AbstractObjective:To discover the anatomist who first identified the upper oesophageal sphincter.Method:The authors searched dozens of antique anatomy textbooks kept in the old section of the ‘Vincenzo Pinali’ Medical Library of Padua University, looking for descriptions of the upper oesophageal sphincter.Results:The oesophageal sphincter was drawn correctly only in 1601, by Julius Casserius, in the book De vocis auditusque organis historia anatomica… (which translates as ‘An Anatomical History on the Organs of Voice and Hearing …’), and was properly described by Antonio Maria Valsalva in 1704 in the book De aure humana tractatus… (‘Treatise on the Human Ear …’).Conclusion:Anatomists Casserius and Valsalva can be considered the discoverers of the ‘oesophageal sphincter’.

Ranuculus gracilis is endemic to the SE Euro-Mediterranean area and its presence in Italy is controversial. Based on analysis of the relevant literature, field surveys and examination of herbarium specimens, a revised distribution of this species in Italy is presented and its conservation status is assessed. Ranunculus agerii, described by Antonio Bertoloni from Bologna (Emilia-Romagna, Northern Italy), and R. schowii, described by Vincenzo Tineo from Vittoria and Terranova (Sicily), usually regarded as synonyms of R. gracilis, are here lectotypified and their taxonomic status discussed. Thanks to our study, the presence of R. gracilis in Italy is confirmed and, now, it is reported in a national conservation framework.

AbstractThe construction from 1386 of Milan Cathedral, the largest Gothic church ever constructed in Italy, was one of the most important episodes in the history of Italian and European architecture. The documentation of the lateTrecentoand earlyQuattrocentodiscussions over how to build the Cathedral is extraordinarily rich and extensive, and permits a consideration of the project from many points of view including the relationship between medieval architectural theory and an actual project. At the same time, any enquiry has to contend with the copious modern literature and the conclusions that have been reached hitherto – often erroneously in our view – about many of the most salient points. We thus re-examine published and unpublished documentation and the existing literature, analysing especially the format of the building's elevation, the proposals by Gabriele Stornaloco and Jean Mignot, and the drawings attributed to Antonio di Vincenzo. We also reconsider the notions ofarsandscientiawhich have previously been misinterpreted in discussions of the cathedral documentation.

Abstracttitle SUMMARY /title The National Archive of Lucca holds an extensive collection of drawings that document a wide-ranging campaign of topographical surveys carried out between 1580 and 1583 for the purpose of tracing a detailed map of fortifications under the domain of the Republic of Lucca. The surveys lie chronologically between the map drawn by the military engineer Alessandro Resta in 1567 and the accurate chorography of the State of Lucca produced at the end of the same century by the Paduan cosmographer Giovanni Antonio Magini. Historical circumstances suggest that the surveys may be attributed to the military engineer Vincenzo Civitali, who had been hired during that period to superintend the State's fortifications along the border between the Duchy of Modena and the Grand Duchy of Tuscany. Apart from providing information on the Republic's cartographic and defense projects, these drawings provide firsthand documentation of the work of a 16th C. topographer, as well as revealing the strategy applied when locating measuring stations which often coincided with a signaling tower, the choice of measuring instruments most readily associated with the simple theodolite, and the adoption of a surveying methodology that consisted in measuring azimuths and distances according to the codified precepts of 16th C. treatise writers.

Here, we reconstruct the history of the Botanic Garden of Brera of Milan from the Restoration of the Austrian Empire up to the early years of the Kingdom of Italy, when in 1863 the garden passed hands from the Liceo di Sant’Alessandro to the Istituto Tecnico Superiore of Milan. The reconstruction is based mostly on unpublished documentation preserved at the Archivio di Stato of Milan, the Biblioteca Braidense of Milan, the libraries of the Museo di Storia Naturale of Milan and the Archivio di Stato of Milan, the Archivio del Liceo Classico Statale Cesare Beccaria of Milan, the historical archives of the Politecnico of Milan, the Biblioteca di Biologia Vegetale, Dipartimento di Scienze della Vita e Biologia dei Sistemi, University of Turin, the Autografoteca Botanica of the Botanical Garden of the University of Modena, and the library of the Botanical Garden of the University of Padova. Overall, the period was one of slow decline for the Botanic Garden of Brera, against which successive directors – namely, Antonio Bodei, Francesco Enrico Acerbi, Giuseppe Balsamo Crivelli, Vincenzo Masserotti and Giustino Arpesani – combatted in vain. In particular, Balsamo Crivelli fought with great passion for many years to keep the level of the Botanic Garden of Brera at a satisfactory level, but he did not achieve the desired aim. However, he complied a partial list of the garden’s plants, of which an updated version is presented here.

ABSTRACTIn the autumn of 1790 Mozart undertook the penultimate journey of his life to participate in the coronation of Leopold II as Holy Roman Emperor in Frankfurt am Main. His attendance and performance at this significant imperial gathering were an investment designed to improve his fortunes. But Mozart's gamble failed. Though it was a key political event, and despite its significance as one of Mozart's final sojourns, not much more is known about the music of the Frankfurt coronation. This article offers a new understanding of Leopold II's imperial accession, positing the coronation as a vibrant context for music culture. Contrary to narratives that position Mozart's concert above all others, I argue that this was far from the case according to his contemporaries. During the coronation festivities the city hosted three theatre companies and many celebrated musicians, including Ludwig Fischer, Johann Hässler, Vincenzo Righini, Antonio Salieri and Georg Vogler, among others. Frankfurt was indeed filled with musicians who cooperated with and competed against one another in the hope of attracting substantial audiences comprised of the Empire's elite. Yet for Mozart, whose concert was poorly advertised and unfortunately timed, this competition proved too intense. By investigating the musical and political events of Leopold II's imperial coronation, I assert that Mozart's investment, which had the potential to alter his life forever, was unsuccessful in part because of a rumour that caused his desired audience to leave Frankfurt temporarily the very morning his performance took place.

The building of the old College of Piarist in Chełm – located on Lubelska Street, near the late baroque Church of Holy Apostles the Messengers – is now the seat of the Wiktor Ambroziewicz Chełm Land Museum. Until now, it has not raised much interest among researchers and – appearing as a work of architecture devoid of expressive style features – has not been the subject of scientific reflection. This situation is changed by the disclosure of the preserved drawing from 1698, showing the building in a horizontal projection and axonometric view, stored in the Archivio Generale delle Scuole Pie in Rome. The information contained in written documents kept there allow to determine the time of construction of the building for the years 1698-1700. The project proves that the preserved edifice did not change substantially its one-story block, set on the plan of the letter H. The innovations concerned only the roof part over the main body, which was originally the Krakow roof, and the extension of one of the side wings in 1720-1724 (so that the college was connected to the church). Neither did the subsequent transformations significantly affect the internal divisions, be it in the two-and-a-half tract main corpus, with the cross-corridor communication system introduced therein, or in the single tract side wings. The shape of the building and the severity of the development of its facade, representing the baroque in its classicizing version, suggests the designer – Giuseppe Piola, an architect working in Warsaw at the turn of the 17th and 18th centuries, building, at the request of the Piarist order, also their church and monastery complex in Szczuczyn. However, the extension of the college wing made in the first half of the 18th century should probably be associated with the person of another capital architect – Carlo Antonio Bay, who at the same time, together with his son-in-law, Vincenzo Rachetti, also an architect, made calculations for the Piarist priests from Chełm for the profitability of their parcel located in the suburbs of the city of Lublin. The building in Chełm was a monastic college, and at certain times also a “profesorium”, in which Piarist clerics learned philosophy at a higher level of education. Contrary to some suggestions, there was never a public school run by the Piarists in this building. It was founded – as a Russian gymnasium – only after the January Uprising and the dissolution of the Scholarum Piarum community.

Abstract Background: Breast cancers are traditionally viewed as immunologically silent. However, recent research has demonstrated that the triple-negative breast cancer (TNBC) and HER-2 positive (HER2) are capable of stimulating the immune system. In contrast, the most commonly diagnosed subtype, the luminal A (hormone receptor positive (HR+)/HER-2 negative), is associated with a lower degree of tumor infiltrating lymphocytes (TILs) and low expression level of immune checkpoint regulators such as programmed death-ligand-1 (PD-L1). Cryoablation, the destruction of cells by ultra-low temperatures, has been used to treat benign breast disease and clinical trials have been conducted to determine its utility for invasive breast cancers. The primary endpoint has been the rate of complete tumor ablation with no assessment of immunological responses. We hypothesized that neoantigens released during cryoablation might be sufficient to trigger immune responses that could aid in the prevention/reduction of metastatic spread and relapse of breast cancers.In this pilot study we evaluated biopsy material, cryoablated specimens and sentinel lymph nodes (SLN) from patients enrolled in the ACOSOG Z1072 trial at Lankenau Medical Center (LMC) [N=18] with cancers < 2 cm. Responses were compared to patients treated with surgical resection.Methods: After obtaining IRB approval for retrospective analyses of specimens from the ACOSOG Z1072 trial, immunohistochemical staining of archival specimens was performed. An on-going biomarker study protocol was amended to include immune markers and used to select control patients of similar age and tumor characteristics. Per trial protocol, tumors had to be be surgically removed within 28 days of cryoablation, but the exact time was left to the surgeons discretion. Participating surgeons at LMC preferred to remove tumors on average 24.7 days after ablation resulting in an average time from diagnosis to surgical removal of 43.3 days (range 25- 56 days). Although most breast surgeries are performed within two weeks of diagnosis; some patients defer their surgery for personal reasons. This allowed us to identify patients (N=26) with a comparable average time from diagnosis to surgery of 37 days (range 21 - 66 days). Sections were stained for CD4, CD8, CD20, CD21, and CD1c to assess possible changes in immune repertoire due to cryoablation. We also evaluated presence of immune check-point regulators such as PD-L1 and CTLA-4 in biopsy, surgical material and SLN together with the immune modulator IDO1 using commercially available antibodies and standard techniques. Results: Cryoablation transformed tumors in patients into a gelatinous mass surrounded by a fibrotic capsule. The ACOSOG trial demonstrated an effective cryoablation-induced destruction of ER+ invasive ductal carcinoma lesions in 92% of patients (N=86). Sections from cryoablation patients displayed a necrotic core and infiltrating lymphocytes in the microenvironment. These masses had a slightly higher presence of CD8+ lymphocytes compared to CD4+. The inverse relation was observed in non-cryoblated specimens. SLNs from cryoablated patients had an elevated presence of CD20+ B cells compared to patients treated by surgery. Follicular dendritic cells (CD21+) were also present at higher numbers in SLNs from cryoablated patients. We detected positive staining for both PD-L1 and IDO1 among surgical and cryoablated patients. Sporadic presence of CTLA-4 was identified with a slight preference for cryoabalted patients. Conclusion: Cryoablation of breast cancer lesions can induce immune responses in vivo with possible anti-tumor activity in immunologically silent tumors such as HR+ breast cancers. All local cryo patients in the clinical trial are currently disease-free (F/U 5 - 13years), while 1 surgical patient is alive with late recurrence (11 years post-surgery). AbstractThe small Van Gogh painting discussed in this article (fig. I) has never been published before. Originally, it belonged to Antoine Philippe Furnée (I86I-I897), whom Vincent had met in I883 in The Hague through Furnée père, the proprietor of a chemist's shop which also sold artists' paint. In the letters (365 (300), 372 (307) and 409 (342)) Furnée is referred to as 'the surveyor'. He was an amateur painter to whom Van Gogh gave advice. The two men would go out into the nearby countryside together to paint landscapes. In April I884 Furnee went to Java, where he remained until I897. He was probably already ailing on his return, for he died a few months later. His property passed to a brother, Antoine Louis Cornelis (I867-I965), a respected pharmacist of The Hague. The little painting, signed 'Vincent' at the bottom left, is done in oil on canvas pasted on cardboard, 30.5 by 23.I cm. Examination under fluorescent light shows it to be covered entirely with old varnish. On the back Furnée the pharmacist wrote: 'Chestnut tree on Broeksloot by Vincent van Gogh. This study was given by Vincent van Gogh to my brother A. Ph. Furnée (the surveyor)'. The gift accounts for the painting's being signed. Like many of the Hague paintings, this early work is of limited artistic merit. The poorly indicated space and forms are however offset by the good rendering of the contrast between light and dark areas. (Compare the paintings F 8a (fig. 3), F I92 (fig. 5) and the watercolour SD I680 (fig. 4).) The authenticity of the painting was established back in I963 in an appraisal by R. W. D. Oxenaar, and again at the Netherlands Institute for Art History by the present authors in I979, the year it was sold by the Furnee family. Two little pictures painted by Furnee on his expeditions with Vincent van Gogh come from the same source (figs. 6 and 7).

En este artículo se investiga el nacimiento de los estudios crítico-literarios en Costa Rica. Se estudia su aparición a finales del siglo XIX e inicios del XX. Se considera que la revisión de este género académico es relevante pues ofrece las claves operativas y las representaciones ideológicas que posibilitan entender los procesos de canonización y descatalogación de la literatura nacional. Se inicia con las primeras antologías poéticas de Costa Rica y la polémica nacionalista de 1894. Luego se consideran los primeros estudios estéticos publicados en el país. Entre los autores que participan destacan Antonio Zambrana, Moisés Vincenzi, Rafael Estrada, Justo Facio, Rogelio Sotela y Roberto Brenes Mesén. El idealismo croceano y el impresionismo crítico francés operan como códigos epistémicos orientadores. A esto se agregan los procesos de adaptación ideológica interna que le permite a la literatura operar como modelo educativo.

During the seventeenth century, the growth of opera created the need for a large number of artists to perform in theatres throughout Italy and, increasingly, in much of Europe. The biographies of nearly all the singers who performed in Venice, the centre of opera during the middle of the century, and in other cities of Italy remain unwritten and, in most cases, unwritable. For some singers, including Giovanni Antonio Cavagna, Nicola Coresi and Vincenza Giulia Masotti, letters survive that convey something of their personalities. Yet, for the most part, we know nothing of their families and the early years of their careers, nor of their lives as performers. This article will explore several episodes in the career of the Roman singer Silvia Gailarti Manni, whose operatic appearances during three decades have been known to scholars through librettos, but whose life has never before come into focus.

Abstract. HBIM methodology is increasingly used for the management of all aspects of architectural heritage, from survey and analysis to conservation, management and restoration issues.Application of HBIM are the so-called Scan-to-BIM processes in which the artifact is surveyed with digital techniques of laser scanning and photogrammetry. These techniques result in point clouds, the basis of the subsequent process of informative and geometric modelling of the artifact. The resulting "smart models" are composed of parametric objects rich in information that can be easily updated at any time.The proposed methodology aims at integrating a study of architectural orders, whose results become preparatory to the subsequent phases of survey and modeling, to the classic Scan-to-BIM workflows. In particular, in the modeling these results have allowed a more targeted choice of techniques used.The method has been applied to the atrium of the former Jesuit College of Santa Croce in Cagliari, which today hosts one of the seats of the Faculty of Engineering and Architecture of the University of Cagliari; in particular, the atrium of the east body of the former Jesuit College, designed by the Piedmontese architect Antonio Felice De Vincenti, has been modelled.

This essay concentrates on early modern expressions of literary hybridity in the 17th-century gallant letters of Vincent Voiture (1597-1648), Charles Cotin (1604-1681) and Antoine Godeau (1605 -1672), circulating in the Parisian salon L’Hôtel de Rambouillet. Firstly, we look at the hybridity of the letter form an sich, within the context of salon sociability and early gallantry. Secondly, we study the multiplicity of both the authorial voice and the audience of the gallant letter. Thirdly, we highlight the intriguing exchange between genres within the gallant letter, as well as their confrontation and, finally, cross-fertilisation. This tripartite case study substantiates our conviction that literary hybridity functions as an intriguing indicator and catalyst of literary evolution and creation.

Abstract Background: In order to determine the optimal treatment strategy for oligometastatic breast cancer (OMBC), effective and safe treatments for metastatic sites and sensitive and specific imaging techniques are needed. But it is also essential to know the incidence of oligometastatic breast cancer and its clinical and biological characteristics [1]. Efficient imaging techniques and therapeutic tools exist, but knowledge of incidence, clinical and biological characteristics of OMBC is scarce. This is partly due to the lack of publications describing these data on recent, consecutive, and unselected series of OMBC. Methods: we retrospectively collected data from 998 patients diagnosed with synchronous or metachronous metastatic breast cancer (MBC) between January 2014 and December 2018 at our institution. The only criterion used to define OMBC was the presence of one to five metastases at diagnosis. Hormone receptor (HR) and HER2 receptor status, histology, SBR grade, number of metastases and organs affected were collected. Results: Of 998 MBC, 15.8% were OMBC (158/998). Among the series, 88% (139/158) of OMBC had 1 to 3 metastases and 86.7% (137/158) had only one organ involved. Among 158 patients, 52.5% (n=83) had bone metastases, 20.9% (n=33) had lymph node metastases, 14.6% (n=23) had liver metastases, 13.3% (n=21) had brain metastases, 8.2% (n=13) had lung metastases, and 3.8% (n=6) had others (skin, pancreas, adrenal). Among these 158 patients, 83.4% (n=131) had ductal breast carcinoma, 55.7% (n=88) had HR+/HER2- OMBC, 25.3% (n=40) had HER2+ OMBC and 19% (n=30) had HR-/HER2- OMBC. HR+/HER2- subtype was statistically associated with bone and bone only metastases (p=0.001), HER2+ subtype with brain metastases (p=0.001) and HR-/HER2- subtype with lymph node metastases (p=0.008). Visceral metastases (lung or liver) are not statistically associated with any biological subtypes. The proportion of OMBC with SBR grade III was statistically higher than in a series of 22,109 patients with MBC [2] (49.4% vs 35.2%; p< 0.001). Conclusion: OMBC is a heterogeneous entity. OMBC incidence is certainly much higher than the commonly used values. Abstract Introduction: Exposition of cardiac conduction system during breast radiotherapy has never been studied, despite increasing use of intensity modulated radiotherapy which expose larger volume to low dose bath. We evaluated conduction node exposure during breast irradiation with volumetric modulated arctherapy (VMAT) and estimated the potential dosimetric benefit with intensity modulated protontherapy (IMPT). Materials and methods: Atrioventricular (AVN) and sinoatrial (SAN) nodes were retrospectively delineated according to published guidelines on the simulation CT scans of twelve breast cancer patients having undergone conserving surgery and adjuvant locoregional VMAT. IMPT treatment was re-planned on the simulation CT scans for all breast cancer patients. Mean and maximum doses to the SAN and the AVN were retrieved and compared. Correlation coefficients were calculated between doses to the SAN or the AVN and to the whole heart. Results: Average mean doses to the SAN and to the AVN were 2.8 Gy and 2.3 Gy respectively for left-sided irradiation and 9.6 Gy and 3.6 Gy respectively for right-sided irradiation. Average maximum doses to the SAN and to the AVN were 3.5 Gy and 2.8 Gy respectively for left-sided irradiation and 13.1 Gy and 4.6 Gy respectively for right-sided irradiation. IMPT significantly reduces doses to conduction nodes. Correlations between doses to the SAN or the AVN and the whole heart were usually significant. Conclusion: SAN and AVN can be substantially exposed during breast VMAT, especially for right-sided irradiation. Cardiotoxicity studies evaluating conduction node exposure might define dose constraints and criteria for additional cardiac sparing techniques, such as respiratory techniques or proton therapy, which could be beneficial to patients with underlying rhythmic or conduction disorders. Abstract Introduction: Patients with BRCA1/2 mutations have a higher risk of developing breast cancer compared to the wild-type population. For patients with a BRCA mutation, there are no specific recommendations for surgical management. The aim of this study was therefore to retrospectively investigate overall survival (OS) and recurrence-free survival (RFS) of BRCA mutated patients with localized invasive breast cancer, by comparing conservative surgery versus mastectomy. Methods: This study was based on data from the Côte d’Or breast and gynecological cancer registry. Data from patients with a constitutional BRCA1/2 mutation who presented with invasive breast cancer were collected retrospectively from 1998 to 2018. The Kaplan-Meier method was used to describe RFS and OS. Results:A total of 104 patients were included in the analysis, of whom 69 had conservative surgery and 35 underwent mastectomy. Regarding survival, there was no significant difference in OS (HR =1.49; 95% confidence interval (CI) [0.76-2.93], p=0.25). Similarly, there was no significant difference in RFS (HR =1.40; 95% CI [0.81-2.40], p=0.22), survival without homolateral recurrence (HR =0.88; 95% CI [0.30-2.61], p=0.89), without contralateral recurrence (HR =1.50; 95% CI [0.55-4.09], p=0.42), or without distant metastatic recurrence (HR =1.42, 95% CI [0.69-2.90], p=0.33). Conclusion: In invasive breast cancer in a patient with a germline BRCA1/2 mutation, conservative surgery, when possible, appears to be a feasible option over total mastectomy, with no difference in overall survival. However, the patient should be informed of the aggressive nature of recurrence in this population requiring chemotherapy in most cases. Abstract Background: The lack of safety clearance of several metallic breast implants in 7T(Tesla) poses a significant hurdle to standard clinical breast cancer care and research from reaping the benefits of ultra-high resolution MR imaging. A breast biopsy clip (Ultracor Twirl, Becton, Dickinson and Company, Vernon Hills, IL) composed of nitinol, was tested for safety and artifact susceptibility clearance in a 7T MRI scanner, using standardized procedures. This clearance is significant in henceforth allowing patients with this implant to be scanned in now FDA approved ultra-high-field MRI scanners of 7T or less for clinical and research purposes. Methods: Tests for magnetic susceptibility (torque and translational attraction), MRI-related heating, and artifacts were conducted as per standardized protocols. The torque and translational attraction tests evaluated the effects of magnetic force by the MRI to cause the clip to move and twist respectively. The heating test was conducted with customized MR parameters of short TR (repetition time) and maximum echo-train length, designed to induce temperature change. The artifact test using T1 weighted spin and gradient echo imaging sequences, evaluated potential localized signal loss that may result in misrepresentation of the imaged area. This may occur due to the presence of the metallic clip in the MR environment. Results: The torque and translational attraction tests respectively indicated that the MR environment did not induce any movement in the clip in eight orientations, with a deflection angle of 0 degrees. Results of the heating test indicated no significant temperature change of the clip. A temperature change of less than 0.45C° was observed in the phantom gel in both the absence and presence of the clip, which is well within the safety threshold (< 1°C). Results of the artifact test indicated a very small artifact, with the largest artifact cross-sectional area appearing on gradient echo images. Conclusion: These cumulative results indicate that the Ultracor Twirl breast biopsy clip is safe for imaging patients at 7T. Citation Format: William Dong, Kanchna Ramchandran, Adam Galloy, Marco A. Abstract Background: Chemotherapy is the main treatment in patients with pre-treated endocrine-resistant HR+/HER2– metastatic breast cancer, but has limited efficacy and substantial toxicities. The antibody-drug conjugate Dato-DXd consists of a humanized IgG1 mAb targeting TROP2 attached via a stable cleavable linker to a topoisomerase I (TopI) inhibitor payload. Heavily pre-treated patients with metastatic triple-negative breast cancer in the TROPION-PanTumor01 (NCT03401385) study of Dato-DXd showed a manageable safety profile and highly encouraging objective response rates (ORR by blinded independent central review [BICR]: 34% in all patients; 52% in patients treatment-naïve to TopI inhibitor-based therapies). The metastatic HR+/HER2– breast cancer cohort of TROPION-PanTumor01 has completed enrollment (n=41); data are currently maturing. Trial design: TROPION-Breast01 (NCT05104866) is an ongoing, global, phase 3, open-label, randomized trial evaluating efficacy and safety of Dato-DXd vs investigators’ choice of chemotherapy (ICC) in patients with inoperable or metastatic HR+/HER2– breast cancer. Patients (n≈700) are randomized 1:1 to Dato-DXd 6 mg/kg IV Q3W or ICC (eribulin, capecitabine, vinorelbine, or gemcitabine) until progression. Adults with an ECOG performance status of 0–1, who experienced progression on or are unsuitable for endocrine therapy, and received 1–2 prior lines of standard-of-care chemotherapy in the inoperable or metastatic setting are eligible. Monotherapy treatment with inhibitors of mTOR, PD-[L]1, CDK4/6 and PARP do not count as prior chemotherapy lines. Patients must have ≥1 measurable lesion per RECIST 1.1 and an archival or fresh formalin-fixed and paraffin-embedded tumor sample. Clinically inactive brain metastases are permitted. Dual primary endpoints are progression-free survival (PFS) by BICR, and overall survival. Secondary endpoints include PFS per investigator, ORR, disease control rate, patient-reported outcomes, and Dato-DXd pharmacokinetics and immunogenicity. Exploratory endpoints include TROP2 expression and exposure–efficacy relationship. Patients are stratified by number of prior chemotherapy lines, prior CDK4/6 inhibitor use, and region. Abstract Background: More than two thirds of patients with advanced breast cancer (ABC) have estrogen receptor-positive (ER+), human epidermal growth factor receptor 2-negative (HER2−) tumors. In most countries, current standard-of-care first-line treatments include an aromatase inhibitor or fulvestrant, a selective ER degrader, combined with cyclin-dependent kinase 4/6 inhibitors. Concurrent use of luteinizing hormone-releasing hormone agonists is recommended for men and premenopausal women with ABC. Nevertheless, almost all ABCs eventually become resistant to endocrine therapy and the disease is incurable in these cases. New therapies are needed to combat endocrine therapy resistance, maintain patient health-related quality-of-life, and delay the need for chemotherapy. AZD9833 (camizestrant) is a highly potent, next-generation selective ER degrader and pure ER antagonist that has demonstrated anticancer properties across a range of preclinical models, including those with ER-activating mutations (Scott et al, AACR 2020). A Phase I dose-escalation and expansion study (SERENA-1) has demonstrated that AZD9833 is well tolerated and has a promising antitumor activity when administered alone or in combination with the cyclin-dependent kinase 4/6 inhibitor palbociclib (Baird et al, SABCS 2020). SERENA-4 (NCT04711252) is a randomized, multicenter, double-blind, Phase III trial to evaluate the safety and efficacy of AZD9833 in combination with palbociclib for patients with ER+/HER2− ABC who have not received systemic treatment in the advanced disease setting. Methods: SERENA-4 will enroll 1342 patients with de novo or recurrent ER+/HER2− ABC who have not previously received systemic treatment for their locoregionally recurrent or metastatic disease. Patients with recurrent disease must have received adjuvant aromatase inhibitor or tamoxifen therapy for at least 24 months without relapse. Patients will be randomized 1:1 to receive oral treatment with either (a) AZD9833 75 mg once daily, palbociclib 125 mg once daily for 21 days followed by 7 days off treatment and a placebo for anastrozole 1 mg once daily or (b) anastrozole 1 mg once daily, palbociclib (same as active arm) and a placebo for AZD9833 75 mg once daily. Men and premenopausal women will also receive a luteinizing hormone-releasing hormone agonist. The primary endpoint is progression-free survival (PFS; up to 5 years). Secondary endpoints include overall survival (up to 8 years), second PFS, time to chemotherapy, objective response rate, and changes in health-related quality-of-life measures. Abstract Goal To investigate the efficacy of alpelisib added to fulvestrant beyond progression in patients with HR+ HER2- advanced breast cancer (ABC). Background The SOLAR-1 study (Andre et al, 2019) has shown that the addition of alpelisib, an alpha-specific PI3K-inhibitor to fulvestrant led to a PFS-benefit of 5.3 months in patients with PIK3CA-mutated HR+ HER2- ABC. In this study, patients were fulvestrant-naive, and only 6% of this population was previously treated with a CDK4/6 inhibitor (CDK4/6i). Nowadays, most patients have been treated with a CDK4/6i, either combined with an aromatase-inhibitor (AI) in 1st line, or with fulvestrant in the 1st or 2nd line setting. Results of the ongoing Dutch SONIA-trial (NCT03425838) will show whether the addition of CDK4/6i to an AI in 1st line is superior to the addition of CDK4/6i to fulvestrant in 2nd line. Patients who have progressed on a CDK4/6i in either line and fulvestrant may be treated with fulvestrant and alpelisib. However, the efficacy of fulvestrant beyond progression combined with alpelisib is unknown. Aim of this study The aim is to determine if treatment with fulvestrant beyond progression and alpelisib results in a clinically meaningful median PFS of ≥6 months. Population Patients must be adults with HR+ HER2- ABC with an activating PIK3CA-mutation. Patients must have progressed on fulvestrant (+/- CDK4/6i), and fulvestrant must be the most recent line of therapy. Patients with ECOG performance score 0,1 or 2 are eligible, as well as those with controlled (HbA1C < 8.4%) diabetes mellitus type 2. Patients with uncontrolled diabetes, visceral crisis, symptomatic CNS metastases or clinically relevant heart disease are ineligible. Patients with diseases or previous surgery that might affect the bioavailability of alpelisib, those in need to use CYP3A4 inhibitors or BRCP inhibitors, as well as patients with any other conditions that would put them at particular risk when partaking in the study, are also ineligible. Abstract Background: Breast cancer (BC) is the most frequent cancer and the second leading cause of cancer death for women. Although there is a need of new treatments to improve BC outcome, patients with BC are still under-represented in early phase trials, possibly because of the availability of numerous approved treatments and widely recruiting phase II and III trials. We aim to characterize the molecular profile, outcome and prognostic factors of patients with BC treated in a phase I trial at our institution. Methods: We retrospectively analyzed medical records of every BC patients treated consecutively in a phase I trial in the Early Drug Development Department (DITEP) at Gustave Roussy Cancer Center. Multivariate logistic regression models were used to determine the association between known prognostic factors such as ECOG, LDH or albumin levels, number of previous lines of treatment, metastatic sites and duration of treatment. When available, molecular profile was also reviewed. Results: Between April 1st 2008 and December 31th 2021, 4682 patients were enrolled and treated in a phase I trial in our department. Among them, 272 were treated for BC (5.8%): 271 women and 1 man, in 74 different trials. The median number of BC patients treated per year was 16.5 (min 1; max 33). Median time between consented date and C1D1 was 14 days (min 1; max 63). Median age at C1D1 was 50 years old (min 24; max 82). 5 patients (1.8%) were treated in an adjuvant setting (therapeutic vaccine) while all the others were metastatic. 12 patients (4.5%) had brain metastases and 127 patients (47.6%) had liver metastases. 186 patients (70%) had only 1 or 2 metastatic sites, and 81 (30%) had three or more sites. Triple negative was the most common subtype, representing 132 patients (48.5%), 125 (46%) were ER+ and 15 (5.5%) Her2+. Patients received a median number of 3 prior lines before enrollment (min 0; max 19). Overall, only 78 patients (28%) had a genetic testing: 30.7% had a germline BRCA1/2 mutation. 146 patients (54%) had a molecular profile (liquid biopsy or tissue sample) before their enrollment and among them 32,9% were oriented in a trial according to their specific profile. Regarding the type of treatments received: 21.7% were immunotherapy, 73.6% were targeted therapies (46.7% in monotherapy, 26.9% in different combinations with immunotherapy, chemotherapy, radiotherapy or endocrine treatments), 2.9% were chemotherapy and 1.8% endocrine therapy. The overall response rate was 11.1% (0.4% complete response, 10.7% partial response, 37.5% stable disease, 43.8% progressive disease and 7.6% not evaluable). Main reasons for discontinuation of treatment were: progressive disease (80.5%), toxicity (9.6%) and end of trial (6.6%). Median duration of treatment was 2 months (min 0; max 44), only 28.6% of patients were still treated after 3 months of trial and 10% at 6 months. At data cut off (June 21st 2022) median overall survival was 11 months. All patients had discontinued the trial by the time of the analysis: median number of lines of subsequent treatment was 2 (min 0; max 10) and 31 patients (11%) were enrolled in another phase I trial in our center. In a multivariate analysis, there was no prognostic factor associated with duration of treatment whether it was albumin (p=0.76) or LDH levels (p=0.68), ECOG (p=0.67), number of prior lines of treatment (p=0.79) or number of metastatic site (p=0.05). Conclusion: These results are consistent with prior published data as only few patients with BC are addressed for inclusion in a phase I trial, despite response rates and survival rates similar to other phase I patients. We did not find any specific factor associated with duration of treatment. It is likely that a thorough molecular profile could open more treatment options and access to phase I trials. Abstract Objective: There are increasing evidences that radiation doses to specific cardiac substructures are associated with cardiac adverse events. Manual delineation of cardiac substructures is time consuming, and auto-segmentation of cardiac substructure atlases has consequently been evaluated. However, proper delineation of small substructures, such as the left anterior descending coronary artery (LADCA), is challenging and conduction system substructures have never been considered, despite frequent reports of radiation-induced arrhythmias for thoracic irradiation. The aim of this study was to propose and evaluate a simplified auto-segmentable functional cardiac atlas. Methods: We created a cardiac substructure atlas from 20 breast cancer patients’ CT scans consisting of the four cardiac cavities, of a high-risk cardiac zone (HRCZ) as a LADCA surrogate and of the two cardiac conduction nodes. Performance evaluation of atlas-based auto-segmentation (ABAS) was evaluated on a validation data set consisting of 20 additional CT scans. Dice similarity coefficients (DSC) were used to evaluate the concordance level between the manual and the automatic segmentations. Results: The average duration of manual segmentation of the proposed cardiac atlas ranged between fifteen to twenty minutes, while ABAS lasted two minutes. The median DSC for the delineated cardiac substructures was 0.718. The highest similarity between manual and automatic segmentation was observed for the left ventricle with a median DSC of 0.87, while the lowest similarity between manual and automatic segmentation was observed for the NAV with a median DSC of 0.15. Regardless of the considered cardiac substructure, auto-segmentation tended to result into smaller volumes than manual segmentation. While the volumes were not statistically different for the left ventricle, the volumes were statistically different for the other substructures. While smaller, the auto-segmented NAV and SAN were systematically localized within the manual contours. The auto-segmented NAV could be approximated by a 1.6-cm sphere and the auto-segmented SAN by a 1.0-cm sphere. Conclusion: We proposed a simplified functional cardiac atlas, for which coronary delineation difficulties were circumvent using a surrogate high risk cardiac zone and cardiac conduction system was considered. Most cardiac substructures were associated with acceptable ABAS properties. Such atlas could be potentially evaluated for epidemiological studies and clinical practice.
