ABSTRACT CONTEXT AND OBJECTIVE: Down syndrome is the most common genetic disorder, affecting 1/700 live births. Among the clinical findings, one constant concern is the high prevalence of congenital heart disease. The objective of this study was to determine the prevalence and profile of congenital heart disease among patients attended at a Down syndrome outpatient clinic in southern Brazil between 2005 and 2013. DESIGN AND SETTING : Cross-sectional study conducted in a referral center. METHODS : Data were retrospectively gathered from the medical files of 1,207 patients with Down syndrome, among whom 604 (50.0%) had been diagnosed with congenital heart disease. These data were subjected to descriptive analysis using the Statistica software. RESULTS : Among the 604 patients with congenital heart disease, 338 (55.8%) were male and 269 (44.5%) were female. The most common heart diseases were atrial septal defect in 254 patients (42.1%); total atrioventricular septal defect in 91 (15.1%); atrial septal defect and ventricular septal defect in 88 (14.6%); ventricular septal defect in 77 (12.7%); patent ductus arteriosus in 40 (6.6%); patent foramen ovale in 34 (5.6%) patients; tetralogy of Fallot in 12 (2%); and other diseases in 8 (1.3%). Pulmonary hypertension was present in 57 (9.4%). Out of the total, 150 patients (24.8%) underwent cardiac surgery. CONCLUSION : The high prevalence of congenital heart disease among the patients at the Down syndrome outpatient clinic (50%) was similar to findings from other studies and justifies investigation during the neonatal period, so as to decrease mortality and morbidity.

Background:Chronic pain is common in rheumatic and musculoskeletal conditions, and a major driver of disability worldwide. Knowledge gaps exist with respect to correct estimates of chronic pain [1], what causes it and how best to manage it [2]. To address this, researchers need validated methods to measure pain in large, representative populations. Though many authors have recognised the potential benefits of paper-based and digital pain manikins [3]–[5], it is unknown to what extent studies have adopted digital manikins as a data collection tool.Objectives:The objective of our review was to identify and characterise published studies that have used digital pain manikins as a data collection tool.Methods:We systematically searched six electronic databases, including Medline, CINAHL, Embase, Scopus, IEEE Xplore digital library, ACM Digital Library, on 3-4 of November 2020 by using a pre-defined search strategy. We included a study in our review if it used a digital manikin for self-reporting any pain aspect (e.g., intensity, type) by people suffering from pain, and if its full text was published in English. We conducted this review by following the PRISMA reporting guidelines and conducted a descriptive synthesis of findings, including manikin-derived outcome measures.Results:Our search yielded 4,685 unique studies. After full text screening of 705 articles, we included 14 studies in our review. Most articles were excluded because they used either paper-based manikins or didn’t include enough details to determine that the manikin was digital (n=386). The majority of included studies were published in Europe (n=11). Most studies collected data on a manikin once (n=11); from people with pain conditions (n=9); and in clinical settings (n=9). There was only one study that collected digital pain manikin data in a large sized (i.e., ~20,000) population-based survey.In most studies participants shaded any painful area on manikin (n=9) and did not enable participants to record location-specific pain aspects (n=11). None of the manikins enabled participants to record location-specific pain intensity. Pain distribution (i.e. number or percentage of pre-defined body areas or locations experiencing pain) and pain extent (i.e. number or percentage of shaded pixels) were commonly used manikin-derived outcome measures. In six studies, a heat map was used to summarise the extent of pain across the population.Conclusion:Digital pain manikins have been available since the 1990s but their adoption in research has been slow. Few manikins enabled location-specific pain recording suggesting that the digital nature of the manikin is not yet fully utilised. Future development of a validated digital pain manikin supporting self-reporting of the location and intensity of pain, usable across any device and screen size, may increase uptake and value.References:[1]S. Aim. To characterize differences in the risk of development (RD) of myocardial infarction (MI) and stroke during 16 years in an open Siberian population aged 25-64 years and suffering vital exhaustion (VE). Materials and methods. We examined a random representative sample of 657 men and 870 women from the Novosibirsk population in the framework of the WHO «MONICA-psychosocial» screening III program (1954). It included registration of socio-demographic data and cases of VE. A total of 15 episodes of MI and 35 strokes were documented in 30 women and 22 men. Results. The level of VE in men was 66,8% (high in 14,6%), in women75,7% (high in 44,4%). RD of MI estimated from the relative risk (RR) in men with VE(RR=2) was higher than in women. It was higher in divorced women (RR=5,4) than in men (RR=4,7). RD of MIwas higher in men with VE (OR=2,2 in subjects having elementary education, OR=3,7 in bachelors, OR = 7 in widowers;at the age from 45 to 54 years OR=3,8, at the age from 55 to 64 years OR=5,9) than in women. The overall RD of stroke in subjects of either sex with VE was not significantly different (OR=3,34 in women and 3,1). However, it was higher than in womenin men with VE having partially completed secondary-leveland elementary education RR=4.8), in divorcedand widowedmen (RR=3.8 and 3.6 respectively. Conclusion.The study showed that the prevalence of VE in the population aged 25-64yearsis higher than in other age groups and higher in women than in men. VE is a more reliable predictor of MI in men than in women; it is a predictor of stroke in either sexes.

ABSTRACTTwo experiments have been carried out to examine the effects of the level of protein supplementation given with grass silage-based diets on the performance and carcass composition of bulls, and to compare diets based on silage and dried forage. The five treatments used consisted of grass silage offered ad libitum and supplemented with 2·5 kg dry matter (DM) of barley-based concentrates containing (1) zero (2) 200 (3) 400 and (4) 600 g soya-bean meal per kg and (5) artificially dried grass and hay supplemented with 3·2 kg concentrate DM. The silages used in both experiments were well preserved, containing on average 200 g DM per kg; 140 g crude protein (CP) per kg DM; 63 g ammonia-nitrogen per kg total N and 731 g digestible organic matter per kg DM. The bulls were of late-maturing breed type and were initially 12 months old and 412 and 405 kg live weight in experiments 1 and 2 respectively. For treatments 1, 2, 4 and 5 in experiment 1 respectively (treatment 3 was not used) total DM intakes were 8·3, 8·3, 81 and 110 (s.e. 0·21) kg/day; CP intakes 1063, 1271, 1664 and 1539g/day; metabolizable energy intakes (MEI) 98, 99, 96 and 87 MJ/day; carcass weights 317, 316, 317 and 316 (s.e. 3·2) kg; carcass saleable meat concentrations 714, 712, 718 and 716 (s.e. 5·8) g/kg and carcass fat trims 73, 81, 73 and 68 (s.e. 3·9) g/kg. In experiment 2 for treatments 1 to 5 respectively total DM intakes were 8·3, 8·5, 8·3, 8·4 and 11·2 (s.e. 0·26) kg/day; CP intakes were 1090, 1329, 1504, 1720 and 1561 g/day; MEI 102, 106, 103, 103 and 94 MJ/day; carcass weights 318, 331, 330, 327 and 321 (s.e. 3·3) kg; carcass saleable meat concentrations 726, 721, 725, 721 and 732 (s.e. 60) g/kg and fat trims 71, 77, 78, 80 and 64 (s.e. 4·5) g/kg. It is concluded that protein supplementation of a silage-based diet did not affect performance or carcass fatness in experiment 1 or carcass fatness in experiment 2, but including 200 or 400 g soya-bean meal per kg concentrate increased performance in experiment 2. Animals given silage produced fatter carcasses than those given dried forage in experiment 2 but not in experiment 1.

El estudio determinó el nivel de flexibilidad, a través del test Sit and Reach en los niños y niñas de los diferentes deportes en formación del Instituto Municipal de Recreación y de Deporte de Tocancipá. Estudio de tipo descriptivo con enfoque cuantitativo, con un rango de edad de la población entre los 8 y 17 años, con la participación de 334 niñas y niños. El mejor registro en el test Sit and Reach, se presentó en niñas a los 14 años con un valor de (25,9±6,4 cm), en niños de (19,3±7,6 cm), para la variable deporte en la que se midieron 13 deportes, capoeira en niñas obtuvo un valor de (29,5±3,8 cm), en niños (24,5±7,1 cm), siendo capoeira el deporte con mejor resultado en ambos sexos, de acuerdo al percentil 90,87 niños y niñas se encuentran por encima del valor, mientras 65 niños y niñas están por debajo del percentil 20, confirmando la hipótesis alternativa, (p: 0,0087).

338 Background: HER2 is highly overexpressed in many kinds of cancers with a poor prognosis. Recently, near-infrared (NIR) fluorescence-based imaging is a growing field for both pre-clinical and clinical application. In this study, we aimed to synthesize Human Epidermal Receptor2 (HER2)-specific near-infrared (NIR) fluorescence probes and evaluate their applicability in cancer-specific image-guided surgeries using an animal model. Methods: An NIR dye emitting light of 800 nm (IRDye800CW, Li-COR, USA) was conjugated to trastuzumab and HER2-specific affibody using click mechanism. HER2 affinity was assessed by the surface plasmon resonance technique. HER2 positive/negative gastric cancer cell lines (NCI-N87 and SNU-601) were subcutaneously implanted into female BALB/c-nu (6 to 8 weeks old) mice. The biodistribution and fluorescence signal intensity were measured by Lumina II (Perkin Elmer, MA, USA) and a laparoscopic NIR camera (InTheSmart, Seoul, Korea) after injecting the probes intravenously. Results: Trastuzumab-IRDye800CW showed higher affinity to HER2 (K(D) = 2.093(3)pM) than unconjugated trastuzumab(K(D) = 25.75pM). The significant signal of fluorescence was targeted to the HER2-positive tumors at 24hr after injection, while no or low signal retention was observed in negative group. The peak appears at 24hr after injection. On the other hand, small difference of affinity was shown between HER2-target affibody-IRDye800CW (K(D) = 4.71nM) and unlabeled pure affibody(K(D) = 1.42nM). The renal clearance of HER2-target affibody conjugated with IRDye800CW was so fast that we could not detect the signal. Conclusions: Our results suggest that trastuzumab conjugated with IRDye800CW can be a feasible tool to monitor HER2 status in pre-clinical cancer imaging. Moreover, this probe can provide complementary means for assessment of HER2 expression in gastric cancer patients and/or be used to further detection of HER2-positive lesions during image-guided surgery.

Introduction: The benefit of intravenous thrombolysis (IVT) in wake-up stroke (WUS), stroke of unknown time of onset (SUKO), or when time exceeds 4.5 h from last-seen-normal (LSN) guided by CT perfusion (CTP) or MRI has been recently suggested. However, there is limited information of IVT in those patients in real-world studies. Objective: Our aim was to evaluate safety and efficacy of IVT selected by CTP in patients with WUS, SUKO, or stroke of time onset beyond 4.5 h. Material and Methods: We studied a prospective cohort of patients who underwent IVT from January 2010 to December 2017. Two groups were defined: standard of care group (SC) included patients with time onset <4.5 h and CTP group included patients with WUS, SUKO, or onset beyond >4.5 h from LSN with penumbra area in CTP. We evaluated baseline characteristics, functional outcomes according to modified Rankin Scale (mRS) at discharge and at 90 days, and intracranial hemorrhages rates. Results: 657 patients were studied: 604 (92%) were treated in the SC group and 53 (8%) in the CTP group. The mean NIHSS score was 9.8 in the CTP group versus 13 in the SC group (p = 0.001). Seventeen patients in the CTP group (32.1%) received bridging therapy with mechanical thrombectomy (MT). Last time seen well-to-needle time was 538 versus 155 min (p < 0.001). The incidence of symptomatic intracranial hemorrhage was equal in both groups (3.8 vs. 3.8%, p = 1). Good functional outcome (mRS < 2) was achieved in both groups (72 vs. 60.4%, p = 0.107). Conclusions: IVT in patients with WUS, SUKO, or stroke beyond >4.5 h from LSN, with salvageable brain tissue on CTP, seems to be safe and has similar functional outcomes at 90 days to the standard therapeutic window, even when combined with MT.

Background: Adequate levels of physical activity combined with a balanced diet are two essential components of healthy lifestyle. Healthy eating and physical activity habits formed during childhood can persist into adulthood and prevent or delay premature onset of a number of chronic diseases.Methods: Present cross-sectional study was carried out using a questionnaire-based interview. It was conducted among school going children of age 8 years to 18 years. A total of 318 students were included in the study by means of total enumeration from class 2nd to class 12th of a selected school of field practice area conducted from July 2019 to September 2019.Results: The total of 318 students participated in the study. Majority of the respondents were involved in Moderate to vigorous physical activity (40.3%). Moreover, majority were found to be normal on the basis of stunting, wasting and body mass index for age 80.2%, 79.2%, 64.4% respectively.Conclusions: Physical activity level was found to be low as the age of children advanced. In female students the level of physical activity was low to moderate as compared to male students. Solutions to improve physical activity and nutritional status in children need to be incorporated for Indian schools’ curricula.

Pneumococcal disease remains an important cause of invasive and non-invasive disease in Scotland and elsewhere. The Scottish Meningococcus and Pneumococcus Reference Laboratory receives isolates of Streptococcus pneumoniae from diagnostic laboratories around Scotland. Here, the serogroups/types and antibiotic-susceptibility patterns of invasive isolates received between 1999 and 2002 are described. There were a total of 1741 invasive isolates received, the most common serogroups/types being 14 (19.8 %), 9 (10.2 %), 6 (8.3 %), 19 (7.9 %), 23 (7.9 %), 4 (6.5 %), 8 (6.4 %), 3 (5.7 %), 1 (3.8 %), 7 (3.8 %) and 18 (3.4 %). Importantly, serotypes 7 and 8 are not represented in the 7-, 9- and 11-valent pneumococcal conjugate polysaccharide vaccines. There were 67 (3.8 %) isolates considered penicillin non-susceptible, although no penicillin resistance (MIC ⩾ 0.002 mg ml−1) was recorded. One hundred and ninety-four (11.1 %) isolates, predominantly of serotype 14, were resistant to erythromycin, and 12 (0.7 %) were resistant to ciprofloxacin. This information provides an important dataset that will prove essential prior to and during the implementation of pneumococcal conjugate vaccines in the UK.

602 Background: The Kyushu Study group of Clinical Cancer conducted a phase II study that evaluated the FIREFOX regimen. (KSCC0701, Akagi et al, J Clin Oncol 28:15s, 2010). This study demonstrated the efficacy and mild neurotoxicity of this regimen. The present study evaluated the efficacy and safety of the FIREFOX plus bevacizumab (bev). Methods: Eligibility criteria included histologically confirmed advanced colorectal cancer, ECOG PS 0-2 and adequate bone marrow, renal and hepatic function. Patients (pts) received an alternating regimen of 4 cycles of mFOLFOX-6 plus bev (oxaliplatin 85 mg/m2, leucovorin 200 mg/m2, bev 5 mg/kg d1 followed by 400 mg/m2 bolus 5-FU and a 46-hr 2,400 mg/m2 5-FU infusion every 2 weeks) followed by 4 cycles of FOLFIRI plus bev (oxaliplatin replaced with irinotecan 150 mg/m2 d1). This schedule was repeated until unacceptable toxicity or disease progression occurred. The primary endpoint is progression-free survival. (UMIN000001312) Results: Of the 52 pts enrolled from May 2008 to July 2009. Two of the patients did not fulfill the eligibility criteria. M/F, 30/20; median age, 59.5 years (range 37 - 75); ECOG PS 0/1/2, 46/4/0. The median number of administration cycles was 14 (range, 2 - 44). Response rate (RECIST criteria) for CR, PR, SD, PD and NE were 2 (4%), 28 (56%), 14 (28%), 4 (8%) and 2 (4%), respectively. An overall response rate was 60% (95% CI: 45 - 74%). Median progression-free survival was14.2 M (95% CI: 10.6 M-16.3 M) and median overall survival was 27.5 M (95% CI; 22.4 M – not determined). The 2-year survival rate was 56.8%. Of the 52 pts evaluated for toxicity. The most common grade 3-4 adverse events were leukopenia (7.7%), neutropenia (32.7%), anemia (1.9%), fatigue (9.6%), anorexia (13.5%), stomatitis (3.8%), neurotoxicity (3.8%), hypertension (1.9%), diarrhea (7.7%), febrile neutropenia (3.8%), nausea (9.6%), vomiting (5.8%), hypersensitivity (3.8%), and thromboembolism (1.9%). Conclusions: The results of this phase II study show that the FIREFOX plus bev regimen is effective and well tolerated in the first-line treatment of advanced colorectal cancer. The low rate of neurotoxicity is also promising.

AbstractFine grained 8 mol% yttria-stabilized zirconia (8YSZ) transparent ceramics with high optical and mechanical properties were fabricated by air pre-sintering and hot isostatic pressing (HIP) using commercial 8YSZ powders as the raw material. The pre-sintered ceramics with fine grains and appropriate relative density play a key role to achieve high transparency and suppressed grain size after HIP post-treatment at relatively low temperatures. With the increase of HIP temperature from 1350 to 1550 °C, the in-line transmittance of 8YSZ ceramics at 600 nm increases from 56.9% to 71.5% (2.5 mm in thickness), and the average grain size increases from 2.4 to 16.3 µm. The corresponding bending strength of 8YSZ transparent ceramics decreases from 328±20 to 289±19 MPa, the hardness (H) decreases from 12.9±0.1 to 12.5±0.2 GPa, and the fracture toughness (KIC) decreases from 1.30±0.02 to 1.26±0.03 MPa·m1/2. Systematical investigations were carried out to study the combination of high optical transparency and excellent mechanical properties in 8YSZ ceramics.

Penelitian ini bertujuan untuk mengetahui pengaruh substitusi tepung daun singkong terfermentasi Aspergillus niger terbaik terhadap kualitas fisik daging dada ayam joper umur 8 minggu, meliputi nilai pH, DIA, dan susut masak. Penelitian ini dilaksanakan pada Januari – Maret 2022 di Laboratorium Nutrisi dan Pakan Ternak, Jurusan Peternakan, Fakultas Pertanian, Universitas Lampung dan Laboratorium Lapang Terpadu, Fakultas Pertanian, Universitas Lampung. Penelitian ini menggunakan Rancangan Acak Lengkap (RAL) dengan 5 perlakuan (0%, 5%, 10%, 15%, dan 20%) tepung daun singkong terfermentasi Aspergillus niger yang diulang 4 kali dengan total 20 unit petak percobaan, masing-masing unit berisi 4 ekor ayam joper tanpa dibedakan jantan ataupun betina, sehingga jumlah ayam joper 80 ekor. Peubah yang diamati meliputi nilai pH, DIA, dan susut masak. Data yang diperoleh dianalisis menggunakan analisis ragam (ANOVA) pada taraf nyata 5%. Hasil penelitian menunjukkan bahwa perlakuan substitusi tepung daun singkong terfermentasi Aspergillus niger 0%, 5%, 10%, 15%, dan 20% tidak berpengaruh nyata (P>0,05) terhadap nilai pH, DIA, dan susut masak daging dada ayam joper umur 8 minggu. Substitusi daun singkong terfermentasi Aspergillus niger hingga taraf 20% masih dapat ditoleransi oleh ayam joper, dengan perolehan rata-rata nilai pH (6,04—6,11), nilai DIA (55,08--60,97%), dan susut masak (19,02--19,04%).

The intermetallic cadmium compounds LaPdCd2 and PrNi0.951(4)Cd2 were synthesized from the elements in sealed tantalum tubes in an induction furnace. Both phases were investigated by X-ray diffraction on powders and single crystals: MgCuAl2-type, Cmcm, Z = 4, a = 431.9(1), b = 1015.7(4), c = 835.7(2) pm, wR2 = 0.0436, 326 F2 values, 16 variables for LaPdCd2 and a = 420.26(8), b = 981.0(2), c = 815.3(1) pm, wR2 = 0.0404, 604 F2 values, 17 variables for PrNi0.951(4)Cd2. A small nickel deficit was observed for the PrNi0.951(4)Cd2 crystal. The cadmium atoms build up orthorhombically distorted three-dimensional networks (Cd-Cd distances: 302 - 334 pm) that resemble the structure of hexagonal diamond, lonsdaleite. Together with the palladium (nickel) atoms, [PdCd2] and [Ni0.951(4)Cd2] networks are formed which leave distorted hexagonal channels for the rare earth atoms.

So, this study was done to determine the prevalence of overweight and obesity and its risk factors among MBBS students.Methods: A cross sectional study was conducted among 318 MBBS students of Gauhati Medical College and Hospital, Assam from June to July 2017 by purposive sampling using a pre-designed, semi-structured questionnaire.Results: Prevalence of overweight and obesity was found to be 16.7% and 2.5% and higher among students who were non-vegetaranian, had irregular dietary habit, consume junk foods and carbonated drinks, used Motorized vehicle as a mode of transport, sleep >8 hrs per day and had family history of obesity, hypertension, diabetes mellitus and heart disease.Conclusions: There is need to increase awareness among the students in order to reduce the future burden of obesity associated chronic diseases.

ABSTRACTDiagnostic mutations in the cytomegalovirus UL97 kinase gene are used to assess the level of associated ganciclovir resistance and therapeutic options. The best-known mutations at codons 460, 520, or 591 to 607 individually confer 5- to 10-fold-decreased ganciclovir susceptibility, except that a 3-fold decrease occurs in the case of the amino acid substitution C592G. Less common point and in-frame deletion mutations at codons 591 to 603 remain incompletely characterized. The ganciclovir susceptibilities of 17 mutants in this codon range were evaluated by use of the same recombinant phenotyping system and extensive assay replicates in two types of cell cultures. Amino acid substitutions K599E and T601M conferred no ganciclovir resistance, while A591V conferred 3.8-fold-decreased susceptibility. In-frame deletions of three or more codons conferred at least 8-fold-increased ganciclovir resistance, while the level of resistance conferred by one- or two-codon deletions varied from 4- to 10-fold, depending on their location. Measured levels of ganciclovir resistance were closely comparable when assays were performed in either fibroblasts or modified retinal epithelial cells. The significant revision of a few previously published resistance phenotypes and the new data strengthen the interpretation of genotypic testing for cytomegalovirus drug resistance.

Objective To compare the efficacy and safety of a triple nucleoside combination to a protease inhibitor-containing triple regimen as first-line antiretroviral therapy (ART) in HIV-1-infected patients. Design Open-label study in HIV-1-infected ART-naive adults, randomized to receive either Combivir® (lamivudine 150 mg/zidovudine 300 mg twice daily) + abacavir (300 mg twice daily), or Combivir® + nelfinavir (750 mg every 8 h) for 48 weeks. Plasma HIV-1 RNA, CD4 cell count and adverse events were assessed at baseline and weeks 4, 8, 16, 24, 32, 40 and 48. Results 195 subjects (131 men, 64 women), median age 34 years, were randomized: 98 received combivir/abacavir and 97 combivir/nelfinavir. Baseline median plasma HIV-1 RNA was 4.2 log10 copies/ml [Interquartile range (IQR): 3.7-4.5.2] and 4.1 log10 copies/ml (IQR: 3.8–4.6), respectively. Baseline median CD4 cell count was 387 cells/mm3 (IQR: 194–501) and 449 cells/mm3 (IQR: 334–605), respectively. Nine patients (3 vs 6, respectively) did not start treatment or did not have any available efficacy data. At week 48, using the intent to treat analysis (switch/missing equals failure), plasma HIV-1 RNA was <50 copies/ml in 54/95 (57%) and 53/91 (58%) of subjects, respectively. Median CD4 increase was +110 and +120 cells/mm3, respectively. Possible hypersensitivity reactions to abacavir were reported in four subjects (4%). Conclusion The triple nucleoside combination combivir/abacavir is well tolerated as a first-line ART regimen in HIV-1-infected adults, with comparable antiviral activity to a nelfinavir-containing regimen at week 48.

У роботі наведені результати проспективного дослідження діагностики та лікування 104 пацієнтів із варикозною хворобою нижніх кінцівок клінічного класу С1 (телеангіектазії та ретикулярні вени). Вперше запропоновано та доведено ефективність застосування програми VEIN SEEK pro як для діагностики, так і в процесі лікування. За допомогою фотодокументування та за результатами анкетування пацієнтів за шкалою Global Aesthetic Improvement Scale, порівняно результати лікування пацієнтів методом черезшкірної лазерної коагуляції та пінної склеротерапії. За результатами дослідження визначено, що найчастіше ускладнення при використання запропонованих методів лікування є залишкова пігментація, яка спостерігається у 3,8% хворих, у яких застосовано черезшкірну лазерну коагуляцію, та у 6,4% – склеротерапію. Ключові слова: варикозна хвороба, ретикулярні вени, телеангіектазії, склеротерапія, черезшкірна лазерна коагуляція.

This work outperforms the previous literatures by proposing a delay-cell-controlled voltage control oscillator (VCO) design for common unipolar, single-gate, and enhancement-mode thin-film transistor (TFT) technologies. A design example with InZnO TFTs is simulated to verify the proposed design. The design example has a 500 μW power consumption, 0.7 mm2 area, 3.8 kHz–8 kHz output frequency range, 600 Hz/V tuning sensitivity, and 4% maximum linear error. This design may have the potential to be used for flexible, low cost, and moderate speed sensor readout interfaces.

We report a new age for the Sääksjärvi impact structure, Finland, a 6 km diameter feature that formed in crystalline rocks of the Precambrian Baltic Shield. Two previous studies reported 40Ar/39Ar data for Sääksjärvi and suggested conflicting formation ages of ≤330 Ma or c. 560 Ma. The former age represents a possible complication for models which indicate that the region was covered by sediments of the Caledonian foreland basin throughout much of the Phanerozoic. We conducted a study combining imaging, microstructural analysis and U–Pb dating of shocked zircon from Sääksjärvi. The U–Pb dataset indicates a c. 600 Ma impact into predominantly c. 1850 Ma target rocks. A concordia age of 608 ± 8 Ma (2σ) confirms Sääksjärvi as the first known Ediacaran impact structure in the Baltic Shield and only the second worldwide. Our data indicate that the Sääksjärvi impact structure formed in exposed crystalline basement rocks of the Baltic Shield prior to the development of the Caledonian foreland basin. Given that most impact structures on Earth are relatively small features, radiometric dating of small impact structures in crystalline basement may place boundaries on the timing and spatial extent of palaeobasins that might otherwise be difficult to constrain.Supplementary material: Additional imaging and microstructural analysis of zircon, additional concordia plots, and U–Pb data tables are available at https://doi.org/10.6084/m9.figshare.c.5043462

e13064 Background: The introduction in the clinical practice of CDK4 / 6 inhibitors, in combination with endocrine therapy, has been a breakthrough in the therapeutic scenario of hormone receptor+ve/Human Epidermal growth Factor 2–ve (HR+/HER2–) MBC pts. However, despite the outstanding results in terms of OS and quality of life achieved up to now, it is important to consider that the highly selected population of clinical trials doesn’t always accurately predict the clinical impact of therapies in the general population, hence having real world data is an essential tool to guide physicians in daily clinical practice. Methods: In this observational, multicentre, prospective study, we evaluated the safety profile and the incidence of dose reductions, temporary interruptions and treatment discontinuation due to adverse events, in a real–world cohort of Italian pre or postmenopausal HR+/HER2– MBC pts who received a first line combination of ribociclib and letrozole. Results: A total of 78 MBC patients were included in the analysis. The median duration of treatment was 17 (1–61) months. At the time of ribociclib initiation, 13 (16.7%) pts already presented an altered transaminase value whereas liver metastases were present in 17 pts (21.8%). Ten (12.8%) pts experienced hepatic toxicity while on treatment. The median number of cycles for liver toxicity was 3 (1–18). The most common hepatic event was transaminase level increase in 7 (8.8%) pts. Such event occurred more frequently in the group of pts with normal baseline liver function. Hepatic toxicity was grade 1 in 3 (3.8%) pts, grade 2 in 3 (3.8%) pts, grade 3 in 3 (3.8%) pts and grade 4 in 1 (1.3%) pt. Dose reduction due to hepatic toxicity occurred in 3 (3.8%) pts, in particular one of them needed dose reduction from 600 mg to 400 mg, one from 600 mg to 200 mg whereas treatment discontinuation was applied in the last patient. Median time of treatment interruption due to hepatotoxicity was 29 (14–45) days and 3 (3.8 %) pts discontinued ribociclib due to adverse events. No deaths due to liver failure have been reported. Conclusions: Hermione–8 trial support a manageable safety profile of the combination ribociclib and letrozole in MBC patients treated in a real–world clinical setting, confirming toxicity profile in line with the results presented in the MONALEESA clinical trials. Dose reductions, temporary interruptions and discontinuation of treatment due to adverse events have been limited and seems comparable to the results from MONALEESA trials. [Table: see text]

Background: Goiter is one of the most visible manifestations of iodine deficiency disorders (IDD) that is caused due to overstimulation of thyroid as an adaptation to Iodine deficiency. IDD is among the major public health threats and important micronutrient deficiencies in developing countries.Methods: This cross-sectional study was conducted among 6-12 years school children in district Chamba of Himachal Pradesh from February to June 2019. The sample size of 336 was calculated and a total of 504 children from government schools were enrolled in the study. The assessment of goitre was done clinically by inspection and palpation of thyroid gland.Results: In this study, 504 school aged children of 6-12 yrs from district Chamba with mean age of 9±1.86 years were included. The prevalence of total goitre rate was 16% with mean age of 8.75±1.37 years. The highest prevalence (%) was observed in age of 10 years (28.4%) followed by 8 years and 9 years (25.9 % and 23.5% respectively).Conclusions: This study showed mild goitre prevalence in school aged children of 6-12 years in district Chamba of Himachal Pradesh. There is dire need of periodic survey to assess the magnitude of IDD in future.

601 Background: Pts with mCRC whose disease progressed after 5-FU, oxaliplatin, irinotecan and monoclonal antibodies have an unmet medical need. There is growing evidence suggesting an antitumoral effect of metformin in several tumor types, including CRC. Methods: Our primary objective was to evaluate the efficacy and safety of MetFU in heavily pretreated CRC pts with current progressive disease Last dose of 5-FU was administred at least 4 months prior to enrollment. Efficacy was defined as disease control rate at 8 weeks, using RECIST 1.1. Secondary endpoints were progression free survival, overall survival and tolerability. Single-arm Simon two-stage phase II trial was used. The treatment consisted of metformin 850 mg bid continuously plus 5-FU 425mg/m2 + Leucovorin (LV) 50 mg weekly for 4 weeks until disease progression, unacceptable toxicity or consent withdrawn in pts with mCRC who had progressed to conventional lines of treatment. Results: In the first stage, 22 pts were included: 12 pts (54%) were men, median age was 55 years and 59% were classified as an ECOG 1.14 pts faced treatment adverse events and 4 pts were excluded due to toxicity G3/4 - 2 pts had thrombocytopenia and 2 had limiting fatigue. Median time on treatment was 3.8 months, and 17 pts were evaluable for response: 6 pts (27%) had stable disease at 8 weeks as best response, with a median progression free survival (PFS) of 8.1 months. For the whole cohort, median overall survival was 5.6 months (IC95%: 3.1-8.2) and PFS was 2.0 months (IC95%: 1.8-2.3). Conclusions: Our results suggest that metformin may have antitumor activity when combined with 5-FU/LV in a subgroup of mCRC pts, with acceptable toxicity. It is unlikely that 5-FU alone had activity in these heavily treated pts. Clinical trial information: NCT01941953.

The effectiveness of three concentrations of either dikegulac-sodium or mefluidide on the growth of two containerized woody vine crops was investigated. A single application of dikegulac-sodium at 1600, 3200, or 4800 mg·liter-1 or mefluidide at 600, 1200, or 1800 mg·liter-1 was applied to either 3.8-liter containers of Asiatic jasmine (Trachelosoerum asiacticum Siebold and Zucc.) or staked confederate jasmine (Trachelosperum jasminiodes (Lindl.) Lem.) on 25 May 1993. Two additional applications were made at 8 week intervals after transplanting to 9.5-liter containers. Plant growth and phytotoxicity were evaluated 0, 4, and 8, and 1, 2, 4, and 8 weeks after application, respectively. Dikegulac-sodium at 3200 mg·liter-1 was the optimum treatment for suppressing the lateral growth of Asiatic jasmine and the vertical growth of confederate jasmine with minimal phytotoxicity. Dikegulac-sodium at 4800 mg·liter-1 excessively inhibited growth of both species and resulted in unacceptable phytotoxicity. All mafluidide treatments had minimal growth inhibitory effect on either species.

TPS506 Background: Which is superior to treat hepatocellular carcinoma (HCC), surgery or percutaneous ablation? This has been one of the most important questions in clinical research of hepatology. However, it has long remained unanswered because it is difficult to approach, as we mentioned previously (Ann Surg 2008;247:557-8). Thus, in 2009, we started this multicenter study in Japan to reach a strong conclusion to the above question (UMIN:000001795). Methods: The major inclusion criteria are: tumor foci numbering three or fewer, and each measuring 3 cm or less); Child–Pugh score of 7 or less; aged between 20 and 79 years; and indications for either surgical resection or radiofrequency ablation (RFA), as described previously (Hepatol Res 2010;40:851-2). In the study design, we adopted two co-primary end-points: overall survival (OS) and recurrence-free survival (RFS). If assuming 3-year RFS following surgery and RFA are assumed as 45% and 35%, respectively, 600 cases are regarded as necessary. Because 600 cases would be also sufficient to detect 10% difference in OS, the target number of cases was set as 600. However, the recruitment was very difficult. The Independent Data Monitoring Committee recommended us to transiently stop the recruitment at the end of August 2015 when the number of the registered cases reached 308. At submission of this abstract, we are considering whether the current number (308) is enough or not to detect 10% difference in RFS by calculating conditional predictive power. Abstract Pancreatic cancer is one of the deadliest cancers primarily because most patients are diagnosed at advanced, unresectable stages. To reduce the incidence and mortality of pancreatic cancer, it is crucial to timely identify and optimally treat its precursors. One such precursor is intraductal papillary mucinous neoplasm (IPMN) as some of these cyst lesions have potential to progress to invasive cancer. Current clinical guidelines for predicting and treating malignant IPMN have a satisfactory sensitivity (&gt;90%) but a dismal specificity (25-30%), leading to a number of unnecessary pancreatic resections in benign IPMN. Therefore, there is an urgent unmet need to identify molecular biomarkers to improve malignant IPMN prediction. The present study was thus conducted to identify metabolomic biomarkers that can preoperatively differentiate malignant IPMN from benign IPMN. A global plasma metabolomic analysis of 607 metabolites was performed on Xevo GS-X2 quadrupole time-of-flight (TOF) mass spectrometers (MS) using a 2D column configuration in 125 cases of low/moderate-grade IPMN and 50 cases of high-grade/invasive IPMN, confirmed by surgical pathology. Of the 607 metabolites, 19 were selected based on their p values for the differences between the two groups compared (all p&lt;0.005). All potential combinations (1 to 8 metabolites) of the 19 metabolites were examined by 3-fold cross validation. The training and test sets included 67% and 33% of total samples, respectively, while keeping the same ratio of low/moderate-grade IPMN to high-grade/invasive IPMN. We repeated such cross validation 100 times by random selection of the training and test sets. The metabolite combination that can distinguish malignant IPMN from benign IPMN with the highest average AUC (area under the receiver operating characteristics curve) (=0.70) consisted of four lipid-related metabolites. AUCs (95% CIs) were 0.63 (0.53 - 0.72), 0.64 (0.55 - 0.73), 0.65 (0.56 - 0.74), and 0.64 (0.54 - 0.73) for the metabolites 1-4, respectively. CA19-9 is the only established biomarker for pancreatic cancer. As shown above, AUCs for the four individual metabolites and for their combination were greater than the AUC of CA19-9 [0.58 (0.47 - 0.69)]. Adding the four metabolites and CA19-9 to the model that contained clinical and imaging variables (including age, sex, BMI, main pancreatic duct diameter, and radiographic main duct involvement) yielded an AUC (95% CI) of 0.82 (0.75 - 0.90). In summary, we identified a plasma signature of four metabolites that outperformed CA19-9 in the discrimination of malignant IPMN from benign IPMN. If replicated in other independent patient populations, the findings of the present study can aid in the risk stratification of IPMN patients prior to surgery. The gastrointestinal tract of 14 false killer whales, 6 males and 8 females, stranded in June 1995 in southern Brazil, with total standard lengths from 338 to 507 cm, were analysed for endoparasites and food items. A pregnant female had a male foetus of 77.5 cm. Parasites were found in all 14 false killer whales. The nematode Anisakis simplex (Rudolphi, 1809) was found in the stomach of 57% of the animals and the acanthocephalan Bolbosoma capitatum (Linstow, 1889) Porta, 1908 was present in the intestine of all specimens and showed densities up to 600 m-1. An unidentified cestode (Tethrabothridae) was found also in the intestines of 14% of the individuals. The high infections of B. capitatum and A. simplex were not directly related with the cause of death. In the stomachs of four females, beaks of at least eight specimens of the oceanic and epipelagic species Ommastrephes bartramii (Lesueur, 1821) were found, with mantle lengths ranging from 189.8 to 360.9 mm. The distribution of O. bartramii in the coast of Rio Grande do Sul is consistent with false killer whales feeding in continental shelf waters.

Background: Worldwide obesity prevalence has nearly doubled since 1980. Current guidelines recommend that “overweight” and “obese” individuals lose weight through engaging in lifestyle modification involving diet, exercise and other behaviour changes. It is necessary to prevent obesity so that the subsequent complications can be prevented.Methods: This community based cross sectional study was conducted in the field practice area of tertiary medical college from June 2019 to August 2019. Totally 154 participants enrolled in the study. Only those individuals who had body mass index (BMI) of more than 23 kg/m2 were included in the study.Results: Majority of the study subjects 57.1% (88) belonged to the age group of 21-40 years. We included only overweight and obese adults in the present study. In this study, 61% (94) of the study population belonged to obese- I category while 33.8% (52) belonged to obese- II and only 5.2% (3) of study population were overweight. Only 43% (66) of the study population had adopted some measures to reduce their weight. Out of these majority 69.7% (46) preferred walking while 12.1% (8) preferred jogging for reducing weight. Mean days of walking adopted was 3.7 days and mean minutes per day was 29.67 minutes.Conclusions: As seen in this study more than half of the study population had not taken any measures for weight reduction. Increased health awareness will improve their level of active participation in the weight reduction program such as diet modification and physical exercise promotes weight loss and weight maintenance.

Background: Adolescence is defined by WHO as the age group of 10-19 years. In India, adolescents (10-19 years) constitute about 21.4 percent of the population, comprising one fifth of the total population. Adolescents are apparently healthy people but they may also have various kinds of problems. If they are to reach adulthood in a healthy state, then it’s necessary to analyze the common health problems prevalent in this age-group so that targeted and concerted services could be provided to them. Objective of current study was to assess the health status of adolescents in urban field practice area of tertiary care hospital in Central India. Methods: A community based descriptive cross-sectional study was conducted in urban field practice area of Government Medical College and Hospital Nagpur. House to house survey was conducted among 330 adolescents out of which 214 (64.84%) were male and 116 (35.16%) were female. Results: In present study most common morbidity found was dandruff in 127 (38.48%) adolescents followed by acne vulgaris in 102 (30.90%). Anemia was present in 52 (15.75%) of study subjects. Out of 237 morbid study subject, 114 (48.10%) study subjects had single morbidity followed by 2 and 3 morbidities in 52 (21.94%) and 63 (26.59%) respectively. About 8 (3.37%) study subjects had more than 3 morbidities. Conclusions: The present study shows that adolescents experience various health problems. Predominant morbidities observed in study subjects are dandruff (pityriasis capitis), acne vulgaris, anaemia, dental carries.

331 Background: Statins have been shown to possess antiproliferative activity in-vitro. Synergism with multiple drug combinations has been of great interest in pancreatic cancer. We examined 2 in-vitro cell models for synergism using a combination of fluvastatin and nab-paclitaxel. Methods: Pancreatic cancer cell lines Mia-Paca-2 (MP2) and Su86 were cultivated and seeded to 25,000 cells/mL and subsequently grown in 96 well plates for 24 hours. The cells were then treated with a fixed concentration of fluvastatin in 9 rows, 8 receiving serial 1:2 dilutions 16 times in triplicate of nab-paclitaxel, 1 fluvastatin only row, and 1 untreated. Upon dosing cells were incubated for 72 hrs. Cellular proliferation was determined by sulforhodamine B proliferation assays and read at 570 nm. A nab-paclitaxel only assay was done as well for comparison. Results: The lowest inhibitory concentration of fluvastatin in combination with nab-paclitaxel was between 500-600 micro mole (mm). Fluvastatin alone at these concentrations attenuated cellular proliferation. Synergism was seen on IC50 curves that are available. Anti-proliferative effects were reduced by 1:256 dilution. Conclusions: Preliminary studies show fluvastatin has an in-vitro anti proliferative effect on Su86 and MP2 cells and synergism in combination with nab-paclitaxel. Fluvastatin is more cytotoxic in Su86 than MP2. Future experimental design will focus on in-vivo models.

333 Background: High-risk, localized prostate cancer has a poor prognosis. We hypothesized that adj docetaxel (D) and prednisone and long-term (24 mos) androgen suppression (AS) and radiation therapy (RT) would improve overall survival (OS) and tested this in NRG/RTOG 0521. Results with med follow-up of 5.7 yrs were reported (JCO 37:1159, 2019), showing a benefit of D (HR=0.69, 90% CI: 0.49-0.97, 1-sided p=0.034). Med follow-up is now 10.4 yrs and we report updated results for OS and metastasis (DM). Methods: NRG/RTOG 0521 opened 12/05 and closed 8/09 with targeted accrual of 600 and designed to detect a HR of 0.49, based on improvement in 4-yr OS from 86 to 93%. With 0.05 1-sided type I error and 90% power >78 deaths were required. Pts were stratified by predefined risk groups. Group 1: Gl 9-10, any T; Group 2: Gl 8, PSA<20, T≥T2; Group 3: Gl 8, PSA≥20, any T; Group 4: Gl 7, PSA≥20, any T. maxPSA ≤150. RT dose was 75.6 Gy. Chemo consisted of 6, 21-day cycles of D starting 28 days after RT. Results: Of 612 accrued, 563 were eligible/available for analysis. By risk group 1-4, there were 297, 116, 64, and 86 pts. Med PSA 15 ng/mL. 10-yr OS rates were 64% [95% CI: 58-70%] for AS+RT and 69% [95% CI: 63-75%] for AS+RT+CT (HR = 0.89, 90% CI: 0.70, 1.13, 1-sided p=0.22). However there was evidence of non-proportional hazards (Grambsch-Therneau test, p=0.016). Thus survival was alternatively evaluated with restricted mean survival time (RMST). The difference in RMST at 10 yrs was 0.42 yrs (90% CI: 0.07-0.77, 2-sided p=0.048). Cumulative incidence of DM at 10 yrs was 22% [95% CI: 17-27%] for AS+RT and 20% [95% CI: 15-25%] for AS+RT+CT (2-sided log-rank p=0.29). At 10 years most deaths occurred in risk group 1: 62 in AS+RT and 50 in AS+RT+CT (HR= 0.93, 95% CI: 0.66-1.32, 2-sided log-rank p=0.16). There was no new related Grade 5 toxicity. Conclusions: OS findings, reported after follow-up of 5.7 yrs, demonstrated a small beneficial effect of adding D to AS and RT. With longer follow-up the benefit of D remains, but the HR varies over time and the OS curves have converged. A 6-week experiment with broilers was conducted to study the effect of diet supplementation with organic acids on performance, characteristics of the tibia and femur bones, and the calcium, phosphorus and zinc balance. A total of 320, one-day-old, Ross 308 chickens were randomly assigned to 1 of 8 treatments. A 2 &times; 4 factorial arrangement was used, with two dietary levels of calcium and available phosphorus (standard &ndash; 9.4/9.2 g Ca/kg and 4.3/4.0 g available P/kg, or reduced &ndash; 8.3/8.1 g Ca/kg and 3.7/3.5 g available P/kg, for the starter/finisher feeding phases, respectively), and with diets supplemented with organic acids (none; short chain fatty acids (SCFA), 4.0 g/kg; medium chain fatty acid (MCFA), 2.0 g/kg or SCFA + MCFA, 3.0&nbsp;+ 2.0 g/kg). Broilers fed diets supplemented with SCFA or MCFA displayed a performance similar to those fed the unsupplemented diet (P &gt; 0.05). At 42 days, reducing the dietary levels of Ca and P decreased such bone parameters as tibia yielding load (256 vs. 270 N) and tibia stiffness (171 vs. 184 N/mm), as well as femur breaking strength (342 vs. 369 N), yielding load (233 vs. 250 N), stiffness (164 vs. 174 N/mm), and cortex thickness (1.47 vs. 1.56 mm). The organic acids had no effect on the parameters of the tibias; however, SCFA and SCFA+MCFA increased the yielding load and stiffness of the femurs. The SCFA diet supplementation significantly increased the relative retention of Ca (45.0 vs. 41.1%). There were no significant Ca and P level&nbsp;&times; organic acids interaction effects on performance parameters, bone quality indices or the Ca, P, and Zn balance. It was thus concluded that SCFA can improve the bone quality and Ca balance in broiler chickens when fed either diets with a standard level of Ca and P, or those with reduced levels of these macrominerals. &nbsp;

The aim of this work was to evaluate the effects of the reduction on nutritional levels of diets for semi-heavy hens during the laying peak on performance, egg quality and bone parameters. A total of eight diets were evaluated, each one with five repetitions of eight birds: 1) control: formulated to supply requirements by the birds; 2) control + 0.03% or 600 FTU; 3) 15% crude protein; 2,800 kcal ME; 3.8% Ca and 0.28% aP; 4) diet 3 + 0.03% or 600 FTU; 5) 14% crude protein; 2,750 kcal ME; 3.4% Ca and 0.23% aP; 6) diet 5 + 0.03% or 600 FTU; 7) 13% crude protein; 2,700 kcal ME; 3.0% Ca and 0.18% aP; and 8) diet 7 + 600 FTU. The reduction of the nutritional levels harmed the performance of the birds, particularly egg production and egg mass, which improved by the inclusion of phytase in the diets. However, the parameters for egg quality did not change as the level was reduced down to 15% crude protein; 2,800 kcal ME/kg; 3.80% Ca and 0.280% aP, which was not repeated in the levels with lower nutritional density. Reduction of nutritional levels did not affect the evaluated bone parameters, except for resistance, which improved as phytase was added at the level 600 FTU. Reduction on the diet nutritional levels to 15% crude protein; 2,800 kcal ME/kg; 3.80% Ca and 0.280% aP with the addition of 600 FTU of phytase does not impair egg production neither egg mass and improve the bone health of birds.

Background: In a new episode of the COVID -19 pandemic in Thailand during the beginning of 2021, cases in Samut Sakhon Province mainly occurred in foreign workers and were mostly asymptomatic or had mild disease. To prevent overwhelming the local hospital, a field hospital was established which used chest radiography as one of screening tools for triaging patients. Objective: To determine the clinical utility of chest radiographs as a screening tool for COVID-19 patients who were asymptomatic or mildly symptomatic. Materials and Methods: Six hundred nineteen patients with COVID -19 (confirmed by reverse transcriptase-polymerase chain reaction) were registered at the field hospital at Samut Sakhon provincial sport stadium during 5-8 January 2021 and had chest radiographs taken. The image readings were based on the consensus of two radiologists and a final decision was made by a third radiologist if the first two did not agree. Findings on chest radiographs and clinical outcomes were evaluated. Results: The study included 619 radiographs; 328/619 (53%) men and 291/619 (47%) women had a mean age of 33.3+/- 9.7 (range, 5-64) years. There was mild disease in 13/619, and asymptomatic infections in 606/619. Chest radiographs were normal in 568 (91.7%) and abnormal in 51 (8.3%) patients; typical findings of COVID-19 were seen in 3 (0.5%) patients. Other abnormal findings were found in 23 (3.8%) patients such as active tuberculosis in 6 (1.0%). Four patients were transferred to the hospital, one of whom required supplemental oxygen. Conclusion: Combined chest radiographic and clinical information allows better decisions regarding hospital transfers of asymptomatic and mildly symptomatic COVID-19 patients at a field hospital.

Context: Successful training involves structured overload but must avoid the combination of excessive overload and inadequate recovery. Objective: The aim of this study was to determine the incidence of functional overreaching (FOR), nonfunctional overreaching (NFOR), and overtraining syndrome in elite female wrestlers during their normal training and competition schedules and to explore the utility of blood markers for the early detection of overreaching. Classification of FOR, NFOR, and overtraining syndrome was based on the European Congress of Sports Medicine position statement. Design: Case series. Setting: China Institute of Sport Science. Patients or Other Participants: Over an 8-year period, 114 wrestlers from the women's Asian wrestling team were monitored to help identify if and when they experienced FOR, NFOR, or overtraining syndrome. Main Outcome Measure(s): Creatine kinase, hemoglobin, testosterone, and cortisol were measured throughout the period to identify whether wrestlers were outside the reference intervals (constructed from normal recovery data) during periods of overreaching and not overreaching. Results: Among the 114 athletes, there were 13 (3.6%) instances of FOR, 23 (6.4%) instances of NFOR, and 2 (0.6%) instances of overtraining syndrome. The diagnostic sensitivity for FOR was 38%, 15%, 45%, and 18% for creatine kinase, hemoglobin, testosterone, and cortisol, respectively. The diagnostic sensitivity for NFOR was 29%, 33%, 26%, and 35% for creatine kinase, hemoglobin, testosterone, and cortisol, respectively. Specificity was 79%, 88%, 90%, and 82% for creatine kinase, hemoglobin, testosterone, and cortisol, respectively. Post hoc analysis showed no mean differences in creatine kinase (F = 0.5, P = .47), hemoglobin (F = 3.8, P = .052), testosterone (F = 0.2, P = .62), or cortisol (F = 0.04, P = .85) between monitoring periods when wrestlers were and were not diagnosed with FOR and NFOR. Conclusions: Coaches and sports scientists should not use single blood variables as markers of overreaching in elite female wrestlers.

Our population in the United Arab Emirates is a multiethnic diverse population.[1, 2]ObjectivesTo create centralised data for JIA in the United Arab Emirates. In this abstract we describe the Demographic data, clinical features, subtype distribution as per ILAR classification, treatment received and outcome of our patients.[3]MethodsRetrospective entry of data from all 3 participating centres (Shaikh Shakhbout Medical City, Tawam and AlJalila). Patients with the diagnosis of JIA included. Electronic clinical research form used to collect data. Data at initial diagnosis and current outcome as per Wallace and (Juvenile Arthritis Disease Activity Score) JADAS 10. Long term out come as per Juvenile Arthritis Disease Activity (JADAI) articular > 0 and extra articular JADAI > 0. All data entered and analysed through Redcap (Research Electronic Data Capture web-based application version 12.1.1 model 2022). Simple statistics used. Ethics approval obtained.Results157 patients entered in to the registry so far. Male to female distribution: 39 (24.8%) to 118 (75.2%). Local Emirati patients of 107 (68.2%). Other Arabs 16 (10.2%). South Asian 12 (7.6%), Western 12 (7.6%) and African 10 6.4%). Mean age at presentation 5.3 years (1 to 15 years). Current mean age 12 (4-22 years). Family history of Rheumatoid Arthritis in 33 patients (21%). History of Consanguinity in 54 families (34.4%).Subtype distribution: Oligo articular 77 (49.4%), extended Oligo articular 5 (3.2%). Poly articular 50 (32.1%), Systemic 11 (7.1%), Enthesitis related Arthritis 1 (0.6%), Psoriatic 9 (5.8%) and other 3 (1.9%).At presentation: Joint pain in 132 patients (84.1%), Joint effusion 115 (73.2%). Affected function in 98 (63.2%). Clinical synovitis in 138 (87.9%). Joint restriction in 100 (63.7%). Uveitis in 6 (3.8%)ANA positive 35 patients (22.3%). Rheumatoid factor positive 10 (6.4%), Anti CCP positive 6 (3.8%). Low Hb less than 10 g/L in 109 (69.4%), high ESR > 20 mm/hr in 30 patients (19.1%). High CRP >5 mg/L in 48 (30.6%).Synovitis in USS 53 (33.8%). Erosions by X-ray or USS 7 patients (4.5%). Uveitis after diagnosis in 30 patients 19.1%. Dental carries at presentation in 51 patients (32.1%). COVID infection in 74 patients (47.1%).Oral systemic steroids given to 108 patients (68.8%). Intravenous methyl prednisolone to 43 patients 27.4%). Triamcinolone Acetonide/Hexa acetonide joint injections to 91 patients (58%). Synthetic Disease modifying medication methotrexate in 108 (68.8%). Biologic treatment 76 patients (49%). Etanercept 64 patients (40.8%), Adalimumab 65 patients (41.4%), Tocilizumab intra venous 32 (20.4%) and sub cut in 10 patients (6.4%). Canakinumab 9 patients (5.7%), Anakinra 8 patients (5.1%) and Infliximab in 10 patients (6.4%). Oral Tofacitinib in 2 patients (1.3%).In regards to outcome as per Wallace criteria and JADAS 10; active disease in 16 patients (10.2%). In remission 141 patients (89.8%). Out of these in remission on treatment 118 (83.7%) and in remission off treatment 23 patients (16.3%). JADAI articular >0 in 8 patients (5%) and JADAI extra articular >0 in 14 patients (8.9%)ConclusionWe have diverse cohort and high percentage of consanguinity in our families of children with JIA (34.4%). Biologic treatment required in 49% of patients. Patients In remission 89.8%. Out of this 83.77% in remission on treatment and 8.9% in remission off treatment. Background: As adolescents are in transit from childhood to adulthood, they are vulnerable to stress and related complications. Measuring stress and its influencing factors is a prerequisite for developing strategy for this age-group. Objective of the study was to assess the level of perceived stress among school going late adolescents and to find out its predictors.Methods: This cross-sectional analytical epidemiological study was conducted among the school going Bengali late adolescents (15-17 years) of a rural block of West Bengal, India within July 2017 to December 2017. Finally, 102 students of class IX and XI were selected via two stage simple random sampling method. Data were collected by interview using pre-designed, pre-tested, semi structured questionnaire including 10 points Cohen’s perceived stress scale (PSS-10). Independent ‘t’ test, ANOVA, Mann Whitney U and Kruskal Wallis H test were utilized to establish the association between independent variables and PSS score. Independent variables found to be associated (p≤0.05) were considered in multiple linear regression to identify the predictors of stress.Results: Mean perceived stress score of participants were 16.8±3.7 (mean±2 SD) out of 40 (highest PSS score) with minimum and maximum score 8 and 26, respectively. In multiple linear regression, de-motivating comment, sibling pattern and love affair were found as predictors of stress perception among Bengali late adolescents. These factors collectively explained 15.6% variations of PSS score and individually contributed 3.8%, 5.6% and 6.2%, respectively.Conclusions: Stress can be alleviated by arranging motivational programme, counselling, and life skill education.

Background: Depression is considered a common mental disorder and a major contributor to the global burden of disease. Depression is estimated to affect 3.8% of the population including 5% of adults and 5.7% among adults older than 60 years, and approximately 280 million people in the world (WHO 2021). Major symptoms of depression include depressed mood, loss of concentration, feeling of excessive guilt, dying thoughts, disrupted sleep, changes in appetite etc. Street vending is informal work that has been contributing in millions of people living status and supporting country’s economy. Methods: A cross-sectional analytical study was conducted among street vendors inside Kathmandu Valley with sample size 316. The study period was from August 2018 to April 2019. Data was collected using standard semi- structured tools using Kobo Toolbox. Data analysis was done in SPSS. Mendeley desktop and statistical package of social science software was used. Results: Out of 316 respondents, higher number were male i.e., 52.2% whereas females were 47.8%. Respondent with age group 25-35 years was high in number. Respondents engaged in street vending for 8-16 years were found high in number. 32.6% respondents had depressive symptoms while measuring with PHQ and the prevalence of depression was 55.4% which was measured using MDI. Conclusions: This cross-sectional study was conducted among the street vendors of Kathmandu valley. Among the respondents with low economic status, lack of physical activity, habit of alcohol and smoking had higher prevalence of depression. Respondent with basic and non- formal education, lack of job and income satisfaction showed a positive association with depression.

Abstract (sommario):
Background: Aim of the study was to assess the clinicopathological profile and outcome of COVID 19 infection in children and newborn in a dedicated COVID tertiary care center.Methods: A retrospective study of a cohort of 105 children (1 month-12 years.) and 128 newborns admitted from 7th July to 7th August 2020. All the admitted children were COVID 19 positive and newborns were delivered from mother with COVID positive status. We collected data from medical records regarding epidemiology, clinical features, comorbidities, investigational report, treatment and outcome of the study population.Results: The median age of 105 children was 3 years with IQR (11 months-6 years) with almost equal sex distribution with higher disease prevalence and severity in younger age group. The mean duration of stay is 10.11 days (SD: 4.93). 35 (33.3%) children were asymptomatic, whereas 53 (50.5%) mildly symptomatic, 8 (7.6%) moderate disease severity, 9 (8.6%) critically ill at time of admission. Fever (57%) and Cough (23.8%) commonest symptoms. Hyper inflammatory syndrome was a new challenge in COVID affected children as well as concomitant infections like Dengue, Scrub and enteric fever, 38 (56%) children had comorbidities and 14 (13.3%) required PICU admission with 4 (3.8%) children. 42 out of 128 neonates became positive being born from 128 COVID positive mother and the rate of longitudinal transmission was more in outborn babies. None of the newborns tested swab positive within 48hrs of life and the preterm and LBW babies were relatively less infected. Most of the neonates were asymptomatic (84%) and we recorded zero death among COVID positive newborn.Conclusions: In the absence of comorbidities and concurrent infections, COVID 19 is relatively benign in Indian children.
