Abstract Introduction Rapid onset obesity with hypothalamic dysregulation, hypoventilation, and autonomic dysregulation (ROHHAD) is a rare disorder associated with sleep disordered breathing. Patients often have obstructive sleep apnea (OSA) and nocturnal hypoventilation (NH). We present a case with a unique sleep disordered breathing pattern demonstrating both severe central and obstructive sleep apnea demonstrated on two separate polysomnograms with no evidence of significant NH. Report of case(s) The patient is a now 8 year old girl who was initially diagnosed with ROHHAD in October 2020 at age 5 years with symptoms of rapid weight gain, hypothalamic dysfunction (hyperprolactinemia), and autonomic dysfunction (recurrent fever, and hypertension). Diagnostic polysomnography in May 2020 demonstrated severe OSA (oAHI 23.4/hr) as well as central sleep apnea (CAI 63.9/hr). The SaO2 nadir was 88% with 15 minutes spent below 90%. There was no evidence of NH, however the CO2 was greater than 50 mmHg for 9.5% of total sleep time. Subsequent titration study recommended BPAP ST of 14/10 cmH20 with back up rate (BUR) of 12 bpm. The patient had a repeat split night study in May of 2022 at age 7 years. During the diagnostic portion, she demonstrated persistent severe OSA (oAHI 39/hr) and central sleep apnea (CAI 21/hr). There was no evidence of NH with a max TCO2 of 50 mmHg and CO2 > 50 mmHg for 0.2% of total sleep time. BPAP ST of 18/10 cmH20 with BUR of 12 bpm led to resolution of obstructive and central apnea events. Conclusion ROHHAD remains a rare disease which is often associated with sleep disordered breathing. While there have been rare reports of CSA in ROHHAD, there have been none of this severity. CSA may be a representation of autonomic dysfunction, and previous literature has described central pauses in breathing in ROHHAD patients while awake. While she did not have evidence of hypoventilation on either polysomnogram, which is one of the hallmarks of this disease, it has been described that nocturnal hypoventilation may develop later in the course of the disease. Thus, it remains important to routinely screen patients with polysomnograms as their sleep disordered breathing evolves. Support (if any)

Abstract Introduction The knowledge about the relationship between sleep and substance use constantly undergoes change. Many questions still remain, including how substance use initiation correlates to sleep parameters. In order to expand this body of knowledge, this study aimed at evaluating how lifetime substance use predicts changes in objective and subjective sleep parameters. Methods Volunteers from the EPISONO 2007, a large-scale epidemiological study assessing sleep in a representative sample from the city of São Paulo, Brazil, filled in questionnaires evaluating their general health, and underwent a full-night polysomnographic exam. For this analysis the first question of the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST), asking if the individual ever used any of the listed substances, was considered. Age and scores in the Beck Depression and Anxiety Indexes were included as covariates. Gender was also added as an independent variable. Results From the total 1,042 participants, 876(388 men and 488 women) were included. Volunteers reported use of cannabis(n=181), cocaine(n=79), inhalants(n=69), amphetamines(n=54), sedatives(n=31), alcohol(n=802) and tobacco(n=538). Lifetime use of cannabis, cocaine and sedatives directly correlated to alterations in sleep latency, REM sleep onset latency, sleep efficiency and scores in the Epworth Sleepiness Scale. Gender significantly interacted with substance use, with women who consumed cocaine and cannabis and men who never did presenting increased total sleep time when compared to their counterpart groups (men who ever consumed these substances and women who never did). Conclusion We found not only an effect of the lifetime consumption of popular drugs, such as cocaine and cannabis, but also identified an important role for gender, which seems to have inversed roles in males and females. These results provide an intriguing lead to unravelling a network in which gender, sleep and substance use are present, and advances in this area will be a major step in the investigation of the importance of sleep in the psychopharmacology scenario. Abstract Introduction Sleep disturbances are common during pediatric cancer treatment and recent evidence suggests a correlation between sleep and symptom burden. Improving nighttime sleep may impact patients’ ability to cope with symptoms. The current study tests the interaction between sleep hygiene and sleep disturbances in predicting cancer-related symptoms to determine if the relationship between sleep and symptoms is different for patients with better sleep hygiene. Methods 102 caregivers of children with cancer (ages 5-17, M=10.12, SD=4.02; 58% female) completed parent-proxy Pediatric Sleep Practices Questionnaire (yielding routine consistency and sleep opportunity scores) and measures of cancer-related symptoms (PROMIS Sleep Disturbance, Fatigue, and Pain Interference; PedsQL—Cancer Module-Nausea subscale). The interaction between sleep disturbances and sleep hygiene (consistency, opportunity) on each symptom (pain, nausea, fatigue) were tested using PROCESS moderation. Results 81% of caregivers report that their child receives sufficient sleep but only 12% reported regular consistent sleep patterns/routines. Sleep opportunity was not related to sleep disturbances or cancer-related symptoms, but more routine consistency was related to fewer sleep disturbances (r=.30, p=.003). The interaction between sleep disturbances and routine consistency significantly predicted pain interference [R2=.16, F(3, 98)=6.37, p<.001; Bint =-0.17, p=.028] and nausea [R2=.16, F(3, 98)=6.47, p<.001; Bint=0.46, p=.004]. The interaction between sleep disturbances and sleep opportunity significantly predicted nausea [R2=.15, F(3, 98)=5.76, p=.001; Bint =0.68, p=.016] but not pain interference. Both interaction models predicting fatigue were not significant. Conclusion The sleep/pain and sleep/nausea relationships are stronger in patients with more consistent sleep routines and the sleep/nausea relationship is also stronger in patients with sufficient/well-timed sleep opportunities. Sleep and fatigue were moderately related across all levels of both sleep hygiene components. Clinical interventions that target sleep hygiene together with sleep disturbances such as nighttime awakenings and poor sleep quality may be more effective in addressing cancer-related symptoms such as pain and nausea. Cattle grazing in riparian pastures adjacent to rivers may increase soil compaction and increase soil nutrients, such as N and P. We conducted a 4-yr study with sampling in 3 yr (2009, 2010, 2012) of riparian zone soils adjacent to fenced and unfenced reaches of the Lower Little Bow River in southern Alberta. We examined the effect of grazing, access of cattle to the river (access versus no-access), and distance (0.25, 1, 2, 4, 6, 8, 10 m) from the river on surface soil bulk density, volumetric water content, NH4-N, NO3, and soil test P. Penetration depth was also measured in 2012. The three grazing treatments consisted of one fenced reach (ungrazed treatment), one unfenced and grazed reach with high cattle impact (high-impact grazed treatment), and one unfenced and grazed reach with low cattle impact (low-impact grazed treatment). We hypothesized that soil compaction would be greater, soil nutrients would be enriched, and soil water content would be lower for grazed compared with ungrazed treatments, and that this same trend would occur for access compared with no-access locations. The soil properties in our study were generally significantly (P≤0.05) influenced by grazing, access, and distance from the riverbank. However, treatment effects were generally dependent on two- or three-way interactions with the other factors. Soil bulk density in 2009 and 2012 was 8 to 20% greater at access compared with no-access locations within 2 m of the riverbank, suggesting soil compaction by cattle was confined close to the wetter riverbank soils. Most soil properties generally supported our hypothesis of greater soil compaction and nutrient enrichment for unfenced compared with fenced reaches, as well as for access compared with no-access locations. The exceptions were soil water content and soil test P results that did not support the grazing hypothesis, and soil water content and NH4-N results that did not support the cattle-access hypothesis.

We present a case report on a spot diagnosis of Thyrotoxic Periodic Paralysis (TPP) with a unique first-person account of events from the patient. It illustrates the importance of pattern recognition and exemplifies how timely treatment enables quick resolution of a life-threatening medical emergency. Patient X’s account affirms the condition’s insidious onset and rapid deterioration. This case highlights the need for raising awareness of diseases that are more prevalent in specific ethnic groups and is particularly crucial for work in culturally diverse environments. We hope by sharing our experience, readers will be prompted to consider TPP as a differential diagnosis for acute limb weakness in an acute setting; with prompt testing of thyroid function and initiation of the appropriate treatments.

BackgroundTreatment of recurrent/refractory (r/r) DLBCL remains an unmet clinical need, and new effective and well-tolerated treatments are required. DPX-Survivac is a unique T cell activation therapy that targets survivin-expressing tumor cells and has shown anti-tumor activity in clinical trials. This trial is evaluating a novel immunotherapy combination with DPX-Survivac, intermittent low dose CPA and pembrolizumab.Methods‘SPiReL’ is a Phase 2 non-randomized, open label, efficacy and safety study of a novel immunotherapy combination with DPX-Survivac (a unique T cell activation therapy that targets survivin-expressing tumor cells), intermittent low dose CPA and pembrolizumab, treatment regimen as described in figure 1. Subjects with r/r incurable DLBCL and survivin expression are eligible for participation. This study was approved by the Ontario Cancer Research Ethics Board, approval number 0981.ORR is assessed by modified Cheson criteria. For translational analyses, baseline and on-treatment PBMCs, along with tumor biopsy samples are collected from each subject. Survivin-specific systemic T cell responses are assessed using IFNγ-ELISPOT assay and tumour immune-infiltrate profile by multiplex-IHC.ResultsTwenty-two subjects have been enrolled to date, 19 are included in the intent to treat (ITT) population and 11 subjects are evaluable in the per protocol (PP) population. In the PP, the ORR is 63.6% including 3 CRs (27.3%), 4 PRs (36.4%) and the DCR is 81.8% (9/11). In the ITT, the ORR is 35% (7/19), and DCR is 52.0% (10/19). Preliminary results show that non-GCB subjects had a higher proportion of clinical response (4/8, 50%), compared to 3/10 (30%) in GCB subjects.DPX-Survivac-induced T cell responses were observed in 8/19 subjects (42.1%) including 6 subjects with clinical response (PR, CR), one SD and one PD. Multiplex-IHC analyses demonstrated baseline tumor PD-L1 expression in 6/7 subjects with a clinical response (85.7%, p<0.05).
