining the typical features of preparing, committing and concealing the crimes, the criminalistic description contributes to the development of the most effective tactics, recommendations and necessary technical means of solving crimes.

The aim of this study is to investigate the relationship between accident types and the regions where the accidents occur, and to examine whether the resources required for accident prevention are distributed appropriately in the accident regions. In the study, the relationship between seven different types of accidents that occurred in seven different regions between the years of 2001-2016 was analyzed through the correspondence analysis. This analysis has been preferred because of its successful applications in various studies on accidents in the literature. As a result of the study, a significant relationship was found between accident types and accident regions, which means accident types vary by regions and there are accident types specific to accident regions. Collision in the Istanbul, grounding in the Canakkale and capsize accidents in the Izmir were determined to be more frequent. These findings are consistent with previous studies. It has been observed that, among regions, there are some imbalances in the distribution of resources used for marine accident response.

Background: The number of aircraft-assisted suicides can only be considered a rough estimate because it is difficult and, at times, impossible to identify all cases of suicide. Methods: Four recent reports of accidents occurring in 1997 in Indonesia, 1999 in Massachusetts in the United States, 2013 in Namibia, and 2015 in France related to commercial aircraft-assisted suicides were analyzed. This analysis relied on data extracted from the accident reports that supported aircraft-assisted suicide from the: (a) cockpit voice recorder (CVR) and flight data recorder (FDR), (b) medical history, (c) psychosocial history, (d) toxicology, (e) autopsy, and (f) any methodology that utilized aviation medicine. There are some limitations in this study. Although all analyzed accident investigations followed ICAO Annex 13 guidelines, there is variability in their accident investigations and reporting. In addition, accident investigation reports represent accidents from 1997 to 2015, and during this time, there has been a change in the way accidents are reported. The nature of this analysis is explorative. The aim was to identify how the various aircraft accident investigators concluded that the accidents were due to suicidal acts. Results: In all four accident reports, FDR data were available. CVR data were also available, except for one accident where CVR data were only partially available. Comprehensive medical and psychosocial histories were available in only one of four of the accident reports. Conclusion: To prevent accidents involving commercial aircraft, it is necessary to identify the causes of these accidents to be able to provide meaningful safety recommendations. A detailed psychological autopsy of pilots can and likely will assist in investigations, as well as generate recommendations that will substantially contribute to mitigating accidents due to pilot suicide. Airborne image recording may be a useful tool to provide additional information about events leading up to a crash and thus assist in accident investigations.

Ship accident consequences, including life loss, property/cargo damage, and environment pollution, could be caused by various factors. This study aims to quantitatively analyze the association between ship accident consequences and related contributing factors based on real ship accident investigation reports. We propose a two-step research methodology. In the first step, we standardize the necessary contributing factor–related information that is extracted from the ship accident investigation reports. In the second step, three statistical analysis methods, including nonparametric statistics of Kendall’s tau coefficient, Cramér’s V analysis and Kruskal–Wallis test, are carried out to figure out the import contributing factors with respect to ship accident consequences. The proposed methodology has been assessed based on 477 ship accident reports. Some insightful findings are obtained: (1) vessel speed is the most significant factor contributing to fatality and injury of collision accidents, whereas no remarkable relationship holds for grounding accidents; (2) for collision accidents, crew number shows a strong association with injury and fatality, but it exhibits weak linkages to other consequences of ship damage, cargo damage and oil spill; and (3) the consequence of oil spill is strongly related to collision position, especially in head-on and crossing collisions.

Abstract This study was conducted to investigate accident-prone roads in Palu City. The method used is direct observation on the road which shows the frequent occurrence of traffic accidents based on traffic accident data obtained from the Municipal Police Offices (Polres). Based on the results of the study showed that 19 roads often occur traffic accidents. R.E Martadinata Sub Districts Tondo (25.07%), Trans Sulawesi Sub Districts Taipa (11.21%), and Trans Sulawesi Sub Districts Mamboro (7.08%) are the most accident-prone roads in Palu City. These three roads are included in the national road network as the primary arterial roads of Palu City. The final result of this study shows that accident-prone roads can be caused by poor pavement road conditions, high side frictions, and poor environmental conditions and visibility.

Investigation at scene is the key issue to Scene Traffic Accident Reconstruction(TAR). Although the photogrammetry technique is widely applied in China due to its convenience and lower cost, it can only provide the final state of the traffic accidents. In this paper, a novel method based on the time difference of arrival (TDOA) technique was proposed to monitor the location of the vehicles passing the accident-prone section on the highway. The methodology of application of TDOA to the Traffic accident scene investigation was proposed, and the arrangement of sensors on the accident-prone sections of the selected freeway is discussed. Based on the coordinates collected by TDOA receivers, real-time position of the each vehicle moving on the accident-prone section could be extracted respectively and when the accidents happen, the traces of the vehicles involved would be obtained. It could be concluded that this method is important supplement to photogrammetry technique during accident scene investigation and could make the forensic judgment of accident responsibility more accurate.

Investigations of the thermal-hydraulic behavior of pressurized water reactors under accident conditions have been carried out in the PKL test facility at AREVA NP in Erlangen, Germany for many years. The PKL facility models the entire primary side and significant parts of the secondary side of a pressurized water reactor (PWR) at a height scale of 1 : 1. Volumes, power ratings and mass flows are scaled with a ratio of 1 : 145. The experimental facility consists of 4 primary loops with circulation pumps and steam generators (SGs) arranged symmetrically around the reactor pressure vessel (RPV). The investigations carried out encompass a very broad spectrum from accident scenario simulations with large, medium, and small breaks, over the investigation of shutdown procedures after a wide variety of accidents, to the systematic investigation of complex thermal-hydraulic phenomena. This paper presents a survey of test objectives and programs carried out to date. It also describes the test facility in its present state. Some important results obtained over the years with focus on investigations carried out since the beginning of the international cooperation are exemplarily discussed.

The Aegean Sea is risky for marine accidents due to its geography and dense vessel traffic. Revealing the patterns of marine accidents in the region is essential for preventing such accidents in the future. With this motivation, this study analyzes the data on maritime accidents in the Aegean Sea Turkish Search and Rescue Region. For this purpose, the descriptive analysis of the 576 marine accidents in the region between 2001 and 2020 was accomplished. Moreover, by applying hypothesis tests, the relationships between the factors that affect the occurrence of marine accidents were investigated. As a result, the most common contributing factors to the event of accidents were revealed. The relationships between the variables of the data set were determined. Accordingly, the type of ship with the most accident are yacht/recreational boat. Hull/machinery failure is the most common type of accident, and summer is the most frequent season for maritime accidents. It was revealed that there are significant relationships between the variables such as the type of accident, the sub-region where the accident occurred, the time of the accident, and the ship type. Finally, deficiencies were evaluated, and suggestions were made for more effective investigation and prevention of accidents.

Abstract: In India fatal road traffic accidents constitute a significant public health problem in many countries around the world. In Saudi Arabia, road accidents are one of the leading causes of death. The delay in detecting and caring for people who have been exposed to traffic accidents increases the risk of injuries. Injury care after the accident is severely affected by time; a few minutes may separate life from death. Several solutions have been proposed to reduce the incidence of accidents or to reduce the mortality rate at a high financial cost. Recent approaches are using built-in vehicle accident notification system. While these approaches work in pleasing manner, they are expensive, maintenance complex task, and are not available in all cars. However, there is a lack of methods and solutions used to detect the rapid occurrence of accidents and thus speed in transfer of patients to medical centres. QR Codes have fast response time and have large storage capacity, QR Codes can be used perfectly to send encrypted data (messages) to the receiver. In user panel, user will upload documents after submission the verification takes place in verification panel then the verified user will be given the unique QR code. At the signal, traffic police will scan the QR code and get all the documents of the respective user. So this system will provide a smart and easy way of verification of documents and identifying the user in accident management system

Abstract. With the rapid development of the economy, the number of vehicles in China has increased rapidly, which has also brought about frequent ills in traffic accidents. How to improve the efficiency of on-site treatment of traffic accidents, and quickly and accurately conduct accident investigation and analysis is imminent. This paper was based on the point cloud to draw the accident scene DLG, and then used the local elevation difference method to automatically extract the point cloud data of the accident vehicle, and analyzes the vehicle speed calculation, the damage area measurement and the road surface flatness, as well as constructs the overall 3D scene of the accident scene. By analyzing the DLG of accident scene, the point cloud data and the constructed 3D model, which could quickly improve the efficiency of traffic accident investigation. The application results show that the method of information collection and rapid exploration of the accident site what based on the laser point cloud not only provides a basis for traffic accident treatment, but also effectively shortens the exploration time of accident site. At the same time, it cloud relieve the traffic congestion in a certain extent with the obvious results.

Accident investigation reports are text documents that systematically review and analyze the cause and process of accidents after accidents have occurred and have been widely used in the fields such as transportation, construction and aerospace. With the aid of accident investigation reports, the cause of the accident can be clearly identified, which provides an important basis for accident prevention and reliability assessment. However, since accident record reports are mostly composed of unstructured data such as text, the analysis of accident causes inevitably relies on a lot of expert experience and statistical analyses also require a lot of manual classification. Although, in recent years, with the development of natural language processing technology, there have been many efforts to automatically analyze and classify text. However, the existing methods either rely on large corpus and data preprocessing methods, which are cumbersome, or extract text information based on bidirectional encoder representation from transformers (BERT), but the computational cost is extremely high. These shortcomings make it still a great challenge to automatically analyze accident investigation reports and extract the information therein. To address the aforementioned problems, this study proposes a text-mining-based accident causal classification method based on a relational graph convolutional network (R-GCN) and pre-trained BERT. On the one hand, the proposed method avoids preprocessing such as stop word removal and word segmentation, which not only preserves the information of accident investigation reports to the greatest extent, but also avoids tedious operations. On the other hand, with the help of R-GCN to process the semantic features obtained by BERT representation, the dependence of BERT retraining on computing resources can be avoided.

The daunting issues arising at experts and forensic bodies when appointing and conducting comprehensive road and pavement forensic investigation and road accident analysis (forensic expert examination) are considered. Types of traffic accidents provided by the statistics of the Department of Patrol Police of Ukraine are analyzed. A number of road accidents with victims committed in conditions of the road and settlements streets unsatisfactory state is defined. More and more judicial and investigative bodies turn to forensic science institutes, in particular to KhRIFE, in order to determine whether the improper design of the road network affected the occurrence of an accident. In other words, the questions not only on the establishment of improper design of the road section where the accident occurred, but also on the existence of cause and effect relationship between the identified discrepancies and the accident are advanced for examination. The study of the above issues belongs to the forensic specialty 10.16 Road and Pavement Forensic Investigation. The demand for forensic research of improper design of the road network and the causes of accidents are constantly growing. At the same time, the practice of conducting road and pavement forensic investigation, namely the analysis of the materials provided for the study, shows that most criminal proceedings are investigated both while pre-trial investigation and in court for violation of traffic safety rules or transport operation by persons who drive vehicles. In the statistical report of the Department of Patrol Police of Ukraine, the data of road accidents are not classified as those that arose due to improper condition of roads and streets of settlements, and when conducting road and pavement forensic investigation it is established that improper design of the road network is in cause and effect relationship with the occurrence of an accident from a technical point of view.

The attainment of the objective of the electric power industry specified in the Energy Strategy for the period up to 2035 and the implementation of the general principles and objectives of regulating reliability and safety in the electric power industry and the quality of electricity established in the Electric Power Industry Law is impossible without the development, adoption and timely updating of regulations governing the procedure for investigating the causes of accidents in the electric power industry. Previously, issues related to the regulation of the procedure for investigating the causes of accidents in the electric power industry were not considered in legal studies, with the exception of certain procedural issues in judicial practices in cases of appeal against investigation reports on the causes of accident. The purpose of the study is to establish the need to amend the legal regulation of the procedure for investigating the causes of accidents in the electric power industry and to form a concept for the development of the accident investigation procedure.

The purpose of this study was: (1) to propose a classification system for the contributory factors behind major coach and bus accidents with mass casualties based on the human factor analysis and classification system (HFACS); and (2) to identify the main contributory factors behind accidents and the main indicators of the causal factors. Based on 56 official investigation reports of major coach and bus accidents with more than 10 fatalities, a qualitative content analysis was conducted to develop a modified classification system for the contributory factors behind these accidents, and a gray correlation analysis was conducted to identify the main causative factors and indicators by calculating the correlation degrees. The results showed that the modified classification system for the contributory factors behind major coach and bus accidents can be divided into seven levels: government regulations, the organizational influence of passenger transportation enterprises, unsafe internal operational supervision, preconditions for drivers’ unsafe acts, drivers’ unsafe acts, proximate causes other than the driver’s act, and moderating factors affecting accident severity and probability. The organizational influence of passenger transportation enterprises is the most significant factor affecting the accidents. Thus, passenger transport enterprises must systematically strengthen their responsibility and safety management to prevent accidents. Accident investigations should begin with the accident process to determine the proximate cause as well as the factors that influence the likelihood and severity of the accident.

Many accidents, such as those involving collisions or trips, appear to involve failures of vision, but the association between accident risk and vision as conventionally assessed is weak or absent. We addressed this conundrum by embracing the distinction inspired by neuroscientific research, between vision for perception and vision for action. A dual-process perspective predicts that accident vulnerability will be associated more strongly with vision for action than vision for perception. In this preliminary investigation, older and younger adults, with relatively high and relatively low self-reported accident vulnerability (Accident Proneness Questionnaire), completed three behavioural assessments targeting vision for perception (Freiburg Visual Acuity Test); vision for action (Vision for Action Test—VAT); and the ability to perform physical actions involving balance, walking and standing (Short Physical Performance Battery). Accident vulnerability was not associated with visual acuity or with performance of physical actions but was associated with VAT performance. VAT assesses the ability to link visual input with a specific action—launching a saccadic eye movement as rapidly as possible, in response to shapes presented in peripheral vision. The predictive relationship between VAT performance and accident vulnerability was independent of age, visual acuity and physical performance scores. Applied implications of these findings are considered.

This article is an attempt to comprehensively identify the sources of weather condition information in relation to the occurrence of aviation accidents. This issue, according to the authors, should be considered in the following main areas: the actual weather condition at the scene of the accident and its impact on the course of the accident, the information obtained by the flight crew about the weather condition before the flight, the correctness of the crew's decision on how to carry out the flight on the basis of the information obtained about the weather condition. This publication first discusses the legal basis and requirements for meteorological protection of aviation activities and the resulting consequences. This is followed by a discussion of the meteorological data available for use in the investigation of aviation incidents. This is realized in relation to various weather conditions. All messages and forecasts are characterized, and the possibilities for their use in conducting air accident investigations are discussed. In doing so, a selection of the main meteorological phenomena that may directly or indirectly impact flight safety is discussed. Sources of weather information for accident investigators and their importance for the investigation are identified. Methods of cross-checking actual weather conditions at the accident scene are indicated.

Construction accident investigation reports contain critical information, but extracting useful insights from the voluminous Chinese text is challenging. Traditional methods rely on expert judgment, which leads to time-consuming and potentially inaccurate results. To overcome this problem, we propose a novel approach that combines text mining techniques and latent Dirichlet allocation (LDA) models to analyze standardized accident investigation reports in the Chinese construction industry. The proposed method integrates an information entropy term frequency-inverse document frequency (TF-IDF) weighting scheme to evaluate term importance and accounts for word and model uncertainty. The method was applied to a set of construction industry accident reports to identify the key factors leading to safety accidents. The results show that the causal factors of accidents in Chinese accident investigation reports consist of keywords and negative expressions, including “failure to timely identify safety hazards” and “inadequate site safety management”. Failure to timely identify safety hazards is the most common factor in accident investigation reports, and the negative expressions commonly used in the reports include “not timely” and “not in place”. The information entropy TF-IDF method is superior to traditional methods in terms of accuracy and efficiency, and the LDA model that considers word frequency and feature weights is better able to capture the underlying themes in the Chinese corpus. And the subject terms that make up the themes contain more information about the causes of accidents. This approach helps site managers more quickly and effectively understand the causal factors and key messages that lead to accidents from incident reports. It gives site managers insight into common patterns and themes associated with safety incidents, such as unsafe practices, hazardous work environments, and non-compliance with safety regulations. This enables them to make informed decisions to improve safety management practices.

However, to date, no studies at least in the Arab world generally and Gulf area specifically, conducted a comprehensive examination of cognitive functioning as potential predictors of car accidents and driving violations.ObjectivesExamining the role of cognitive functions e.g., verbal working memory, attentional control as predictors of traffic accidents and driving violations.AimsExamining the predictability of individual's cognition of occurrence of driving violations and accidents.MethodsThe study was carried on a sample of hundred and thirty two participants whose age ranged between 24 and 31 years. They were classified into groups of violators and non-violators, accident free and accident involved as well. Cognitive functioning were measured using self-reports and task performance, and a series of ANOVAS as well as stepwise multiple regressions were conducted to test the research hypothesis.ResultsFindings showed significant differences between violators and non-violators and between the accident free and accident involved groups in almost all of the considered factors, except for the decision making factor. Moreover, Pearson product-moment correlations showed that there were significant negative correlations between age, driving violations, and cognitive performance and the accidents.ConclusionsHuman cognition such as executive functioning and mental planning are key factors for predicting driving behavior and traffic accidents. Air accident investigation has provided a wealth of knowledge that has advanced the aviation industry and its safety. International Civil Aviation Organization has exercised international regulatory leadership since 1947, developing tools with international cooperation, such as the air accident investigation methodology. ICAO has forged a change in perspective on safety attributions or factors in different historical eras, using this methodology to deepen the understanding of the causes and thereby achieve aviation safety improvement. Authors aimed to analyze, through a detailed study of the world’s worst aviation accidents, their contribution to understanding the details of aviation safety culture. Beyond the technical issues and fatality rates, the necessary analysis is what knowledge researchers have gained from the beginning, exploring the attributions of reports and knowing what diverse factors have predominated in different eras. Descriptive analyzes of air disaster investigation have two objectives: to identify the beginnings of a global safety culture resulting from evolution in operational safety and to relate the different eras to the attributions of air accident investigations.

Based on the defensive attribution hypothesis, we assume that the victims of serious occupational accidents tend to attribute their accidents to external factors, while their coworkers and foremen tend to attribute the accidents to the victims' own action. The data to test this hypothesis consisted of 99 serious occupational accidents in southern Finland. In connection with the accident, or after the investigation at the accident site, 73 victims, 65 coworkers, and 71 foremen were interviewed. The results support our assumption, as the victims tended to use external attributions, whereas the foremen and coworkers attributed the accidents to internal factors. The defensive attribution hypothesis seems to explain the differences in how each party interpreted the basis for each accident.

Work accidents occur when the system doesn’t work properly and the safety barriers are avoided. The analysis of such events can be made by different methods. This paper aims at demonstrating how a work accident can be investigated by the use of the Events’ Tree decision method. Applying the steps of the algorithm, the method leads to determinate the causes of a work accident that occurs during the starting procedure of an equipment. By building the events’ tree, a graphical representation of the operations and decisions lead to a logical framework that explains the work accident. The case study consists of the description, analysis, investigation and mitigation of a work accident that occurred at the Rehabilitation of a road in Arad County. Using the Events’ Tree decision method, the cause of the accident was found to be the weak training of the operator and the carelessness of the job manager.

This study aimed to develop the accident mechanism of fatal accidents taking place in multiple processes at the workplace. Multiple processes were defined as the existence of multiple work types and work processes in the same workspace. Recently, various processes are frequently conducted simultaneously in one workplace with the participation of several companies, and more workers are increasingly experiencing industrial accidents while working in multiple processes. To prevent accidents in the multiple processes caused by the sameness of work period and space, the accident process was investigated by analyzing the investigation reports on serious industrial accidents in South Korea, and then the accident mechanism model was developed. By utilizing the developed model, the major safety measures to be taken by the contractor for each of the 8 types of accidents caused by the multiple processes were drawn. The roles and responsibilities of the contractor to be implemented in order to prevent accident occurrence in multiple processes were proposed through the accident mechanism of each type of fatal accident. It is expected that the accidents taking place in the multiple processes can be prevented with the drawn results.

Abstract In these times, dominated by innovation, technology is an important asset in optimizing maintenance, aircraft operations and even the investigation process of aviation accidents, with unprecedented results so far. All the information related to an aircraft, generated over time, can be included in a database, in order to be analyzed for better technical insights. The same approach can be applied in analyzing evidence and reports generated after an aircraft accident occurred. Aircraft investigation involves tones of hard-working hours and evidence analysis. This research aims to identify a new tool to improve the investigation process of aircraft incidents caused by human factors errors. Using artificial intelligence to process the resulted evidence during the aircraft accidents investigation, will shed light on the root cause of the incident much faster. Accelerating an investigation means huge benefits for all parties involved. In this sense, a proof-of-concept software has been developed to emphasize the power of artificial intelligent in the aircraft accidents investigation. Using machine learning algorithms, a program was trained with data, to develop an application capable of automatically determining the root cause of an accident based on witness statements. All data was derived from the Aviation Safety Reporting System database from the U.S, which was queried for incidents, occurred between 2000 and 2020. This application is not only about automating a stage in an aircraft accident investigation, is rather about setting up new standards in aviation industry with the help of artificial intelligence.

The number of cases of train accidents in Indonesia continues to be large in the past few years. This paper analyzes train accident cases in Indonesia using a socio-technical system approach accident model called AcciMap, developed by Jens Rasmussen. The source of data comes from train accident investigation reports from 2015 to 2021 published by the National Transportation Safety Committee. This study uses a qualitative approach with thematic analysis to identify contributing factors and their interactions within the reports. Based on the socio-technical system approach, there are roles and contributions from the five levels of the socio-technical railway transportation system in Indonesia that interact with each other in the occurrence of accidents, including the railway regulatory bodies; other organizations; management of railway service company; processes and physical actions by staff; and condition of equipment and environment. This study has shown that the AcciMap methodology can provide a comprehensive view of the problems and their interactions across the socio-technical levels that influence each other in the occurrence of train accidents in Indonesia. The results also suggest that the socio-technical system approach is expected to be applied in the accident investigation process for the related industry to get more comprehensive insights into the accident.

To thoroughly figure out the distribution and formation mechanism of maritime accidents, this study proposes a domino effect model based on temporal association rules (TAR) to analyze and mine the secrets behind the accident—the formation mechanism of accident chains. In this study, the British Marine Accident Investigation Branch (MAIB) accident reports are gathered and examined. Of which, Ro-Ro ships, general cargo ships, and container ships are the top three ship types discussed. The domino effect model is applied to the detected accidents, yielding a series of results. These show that the resulting values from unsafe working practices to death while working are very high and are 8.622 (Ro-Ro ship), 5.920 (General cargo ship) and 6.441 (Container ship), respectively. It indicates that unsafe working practices are very prone to accidents involving death while working. The approach is ubiquitous, and the accident chains compiled from them may be widely employed in marine accident prevention and proactive safety management.

Urgency of the research. The authors solve the following praxeological questions: How do we perceive a dangerous flight situation, a flight incident and an air accident? Which basic stages of the investigation do we use? How to implement a situational management methodology for investigating the circumstances of an air accident? Target setting. The article presents the analysis of the current state of information issues in the investigation of air accident circumstances in the military and civil aviation by using the methodology of situational management of forensic investigation processes. Actual scientific researches and issues analysis. An integral part of this development is also the tackling the concept of development of the Air Force, with an emphasis on the training of personnel and the level of the flight operations and the flight safety. Uninvestigated parts of general matters defining. The forensic investigation of air accidents and incidents is perceived as the specific application area of the Forensic Engineering. The research objective. The core output of article is in the proposal of the investigation of air accident circumstances in the military and civil aviation. The statement of basic materials. We use the tools of analysis and synthesis, the expert method with the inductive method based on the close observations and experimentation/investigation in the terrain, in the aviation practice. Conclusions. It is therefore necessary to subject on the permanent pay attention to the theoretical and practical level. Some specific issues addressed in the monograph can be extended with the further proposals and selected issues to address within the scope of research and scientific base of the armed forces.
