ABSTRACTObjectives:To examine the safety of emergency department (ED) procedural sedation and analgesia (PSA) and the patterns of use of pharmacologic agents at a Canadian adult teaching hospital.Methods:Retrospective analysis of the PSA records of 979 patients, treated between Aug. 1, 2004, and July 31, 2005, with descriptive statistical analysis. This represents an inclusive consecutive case series of all PSAs performed during the study period.Results:Hypotension (systolic blood pressure ≤ 85 mm Hg) was documented during PSA in 13 of 979 patients (1.3%; 95% confidence interval [CI] 0.3%–2.3%), and desaturation (Sao2≤ 90) in 14 of 979 (1.4%; CI 0.1%–2.7%). No cases of aspiration, endotracheal intubation or death were recorded. The most common medication used was fentanyl (94.0% of cases), followed by propofol (61.2%), midazolam (42.5%) and then ketamine (2.7%). The most frequently used 2-medication combinations were propofol and fentanyl (P/F) followed by midazolam and fentanyl (M/F), used with similar frequencies 58.1% (569/979) and 41.0% (401/979) respectively. There was no significant difference in the incidence of hypotension or desaturation between the P/F and M/F treated groups. In these patients, 9.1% (90/979) of patients received more than 2 different drugs.Conclusions:Adverse events during ED PSA are rare and of doubtful clinical significance. Propofol/fentanyl and midazolam/fentanyl are used safely, and at similar frequencies for ED PSA in this tertiary hospital case series. The use of ketamine for adult PSA is unusual in our facility.

This study aims to develop modules of course on the Language Philosophy in STKIP PGRI Pacitan. It held in the first year of 2013/2014 and the second year of 2015/2016. There are 3 phases of the development: problem identification, potential problems, and design of language philosophy modules. The result of the development in the first year are a module design of scientific language philosophy based on character values of Vol 1 and 2" ISBN 978-979-1533-19-5 and 978-979-1533-18-8. The second year study consisted of: student and expert validation, module revision, limited trial, module revision, extensive testing, module revision, print-ready module. Expert validation result was 89%, student debriefing questionnaire was 88,6%, mean score 88,8%. The result of the next phase of limited trial based on counts table (t12 = 2.77, t13 = 7.89, t14 = 8.96, t23 = 2.88, t24 = 4.69, t34 = 2.79) is bigger thant-table with 2,021 probabilities. After a limited test, we conclude that the scientific-language philosophy module based on the value of local wisdom is very feasible to use. Test process is broadly obtained that t-count table is bigger (t56 = 2.77, t57 = 6,48, and t67 = 2,29) than t-table 2,021. Abstract (sommario):

Background: Iodine uptake is a main factor affecting thyroid disease. In Turkey, mandatory salt iodization began in 1999–2000. Aims: This study in 2009 determined the prevalence of thyroid diseases in older people in Mamak district, Ankara after iodization to ascertain if salt iodization alone is sufficient to reach adequate iodine levels in the older population. Methods: All Mamak residents ≥ 65 years were eligible for inclusion in the study. Demographic data and medical history were recorded. All participants had a thyroid ultrasound. Blood samples were taken to assess thyroid function and autoantibodies, and urine samples to assess iodine concentration. Participants with low levels of thyroid stimulating hormone underwent scintigraphy to assess thyroid uptake. Fine-needle aspiration biopsy was done of nodules ≥ 1.5 cm where thyroid stimulating hormone was not suppressed. Results: Of 1200 eligible residents, 979 were included. Their mean age was 70.9 (standard deviation (SD) 5.7) years; 49.7% were women. Mean urinary iodine concentration was 98 (SD 81.29) µg/L. Goitre was found in 18.2% (89/487) of women and 6.7% (33/492) of men (P < 0.001) and 43.8% (428/979) had nodules. Subclinical hypothyroidism was found in 5.8% (57/979) of the participants, overt hyperthyroidism in 0.8% (8/979), subclinical hyperthyroidism in 2.2% (22/979) and T3 thyrotoxicosis in 0.3% (3/979). Toxic multinodular goitre and toxic adenoma caused 80% of hyperthyroidism cases. Biopsy detected no malignant pathology. Conclusion: After salt iodization, iodine levels have not yet reached favourable levels in older people. Iodization of salt seems insufficient to achieve these levels in older people; alternative iodine supplementation should be considered.

The present investigation was carried out with the multiple objective of analyzing the psychometric properties of the Organizational Justice Scale (EJOC) and establishing its percentile norms for workers in the city of Piura. 268 workers from different companies in the city of Piura participated through a non-probabilistic convenience sampling. Among the results, it was reported that the 20 items that make up the scale have an adequate relationship with the main variable obtained by the descriptive statistical analysis. In addition, the internal structure of the instrument was analyzed through the four dimensions initially proposed, obtaining adequate fit and error indices: Chi= 217.571, df= 164, Chi/df= 0.03, CFI= .979, TLI= ,975, RMSEA= .035 and SRMR= .046. Likewise, a high reliability index through the Alpha Coefficient of Cronbach and McDonald whose values ranged between .776 and .959. Finally, the percentile norms were determined through three levels (low, medium and high) by dimensions and for the total scale. In this sense, it is concluded that the Colquitt EJOC is valid and reliable for workers from Piura.

The purpose of this study is to investigate how PG Trangkil Pati workers' work discipline is affected by pay, inspections, and penalties. All PG Trangkil Pati employees that were chosen using the proportionate random sample method provided the data for this study. 194 employees in all, with 130 replies in the sample. A questionnaire is used to collect data. Following data collection, multiple linear regression analysis is performed with the aid of the SPSS application. According to the study's findings, which are partially supported by the 5, 886> 1, 979, employee work discipline is positively and significantly impacted by compensation. 4. 316> 1. 979 shows that inspections have a good and significant impact on employee work discipline. The 4. 688> 1. 979 shows that work penalties have a favorable and substantial influence on employee work discipline to a partly extent. The result of 46, 701> 2. 68 indicates that employee work discipline is positively and significantly impacted by compensation, inspections, and work penalties all at the same time

Abstract: Progranulin is a recently recognized multifunctional glycopeptide shown to be related to obesity and diabetes mellitus. Progranulin is an endogenous antagonist of tumor necrosis factor-α by competitively binding to its receptor, therefore, it exerts anti-inflammatory activity. Paradoxically, previous studies have shown that serum levels of progranulin were elevated in patients with diabetes and associated to its complications including micro- and macroangiopathies, macroalbuminuria or reduced renal function. Moreover, hyperprogranulinemia may be involved in the pathogenesis of obesity-associated insulin resistance. Background Since December 2019, coronavirus disease (COVID-19) has been rapidly spreading worldwide. Nurses play a key role in fighting this disease and are at risk of COVID-19 infection. Therefore, there is an urgent need to assess the mental health condition of nurses and establish appropriate interventions to reduce the negative psychiatric outcomes of the pandemic. Objective The objectives of this study were to evaluate the knowledge and psychological stress related to COVID-19 among nursing staff and to provide evidence of the need for targeted training and psychological intervention. Methods This cross-sectional web-based survey study was performed in a class 3 grade A general hospital in a southwest province of China from March 1 to March 15, 2020. A self-designed questionnaire with questions about COVID-19–related prevention and control knowledge and the Triage Assessment Form (TAF) were used to assess nursing staff’s knowledge of COVID-19 and their degree of psychological stress, respectively. SPSS 23.0 was applied for statistical analysis of the collected data. Results A total of 979 nurses completed the questionnaire. The results showed that the nursing staff provided the fewest correct answers to questions about continuous viral nucleic acid testing specifications (379/979 correct answers, 38.7%), isolation/discharge criteria (539/979 correct answers, 55.1%), and management measures for patients with suspected symptoms (713/979 correct answers, 72.8%). The median total score of the TAF was 7.0 (IQR 5.0-12.0), and there were statistically significant differences in scores between different nursing roles, years of work experience, and hospital departments (P<.05). Conclusions This study indicated that nursing staff have insufficient knowledge about COVID-19. Meanwhile, although the psychological damage to nurses during the pandemic was found to be low, nurse managers must continue to monitor the mental health of nursing staff and perform timely interventions.
