Abstract (sommario):
A myoepithelial-like cell line (Rama 401), isolated from rat mammary gland, has been transformed with a temperature-sensitive mutant of Rous sarcoma virus (tsRSV). Rama 401-tsRSV cells adopt a spindle morphology and fail to deposit basement membrane proteins when grown at the permissive temperature (35 degrees C). When switched to the non-permissive temperature (41 degrees C), the cells flatten (with a 5-fold increase in area), and deposit an extracellular matrix containing basement membrane proteins. When the cells are switched from 35 degrees C to 41 degrees C in the presence of monensin (an ionophore that inhibits protein secretion), basement membrane proteins are no longer deposited extracellularly although the cells flatten, their area increasing by ninefold. Cells switched from 35 degrees C to 41 degrees C in the presence of cycloheximide still flatten and deposit basement membrane proteins, whereas the morphological change on switching from 41 degrees C to 35 degrees C is inhibited by cycloheximide. These experiments indicate that the ability of Rama 401-tsRSV cells to spread on a plastic substratum is not dependent on the de novo synthesis and deposition of basement membrane proteins.

Abstract (sommario):
Evaluation of patients with signs and symptoms of biliary tract disease usually includes ultrasound assessment of the gallbladder. Does measurement of the thickness of the gallbladder wall yield any significant information to the clinical surgeon? The records of all my patients undergoing cholecystectomy since 1990 were reviewed. The entire series consists of 401 consecutive patients, in whom 388 procedures were completed laparoscopically, with 14 patients requiring conversion to an open cholecystectomy. Each patient's preoperative evaluation included a gallbladder ultrasound, which included measurement of the diameter of the gallbladder wall. The entire series of cholecystectomies was evaluated according to the ultrasound measured diameter of the gallbladder wall. A thin gallbladder wall was less than 3 mm in diameter. A thick gallbladder wall was 3 mm or greater in diameter. Of the 401 consecutive patients who underwent cholecystectomy for symptomatic gallbladder disease, 86 (21.5%) were removed laparoscopically for acalculous disease. Eleven per cent of patients with acalculous cholecystitis had acute cholecystitis and 89 per cent had chronic cholecystitis. Every patient with either a thin or thick gallbladder wall with acalculous cholecystitis had a successful laparoscopic cholecystectomy. Three-hundred fifteen patients had a laparoscopic cholecystectomy for calculous cholecystitis. In patients with calculous cholecystitis, 28.3 per cent had acute cholecystitis and 71.7 per cent had chronic cholecystitis. The gallbladder wall was found to be greater than 3 mm in 38 per cent of patients with acute calculous cholecystitis and greater than 3 mm in 41 per cent of patients with chronic calculous cholecystitis. One-hundred, forty-two patients, out of a series total of 401, had a gallbladder wall thickness greater than 3 mm by preoperative sonography and 14 of these patients (10%) required conversion to an open cholecystectomy. A preoperative gallbladder ultrasound evaluation for symptomatic cholecystitis, which documents a thick gallbladder wall (≥3 mm) with calculi, is a clinical warning for the laparoscopic surgeon of the potential for a difficult laparoscopic cholecystectomy procedure which may require conversion to an open cholecystectomy procedure.

AbstractThe purpose of this paper is to determine how the following manuscripts of Cicero’s De oratore relate to each other: Avranches 238 (A), Erlangen 380 (E), Leiden, Voss. Lat. O. 26 (F) and Vatican, Reg. lat. 1762 (K; a florilegium). In the first section it is shown, on the basis of conjunctive and separative errors, that E and F must have had a common exemplar (α). Next, I argue that E and F (and hence also α) are derived from A (Section 2). In Section 3 I turn to K and argue that K’s excerpts must have been taken from two different exemplars: §§ 429-41 from α, §§ 401-28 from some related manuscript, probably A itself. Section 4 is devoted to a few questions of secondary importance. Finally, I give a stemma.

Bacterial sepsis is generally a major concern in ill infants. To help triaging decisions by front-line health workers in these situations, the World Health Organization (WHO) has developed danger signs (DS). The objective of this study was to evaluate the extent to which nine DS predict bacterial sepsis in young infants presenting with suspected sepsis in a low-income country setting. The study pragmatically evaluated nine DS in infants younger than 3 months with suspected sepsis in a regional hospital in Lilongwe, Malawi, between June 2018 and April 2020. Main outcomes were positive blood or cerebrospinal fluid (CSF) cultures for neonatal pathogens, and mortality. Among 401 infants (gestational age [mean ± SD]: 37.1±3.3 weeks, birth weight 2865±785 grams), 41 had positive blood or CSF cultures for a neonatal pathogen. In-hospital mortality occurred in 9.7% of infants overall (N = 39/401), of which 61.5% (24/39) occurred within 48 hours of admission. Mortality was higher in infants with bacterial sepsis compared to other infants (22.0% [9/41] versus 8.3% [30/360]; p = 0.005). All DS were associated with mortality except for temperature instability and tachypnea, whereas none of the DS were significantly associated with bacterial sepsis, except for “unable to feed” (OR 2.25; 95%CI: 1.17–4.44; p = 0.017). The number of DS predicted mortality (OR: 1.75; 95%CI: 1.43–2.17; p<0.001; AUC: 0.756), but was marginally associated with positive cultures with a neonatal pathogen (OR 1.22; 95%CI: 1.00–1.49; p = 0.046; AUC: 0.743). The association between number of DS and mortality remained significant after adjusting for admission weight, the only statistically significant co-variable (OR 1.75 [95% CI: 1.39–2.23]; p<0.001). Considering all positive cultures including potential bacterial contaminants resulted a non-significant association between number of DS and sepsis (OR 1.09 [95% CI: 0.93–1.28]; p = 0.273). In conclusion, this study shows that DS were strongly associated with death, but were marginally associated with culture-positive pathogen sepsis in a regional hospital setting. These data imply that the incidence of bacterial sepsis and attributable mortality in infants in LMIC settings may be inaccurately estimated based on clinical signs alone.

The descriptions and measurements in following entries have been translated directly from the inventories compiled by Antonio d’Ambrosio. D’Ambrosio carried out the organization of the finds that were reportedly removed from the Casa del Menandro between 1979 and 1981, and stored in the Casa di Bacco storerooms in Pompeii. These finds had been left in situ, presumably in this house, since its excavation in the 1930s. However, it is not absolutely certain that the artefacts now inventoried as being from the Casa del Menandro were actually all excavated from that house, particularly figurative pieces. Such artefacts are unlikely to have been left in the house and not inventoried at the time of excavation. Also precise provenances, within the house, have not been provided in these recent inventories. Some of these items can be identified as particular finds recorded in the excavation reports, and these have been included in the main catalogue for the Casa del Menandro. For others there are a number of possibilities which are included here and in the main catalogue. None of these artefacts have been studied in detail. Dimensions: 185 mm × 215 mm; d.: 41 mm. Aim/Purpose. Patients’ length of stay in emergency departments (ED) is a widespread problem that poses great hardship on patients and health providers alike. This paper’s purpose is to reduce length of stay (LOS) for patients presenting to the ED with headaches. Background. The increasing number of patients admitted to emergency departments challenges administrators to find ways to reduce the length of stay in the ED. The purpose of this paper is to quantify the potential reduction in LOS by modifying patient flow in the ED, for patients presenting with non-traumatic headache who require a non-contrast head computerized tomography (CT). Methodology. A 41-month retrospective review was performed for all patients presenting to the Sheba ED with non-traumatic headache as the chief complaint and that were referred to CT during their visit. We distinguished between patients that had undergone the standard patient flow of first seeing a physician and only then referred to CT and patients that were sent to CT directly from the triage station, which is run by a triage nurse. For the former group, we identified their main patient flow epochs: arrival, triage nurse, physician referral to CT, performing CT, discharge. Contribution. The contribution is two-fold. First, a practical recommendation for changes in patient workflow to reduce LOS. Second, it demonstrates how medical records can be used to analyze “what-if” scenarios on patient flow. Findings During the period under review, there were 5501 headache patients out of a total of 196681 walk-in ED visits, a ratio of 2.79% (95% confidence interval [95% CI]: 2.72%-2.86%). Of the headache patients, 2961 patients were referred to CT and their LOS was 394 minutes (95% CI: 387-401). Modifying the standard patient flow so that patients are sent from triage immediately to CT will potentially reduce 121 minutes (95% CI: 118-124) from their LOS. These potential savings are concentrated mainly in the p.m. hours. Recommendations for Practitioners. The potential LOS reduction can be achieved by modifying current patient flow for patients presenting to the ED with non-traumatic headache. Modifications should focus on late afternoon and evening hours. Accordingly, different proposals addressing the nature of these proposals are discussed. Recommendations for Researchers. There is a plethora of information available in electronic medical records, which is yet to be harnessed to improve the management of health systems. Researchers could apply techniques used in this paper to benefit the health systems. Impact on Society. Reducing LOS will positively affect not only patients who will receive faster service, but also health provider that are currently are operating in a crowded and stressful environment. Future Research. The research can be expanded to other common patient main complaints such as abdominal pain or orthopedic injuries.

Background: Cannabis is the most commonly used federally illicit substance among emerging adults (18-25). As cannabis use rates increase it is important to further understand reasons why individuals use cannabis and the problematic consequences of cannabis use. Previous research has shown a variety of gender differences in recreational use vs. medical use, methods of cannabis use, and consequences related to cannabis use. However, little is known about gender differences in cannabis use motives and consequences related to cannabis use. To further understand the differential impact of cannabis use motives on problematic cannabis use, the present study examined whether gender moderated the connection between cannabis use motives and cannabis-related problems. It is hypothesized that gender will moderate the connection between cannabis use motives and cannabis-related consequences. Method: 401 emerging adults (51.9% female; 59.1% Caucasian) completed an online survey assessing gender, frequency of cannabis use, cannabis use motives, and cannabis related problems. Motives for cannabis use were measured using the Comprehensive Marijuana Motives Questionnaire (CMMQ) and problems related to cannabis use was measured using the Marijuana Problems Scale (MPS). The CMMQ consists of 36 items that assess 11 subtypes of cannabis use motives including alcohol, celebration, relative low risk, conformity, boredom, altered perception, sleep, availability, social anxiety, coping, and enjoyment. Bivariate correlations were used to examine the association between cannabis use motives, consequences related to cannabis use, and gender. Moderation analyses were then conducted to assess whether gender moderated the relationship between each cannabis use motive and consequences related to cannabis use. Results: Bivariate correlations revealed significant positive correlations for each cannabis use motive and consequences related to cannabis use (alcohol r = .28, p < .001 , celebration r = .20, p =.01, relative low risk r = .29, p < .001, conformity r = .23, p = .003, boredom r = .49, p < .001, altered perception r = .47, p < .001, sleep r = .30, p < .001, availability r = .29, p < .001, social anxiety r = .41, p < .001 , coping r = .45, p < .001 , and enjoyment r = .31, p < .001). Several moderation analyses revealed gender moderated the relationship for cannabis use motives and cannabis related consequences, but only for boredom motives (95% CI [-.75, -.26]), altered perception motives (95% CI [-.52, -.03]), availability motives (95% CI [-.85, -.28]), coping motives (95% CI [-.72, -.21]), and sleep motives (95% CI [-.67, -.18]). The connection between each of these motives and cannabis-related problems was significantly stronger for males vs. females. Conclusions: The results indicate that gender moderated the relationship between boredom motives, altered perception motives, availability motives, coping motives, and sleep motives and cannabis-related problems. The connection between these motives and cannabis-related problems was significantly stronger for males, suggesting that males who use cannabis for these reasons may be at elevated risk for experiencing cannabis-related problems relative to females who use cannabis for these reasons. Interventions that target cannabis-related problems may benefit from tailoring intervention elements based on gender.
