Yasuhiko Sugimura’s review of my book La Religion dans la philosophie (Hermann, 2021) for the present issue of Études Ricœuriennes / Ricoeur Studies affords me an opportunity to return to my intention in this work. I verify the heuristic hypothesis that the complex relationship that Ricoeur’s philosophy established with religion can be explained by a process of secularization [laïcisation] of Ricoeur’s religious convictions. In dialogue with the aforementioned review, I propose to reconsider certain aspects of my criticism of Ricoeur on the question of truth.

Physiol. 83(6): 1799–1813, 1997.—The effects of pericardial constraint on cardiac chamber interactions were evaluated by mathematical model analyses based on a novel concept of coupled vs. uncoupled pericardial constraint. We hypothesized that the nature of pericardial constraint can be classified as a “coupled” constraint exerted by uniform liquid pressure or an “uncoupled” constraint exerted by regional surface pressure. The numerical solution of the model of atrioventricular interaction produced the characteristic waveforms in venous flows and right atrial/ventricular pressures in classical pericardial diseases. Coupled constraint accounted for the patterns in cardiac tamponade; uncoupled constraint accounted for those in constrictive pericarditis. Analytic solution of the model of ventricular interdependence demonstrated that coupled constraint (tamponade) produced greater gains in ventricular interdependence, increasing the occurrence of pulsus paradoxus, whereas uncoupled constraint (constriction) produced a greater effective right ventricular elastance, increasing the likelihood of Kussmaul’s sign. Thus the concept of coupled vs. uncoupled constraint may offer a coherent framework to understand the characteristic steady-state and respiratory-induced hemodynamic events in multiple forms of pericardial diseases.

The title of Yasuhiko Tomida’s paper ‘“Separation” of Ideas Reconsidered: A Response to Jonathan Walmsley’ portends sustained criticism of my analyses of Locke’s views on abstraction. The impression is misleading. My conclusion, that Locke’s theory of abstraction was one of mental separation of parts of images, is cited several times, but mainly as an opportunity for Professor Tomida to commence his own interpretation of Locke’s views. There is relatively little direct consideration of the arguments advanced for my conclusions. This observation is not intended as a criticism, but rather as a means to understand Tomida’s enterprise. He has advanced an analysis of Locke’s theory of ideas in a series of publications. His recent ‘Sensation and Conceptual Grasp in Locke’contains the kernel of this interpretation. After considering the origin and nature of important ‘simple’ ideas, viz. solidity, extension, power, existence, and unity, the paper ends with a brief discussion of the abstract idea of a triangle. His subsequent response to my work is a continuation of these arguments. Rather than immediately countering Tomida’s response to my interpretation, I will first place his views on abstraction in the context of his larger project, and only then consider his views on abstract ideas, and their relation to my position.

BackgroundMalignancies under the treatment of biologic agents (Bio) or JAK inhibitors (JAKi) are attracting public attention[1] [2]. It is important to administer Bio or JAKi safely to the patients, it seems to be absolutely necessary of the screening of cancer. Intensive pre-screening examinations for malignancies before the treatment of Bio or JAKi has been performed in our department since 2016.ObjectivesTo elucidate the utility of the intensive pre-screening examination and the prevalence of malignancies prior to the therapy of Bio or JAKi to the patients of connective tissue diseases (CTD).MethodsThree hundred thirty-six CTD patients (RA:254, SLE:27, SpA:25, Vasculitis:13, Overlap syndrome:6, others:11) before the administration of Bio or JAKi were enrolled from April 2016 to September 2021. Upper gastric intestinal endoscope, fecal occult blood test (twice), thyroid and abdominal ultrasound, chest and abdominal non-contrast enhanced CT, serum prostate-specific antigen (only for male) and screening for the cancer of the cervix and mammography (only for female) were performed to all patients above as an intensive pre-screening examination. Herein, we retrospectively analyzed the prevalence of both benign and malignant tumors including species before the therapy of Bio or JAKi, and the patient-treatment outcomes with cancers.ResultsThe patient characteristics were as follows: Median age: 68 yr.(n=336). (RA:69 yr., SLE:48yr., SpA:63yr., Vasculitis:73yr., Overlap syndrome:70yr., others:57yr.). The prevalence of benign tumors and malignant tumors were 24.7% (n=83, median age:68yr.) and 5.1% (n=17, median age:74yr.<Max:89yr. Min:50yr.>), respectively. The details of the malignancies were prostate cancer (n=6, 35.3%), uterus or ovarian cancer (n=3, 17.6%), gastric cancer (n=3, 17.6%), colon cancer (n=2, 11.8%), breast cancer (n=1,5.9%), lung cancer (n=1, 5.9%) and thyroid cancer (n=1, 5.9%). Most of the age complicated with cancers was 80yr (median) except for those of uterus or ovarian cancer:50yr(median). Eleven patients (64.7%) among cancer bearing patients (n=17) successfully administered Bio or JAKi after radical operation such as endoscopic or surgical resection and all of the 11 patients achieved clinical remission.ConclusionSubclinical malignancies were detected by means of intensive pre-screening examinations. The most common cancer was prostate cancer, subsequently uterus or ovarian cancer and gastric cancer. For the widespread use of fuel cell vehicles, it is necessary to improve the performance and durability of the cathode catalyst of polymer electrolyte fuel cells (PEFCs). In particular, high-temperature operation of PEFCs is attracting attention for application to commercial vehicles for heavy-duty application. Operation at higher temperatures than the conventional one (60-80°C) is expected to enhance oxygen reduction kinetics and mass transfer and reduce specific adsorption of ionomers. Despite such advantages, due to the thermal effect, the oxide formation on Pt surface is expected to be enhanced and decrease the activity of the catalyst. However, the quantitative relationship between catalyst behavior and the electronic structure of Pt under high temperature operation has not yet been elucidated. Here, we developed an MEA cell for high temperature and high pressure conditions and analyzed the oxidation state and local structure of Pt on Pt/C catalysts at different temperatures by operando XAS measurements. MEAs were prepared with Pd/C as anode catalyst, Pt/C as cathode catalyst. XAS measurements of the Pt LIII absorption edge were made at 80°C, 105°C, and 120°C with 100% H2 flowing to the anode and Air flowing to the cathode, and spectra were collected by the transmission mode. The area values of the XANES spectra were calculated to compare the relationship of Pt oxide formation with operating temperature and the local structural model of Pt oxide at high temperature by EXAFS. Comparison of operando XANES results of cathode catalyst Pt/C under conventional operating temperature (80°C), 105°C, and 120°C showed a marked difference, especially at 80°C and 120°C. In addition, the XANES area values increased as the operating temperature was increased. This is thought to be due to the fact that Pt oxide formation is accelerated at higher temperatures. Next, comparing the EXAFS analysis results at each temperature, the Pt-O bond peak is stronger at 120°C, indicating that Pt oxidation is more advanced than at 80°C. The 120°C results can be explained by a mixed model of β-PtO2 and Pt metal. Pt-O bond coordination number analysis shows that β-PtO2, in which oxygen is latent inside Pt, is formed more frequently at high temperatures. From this result, it was found that the structure of Pt oxide is different in high-temperature systems, and that catalyst design must take this into account. Fig ure 1. Abstract Background: HER3 (ErbB-3) is a member of the epidermal growth factor receptor family of receptor tyrosine kinases, and its overexpression is associated with poorer prognosis in several cancer types. It is unclear whether HER3 expression status changes in tumor tissue at relapse. The purpose of this study is to evaluate changes in HER3 expression between initial diagnosis and recurrence in gynecologic cancers. Methods: This retrospective study included gynecologic cancer patients with matched paired tissues at the time of initial diagnosis and at the time of recurrence between 1999 and 2019 at National Cancer Center Hospital, Japan. Immunohistochemical (IHC) staining for HER3 was performed using formalin-fixed paraffin-embedded specimens. The primary antibody was HER3/ErbB3 (D22C5) XP Rabbit mAb (Cell Signaling Technology). HER3-positive was defined as an IHC score of 2+ or 3+, and HER3-negative was an IHC score of 0 or 1+, scored according to HER2 testing guidelines for gastroesophageal cancer. The H-score (range, 0-300) was calculated using the following formula: 3X+2Y+Z, where X, Y, and Z are the percentage of tumor cells showing strong, moderate, and weak staining intensity. The difference in HER3 expression between initial diagnosis and recurrence was evaluated using the χ2 test. Results: Eighty-six patients with gynecologic cancers were included (40 ovarian; 32 endometrial; 14 cervical). In ovarian cancer, 67.5% and 80.0% of the patients were HER3-positive at initial diagnosis and recurrence, respectively. There was a statistically significant increase in H-Score at recurrence (p=0.004). The HER3-positive rate in patients with endometrial cancer increased from 46.9% at initial diagnosis to 68.8% at recurrence, and H-Score tended to increase at recurrence (p=0.08). Twelve of the 14 patients (85.7%) with cervical cancer were HER3-positive, both at initial diagnosis and at recurrence, and H-Score tended to increase at recurrence (p=0.19). The discordance rate of HER3 expression determination in samples at initial diagnosis and recurrence was 27.5%, 46.9%, and 14.3% for ovarian, endometrial, and cervical cancers, respectively. Conclusion: Our findings suggest that HER3 expression may increase at recurrence in patients with gynecologic cancers. HER3-targeted therapy may represent a promising option to overcome treatment failure and improve patient outcomes. Abstract Introduction: Currently, no widely accepted molecular classification of colorectal cancer (CRC) has been developed except for the distinction between hypermutated and nonhypermutated CRC. No reliable prognostication model has been available to optimize the effect of adjuvant chemotherapy either. To develop novel molecular classification and prognostication systems, we conducted large-scale studies of gene mutations and transcriptomes involving 3056 well-annotated CRC patients. Methods: All patients were analyzed for mutations in 169 known/putative driver genes in CRC using targeted-capture sequencing, which also include1,630 SNPs sequenced to assess genome-wide copy numbers. RNA sequencing was also performed for gene expression profiling and comprehensive detection of gene fusions. Results: Based on the strong mutually exclusive and co-occuring mutation patterns of mutations in Wnt pathway, non-hypermutated CRC patients (90%) were further classified into APC-mutated (81%) and unmutated (9%) case. The former was further divided into TP53-mutated (70%) and unmutated (11%), while the latter was into CTNNB1-mutated (2.7%), RNF43-mutated (1.7%), RSPO-fusion-positive (2%), and other cases (2.7%). Among these, TP53-unmutated CRC was characterized by significantly higher and lower numbers of mutations and copy number alterations, respectively, while accounting for 70% of all CRCs, the APC/TP53-double mutated ‘canonical CRC’ cases were classified into 13 unique subtypes having unique mutational profiles using non-negative matrix factorization consensus clustering. Next we modeled overall (OS) and relapse free (RFS) survival using Cox proportional hazard modeling with 1000 times cross validations using lasso. The final prediction model included gene mutations and copy number abnormalities, together with age, sex, stage, and tumor location, based on which we classified patients into groups having distinct OS and RFS in each stage. Importantly, adjuvant chemotherapy (ACT) was shown to have significantly different impacts on RFS between different risk groups within Stage2 and 3 CRC. Conclusions: On the basis of comprehensive mutation analysis in a large cohort, we developed novel molecular classification of CRC. We further identified distinct risk groups showing different RFS and OS, which showed relationship to the efficacy of ACT, which could be used for risk stratification in Stage2 and Stage3 CRC for optimized use of ACT. Internationalization of Japanese Higher Education is not a new issue, but has begin since 1983. In 1983, the Japanese Government lead by Prime Minister Nakasone Yasuhiro announced its plan to accept 100.000 international students by 2000. The target number of international student was reached in 2003. In 2008, Japanese Government announced new plan to accept 300.000 international students by 2020. This plan called Global 30 or Ryuugakusei 30-mannin keikaku. To achieve this target, thirteen universities has been chosen to be pilot for the project. One of main aim of Global 30 is to develop system where student can learn in international method, so some university offer English program by invite English speaker lecturer, and English training for university’s staff. This paper reviews the internationalization of Higher Education in Japan, focus on Global 30 project.
