Texte intégralIntroduzione: Il citomegalovirus umano (CMV) costituisce una frequente causa di malattia e mortalità nei pazienti sottoposti a trapianto di organo solido (SOT). La ricostituizione dell'immunità cellulo-mediata specifica svolge un importante ruolo nel ridurre gli effetti dannosi di tale infezione. Scopo: Confrontare il recupero dell'immunità cellulo-mediata CMV-specifica attraverso il test ELISpot nei pazienti adulti sottoposti a SOT (cuore, rene e fegato), al fine di standardizzare l'iter diagnostico e terapeutico e poter effettuare delle terapia su misura in base al livello individuale di rischio di infezione. Metodi: Studio osservazionale su 209 pazienti adulti sottoposti a SOT (47 trapianti di cuore, 102 trapianti di rene e 60 trapianti di fegato). Tempo di osservazione 360 giorni dopo il trapianto. I parametri esaminati sono stati: test ELISpot, CMV-DNA con tecnica PCR Real Time, CMV-IgG e IgG-avidity. Risultati: 1) i pazienti nefrotrapiantati recuperano l'immunità cellulo-mediata più lentamente rispetto ai pazienti con trapianto di cuore e fegato; 2) la ricostituzione dell'immunità cellulo-mediata specifica avviene più lentamente nei pazienti sieronegativi per CMV; 3) l'immunità umorale svolge un ruolo marginale nel controllo dell'infezione da CMV. Conclusioni: l'utilizzo del test ELISpot permetterebbe di individuare i pazienti a rischio di infezione da CMV e di ottimizzare il loro trattamento, evitando casi di infezione tardiva.

Texte intégralContesto: Il trapianto di cuore è l'unico trattamento curativo disponibile per i pazienti con insufficienza cardiaca allo stadio terminale. Durante il primo anno dopo il trapianto più del 25% dei pazienti può subire episodi di rigetto e affrontare il rischio di sviluppare rigetto con conseguente disfunzione dell’ organo trapiantato con un aumento della morbilità e mortalità. Prevenire e trattare il rigetto acuto è l’ obiettivo principale per i medici che lavorano con pazienti trapiantati. Le linee guida ISHLT 2005 e 2013 hanno definito il profilo istopatologico di tre tipi di rigetto: Cellulare (ACR) Humoral (AMR) e Mixed (MIX). Al giorno d'oggi le biopsie endomiocardiche seriali (EMB) a intervalli decrescenti durante il primo anno dopo il trapianto e gli esami di laboratorio, come le misurazioni di anticorpi donatore specifici (DSA), rimangono parametri di riferimento nella diagnosi e nel monitoraggio del rigetto acuto, ma sono soggetti a artefatti dovuti alle metodologie di campionamento, interpretazione e test. Pertanto questa valutazione istopatologica necessita di nuovi biomarcatori integrativi per caratterizzare la stratificazione del rischio nel rigetto da trapianto di cuore. Ad oggi, i meccanismi esatti coinvolti nel rigetto dopo il trapianto non sono completamente compresi, quindi la ricerca sui processi che governano i meccanismi di rigetto e la scoperta di biomarcatori efficaci per diagnosticare, monitorare e prevedere il rigetto sarà di grande valore per lo sviluppo e miglioramento delle terapie contro il rigetto. L'avvento della tecnologia di sequenziamento come Next Generation Sequencing (NGS) sta cambiando la genomica medica accelerando la scoperta di nuovi biomarcatori di malattie. I microRNA (miRNA) sono piccole molecole di RNA non codificanti (19-24 nucleotidi), altamente conservate, che regolano l'espressione dei geni a livello post-trascrizionale. Obiettivo: Lo scopo di questo studio è identificare il profilo di espressione di MicroRNA (miRNA) nel primo anno dopo il trapianto di cuore (HTX) con la tecnologia Next Generation Sequencing (NGS) in biopsie endomiocardiche (EMB) fissate in formalina e incluse in paraffina (FFPE), per caratterizzare i tre diversi tipi di rigetto da trapianto di cuore classificati come Cellulare, Umorale e Misto. Metodi: due gruppi di pazienti (pz.) sono stati inclusi: un gruppo di studio di 19 pz. e un gruppo di validazione di 14. Per ogni paziente abbiamo selezionato la prima biopsia endomiocardica (EMB) fissata in formalina ed inclusa in paraffina (EMB) per ogni tipo di rigetto. Abbiamo escluso i pz. presensibilizzati con precedente impianto del dispositivo di assistenza ventricolare sinistro (LVAD) e con precedenti episodi di infezione. Le biopsie sono state esaminate per la presenza di rigetto secondo i criteri di classificazione internazionali aggiornati (ISHLT 2005 e 2013). Abbiamo quindi individuato quattro gruppi: Acute Cellular Rejection (ACR) con ACR a 12 punti:> = 2R, pAMR: 0, DSA: Neg; Misto con 6 pts ACR:> = 2R, pAMR> 1 (i +), DSA: Pos; Reiezione mediata da anticorpi (AMR) con 5 punti ACR: 0, pAMR> 1 (i +), DSA: Pos; Controllo con 10 punti: ACR: 0, pAMR: 0, DSA: Neg. La piccola frazione di RNA della coorte di studio è stata sequenziata con NGS Ion Proton per definire l'espressione dei miRNA maturi. Abbiamo eseguito un'analisi successiva con edgeR confrontando a coppie i gruppi per identificare i miRNA espressi differenzialmente nei diversi rigetti. Abbiamo selezionato 13 microRNA secondo l'analisi bionformatica come possibili biomarcatori i quali sono stati confermati da qRT-PCR in tutti i pz. Con l'analisi di regressione logistica multivariata abbiamo identificato gruppi univoci di miRNA come modelli predittivi specifici per ciascun rigetto. Inoltre, la PCR in situ è stata eseguita sulle stesse EMBs per rilevare l'espressione e la localizzazione dei miRNA nei tipi di cellule all'interno delle EMBs. Risultati: l'identificazione del miglior metodo di estrazione di microRNA da EMBs FFPE è stato il primo risultato che ho raggiunto. Ho testato diversi metodi sia manuali che kit commerciali e ho modificato i protocolli per ottenere una buona qualità e una quantità adeguata di microRNA per l'applicazioni successive. Con NGS abbiamo ottenuto e analizzato oltre 2257 microRNA maturi in tutte le biopsie del gruppo di studio. I tre tipi di gruppi di controllo e di rigetto sono stati confrontati in coppia con l'analisi non supervisionata che mostra per ciascun gruppo un profilo tipico di miRNA differenzialmente espressi; in particolare: Misto vs AMR: solo 2 miRNA sovraespressi nel gruppo Misto suggeriscono una somiglianza tra i due tipi di rigetto. ACR vs AMR: 18 miRNA sovraespressi e 2 miRNA sottoespressi nell'ACR. Mixed vs ACR: 7 miRNAs non sovraespressi e 39 miRNA sovraespressi nel gruppo ACR. L'analisi ha rivelato che ci sono microRNA de-regolati tra i tre tipi di rigetto confermando la nostra ipotesi che i microRNA possano caratterizzare le tre condizioni patologiche. I MiRNA sono stati selezionati per un'ulteriore valutazione e convalida, in base al numero di reads risultanti da NGS, sulla loro FDR significativa (<0,05), fold change, p-value e il loro coinvolgimento in processi rilevanti correlati al rigetto come mostrato dalle analisi bioinformatiche basate su geni target validati e riportati in database pubblici come TarBase (versione 6.0) (111), miRTarBase (112), miRWalk (113), miRecords (114), DIANA-microT-CDS (115), miRmap (116) , miRDB (117), TargetScan (118) e miRanda (119). Alla fine abbiamo selezionato 13 microRNA. Per validare i dati NGS tramite qRT-PCR abbiamo arruolato altri EMBs da 14 pz. selezionati in base ai nostri criteri e abbiamo testato su tutte le 33 EMbs, sia quelle della coorte di studio che quelle della coorte di validazione, i microRNA selezionati. L'analisi di validazione ha mostrato un pattern di espressione simile per tutti i microRNA in particolare: 6 hsa-miRNA: 29c-3p / -29b-3p / 199a-3p / 190a-5p / 27b-3p / 302b-3p possono differenziare tutti i rigetti rispetto a controlli; 3 hsa-miRNA: 31-5p / 144-3p / 218-5p sono peculiari di AMR e MIX rispetto al controllo e ACR 2 hsa-miRNA: 451a / 208a-5p identificano MIX rispetto ai controlli. Usando l'espressione di miRNA e la condizione patologica come variabili dipendenti abbiamo creato modelli di regressione logistica: MIX: (miR-208a, 126-5p, 135a-5p); ACR: (miR-27b-3p, 29b-3p, 199a-3p, 208a, 302b-3p); AMR: (miR-208a, 29b-3p, 135a-5p, 144-3p) che identificano con alta specificità e sensibilità ciascun tipo di rigetto. Infine con PCR in situ abbiamo rilevato alcuni di questi microRNA in diversi tipi di cellule: miR-29b-3p era principalmente espresso nelle cellule muscolari lisce in ACR; miR-144-3p era espresso nei macrofagi e nelle cellule endoteliali; inoltre l'espressione di questo microRNA nei macrofagi era predominante e diffusa nell'ACR rispetto all'AMR. Il miR-126-5p è risultato espresso in campioni ACR e AMR non solo nelle cellule endoteliali ma anche nei cardiomiociti e nelle cellule muscolari lisce. Per il MicroRNA 451a abbiamo trovato una co-localizzazione del segnale nelle cellule endoteliali e nei linfociti. Conclusioni: Questo studio dimostra che i microRNA possono essere ottenuti facilmente dai tessuti fissati in formalina e inclusi in paraffina, i miRNA differenzialmente espressi sono coinvolti in meccanismi patofisiologici del rigetto quali regolazione e proliferazione del ciclo cellulare del sistema immunitario, vie infiammatorie mediate da NFkB e rimodellamento endoteliale. Secondo i nostri risultati, i miRNA sovra o sotto espressi hanno mostrato una modulazione di questi processi in un modo peculiare per ciascun tipo di rigetto. I modelli di regressione logistica identificati potrebbero rappresentare un potente strumento diagnostico e il rilevamento in situ dei miRNA getta nuova luce sui meccanismi patofisiologici del rigetto. Inoltre l'espressione di MiRNA 144-3p, 126-5p, 29b-3p e 451a identificati mediante PCR in situ in cellule endoteliali, cellule muscolari lisce e infiammatorie è diagnostica e costituisce un potenziale bersaglio farmacologico contro il rigetto da trapianto di cuore.

Texte intégralAllogeneic hematopoietic stem cell transplantation (HSCT) is a well-established therapy for several life-threatening hematological disorders, mostly malignancies. However, non-relapse mortality (NRM) has limited its use in patients aged over 65 years. Nowadays age cannot be considered an absolute contraindication for this treatment and the use of transplantation, in patient groups once considered unsuitable, is increasing significantly. NRM is due to three types of complications: immunological (graft versus host disease), infectious and toxic. Organ toxicity is directly related to the intensity of conditioning which is therefore reduced in case of comorbidities and in elderly patients. However, reducing the intensity of chemotherapy also means increasing the risk of relapse of the underlying haematological disease and therefore this adjustment must consider aging and comorbidity indexes in order not to reduce the curative potential of the transplant. To evaluate comorbidities we have a highly predictive score (the Comorbidity Index Score by Sorror, HCT-CI), while to evaluate aging there is a great clinical need to apply innovative biological age markers. The present work aims to evaluate the state of DNA methylation, a biological age index, in the setting of patients undergoing allogeneic haematopoietic stem cell transplantation for all haematological indication. The aim is also to correlate the epigenome with the transplant risk of the single individual.

Texte intégralL’osso è un tessuto connettivo specializzato costituito da cellule e matrice extracellulare mineralizzata. Le cellule principali sono gli osteoblasti, gli osteociti e gli osteoclasti. Gli osteoblasti depongono la matrice ossea, mentre gli osteoclasti sono i responsabili del suo riassorbimento. La fisiologia dell’osso è quindi il risultato di un delicato equilibrio tra deposizione di matrice ossea e suo riassorbimento. Quando l’azione catabolica degli osteoclasti è maggiore rispetto a quella anabolica degli osteoblasti si produce una progressiva fragilità ossea che porta ad un quadro clinico osteoporotico. L’osteoporosi primaria colpisce soprattutto la popolazione femminile dopo la menopausa. Molte patologie come il diabete mellito, l’iperparatiroidismo ed il trattamento a lungo termine con glucocorticoidi causano l’osteoporosi secondaria. L’osteoporosi indotta da glucocorticoidi è la più comune causa di osteoporosi secondaria. Il trattamento con glucocorticoidi è un noto procedimento di induzione dell’osteoporosi in modelli animali e può dunque rappresentare un primo esempio di modello “traslazionale” potenzialmente applicabile in clinica per indurre un ripopolamento dell’osso con cellule staminali mesenchimali o precursori osteogenici. Lo scopo di questo lavoro è stato pertanto valutare nel modello animale se sia possibile ripopolare l’osso danneggiato con preosteoblasti. I crani di topi neonati transgenici (GFP) sono stati prelevati e messi in coltura per ottenere preosteoblasti. Nelle colture in vitro è stata valutata l’espressione del gene Runx2 con la tecnica di Real time PCR, mentre la capacità osteogenica è stata analizzata con colorazioni citochimiche per la fosfatasi alcalina e per la deposizione di matrice ossea mineralizzata (Alizarin Red e Von Kossa). Per la realizzazione del modello in vivo topi C57BL/6 maschi di 3 mesi sono stati divisi in 3 gruppi [gruppo I (n=4): topi non trattati con farmaco e non infusi con cellule; gruppo II (n=4): topi trattati con farmaco non infusi con cellule; gruppo III (n=4): topi trattati con farmaco ed infusi con cellule]. Il farmaco (metilprednisolone) è stato somministrato per un mese alla dose di 75 mg/Kg/settimana. Negli animali appartenenti al gruppo III sono state infuse, attraverso iniezione nella vena della coda, 5 x 105 preosteoblasti GFP precedentemente espansi in vitro. I topi sono stati sacrificati, le tibie ed i femori sono stati prelevati e processati per l’analisi istomorfometrica e della microarchitettura ossea e per l’ immunoistochimica. In questi tessuti, l’espressione genica di Runx2, osteonectina (SPARC) e fosfatasi alcalina (ALP) è stata valutata tramite Real time PCR. In vitro i preosteoblasti producono fosfatasi alcalina durate i primi giorni di coltura in medium non differenziante, mentre il livello decresce in condizioni differenzianti, cioè in medium contenente acido ascorbico e β-glicerofosfato. I preosteoblasti mantenuti in medium di differenziamento per 30 giorni sono positivi alle colorazioni Alizarin Red e Von Kossa, quindi sono in grado di produrre matrice ossea mineralizzata, caratteristica degli osteoblasti funzionali e maturi. L’espressione del gene Runx2 aumenta durante il differenziamento, si ha un aumento del 50% nelle cellule differenziate per 30 giorni rispetto alle cellule non differenziate (p<0.05). L’inoculazione dei preosteoblasti nei topi del gruppo III ha evidenziato un aumento dei parametri statici di neoformazione ossea relativi all’osteoide (O.Th, OS/BS, OV/BV) ed un aumento del numero di osteoblasti attivi, cioè in corso di deposizione di osteoide, rispetto al gruppo II. Tra questi due gruppi non si sono osservate, invece, variazioni significative in termini di volume osseo (BV/TV), spessore trabecolare (Tb.Th) numero delle trabecole (Tb.N) e separazione fra esse (Tb.Sp). Non sono state rilevate, inoltre, differenze dei parametri di microarchitettura (Nd.N/TV, Nd/Tm). Risultati simili sono emersi dalla valutazione dei parametri indiretti di microarchitettura (Marrow Star Volume e Fractal Dimension). L’espressione genica ha dimostrato che nel gruppo II si ha una riduzione dell’espressione dei geni osteogenici rispetto al gruppo I (ALP: -50%, p<0.01; Runx2: -56.75%, p<0.01; SPARC: -44.5%, p<0.05). Nel gruppo III si è avuto un recupero dell’espressione dei geni osteogenici (ALP: +40%, p<0.05; Runx2: +66.28%, p<0.001; SPARC: +55%, p<0.01) rispetto al gruppo II. I campioni di tessuto per l’ immunoistochimica devono essere processati. Nel nostro modello sperimentale di osteoporosi indotta da glucocorticoidi nel topo, abbiamo sacrificato gli animali solo una settimana dopo l’infusione delle cellule; questi dati preliminari dimostrano che il nostro modello induce l’engraftment dei preosteoblasti nell’osso danneggiato. Tuttavia è richiesto un tempo di osservazione più lungo, di almeno 1-2 mesi per valutare se le cellule trapiantate siano in grado, non solo di integrarsi nel tessuto dell’ospite, ma anche di proliferare in vivo e di differenziare in osteoblasti maturi e funzionali.

Il trapianto di cellule staminali ematopoietiche (HSCT) rappresenta ad oggi la terapia per alcune patologie ematologiche maligne e non maligne e costituisce una sfida per iI paziente in età pediatrica e per la sua famiglia. Il progresso medico relativo alla procedura di HSCT ha significativamente aumentato il numero di trapianti eseguiti ogni anno ed ha migliorato le percentuali di sopravvivenza a lungo termine, pertanto gli studi scientifici hanno spostato il loro focus dalla valutazione dei tassi di sopravvivenza all’esplorazione della Qualità di Vita (QoL). I sopravvissuti a HSCT avvenuto in età pediatrica sono stati dimostrati essere una popolazione più suscettibile allo sviluppo di sequele psicologiche, cognitive, sociali e familiari, oltre che mediche (Khera et al., 2012; Syrjala et al., 2012). L’obiettivo dello studio è la valutazione degli aspetti psicosociali e comportamentali oltre che della QoL dei soggetti pediatrici sopravvissuti ad HSCT e delle loro famiglie (includendo i fratelli). Obiettivi secondari sono: la valutazione del livello di accordo tra survivor e genitori relativamente agli aspetti psicopatologici ed alla QoL, la valutazione dello stress genitoriale e infine l’identificazione di fattori di rischio e fattori protettivi per la salute mentale di questi soggetti. Metodo. Sono stati testati soggetti pediatrici sopravvissuti ad HSCT (almeno 5 anni dopo il trapianto) e i loro genitori. Ad ognuno è stato richiesto di completare una batteria di test che includeva i questionari di Achenbach (Child BehaviourChecklist-CBCL; Youth Self Report-YSR) e il PediatricQuality of Life Inventory 4.0 (PedsQL) e di produrre un breve testo libero. Ai genitori è stata richiesto inoltre di completare una intervista anamnestica ed ulteriori due test: il Short Form-36 (SF-36) e il Parenting Stress Index (PSI). Ai fratelli del sopravvissuto è stato chiesto di completare la YSR e di produrre un breve testo libero. I dati sono stati elaborati con l’utilizzo del software SPSS. Risultati. Dal 2012 sono stati reclutati 37 soggetti sopravvissuti ad HSCT (62% maschi). L’età media al momento dell’HSCT era 10 anni (range 0,7-11,2 anni). Cinquantaquattro percento dei sopravvissuti era stato sottoposto a trapianto allogenico non familiare. Sono stati inoltre testati 74 genitori e 38 fratelli. I punteggi dello YSR superano i cut off per competenze totali (22%), competenze sociali (13%) e lamentele somatiche (8%), mentre i punteggi alle CBCL risultano patologici principalmente nelle aree di competenze totali (49%), attività (22%) e sintomi somatici (11%). I sintomi riportati alla CBCL appaiono più significativi se: il sopravvissuto è femmina, in caso di trapianto allogenico familiare, presenza di tossicità mediche e se il tempo intercorso tra la comunicazione della scelta terapeutica e l’HSCT era più lungo. CBCL e YSR presentano un buon grado di accordo solo relativamente alle scale competenze totali (K=0.06, p= 0.002), sintomi somatici (K=0.21, p= 0.003) e problemi di attenzione (k=0.13; p=0.02). Rispetto alla PedsQL genitori e survivor non appaiono in accordo per l’area fisica (K=0.05; p=0.31) ed emotiva (K=0.09; p=0.06). Nella SF-36 le madri percepiscono una peggiore QoL nella scala relativa alla “limitazione del ruolo” quando hanno un solo figlio o più di tre figli, se il tempo intercorso tra la comunicazione della scelta terapeutica e l’HSCT è breve e in caso di trapianto allogenico familiare. I padri percepiscono una salute mentale peggiore se hanno un unico figlio o più di due e se sono sposati. I fratelli più vicini al sopravvissuto per ordine di genitura sembrano presentare una peggiore QoL secondo i genitori. Conclusioni.I soggetti sopravvissuti a HSCT in età pediatrica presentano livelli di stress psicologico in diverse aree (internalizzante, esternalizzante, dell’attenzione e della socializzazione), con una prevalenza di sintomi somatici. Rispetto alla QoL, i genitori riportano un funzionamento emotivo del sopravvissuto migliore a fronte di un peggiore funzionamento fisico. Il primo fratello sembra percepire una QoL peggiore. Madri e padri percepiscono diversamente il proprio distress, ma il fatto di avere due figli appare rappresentare in modo omogeneo un fattore protettivo. Saranno necessari ulteriori studi per approfondire la discordanza tra le percezioni e infine migliorare l’outcome e la cura dei pazienti pediatrici sottoposti ad HSCT e delle loro famiglie.

Numerosi studi hanno mostrato come i pazienti sottoposti a HSCT in età pediatrica possano manifestare alcune problematicità su diversi ambiti, come quello psicologico e sociale oltre che su quello medico (Khera et al., 2012; Syrjala et al., 2012). Pochi studi si sono concentrati sull’analisi simultanea di dati quantitativi con dati qualitativi (Bingen et al., 2012). Lo scopo di questo studio è quello di analizzare la percezione dell’esperienza di malattia sia nei pazienti, o survivor, che nei loro familiari. Altro scopo è quello di correlare dati di natura qualitativa con dati di natura quantitativa. Metodo. Abbiamo chiesto a tre popolazioni differenti di soggetti di rispondere ad una singola domanda aperta con una breve composizione scritta rispetto alla loro esperienza di malattia. Per il primo gruppo (Gruppo A), che ha compreso pazienti pediatrici, genitori e fratelli tale scritto è stato raccolto prima del trapianto e ad un anno da questo. In maniera medesima sono state indagate eventuali problematiche psicologiche nei pazienti e nei loro fratelli mediante il questionario Child Behavior Checklist 6-18 (CBCL) (Achenbach and Rescorla, 2001) e la QdV per i genitori attraverso il 36-Item Short Form Health Survey (SF-36). Il secondo gruppo (Gruppo B) invece ha compreso ex-pazienti pediatrici e i loro familiari testati a cinque anni dal trapianto. È stato utilizzato il questionario CBCL per l’individuazione di problematiche psicologiche negli ex-pazienti. Il terzo gruppo (Gruppo B-over) ha compreso soggetti adulti sottoposti a HSCT in età pediatrica, ad almeno 5 anni distanza dal trapianto, per questo gruppo è stata valutata anche la QdV mediante il questionario SF-36. Dati socio-demografici e sanitari sono stati raccolti. Per le analisi qualitativa dei testi è stato utilizzato il software T-LAB (Ver. 8.1.4; Lancia, 2012), che permette anche la valutazione congiunta di dati quantitativi raccolti mediante i test psicometrici e i dati qualitativi ottenuti attraverso le composizioni scritte. Risultati. Per il gruppo A sono stati raccolti 47 testi in T0 e 27 testi in T1. Punteggi anomali nelle scale indagate della CBCL (Problemi Internalizzanti, Problemi Esternalizzanti) e del SF-36 (Salute mentale) indicano in maniera ricorrente alcuni nuclei semantici. Nell’attesa del trapianto sono frequenti lemmi come ALTRI_BAM_MALATI, ALTRI_GENITORI, INCERTEZZA, DISPERAZIONE, MD_OGGETTI, HSCT E CON_INSIEME. Ad un anno di distanza invece i lemmi che facilmente ricorrono sono ANSIA_ANGOSCIA, ABITUARE, DIO, FORTUNA, MOLTO_TANTO e MORTE L’assenza di punteggi anomali nelle scale indagate invece si correlano a temi semantici differenti come ATTESA, NON_VEDERE, PERCHE_MOT, RIUSCIRE, REAGIRE, AFFRONTARE, UNITI. Ad un anno di distanza invece vi sono lemmi come AIUTO_AIUTARE, FORZA, MAMMA, PAPA, MET_GUERRA, SERENITA. Per il gruppo B sono stati ottenuti 98 scritti. I punteggi anomali delle scale indagate mediante la CBCL (Problemi Internalizzanti, Problemi Esternalizzanti, Problemi Totali) indicano una ricorrenza con temi semantici come FRATELLO_SORELLA, FILGIO_A, FIGLIO_SAN, MAMMA, BATTAGLIA. Punteggi migliori invece correlano maggiormente con temi come DESCRIVERE, PENSARE, PSICHICO_MENTALE, MORTE. Per il gruppo B-over invece sono stati ottenuti 18 brevi scritti. Possiamo rilevare delle differenze tra i generi per cui i maschi si concentrano maggiormente su lemmi come CORPO, CHEMIO_RADIO, OPERAZIONE_MD, CURE_MEDICHE, ESAMI_MD; mentre le femmine su temi semantici come MAMMA, PERSONE, DONATORE, FRATELLO_SORELLA, PADRE, PSICOLOGI, MD. I punteggi anomali indagati mediante il questionario SF-36 (Salute Generale) sono correlati con il gruppo di soggetti sottoposti ad autotrapianto e lemmi come CORPO, CHEMIO_RADIO, OPERAZIONE_MD, MOLTO_TROPPO. Punteggi migliori invece sono legati a parole chiave come MADRE, PADRE, PERSONE, DONATORE, SOSTENERE e con il trapianto allogenico da banca. Conclusioni. Per il primo gruppo emergono differenze significative rispetto alle narrazioni tra i vari membri del gruppo famiglia anche rispetto alle scale dei questionari utilizzati. Tali differenze oltre ad essere presenti rispetto ai due tempi della ricerca indicano come l’unità o la segregazione familiare abbia un’importante impatto sul proprio stato di salute mentale e dei propri figli, così come l’atteggiamento di accettazione e di come viene affrontata la malattia. La segregazione familiare compare come nucleo semantico fondamentale anche nel secondo gruppo per i punteggi anomali rispetto alle scale da noi indagate, come anche tutte le espressioni riferite alla malattia come lotta, battaglia e guerra. Nel terzo gruppo emergono differenze significative rispetto al genere come al tipo di trapianto effettuato. In generale possiamo ritenere che l’approccio di cura debba avere una particolare attenzione su come la malattia agisca sul vincolo familiare di tutti i suoi componenti.

Texte intégralWe investigated a strategy to ameliorate the motor symptoms of mice that received MPTP lesions, a rodent model of Parkinson’s disease, through transplantation of adult precursor post-mortem cells into the striatum. Post mortem neural precursors (PM-NPCs) were isolated from the subventricular zone (SVZ), grown in vitro and their differentiation capability was investigated by evaluating the expression of different neuronal markers. PM-NPCs differentiate mostly in neurons and unlike most other precursor cells, retain the capacity for migration and integration when transplanted into the adult brain. We performed PM-NPCs transplantation into the basal ganglia of MPTP lesioned mice. Transplanted PM-NPCs survived, differentiated and integrated into host circuitry, and modified motor behavior in MPTP lesioned mice. Our data suggest that PM-NPCs transplantation into the the striatum is a promising approach to invenstigate the potential benefits of remodeling basal ganglia circuitry in neurodegenerative diseases.

Texte intégralBackground. Chronic graft versus host disease (cGVHD) is the major late complication after allogeneic stem cell transplantation. Standard therapy is steroid and Cyclosporine-A (CyA); however, immune suppression (ISS) related infections or unresponsiveness to ISS, are major mortality causes. Extracorporeal photopheresis (ECP) has shown activity in treatment of cGVHD, but its use has been limited to first-line-unresponsive cGVHD. Aim of the study. This is a single center pilot study testing feasibility of a programme of photopheresis in association with standard therapy as first line treatment in high risk cGVHD. High risk was defined as the presence of parameters predicting high cGVHD-related mortality. Secondary objectives were response and complications incidence. Patients. Among 10 pts fitting enrolling criteria, 2 refused due to logistic problem or low compliance with the procedure, 8 were enrolled. Median age was 40. Donor was HLA identical sibling in 7 cases and MUD in 1. All cases presented with extensive/moderate-severe cGVHD; Akpek score was > 0 in 3/8 pts. Treatment plan. Pts started with Prednison (PDN) 1 mg/kg and CyA at cGVHD diagnosis; ECP was started with a frequency of 4 application/month in the first 3 months and 2/month for the subsequent 9 months; PDN and CyA were slowly reduced until suspension, or otherwise modulated. Study duration was 1 year. Pts were ruled out the study in case of ECP suspension, requirement of other ISS drugs in case of GVHD progression unresponsive to standard therapy, or severe infections. Response was evaluated with standard criteria, as progression, partial response (PR), very good PR (vgPR) or complete response (CR). Results. Adherence to protocol was: 6/8 pts at 3 months, 4/8 at 6 and 9 months, 3/8 at 12 mm; exit from the study was due to infectious complications (2), ECP suspension due to venous access related thrombosis (1) and clear cGVHD progression (2). In evaluable pts, response (CR+very good PR / <=PR) per trimester was 4/6, 2/4 and 3/4 at I, II and III respectively; at the IV trimester, 1 very good PR, 2 PR and 1 progression were observed. Complications were observed in 4 pts with : 1 case of pneumonia, 1 case of urinary tract infection, 3 cases of CMV antigenemia activation, 1 case of condilomatosis, 1 case of catheter related infection, 1 case of catheter related thrombosis and 1 case of hemolitic uremic syndrome. At 1 year, 2/8 pts died (1 TRM, 1 relapse). Conclusion. ECP in association with standard therapy is feasible; complications incidence seems to be similar to those observed in patients not treated with ECP; a larger group of patients is needed to evaluate response in this setting.

Texte intégralLo scopo principale della terapia immunosoppressiva dopo trapianto di fegato è passato dalla prevenzione del rigetto acuto alla preservazione della funzionalità a lungo termine dell’organo trapiantato e alla prevenzione degli effetti collaterali dovuti alla terapia immunosoppressiva. Per perseguire tale scopo è necessaria una gestione ottimale della terapia immunosoppresiva stessa. Tuttavia, la misurazione dei livelli ematici dei farmaci immunosoppressori, generalmente utilizzati come surrogato dei livelli di immunosoppressione, non fornisce informazioni relative alla reale intensità della soppressione del sistema immunitario. Pertanto l’individuazione di marcatori biologici di rigetto acuto e/o di tolleranza risulta fondamentale per poter migliorare la gestione della terapia immunosoppressiva dopo-trapianto di fegato. Gli scopi degli studi riportati in questa tesi sono: 1) determinare l’incidenza e gli eventuali fattori di rischio di rigetto acuto dopo trapianto di fegato, valutare in che l’influenza del rigetto acuto e della sua severità istologica sulla sopravvivenza dell’organo e del paziente dopo trapianto di fegato; 2) valutare il ruolo degli indici di funzionalità epatica e della conta eosinofilica ematica come potenziali marcatori biologici di rigetto acuto dopo trapianto di fegato, in particolare di grado moderato/severo; 3) valutare, prima e dopo trapianto di fegato l’espressione di specifici marcatori immunologici di rigetto acuto. I risultati degli studi condotti hanno evidenziato come pazienti con diagnosi di rigetto acuto alla biopsia di protocollo presentino una sopravvivenza di organo e paziente, a 1, 5 e 10 anni dal trapianto di fegato, del tutto sovrapponibile a quella di pazienti senza evidenza istologica di rigetto acuto alla biopsia di protocollo. L’insorgenza di rigetto acuto di grado moderato/severo non sottoposto a trattamento farmacologico è tuttavia associata ad aumentata incidenza di decesso o perdita dell’organo post-trapianto. Nel valutare potenziali marcatori biologici di rigetto acuto, abbiamo dimostrato che nonostante la conta eosinofilica periferica non sia sufficientemente predittiva per lo sviluppo di rigetto acuto post-trapianto, la differenza nella conta eosinofilica tra la prima e la seconda biopsia epatica può essere considerato un fattore predittivo di miglioramento istologico, indipendentemente dall’utilizzo o meno di terapia con boli steroidei. Non è stata invece evidenziata alcuna associazione tra l’alterazione degli indici di funzionalità epatica e l’insorgenza di rigetto acuto. Infine, è stato dimostrato che l’insorgenza di rigetto acuto risulta associata ad aumentata espressione di CD28 e CD38 sia sui linfociti T CD4+ che CD8+ e ad un aumento dei livelli di IL-17. Tali alterazioni del sistema immunitario potrebbero essere utilizzate nella pratica clinica per valutare lo stato di soppressione del sistema immunitario in pazienti sottoposti a trapianto di fegato con il fine ultimo di una gestione ottimale e personalizzata della terapia immunooppressiva

Twenty-six mHAgs have been identified and reported to be differently and variably correlated with GVHD or GVL, but a simultaneous method to genotype a so large panel of mHAgs has never been employed. The aim of this work has been to develop a feasible method to genotype all the 26 mHAgs described so far and to test them for their correlation with GVHD and GVL in a group of donor/recipient pairs submitted to allo-SCT. For a multi-genotyping of 23 mHAgs we used iPlex Gold Mass Array technology (3 multiplex). For the other three mHAgs we designed other three assays based on conventional molecular biology. By these methods, we tested the 26 mHAgs in 46 donor/recipient pairs full-matched that underwent allo-SCT (sibling or MUD) because of Philadelphia positive CML (n=46) or ALL-Ph+ (n=24). Maldi-Tof IPlex Gold technology proved a high degree of efficiency (98.6%). As expected, sibling pairs showed most identity of MUD pairs. Notably, donor/recipient mismatch on ACC-1, ACC-4, ACC-5, LB-MTHFD1-1Q, UGT2B17, DPH1, LRH1 can drive GvHD effect (p<0.01). Next we identified that LB-ADIR1 can enhance (p=ns, but there is a trend) GvL effect specially on ALL-Ph+ that is otherwise un-responsible to allo-immunotherapy. Our data generated by a multi-genotype technique confirm the role of mHAgs in addressing GvL (in some cases without GvHD) and suggest that a study of mHAgs could be perfomed before transplant in order to better investigate the role of the known and new mHAgs involved in GvHD and GvL effects.

Texte intégralNUOVE STRATEGIE MANIPOLATIVE EX VIVO PER IL PURGING DELLE UNITA DI CELLULE STAMINALI EMATOPOIETICHE PERIFERICHE DESTINATE AL TRAPIANTO AUTOLOGO DI MIDOLLO Questo lavoro di tesi si propone di valutare la possibilità di utilizzare il Gemtuzumab ozogamicina nel purging ex vivo delle PBSC autologhe, al fine di eliminare le possibili cellule leucemiche contaminanti. L'impiego di tale farmaco è subordinato alla verifica che la deplezione delle cellule maligne non provochi effetti citotossici sulle cellule staminali pluri/multi potenti normali, alle quali è deputato il ripopolamento stabile e duraturo del midollo osseo. Pertanto, tale verifica è stata condotta esponendo al Gemtuzumab ozogamicina cellule ematopoietiche mobilizzate nel periferico e cellule ematopoietiche del sangue cordonale e determinando il loro "rate di crescita" mediante test clonogenici su terreno semisolido. In base ai dati ottenuti è possibile concludere che: 1) il Gemtuzumab ozogamicina non inibisce significativamente l'efficienza clonale del subset primitivo CD34+/CD38- e delle cellule mononucleate normali. 2) Il GO riduce significativamente la crescita clonogenica delle cellule capostipiti granulocitiche-macrofagiche del sangue periferico. 3) La criopreservazione incrementa significativamente gli effetti tossici del GO a causa probabilmente di danni a carico della membrana cellulare, che incentivano l'endocitosi. 4) In previsione di un impiego clinico nel purging del GO, dovranno essere evitati trattamenti superiori alle 24 h. Questi dati offrono lo spunto per ulteriori future sperimentazioni atte a chiarire i fenomeni di tossicità rilevati e motivano la pianificazione di trials clinici finalizzati all'impiego del Gemtuzumab ozogamicina nel purging delle cellule staminali emopoietiche raccolte con procedure aferetiche di pazienti affetti da AML eleggibili al trapianto di midollo osseo autologo.

Texte intégralThe impact of the gut microbiota(MI) on allogeneic hematopoietic stem cell transplantation(HSCT) has been recently demonstrated. This observation supported the idea of a significant role of MI in the immunological reconstruction after HSCT and in the genesis of acute Graft-versus-Host-Disease(GvHD). We therefore conducted the first longitudinal study on the role of MI in the onset of GvHD in pediatric patients undergoing HSCT. 10 patients were enrolled, including 5 with GvHD. For each patient were collected seriated fecal samples every 10-15 days up to 100 days after HSCT. The phylogenetic profile of MI was characterized by pyrosequencing 454 of the hypervariable V4 region of the 16S rRNA subunit. The functional profile was evaluated by analysis of fatty acid-fat-to-short-chain using the gas chromatography-mass spectroscopy. After HSCT a profound structural and functional destruction of the normal position mutualistic gut ecosystem was observed. The reconstruction of the trajectory of the MI after HSCT was significantly different between patients with and without GVHD. In particular, in patients without GVHD, a precise of the MI signature before the HSCT was highlighted, characterized by a high concentration of Bacteroidetes and Parabacteoidetes(p <0.05, Fig. 1). In parallel in patients without GVHD it was observed a significant increase of the fatty-acid-to-short-chain and in particular propionate(p <0.05). This characteristic signature was projected after HSCT, persisting to the intestinal ecosystem destruction and demonstrating the high adaptability of these germs. Our data indicate that the intestinal microbial ecosystem dynamics can be a factor influencing the onset of GvHD. In particular, the presence of a mutualistic profile pre-HSCT of the MI, characterized by the presence of germs producers of acid-fat-to-short-chain recognized with immunomodulatory, seems to mitigate the risk of developing GVHD. These results therefore open new perspectives on the possibility of manipulating the MI pre-HSCT to modulate the reconstruction of the immune system.

Texte intégralThe impact of the gut microbiota(MI) on allogeneic hematopoietic stem cell transplantation(HSCT) has been recently demonstrated. This observation supported the idea of a significant role of MI in the immunological reconstruction after HSCT and in the genesis of acute Graft-versus-Host-Disease(GvHD). We therefore conducted the first longitudinal study on the role of MI in the onset of GvHD in pediatric patients undergoing HSCT. 10 patients were enrolled, including 5 with GvHD. For each patient were collected seriated fecal samples every 10-15 days up to 100 days after HSCT. The phylogenetic profile of MI was characterized by pyrosequencing 454 of the hypervariable V4 region of the 16S rRNA subunit. The functional profile was evaluated by analysis of fatty acid-fat-to-short-chain using the gas chromatography-mass spectroscopy. After HSCT a profound structural and functional destruction of the normal position mutualistic gut ecosystem was observed. The reconstruction of the trajectory of the MI after HSCT was significantly different between patients with and without GVHD. In particular, in patients without GVHD, a precise of the MI signature before the HSCT was highlighted, characterized by a high concentration of Bacteroidetes and Parabacteoidetes(p <0.05, Fig. 1). In parallel in patients without GVHD it was observed a significant increase of the fatty-acid-to-short-chain and in particular propionate(p <0.05). This characteristic signature was projected after HSCT, persisting to the intestinal ecosystem destruction and demonstrating the high adaptability of these germs. Our data indicate that the intestinal microbial ecosystem dynamics can be a factor influencing the onset of GvHD. In half the cases, human ARO is due to mutations in the Tcirg1 gene. The oc/oc mutant mouse closely recapitulates human Tcirg1-dependent ARO. In ths work we demonstrate that in utero injection of allogenic fetal liver cells on 12.5 days into oc/oc fetuses at 13.5 day post coitum completely rescue the osteopetrotic phenotype. Moreover, an embryonic stem cells line transgenic for GOF18eGFP was produced. The goal is to use the GFP under the transcriptional control of the Oct-4 promoter as a marker of pluripotency of the ESC that are to microinject into oc/oc blastocysts.

Texte intégralL’immunoterapia costituisce un’opzione terapeutica per alcuni sottogruppi di pazienti con leucemia dell’età pediatrica i quali, nonostante i notevoli progressi degli ultimi decenni, ancora non mostrano una prognosi soddisfacente perché affetti da malattia ad alto rischio oppure da ricaduta. Una comprensione più profonda di come il sistema immunitario fisiologicamente riconosce ed elimina le cellule tumorali è essenziale. Gli antigeni peptidici sono di grande interesse nel settore dell’immunoterapia perché possono essere utilizzati come vaccini per potenziare l’immunità. La proteina mutante TEL/AML1, la cui sequenza è nota, è il risultato di una traslocazione bilanciata t(12;21) che genera un gene di fusione. Dalla proteina di fusione TEL/AML1 si possono sintetizzare artificialmente peptidi, la cui capacità di legare le molecole HLA ed immunogenicità si può prevedere attraverso strumenti bioinformatici. Questo progetto ha l’obiettivo di indagare se l’eccellente prognosi dei pazienti affetti da leucemia linfoblastica di linea B con la mutazione TEL/AML1 possa essere correlata ad una risposta immunologica nei confronti di peptide di fusion derivati dalla proteina mutante TEL/AML1. A tale scopo, sono stati realizzati 8 esperimenti di priming con leucociti di donatori sani. Sei sono stati realizzati secondo un protocollo mediato da cellule dendritiche, mentre altri due esperimenti sono stati condotti secondo un protocollo mediato da beads. I linfociti responsivi al processo di priming (identificati mediante la produzione intracellulare di citochine) sono stati selezionati mediante sorting citofluorimetrico e coltivati a singola cellula in modo da ottenete cloni T-cellulari. Ciò è stato possibile in 3 esprimenti su 8. I cloni T-cellulari con evidenza di crescita sono stati testati dopo re-stimolazione con i peptidi (o mediante tetramer-staining) ma non hanno dimostrato sufficiente specificità- Abbiamo inoltre provato a dimostrare una risposta immunologica nei confronti dei peptidi di fusione nei leucociti (da sangue periferico) di pazienti con leucemia linfoblastica di linea B TEL/AML1 positiva in remissione, mediante esposizione ai peptidi e una breve stimolazione con citochine. Sono stati testati 22 pazienti, ma purtroppo non è stato possibile evidenziare una risposta nei confronti dei peptidi di fusione in nessuno di loro. Possibili spiegazioni potrebbero essere la mancanza di specificità dei marcatori di attivazione che sono stati utilizzati per identificare le cellule reattive, i gate non sufficientemente restrittivi utilizzati per il sorting, il fatto che il supertipo HLA B*07 (i peptidi B*07 ristretti avevano il migliore score predittivo) era sotto-rappresentato nella coorte di pazienti presa in esame. Ci riproponiamo di realizzare ulteriori esperimenti utilizzando nuovi marcatori di attivazione, come CD25 o PD-L1, oppure differenti tecniche per identificare le cellule reattive (come l’Elispot), di usare gates più restrittivi per il sorting e di utilizzare esclusivamente il protocollo mediato da beads per il priming. Per riuscire a includere nel campione popolazioni linfocitarie (cellule T antigene-specifiche) la cui frequenza è estremamente bassa, una possibilità potrebbe essere eseguire prelievi ematici ripetuti nel tempo nello stesso paziente. Sono necessari ulteriori studi per testare l’ipotesi di una risposta immune autologa, spontanea, nei confronti dei peptidi di fusione TEL/AML1 come spiegazione della buona prognosi della leucemia linfoblastica di linea B TEL/AML1 positiva. Un altro possibile approccio per validare i peptidi di fusione potrebbe essere quello di testarli in una situazione di HLA B*07 mismatch tra linfociti ed APCs. La ricaduta clinica potrebbe essere la generazione di cloni T-cellulari dalle cellule reattive al priming, il clonaggio del loro TCR e la sua transduzione nei linfociti del paziente o del suo donatore, in quest’ultimo caso nella prospettiva di un’immunoterapia adottiva post-trapianto di cellule staminali ematopoietiche.

Texte intégralLa Leucemia Linfoblastica Acuta (LLA) rappresenta la patologia tumorale più frequente in età pediatrica. Oltre l’80% dei bambini affetti da LLA viene trattato con successo grazie agli attuali protocolli di chemioterapia intensiva e basata sul rischio di ricaduta, ma sfortunatamente, il restante 20% ricade. Il trapianto di cellule staminali ematopoietiche (TCSE) ha un ruolo fondamentale nella guarigione di circa il 10% dei pazienti definiti ad alto rischio di ricaduta LLA in prima linea e per gran parte dei pazienti recidivati. Sfortunatamente, anche dopo il TCSE, la ricaduta si conferma come principale causa di fallimento terapeutico nelle LLA pediatriche. Il principale indicatore prognostico nelle LLA infantili è rappresentato dalla Malattia Residua Minima (MRM). La MRM è definita come la persistenza, all’interno del midollo osseo, di cellule leucemiche a livelli non identificabili attraverso esame citomorfologico. La valutazione della MRM è ormai parte integrante dei principali schemi terapeutici di prima linea, in cui viene usata per stratificare i pazienti in diverse classi di rischio di ricaduta (standard, intermedio o alto), con l'obiettivo di adattare la terapia al rischio individuale di ciascun paziente, ottimizzando le cure e riducendo al minimo la tossicità. Il monitoraggio della MRM è stato identificato come uno dei maggiori fattori predittivi di prognosi, anche per i pazienti ricaduti e per quelli trapiantati, in cui risulta ulteriormente vantaggioso per valutare la risposta alla terapia dei pazienti LLA. Ciononostante, il significato clinico della MRM nei pazienti sottoposti al TCSE non è ancora stato pienamente validato. L’approccio standard utilizzato per monitorare la MRM è attualmente rappresentato dalla real-time quantitative PCR (RQ-PCR), un saggio molecolare altamente sensibile e specifico, basato sulle regioni giunzionali dei riarrangiamenti dei geni delle immunoglobuline e del recettore dei linfociti T, identificati sugli aspirati midollari della diagnosi (o ricaduta) del paziente LLA. Nel primo progetto (descritto nel capitolo 1) del mio percorso di dottorato, abbiamo quantificato la MRM mediante PCR quantitativa immediatamente prima del TCSE, per valutare il suo significato clinico e l’impatto sull’outcome in una vasta coorte di pazienti pediatrici affetti da LLA (119), in prima remissione completa (1RC), seconda (2RC) o altre. Abbiamo poi analizzato MRM mediante RQ-PCR in 98/119 e 59/119 pazienti, rispettivamente durante il primo (post-TCSE1) e il terzo (post-TCSE3) trimestre dopo il trapianto. L’obiettivo di queste analisi è stato quello di capire se la valutazione MRM potesse fornire ulteriori informazioni, utili a identificare preventivamente i pazienti con maggior rischio di ricaduta leucemica dopo il trapianto. Dalle analisi di sopravvivenza in relazione ai livelli di MRM pre-TCSE nei pazienti LLA, qualsiasi livello di positività correla con un outcome sfavorevole (pEFS = 39% per MRM positiva < 1x10-3 e pEFS = 18% per MRM positiva ≥ 1x10-3), rispetto ai pazienti con MRM negativa (pEFS = 73%, P<0.0001). Inoltre, analizzando i pazienti in base al tipo di remissione al TCSE, livelli diversi di positività MRM correlano con un diverso impatto sulla pEFS: bassi livelli di positività MRM indicano infatti, una prognosi sfavorevole solo in pazienti trapiantati in seconda o altre RC, mentre in prima RC solo una positività alta si associa ad un aumentato rischio di ricaduta. Pertanto la MRM pre-TCSE si conferma come importante fattore predittivo di outcome e il suo effetto varia col variare del tipo di remissione al trapianto. È stata valutata, inoltre, la pEFS dei pazienti in base ai livelli di MRM post-TCSE1 e post-TCSE3; MRM negativa post-TCSE correla significativamente con un outcome favorevole, sia al 1° trimestre (pEFS = 63%), che ancor più se riscontrata al 3° trimestre (pEFS = 84%). Anche la valutazione prospettica del cambiamento di MRM è risultata significativa. In particolare, valutando la variazione di MRM dal 1° al 3° trimestre post-TCSE, i pazienti con MRM crescente hanno una prognosi sfavorevole (pEFS = 8%), mentre tutti gli altri gruppi correlano con una buona prognosi (pEFS ≥ 80%). Questi risultati confermano l’importanza del monitoraggio della MRM sia nel periodo precedente che successivo al TCSE, nell’identificare preventivamente pazienti ad alto rischio di ricaduta, possibili beneficiari di interventi immunologici preventivi. Il secondo progetto trattato (descritto nel capitolo 2) è stato uno studio preliminare, focalizzato su una PCR di terza generazione, la Droplet Digital PCR (ddPCR). Essa consente una quantifica di tipo assoluto, con un’accurata concentrazione del DNA target. La RQ-PCR fornisce, invece, una quantifica di tipo relativo, basata su una curva standard di calibrazione fatta con il DNA della diagnosi del paziente, per la quantificazione dei livelli di MRM di ciascun follow-up. Per cui, la PCR quantitativa può essere limitata dalla disponibilità di materiale diagnostico. Un ampio spettro di marcatori molecolari è già stato indagato mediante ddPCR per scopi diagnostici in varie patologie tumorali. Studi recenti hanno valutato l’applicabilità della ddPCR nell’ambito delle malattie linfoproliferative dell’adulto, come i linfomi e le LLA, mostrando una buona correlazione dei risultati fra le due metodiche in entrambi gli ambiti. Tuttavia, non sono ancora disponibili lavori che valutino questa correlazione nel campo delle leucemie pediatriche. Alla luce di questo, abbiamo eseguito analisi ddPCR sugli aspirati midollari di 65 pazienti pediatrici sottoposti a TCSE, utilizzando stessi primer e stesse sonde fluorescenti usati negli esperimenti RQ-PCR, nelle medesime condizioni di reazione. Mettendo a confronto i livelli di MRM emersi coi due approcci molecolari, si è investigata l’applicabilità della metodica assoluta per il monitoraggio della MRM anche in questo contesto. Inizialmente, sono stati valutati campioni risultati, mediante RQ-PCR, positivi ma non quantificabili (PNQ), per verificare se invece si potessero quantificare mediante ddPCR. Successivamente, è stato valutato anche l’impatto prognostico dei livelli MRM pre-TCSE ottenuti tramite ddPCR. Un buon livello di concordanza è emerso dai risultati ottenuti con entrambe le metodiche (Pearson r = 0.98, P < 0.0001); la ddPCR ha permesso, inoltre, di quantificare numerosi campioni risultati non quantificabili tramite RQ-PCR. I risultati suggeriscono che il metodo assoluto possieda sensibilità, accuratezza e riproducibilità almeno paragonabili alla PCR quantitativa convenzionale. I pazienti LLA analizzati sono stati stratificati sulla base dei livelli di MRD ottenuti con le due tecniche molecolari, ma nelle analisi di sopravvivenza non sono emerse differenze significative sulla prognosi. Infatti le pEFS dei pazienti con MRM negativa e positiva quantificabile per i due metodi risultano molto simili (rispettivamente 71% e 68%). Ciononostante, dal presente studio è emerso che col metodo digital sia stato possibile misurare un valore di MRM positivo e quantificabile per almeno 12 pazienti LLA che, in seguito al trapianto, hanno presentato una recidiva; viceversa, la RQ-PCR non era stata in grado di identificare anticipatamente la ricaduta di questi pazienti. Questi dati preliminari mostrano che la ddPCR possa essere un valido strumento per il monitoraggio della MRM e applicabile anche nel contesto dei trials clinici per pazienti LLA pediatrici. Tuttavia una prosecuzione dello studio ddPCR, con estensione della casistica analizzata, potrebbe essere utile a definire con precisione la significatività delle misurazioni con questa recente metodica.
