Railroad, aviation, and shipping transport accidents tend to be handled as organizational incidents. The objective of accident investigation is to effectively decrease and prevent accident recurrence, rather than to ascribe blame to any person. Such investigations clarify technological, system-based, and managerial shortcomings. By eliminating factors that cause accidents, we may be able to prevent similar recurrences. In this article, we first review the global status of accident, then, look at current situations and issues as they apply to accident investigation in Japan.

Abstract (sommario):
Accident investigation is critical to the success of an effective safety program. The causes of the accidents have to be determined. A proper accident investigation report will also lead to a proper determination of whether an accident was preventable or not. The purpose of this study is to adopt the advantages from aircraft accident investigation report format. The prudent report format of accident is to enhance system and system safety. This study emphasizes the importance and necessity of element through studying an accidental investigation report. The results indicate that the conclusions to be raised and the forms of safety recommendations are best in all situations.

Abstract Accident models and theoretical foundations underpinning safety investigations are key to understanding how investigators construct causality and make recommendations. Safety science has devoted large efforts to investigating and theorizing about accidents. Why doesn’t healthcare pay more interest to these theories when investigating healthcare accidents? We use established accident theories to suggest how these can support safety investigations in healthcare and provide new lenses to investigatory bodies. We reflect on examples from research and practice in healthcare systems and other high-risk industries. Investigation processes and reports serve multiple purposes. We argue there is an untapped improvement potential for healthcare safety investigations and suggest new ways of integrating different accident theoretical reflections with investigatory practice.

Accidents provide important learning points to prevent future mishaps. In its approach to accident prevention one technique that Shell uses is the Tripod accident analysis tree.In the past there has been a tendency in all organisations to blame accidents on the people who suffer them and to see unsafe acts as arising from the stupidity, carelessness or recklessness of particular individuals. Accident investigations frequently cite 'human error' as the cause and many people still see such errors as the beginning and end of the accident sequence. The three more tangible components of such accident investigations are unsafe acts, failures of defences and the accidents themselves. Such unsafe acts or 'active failures' are often seen as the most important part of the accident story. In reality the story has its beginnings much earlier in areas such as Design, Procedures, Training or Maintenance Management, areas known as 'latent failures',To better understand the underlying causes of accidents, in 1988 Shell commissioned a behavioural research program called Tripod. Tripod does not address unsafe acts and hazards directly, but places them in context in a model of an accident which seeks to define the underlying organisational factors that provoke unsafe acts, and that allow errors to turn into accidents. The research has established that latent failures may be categorised into a small number of disruptive processes. These latent failures are incorporated into an accident diagram in the form of a 'tree', and improve our understanding of the causes of accidents, and direct our recommendations for improvement into areas that are likely to be the most effective.

The sole objective of an air accident investigation is the prevention of future accidents; its purpose is not to apportion blame or liability. This mantra of air accident investigators combined with requirements to ensure the independence of investigations – all contained in ICAO Annex 13 to the Chicago Convention – affords investigators the opportunity to review crucial evidence and reach conclusions on the cause of an accident without fear of recrimination or the presence of undue pressure. Ultimately, these provisions allow the proper and independent investigation of accidents and encourage aviation safety to be furthered. Recent high profile accidents have laid bare the pressures faced by investigators from within the investigation itself and external sources including the media, victims’ representatives, manufacturers, operators, and the spectre of blame proceedings. This article will study these stress points in the context of recent accidents and seek to illustrate why it is of fundamental importance to the independence of future air accident investigations, and the quality of the resultant safety outcomes, that such pressures are recognized and addressed by the International Civil Aviation Organization (ICAO). It will also consider whether the recently issued 11th edition of Annex 13 incorporating Amendment 15 achieves these aims.

Wamena airport experienced accidents in 2002, 2008, 2009, 2013, 2015, and 2016. All accidents were cargo flights and in approach and landing flight phases. As the Swiss Cheese concept, accident happened when errors penetrated safety defenses’ layers in straight line. Structuring NTSC’s investigations, under HFACS framework to understand the human factor failures type and HFIX strategy to close the failures by applying the recommendations, need to be done in air accident investigation. Eleven aviation experts and practitioners were interviewed in this study, to validate the framework. There were layers without any failures in accident 2008, 2013, and 2016. Accident in 2016 has no recommendation due operators’ safety actions were considered relevant to block failures. Accidents in 2002, 2009, 2013, and 2015 have failure in a layer which intervened by two or more recommendations. There were failures remain open in accident 2002, 2009, 2013, and 2016. Repetitive failure, error or violation of repetitive accidents in 2002, 2009, 2013, 2015, and 2016 is un-stabilized approach and has not been blocked with effective interventions. HFACS and HFIX are useful to framework the accident investigation, preventing similar accident happened in the future.

The collapse of engineering structures can cause significant casualties and have negative social effects. Collapse accident investigation can elucidate the potential causes and mechanisms of the collapse accident, thus remediating future structural collapse and enhancing the resilience. However, there are some obstacles to investigating complicated collapse accidents using conventional methods. For example, the out-syncs between on-site investigation and simulation analysis are intractable and can make discovering the cause of collapse accidents difficult. Hence, a digital twin-based investigation method for collapse accidents was proposed. First, basic virtual digital building models are established using real-world information. Then, after mapping the data from the real world into the virtual space, the corresponding highly realistic multistage models before and after the building collapse accident are constructed and synchronized. Using the digital twin method, investigators with multidisciplinary knowledge can efficiently integrate, update, and check the models. Finally, the potential collapse mechanism was revealed with the assistance of the corresponding models. To demonstrate the effectiveness of the proposed digital twin-based investigation method, a real collapse accident investigation is utilized as an example. These results validated our method.

One of the most important aims of incident management is the clearance of the incident scene as fast as possible. The accident Investigation provides physical evidence at the accident site for the investigators. This physical evidence is much more reliable than the witness's statements and they are very crucial for the incident reconstruction. The cars accidents investigation is a dangerous activity, so it should be undertaken with suitable, accurate, and fast equipment. Many law enforcement agencies in the world have used different surveying techniques for accident investigations including the coordinate method, total station, photogrammetry, laser scanner, etc. Therefore, this research has been carried out in order to introduce the benefits of using surveying techniques in traffic accident investigations, and show their impacts on evidence documentation and scene clearance. This is done by focusing on the advantages and the disadvantages of each method based on the relevant works of literature and compares between them. Although comparison result shows that the traditional method(coordinate method) is simpler and cheaper than other methods, surveying techniques methods are safer, and faster in clearing the accident scene, fewer investigators are needed, the scale can be provided directly, high accuracy measurements can be obtained, and three dimensions models can be produced. So it's worth using the surveying equipment in cars accidents investigations.

The investigation of traffic accidents within the framework of the crime under Art. 264 of the Criminal Code of the Russian Federation is, in most cases, a complicated and labor-intensive process primarily connected with the establishment and further investigation of the traffic accident’s mechanism. The investigation of a “classic” type of accident, in spite of the availability of investigation techniques, is a rather complicated process, while the investigation of “non-classic” types of traffic accidents poses legal challenges for the investigator acting as a law enforcer. This is explained by the absence of the investigation-court practice and investigation techniques of other types of traffic accidents, such as “no-contact” traffic accidents. The technique of investigating specific types of crimes has a great importance for a complete and comprehensive investigation of the analyzed type of accidents. By determining the typical features of preparing, committing and concealing the crimes, the criminalistic description contributes to the development of the most effective tactics, recommendations and necessary technical means of solving crimes.

The aim of this study is to investigate the relationship between accident types and the regions where the accidents occur, and to examine whether the resources required for accident prevention are distributed appropriately in the accident regions. In the study, the relationship between seven different types of accidents that occurred in seven different regions between the years of 2001-2016 was analyzed through the correspondence analysis. This analysis has been preferred because of its successful applications in various studies on accidents in the literature. As a result of the study, a significant relationship was found between accident types and accident regions, which means accident types vary by regions and there are accident types specific to accident regions. Collision in the Istanbul, grounding in the Canakkale and capsize accidents in the Izmir were determined to be more frequent. These findings are consistent with previous studies. It has been observed that, among regions, there are some imbalances in the distribution of resources used for marine accident response.

Crashworthiness can be defined as the ability of the airplane in protecting its occupants in crash situations. This concept is only applicable in accidents that are survivable, that is, accidents for which the decelerations peaks are into human tolerance limits. There is na international concern about flight safety, because statistics show that the number of accidents per flight departures hás been Constant since 1975 despite the associated efforts in technology and crew training. Then, assuming this scenario of constant accident rate, the form found by researchers, manufacturers and authorities to protect the passengers in the accidents hás been to invest in the aircraft protection capability. A crashworthiness accident investigation can recover information about the aircraft behavior submitted by impact, and can supply the design team with the necessary information in case of a redesign that seeks safety improvement. This work proposes a crashworthiness investigation checklist that is structured according to the methodology presented by National Transportation Safety Board, and the goal is to establish investigative techniques that can facilitate the collection and analysis of crashworthiness data for accidents occurring in commercial aviation.

The aim of this research is to determine the effect of unsignalised intersection geometry on the rates of the various types of accidents occurring at unsignalised intersections. A literature review has identified that there is little consistency between the results of previous studies. Some studies found that particular parameters had an opposite effect to what was expected. With this in mind, the research identified reasons for these results and developed two basic approaches to mitigate some of the problems with multi-factor type studies. These approaches are 'maximise the efficiency of data collection' and 'develop techniques for analysing less than perfect data'. A database consisting of 206 unsignalised intersection sites from throughout Queensland was used for analysis. The outcome of this research confirms the validity of several of the current design standards for unsignalised intersections, in addition to identifying new engineering procedures.

In order to prevent accidents we need to understand them, this is achieved through effective accident investigation. Accident investigation is a complex process of gathering and evaluating information to determine factors that may have implications on the final event. One of the fundamental aspects in the investigation process is to capture geospatial data of the incident, to document the scene in its current condition, providing the investigation team with a record for future reference. The production of plans have conventionally remained the same, with a surveyor tasked to illustrate a 3D scene with 2D representations. Recent developments in instrumentation have provided the geospatial industry with the means to capture vast amounts of 3D data directly using laser scanning. In addition, there have been considerable advancements in software applications which can be used to process the surveyed datasets. This research evaluates the use of the latest technology in respect of accident investigation applying the methodology to fire related incidents, industrial accidents and mining incidents. This is achieved by using a number of case studies that have been undertaken throughout the timeline of the project and whilst working with industry professionals in the field.

Objetivos. Analisar investigações de acidentes e materiais didáticos e educativos, explorando aspectos da construção das análises e de atribuição de culpa. Métodos. Registros de investigações e materiais didáticos de empresas de Botucatu-SP, em 1997, obtidos de instituições e empresas, foram analisados, identificando-se fatores causais, recomendações e referências a comportamentos faltosos ou características da vítima. Árvores de causas foram checadas quanto a cuidados de linguagem, desenhos, interrupções da investigação, tipos de relações entre fatos e exploração para prevenção. Materiais educativos foram analisados, identificando-se concepções de acidente, orientações para investigações e recomendações. Bancos de dados foram formados e gerenciados com EPIinfo. Resultados. Foram analisadas 203 investigações. A média de “causas” por AT foi 1,68 e a de recomendações foi 1,4 por AT, com predomínio de referências a comportamentos ou características da vítima. A maioria das análises baseava-se em formulário obrigatório. Todas as árvores analisadas mostravam desrespeito flagrante a regras e princípios do método. Materiais didáticos veiculavam mesma concepção de acidente das investigações, método de análise centrado em comportamentos faltosos do operador, estímulo ao medo de lesões e recomendações para obediência a regras e uso de equipamentos de proteção. Conclusão. As investigações adotam concepção de acidente baseada na identificação de situações de desrespeito a regras idealizadas, atribuem culpa ao acidentado e não subsidiam a gestão de riscos nas empresas. A atribuição de culpa ocorre independentemente da natureza e/ou tipo de perigo presente nos acidentes. Tentativas de uso do método de árvore de causas mostraram distorções, revelando insuficiência no seu domínio. A construção da culpa foi reforçada por materiais didáticos e educativos. Objectives: To analyze accident investigations and teaching and educational materials exploring aspects of analysis construction and blame attribution. Methods: Investigation reports and educational materials of companies in Botucatu, SP, in 1997, obtained from institutions and companies were analyzed and causal factors, recommendations and references to faulty behavior or victim characteristics were identified. Causal tree were checked in regard to language, design, investigation interruptions, types of relations between facts and exploration for prevention. Educational materials were analyzed identifying the conception of accident, orientation for investigations and recommendation. Data banks were formed and managed by EPIinfo. Results: 203 investigations with an average of 1,68 causes and 1,4 recommendations were analyzed and revealed that references to victim’s behavior or characteristics prevailed. Most analyses were based on an obligatory form. All analyzed trees showed evident disrespect to rules and methodological principles. The educational and teaching materials convey the same accident concept as the investigations, the analyses method centered in the faulty behavior of the operator, stimulus of fear to injury and recommendation for obedience to rules and use of protection equipment. Conclusion: The investigations studied use an accident concept based on the identification of situations of disrespect to idealized rules, blame the injured subjects, and do not support the risk management in the companies. The blame bias occurs independently of the nature or kind of danger involved in the accidents. The attempts to use the causal tree method revealed distortions due to insufficient command. Educational materials strengthened the construction of blame.

This project examines the safety management of civil aircraft accident investigation authorities in International Civil Aviation Organization (ICAO) Member States, with particular emphasis on the independence of the investigations. The research aims to establish the current level of resources and methodology adopted by Member States’ accident investigation authorities. The output of this work not only identifies the current situation but informs initiatives for some of the States in the process of establishing their investigation capability. ICAO Annex 13 was analysed and found to be based on the principle of independent accident investigations. Also, a four dimensional measuring index (4DMI) has been developed to measure the independence of accident investigations in ICAO Member States. Data were collected from 45 States and are presented in the thesis. As a result of applying the 4DMI to the collected data, the States were ranked according to their scores, and divided into four categories of independence. Analysis of the four categories and the scores from the four dimensions revealed that States approach the concept of investigation independence in different ways; however, there are several practices that are common within the highest independence category and several other practices that are common within the lowest independence category. The research recommends that States should work towards improving their overall investigation independence by implementing the seven identified practices in the High-Independence category and distance themselves from the five practices identified as common in the Low-Independence category.

Civil aviation is a strategic priority of geopolitical, social and economic development of Ukraine and an important part of production and social infrastructure. Its sustainable, efficient operation is a necessary condition for national security, sustainable economic growth and improving living standards. With the beginning of the restructuring of economic relations, the volume of aviation activity in Ukraine has decreased significantly. The financial situation of aviation enterprises has become more complicated. It has led to a reduction in the development and improvement of civil aviation, and led not only to a slowdown in scientific and technological progress, but also to a deterioration in its technical condition. The fashion for the creation of "independent structural units" within enterprises and the industry as a whole in search of economic benefit has pushed to the background the issue of flight safety. Annual, long-term structural reorganizations with the Aviation Administration of Ukraine do not allow effective and efficient management. The predominant interests of "commerce" lead to the widespread use of strictly prohibited methods: the irreversible process of deconstruction aircraft and rearranging units, engines and equipment from one aircraft to another, extending the resources of aircraft without a proper assessment of its condition which inevitably leads to complete lack of control on the part of the Aviation Administration of Ukraine. It indicates that the level of flight safety in the air navigation system of Ukraine is not provided. A number of problems in safety theory are caused by imperfect methods of scientific research, in particular, when planning airspace.
Цивільна авіація є стратегічним пріоритетом геополітичного, соціального та економічного розвитку України та важливою частиною виробничої та соціальної інфраструктури. Його стійке, ефективне функціонування є необхідною умовою національної безпеки, стійкого економічного зростання та підвищення рівня життя. З початком перебудови економічних відносин обсяг авіаційної діяльності в Україні значно зменшився. Фінансовий стан авіаційних підприємств ускладнився. Це призвело до зменшення розвитку та вдосконалення цивільної авіації та призвело не лише до уповільнення науково-технічного прогресу, але й до погіршення її технічного стану. Мода на створення "самостійних структурних підрозділів" на підприємствах та в цілому в галузі в пошуках економічної вигоди відсунула на другий план питання безпеки польотів. Щорічні, довгострокові структурні реорганізації з Авіаційною адміністрацією України не дозволяють ефективно і результативно керувати. Переважаючі інтереси "торгівлі" призводять до широкого використання суворо заборонених методів: незворотний процес деконструкції літаків та перестановка агрегатів, двигунів та обладнання з одного літака на інший, розширення ресурсів літака без належної оцінки його стану, що неминуче призводить до повної відсутності контролю з боку авіаційного управління України. Це свідчить про те, що рівень безпеки польотів в аеронавігаційній системі України не забезпечується. Ряд проблем в теорії безпеки спричинений недосконалими методами наукових досліджень, зокрема, при плануванні повітряного простору.

The top management of every mine should prove their commitments to the occupational health and safety policy to carry out this goal by means of preparing and implementing an accident preventing program.

In the immediate aftermath of major transport accidents, there are often calls for blame and accusations of ‘human error’. From the perspective of human factors and ergonomics (HF/E), though, any such errors are the starting point – not the conclusion – of a safety investigation into the deeper, sociotechnical system factors that created the context for the accident to occur. This paper aims to challenge both investigators and HF/E practitioners regarding both the integration and boundaries of HF/E in transport accident investigations. The systems model aligns with the approaches taken by the major transport accident investigation bodies around the world. But there remain barriers in integrating HF/E within investigations, not least around the validity and usability of causal analysis methods. Similarly, although techniques for interviewing witnesses are well served in the literature, there is a gap in the HF/E knowledge base concerning guidance for collecting and analysing evidence relating to human factors in a safety investigation, due to the variable nature of human performance. Such variability can also be at the heart of many positive outcomes aside from accidents and incidents which, whilst seemingly contrary to the very approach of investigating accidents, can also be integrated into a safety investigation. This paper concludes by considering how the HF/E perspective applies to the ultimate output of safety investigations in the form of recommendations.

The aim of the study is to evaluate road traffic accident analyses reports as a means to improve the system of medical driver examinations. Qualitative analyses was made of accident reports from the Norwegian Public Roads Administration region East and South throughout 2008 and 2009. Only 23 of 235 reports contained information on driver health. Many different diagnoses were represented. Intoxication was a major contributor to accidents. Straight-ahead accidents is discussed as a field of further investigations. In conclusion, the accident analyses reports are not a good source of information on drivers’ health. The system needs wider authorities and mandate to be an effective contribution to public health work in Norway.

The National Transportation Safety Board in the United States and the Transportation Safety Board of Canada publish reports about major railroad accidents. The text from these accident reports were analyzed using the text mining techniques of probabilistic topic modeling and k-means clustering to identify the recurring themes in major railroad accidents. The output from these analyses indicates that the railroad accidents can be successfully grouped into different topics. The output also suggests that recurring accident types are track defects, wheel defects, grade crossing accidents, and switching accidents. A major difference between the Canadian and U.S. reports is the finding that accidents related to bridges are found to be more prominent in the Canadian reports.

Every year, over 60,000 work zone crashes are reported in the United States (FHWA 2016). Such work zone crashes have resulted in over 4,400 fatal and 200,000 non-fatal injuries in the last 5 years (FHWA 2016, BLS 2014). Apart from the physical and emotional trauma, the annual cost of these injuries exceeds $4 million-representing significant wasted resources. To improve work zone safety, this research developed a system architecture for unveiling high-risk behavioral patterns among highway workers, equipment operators, and drivers within dynamic highway work zones. This research implemented the use of a connected virtual environment, which is an immersive hyper-realistic and virtual environment where multiple agents (e.g. workers, drivers, and equipment handlers) control independent simulators but experience an interactive and shared experience. For this project, the team conducted an in-depth analysis of accident investigation, simulated accident scenarios, and tested diverse interventions to prevent high-risk behavior. Overall, the research improved understanding of behavioral patterns that lead to injuries and fatalities of highway workers in order to better protect them in high-risk work environments. As part of making transportation smarter, this project contributes to smart behavioral safety analysis.

The relevance of the research is aimed at an effective system of organization and management of industrial safety, which allows you to manage risks and helps to ensure favorable working conditions for the health of employees at the enterprise. A mobile emergency and emergency response system was created. The system can also be used for accounting and accident investigation, based on the use of corporate communication devices and applications for mobile operating systems.
