Abstract (sommario):
Context Medical documentation is a required component of patient care in all health care professions. Objective To evaluate athletic trainers' perceived behaviors toward, barriers to, and confidence in their medical documentation. Design Cross-sectional study. Setting Web-based survey. Patients or Other Participants We purchased a list of randomly selected e-mails from the National Athletic Trainers' Association. Of the 9578 participants, 1150 accessed our questionnaire (12.0% access rate), 1053 completed at least 1 portion, and 904 completed the questionnaire in its entirety (85.8% completion rate). Of the participants, 60.1% (n = 569/947) were female, 66.6% (n = 632/949) held a master's degree, 39.3% (n = 414/1053) worked in a collegiate or university setting, and 36.1% (n = 381/1053) worked in a secondary school setting. Main Outcome Measure(s) We used a 31-item questionnaire with demographics (12 items), medical documentation behaviors (16 items), barriers (2 items), and perceptions (1 multipart item) sections. The questionnaire explored athletic trainers' behaviors as well as confidence in, comfort with, and concerns about their documentation practices (Cronbach α = 0.887). We used descriptive statistics to identify characteristics of central tendency (mean, standard deviation, mode, frequency). Results Participants always (45.7%, n = 478/1046) or very frequently (39.0%, n = 408/1046) used a double documentation system consisting of electronic and paper records (50.4%, n = 523/1038). They most often documented to provide legal protection (86.8%, n = 914/1053), because it is a legal obligation (79.1%, n = 833/1053), or to track patient progress (83.9%, n = 883/1053). The most frequently cited barriers to medical documentation were time (76.5%, n = 806/1053), managing too many patients (51.9%, n = 547/1053), technological concerns (17.2%, n = 181/1053), and software limitations (17.2%, n = 181/1053). Respondents believed they were competent, comfortable, and confident in their documentation practices. Conclusions We were able to evaluate the generalizability of previous research while adding to the understanding of the behaviors toward, barriers to, and perceptions about medical documentation. We confirmed that time and patient load affected the ability to perform high-quality medical documentation.

Abstract (sommario):
Purpose: To introduce the novel effective Y-shaped silicon stent for treating canalicular laceration. Methods: Fifty-four canalicular laceration cases (54 eyes) underwent the lacrimal surgery using the novel Y-shaped silicon stents were collected, and the stents were removed 3 months after surgery. All cases were followed up for 12 months. The operability of stenting and removing the stent were assessed. The data we collected during each follow-up are: stent position and integrity, aesthetic results, the discomfort, the symptom of epiphora, the patency of lacrimal passage, complications, and patient’s satisfaction. Results: The stenting time was 14.3±5.8 min and the time of removing it was 1.8±0.5 min. Both of the operability of stenting and removing were accessed to be easy in more than 94.4% cases. All stents were in position and integrated. 51 cases (94.4%) had no problems on aesthetic results. Three patients (5.4%) complained about the discomfort. Two cases (3.7%) had the symptom of epiphora, which were obstructed after removing the stents. Fifty-two cases (96.3%) were with patent canaliculus and satisfied with the surgery and symptom improvement. Only 5 cases (9.3%) had complications. Conclusion: The novel Y-shaped silicon stent is an effective stent for treating canalicular laceration.

Abstract Context: Sex differences in dynamic measures have been established in physically mature populations. Gaining information on maturation's effect on dynamic performance measures implicated in injury risk may enable us to better design injury prevention programs. Objective: To examine sex differences in dynamic valgus alignment and triple-hop distance measures across maturational stages in males and females. A secondary purpose was to determine if a field test of strength and power predicts dynamic valgus alignment. Design: Cross-sectional study. Setting: Laboratory. Patients or Other Participants: 157 young athletes (78 females, 79 males) aged 9 to 18 years. Intervention(s): Subjects performed drop-jump landings and single-leg triple-hop tests as part of a broader injury screening. Main Outcome Measure(s): Maturational status was ascertained from self-report questionnaires and grouped according to Tanner stages 1 and 2 (MatGrp1), 3 and 4 (MatGrp2), and 5 (MatGrp3). Frontal-plane knee valgus displacement, which served as a measure of dynamic valgus alignment, and single-leg triple-hop distance were assessed. Results: Males demonstrated less dynamic valgus alignment during drop jumps in the latter maturational stages (MatGrp1 = 13.1° ± 8.7°, MatGrp2 = 9.0° ± 6.2°, MatGrp3 = 9.2° ± 9.4°), whereas females increased dynamic valgus alignment throughout maturation (MatGrp1 = 11.5° ± 6.9°, MatGrp2 = 12.8° ± 8.8°, MatGrp3 = 15.5° ± 8.7°). Thus, in the more mature groups, males had less dynamic valgus alignment than females. Both males (MatGrp1 = 393.5 ± 63.7 cm, MatGrp2 = 491.8 ± 95.1 cm, MatGrp3 = 559.3 ± 76.3 cm) and females (MatGrp1 = 360.3 ± 37.1 cm, MatGrp2 = 380.1 ± 44.3 cm, MatGrp3 = 440.0 ± 66.2 cm) increased triple-hop distance, but males increased more. Within each subgroup of MatGrp and sex, triple-hop distance had no predictive ability for dynamic malalignment. Conclusions: When dynamic valgus alignment and strength were assessed, sex and maturational status displayed an interaction. However, functional strength did not predict degree of dynamic valgus alignment.

Abstract (sommario):
The works of Mikhail Fedorovich Istomin (1821-1862) are part of the unexplored and, until recently, not included in the history of Komi literature material - essays created by Komi writers of the XIX century in Russian language. Meanwhile, the work of M. Istomin is very representative both in terms of the formation of General regional characteristic of all the literature of the North, and the ways of familiarizing Komi intelligentsia to literary work. The analysis of works of M. Istomin made in this work, allows to judge about actual for the writer-“foreigner” of the XIX century connection to all-regional and all-Russian literary process, and also about a problem of creation of a native literary portrait for different people of the North. The description of the territory, its geographical and climatic features, the creation of descriptions of places was chosen by the writer as a priority, the most important task of creativity. This allows him to embody fragments of the Northern world through the focus of view of autochthon, to identify and record the most important, especially important for the northerner, locations and objects of environment. The writer paid special attention to the rivers as the most important geographical and natural attractions of the region. In essence, the North in the essays of M. Istomin takes the form of world saturated with many waters. The writer is far from symbolizing natural objects, does not endow their images with figurative meanings, at the same time, many descriptions of different rivers in his essays indicate the movement of regional and Komi literature by the way of creation its own concept of the North as a natural environment-centered world.

The article analyzes Udmurt traditional beliefs and rules that regulate human behavior before going to bed, during a sleep and after waking up. The regulations are shaped by the perception of a sleep as the liminal state and as a surprising combination of space and time in which boundaries between this world and the other world are blurred. Compliance with the regulations is supposed to guarantee safety: it can protect the sleeper from the lurking danger during and after a sleep, or in case if the dream foreshadows the upcoming negative changes in life. The main apotropaic measures used before going to bed and after waking from sleep are an appeal to God, warding off a bad dream by water or spitting on excreta, hiding / placing the sleeper in a safe space, creating a barrier, and propitiating the dead. Some rules are aimed at preventing or regulating communication between the living and the dead in dreams, while others, on the contrary, simulate meeting and communicating with the deceased in a dream. It has been revealed that some sleep-related traditional beliefs and prohibitions have disappeared so far - the information about them can only be found in archival and published materials nowadays, while others continue to actively exist, undergoing certain transformations caused primarily by the influence of Christian views.

Major and trace elements (Al, As, Ba, Ca, Cd Co, Cr Cs, Cu Eu Fe, Hf, K, La, Lu, Mg, Mn, Na, Ni, Pb, Rb, Sb, Sc, Si, Sm, Sr, Ta, Tb, Th, Ti, U, V, Yb and Zn) in forest soils from three acid-deposition-affected locations in southern Quebec were investigated. Total element concentrations in the surface layer (0–20 cm) of the Podzolic soils were mostly well within the range of element concentrations in Podzols reported worldwide but showed higher mean values for most elements. The mean concentrations of elements (except Zn, at 124 μg g−1) were also generally comparable to the respective elemental baseline data established for Canadian soils. Element concentrations (μg g−1) of Cs (6.2) Sc (19 8) V (106) Rb (194) K (26 500) and Al (76 900) in the surface soils were significantly higher at the 940-m elevation than the concentrations from the sites at 650 and 770 m along the Roundtop Mountain slope, but Ca (1120) and Ni (4.4) were lowest at the 940-m elevation A similar trend with elevation was not observed at the other mountain location (White Peak). Enrichment-factor (EF) analysis showed significant element enrichment (EF ≥ 10) for Cd, Rb and Sb but impoverishment (EF ≤ 0.1) for Ca, Cu and Ni in topsoils. Abstract Background While morbidity attributable to podoconiosis is relatively well studied, its pattern of mortality has not been established. Methods We compared the age-standardised mortality ratios (SMRs) of two datasets from northern Ethiopia: podoconiosis patients enrolled in a 1-y trial and a Health and Demographic Surveillance System cohort. Results The annual crude mortality rate per 1000 population for podoconiosis patients was 28.7 (95% confidence interval [CI] 17.3 to 44.8; n=663) while that of the general population was 2.8 (95% CI 2.3 to 3.4; n=44 095). The overall SMR for the study period was 6.0 (95% CI 3.6 to 9.4). Conclusions Podoconiosis patients experience elevated mortality compared with the general population and further research is required to understand the reasons.

The research objective was to study the qualitative composition and physicochemical properties of raw game meat obtained from the area near the Beloosipovo mercury deposit in order to define any possible contamination w ith xenobiotics. Study objects and methods. The research featured rib eye muscle tissue and soft flesh of elks shot on the hunting farms of the Kemerovo Region aka Kuzbass. Results and discussion. A complex set of experiments revealed the chemical composition of elk muscle tissue and flesh, as well as the mineral composition of elk muscle tissue. The samples were obtained from different parts of carcasses. The amino acid and fatty acid composition of elk muscle tissue made it possible to describe the biological value, mineral composition, and vitamin profile of elk meat. The physicochemical analysis included toughness, cooking losses, and moisture-retaining capacity, i.e. the properties that ensure juiciness. The research also featured the accumulation of xenobiotics in elk meat samples obtained from the biosinosis near the Beloosipovo merc ury deposit. Conclusion. The slaughter yield of elk meat was 51–53%, which exceeds the average yield of farm cattle meat by 4–6%. The moisture content was 73–78%, while the content of protein was between 20–24% and depended on the anatomical location of the muscle sample; the fat content reached 0.75–1.75%. The mercury accumulation at different storage temperature conditions ranged from 0.004 ± 0.001 to 0.009 ± 0.001 mg/kg, while the max imum allowable concentration of mercury is 0.03 mg/kg.

e21510 Background: In Children and adolescents aged 0-14, each year more than 200.000 new cancer cases are expected at global level. For the planning and implementation of an effective pediatric cancer control program, pediatric cancer registries are essential. The long term survival rates have been improved to 85% in high income countries, however it is still less than this in LMICs. This work presents the most updated results of the pediatric cancer registry in Turkey. Methods: Turkish Pediatric Oncology Group and Turkish Pediatric Hematology Association has established the Pediatric Cancer Registry in 2002. The childhood cancer cases registered between 2009-2018 was included in this analysis. International Childhood Cancer Classification System was used for the classification. Essential demographic findings, ICD-O-3 morphology and topography codes were recorded for each case. Results: During the 10 years from 2009 to 2018, 15713 cases were registered. For all cases, median age was 6.7 year (0-17; M/F 8838/6867, 3 hermaphrodite, 5 unknown). Age distribution was 0-4 yrs, 40.7%; 5-9 yrs, 24.4%; 10-14 yrs, 23.2%; 15-19 yrs, 11.7%) The distribution of the tumor types were [number of cases, percentage of total, median age yrs, M/F]: Leukemia (4368, 27.8%, 5.4, 2519/1849); Lymphoma & other RES tumors (2996, 19.1%, 9.7, 2012/979, 1 hermaphrodite & 4 unknown); CNS [brain & spinal] (2089, 13.3%, 7.1, 1142/947); Symphatetic system (1243, 7.9%, 2.4, 650/593); Retinoblastoma (358, 2.3%, 1.4, 204/154); Renal (788, 5.0%, 3.3, 369/419); Liver (260, 1.7%, 1.8, 143/117); Malignant bone (1030, 6.6%, 12.6, 566/464); Soft tissue sarcomas (1052, 6.7%, 7.4, 611/441); Germ cell (971, 6.2%, 8.4, 346/622, 2 hermaphrodite, 1 unknown); Carcinoma & other malignant epithelial (462, 2.9%, 13.7, 226/236); Other/non-specific malignant (96, 0.5%, 7.8, 50/46). Five year survival rate was found as 70.8%. Conclusions: This registry has been used widely among health care professionals since its establishment in 2002. Survival rates for children and adolescents has been improved to 70%. This level of survival is at the acceptable level for an upper middle income country. This registry became a useful source for investigator and decision makers at national and international level.

The beginning of the 21st century opened a new page in the development of Komi literature connected with the activity of female authors earlier unprecedented on a dramatic field. In the 2000s Nina Kuratova, Elena Kozlova and Nina Obrezkova addressed dramaturgic genres. By this time they had already been popular writers. Having addressed a dramaturgic genre, new to itself, each writer introduced organic features in a palette of dramaturgic poetics, enriching and updating national culture. We devoted this work to research the specifics of Komi female dramatic art. Gender approach and method of the comparative analysis allowed us to reveal features of Komi female dramatic art. The Komi female dramatic art is not stereotypical and cliched, there is no similarity in the creation of plots and the features of characters in plays by the playwrights. N. Kuratova's, E. Kozlova's and N. Obrezkova's plays differ on the one hand from each other, on the other hand from plays by male Komi playwrights. It is not only about female playwrights reproducing in plays the female character and especially female household reality, but also about reproduction of how women feel and understand the world. In the Komi female drama we see the desire of the woman to live in harmony with the world, to constantly feel safe, to arrange family cosiness, to keep and transfer to the subsequent generations family values, love and respect (Kuratova). Also female drama transmits that pain which is felt by the woman, building relations with men, feeling their pressure and even violence (Kozlova). The female drama reveals changes the woman in modern society experiences, her finding new social roles, finding male features against the background of spiritual weakening of men (Obrezkova). So, the female consciousness gets into Komi dramaturgic space, opening new ways in an artistic judgment of reality, enriching an aesthetic paradigm.

The article is an analysis of the results of sociological surveys conducted by its authors in Mari El, Mordovia, Komi, Udmurtia in the fall of 2018.The study was conducted according to the methodology of the Ethnological Monitoring and Early Warning Conflict Network of the Institute of Ethnology and Anthropology of the Russian Academy of Sciences and was aimed at identifying and assessing conflict and integration, the potential of the local population in the field of migration relations. On the whole, these polls revealed a rather high degree of tolerance of the residents of these republics towards migrants, including foreign workers from Central Asia. Manifestations of migrant phobia in figures and comments reflected the opinion of a smaller part of respondents, which, however, is a significant proportion. Respondents indicated both positive and negative aspects and consequences of labor migration. Most respondents are still inclined to consider migration as a positive phenomenon. As recommendations, it is necessary to indicate the need for further intensification and further improvement of measures aimed at helping to adapt and stimulating the process of integration of labor migrants of various ethnic groups into regional societies. Informing migrants about the cultural features of the “host country”, involving them in national and local events and holidays (City Day, Russia Day, various ethnocultural festivals) should be carried out more often, also, being more consistent and less formal is also favorable. In addition, do not underestimate the potential of traditional faiths that “patronize” migrants. In addition to the socio-economic context, the effective integration of migrants is an important incentive for cultural development and for the development of dialogue of cultures.

The article focuses on one of the pressing problems of modern humanitarian knowledge - the problem of self-identification of hybrid cultures, in particular, the culture of the Finnish Americans. The ethnic diversity of the United States leads to a constant intersection of various cultures, giving rise to the uniqueness of contemporary American literary life. National, cultural, personal identity associated with the revision of the traditions that were assimilated in their diaspora acts as the main problem of multiculturalism - the ideology of plurality and diversity. Self-identification strategies may include the definition of one's religious, ethnic, gender determination. Very often, the community's collective trauma and collective memory of the historical past can act as a strategy. The main strategies of ethnic and cultural self-identification of Finnish American literature are represented by the collective memory, collective trauma, “Finnishness”, and the national Finnish epic “Kalevala”, which is used as a source of archetypal images and poetic imagination. Basing on a number of works by Finnish American writers of the second half of the 20 and 21 centuries in different genres (science fiction, historical novel, short stories), the article examines the creative application of the Finnish epic “Kalevala”, integrated into the experience of American existence as one of the main strategies for self-identification of its own culture, which is built on the dialogical interaction of the Anglo-Saxon and Finnish cultures. The article actualizes the problems of the interrelations of the two cultures, the transformation of archetypal images, the reflection of collective memory in the works of contemporary Finnish American writers.

The article is devoted to a little-studied period in the history of archaeological research on the territory of Udmurtia in the post-war years, as reflected in letters addressed to the famous archaeologist Otto Nikolayevich Bader. O. N. Bader worked at the Perm (Molotov) University from the second half of the 1940s until 1954, he formed a scientific archaeological school. Together with his students, he engaged in the archaeological study of the vast Ural-Volga region, including the territory of Udmurtia. O. N. Bader maintained contacts with many scientific, educational and museum organizations. The article publishes letters addressed to the scientist from the director of the Udmurt Institute of History, Language and Literature under the Government of the Udmurt ASSR (UdNII) I.F. Kutyavin, the director of the Republican Museum of Local Lore M.N. Sutyagin and researcher of the UdNII A.F. Trefilov. They allow us to see the direction and content of archaeological research, participation of the UdNII and the Republican Museum of Local Lore in them. The letters reflect the formation process of archaeological personnel for Udmurtia, in particular, the beginning of the scientific biography of the famous Soviet archaeologist V.F. Gening. The letters reflect the process of work on the two-volume edition "Essays on the history of the Udmurt ASSR" in terms of writing sections on ancient and medieval history. They reveal the daily life of the researchers in the post-war years. The letters are part of a significant epistolary heritage - letters addressed to O.N. Bader in Perm in the 1940s-1950s. They are kept in the Museum of History of the Perm State Research University and are given to the author for study.

A positive fluid balance has been found to be deleterious in critically ill patients; however, the impact of early fluid balance, particularly on long-term outcomes, in critically ill patients with cancer remains unclear. We performed this retrospective study at a tertiary-care referral hospital with 1500 beds and 6 intensive care units (ICUs) in central Taiwan, and 942 patients with cancer admitted to ICUs during 2013 to 2016 were enrolled. The primary outcome was 1-year mortality. Cancer-related data were obtained from the cancer registry, and data during ICU admissions were retrieved from the electronic medical records. The association between fluid balance, which was represented by median and interquartile range, and 1-year mortality was determined by calculating the hazard ratio (HR) with 95% confidence interval (CI) using a multivariable Cox proportional hazards regression model. The in-hospital mortality rate was 22.9% (216 of 942), and the mortality within 1 year after the index ICU admission was 38.7% (365 of 942). Compared to survivors, nonsurvivors tended to have a higher Acute Physiology and Chronic Health Evaluation II score (24.1 ± 6.9 vs 20.5 ± 6.2, P < .01), a higher age (65.0 ± 14.4 vs 61.3 ± 14.3, P < .01), a higher serum creatinine (1.5 ± 1.3 vs 1.0 ± 1.0, P < .01), and a higher cumulative day 1 to 4 fluid balance (2669, 955-5005 vs 4103, 1268-7215 mL, P < .01). Multivariable Cox proportional hazards regression analysis found that cumulative day-4 fluid balance was independently associated with 1-year mortality (adj HR 1.227, 95% CI: 1.132-1.329). A positive day 1 to 4 cumulative fluid balance was associated with shorter 1-year survival in critically ill patients with cancer. Further studies are needed to validate this association.

7024 Background: The eicosanoid pathway is altered in NSCLC. Pharmacological activators of peroxisome proliferator-activated receptor-gamma (PPARγ) have been shown to inhibit lung tumors in vitro. LY293111 inhibits the eicosanoid pathway, is a PPARγ agonist and modulates adipophilin and adiponectin expression levels in vitro. LY293111 is well tolerated and can be administered with cisplatin/gemcitabine. A randomized phase II study in NSCLC was undertaken. Methods: Pts with stage IIIB/IV NSCLC were randomized to two doses of LY293111 (200 mg BID (Arm A) or 600 mg BID (Arm B) or placebo (Arm C) alone for 7 days followed by concurrent cisplatin (75 mg/m2;d1) and gemcitabine (1250 mg/m2;d1,8) q21 days. Treatment was continued until disease progression or toxicity. Primary endpoint was PFS with 75% power to detect 33% improvement compared to placebo (5 months). Calculated sample size was 65 pts/arm (195 total) with a one sided type I error of 0.2. Changes in adipophilin mRNA expression and adiponectin serum levels were assessed at baseline and day 7. Results: Between 09/03 and 07/04, 201 pts were randomized and 195 treated. Demographics (all pts): M:F (65%:35%); median age 61 (range 27–87); stage IIIB/IV (15%:85%); ECOG PS (0:36%;1:64%). Demographics were well balanced between the 3 arms. Most common CTC grade 3/4 toxicities were nausea (8%), vomiting (7%), fatigue (6%) and diarrhea (6%) with only diarrhea related to dose of LY293111 (p=0.03). Response rates (CR+PR) were similar in all 3 arms (A:20%; B:27%; C:21%). There was no significant difference in median PFS (95% CI) between the arms (A: 4.6 months (3.2–5.0); B:6.0 months (4.6–7.2); C:6.0 months (5.2–7.0)). Median survival (95% CI) was similar in all 3 arms (A: 7.3 months (6.2–9.4); B:7.9 months (7.2–9.7); C: 9.0 months (7.7–12.5). Median change in serum adiponectin levels at d7 were significantly higher in arm B vs. placebo (p < 0.05). Adipophilin expression at d7 was increased in a dose dependent manner (p=NS) in arms A and B vs. placebo. Conclusions: LY293111 in combination with cisplatin/gemcitabine did not increase median PFS compared to placebo in pts with NSCLC. Pharmacodynamic modulation was observed but did not impact efficacy. Abstract Objectives Geographic tongue is the mucositis of the dorsal surface of the tongue with no definite etiology, although it may be associated with atopic conditions. In this study, the salivary immunoglobulin E (IgE) and eosinophilic cationic protein (ECP) levels were assessed in patients with geographic tongue to examine the relationship between this condition and allergy-related biomarkers. Materials and Methods In this case-control study, unstimulated saliva was collected from 45 geographic tongue patients and 45 controls, and the salivary IgE and ECP levels were assessed. The data were analyzed with SPSS 20.0 using the Mann–Whitney test with a significance level of p < 0.05. Results The salivary IgE level in the case and control groups was 123.76 ± 8.1 and 74.34 ± 6.2 IU/mL, respectively, and the salivary ECP level was 9.4 ± 6.0 and 7.6 ± 3.2 ng/mL, respectively. There was a statistically significant difference between the two groups in terms of salivary IgE (p = 0.007) and ECP (p = 0.001) levels. Conclusion Salivary IgE and ECP levels increase in patients with geographic tongue. They can, therefore, be used for the initial diagnostic workup and to investigate the possible association of geographic tongue with allergic reactions.

SummaryHeart failure is characterised by activation of haemostasis. We sought to explore the prognostic impact of deranged haemostasis in chronic heart failure. In stable, optimally managed outpatients with chronic heart failure, baseline levels of prothrombin fragment F1+2, D-dimer, and tPA and PAI-1 antigens were determined. Clinical follow-up was obtained and the rate of events (heart failure related deaths or hospitalisations) was recorded. We included 195 patients [32.3% female, NYHA class II (66.2%) or III (33.8%), mean age 71 years]. During a median follow up of 693 (interquartile range [IQR] 574–788) days, 63 (30.9%) patients experienced an event; those with an event had higher levels of tPA antigen (median 11.8 [IQR 8.7–14.0] vs. 9.4 [7.9–12.1] µg/l; p=0.033) and D-dimer (938 [485–1269] vs. 620 [37–1076] µg/l; p=0.018). However, on Cox multivariate analysis, only tPA levels above optimal cut-off value of 10.2 µg/l (but not D-dimer) emerged as an independent predictor of prognosis (HRadjusted 2.695, 95% confidence interval 1.233–5.363; p=0.017). Our findings suggest that elevated tPA antigen levels are an independent prognostic predictor in patients with chronic stable heart failure.

Construction and demolition waste residue soil (CDWRS) recycled foamed concretes were prepared by introducing the original CDWRS into modified binders. Pore structure, hardened performance, and sandwich wallboard application were also investigated. The results indicated that 51 kg/m3 of water glass and 7.5 kg/m3 of gypsum could significantly increase the strength and generate a slight influence on the thermal insulation performance of CDWRS recycled foamed concrete. The largest enhancing rate of 28-day compressive strength at a density of 600 kg/m3 could reach 205.5%. Foamed concrete with 1126 kg/m3 of CDWRS, modified with water glass and gypsum, showed a low thermal conductivity of 0.11 W/(m·K) and a dry density of 626 kg/m3. In total, 988 kg/m3 of CDWRS in foamed concrete led to a compressive strength of 7.76 MPa, a thermal conductivity of 0.14 W/(m·K), and a dry density of 948 kg/m3. Utilization of the foamed concrete in the sandwich structure could fabricate energy-saving wallboards with a minimum heat transfer coefficient of 0.75 W/(m2·K) and a relatively high compressive strength of 16.5 MPa, providing great confidence of CDWRS consumption in the building energy-saving field.

Background. The diagnosis of acute myocarditis (AMC) and inflammatory cardiomyopathy (DCMi) can be difficult. Speckle tracking echocardiography with accurate assessments of regional contractility could have an outstanding importance for the diagnosis.Methods and Results.N=25patients with clinically diagnosed AMC who underwent endomyocardial biopsies (EMBs) were studied prospectively. Speckle tracking imaging was examined at the beginning and during a mean follow-up period of 6.2 months. In the acute phase patients had markedly decreased left ventricular (LV) systolic function (mean LV ejection fraction (LVEF)40.4±10.3%). At follow-up inn=8patients, inflammation persists, correlating with a significantly reduced fractional shortening (FS,21.5±6.0%) in contrast to those without inflammation in EMB (FS32.1±7.1%,P<0.05). All AMC patients showed a reduction in global systolic longitudinal strain (LS,−8.36±−3.47%) and strain rate (LSR,0.53±0.29 1/s). At follow-up, LS and LRS were significantly lower in patients with inflammation, in contrast to patients without inflammation (−9.4±1.4versus−16.8±2.0%,P<0.0001;0.78±0.4versus1.3±0.3 1/s). LSR and LS correlate significantly with lymphocytic infiltrates (for CD3r=0.7,P<0.0001, and LFA-1r=0.8,P<0.0001).Conclusion. Speckle tracking echocardiography is a useful adjunctive assisting tool for evaluation over the course of intramyocardial inflammation in patients with AMC and DCMi.

The aim of this study was to compare the warm-up effects of treadmill walking (TW) with a dynamic (DY) bodyweight warm-up on maximal aerobic exercise performance in children. Sixteen children (10.9 ± 1.5 vrs) were tested for peak oxygen uptake (VO2 peak) on 2 nonconsecutive days following different 6 min warm-up protocols. TW consisted of walking on a motor-driven treadmill at 2.2 mph and 0% grade whereas the DY warm-up consisted of 9 body weight movements including dynamic stretches, lunges, and jumps. Maximal heart rate was significantly higher following DY than TW (193.9 ± 6.2 vs. 191.6 ± 6.1 bpm, respectively; p = 0.008). VO2 peak (54.8 ± 9.6 vs. 51.8 ± 8.7 mL/kg/min; p = 0.09), maximal minute ventilation (68.9 ± 14.8 vs. 64.9 ± 9.4 L/min; p = 0.27), maximal respiratory exchange ratio (1.12 ± 0.1 vs. 1.11 ± 0.1; p = 0.85) and total exercise time (614.0 ± 77.1 vs. 605 ± 95.0 s; p = 0.55) did not differ significantly between DY and TM warm-ups, respectively. These findings indicate that the design of the warm-up protocol can influence the heart rate response to maximal aerobic exercise and has a tendency to influence VO2 peak. A DY warm-up could be a viable alternative to a TW warm-up prior to maximal exercise testing in children.

Проблема біполярного афективного розладу (БАР) є однією з найактуальніших у сучасній психіатрії. Мета – дослідити особливості копінг-поведінки хворих із первинним епізодом біполярного афективного розладу з урахуванням гендерного фактора і клінічного варіанту дебюту БАР. Матеріал і методи. Проаналізовано особливості копінг-поведінки у 153 хворих із первинним епізодом БАР з використанням опитувальника «Способи долаючої поведінки» S. Folkman і R. Lazarus в адаптації Т. Л. Крюкової. Результати. Встановлено, що при депресивному варіанті первинного епізоду переважають стратегії втечі-уникнення – у 26,5 % всіх хворих, у 25,2 % чоловіків, у 27,3 % жінок; прийняття відповідальності – 17,5 %, 15,2 %, 18,9 %; пошуку соціальної підтримки – 17,5 %, 16,3 %, 18,3 %; менше значення мають самоконтроль – 13,0 %, 14,4 %, 12,1 %; дистанціювання – 8,3 %, 8,0 %, 8,5 %; планування вирішення проблеми – 7,7 %, 9,1 %, 6,9 %; конфронтаційний копінг – 6,5 %, 8,5 %, 5,3 % і позитивна переоцінка – 2,9 %, 3,3 %, 2,7 %. При маніакальному варіанті домінують конфронтаційний копінг – 26,6 %, 28,4 %, 23,1 %; рідше дистанціювання – 17,8 %, 21,0 %, 11,9 %; втеча-уникнення – 15,6 %, 13,8 %, 19,0 %; позитивна переоцінка – 15,3 %, 15,7 %, 14,5 %; планування вирішення проблеми – 9,1 %, 9,0 %, 9,2 %; самоконтролю – 7,3 %, 6,0 %, 9,8 %; пошуку соціальної підтримки – 6,6 %, 4,9 %, 9,9 % і прийняття відповідальності – 1,7 %, 1,2 %, 2,6 %. Висновки. Хворим із первинним епізодом БАР притаманні гендерні особливості копінг-поведінки, що полягають у переважанні в чоловіків конфронтаційного копінгу, самоконтролю, планування вирішення проблеми і позитивної переоцінки, а у жінок – дистанціювання, пошуку соціальної підтримки, прийняття відповідальності і втечі-уникнення. Вирішальне значення для копінг-поведінки має клінічний варіант первинного епізоду, при цьому депресивний варіант асоційований зі стратегіями втечі-уникнення, прийняття відповідальності і пошуку соціальної підтримки; маніакальний – зі стратегіями конфронтаційного копінгу, дистанціювання та втечі-уникнення, а при змішаному варіанті вибір копінг-стратегії визначається актуальним психічним станом.

Cinnamomum verum is a commonly used herbal plant that has several documented properties against various diseases. The existing study evaluated the inhibitory effect of acetonic extract of C. verum (AECV) and ethyl acetate extract of C. verum (EAECV) against piroplasm parasites in vitro and in vivo. The drug-exposure viability assay was tested on Madin-Darby bovine kidney (MDBK), mouse embryonic fibroblast (NIH/3T3) and human foreskin fibroblast (HFF) cells. Qualitative phytochemical estimation revealed that AECV and EAECV containing multiple bioactive constituents namely alkaloids, tannins, saponins, terpenoids and remarkable amounts of polyphenols and flavonoids. AECV and EAECV inhibited B. bovis, B. bigemina, B. divergens, B. caballi, and T. equi multiplication at half-maximal inhibitory concentrations (IC50) of 23.1 ± 1.4, 56.6 ± 9.1, 33.4 ± 2.1, 40.3 ± 7.5, 18.8 ± 1.6 µg/mL, and 40.1 ± 8.5, 55.6 ± 1.1, 45.7 ± 1.9, 50.2 ± 6.2, and 61.5 ± 5.2 µg/mL, respectively. In the cytotoxicity assay, AECV and EAECV affected the viability of MDBK, NIH/3T3 and HFF cells with half-maximum effective concentrations (EC50) of 440 ± 10.6, 816 ± 12.7 and 914 ± 12.2 µg/mL and 376 ± 11.2, 610 ± 7.7 and 790 ± 12.4 µg/mL, respectively. This study aimed to evaluate the risk of eating disorders, psychological distress, and health-related quality of life (HRQoL) in people with class 3 obesity (body mass index (BMI) ≥ 40 kg/m2), and the effect of multidisciplinary weight management over 12 months. This retrospective cohort study included all adults with class 3 obesity who enrolled in a weight management program from March 2018 to December 2019. Questionnaires included the Eating Disorder Examination Questionnaire Short (EDE-QS), Kessler Psychological Distress Scale (K10), and 36-Item Short Form Survey (SF-36) for HRQoL. Physical and Mental Component Summary scores (PCS and MCS) were derived from the SF-36. Of 169 participants who completed 12 months in the program, 65.7% (n = 111) completed questionnaires at baseline and 12 months, with 6.0 ± 6.8% weight loss over this period. Compared to baseline, there was significant improvement at 12 months in EDE-QS (15.7 ± 6.6 vs. 13.6 ± 6.2, p = 0.002), K10 (25.7 ± 9.7 vs. 21.2 ± 9.4, p < 0.001), PCS (29.4 ± 10.1 vs. 36.1 ± 10.9, p < 0.001), and MCS scores (40.2 ± 12.4 vs. 44.0 ± 13.4, p = 0.001). All, apart from EDE-QS scores, remained significant after adjusting for weight change. This study highlights the importance of multidisciplinary management in people with class 3 obesity to help reduce eating disorder risk and psychological distress, and improve HRQoL, in addition to weight loss.

Aim/Background: Mental-health problems among coronavirus disease 2019 (COVID-19) inpatients is an underinvestigated issue. The study reported herein was conducted to assess obsessive-compulsive, borderline, schizoid and narcissistic personality disorder beliefs and the influence of these dysfunctional beliefs on the anxiety and depression levels of COVID-19 inpatients. Methods: A total of 75 COVID-19 inpatients were recruited for the study. Their anxiety and depression levels were evaluated using Hospital Anxiety and Depression Scale, and the aforementioned personality disorder beliefs were evaluated using Personality Belief Questionnaire-Short Form. Results: According to the cut-off values, 9.3% (n = 7) of the inpatients were considered having anxiety and 34.7% (n = 26) were considered having depression. The mean scores for obsessive-compulsive, schizoid, borderline and narcissistic personality disorder beliefs were 15.1 ± 6.2, 12.9 ± 6.0, 10.0 ± 5.5 and 9.4 ± 5.6, respectively. The anxiety levels of the female inpatients were significantly higher than those of the male inpatients (z = 2.152; p = 0.031). The inpatients’ borderline personality disorder belief scores were significantly correlated with their anxiety levels (r = 0.390; p = 0.001). Conclusion: The study participants’ obsessive-compulsive personality disorder beliefs were higher than their other aforementioned personality disorder beliefs. Female gender and borderline personality disorder beliefs were found to be related with anxiety level. The influence of these factors should be considered in the evaluation of COVID-19 inpatients in daily clinical practice.

BackgroundThere are limited real world data on the use of JAK inhibitors (JAKi). In 2022, the EMA suggested that JAKi should only be used if alternative therapies are not available in patients over 65 years, smokers or in those with cardiovascular or cancer risk factors.ObjectivesWe conducted a retrospective study to describe the demographics, duration of therapy and adverse events profile of patients prescribed JAKi.MethodsBaseline data including age, gender, smoking status, ethnicity, co-morbidities, cardiovascular risk factors, rheumatic diagnosis, concomitant csDMARD use and prior biological therapies was collected. Duration on therapy and reasons for stopping JAKi were identified. A Cox proportional hazard model was used to plot survival of drug therapy, by JAKi drug. Data was analysed on the STATA platform.ResultsIn total, 151 patients were prescribed a JAKi since 2017. The average age was 55, with a female predominance, (n=125, 83%), and the majority had rheumatoid arthritis. Baricitinib and Filgotinib represented the most commonly prescribed JAKi. Over ¼ of the cohort were using JAKi monotherapy (28%). For those on combination therapy, methotrexate was the most commonly prescribed (53%). The median number of prior biologics was 1 (IQR 1-2), with TNFi the most frequently prescribed (66%). 48% had cardiovascular risk factors or cardiovascular disease; this was more prevalent in patients prescribed baricitinib or tofacitinib compared to upadacitinib or filgotinib (57%) versus (39%). During JAKi therapy 3 patients developed cancer, 2 had VTEs (both on Baricitinib) and 5 had new mental health diagnoses. The reason for stopping therapy was equally spilt between drug failure (46%) and adverse events (45%). Drug survival over 2 years was numerically higher for Filgotinib and Upadacitinib. Cycling between JAKi occurred in 28 patients. Patients prescribed a second JAKi (n=26) or a third JAKi (n=2) had cycled through more prior biologics than patients prescribed their first JAKi [median number prior biologics 2 (IQR 1-3) versus 1 (IQR 1-2) p=0.007].ConclusionOur real world experience of JAKi is broadly similar to expectations from clinical trials. Abstract Background Most studies comparing the efficacy of hypothermic machine perfusion (HMP) versus static cold storage (SCS) are based on short-term outcomes. We aimed to better evaluate the mid-term impact of HMP in patients receiving expanded criteria donor (ECD) kidneys. Methods The analyses were based on the French Données Informatisées et VAlidées en Transplantation (DIVAT) observational cohort. Patients aged ≥45 years transplanted for the first or second times from an ECD donor since 2010 were studied. Our study reported the graft and/or patient survivals and the incidence of acute rejection episode. The Cox models and the Kaplan–Meier estimators, weighted on the propensity score, were used to study the times-to-events. Results Among the 2019 included patients, 1073 were in the SCS group versus 946 in the HMP group. The mean life expectancy with functioning graft was 5.7 years [95% confidence interval (CI) 5.4–6.1] for the HMP cohort followed-up for 8 years post-transplantation versus 6.0 years (95% CI 5.7–6.2) for the SCS group. These mid-term results were comparable in the patients receiving grafts from donors aged ≥70 years and in the transplantations with cold ischaemia time ≥18 h. Conclusions Our study challenges the utility of using HMP to improve mid-term patient and graft survival. Nevertheless, the improvement of the short-term outcomes is indisputable. It is necessary to continue technological innovations to obtain long-term results.

265 Background: The role of laparoscopic gastrectomy and D2 lymphadenectomy is not yet fully defined in patients treated with modern perioperative chemotherapy. In 2016, Kaiser Permanente Northern California regionalized gastric cancer care, introducing a regional comprehensive multidisciplinary care team, standardizing staging and chemotherapy, and implementing laparoscopic gastrectomy and D2 lymphadenectomy for eligible patients with curative-intent surgery. We previously reported a retrospective cohort study comparing treatments and outcomes before and after regionalization, including 2-year overall survival. Here we present updated 3-year overall survival. Methods: As we described previously ( https://ascopubs.org/doi/full/10.1200/JCO.21.00480 ), the study included all 1429 patients with gastric cancer diagnosed during January 2010 to May 2018, of whom 942 were pre-regionalization, and 487 were post. Of the 1429, 650 had Stage I-III disease, and 394 patients underwent curative intent gastrectomy. 34.9% received neoadjuvant chemotherapy, and 18.4 % of patients received laparoscopic gastrectomy in the pre-regionalization period. In contrast, 66.2% received neoadjuvant chemotherapy, and 91.8% received laparoscopic gastrectomy with D 2 lymphadenectomy in the post regionalization period (p < 0.0001). Lymph node metastasis was noted in 50.7% and 45.1% pre- and post-regionalization periods, respectively (p = 0.29). Survival was analyzed using Kaplan-Meier analysis with entry on the diagnosis date, the outcome on the death date, and censoring on disenrollment or the end of the study. In this updated analysis, the end of the study was increased by one year to December 31, 2019. Results: The statistical analysis showed no difference in patients’ selection for surgery between the pre-and post-regionalization period and no change in the rate of surgical complications. At three years, overall survival among Stage I-III patients who received surgery was 66.0% in the pre-regionalization cohort and 83.0% in the post-regionalization cohort (p < 0.002), a more significant difference than was observed at two years. Conclusions: The utilization of laparoscopic surgery and radical D2 lymphadenectomy in the era of perioperative chemotherapy continues to demonstrate improved postoperative overall disease survival.[Table: see text]

According to hospital-based registry, to evaluate the age characteristics and prevalence of concomitant cardiovascular and non-сardiovascular diseases in patients hospitalized with COVID-19 during epidemic wave.Material and methods. The TARGET-VIP register included 1130 patients aged 57,5+12,8 years (men, 51,2%) hospitalized at the Pirogov National Medical and Surgical Center from April 6, 2020 to June 22, 2020 with COVID-19. Cardiovascular diseases (CVDs) were diagnosed in 51,6% of patients, non-сardiovascular chronic diseases — in 48,6%, while CVDs and/or non-сardiovascular chronic diseases — in 65,8% of patients.Results. The average age of patients significantly increased by an average of 0,77 years per week (p<0,001), while the difference between the 1st week (52,8 years) and 11th week (62,2 years) was 9,4 years; the proportion of men did not change significantly. The proportion of patients with CVDs increased significantly — from 34,2% to 66,7%, on average by 3,7% per week (p<0,001; Incidence Risk Ratio (IRR)=1,037; 95% confidence interval (CI), 1,017-1,058), with chronic non-cardiovascular diseases — from 32,5% to 43,2%, on average by 2,5% per week (p<0,001; IRR=1,025; 95% CI, 1,002-1,049), as well as those with CVDs and/or chronic non-cardiovascular diseases — from 47,5% to 75,3%, on average by 3,2% per week (p<0,001; IRR=1,032; 95% CI, 1,017-1,048). Over the entire period, the proportion of people with hypertension (HTN) was 47,0%, with coronary artery disease (CAD) — 15,4%, with heart failure (HF) — 4,0%, and with atrial fibrillation (AF) — 10,1%. The proportion of patients with HTN increased by 9,5% (p<0,001; OR=1,095; 95% CI, 1,047-1,144), with СAD — by 9,4% (p=0,01; OR=1,094; 95% CI, 1,022-1,172) and with AF — by 9,4% (p<0,001; OR=1,094; 95% CI, 1,023-1,170) per week. The proportion of patients with diabetes was 16,5%, with respiratory diseases — 11,4%, with chronic kidney disease (CKD) — 12,6%, with digestive diseases — 22,5%, with obesity — 6,1%. During the epidemic wave, the most pronounced increase in the proportion of patients with CKD was by 6,2% (p=0,036; OR=1,062; 95% CI, 1,004-1,124) and with digestive diseases — by 6,0% (p=0,01; OR=1,060; 95% CI, 1,014-1,109) per week.Conclusion. According to the 11-week TARGET-VIP registry, the age of patients increased by 9,4 years, CVD cases — by 1,9 times (mainly HTN, CAD, AF), and chronic non­сardiovascular pathology — by 1,3 times (mainly CKD and digestive diseases). These trends in hospital practice corresponded to a weekly increase in the proportion of patients with a higher risk of fatal and non-fatal complications, which is the basis for further research in order to develop a system for a comprehensive prognostic assessment of the degree and rate of increase in the load on hospitals during COVID-19 epidemic wave.

Objective Unilateral hypoglossal nerve stimulation (uniHNS) is an effective treatment for obstructive sleep apnea. Bilateral hypoglossal nerve stimulation (biHNS) is a novel therapeutic option and a different approach to hypoglossal nerve stimulation. The aim of this study was to analyze the clinical outcome of the first 10 biHNS cases vs the first 10 uniHNS cases ever implanted. Study Design Prospective data analysis. Setting International multicenter comparative clinical trial. Methods The first 10 patients in 2020 who received a biHNS device (Genio System; Nyxoah) and the first 10 patients in 2014 who received a uniHNS system (Inspire II; Inspire Medical Systems) were included. Treatment outcome was evaluated at 3 months after surgery. Data collection included demographics, apnea hypopnea index (AHI), oxygen saturation and desaturation index, Epworth Sleepiness Scale, and adverse events. Results The mean ± SD age was 52.1 ± 9.6 years (biHNS) and 58.3 ± 8.6 years (uniHNS). The mean body mass index was 26.4 ± 5.6 kg/m2 (biHNS) and 26.2 ± 2.2 kg/m2 (uniHNS). The mean preimplantation AHI (biHNS, 39.9 ± 14.8/h; uniHNS, 32.2 ± 11.0/h) decreased in both groups after 3 months (biHNS, 19.2 ± 14.0/h, P = .008; uniHNS, 13.1 ± 16.8/h, P = .037) with no significant difference between groups ( P = .720). The mean preimplantation Epworth Sleepiness Scale (biHNS, 11.8 ± 6.2; uniHNS, 11.1 ± 4.9) decreased as well after 3 months (biHNS, 9.4 ± 6.3; uniHNS, 6.0 ± 5.0). Conclusion Preliminary postmarket data suggest that biHNS may be as safe and effective as uniHNS. Long-term follow-up in a larger sample size is required to assess the stability of biHNS.

Short-term protein removal in vitro improves long-term blastocyst competence to survive vitrification. We investigated the mechanisms and effects underlying protein removal. Day-6 morulae and early blastocysts were cultured individually with and without protein for 24 h. Development and lipid content were analysed in expanded blastocysts derived from morulae (M-XB) and from early blastocysts (EB-XB). Expression of genes involved in lipid metabolism, stress responses and apoptosis was analysed in fresh and vitrified–warmed M-XB produced with and without protein. Pregnancy rates, birth rates and birthweight (BW) were recorded after transfer of embryos. Day-7 EB-XB production rates (with, 66.9 ± 6.2 and without, 68.8 ± 6.0 protein) were higher than M-XB rates (with, 21.4 ± 4.6 and without, 9.4 ± 4.6 protein; P < 0.005). EB-XB showed fewer lipids than M-XB (P = 0.03). In fresh M-XB, expression of sterol regulatory element binding protein (SREBP1) was lower with (4.1 ± 2.2) than without (13.6 ± 2.2) protein, contrary to results obtained for Patatin-like phospholipase domain containing 2, Hormone-sensitive lipase and Bcl-2–associated X protein (P < 0.05). Protein did not affect pregnancy rates and birth phenotypes (P > 0.05). However, BW was higher (P < 0.01) in calves born from vitrified M-XB (48.6 ± 3.4 kg) than from EB-XB (39.8 ± 2.9 kg). Such effects were more pronounced in females (P < 0.001). Calves from fresh embryos did not show BW differences. These results indicate that embryonic kinetics and vitrification impact birth phenotypes, at least in females. Alterations might involve exogenous protein and mobilisation of lipid stocks.

Aim of the study. To estimate the effectiveness and safety of substitution therapy with growth hormone (GH) in children with various brain tumours. Materials and methods. The study involved 68 patients admitted to N.N. Burdenko Institute of Neurosurgery between 2001 and 2011 including 35 ones with craniopharyngioma, 18 with medulloblastoma, and 15 with germ cell tumours (GCT) in the chiasmosellar region. All patients suffered growth hormone insufficiency and received GH replacement therapy. Their antropometric characteristics (height and growth rate) and IGF-1 levels were measured before, 6 and 12 months after the onset of the treatment. The doses of GH varied from 0.03 to 0.034 mg/kg/day. Results. The substitution therapy with GH resulted in an increase of the growth rate in patients with craniopharyngioma from 2.3±1.6 to 9.4±1.9 cm/year (p<0.001), in those with GCT from 1.2±0.9 to 7.4±2.6 cm/year (p=0.01), and in patients with medulloblastoma from 2.3±1.5 to 6.2±2.6 cm/year (p<0.01). The growth rate in patients treated by spinal irradiation was lower than that in those given no such treatment (6.0±2.3 cm/year and 9.2±2.1 cm/year respectively; p<0.001). The tumour recurred in eight of 35 (23%) patients with craniopharyngioma during GH therapy. The frequency of relapses was not significantly different from that in patients who did not receive growth hormone. Patients with medulloblastoma and GCT did not develop relapses. No adverse reactions to GH therapy were documented in this study.

Cyanide (CN−) pollution in agricultural systems can affect crop production. However, no data are available to describe the full picture of the responsive metabolic mechanisms of genes with known functions related to exogenous KCN exposure. In this study, we examined the transcriptome in rice seedlings exposed to potassium cyanide (KCN) using an Agilent 4×44K rice microarray to clarify the relationship between the differentially expressed genes (DEGs) and their function classifications. The number of DEGs (up-regulated genes/down-regulated genes) was 322/626 and 640/948 in the shoots and roots of CN−-treated rice seedlings, respectively. Functional predication demonstrated that a total of 534 and 837 DEGs in shoots and roots were assigned to 22 COG categories. Four common categories listed on the top five COG classifications were detected in both rice tissues: signal transduction mechanisms, carbohydrate transport and metabolism, post-translational modification, protein turnover and chaperones, and transcription. A comparison of DEGs aligned to the same COG classification demonstrated that the majority of up-regulated/down-regulated DEGs in rice tissues were significantly different, suggesting that responsive and regulatory mechanisms are tissue specific in CN−-treated rice seedlings. Additionally, fifteen DEGs were aligned to three different COG categories, implying their possible multiple functions in response to KCN stress. The results presented here provide insights into the novel responsive and regulatory mechanisms of KCN-responsive genes, and will serve as useful resources for further functional dissections of the physiological significance of specific genes activated in the exogenous KCN stress response in rice plants.

Abstract BACKGROUND Glioblastoma (GBM) is known for its high local failure rate, as well as its migration, leading to these possible locations of disease progression: 1. Local-only; lesion of progression is connected to the initial tumour location 2. Non-local; lesion of progression is not connected to the initial tumour location 3. Combined; both local-only and non-local progression lesions are present An upfront prediction of progression location could potentially be used to select patients for differential treatment strategies. The aim of this retrospective national study was therefore to predict location of first progression using clinical variables. MATERIAL AND METHODS All adults with newly diagnosed GBM between 2014-2019 who started long-course (chemo)radiotherapy were included, if they had a radiotherapy-planning MRI made. The MRI of first progression was identified by retrospective review following RANO-criteria, and used to assess the location of contrast-enhancing progression. Median time to progression (TTP) and survival beyond progression were estimated using the Kaplan-Meier method (log rank test for comparison). To predict location of progression in subgroups of patients, we used a multivariable multinomial model (age, biological sex, performance status, tumour location & focality & laterality, MGMT-methylation status, and extent of surgery). RESULTS We included 940 patients (MGMT methylated n=376, unmethylated n=483, not reported n=81), of whom 760 had radiographic progression. Of those, progression location was local-only in 69%, non-local in 16%, and combined in 15%. Median (95% CI) TTP in months was shortest in the combined group (8.4, 7.3-9.4), followed by the local-only (9.2, 8.5-9.8) and non-local (13.9, 12.3-15.6) (p-value &lt;.001). Survival beyond progression in months was shortest in the combined group (5.0, 3.8-6.2), followed by the non-local (6.0, 4.4-7.6) and local-only (9.0, 8.3-9.7) (p-value &lt;.001). When comparing patients with MGMT-methylated and unmethylated tumours, the probability (95%CI) of local-only progression was .67 (.62-.73) vs. .73 (.69-.78), non-local was .21 (.17-.26) vs. .11 (.08-.14), and combined was .12 (.08-.15) vs. .16 (.12-.19). In the multivariable model, the probability (95%CI) of local-only progression varied from .49 (.29-.69) to .86 (.74-.97), of non-local from .04 (.00-.09) to .34 (.15-.53), and of combined from .05 (.01-.10) to .34 (.07-.60) between respective subgroups. CONCLUSION Location of progression was associated with time to progression and survival beyond progression. In our study population it was feasible to identify subgroups of patients with a high chance of local-only progression, who may potentially benefit most from radiation dose-escalation. Both patients with MGMT-methylated and unmethylated tumours had the highest chance for first progression located at the initial tumour site only.

Objective: To evaluate the impact of COVID-19 pandemic on termination of pregnancy. Methods: This was a retrospective study aimed to assess the impact of COVID-19 pandemic on termination of pregnancy in a single center in Italy. Consecutive data on pregnant women who requested induced termination of pregnancy (I-TOP) from February 2018 to December 2021 were included in a dedicated database. The data were divided into two groups according to the COVID-19 outbreak. Women who requested I-TOP from February 2018 to January 2020 were included into the group ‘before COVID-19 pandemic’. Women who requested I-TOP from February 2020 to January 2022 were included into the group ‘during COVID-19 pandemic’. Indications for I-TOP included elective abortion and therapeutic for fetal or maternal indication. Results: A total of 2578 women were included in the study. Of them, 1637 had I-TOP before COVID-19, and 941 had I-TOP during COVID-19. During the pandemic, the request for elective abortion decreased from 76.2% to 67.7% (p<0.01). Therapeutic abortion were performed in 141/693 cases in the first trimester, and in 552/693 cases in the second trimester. Overall, 91 were for maternal indications and 602 for fetal indications. No differences were noticed between before and during pandemic (p=0.99). Follow-up visits two weeks after abortions were offered to all women. However, only 35.5% women visited for follow-up during pandemic vs. 65.0% before COVID-19 (p<0.01). Conclusion: The COVID-19 pandemic had impact on access to abortion services, reducing request for elective abortion and post-abortion follow-up visits.

Abstract We present a joint spectral analysis of ROSAT PSPC, Swift X-ray Telescope, and Nuclear Spectroscopic Telescope Array Focal Plane Module A/B (FPMA/B) data of the nova-like (NL) cataclysmic variables (CVs) BZ Cam and V592 Cas in the 0.1–78.0 keV band. Plasma models of collisional equilibrium fail to model the 6.0–7.0 iron line complex and continuum with χ ν 2 larger than 2.0. Our results show nonequilibrium ionization (NEI) conditions in the X-ray plasma with temperatures of 8.2–9.4 keV and 10.0–12.9 keV for BZ Cam and V592 Cas, respectively. The centroids of He-like and H-like iron ionization lines are not at their equilibrium values as expected from NEI conditions. We find power-law spectral components that reveal the existence of scattering and Comptonization with a photon index of 1.50–1.87. We detect a P Cygni profile in the H-like iron line of BZ Cam translating to outflows of 4500–8700 km s−1 consistent with the fast winds in the optical and UV. This is the first time such a fast collimated outflow is detected in the X-rays from an accreting CV. An iron Kα line around 6.2–6.5 keV is found revealing the existence of reflection effects in both sources. We study the broadband noise and find that the optically thick disk truncates in BZ Cam and V592 Cas consistent with transition to an advective hot flow structure. V592 Cas also exhibits a quasiperiodic oscillation at 1.4 − 0.3 + 2.6 mHz. In general, we find that the two NLs portray spectral and noise characteristics as expected from advective hot accretion flows at low radiative efficiency.

Seed vigor is an important index to evaluate seed quality in plant species. How to evaluate seed vigor quickly and accurately has always been a serious problem in the seed research field. As a new physical testing method, multispectral technology has many advantages such as high sensitivity and accuracy, nondestructive and rapid application having advantageous prospects in seed quality evaluation. In this study, the morphological and spectral information of 19 wavelengths (365, 405, 430, 450, 470, 490, 515, 540, 570, 590, 630, 645, 660, 690, 780, 850, 880, 940, 970 nm) of alfalfa seeds with different level of maturity and different harvest periods (years), representing different vigor levels and age of seed, were collected by using multispectral imaging. Five multivariate analysis methods including principal component analysis (PCA), linear discriminant analysis (LDA), support vector machine (SVM), random forest (RF) and normalized canonical discriminant analysis (nCDA) were used to distinguish and predict their vigor. The results showed that LDA model had the best effect, with an average accuracy of 92.9% for seed samples of different maturity and 97.8% for seed samples of different harvest years, and the average sensitivity, specificity and precision of LDA model could reach more than 90%. The average accuracy of nCDA in identifying dead seeds with no vigor reached 93.3%. In identifying the seeds with high vigor and predicting the germination percentage of alfalfa seeds, it could reach 95.7%. In summary, the use of Multispectral Imaging and multivariate analysis in this experiment can accurately evaluate and predict the seed vigor, seed viability and seed germination percentages of alfalfa, providing important technical methods and ideas for rapid non-destructive testing of seed quality.

Management guidelines assume that results from clinical trials can be generalised, although seldom is data available to test this assumption. We aimed to determine the proportion of patients commencing tumour necrosis factor inhibition (TNFi) who would have been eligible for relevant clinical trials, and whether treatment response differs between these groups and the trials themselves. The British Society for Rheumatology Biologics Register for Ankylosing Spondylitis (BSRBR-AS) recruited a real-world cohort of TNFi-naïve spondyloarthritis patients with data collection from clinical records and patient questionnaires. Participant characteristics were extracted from trials identified from a recent Health Technology Assessment of TNFi for ankylosing spondylitis/non-radiographic axial spondyloarthritis. Descriptive statistics were used to determine the differences, including treatment response, between BSRBR-AS participants who would/would not have been eligible for the clinical trials and with trial participants. Among 2420 BSRBR-AS participants, those commencing TNFi (34%) had shorter symptom duration (15 vs 22 years) but more active disease (Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) 6.4 vs 4.0; Bath Ankylosing Spondylitis Disease Functional Index (BASFI) 6.2 vs 3.8). Of those commencing TNFi, 41% met eligibility criteria for ≥1 of fourteen relevant trials; they reported higher disease activity (BASDAI 6.9 vs 6.1) and poorer function (BASFI 6.6 vs 6.0). 61.7% of trial participants reported a positive treatment response, vs 51.3% of BSRBR-AS patients (difference: 10.4%; 95% CI 4.4% to 16.5%). Potential eligibility for trials did not influence treatment response (difference 2.0%; -9.4% to 13.4%). Fewer patients in the real world respond to TNFi than is reported in the trial literature. This has important implications for the generalisability of trial results, and the cost-effectiveness of TNFi agents.

ABSTRACTScientific and regulatory interest in assessing clinical endpoints after 48 to 72 h of treatment for acute bacterial skin and skin structure infections (ABSSSI) has increased. Historical, pre-antibiotic-era data suggest that a treatment effect relative to untreated controls can be discerned in this time interval. Ceftaroline fosamil, a broad-spectrum bactericidal cephalosporin with activity against Gram-positive organisms, including methicillin-resistantStaphylococcus aureus(MRSA), and Gram-negative organisms was efficacious in two phase 3 trials of complicated skin infections (CANVAS 1 and 2) using clinical cure rates at the test-of-cure visit. To assess an early clinical response in the CANVAS trials, a retrospective analysis using a day 3 clinical endpoint was conducted. Adults with ABSSSI received intravenous ceftaroline fosamil at 600 mg every 12 h (q12h) or vancomycin at 1 g plus aztreonam at 1 g (V/A) q12h for 5 to 14 days. Clinical response at day 3, defined as cessation of infection spread and absence of fever, was analyzed in patients with a lesion size of ≥75 cm2and either deep and/or extensive cellulitis, major abscess, or an infected wound. Day 3 integrated CANVAS clinical response rates were 74.0% (296/400) for ceftaroline and 66.2% (263/397) for V/A (difference, 7.8%; 95% confidence interval [CI], 1.3% to 14.0%). In the individual studies, absolute treatment differences of 9.4% (CANVAS 1) and 5.9% (CANVAS 2) favoring ceftaroline were observed. For ABSSSI due to MRSA, response rates were 81.7% and 77.4% in the ceftaroline and V/A groups, respectively. In this retrospective analysis, ceftaroline fosamil monotherapy had a numerically higher clinical response than V/A at day 3 in the treatment of ABSSSI.

Background: Financial relationships between healthcare institutions and pharmaceutical companies can lead to conflicts of interest (COIs), potentially compromising patients’ care. In Japan, scholarship donations, unique type of payments made to healthcare institutions and their subunits by pharmaceutical industries without restricting their use including non-educational or research purpose, may often have implicit promotional purposes. However, detailed information about these payments remains scarce. Methods: This study employed a cross-sectional design to analyse the extent and distribution of all scholarship donations made by all 73 pharmaceutical companies belonging to the Japan Pharmaceutical Manufacturers Association (JPMA) to healthcare institutions in 2017. Data were obtained from publicly available sources from the companies, and the total number of payments, their distributions across various institutions and specialties were analysed. Results: A total of 27 007 payment contracts amounting to $178 703 721 in scholarship donations were made to 4839 specific departments and laboratories at 251 different institutions by 67 pharmaceutical companies. National universities received 50.8% of total payments. All universities setting medical school in Japan received one or more payments. Domestic pharmaceutical companies contributed to $137 797 302 (77.1%) in total. Clinical medicine departments received 89.6% ($160 113 147) with 6.2% ($11 011 946) and 2.0% ($3 600 456) allocated to basic medicine and social medicine specialties, respectively. Conclusion: This study provided a comprehensive overview of scholarship donations from pharmaceutical companies to healthcare institutions in Japan, revealing significant financial support primarily directed to national universities and clinical medicine departments. Japanese policy-makers should consider implementing regulations that promote transparency and mitigate potential COIs arising from scholarship donations, which may be useful in other countries with similar schemes.

Probiotic administration might strengthen intestinal barrier function and provide protection; this is supported by published meta-analyses, but there is a lack of large well-designed trials.ObjectiveTo test the use of the probioticBifidobacterium brevestrain BBG-001 to prevent NEC, late-onset sepsis and death in preterm babies while monitoring probiotic colonisation of participants.DesignDouble-blind, randomised, placebo-controlled trial.SettingRecruitment was carried out in 24 hospitals, and the randomisation programme used a minimisation algorithm. Parents, clinicians and outcome assessors were blinded to the allocation.ParticipantsBabies born between 23 and 30 weeks’ gestation and randomised within 48 hours of birth. Secondary outcomes included stool colonisation withB. breve.ResultsIn total, 654 babies were allocated to receive probiotic and 661 to receive placebo over 37 months from July 2010. Five babies were withdrawn; 650 babies from the probiotic group and 660 from the placebo group were included in the primary analysis. Baseline characteristics were well balanced. There was no evidence of benefit for the primary outcomes {sepsis: 11.2% vs. 11.7% [adjusted relative risk (RR) 0.97, 95% confidence interval (CI) 0.73 to 1.29]; NEC Bell stage ≥ 2: 9.4% vs. 10.0% [adjusted RR 0.93, 95% CI 0.68 to 1.27]; and death: 8.3% vs. 8.5% [adjusted RR 0.93, 95% CI 0.67 to 1.30]}.B. brevecolonisation status was available for 1186 (94%) survivors at 2 weeks’ postnatal age, of whom 724 (61%) were positive: 85% of the probiotic group and 37% of the placebo group. There were no differences for subgroup analyses by minimisation criteria and by stool colonisation withB. breveat 2 weeks. No harms associated with the interventions were reported.LimitationsCross-colonisation of the placebo arm could have reduced statistical power and confounded results; analyses suggest that this did not happen.ConclusionsThis is the largest trial to date of a probiotic intervention. It shows no evidence of benefit and does not support routine use of probiotics for preterm infants.Future work recommendationsThe increasing understanding of the pathogenesis of NEC and sepsis will inform the choice of probiotics for testing and better define the target population. Hematology and serum chemistry references are essential diagnostic tools to assess the impact of abiotic, biotic, and anthropogenic activities on wildlife and the ecosystem. The Brandt’s Vole (Lasiopodomys brandtii Radde, 1861) is a widely distributed and endemic species in the Mongolian plateau and plays a crucial role in the steppe ecosystem as a secondary consumer in the food chain. This study aimed to provide baseline hematology and serum chemistry data for wild Brandt’s vole and to evaluate whether these parameters differ among season, sex, and age classes. Twenty-four variables were analyzed for 660 individuals (330 male and 330 female) blood samples collected by 11 field and semi-stationary surveys conducted in Khentii, Sukhbaatar, Tuv, Bulgan, and Zavkhan provinces from 2016 until 2021. Season significantly influenced lymphocyte, erythrocyte, platelet, and hemoglobin range. A 94.4% of all hematological parameters were season dependent. Aspartate aminotransferase (AST) and glucose levels changed with sex in Brandt’s Vole, and 83% of all 24 parameters were significantly different in age classes. Хэнтий, Сүхбаатар, Төв, Булган, Завхан аймгийн нутагт 2016-2021 онд нийт 11 удаагийн хээрийн ба хагас суурин судалгааг хийж, дээж материалыг бүрдүүлсэн. Context Operator training, quality control, and proper follow-up for out-of-range quality control (QC) events are crucial steps that must be adequately performed and documented to ensure excellent patient care and regulatory compliance. Objective To examine point-of-care testing (POCT) personnel training and QC documentation/compliance. Design Participants in a POCT documentation study of the College of American Pathologists Q-Probes program collected data retrospectively for glucose and urine dipstick testing regarding test operators, operator competency assessment, and QC documentation. Documentation was assessed for participant adherence to 4 quality indicators: (1) whether test operator training was up to date, (2) whether the test operator names were noted in the test records, (3) whether QC was performed, and (4) whether out-of-range QC events were followed up. Data were analyzed for associations with institutional demographic and practice variables. Results The institutional median number of POCT personnel was 648 for blood glucose and 76 for urine dipstick testing, with a median number of 105 948 glucose tests and 9113 urine tests performed. Ninety-four percent (3830 of 4074) of the test operators completed training or competency assessment within the prior 12 months, 96.8% (21 603 of 22 317) of the test records documented the operator, and 95.7% (19 632 of 20 514) of the expected QC events (per institutional regulations) were documented. Approximately 3% (659 of 20 514) of the QC events were outside the designated range (an average of 6 out-of-range QC events were identified per institution [n = 106]). Of the out-of-range QC events, 92.6% (610 of 659) had documentation of appropriate follow-up. Most laboratories (176 of 179; 98.3%) violated specimen requirements by storing POCT urine specimens for less than 24 hours. Conclusions There was greater than 90% compliance for POCT documentation and nearly 96% of expected QC events were properly documented.

The aim of the present study was to investigate the changes in the content of phytochemical compounds and in vitro antioxidant, antibacterial, and anti-inflammatory activities of Teucrium polium L. aerial parts and root methanolic extracts at different phenological stages (vegetative, flowering, and seeding). The T. polium extracts were analyzed using gas chromatography–mass spectrometry (GC-MS), and their antioxidant properties were tested with the 2,2-diphenyl-1-picrylhydrazyl (DPPH), nitric oxide (NO), ferrous ions (Fe2+), and 2,2′-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid (ABTS) methods. Forty-nine compounds were identified with the majority of germacrene D, t-cadinol, β-pinene, carvacrol, bicyclogermacrene, α-pinene, and limonene. The results show that the extracts significantly differ between different phenological stages of the plant material used in terms of the phytochemical composition (total phenolic compounds, total flavonoids, total alkaloids, and total saponin contents) and bioactivities (antioxidant, antibacterial, and anti-inflammatory) (p < 0.05). The highest total contents of phenolics (72.4 ± 2.5 mg gallic acid equivalent (GAE)/g dry weight), flavonoids (36.2 ± 3.1 mg quercetin equivalent (QE)/g dry weight), alkaloids (105.7 ± 2.8 mg atropine equivalent (AE)/g dry weight), and saponins (653 ± 6.2 mg escin equivalent (EE)/g dry weight), as well as antioxidant, antibacterial, and anti-inflammatory activities, were measured for the extract of the aerial parts obtained at the flowering stage. The minimum inhibitory concentration (MIC) values for the extracts were varied within 9.4–300 µg/mL, while the minimum bactericidal concentration (MBC) values were varied within 18.75–600 µg/mL. In addition, they were more active on Gram-positive bacteria than Gram-negative bacteria. The data of this work confirm that the T. polium extracts have significant biological activity and hence can be used in the pharmaceutical industry, clinical applications, and medical research, as well as cosmetic and food industries.

Background: The neurobiology of mental disorders remains poorly understood despite substantial scientific efforts, due to large clinical heterogeneity and to a lack of tools suitable to map individual variability. Normative modeling is one recently successful framework that can address these problems by comparing individuals to a reference population. The methodological underpinnings of normative modelling are, however, relatively complex and computationally expensive. Our research group has developed the python-based normative modelling package Predictive Clinical Neuroscience toolkit (PCNtoolkit) which provides access to many validated algorithms for normative modelling. PCNtoolkit has since proven to be a strong foundation for large scale normative modelling, but still requires significant computation power, time and technical expertise to develop. Methods: To address these problems, we introduce PCNportal. PCNportal is an online platform integrated with PCNtoolkit that offers access to pre-trained research-grade normative models estimated on tens of thousands of participants, without the need for computation power or programming abilities. PCNportal is an easy-to-use web interface that is highly scalable to large user bases as necessary. Finally, we demonstrate how the resulting normalized deviation scores can be used in a clinical application through a schizophrenia classification task applied to cortical thickness and volumetric data from the longitudinal Northwestern University Schizophrenia Data and Software Tool (NUSDAST) dataset. Results: At each longitudinal timepoint, the transferred normative models achieved a mean[std. dev.] explained variance of 9.4[8.8]%, 9.2[9.2]%, 5.6[7.4]% respectively in the control group and 4.7[5.5]%, 6.0[6.2]%, 4.2[6.9]% in the schizophrenia group. Diagnostic classifiers achieved AUC of 0.78, 0.76 and 0.71 respectively. Conclusions: This replicates the utility of normative models for diagnostic classification of schizophrenia and showcases the use of PCNportal for clinical neuroimaging. By facilitating and speeding up research with high-quality normative models, this work contributes to research in inter-individual variability, clinical heterogeneity and precision medicine.
