Monomeric cyclophanes (3a,b) exhibit a well defined cleft, while dimeric (3c,d) adopt a flat conformation devoid of cavities. Binding properties were determined by 1H n.m.r. titrations in CDCl3 at T = 296 K. Complexation experiments with quaternary ammonium salts revealed that host (3c) possesses appreciable binding ability toward N-methylpyridinium (–ΔG° = 5·9 kJ mol-1), tetramethylammonium (–ΔG° = 8·8 kJ mol-1) and acetylcholine (–ΔG° = 7·5 kJ mol-1) cations, despite its lack of a preorganized cavity.

In the title molecular salt, C9H17N2+·C2H12B9−, the carborane cage has a bridging B—H—B bond on the open B3C2face. The butyl side chain of the cation adopts an extended conformation [C—C—C—C = 179.6 (1)°]. In the crystal, the imidazolium ring is almost coplanar with the open face of the carborane anion. The cations stack in the [010] direction and the dihedral angle between the imidazolium rings of adjacent cations is 68.45 (6)°. The butyl chains extend into the space between carborane anions.

BackgroundThe Global Burden of Disease Study (GBD) has historically produced estimates of causes of injury such as falls but not the resulting types of injuries that occur. The objective of this study was to estimate the global incidence, prevalence and years lived with disability (YLDs) due to facial fractures and to estimate the leading injurious causes of facial fracture.MethodsWe obtained results from GBD 2017. First, the study estimated the incidence from each injury cause (eg, falls), and then the proportion of each cause that would result in facial fracture being the most disabling injury. Incidence, prevalence and YLDs of facial fractures are then calculated across causes.ResultsGlobally, in 2017, there were 7 538 663 (95% uncertainty interval 6 116 489 to 9 493 113) new cases, 1 819 732 (1 609 419 to 2 091 618) prevalent cases, and 117 402 (73 266 to 169 689) YLDs due to facial fractures. In terms of age-standardised incidence, prevalence and YLDs, the global rates were 98 (80 to 123) per 100 000, 23 (20 to 27) per 100 000, and 2 (1 to 2) per 100 000, respectively. Facial fractures were most concentrated in Central Europe. Falls were the predominant cause in most regions.ConclusionsFacial fractures are predominantly caused by falls and occur worldwide. Healthcare systems and public health agencies should investigate methods of all injury prevention. It is important for healthcare systems in every part of the world to ensure access to treatment resources.

ImportanceDespite federal initiatives encouraging the enrollment of individuals from racial and ethnic minority groups in US clinical trials, no studies to date have specifically examined demographic disparities among participants in phase 1 drug development trials for patients with metastatic cancer.ObjectiveTo assess trends in the enrollment of patients from racial and ethnic minority groups in US phase 1 therapeutic drug trials for metastatic cancer from 2000 to 2018.Design, Setting, and ParticipantsIn this cross-sectional study, ClinicalTrials.gov was queried in July 2021 to identify completed phase 1 drug trials for metastatic cancer in the US from January 1, 2000, to December 31, 2018, with published results, yielding 221 phase 1 trials with 8309 participants aged 18 years or older with metastatic solid tumors. Proportions of each racial and ethnic group of trial participants were compared with that from the North American Association of Central Cancer Registries’ Cancer in North America (CiNA) database. Statistical analysis was performed from July 12, 2021, to March 15, 2022.Main Outcomes and MeasuresFor each racial and ethnic group, the difference between trial and CiNA proportions was examined using a 2-sample test for equality of proportions with continuity correction.ResultsThe 8309 phase 1 trial participants (4198 men [50.5%]; median age, 59 years) included 23 American Indian or Alaska Native participants (0.3%), 371 Asian or Pacific Islander participants (4.5%), 514 Black participants (6.2%), 401 of 5076 Hispanic or Latinx participants (7.9%), and 7154 White participants (86.1%). Industry funded 165 of the 221 trials (74.7%). White patients were overrepresented overall compared with the corresponding CiNA cohort (7154 of 8309 [86.1%] vs 4 113 096 of 4 891 486 [84.1%]; difference, 2.0 percentage points; P < .001). There was an increase in overrepresentation of White patients from 2000 to 2011 (trials, 2780 of 3245 [85.7%]; CiNA, 2 378 019 of 2 800 711 [84.9%]; difference, 0.8 percentage points; P = .23) to 2012-2018 (trials, 4374 of 5063 [86.4%]; CiNA, 1 735 077 of 2 090 775 [82.9%]; difference, 3.5 percentage points; P < .001) and corresponding worsening representation of American Indian or Alaska Native patients (2000-2011: trials, 10 of 3245 [0.3%]; CiNA, 10 905 of 2 800 711 [0.4%]; difference, −0.08 percentage points; 2012-2018: trials, 13 of 5063 [0.3%]; CiNA, 9484 of 2 090 775 [0.5%]; difference, −0.20 percentage points), Asian or Pacific Islander patients (2000-2011: trials, 121 of 3245 [3.7%]; CiNA, 75 033 of 2 800 711 [2.7%]; difference, 1.1 percentage points; 2012-2018: trials, 151 of 5063 [3.0%]; CiNA 70 535 of 2 090 775 [3.4%]; difference, −0.75 percentage points), Black patients (2000-2011: trials, 244 of 3245 [7.5%]; CiNA, 322 701 of 2 800 711 [11.5%]; difference, −4.0 percentage points; 2012-2018: trials, 270 of 5063 [5.3%]; CiNA, 255 625 of 2 090 775 [12.2%]; difference, −6.9 percentage points), and Hispanic or Latinx patients (2000-2011: trials, 161 of 1792 [9.0%]; CiNA, 169 297 of 2 800 711 [6.0%]; difference, 3.0 percentage points; 2012-2018: trials, 240 of 3295 [7.3%]; CiNA, 156 118 of 2 090 775 [7.5%]; difference, −0.2 percentage points). Similar disparities were observed when comparing industry-funded and academic center–sponsored trials.Conclusions and RelevanceIn this cross-sectional study of participants in phase 1 clinical trials of drugs for metastatic cancer, worsening disparities were observed over time in the accrual of patients from racial and ethnic minority groups. These findings may represent widening inequalities in access to trial sites and worsening systemic biases. More efforts are needed to diversify phase 1 cancer drug trials to improve equity in access to new treatments and to ensure that safety and efficacy findings from early drug trials are generalizable across populations.

Context: Endogenous sex hormones are risk factors for postmenopausal breast cancer. A potential route for favorable hormonal modification is weight loss. Objective: The objective of the study was to measure change in plasma estradiol and testosterone levels in postmenopausal women in relation to change in body mass index (BMI) and plasma leptin. Setting: The setting was a cohort study of over 100 000 female volunteers from the general population, United Kingdom. Participants: The participants were a sample of 177 postmenopausal women aged over 45 years who provided blood samples during 2004–2005 and again during 2010–2011. Main Outcome Measure: Outcomes were percentage change in plasma estradiol and testosterone levels per 1 kg/m2 change in BMI and per 1 ng/mL change in plasma leptin. Results: Among women with reduction in BMI, estradiol decreased 12.7% (95% confidence interval: [6.4%, 19.5%]; P < .0001) per kg/m2 and among women with increased BMI estradiol increased 6.4% [0.2%, 12.9%] (P = .042). The corresponding figures for testosterone were 10.7% [3.0%, 19.0%] (P = .006) and 1.9% [−5.4%, 9.7%] (P = .61) per kg/m2. For women with decreases and increases in leptin, estradiol decreased by 3.6% [1.3%, 6.0%] (P = .003) per ng/mL and increased by 1.7% [−0.3%, 3.6%] (P = .094), respectively. The corresponding figures for testosterone were 4.8% [2.0%, 7.8%] (P = .009) and 0.3% [−2.0%, 2.6%] (P = .82) per ng/mL. Conclusions: In postmenopausal women, changes in BMI and plasma leptin occurring over several years are associated with changes in estradiol and testosterone levels. The results suggest that fat loss by an individual can result in substantial decreases in postmenopausal estradiol and testosterone levels and provides support for weight management to lessen breast cancer risk.

ObjectiveThis study investigated the association between syphilis seroprevalence and age among blood donors, and described the distribution of serological titres among syphilis-infected donors, aiming to confirm the syphilis epidemic characteristics and to promote effective interventions for older adults.MethodsData were obtained from the Shenzhen Programme for Syphilis Prevention and Control in 2014–2017. Blood samples were screened using the ELISAs, and confirmed using the Treponema pallidum particle agglutination assay (TPPA) and toluidine red unheated serum test (TRUST).ResultsAmong 394 792 blood donors, 733 tested TPPA and TRUST positive (active infection), and 728 tested only TPPA positive (historical infection). The overall prevalence of syphilis seropositivity was 370.1 per 100 000 (95% CI 351.1 to 389.0 per 100 000); the prevalence of active infection was 185.7 per 100 000 (95% CI 172.2 to 199.1 per 100 000). People aged ≥45 years displayed a prevalence of 621.8 per 100 000 in syphilis seropositivity and 280.5 per 100 000 in active infection, which were 3.8 times and 2.4 times higher than that for people aged <25 years, respectively. The prevalence of syphilis seropositivity (χ2trend=311.9, p trend<0.001) and active infection (χ2trend=72.1, p trend<0.001) increased significantly with age. After stratification by gender and year of donation, the increasing trend of prevalence with age remained (p trend<0.05), except for the prevalence of active infection in males and females in 2014. About 16.3% of donors with active infection and aged ≥45 years had a TRUST titre of ≥1∶8, lower than that of patients aged <25 years (51.3%) and 25–34 years (34.1%).ConclusionsThe findings confirm the high prevalence of syphilis among older adults, and suggest the need to increase awareness among healthcare providers and deliver more targeted prevention interventions for older adults to promote early testing.

O objetivo deste trabalho foi avaliar a distribuição do sistema radicular de plantas de videira, do porta-enxerto IAC572‘Jales’ sob a cultivar Niágara rosada no cerrado goiano em duas idades de plantas. As plantas avaliadas localizavam-se em áreas de Latossolo Vermelho distrófico, arranjadas no espaçamento 2,5mx2,5m. Avaliaram-se espessura e densidade das raízes a partir de amostras de solo coletadas à cinco distâncias do caule (0,25; 0,50; 0,75; 1,0; e 1,25m) e cinco profundidades (0,0-0,20m; 0,20-0,40m; 0,40-0,60m; 0,60‑0,80m; e 0,80‑1,0m). Utilizou-se o delineamento inteiramente casualizado, em esquema fatorial triplo 2x5x5 (idades x distâncias do caule x profundidades), com três repetições. A idade das plantas não influenciou na densidade. As raízes com diâmetro menor que 2mm concentram-se na camada de 0,0-0,20m de profundidade. Raízes grossas com diâmetro acima de 2mm concentram-se até 0,50m de distância do tronco e na profundidade de 0,20‑0,40m. A profundidade efetiva do sistema radicular foi encontrada a 0,60m, e a distância efetiva até 1,00m, sendo esta zona onde concentram-se 80% das raízes, já a partir do segundo ano de idade do pomar. The objective of this work was to evaluate the roots distribution of rootstock of vine plants IAC572'Jales' under Niagara rosada cultivar in cerrado of Goiás at two plant ages. The evaluated plants were located in areas of dystrophic Red Latosol, arranged in spacing 2.5mx2.5m. Root thickness and density were determined from soil samples collected at five distances of the stem (0.25, 0.50, 0.75, 1.0, and 1.25m) and five depths (0.0-0.20m, 0.20-0.40m, 0.40-0.60m, 0.60-0.80m, and 0.80-1.0m). A completely randomized design was used, in a triple factorial scheme 2x5x5 (ages x distances of the stem x depths), with three replications. The age of the plants did not influence the roots density. The finer roots with a diameter of less than 2mm are concentrated in 0.0-0.20m layer. Thick roots with a diameter above 2mm are concentrated up to 0.50m away from the trunk and at a depth of 0.20-0.40m. The effective depth and distance of the root system (80% of roots concentrated) were found at 0.60m and up to 1.00m, respectively, already from the second year old. Abstract (sommario):
У дисертації розглядаються основні принципи музичного розвитку піснеспівів, які розкрито через структурний аналіз мелодичної складової піснеспівів. Виявлено повторність на різних масштабно-тематичних рівнях. У роботі вперше бґрунтовано використання методу семіологічного аналізу для вивчення музичної форми піснеспівів сакральної монодії східного обряду. Аналізуючи основні жанри — ірмос і стихиру, за матеріалами українських нотолінійних ірмологіонів знайдено характерні повторні фраґменти та зроблено порівняння з їх відповідниками в невменній системі. Відзначено наявність спільних ознак та збереження тотожних контурів музичної форми в різних нотаційних системах за виявленою збіжністю послідовностей знаків ідентичного призначення від візантійського зразка через слов’янську музику до українських ірмологіонів ранньомодерної доби. Слов’янський невменний матеріал тісніше споріднений з лінійними записами, особливо у порівнянні повторності знаково-звукових комплексів. Візантійські невменні записи часто виявляють інші комбінації графічних форм, що свідчить про певну творчу еволюцію слов’янської церковної монодії.

This thesis deals with the principles of musical development in the church monody chants of the Byzantine rite. It is focused on the comparative study of the different musical systems and namely the Byzantine and Old Slavonic neumatic and Kyivan five-line square notation. Musical notation manuscripts dating back from the 12th through 17th centuries were used as source material for the study. Such approach allows an examination of the logical organization of the same composition notated in neumatic and linear systems. The continuity of the Byzantine chant tradition is observed through a combination of melodic phrases that create distinctive musical and verse forms.

The use of pre-fabricated concrete components and their related coupling systems in seismic engineering constitutes a subject of wide and deep interest among researchers, practitioners and manufacturers all over the world, as demonstrated by a large number of studies conducted, among other Countries, especially in Japan, New Zealand and United States since the early ‘80s and, in relatively more recent times, in Italy. A key issue is given by the possibility to apply the typical benefits of the pre-fabrication not only to low rise industrial/commercial structures, but also to multi-storey frames for public and strategic buildings such as schools, hospitals and many others, as well as to high-rise residential premises built in areas characterized by a medium to high seismic intensity. On the basis of what stated above, an original structural system made by prefabricated composite steel truss-concrete beams and centrifuged high-strength concrete columns is presented in the following. Specifically designed joints are provided to couple the different structural components in order to guarantee an overall ease of construction with reduced tolerance problems and self-bearing capacity during temporary erection phases, with a consequent reduction in schedule and costs. The use of high performance concrete for columns allows for a high bearing capacity with limited overall dimensions and the consequent maximization of the commercial or saleable space. The original layout of the system proposed has led to the need to perform an intensive theoretical and experimental research activity. The finite element model of the structural system was calibrated upon both static and cyclic testing evidence carried out on full scale samples built in Italy and tested at the Tongji University-Shanghai, China. On the basis of the data collected, the tuned model was used to carry out further analyses and to deepen the specific knowledge on several further aspects, as specified in the following. Firstly, an estimation of the joint’s strength domain, suitable for everyday’s design was carried out based on a component-approach. Then, a structural optimization on the component used to guarantee hogging and sagging bending moment resistance to the joint, was carried out in order to achieve the minimization of the construction material employed. Furthermore, the estimation of the seismic performance of the joint, based on the evaluation of a purposely defined vulnerability parameter, supplied encouraging results with reference to the applicability of the investigated technology over most of the National territory. Finally an improved layout of the joint, with reference to confined concrete and the related possibility to achieve a suitable seismic response also at edge joints, is presented.

The concept of "integral abutment bridge" has recently become a topic of remarka-ble interest among bridge engineers, not only for newly built bridges but also during refurbishment processes. The system constituted by the substructure and the superstructure can achieve a composite action responding as a single structural unit; the elimination of expansion joint and bearings on the abutments, greatly reduce the construction and maintenance costs. To maximize the benefits from integral abutment bridges, the direct way is to achieve the super-long integral abutment bridge. However, as the environment temperature changes, the lengths of superstructure increase and decrease, pushing the abutment against the approach fill and pulling it away. The responses of bridge superstructure, the abutment, the approach system, the foundation/piles and the foundation soil are all different. And it's important to understand their interactions effective design and satisfactory performance of integral abutment bridges. In order to build longer integral abutment bridges, therefore in this research, the lit-erature survey on the applications of integral abutment bridges in worldwide, espe-cially the current development of super-long integral abutment bridges was carried out firstly. Another literature review on soil-structure interaction was conducted to find out the most suitable methods in considering this kernel issue in design of integral abutment bridge. Through proposing finite element models for integral abutment bridges that could involve the soil-structure interaction, thermal actions, non-linearity in materials and so on, structural study was performed on an existing super-long integral abutment bridge, including parametric analysis, pushover analysis, and dynamic tests. Then, the performance of integral abutment bridge was better understood, and no critical structural problem was found for integral abutment bridge. Based on that, length limit for this kind of bridge was evaluated and investigated in an analytical way. Considering capacities of abutments and piers, and under the conditions of an existing integral abutment bridge, the length limit was found to be around 540m. With this super length, the piles need to be designed with capacity of large lateral displacement. Therefore, an effective optimization approach, associating the finite element method with global optimization algorithm was presented for pile shape design. At the end, considerations accounted in the design of super-long integral abutment bridges were discussed, making construction of super-long integral abutment bridge of great possibility.

Abstract (sommario):
Intellectual disability (ID) is the unavoidable hallmark and the most invalidating feature of DS, a genetic pathology due to triplication of chromosome 21. ID in DS is mainly due to neurodevelopmental alterations among which neurogenesis reduction appears to play a prominent role. Considering the time course of brain development and that the bulk of neurogenesis takes place prenatally, therapeutic interventions aimed at correcting brain developmental defects should be started prenatally. 7,8-DHF is a natural flavonoid that crosses the blood brain barrier, and specifically binds the TrkB receptor activating its downstream signaling cascade. We previously found that neonatal treatment with 7,8-DHF improves cellularity and restores dendritic pathology in the hippocampus of the Ts65Dn mouse, the most studied DS mouse model that largely mimics the human condition. Based on these premises the goal of the current study was to establish whether prenatal treatment with 7,8-DHF is able to rescue overall brain neurogenesis. Ts65Dn pregnant females received 7,8-DHF from embryonic day 10 until delivery. In 2-days-old pups, we evaluated the number of proliferating (BrdU+) cells in various forebrain neurogenic niches. Namely, the rostral and caudal subventricular zone, dentate gyrus, rostral and caudal neocortex, striatum, thalamus and hypothalamus. We found that in treated Ts65Dn mice the proliferation potency was improved or even restored in most of the examined regions. Moreover, treatment restored cellularity in the neocortex layer II but not in the hippocampus. These results show that 7,8-DHF has a positive influence on prenatal neurogenesis and cortical cellularity in the Ts65Dn mouse model, but, unlike neonatal treatment, it has moderate effects on hippocampal cellularity. A treatment with 7,8-DHF that starts embryonically and continues in the early postnatal period should be considered as a promising strategy for the improvement of overall brain development in DS.

AbstractTo face the major challenges posed by the increasing product variants with shortening life cycles and fluctuating market conditions, modular and adaptable assembly systems are used. Their strong dependency on software creates a new void in the planning and implementation processes of these assembly systems. Usually, the programmer fills this void based on his knowledge, which leads to frequent and large adaptations of the code base. This is rather counter-productive. To address this challenge, we developed a generic user-friendly graphical API (Application Programming Interface) for a process owner in our previous work (Müller et al, Proc CIRP 81:730–735, 2019, [1]). This API can describe any assembly system and the associated task and is used to get the boilerplate code needed to execute the process on a programmable logic controller (PLC)—the standard hardware used in the industry. In this paper, the virtual description of an assembly system used by the API is extended to include a goal-oriented task description by defining the process and the structure. We believe that this extension provides the proper abstraction needed by the process owner. In addition, this extension significantly reduces the modeling effort.

AbstractSince the 1990s, the European Union has committed to gaining global leadership in clean energies such as solar photovoltaic and wind power. The joint amount of wind and solar capacity grew from 12.5 GW (or 2% of total installed electricity capacity) in 2000 to 261.2 GW in 2018 (or 28.1% of total installed electricity capacity). This came at a cost: In 2018 alone, the European Union (excluding the United Kingdom) spent €73 billion to subsidize green energy production. These financial aids were paid for by European energy consumers, mainly through levies charged on top of their power bills. According to proponents, these subsidies were needed to achieve sustainability while promoting the emergence of the European renewable industry. This chapter focuses on the European venture into renewable energies to answer the following three questions: (1) Was the subsidization of green electricity sources an effective environmental policy? (2) Was it an effective industrial policy? (3) Was it an effective social policy? The answer is: no, no, no.

AbstractAdoptive cell therapy with BCMA-directed autologous CAR-T cells has shown very encouraging results in end-stage relapse and refractory multiple myeloma (MM), with overall response rates ranging between 73% and 96.9%, complete response (CR) rates between 33% and 67.9%, and MRD negativity in 50–74% of patients in the two largest phase 2 studies of ide-cel (idecabtagene autoleucel, KarMMa) and cilta-cel (ciltacabtagene autoleucel, CARTITUDE 1) reported thus far (Madduri et al. 2020; Munshi et al. 2021). Unfortunately, responses are usually not maintained, and no plateau has yet been seen in the survival curves. The median progression-free survival (PFS) in the KarMMa study of ide-cel was 8.8 months (95% CI, 5.6–11.6) among all 128 patients infused, increased to 12.1 months (95% CI, 8.8–12.3) among patients receiving the highest dose (450 × 106 CAR + T cells) and increased to 20.2 months (95% CI, 12.3–NE) among those achieving a CR. In the CARTITUDE-1 study, with a median follow-up of 12.4 months, the median PFS has not yet been reached, and the 12-month PFS rate was 76.6% (95% CI; 66.0–84.3). The absence of a clear plateau in PFS differs from what has been observed in DLBCL or B-ALL with currently approved CD-19-directed CAR-T cells, where (albeit with a shorter PFS and lower rates of CR) patients remaining free from relapse beyond 6 months are likely to enjoy prolonged disease control or even be cured.

AbstractSince 1978 and the initial French data protection law (Loi n°78-17 du 6 Janvier 1978), consecutive modifications regarding the protection of personal health data, especially in 2004, 2016 and 2018, set up a strict legal regime for processing sensitive personal data, including for research purposes. In recent years, French law has evolved proactively and in parallel with the work of the European Union (EU) on the preparation of what became the General Data Protection Regulation (GDPR), which has been in force since May 2018. This Chapter performs a state-of-art analysis (as of 1 July 2019) of the French legal framework for research biobanks and data protection rules applying to biobanking, in particular those related to data subjects’ rights and Article 89 of the GDPR. Firstly, it provides updated information about the national landscape of active research biobanks in France (Sect. 1). Secondly, it explores how the French law embodies the developments brought by the GDPR and how it envisages individuals’ rights in the context of research biobanking (Sects. 2 and 3). Thirdly, this Chapter analyses existing and potential national exemptions to individuals’ rights, including with regard to Article 89 GDPR, and how France conceives of processing activities of ‘public interest’ (Sect. 4). Finally, the authors address ongoing debates around bioethics law in France and argue for the creation of a specific Act focused on biobanking as a means of integrating, clarifying and developing not only data protection rules but also other activities related to samples, human or not, in a unique, operational and compact act (Sect. 5).

Abstract The objectives of this study were (1) to provide an overview of review studies on hope in health care, and to describe (2) conceptualizations of hope, (3) antecedents and consequences of hope, and (4) ethical topics related to hope. Electronic databases were searched and 73 review studies were selected and thematically analyzed. Hope was conceptualized as (a) an expectation: appraisal of a future outcome, (b) resilience: endurance of adversity, and (c) a desire: expression of meaning. Opposite concepts to hope were fear/anxiety, hopelessness, despair, and depression. Inspiring relationships, particularly relationships with peers, were an important factor that increased hope in patients. Losses, like the loss of health or (inspiring) relationships, had a negative impact on hope. Also, hope had effects on motivation for change and making decisions. The ethics of hope was addressed in palliative care, where health care providers wanted to maintain patients’ hope while being honest or realistic. In conclusion, this chapter offers an overview of hope in health care and offers conceptual clarification, including ethical issues related to hope. Future studies should broaden the ethics of hope by including other values than realism and they should include the hope of health care providers.

1.- OBJETIVOS: dar a conocer a la poblacion mundial la gran responsabilidad en el area medica, psicoterapeutica y de acompanamiento sobre las personas que tienen una patologia dual asociado al consumo de sustancias psicoactivas. 2.-MATERIAL Y METODOS: utilizamos un tipo de investigacion documental,asi como su diseno cuantitativo y cualitativo (mixto) y la explicacion descriptiva utilizando tambien materiales derivados de la observacion clinica, asi como algunas pruebas psicometricas derivadas de medir psicosis, ansiedad,depresion (como scl-90, beck para ansiedad y depresion)asi como la prueba assitt de medicion de sus grado de consumo de sustancias asi como las historias clinicas psicologicas y medicas para determinar si el uso y abuso proviene de alguna situacion de la ninez o adolescencia o meramente por solo el uso de sustancias psicoactivas. 3.-RESULTADOS Y CONCLUSIONES:el 94% de los usuarios hombres pudimos detectar que antes de consumir sustancias fueron vulnerables a decir que si a las drogas o alcohol y que tenia una relacion con la hiperactividad o deficit de atencion o las dos juntas (mixta). Tambien pudimos investigar que el 78% de los usuarios mujeres pudimos detectar que antes de consumir sustancias fueron vulnerables a decir qque si a las drogas o alcohol y que tenia una relacion con el tlp (transtorno limite de personalidad y sus derivados. Tambien dentro del la exploracion nos dimos cuenta de la importancia de diagnosticar como transtorno mental o del aprendizaje y su diferencia en su aplicación del tratamiento contra las adicciones.

Introdução: A quimioterapia da doença de Chagas tem demonstrado nítidas diferenças de suscetibilidade ao Benzonidazol (Benz). Caracterizações biológicas e moleculares realizadas por diversas técnicas classificaram as cepas de Trypanosoma cruzi em diferentes graus de suscetibilidade como a Y (suscetível) e a Colombiana (resistente). Esses achados revelam que o tratamento com Benz, além determinar a destruição do parasito tem ação sobre o sistema imunológico. Objetivo: analisar a influência do tratamento com o Benzonidazol sobre a resposta humoral em camundongos infectados com as cepas Y e Colombiana. Material e métodos: Foram utilizados 150 camundongos, subdivididos em grupos experimentais: Infectados tratados cepa Y (YT) e não tratados (Y-NT); Colombiana tratados (COL-T) e não tratados (COL-NT), Tratados não infectados (TNI) e Controles sem tratamento (CI). O inóculo foi de 1,0 x 104 por via intraperitoneal. A parasitemia foi iniciada no sétimo dia nos animais infectados. Os procedimentos experimentais estiveram de acordo ao protocolo CEUA, 013/09. O tratamento foi iniciado no pico parasitêmico de cada cepa, sendo no 7º dia após a infecção nos animais infectados: pela cepa Y e, nos tratados e não infectados, no 18º dia de infecção com os da cepa Colombiana. A quimioterapia foi realizada em 60 doses (100mg/kg/dia de Benz). Investigou-se a resposta humoral pela sorologia (Imunofluorescência indireta) e pela reação de Elisa. Resultados: Os títulos sorológicos confirmaram a susceptibilidade e resistência, pois, variaram de negativo nos grupos YT - 1/640 e de 1/80 a 1/640 Y-INT, e no grupo COL-T, de 1/80 a 1/2560 COL-NT, de negativo a 1/10 nos grupos TNI e CI. A dosagem das imunoglobulinas IgM, IgG1, IgG2a e IgG total, mostraram-se mais elevadas no grupo COL-NT em relação aos grupos avaliados, o que indicam auxílio dessas imunoglobulinas na resposta celular citotóxica mediada. Conclusão: Os dados sugerem que o tratamento com Benzonidazol foi capaz de influenciar a resposta imunológica humoral em camundongos.

INTRODUÇÃO: A síndrome de Takotsubo (STT) é caracterizada pela disfunção transitória do ventrículo esquerdo com ausência de coronariopatia significativa. Ocorrem anormalidades de movimentação de segmentos ventriculares, como hipocinesia, discinesia e acinesia. Estima-se que 90% dos casos ocorra em mulheres pós-menopausa. A sintomatologia assemelha-se à da Síndrome Coronariana Aguda e à do Infarto Agudo do Miocárdio, podendo ser precedida por gatilhos físicos e emocionais negativos ou positivos. METODOLOGIA: Realizou-se a coleta de dados a partir da análise de prontuário eletrônico e anamnese por videoconferência. O Termo de Consentimento Livre e Esclarecido foi assinado pela paciente. DESCRIÇÃO DO CASO: Mulher de 78 anos, após episódio de estresse emocional exacerbado devido à discussão familiar, foi admitida no pronto-socorro de Balsas com plenitude pós-prandial, hiporexia e dor torácica aguda em compressão. Iniciaram-se os cuidados intensivos e, com o intuito de identificar a existência de obstruções nas artérias coronárias ou lesão do miocárdio, solicitou-se a cineangiocoronariografia, a qual foi realizada no dia 18/09/2020 e evidenciou ausência de coronariopatia significativa e ventriculografia com balonamento por hipocinesia apical e discinesia das demais paredes. No dia 23/10/2020, compareceu ao ambulatório em Imperatriz para atendimento. Apresentou histórico clínico de Hipertensão Arterial, Diabetes Mellitus, Ansiedade, Insuficiência Cardíaca, Sarcoidose e Tireoidectomia há 7 anos. Após 4 dias, realizou-se o Ecocardiograma Transtorácico (ECOTT), que evidenciou hipertrofia do ventrículo esquerdo, discreto aumento atrial esquerdo, função sistólica biventricular preservada, Fração de Ejeção (FE) de 57%, insuficiência mitral moderada. Passado 1 ano e 4 meses do último atendimento, a paciente compareceu com tosse persistente e variações pressóricas. Eletrocardiograma: ritmo sinusal, bloqueio atrioventricular de 1º grau, bloqueio divisional anterossuperior esquerdo e bloqueio incompleto de ramo esquerdo. ECOTT: disfunção diastólica de 1º grau e melhora da “FE” para 65% e dosagem de NT-ProBNP de 665 pg/mL. CONCLUSÃO: Portanto, esse relato descreveu um caso de STT que, apesar da gravidade clínica, evoluiu de forma satisfatória com a resolução do quadro clínico agudo e teve o período de 18 meses de acompanhamento médico. Conclui-se que essa paciente necessita estar sob constante monitorização e assistência médica, uma vez que está em uso de polifarmácia, apresenta comorbidades concomitantes e fatores associados à recorrência da síndrome.

Abstract (sommario):
This paper describes the most important characteristics of cloud computing paradigm in order to be used as powerful arrows towards efficient approaches in teaching data analysis. Also High Performance Computing aspects are considered for solving large complexity data analysis problems. The usage of both Cloud Computing and High Performance Computing will assure not only an excellent framework for research but also a powerful and flexible environment for e-Learning. The presentation is structured in three sections, being closed with the list of references. The first section considers the resources of cloud computing: SaaS (Software as a Service), IaaS (Infrastructure as a Service), PaaS (Platform as a Service - predefined integrated platform), and Managed Services. For every mentioned resource the most important features which are valuable for e-learning are outlined. High Performance Computing for data analysis is described in the second section. Also, software tools useful to analyse collections of data having small, medium or large size are presented and compared in order to identify the best tool to be used in a virtual learning framework. The third section describes both data manipulation techniques and results of experiments. Finally, concluding remarks are presented related to an online course on data analysis based on cloud computing paradigm. Selected references 1. Abstract (sommario):
The results of sequencing and bioinformatics analysis of genomic changes in 9 F1 progeny of males from the isogenic line D. melanogaster irradiated by Co60 Y—rays at a dose of 40 Gy (LD85) and in 3 control samples are presented. In 9 progeny from irradiated males, a total of 46 genomic changes (32 significant and 15 mosaic de novo mutations) were found, which is equal to a frequency of 5.2 mutations/genome. The spectrum of changes included 33 deletions (17-78 000 bp in size), 4 duplications (322-1371 bp), 4 reciprocal translocations and 6 inversions in X, 2 and 3 chromosomes. In 3 studied control samples, 2 deletions (98 and 128 bp in length) were found in 3 chromosome (frequency - 0.66 mutations/genome). This shows that in the progeny of irradiated males, the frequency of de novo mutations at the genome level is 7.9 times higher than in the control, even without taking into account base substitutions and indels, the analysis of which is ongoing. Almost half of the identified structural changes in the genome affect coding genes. Thus, the results show that next-generation genome sequencing can detect a much wider range of mutations of any size. This indicates a much higher genetic hazard of sparsely ionizing radiation than previously thought.

In early 1994 a 100 MW LM6000 combined cycle cogeneration plant will begin operation at New York City’s John F. Kennedy International Airport. Thanks to the extremely high simple cycle efficiency of the LM6000 gas turbine (8200 BTU/KWH, 8650 kJ/KWH-LHV dry) and a sophisticated three-pressure steam generating system, a heat rate below 7000 BTU/KWH-LHV (7380 kJ/KWH-LHV) is expected when operating in combined cycle mode. The dual-spool LM6000 achieves its efficiency by means of a 30:1 compression ratio. 2100 F. (1149 C.) firing temperatures and the direct coupling of the low compressor/turbine rotor to the electric generator. The efficiency of the heat recovery steam generator results from the use of three economizers, three evaporators and two superheaters combined with a patented feedwater heating system which yields a 245 F. (118 C.) exhaust stack temperature. Operating flexibility is essential in this application. While the dual-fueled plant is designed for pure combined cycle operation, most of the time it will operate in a cogeneration mode — producing up to 250 × 106 BTU/HR (264 × 106 kJ/HR) of steam for heating in the winter and 7000 (24,618 KW) tons of chilling for air conditioning airport terminals in the summer. The waste heat boilers are designed to be supplementary fired on gas fuel when the airport requires the 110 MW maximum capacity of the plant simultaneously with the maximum thermal load of 250 × 106 BTU/HR (264 × 106 kJ/HR). NOx emissions are controlled with a combination of water injection in the turbine combustors and a dual-function catalyst SCR/CO converter. CO is controlled by means of the converter. Combined gas turbine and duct burner NOx is maintained below 9.0 PPMV dry (@ 15% O2) and CO below 1.5 PPH (0.68 KG/HR) dry (@ 15% O2) when operating on gas fuel. Cycle details, equipment selection and operation as well as the plant economics provide a useful insight into the benefits of these recent developments in gas turbine and heat recovery combined cycle cogeneration technology.

Externally fired gas turbine (EFGT) cycles in combination with a biomass-fueled, atmospheric circulating fluidized bed (CFB) furnace are investigated for small scale production of power and district heat. Both closed and open gas turbine cycle configurations are considered. The thermodynamic performance analysis includes an evaluation of cycle-to-furnace heat matching and cycle-side pressure loss considerations. An economic analysis is presented for the various EFGT configurations, and comparisons are made with existing and emerging alternatives for small-scale biomass cogeneration. Simulation results show that thermodynamic performance varies between the different configurations and working fluids, with electric efficiencies in the range of 31–38% (LHV) and total efficiencies of 86–106% (LHV). An economic evaluation for an 8 MWf plant shows that the specific capital cost is about 3200–3900 $/kWe for a first commercial plant. The turbomachinery and the CFB furnace dominate the total plant cost, with each contributing about 1/3 of the total installed equipment cost. The cost of electricity, COE, is estimated to be 73–84 $/MWhe for 4 000 full load hours per year in a cogeneration application.

INTRODUCTION Trypanosoma brucei rhodesiense human African trypanosomiasis (r-HAT), the zoonotic, acute form of sleeping sickness in eastern Africa, is lethal if untreated. Today, only one arsenic- based, neurotoxic drug, melarsoprol, is available for treatment of the advanced meningo-encephalitic stage. A new oral treatment would simplify HAT elimination as proposed by WHO. Fexinidazole was recommended by the European Medicines Agency (EMA) in 2018 as the first oral treatment for Trypanosoma brucei gambiense HAT, but it was not yet evaluated for r-HAT. METHODS This single-arm clinical trial, sponsored by DNDi, began in October 2019 and tested fexinidazole treatment in patients with r-HAT as an alternative to existing treatment in Malawi and Uganda. Patients (aged ≥6 years) with both stages of the disease were recruited up to the target of 34 patients with stage 2 disease evaluable at the end of hospitalization. Patients were hospitalised during the 10 days of treatment and followed up to 12 months after hospital discharge. The primary outcome was r-HAT-related or treatment-related fatality at the end of hospitalisation in patients with stage 2 disease and was compared with an unacceptable fatality rate of 8.5%, a threshold defined according to results from a previous clinical trial with melarsoprol. This trial is registered with ClinicalTrials.gov, NCT03974178. RESULTS 45 patients received treatment between 1 October 2019 and 28 November 2021 (35 [78%] had stage 2 disease and ten [22%] stage 1, 31 [69%] were male and 14 [31%] female, with median age of 24 years). The primary efficacy result of the clinical trial, analysed among the 34 evaluable patients, was achieved, with no r-HAT-related or treatment-related deaths during hospitalisation (0.0%, 95% CI 0.0–8.4), compared with a benchmark of 8.5% lethality attributable to melarsoprol. Safety was acceptable, with no severe adverse events related to fexinidazole; and one patient relapsed. CONCLUSION Fexinidazole, an oral treatment, was shown to be a good alternative to existing injectable and toxic drugs. Results were submitted for EMA regulatory review in preparation for use in endemic countries, and a positive scientific opinion was received in December 2023 to extend the indication of fexinidazole for the treatment of r-HAT. Fexinidazole is expected to be deployed in 2024 as a new r-HAT therapeutic.

ABSTRACT Introduction: Paneurhythmy is a unique Bulgarian and European recreational physical activity, appropriate for a proactive approach in maintaining and improving health. The aim of the present study is to find out if there are changes in the affective balance and the feeling of happiness with adults practicing Paneurhythmy. Method: The subjects (n=155) of the experimental study were adults (18-69 years old), divided into two groups: an experimental group (EG, n=77, mean age 41.7), participating in a course of Paneurhythmy, and a control group (CG, n=78, mean age 41.8). Paneurhythmy courses were held indoors in the evening during the autumn-winter season. The experimental group subjects conducted an average of 22 Paneurhythmy lessons (SD 7.23). The following questionnaires were used to measure the effect of the experiment: 1) Fordyce Emotions Questionnaire (FEQ) – Fordyce, 1988, Bulgarian adaptation by Zsheliaskova-Koynova & Ianovska (2008). 2) PANAS (Watson, Clark & Tellegen, 1988), Bulgarian version by Yordanova (2006). Results: Comparing the results of the EG and the CG, we found the following significant differences: higher positive emotions (t152=2.67, p=.008), lower negative emotions (t152=2.15, p=.033) and stronger feeling of happiness (t146=2.65, p=.009) with the EG after the Paneurhythmy training period. No changes were found in the CG for the same period. Conclusions: Paneurhythmy training significantly improved the affective balance and the feeling of happiness in adults who trained indoors during the autumn-winter season.

This report presents the geochemical data, quality assurance and quality control (QA/QC) results of the reanalysis of lake sediment samples collected from east-central Labrador (NTS 013-I, 013-J, 013-K, 013- N, and 013-O). The original lake surveys were conducted in 1977, 1978, 1983, 1987 and 1992, and the reanalysis was initiated in 2022. Original survey results are presented in Geological Survey of Canada open files 513, 558, 997, 1636, 1637, 2645, 2646, 2648, and 2650. Lake sediments from 3 736 archived site samples were re-analyzed, covering an approximate area of 44 000 km2, with an average density of 1 sample per 18 km2. Samples were measured for 65 elements via modified aqua-regia - ICP-MS. To ensure high quality geochemical data, the data were evaluated for accuracy, precision, and fitness-forpurpose. QA/QC results have identified several elements to be monitored carefully for future analyses.
