Clostridioides difficile is an opportunistic gut pathogen which causes severe colitis, leading to significant morbidity and mortality due to its toxins, TcdA and TcdB. Two intra-muscular toxoid vaccines entered Phase III trials and strongly induced toxin-neutralising antibodies systemically but failed to provide local protection in the colon from primary C. difficile infection (CDI). Alternatively, by immunising orally, the ileum (main immune inductive site) can be directly targeted to confer protection in the large intestine. The gut commensal, non-toxigenic C. difficile (NTCD) was previously tested in animal models as an oral vaccine for natural delivery of an engineered toxin chimera to the small intestine and successfully induced toxin-neutralising antibodies. We investigated whether NTCD could be further exploited to induce antibodies that block the adherence of C. difficile to epithelial cells to target the first stage of pathogenesis. In NTCD strain T7, the colonisation factor, CD0873, and a domain of TcdB were overexpressed. Following oral immunisation of hamsters with spores of recombinant strain, T7-0873 or T7-TcdB, intestinal and systemic responses were investigated. Vaccination with T7-0873 successfully induced intestinal antibodies that significantly reduced adhesion of toxigenic C. difficile to Caco-2 cells, and these responses were mirrored in sera. Additional engineering of NTCD is now warranted to further develop this vaccine.

Abstract Introduction Cases of depressive symptoms are highly prevalent among older adults. Moreover, depression among older adults is strongly associated with cognitive impairment, with a prevalence of approximately 8% to 16% of aging adults (Blazer, 2003). Therefore, this study sought to investigate and examine the interplay between physical activity, insomnia, and depressive symptoms among cognitively impaired older adults without dementia using data from the Health and Retirement Study. Methods Depressive symptoms and insomnia levels are based on self-reports. We used a negative binomial to assess these relationships and present the incident rate ratios (IRRs) and 95% confidence intervals from our analysis of depressive symptoms associated with insomnia and moderate physical activity. Results We observed that insomnia (IRR: 1.18, 95% CI: 1.13-1.24) and moderate physical activity (IRR: 1.13, 95% CI: 1.07 – 1.19) are positively associated with an increase in depressive symptom levels among older adults. Model Two included an interaction term for insomnia and physical activity; results indicate that the interaction term is negative (IRR: .97 95% CI: .94 - .99). Model three tested gender to moderate the relationship between physical activity and depressive symptoms. Conclusion The results from the non-significant interaction term suggest that this relationship is non-significant. Similarly, the interaction term (gender x physical activity) results show that the link between physical activity and depressive symptoms did not vary by gender. Support (if any)

Abstract Introduction The intricate relationship between atrial fibrillation (AF) and obstructive sleep apnea (OSA) has garnered attention due to its potential impact on morbidity and mortality. This study explores the characteristics of individuals with both AF and OSA, aiming to shed light on associated outcomes. Methods Utilizing data from the Geisinger Health System (2007-2023), we focused on cardiology patients with AF. Inclusion criteria comprised age > 18, AF and OSA diagnosis, and polysomnography with an apnea-hypopnea index (AHI) > 5. Data collected included age, gender, body mass index (BMI), mean AHI, oxygen desaturation index (ODI), ablation, cardioversion rates, and electrophysiology (EP) clinic follow-ups. Results Among 29,000 patient encounters, 2,971 unique patients met inclusion criteria. 64% were male, with a mean age of 74.9 for deceased patients versus 68.1 for the alive cohort (p < 0.001). Deceased patients had a higher mean BMI (35.8 vs. 34.3, p = 0.003) and AHI (27 vs. 25, p = 0.0143). Oxygen desaturation was observed in 34% of deceased patients compared to 28% in the alive cohort (p < 0.0001). Ablation and cardioversion did not significantly impact mortality. The all-cause mortality rate was 12.7%. Conclusion Analysis revealed that prolonged hypoxia increased mortality risk in AF and OSA patients. Although higher AHI was noted in deceased patients, its clinical significance was limited. We also found that ablation and cardioversion did not improve mortality. Effective OSA treatment in AF patients can potentially improve outcomes. Further studies are necessary to determine if reducing oxygen burden reduces mortality in this population. Support (if any) A study published in the American Journal of Respiratory and Critical Care Medicine found that CPAP therapy reduced the incidence of AF-related mortality. The data from over 10,000 patients, demonstrated that those receiving effective sleep apnea interventions, such as CPAP, exhibited a 35% reduction in all-cause mortality compared to untreated counterparts. This research highlights the potential life-saving benefits of addressing sleep-disordered breathing in the management of AF.

Urgent and emergency care services remain under huge pressures with concerns regarding overcrowding, delays in patient care and exhausted staff with increasing unsustainable workforce pressures. Short term mitigation measures are needed alongside the imperative need to ensure long-term innovative and sustainable measures to ensure that the NHS can deliver high quality acute and emergency care. Virtual wards are an innovation that have the potential to support acute care and reduce pressures in the long term. Increasing availability and utility of telemedicine services mean it may be possible to observe and manage patients in their own home supported with technological innovations that will enable monitoring of a person’s vital signs and blood tests that currently are performed in a hospital setting. The implementation of virtual wards requires careful clinical and operational implementation. It is important that those with clinical expertise in the field, such as those working in Acute Medicine and Primary Care, are at the forefront of this work to ensure it delivers its potential impacts. In this edition of Acute Medicine, Gallier et al. assess both patient outcomes and the workforce requirements of a COVID virtual ward.1 Their retrospective study reported that approximately 25% of patients with COVID19 within the first 24 hours of hospital presentation met NHS England criteria for consideration of virtual ward management. Virtual wards, in the short-term, will be resource intense. The workforce pressures of virtual wards are likely to be under-estimated and this could lead to a risk to both the success of the implementation and more importantly patient safety. To deliver a COVID virtual ward telephone line require staffing for at least 12 hours a day, seven days a week, with locally arranged provision of out of hours cover. These staff should be supervised by an experienced, clinically registered professional who is also responsible for making the proactive daily calls – a “virtual ward round”. It is also important to identify patients that will benefit from being managed on virtual wards and not simply creating work that does not ultimately reduce the pressure on the system. Gallier et al. compared COVID patients discharged within 24 hours with and without the support of a virtual ward. They found no difference in key outcomes such as hospital readmission, subsequent ITU admissions and death for those managed on a virtual ward.1 Virtual wards are a potentially important innovation to support urgent and emergency care. It is essential they are appropriately planned, resourced and importantly staffed. They should not be seen as a short-term mitigation measure and incorrect implementation with inadequate evaluation could significantly impact clinician and patient confidence. It is essential this does not happen.

After the first wave of the coronavirus epidemic, temporary stability in the disease spread dynamics was observed in many regions. This behavior can potentially be attributed to the measures implemented to contain the spread, particularly close contact testing. We propose a deterministic mathematical model that simulates the dynamic spread of the disease while considering the actions of the public health system. The developed model achieves a non-forced balance in daily confirmed cases, reproducing the observed epidemic behavior during the COVID-19 outbreak in South Korea and New York City. Our finding indicated that, although the quasi-steady state behavior can only be attained within a certain range of model parameters, an increase in the health system’s interventions does not eliminate the epidemic. We conclude that the observed stationary state of daily COVID-19 cases does not result from setting the basic reproductive number to one. Instead, it emerges as a natural consequence of the policies implemented by authorities to mitigate its spread.

Objective: This study assessed the impact of erector spinae plane block (ESPB) and intravenous (IV) dexmedetomidine in reduction of perioperative opioid consumption following bariatric surgery and their impact on post-operative recovery, analgesia, and pulmonary functions. Design: A randomized controlled trial. Setting: Tanta University Hospitals, Tanta, Gharboa, Egypt. Patients: Forty obese patients with obstructive sleep apnea syndrome (OSAS), aged 20-55 years, and eligible for bariatric surgery were included. Interventions: Patients randomized into group I (received general anesthesia [GA] with opioid, sham ESPB, and IV normal saline) or group II (received GA [without opioid], ESPB [at T7 level] using 20 mL bupivacaine 0.25 percent and bolus IV dexmedetomidine 1 μg/kg and then 0.25 μg/kg/h). Main outcome measures: Fentanyl consumption (primary outcome), sevoflurane consumption, recovery time, Visual Analog Scale (VAS), and pulmonary functions (secondary outcomes) were recorded. Results: Perioperative fentanyl (intraoperative, post-operative, and total) consumption and sevoflurane consumption were substantially lower in group II compared to group I (p = 0.010, <0.001, <0.001, and <0.001, respectively). Moreover, recovery time was shorter in group II (p < 0.001). At 2, 4, 8, and 24 hours after surgery, group I patients had VAS values considerably higher. Relative to preoperative values, pulmonary function did not significantly alter after surgery. Oxygen desaturation was significantly lower in group II (p = 0.001). Conclusions: The ESPB with IV dexmedetomidine is advantageous for OSAS patients having bariatric surgery as it provides anesthesia and opioid-sparing effect with short recovery, adequate analgesia, and nonsignificant complications. Yet, it had no effect on post-operative pulmonary function.

Abstract Introduction Obstructive sleep apnea (OSA) is a breathing disorder characterized by episodes of nocturnal hypoxia and chronic systemic inflammation, affecting more than 50% of obese youths. Both obesity and OSA independently have a negative impact on brain structure and function, but their combined effect on the developing brain is unknown. The purpose of this study was to assess MRI measurements of cortical thickness (CT) in obese youths with various degrees of OSA severity. We hypothesized that CT is abnormal in obese adolescents with OSA. Methods 55 obese subjects (26 females, 29 males, mean 14.3 ± 2.4 years) were included in the analysis. All subjects were assessed with polysomnography (PSG) to evaluate presence and severity of OSA. T1-weighted MPRAGE images were acquired using a 3T MRI scanner following PSG. CT was extracted using the CIVET 2.1.1 pipeline, and statistical analysis was performed on SurfStat to examine global and regional CT in relation to age using a general linear model. Results Based on PSG outcome, subjects were divided into 3 groups, no OSA (OAHI &lt; 1.5 events/hr., n = 15), mild OSA (OAHI &lt; 5, n = 14), and moderate/severe OSA (OAHI &#8805; 5, n = 26). Cortical thickness analysis revealed a negative-trending correlation between global CT and age in no OSA (T = -0.49, P &gt; 0.6), as seen in typical development. This correlation weakened in the presence of mild OSA (T = -0.20, P &gt; 0.8) and became significantly positive in moderate/severe OSA (T = 3.87, P = 0.001), affecting several cortical areas. Conclusion These results indicate that brain development in obese adolescents with moderate/severe OSA significantly deviates from the typical trajectory of cortical thinning. This thickening could be due to exacerbated inflammation from the combined effect of both diseases, or a neurotrophic effect of leptin. More data is needed to validate these findings. Support None

In 2012, the Global Observatory for Physical Activity (GoPA!) was established to provide information that would enable countries to initiate or improve research capacity, surveillance systems, program development, and policymaking to increase physical activity levels. Findings from the first GoPA! Country Cards showed an unequal distribution of physical activity surveillance, research productivity, and policy development and implementation around the world. Regular global monitoring of these factors, especially in countries with the largest data gaps, was recommended to combat the global pandemic of physical inactivity. After 6 years and using standardized methods, GoPA! is launching the second set of Country Cards based on data up to 2019 from 217 countries. Overall results showed that periodic national surveillance of physical activity was less common in low-income countries, compared with middle- and high-income countries. Large inequities were seen with more than a 50-fold difference in publications between high- and low-income countries and 32% of the countries worldwide had no physical activity policy. GoPA! has a critical role in facilitating evidence-based physical activity promotion building on international guidelines and the World Health Organization Global Action Plan. GoPA! will continue to monitor progress as we battle the global pandemic of physical inactivity.

For patients with oligometastatic esophageal squamous cell carcinoma, the efficacy of local therapy is still controversial because of patient selection and lack of adequate controls in most studies. Here the authors design the ESO-Shanghai 13 trial, a prospective, multicenter, randomized, Phase II trial, to assess the impact of combined local therapy and systemic therapy on progression and survival compared with systemic therapy alone for patients with four or less metastases. A total of 102 patients will be recruited over 3 years from approximately five centers and randomized in a 1:1 ratio to receive either systemic therapy alone or systemic therapy and local therapy, such as radiation, surgery and thermal ablation. The primary endpoint is progression-free survival. The secondary endpoints are overall survival, local control, toxicity and quality of life.
