Résumé :

Objective. To evaluate the clinical efficacy of sertaconazole in the treatment of acute vulvo-vaginal candidiasis (VVC) in pregnant women with caesarean section. Subject and methods. The study included 136 pregnant women. In the treatment, 1 candle of sertaconazole 300 mg once was used. Efficacy was evaluated after 7 and 28 days. Results. At 74.0% of pregnant women identified VVK. After treatment with sertaconazole, positive dynamics was observed in 91.9% of patients, in whom clinical manifestations of the disease disappeared by the third day. Culture testing revealed a normalization of the vaginal biotope at 97.1% of pregnant women. Our experience has shown the effectiveness of the drug, the absence of adverse events and good compliance due to the ease of use and rapid achievement of the therapeutic effect in pregnant women throughout pregnancy.

Résumé :

Infections of the vulva and vagina remain one of the most pressing problems of modern gynecology due to the high prevalence, frequent relapses, and negative effects on the functions of the reproductive system, which increases the risks of gynecological diseases and obstetric complications. The most common forms of vulvovaginal infection are bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), aerobic vaginitis (AB) and trichomoniasis. Features of the biocenosis and immune status in infections caused by conditionally pathogenic microorganisms predispose to relapses of BV, AB, and VVK, against the background of which a mixed infection develops. Its occurrence is also the result of repeated courses of treatment for recurrent monoinfection. Therapy of BV consists in the use of metronidazole and clindamycin, the benefits of clindamycin are in a wider spectrum of action, covering difficult to identify anaerobic microorganisms, as well as aerobes. The treatment of mixed infection and the prevention of the development of IHC in patients with BV involves the use of azole antimycotics, which remain the first line treatment option for the infection caused by Candida albicans. Thus, the combination therapy with clindamycin and azole antimycotics can be considered the optimal treatment option for mixed and recurrent vulvovaginal infections.

Вместе с тем спецификой такого фундаментального общественного блага, как мощь державы, является его превентивный характер, что не дает возможности всегда справедливо оценивать приносимые этим благом выгоды. Как следствие, для таких благ становятся вероятными ситуации несовпадения их подлинной, справедливой цены с ценой общественной, исчисляемой готовностью населения платить. В настоящей работе предлагается и тестируется на эмпирических данных основанный на экспериментальном решении классической бюджетной дилеммы подход к измерению общественной цены блага «дополнительная мощь державы» в сравнении с двумя альтернативами – экономической (в альтернативном росте материального благосостояния граждан) и гуманитарной (в альтернативном усилении институтов охраны жизни и здоровья граждан). Показано существование биполярной мировоззренческой структуры российского общества. Даются количественные оценки общественно допустимых альтернативных издержек дополнительного финансирования общественного блага «мощь державы» в двух ее коннотациях: (1) как инструмента защиты суверенитета и обеспечения независимости в международных отношениях и (2) как инструмента восстановления геополитического равновесия. Выявляются социально-групповые факторы, определяющие общественный выбор в рассматриваемой системе альтернатив.

In this paper, we propose a novel method for cross-view action recognition via multiple continuous virtual paths which connect the source view and the target view. Each point on one virtual path is a virtual view which is obtained by a linear transformation of an action descriptor. All the virtual views are concatenated into an infinite-dimensional feature to characterize continuous changes from the source to the target view. To utilize these infinite-dimensional features directly, we propose a virtual view kernel (VVK) to compute the similarity between two infinite-dimensional features, which can be readily used to construct any kernelized classifiers. In addition, a constraint term is introduced to fully utilize the information contained in the unlabeled samples which are easier to obtain from the target view. The rationality behind the constraint is that any action video belongs to only one class. To further explore complementary visual information, we utilize multiple continuous virtual paths. The original source and target views are projected to different auxiliary source and target views using the random projection technique. Then we fuse all the VVKs generated from all pairs of auxiliary views. Our method is verified on the IXMAS and MuHAVi datasets, and the experimental results demonstrate that our method achieves better performance than the state-of-the-art methods.

Properties of epoxy glues of cold curing VK-9 and VKV-9, the components received with use which are let out by modern producers, depending on the filler used in their structure are considered. Tests of glued joints on VK-9 glue in wider interval of temperatures are carried out. Properties of VK-36 glue and its updating’s on the basis of components let out now are shown. Properties of glue binding brands VSK-14-1, VSK-14-2, VSK-14-4, VSK-14-4m and VSK-14-4k received, under production conditions by VIAM Federal State Unitary Enterprise are given. It is shown that on the properties they completely meet the requirements of existing normative documentation.

Abstract This paper examines 2 Kgs 17:7-23 which is often understood as a fragmented text, compounding various theological reactions to the fall of Samaria. Applying a text-linguistics approach, I find vv7-20 to be a unit that makes sense in its present form, complementing a second unit beginning in v21. My reading understands the relationship between vv6 and 7 differently to what is usually understood, and to the standard English translations. I argue based on the Hebrew syntax that vv7-20 form a narrative unit that summarises the history of both northern and southern kingdoms together, as a history of idolatrous activity. Thus, the concern of the narrator is to explore the theological reason for the series of Assyrian invasions of the last quarter of the 8th Century bc, and the different fates of Israel and Judah.

Abstract INTRODUCTION Carotid endarterectomy (CEA) has been shown to be beneficial in patients with high-grade symptomatic carotid artery stenosis. Subintimal and intraplaque hemorrhages are frequently seen during CEA in the absence of any visible breach in the intima, as these changes are derived from the vasa vasorum system rather than from blood in the vessel lumen. Imaging modalities to characterize unstable, vulnerable plaques are strongly needed for better risk stratification in these patients. The aim of this study was to investigate the correlation between the activities of the carotid vasa vasorum and carotid plaque vulnerability using indocyanine green-video angiography (ICG-VA) during CEA, focusing on how the carotid artery vasa vasorum is depicted. METHODS Sixty-nine patients (mean age, 68.5 ± 2.5 years; mean degree of stenosis, 78.9 ± 3.8) who underwent CEA were enrolled prospectively from September 2013 to December 2014. ICG was injected intravenously as a bolus before and after resecting the atheroma during CEA. We also performed immunohistochemistry using CD68 (macrophage), CD117 (mast cell), CD4 (T-cell), and CD8 (T-cell) antibodies for resected plaque specimens. RESULTS >Active vasa vasorum density was observed in all patients on ICG-VA (n = 69). The vasa vasorum externa (VVE) and interna (VVI) were seen in 11 (16%) and 57 patients (82.6%), respectively. The types of VVE were strongly associated with preoperative angiographic instability (90.0%, P = 0.005) and carotid plaque vulnerability (100%, P = 0.007) macroscopically. In contrast, the types of VVI were less associated with angiographic instability (36.1%) and plaque vulnerability (49.1%, P = 0.003). CD86- and CD117-stained macrophages and mast cells were observed more frequently in unstable plaque, compared to those in stable plaque (P < 0.0001, P = 0.002, respectively). CONCLUSION The early appearance of VVE on ICG-VA was strongly associated with unstable carotid plaque and many microvessel channels that provided nutrients to the developing and expanding intima and potentially created an unstable hemorrhagic environment prone to rupture. Macrophages and mast cells were involved in the formation of microvessels in the atherogenic plaque and accelerated plaque progression into an unstable plaque phenotype.

Kemampuan komunikasi matematis merupakan salah satu indikator untuk mengetahui hail belajar matematika siswa. Jika kemampuan komunikasi matematis baik, maka besar kemungkinan hasil belajar matematika pun baik. Namun berdasarkan hasil observasi di SMA Negeri 1 Mawasangka tengah, bahwa kemampuan komunikasi siswanya belum optimal. Salah satu faktor penyebabnya adalah gaya belajar siswa. Beberapa penelitian tentang kemampuan komunikasi matematis siswa belum ada yang meninjau dari sudut pandang gaya belajar. Oleh karena itu, penelitian ini bertujuan mendeskripsikan profil kemampuan komunikasi matematis siswa berdasarkan gaya belajar (visual, auditroy, read/write, kinestetic). Indikator gaya belajar dalam penelitian ini adalah indikator yang dikembangkan oleh Fleaming yang meliputi gaya belajar unimodal (V,A,R,K), bimodal (VK, RK, VA, VR), trimodal (ARK, VRK, VAK, VAK), dan quadmodal (VARK). Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif, dengan subjek penelitian sebanyak 52 orang siswa kelas XI SMA Negeri 1 Mawasangka Tengah. Data hasil penelitian diperoleh dengan memberikan soal tes kemampuan komunikasi matematis dan kuisioner gaya belajar, selanjutnya dianalisis dengan teknik reduksi data, penyajian data, dan penarikan kesimpulan. Hasil penelitian menunjukkan bahwa kemampuan komunikasi matematis siswa ditinjau dari gaya belajar, termasuk dalam kategori sedang, baik pada unimodal, bimodal, trimodal, maupun quadmodal.Kata kunci: komunikasi matematis, gaya belajar, unimodal, bimodal, trimodal, quadmodal.

D. I Timbang Deli is 520 Ha of flow area and 5000 meters of secondary channel is located in Deli Serdang Regency, North Sumatra Province. The turn of the irrigation network in overgrown vegetation on the cliffs and bottom of the channel, is expected to reduce the flow rate. The purpose of the study was to see the distribution of velocity, resistance and energy loss in the secondary channel due to vegetation. The method used is conducting surveys in the field by measuring water depth, vegetation volume and flow velocity. Trapezoidal channel with 36 m length, 1.45 lower width, 3.15 m upper width and 33,510 turn angle. The velocity measured in the middle and downstream regions across the channel is divided from points X1 to X5 with Q1 = 0.62m3/ sec and Q2 = 0.83 m3/sec. The results obtained in the speed distribution in Q1 Vmax mean X1 = 0.296 m/s and X5 = 0.199 m/s, the speed decreases due to the turn of 48.82%. On VV3 Vmaks the average is X4 = 0.216 m/sec and (X1 and X2) = 0,000 m/sec, the volocity decreases due to the presence of vegetation between X1, X2 and X5 at Q1 = 100%. In Q2 the average VV0 Vmax condition of the flow X1 = 0.477 m/s and X5 = 0.323 m/s between X1 and X5 the volocity decreases due to turn 28.45%. VV3 Vmax conditions on average X5 = 0.312 m / s and X1 = 0.000 m / s, between X1 and X5 the speed decreases due to the presence of 100% vegetation. In connection with the above results, with this research the local community and local government can carry out cleaning on the channel on a scale basis.

Abstract. The primary process occurring at a collisionless shock is the redistribution of the bulk upstream energy into other degrees of freedom. One part of this process results in the acceleration of electrons at the shock front. Accelerated electrons are observed at the terrestrial and other planetary shocks, comets, and their effects are observed in astrophysical phenomena such as supernova remnants and jets in the form of X-ray bremsstrahlung radiation. One of the physical models for electron acceleration at supercritical shocks is based on low-hybrid turbulence due to the presence of reflected ions in the foot region. Since lower hybrid waves propagate almost perpendicular to the magnetic field they can be simultaneously in resonance with both the unmagnetised ions (ω=Vik⊥) and magnetised electrons (ω=Vek||). In this paper, Cluster observations of the electric field are used to study the occurrence of lower hybrid waves in the front of the terrestrial bow shock. It is shown that the lower hybrid waves exist as isolated wave packets. However, the very low level of the observed lower hybrid turbulence is too small to impart significant energisation to the electron population.

Vaccinia virus–related kinase (VRK) is an evolutionarily conserved nuclear protein kinase. VRK-1, the single Caenorhabditis elegans VRK ortholog, functions in cell division and germline proliferation. However, the role of VRK-1 in postmitotic cells and adult life span remains unknown. Here, we show that VRK-1 increases organismal longevity by activating the cellular energy sensor, AMP-activated protein kinase (AMPK), via direct phosphorylation. We found that overexpression of vrk-1 in the soma of adult C. elegans increased life span and, conversely, inhibition of vrk-1 decreased life span. In addition, vrk-1 was required for longevity conferred by mutations that inhibit C. elegans mitochondrial respiration, which requires AMPK. VRK-1 directly phosphorylated and up-regulated AMPK in both C. elegans and cultured human cells. Thus, our data show that the somatic nuclear kinase, VRK-1, promotes longevity through AMPK activation, and this function appears to be conserved between C. elegans and humans.

Vulvar and vaginal atrophy (VVA) is a chronic and progressive medical condition common in postmenopausal women. Symptoms of VVA such as dyspareunia, vaginal dryness, irritation, and itching can negatively impact sexual function and quality of life. The REVIVE (REal Women's Views of Treatment Options for Menopausal Vaginal ChangEs) survey assessed knowledge about VVA and recorded attitudes about interactions with healthcare providers (HCPs) and available treatment options for VVA. The REVIVE survey identified unmet needs of women with VVA symptoms such as poor understanding of the condition, poor communication with HCPs despite the presence of vaginal symptoms, and concerns about the safety, convenience, and efficacy of available VVA treatments. HCPs can address these unmet needs by proactively identifying patients with VVA and educating them about the condition as well as discussing treatment preferences and available therapies for VVA.

BackgroundA recent Dutch study in general practice showed a clear relationship between the diagnosis of vulvovaginal candidiasis (VVC) and symptoms suggestive of provoked vulvodynia (PVD). PVD accounts for the largest group of vulvar pains, but is often not recognised by GPs.AimTo investigate whether diagnostic uncertainty about VVC in general practice could also point to the diagnosis of PVD, and whether and how this diagnostic uncertainty affects management.Design & settingAn observational study in 2014 in Dutch general practices of the NIVEL Primary Care Database.MethodWomen with an uncertain diagnosis of VVC were distinguished from those with certain VVC based on the occurrence of recurrent episodes and persisting complaints, despite treatment. Factors known to be associated with PVD were hypothesised to be more prevalent in women with uncertain VVC. Data on symptom management by GPs were collected.ResultsIn total 7066 women with VVC or uncertain VVC were included. Uncertain VVC was found to account for 28% of these patients. Compared to VVC, the group uncertain VVC included significantly more women with female genital symptoms, tiredness, irritable bowel syndrome (all P<0.001), feeling anxious, reduced sexual desire, depressive disorder, relationship problems, and micturition symptoms (all P<0.05). Compared to VVC, the group uncertain VVC included significantly higher mean numbers of telephone consultations (P<0.001), more referrals to gynaecology (P = 0.009), and higher mean numbers of prescriptions per patient (P<0.001).ConclusionThis study's findings indicate that uncertain VVC could be a marker of PVD. GPs might reconsider their diagnostics and management when women present recurrent and persistent vulvovaginal complaints, especially if accompanied by dyspareunia, functional syndromes, micturition symptoms, and psychological conditions.

Vlk is a secreted tyrosine kinase that plays crucial roles during vertebrate embryonic development including skeletal formation. Genetic studies suggest that Vlk can modulate the Hedgehog signaling pathway during skeletal development. Despite its potential roles as an extracellular regulator of signaling pathways, little is known regarding the molecular functions of Vlk. Here we show that Vlk can negatively regulate the Hedgehog signaling pathway. We found that Vlk can induce lysosomal degradation of Smoothened, a crucial transmembrane signal transducer of the Hedgehog pathway, through the interaction with the extracellular domain of Smoothened (Smo-ECD). In addition, we observed that Vlk can attenuate Hedgehog signaling-induced ciliary localization of Smoothened. Furthermore, Vlk-mediated suppression of Hedgehog signaling can be diminished by tyrosine-to-phenylalanine substitutions in Smo-ECD. Taken together, these results suggest that Vlk may function as a signaling regulator in extracellular space to modulate the Hedgehog pathway.

Résumé :

Cardiovascular responses to stimulation of cardiopulmonary receptors were evaluated in standard breed developing swine (1-2 days, 1-2 wk, and 2 mo of age) and sexually mature miniswine anesthetized with 0.25-0.50% halothane in O2. Cryptenamine, a mixture of veratrum viride alkaloids (VVA), was administered as right atrial or left ventricular bolus injections in doses of 5-20 micrograms/kg. In developing swine, mean aortic pressure (AoP) decreased after 10-20 micrograms/kg VVA in 1- to 2-wk olds and after 5-20 micrograms/kg VVA in 2-mo olds. Bradycardia was always elicited. Renal (Ren) and femoral (Fem) vasodilation occurred in the 1- to 2-wk-old and 2-mo-old groups after 5-20 micrograms/kg VVA. Mesenteric (Mes) vasodilation was elicited with 10-20 micrograms/kg VVA in the 1- to 2-wk-old group and with 5-20 micrograms/kg VVA in 2-mo olds. In the 1- to 2-day-old group, VVA did not significantly alter Ren, Fem, and Mes vascular resistance. In mature miniswine, 20 micrograms/kg VVA elicited decreases in AoP and heart rate that were similar in magnitude to responses obtained with only 5 micrograms/kg VVA in dogs. Cardiovascular responses to VVA were abolished after bilateral vagotomy but were not altered after denervation of the carotid sinuses. The results indicate postnatal maturation of the Bezold-Jarisch reflex in swine.

Between 70-75% of sexually active women experience at least one episode of VVC in their lifetime, and 8-10% of adult women have recurrent VVC. The treatment of VVC remains as one of the most pressing challenges of gynecology. The share of recurrent disease event rates remains high, despite the modern medicines used to treat various forms of VVC.

ABSTRACT The oncogenic herpesvirus, Kaposi's sarcoma-associated herpesvirus, also identified as human herpesvirus 8, contains genes producing proteins that control transcription and influence cell signaling. Open reading frame 36 (ORF36) of this virus encodes a serine/threonine protein kinase, which is designated the viral protein kinase (vPK). Our recent efforts to elucidate the role of vPK in the viral life cycle have focused on identifying viral protein substrates and determining the effects of vPK-mediated phosphorylation on specific steps in viral replication. The vPK gene was transcribed into 4.2-kb and 3.6-kb mRNAs during the early and late phases of viral reactivation. vPK is colocalized with viral DNA replication/transcription compartments as marked by a polymerase processivity factor, and K-bZIP, a protein known to bind the viral DNA replication origin (Ori-Lyt) and to regulate viral transcription. The vPK physically associated with and strongly phosphorylated K-bZIP at threonine 111, a site also recognized by the cyclin-dependent kinase Cdk2. Both K-bZIP and vPK were corecruited to viral promoters targeted by K-bZIP as well as to the Ori-Lyt region. Phosphorylation of K-bZIP by vPK had a negative impact on K-bZIP transcription repression activity. The extent of posttranslational modification of K-bZIP by sumoylation, a process that influences its repression function, was decreased by vPK phosphorylation at threonine 111. Our data thus identify a new role of vPK as a modulator of viral transcription.

We evaluated Gram stain as a rapid diagnostic method for vulvovaginal candidiasis [VVC]. Vaginal swabs were taken from 100 pregnant women and subjected to Gram stain and culture; 39% were diagnosed as having VVC [29% had symptomatic VVC and 10% had asymptomatic VVC]. Candida albicans was isolated from 94.9% of all cultures while other Candida species were isolated from 5.1% of cases. Multigravidae were significantly more affected than primigravidae. Gram stain is a valuable method in rapid accurate diagnosis of symptomatic VVC and even superior to culture as it demonstrates the invasive forms of the yeast. However, its low sensitivity could restrict its use in routine practice. A combination of culture and Gram stain is the ideal approach for the diagnosis of VVC.

Introduction: Vesicovaginal fistula (VVF) is one of the most distressing maternal morbidities. It is a physically and socially disabling obstetric complication that affects many women annually. It has a devastating social, economic and psychological effect on the health and well-being of the affected women. The aim of the study was to find out awareness regarding vesicovaginal fistula among women. Material and Method: Descriptive study design was used for the study to find out awareness regarding vesicovaginal fistula. Hundred women were selected as the study sample by non purposive and snow ball sampling technique. Pretested semi structured interview schedule was used for data collection and analyzed by using descriptive and inferential statistics. Results: The findings of the study revealed that 81% and 100% of women knew that delivery by non skilled and wetting under garments is risk factor and symptom of VVF. 82% and 73% of women knew that VVF is preventable and curable respectively. 58% of women had knowledge that women can become pregnant in case of VVF and 47 % knew that VVF can be reoccurred. Out of 73 women, 23% knew that surgery as the treatment of VVF. Forty nine percentage of women had answered regarding family and social misbehave and depression (5%) being social consequences of VVF. Seventy percentage of women answered that cervicitis is complication of VVF. Conclusion: Based on the findings of the study half of the women had low knowledge regarding VVF. So, it is recommended to organize continue awareness program to the women regarding VVF.

Murine L3T4 T cells freshly isolated from Peyer's Patch were fractionated based on differential adherence to Vicia villosa agglutinin (VVA). VVA adherent cells secreted IL-5, but not IL-2, after stimulation with Con A and IL-1. In striking contrast, VVA nonadherent PP L3T4 T cells secreted IL-2, but not IL-5, under the same conditions. In addition, supernatants from VVA adherent, but not from VVA nonadherent cells cultures, enhanced IgA secretion by LPS-stimulated splenic B cells to the same extent as purified IL-5. Thus, IL-5-producing T cells are present in PP in situ and may play an important role in the development of mucosal immunity. Further, differential adherence to VVA can be used to separate T cell populations that preferentially secrete IL-5 or IL-2.

Introduction. Vesicovaginal fistulas (VVF) are the most common and the most frequent type of urogenital fistulas. They may have a different etiology. In most cases, surgery is required for treatment. Objective. The aim of the study was to analyze the results of surgical therapy of VVF in a tertiary level institution over the last decade. Methods. The retrospective study of results was conducted involving a total number of 99 women who were surgically treated for VVF at the Clinic of Urology of the Clinical Center of Serbia in Belgrade in the period from 2001?2010. Results. Over the past ten years VVF occurred after hysterectomy in 74.7% of cases, and almost twice more frequently after hysterectomy for a benign gynecological disease. In the surgical treatment of primary VVF transvesical approach was used in 53.53% (52/99) of cases. Relapses were treated surgically using transperitoneal approach in all cases. No significant difference was found in respect to the applied approach in the surgical treatment of primary fistulas. In even 94% (93/99) of cases the problem of VVF was solved by a single surgical intervention only. In total, in 107 interventions 8 (7.4%) relapses were recorded. Conclusion. The study showed that VVF occurred after hysterectomy in three quarters of cases, more frequently after hysterectomy due to a benign gynecological disease. Early detection with timely and appropriate treatment of gynecological diseases will reduce the number of conditions that may lead to VVF. Surgical treatment of VVF yielded satisfactory results; it was shown that even in 94% of cases the problem of VVF was solved by a single surgical intervention only, however further improvement is certainly necessary.

Penelitian ini bertujuan untuk mengetahui pengaruh penggunaan desain pembelajaran ELPSA dalam model pembelajaran VAK terhadap hasil belajar kimia siswa pada pokok bahasan larutan elektrolit dan non elektrolit kelas X SMA Negeri 3 Samarinda. Teknik pengambilan sampel yang digunakan adalah purposive sampling sehingga diperoleh sampel penelitian yaitu siswa kelas X MIPA 1 dan X MIPA 2 dengan masing-masing kelas berjumlah 36 siswa. Kelas X MIPA 2 merupakan kelas yang menggunakan desain pembelajaran ELPSA dalam model pembelajaran VAK sedangkan kelas X MIPA 1 menggunakan model pembelajaran VAK. Hasil penelitian menunjukkan bahwa rata-rata nilai hasil belajar siswa yang menggunakan desain pembelajaran ELPSA dalam model pembelajaran VAK lebih baik daripada rata-rata hasil belajar siswa yang menggunakan model pembelajaran VAK. Rata-rata nilai hasil belajar siswa yang menggunakan desain pembelajaran ELPSA dalam model VAK adalah 81,06 dan rata-rata hasil belajar siswa yang menggunakan model pembelajaran VAK adalah 78,32. Berdasarkan hasil analisis dengan menggunakan uji t, diperoleh t hitung = 2,68 dan t tabel = 1,99 pada taraf signifikan 5%. Karena t hitung > t tabel, maka H diterima sehingga terdapat pengaruh penggunaan desain pembelajaran ELPSA dalam model pembelajaran VAK terhadap hasil belajar kimia siswa pada pokok bahasan larutan elektrolit dan non elektrolit kelas X SMA Negeri 3 Samarinda.

Introduction. We sought to determine the association between HIV-induced immunosuppression, virologic correlates, and vulvovaginal candidiasis (VVC).Methods. This is a retrospective cohort study, where HIV infected and uninfected women were studied with VVC being the primary outcome. Ninety-seven HIV-infected and 101 HIV-uninfected women were enrolled between June and December 2011. Cases of VVC were confirmed. HIV RNA load was determined by RT-PCR and CD4 counts were obtained from medical records.Results. Fifty-two of 97 (53.6%) HIV-infected and 38/101 (37.6%) HIV-uninfected women were diagnosed with VVC (P=0.032). The relative risk for VVC amongst HIV-infected patients was 1.53 (95% CI: 1.04–2P=0.024). Cases of VVC increased at CD4+ T cell count below 200 cells/mm3(P<0.0001) and plasma HIV RNA load above 10 000 copies/mL (P<0.0001). VVC was associated with increased genital shedding of HIV (P=0.002), and there was a linear correlation between plasma HIV load and genital HIV shedding (r=0.540;R2=0.292;P<0.0001). Women on HAART were 4-fold less likely (P=0.029) to develop VVC.Conclusion. CD4 counts below 200 cells/mm3and plasma HIV loads ≥10 000 copies/mL were significantly associated with VVC.

Objectives: To evaluate outcome of surgical repair of VVF with transabdominal and transvaginalapproaches. Design of study: Prospective study. Setting: Depart of Urology Bahawal Victoria Hospital Bahawalpur.Period: Jan 1999 to Dec 2004. Materials & Methods: All consecutive patients with VVF irrespective of age andaetiology were included in this study. Patients with very large VVF and involvement of bladder neck were excluded.These patients were analysed for results of surgical repair by trans-abdominal and transvaginal approaches. ResultsThis study included 26 patients with age range between 20-48 years (mean age of 34 years). Etiology of VVF wasobserved to be transabdominal hysterectomy in 15 patients, transvaginal hysterectomy in one patient. While obstructedprolonged labour caused VVF in 10 patients. Transabdominal repair was done in 18 patients while 08 patients haveundergone transvaginal repair after investigations and evaluation. We achieved 94.45% success with transabdominalrepair of VVF while 100% success with transvaginal repair. Conclusions:The etiology of this disease is preventable.It is best to wait for at least 03 months after occurrence of VVF, so that inflammatory changes due to previous surgery/birth trauma may have settled completely before attempting at repair. Best resultrs are achieved at first attempt ofrepair. Both approaches of surgical repair of VVF have good results.

Aim. The article aims to evaluate the functional status of the hypothalamic-pituitary-adrenocortical and hypothalamic-pituitary-gonadal systems in females with vascular dystonia from a special medical group. Materials and methods. The study involved first-year female students divided into 3 groups: 1 (control, n = 45) – apparently healthy students; 2 (experimental, n = 27) – students from a special medical group with vascular dystonia (VVD): 1st experimental (n = 12) – students diagnosed with VVD of the hypertensive type, 2nd experimental (n = 15) – students with VVD of the hypotonic type. The functional status of the hypothalamic-pituitaryadrenocortical and hypothalamic-pituitary-gonadal systems was revealed by the concentration of cortisol (K), testosterone (T) and estradiol (E) in saliva and the E / T ratio. Results. The content of cortisol in saliva in females with VVD in both experimental groups was significantly higher (P < 0.001, P < 0.05) than in the control group. Cortisol levels are higher in females with hypertensive VVD than in females with hypotonic VVD (P < 0.05). In females with VVD of the hypotonic type, the level of estradiol is higher by 24% than in the control group, and in females with VVD of the hypertensive type, estradiol is more than twice higher. The concentration of testosterone was significantly higher in both experimental groups compared to the control group (P < 0.001), an increase in T was observed in females with VVD of the hypertensive type. The E/T ratio in students with VVD in both groups showed no significant changes compared to the control group. Significant positive differences were revealed in females with VVD of the hypertensive type compared with VVD of the hypotonic type (P < 0.01). Conclusion. The results of the study showed that changes in hormone secretion can be considered as a chain of closely connected reactions in the regulatory mechanisms of the cardiovascular system for maintaining self-regulation of the body and ensuring adaptation under stress. Therefore, the disruption of any link requires the use of reserve capacities of the body, which affects the performance of the leading adaptation systems.

The article presents the results of a study of the combined effect of plaster coatings and facade paints on the humidity regime in the walls made of aerated concrete brands D400 and D600. The influence of the characteristics of paints and plaster coatings on the temperature of the beginning of condensation is estimated. The position of the planes in which condensate begins to precipitate for all the structures under consideration is determined. It is found that in enclosing structures using cement-slag plaster, the temperature of the beginning of condensation is higher than the temperature of the beginning of condensation in enclosures in which cement-vermiculite plaster is used. When using VAK "Silicate" and VAK "Silicon 2" facade paints, which are distinguished by the highest vapor permeability, this difference is maximal and amounts to 2 C and 2.3 C, respectively. When using VAK "Tsokol" and VAK "Acryl 1" facade paints, this difference is minimal and amounts to 0.2 C and 0.5 C, respectively. It has been established that when using cement-slag plaster and paints VAK "Tsokol", VAK "Acryl 1", VAK "Acryl 2", VAK "Silicone 1", VAK "Silicone 2", moisture condensation occurs at the border of aerated concrete-external finish, which creates the danger of detachment of the plaster composition from aerated concrete. These structures are also characterized by rather high temperatures of the onset of condensation from -5.4 °C and above. When using VAK "Silicate" paint and cement-slag plaster, moisture condensation begins in the thickness of aerated concrete, which is more favorable for the enclosing structure. Recommendations are given on the choice of the type of paint and the type of plaster composition for walls made of aerated concrete grades D400, D600.

ABSTRACTWe report an outbreak of vancomycin-variablevanA+enterococci (VVE) able to escape phenotypic detection by current guidelines and demonstrate the molecular mechanisms forin vivoswitching into vancomycin resistance and horizontal spread of thevanAcluster. Forty-eightvanA+Enterococcus faeciumisolates and oneEnterococcus faecalisisolate were analyzed for clonality with pulsed-field gel electrophoresis (PFGE), and theirvanAgene cluster compositions were assessed by PCR and whole-genome sequencing of six isolates. The susceptible VVE strains were cultivated in brain heart infusion broth containing vancomycin at 8 μg/ml forin vitrodevelopment of resistant VVE. The transcription profiles of susceptible VVE and their resistant revertants were assessed using quantitative reverse transcription-PCR. Plasmid content was analyzed with S1 nuclease PFGE and hybridizations. Conjugative transfer ofvanAwas assessed by filter mating. The only genetic difference between thevanAclusters of susceptible and resistant VVE was an ISL3-family element upstream ofvanHAX, which silencedvanHAXgene transcription in susceptible VVE. Furthermore, the VVE had an insertion of IS1542betweenorf2andvanRthat attenuated the expression ofvanHAX. Growth of susceptible VVE occurred after 24 to 72 h of exposure to vancomycin due to excision of the ISL3-family element. ThevanAgene cluster was located on a transferable broad-host-range plasmid also detected in outbreak isolates with different pulsotypes, including oneE. faecalisisolate. Horizontally transferable silencedvanAable to escape detection and revert into resistance during vancomycin therapy represents a new challenge in the clinic. Genotypic testing of invasive vancomycin-susceptible enterococci byvanA-PCR is advised.

A damage identification method named virtual vibration deflection (VVD) was developed, the principle of which was formulated based on the “weak” modality of the pseudo-excitation (PE) approach previously established. In essence, VVD is based on locating structural damage within a series of “sub-regions” divided from the entire structure under inspection, and each sub-region was considered as a “virtual” structure undergoing independent vibration. The corresponding vibration deflection of the “virtual” structure was then used to derive the damage index of VVD. Besides various advantages inheriting from the PE approach, for example, capability of detecting damage without baseline signals and pre-developed benchmark structures, VVD exhibits improved detection accuracy and particularly enhanced noise immunity compared with the PE approach, attributed to a hybrid use of multi-types of vibration signatures (MTVS). As a proof-of-concept investigation, a beam model was used in a numerical study to examine the philosophy of VVD. And the influences from different factors (i.e., level of measurement noise and measurement density) on the detection accuracy of VVD were discussed based on the numerical model. An experiment was carried out subsequently to identify the locations of multiple defects contained in an aluminum beam-like structure. Identification results constructed by the PE approach, VVD using single-type of vibration signatures, and VVD using MTVS, were presented, respectively, for the purpose of comparison.

Female genital fistula is a serious medical condition in which a perforation develops most commonly between bladder and vagina (VVF). Although the majority of genitourinary fistulas can be closed surgically, the successful closure depends on many factors. In this retrospective study, the records of 30 women with a mean age of 23.8 years were assessed; 13% of the VVF occurred after abdominal hysterectomy, 67% after Caesarean section, and 20% after difficult vaginal delivery. Six (20%) women had previous repair. The median duration of the VVF was 5.9 months. Of the 30 patients of VVF, 24 were high and 6 were low. Twenty seven had single fistula opening and 3 had two fistulous openings. An abdominal approach was used in 24 patients and vaginal approach in 6 patients. At a mean follow up of 24 months, the VVF was cured in 90% patients. In conclusion, surgical correction of the VVF is more successful when done earlier, probably in the first 6 months. Abdominal approach seems to be more successful technique and recurrent VVF being associated with lower success rates than primary repair. High variety also has good result. Faridpur Med. Coll. J. Résumé :

During the reproductive age, 75% of women experience at least one episode of vulvovaginal candidiasis (VVC) and 40–50% of women experience two or more episodes. Recurrent VVC is referred to as at least four episodes of the disease within a 12-month period. The aim of this paper is to review the etiology, pathogenesis, clinical signs, diagnosis, and treatment for VVC. Both domestic and foreign published data are analyzed. It is now recognized that urogenital disorders and systemic comorbidities are the key predisposing factors to recurrent VVC. Other risk factors include the use of broad-spectrum antibiotics, immunosuppressants, and hormone replacement therapy, endocrine disorders, and intestinal Candida persistence. Intravaginal medications are the preferred treatment for VVC. A 6-month maintenance therapy reduces the rate of VVC recurrences by 90%. Further studies on the etiopathogenesis of recurrent VVC will improve treatment efficacy.
