Across the Western world, healthcare services are contending with the challenge of ageing populations. Switzerland is no exception, and faces the need to adapt its healthcare system to the needs of older persons. A disease-oriented approach is ill suited to the varied abilities, preferences and degrees of resilience among older people, and person-centred care is better placed to respond effectively to this situation (Ekman et al., 2013). Our team at the Institute of Higher Education and Research in Healthcare (IUFRS) of the University of Lausanne has developed a research programme to improve the healthcare experiences of older persons during hospitalisation and transition to discharge. We have identified different models and theories that promote a better understanding of the factors that impact on older persons ’lives during these phases and of how to take them into account in nursing practice in order to encourage a person-centred approach. The transition of care from hospital to home is a vulnerable time in the continuum of care for older persons (Arbaje et al., 2014). Transitional care is defined by Coleman and Boult (2003, p 549) as a ‘set of actions designed to ensure the coordination and continuity of healthcare as patients transfer between different locations or different levels of care within the same location’. At the theoretical level, Meleis ’transitions theory (2000) provides a perspective for interpreting and planning comprehensive discharge for hospitalised older persons. In designing our research programme, this theory helped us to link the older person’s health problems (conditions of transition) in relation to hospitalisation (nature of transition), discharge preparation (nursing interventions) and the effects on the person (response models) (Mabire et al., 2015). From the transitions theory, Naylor et al. (2017) developed their transitional care model to guide nursing practice during this period. This model includes eight components: Patient engagement Caregiver engagement Complexity and medication management Patient education Caregiver education Patient and caregiver wellbeing Care continuity Accountability

Gait recognition technology is the key technology in the field of exoskeletons. In the current research of gait recognition technology, there is less focus on the recognition of the transition between gait patterns. This study aims to determine which kinematic parameters have significant differences in the transitions (between level and stair walking and between level and ramp walking) of different gait patterns, to determine whether these parameters change differently in different gait pattern transitions, and the order the significant differences occur through a comparative analysis of various kinematic parameters between the transition stride and the before stride in the former pattern. We analyzed 18 parameters concerning both lower limbs and trunk. We compared each time point of the transition strides to the corresponding time points of the before stride using a series of two-sample t-tests, and we then evaluated the difference between the transition stride and the before stride based upon the number of time points within the gait cycle that were statistically different. We found that the sagittal plane angular velocity and the angular acceleration of all joints and the resultant velocity of the thigh and shank of the leading limb had significant differences in the process of transition; the sagittal plane angular velocity of all joints of the trailing limb and the velocity of the trunk in the coronary axis direction also showed a significant difference. The angular acceleration of all joints, the sagittal plane angular velocity of the ankle joint of the leading limb, and the acceleration of the trunk in the coronal axis direction showed a difference in the early stage of the transition. In general, the leading limb had a significant difference earlier than the trailing limb, and the acceleration parameters changed earlier than the velocity parameters. These parameters showed different combinations of changes in the transition of different gait patterns, and the changes in these parameters reflected different gait pattern transitions. Therefore, we believe that the results of this study can provide a reference for the gait pattern transition recognition of wearable exoskeletons.

The paper explores the relationship among technological innovation, technological trajectory transition, and firms’ innovation performance. Technological innovation is studied from the perspectives of innovation novelty and innovation openness. Technological trajectory transition is categorized into creative cumulative technological trajectory transition and creative disruptive technological trajectory transition. A structural equation model is developed and tested with data collected by surveying 366 Chinese firms. The results indicate that both innovation novelty and innovation openness positively affects creative cumulative technological trajectory transition as well as creative disruptive technological trajectory transition. Innovation openness and creative disruptive technological trajectory transition both positively affect firms’ innovation performance. However, neither innovation novelty nor creative cumulative technological trajectory transition positively affects firms’ innovation performance. Implications for managers and directions for future studies are discussed.

The paper explores theoretical and methodological aspects of transition to adulthood. The concept is discussed within the framework of life course paradigm. The concept of transition to adulthood is operationalized as four dimensions: family transition, professional (career) transition, financial status transition and housing transition. We discuss in detail the process of becoming an adult in Serbian society. The thesis of specific type of transition to adulthood, named prolonged adolescence, has been fully examined. The analysis is based on data collected in a survey (2003) of young people aged 17-35. In the paper we focus on the sub sample of respondents aged 34-35 being a post-transitional cohort. The 34-35 cohort trajectory to adulthood is explored as an indicator of general trend in transition to adulthood in a period of radical social turmoil. .

Social media also experienced transitions from paper material to digital forms. In the previous years of the COVID outbreak, a particular part of social media---tourism, has been affected and is worth paying attention to. Lonely Planet (abbreviated as LP) is one of the most famous tourism media brands, previous researches have shown its development or transitions during the pandemic. Through using surveys, questionnaires and interviews, this research aims at investigating the strategy of LP for digital transition over the pandemic period, which could be applied for other media companies as an inspiration. The result from surveying LP in a chronicle order suggests negative impacts the COVID has on the business, while the transition of the company is spontaneous, also containing positive potentials for LP to get recovered or developed. Researchers receive 107 participants about their attitudes and understandings toward the transition of travel media and pandemic effects, and it shows the majority of audience focus on convenience of the journalist media, and most of them are optimistic about the digital transition of traditional media, agreeing with the statement that digital media would receive a wider audience, which indicates a benefit in digital transition. Two interviews are also carried out with loyal fans of Lonely Planet. The interviewees are used to reading printed material and are convinced that COVID affected traditional media use. However, they agree that digital platforms would not completely replace the traditional media in Lonely Planet’s case, presenting a surprising point of view.

The purpose of this research is to find out types and the dominant one of transition signals used in narrative text. This research aimed to find out the use of transitional signals in narrative text specifically to find out the types of transitional signals and the accuracy of its use. The method of this research is descriptive qualitative by doing library research. The data were collected by analyzing the transition signal used in five narrative texts. The transitional signals that were analyzed are transitional signals for adding ideas and emphasizing, showing contrast and comparison, showing cause and effect, providing examples, introducing time order or sequence, and concluding or summarizing. The results shows that the transition signals used in narrative text are the transition signal indicates time, comparison, additional idea, and contrast. The dominant transition signal used the transition signal indicates additional idea. The findings shows that the transition signal used on the narrative text is developed in a single central theme.

In ethnic social life each race has its differences due to the environment in which they live. Some common customs are found in all communities. Transition in cultural life is natural. It is human nature to set each of the transitions in relation to a ritual. Salem Chemmanur Chettiars perform biological rituals during each transition. Human life is characterized by various transitions from birth to death. Rituals and beliefs are found in every community. Putting the sugar water on the baby at birth is seen as a ritual. These rituals are performed according to the developmental stage of the child as he or she grows to a certain size, such as a hemisphere rope around the waist. Such rituals are common among the girl child and some other rituals are performed as she grows up. Women perform the flowering ritual during flowering. Booppu ritual is seen as a defilement ritual. Poop is the first menstrual period. The flower is a sign that the woman has become fertile. This event is considered as an important transformation of life. Like this, wedding ceremonies are considered to be the primary transition ceremony. Death rites are the final event of human life. Various such rituals are observed in the lives of the Chettiars of Salem Semmanur.

This paper presents a modeling and transition algorithm of a novel wall-climbing robot with biped-wheel hybrid mechanism. In order to realize robot transitions between inclined surfaces, the robot’s locomotion gait is analyzed and a locomotion gait planning model based on Finite State Machine (FSM) is established. Moreover a transition algorithm between inclined surfaces is proposed based on multi-sensors data fusions and logical reasoning networks. The results of simulations and experiments show that the model and algorithm are valid and can be applied for the wall-climbing robot’s transition between the concave surfaces.

Purpose Transitions – as a focus of study – have been missing from information behaviour research. The purpose of this paper is to explore the topic of transitions – their characteristics and influences, the related concept of liminality and Transitions Theory – and what it can contribute to the field of information behaviour. This exploration includes the application of liminality and Transitions Theory to an empirical study of participants making the transition from doctoral student to early career academic. Design/methodology/approach In addition to an extended literature review, this paper reports on a qualitative study that used constructivist grounded theory methodology for data collection and analysis. Early career academics were followed for a five- to seven-month period and data were collected using interviews and “check-ins”. Transitions Theory and liminality were used to guide the analysis. Findings Three important findings were highlighted: the complicating effects of being in a liminal space on information behaviour; the changing information needs of those undergoing a transition; and the importance of comparison as a way of using information to understand new situations. A revised model of Transitions Theory (Meleis et al., 2000) is also proposed, to incorporate information behaviour. Originality/value This paper demonstrates that by examining information behaviour over longer periods of time and by making transitions a focus of research, new understandings and insight can be gained into what information individual needs, how they find, share and use that information. This research demonstrates that information behaviour research adds important elements to the study of transitions and, conversely, that transitions (and Transitions Theory) add important elements to the study of information behaviour.

Although numerous healthcare organizations have transitioned from one electronic health record (EHR) to another or are currently planning a transition, there are few documented artifacts, such as published studies or operationalizable resources, that offer guidance on such transitions. This panel seeks to begin a conversation about human factors considerations in EHR transitions from a legacy system. Panel members will discuss current literature and research on the topic as well as experiences with and lessons learned from transitions within their organizations. Panel discussion can be expected to identify new research opportunities, needed resources, and guidance for EHR vendors or healthcare facilities in the midst of or preparing for an EHR transition. Panelists will also lay out systemic issues that need to be addressed at the national policy and regulatory level. This topic is relevant not only to full-scale EHR transitions, but also has applicability for significant EHR version changes.

Transition education should be grounded in quality research. To do so, educators need information on which practices are effective for teaching students with disabilities transition-related skills. The purpose of this systematic literature review was to identify evidence-based and research-based practices in secondary special education and transition for students with disabilities. This systematic review resulted in the identification of nine secondary transition evidence-based practices and 22 research-based practices across more than 45 different transition-related skills. The range of effects for each of the secondary transition evidence-based and research-based practices identified are also included. Limitations and implications for future research, policy, and practice are discussed.

In this paper, Wien’s displacement law as one of the most important laws of thermal radiation, for which Wien has won the Nobel Prize in Physics, is studied and derived from the perspective of phase transition by using catastrophe theory. This paper describes in detail how the control variable causes the phase transition of the system potential function and the main indexes for judging whether the phase transition occurs. By extending the control variable in the catastrophe theory affecting the thermal radiation into multi-parameters form, the function equilibrium curve of the blackbody radiation is solved, and then the two expressions of Wien’s displacement law including wavelength-temperature and wavelength-temperature are derived theoretically. Furthermore, a phase transition method is proposed to study the thermal radiation of blackbody. This not only provides a new perspective for the research of thermal radiation, but also provides a new idea for the study of other phase transitions.

Forest transition is a trend change process from decreasing to increasing forest area in a country or region. Since the 1990s, ecological and environmental problems such as climate change and loss of biodiversity have received constant attention. The research theory and method of forest transformation has gradually become the frontier and hot topic pursued by international academic circle. With forest transformation as the theme, on the basis of introducing the origin of forest transformation research, along the development vein and internal logic of forest transformation research, this paper reviews the research progress of forest transition theory from the perspectives of Kuznets curve of forest environment and forest transition path, and summarizes the major issues in forest transformation research. The main direction of future research is proposed, including the impact of economic globalization on forest transition, the refinement of research units and the analysis of forest quality transition.

The multi-level perspective (MLP) is a prominent transition framework. The MLP posits that transitions come about through interaction processes within and among three analytical levels: niches, socio-technical regimes and a socio-technical landscape. This systematic review provides an overview on the use of the MLP in research on agro-food sustainability transitions. In particular, it analyses the understanding, conceptualisation and operationalisation of niches, regimes and landscapes. Niches considered in the selected papers include agro-ecology, organic agriculture, permaculture, conservation agriculture, integrated farming, and alternative food networks. Regime refers to industrial, conventional agriculture. The researched regime is often not clearly described and its operationalisation is a matter of deliberation. Landscape level is generally overlooked; when it is considered it refers to international trends and developments. Many scholars highlight the inadequacy of transition pathways in the MLP for the agro-food sector. Moreover, transition impacts are rarely addressed and the research field generally overlooks the analysis of the sustainability of niches and, consequently, of transitions. Research on transitions in the agro-food sector borrows from the MLP its generalizability and poor empirical operationalisation of niche, regime and landscape concepts. Therefore, integrative conceptualisation and operationalisation of the MLP elements is required to accommodate the complexity of sustainability transition processes and the peculiarities of the agro-food system.

58 Background: Emerging evidence indicates that the transition of well breast cancer survivors to primary care is safe and effective. Methods: Prospective longitudinal cohort study across 14 health regions in Ontario, Canada. Each region received $100 000 (CAN) to develop and implement a sustainable new model of survivorship care for BC survivors that involved transition from medical oncology-led practice. Each region had a designated lead, and support from primary care. Funding could be used to develop any aspect of the model including personnel support, development of communication materials and outcome measurement. A minimal dataset reporting requirement included a description of the program, documentation of transitioned BC survivors as well as standardized patient and provider experience feedback once transition completed. Results: All 14 health regions in Ontario participated and all developed a survivorship care plan and patient educational materials. The models developed included direct transition to primary care in 6 regions, a nurse-led transition clinic in 4 and a GP-led transition clinic in 4. To date, 3,418 BC survivors have transitioned. Of 676 BC respondents to date, 83% felt there was a clear plan for follow up and 87% felt adequately prepared for transition. Feedback from primary care providers demonstrated that many feel informed about intent of transition and understand their role in on-going follow-up care. Process outcomes such as re-referral back to cancer center and adherence to follow-up guidelines are currently underway. Conclusions: The wide scale transition of appropriate BC survivors to a primary setting appears feasible with high acceptability by patients and providers.

Patients with end-stage kidney disease (ESKD) have different options to replace the function of their failing kidneys. The “integrated care” model considers treatment pathways rather than individual renal replacement therapy (RRT) techniques. In such a paradigm, the optimal strategy to plan and enact transitions between the different modalities is very relevant, but so far, only limited data on transitions have been published. Perspectives of patients, care-givers, and health professionals on the process of transitioning are even less well documented. Available literature suggests that poor coordination causes significant morbidity and mortality. This review briefly provides the background, development, and scope of the INTErnational Group Research Assessing Transition Effects in Dialysis (INTEGRATED) initiative. We summarize the literature on the transition between different RRT modalities. Further, we present an international research plan to quantify the epidemiology and to assess the qualitative aspects of transition between different modalities.

e19009 Background: Patients with high-risk AML often experience aggressive medical care at the end of life (EOL) such as hospitalization and intensive care unit (ICU) admission. Despite this, studies examining code status transitions in this population are lacking. Methods: We conducted a mixed methods study of 107 patients with high-risk AML enrolled in supportive care studies at Massachusetts General Hospital between 2014-2019. High-risk AML was defined as 1) new diagnosis > 60 years or 2) relapsed/refractory AML. Two physicians used consensus-driven medical record review to characterize code status transitions. Code statuses were coded as ‘full’ (confirmed or presumed), ‘restricted’ (i.e., do not resuscitate), or ‘comfort measures only’ (CMO); confirmations of presumed status were not coded as transitions. Results: At diagnosis of high-risk AML, 91.9% of patients were ‘full code’ (48.5% presumed, 43.4% confirmed) and 8.1% had restrictions on life-sustaining therapies. Overall, 55.1% (59/107) of patients experienced a code status transition, with a median of two transitions (range 1-4). Median time from first to last transition was 11 days (range 1-306) and from last transition to death was 1 day (range 0-11). Most of these transitions (79.6%; 48/59) were transitions to CMO near EOL. We identified three processes leading to code status transitions (Table): 1) pre-emptive conversations prior to any clinical change (15.3%; 9/59); 2) anticipatory conversations at the time of acute clinical deterioration (15%; 9/59); and 3) futility conversations after acute clinical deterioration, focused on withdrawing life-sustaining therapy (64.4%; 38/59). Only 55.9% (33/59) of patients participated in their last code status transition and 22.0% (13/59) of these transitions occurred in the ICU or Emergency Room. Conclusions: Most patients with high-risk AML had code status transitions at EOL, often following clinical deterioration that limited their ability to engage in EOL discussions. Interventions to promote earlier and more specific code status conversations are needed to improve patients’ ability to voice their EOL preferences.[Table: see text]

This article examines the early-and mid-career transition to research leadership in Africa. Much of the available African literature on research leadership indicate several challenges related to poor conceptualisations of career transitions and gaps in the availability of research training. Qualitative data were collected using individual interviews (n=24) and focus groups (n=27) to identify key transition points of early career researchers (ECRs) and mid-career researchers (MCRs) in selected African countries. The qualitative data was complemented with quantitative survey questionnaires (n=250) and a triangulation approach was adopted to analyse the results. The findings were themed into different categories describing the common career paths, stages and challenges of research leaders. The latter part of the findings present a discussion on development approaches to attract and retain researchers in African universities. By focusing on the African continent, this study contributes to the current body of literature on research leadership in the Global South.

Abstract Cellular plasticity and transitional phenotypes add to complexities of cancer metastasis that can be initiated by single cell epithelial to mesenchymal transition (EMT) or cooperative cell migration (CCM). Our study identifies novel regulatory cross-talks between Tcf21 and Slug in mediating phenotypic and migration plasticity in high-grade serous ovarian adenocarcinoma (HGSC). Differential expression and subcellular localization associate Tcf21, Slug with epithelial, mesenchymal phenotypes, respectively; however, gene manipulation approaches identify their association with additional intermediate phenotypic states, implying the existence of a multistep epithelial-mesenchymal transition program. Live imaging further associated distinct migratory modalities with the Tcf21/Slug status of cell systems and discerned proliferative/passive CCM, active CCM and EMT modes of migration. Tcf21–Slug balance identified across a phenotypic spectrum in HGSC cell lines, associated with microenvironment-induced transitions and the emergence of an epithelial phenotype following drug exposure. Phenotypic transitions and associated functionalities following drug exposure were affirmed to ensue from occupancy of Slug promoter E-box sequences by Tcf21. Our study effectively provides a framework for understanding the relevance of ovarian cancer plasticity as a function of two transcription factors.

Abstract During their life course, many older adults encounter a transition between care settings, for example, a permanent move into long-term residential care. This care transition is a complex and often fragmented process, which is associated with an increased risk of negative health outcomes, rehospitalisation, and even mortality. Therefore, care transitions should be avoided where possible and the process for necessary transitions should be optimised to ensure continuity of care. Transitional care is therefore a key research topic. The TRANS-SENIOR European Joint Doctorate (EJD) network builds capacity for tackling a major challenge facing European long-term care systems: the need to improve care for an increasing number of care-dependent older adults by avoiding unnecessary transitions and optimising necessary care transitions. During this symposium, four presenters from the Netherlands and Switzerland will present different aspects of transitions into long-term residential care. The first speaker presents the results of a co-creation approach in developing an intervention aimed at preventing unnecessary care transitions. The second speaker presents an overview of interventions aiming to improve a transition from home to a nursing home, highlighting the clear mismatch between theory and practice. The third speaker presents the impact of the COVID-19 pandemic on transitions into long-term residential care using an ethnographic study in a long-term residential care facility in Switzerland. The final speaker discusses the results of a recent Delphi study on key factors influencing implementing innovations in transitional care. The discussant will relate previous findings on transitional care with a U.S. perspective.

Abstract Transitional justice seeks to address legacies of violence around political transition from authoritarianism and armed conflict. It does so in ways driven by a global discourse that is prescriptive and often remote from the contexts in which it is articulated and the populations it claims to serve. Transitional justice is also embedded in teleological liberal approaches to transition, with a perceived endpoint of liberal democracy. Critical approaches to transitional justice have used qualitative methodologies to understand the agendas of those—notably victims of violence—that transitional justice foregrounds, and to demonstrate that transitional justice mechanisms often serve elite agendas, while minimizing the agency of socially excluded populations. An alternative, minimally explored route to victim engagement with such processes has been the mobilization of victims and victim organizations, an emancipatory approach that seeks to provide a space for victims to engage in transitional justice debates on their own terms. Here, a research engagement with a victims’ organization through a Participatory Action Research modality is described in which researchers support victim engagement in peer research to catalyze a social movement of victims in post-conflict Nepal.
