Background and PurposeThe World Health Organization's International Classification of Functioning, Disability and Health (ICF) is gaining recognition in physical therapy. The Extended ICF Core Set for Stroke is a practical tool that represents a selection of categories from the whole classification and can be used along with the ICF qualifier scale to describe patients’ functioning and disability following stroke. The application of the ICF qualifier scale poses the question of interrater reliability. The primary objective of this investigation was to study the agreement between physical therapists’ ratings of subjects’ functioning and disability with the Extended ICF Core Set for Stroke and with the ICF qualifier scale. Further objectives were to explore the relationships between agreement and rater confidence and between agreement and physical therapists’ areas of core competence.Subjects and MethodsA monocentric, cross-sectional reliability study was conducted. A consecutive sample of 30 subjects after stroke participated. Two physical therapists rated the subjects’ functioning in 166 ICF categories.ResultsThe interrater agreement of the 2 physical therapists was moderate across all judgments (observed agreement=51%, kappa=.41). Interrater reliability was not related to rater confidence or to the physical therapists’ areas of core competence.Discussion and ConclusionThe present study suggests potential improvements to enhance the implementation of the ICF and the Extended ICF Core Set for Stroke in practice. The results hint at the importance of the operationalization of the ICF categories and the standardization of the rating process, which might be useful in controlling for rater effects and increasing reliability.

Abstract Introduction Pediatric pilocytic astrocytomas (PA) are the most common pediatric central nervous system tumor. Surgical resection is the primary treatment for PA with five-year survival rates up to 95%. Despite a favorable prognosis, our understanding about the prognostic value of histopathological findings, such as histopathologic qualifier* or BRAF alterations is evolving. Methods Patients treated for a WHO grade 1 PA at Washington University in St. Louis/St. Louis Children’s Hospital were analyzed for clinical details, including pathology diagnosis (*histopathologic qualifier refers to designations in the diagnosis such as “WHO Grade I pilocytic astrocytoma with increased proliferative index”). BRAF alterations include gene fusions and point mutations. Results 224 patients were analyzed (51% female, mean age 9.6 years). Tumors were located in the cerebellum/fourth ventricle (50%), optic pathway/hypothalamus (15%), brainstem (12%), and cerebral cortex (11%). BRAF alterations were identified in 55/77 patients (71.4%) and additional histopathologic qualifiers were present in 27/220 patients (12.3%). 196 patients (87.5%) underwent surgical treatment and 22 (9.8%) had biopsy alone. 45 patients (22%) displayed tumor progression or recurrence after resection. The presence of a histopathologic ‘qualifier’ in the topline or BRAF alteration was not associated with tumor progression or recurrence (p=0.36, p=0.77). Ki-67 proliferative indices were not predictive of progression or recurrence (p=0.94), including when controlling for extent of resection and adjuvant therapy. BRAF alterations, specifically KIAA1549 fusions, were associated with cerebellar/fourth ventricular tumor location (p<0.001) and younger patient age (p=0.03). Extent of resection was the only predictor of outcome identified in this study; patients with gross total resection had significantly lower rates of progression and recurrence (p<0.0001). Conclusion BRAF alterations and histopathologic qualifiers were not associated with tumor progression or recurrence in pediatric PA, although BRAF fusions were more common in tumors located in the cerebellum/fourth ventricle and in younger patients.

(1) Background: Secondary acute myeloid leukemia (AML), i.e., AML arising from prior therapy (therapy-related) and/or an antecedent hematologic disorder (AHD) is generally associated with worse outcomes compared to de novo AML. However, recognizing the prognostic importance of genetic characteristics rather than clinical history, secondary AML is now considered a diagnostic qualifier rather than a separate disease entity. (2) Methods: To assess the association between clinical history and AML outcomes in the context of allogeneic hematopoietic cell transplantation (HCT), we retrospectively analyzed 759 patients with de novo AML, 115 with AHD AML, and 105 with therapy-related AML who received first allografts while in first or second remission. (3) Results: At the time of HCT, these three cohorts differed significantly regarding many patient and disease-specific characteristics, including age (p < 0.001), gender (p < 0.001), disease risk (p = 0.005), HCT-CI score (p < 0.001), blood count recovery (p = 0.003), first vs. second remission (p < 0.001), remission duration (p < 0.001), measurable residual disease (MRD; p < 0.001), and conditioning intensity (p < 0.001). Relative to patients with de novo AML, relapse rates were similar for patients with AHD (hazard ratio [HR] = 1.07, p = 0.7) and therapy-related AML (HR = 0.86, p = 0.4) after multivariable adjustment, as were relapse-free survival (HR = 1.20, p = 0.2, and HR = 0.89, p = 0.5) and overall survival (HR = 1.19, p = 0.2, and HR = 0.93, p = 0.6). Non-relapse mortality was higher for AHD AML (HR = 1.59, p = 0.047). (4) Conclusions: These data suggest that the clinical history by itself contains limited prognostic value for adults with AML undergoing allografting, supporting the most recent approach to use this information as a diagnostic qualifier rather than a disease entity.

Background: Children with central nervous dysfunction should be given a variety of long-term physical and occupational therapy interventions to facilitate their self-development and to enhance functional independence in movement, self-care, play, school activities and leisure. The present study aimed to investigate the benefit of implementing context-focused intervention approach along with occupational therapy intervention in children with CNS dysfunctions.Methods: This randomized case control study conducted in 30 children with CNS dysfunction aging between 12 months to 48 months were randomly divided into control and experimental groups consisting of 15 children in each group. Control group received conventional occupational therapy for 45 min, and experimental group received context-focused intervention approach for 30 min along with conventional occupational therapy for 15 min (thrice a week). The patients were evaluated at baseline, after 12 and 24 weeks of follow up period on Canadian occupational performance measure (COPM) to evaluate parental perception about intervention, GMFM (gross motor function measure) to study the motor ability of the patient and ICF-CY (International classification of functioning, disability and health for children and youth) for activity participation and performance with evaluation of context.Results: Significant progress in the satisfaction score during 2nd and 3rd follow ups were noted (p <0.001) in experimental group. All the activities in GMFM score except lying and rolling showed significant improvement in experimental group (p <0.001). In experimental group a significant improvement in performance qualifier of ICF was seen during follow ups (p<0.001). The capacity qualifier scoring of ICF was improved in experimental group during 2nd follow up (p = 0.006). Progress in environmental scores of ICF during all the follow ups was noted (p <0.001) in experimental group. Significant correlation in GMFM scores and COPM performance score was noted during last follow-up in both the groups.Conclusions: The findings of the study showed positive results with context focused therapy and thus can be inferred that new intervention approach context focused therapy along with the conventional occupational therapy is very effective in improving the performance of functional tasks in children with CNS dysfunction.

Study Objective: Studies have shown increased prevalence of restless legs syndrome (RLS) in sleep disordered breathing (SDB), however limited data have focused on the impact of SDB therapy on RLS. We hypothesize that positive airway pressure (PAP) will improve the International Restless Legs Syndrome (IRLS) score among SDB patients compared to patients without PAP. Methods: Patients with AHI ≥ 5 who responded positively to a RLS qualifier question from January 2010 to May 2015 were included in this retrospective study. IRLS score was used to measure RLS symptom severity. Two-sample t-tests and one-way analysis of variance were used to compare changes in IRLS score and linear regression models were created to examine IRLS change with PAP use and PAP adherence (PAP usage ≥4 h nightly for ≥70% of nights), adjusting for potential confounders. Results: In 434 patients (51.9 ± 13.4years, 50.5% female, 77.6% Caucasian; 325 PAP, 109 control), IRLS scores improved from baseline to follow-up, with the PAP group achieving significant improvement after adjustment for covariates (difference in IRLS: −1.8 (CI −3.6,0.00), p = 0.050). In self-reported PAP adherent patients, IRLS improvement was greater than controls (−5.3 ± 7.4 vs. −2.7 ± 7.6 respectively, p = 0.045), and comparable to non-adherent patients (−5.3 ± 7.4 vs. −3.0 ± 7.0, p = 0.091). Conclusions: Among SDB patients with a positive RLS qualifier, those who used PAP therapy achieved significantly greater improvement in IRLS scores than patients who did not use PAP, with more significant changes in the PAP adherent group. This is the first large clinical study to examine these relationships, providing a basis for future prospective interventional trials and informing clinicians of expected improvement in IRLS score in PAP treated SDB populations.

La situation d’apprenant induit des phénomènes que l’on pourrait qualifier d’émersifs. De très nombreuses études se sont intéressées aux corrélats neuro-végétatifs de l’apprentissage de tâches cognitives, en particulier dans le cadre des apprentissages scolaires. Il y a cependant très peu de résultats qui concernent l’apprentissage moteur des habiletés complexes. Nous avons enregistré le rythme cardiaque (HR) et la réponse électrodermale (EDR) de participants impliqués dans une tâche d’adaptation à une perturbation visuo-manuelle. Vingt et un adultes droitiers ont effectué une tâche de pointage de cibles statiques avec une tablette graphique. Après une période de pré-test, nous avons introduit une discordance inattendue en rotation entre l’espace moteur (tablette) et l’espace visuel (écran), discordance disparaissant après une période d’adaptation. Quatre participants furent exposés à une perturbation faible (5°) alors que tous les autres subirent une perturbation plus importante (60°). Nous avons trouvé des changements significatifs de HR et de l’EDR lors de l’apparition et disparition de la perturbation, mais aussi lors de la phase d’adaptation dans le groupe 60°. Nous avons en particulier mis en évidence une baisse significative de HR et une augmentation significative de l’EDR lors de l’apparition de la rotation. Ces résultats confirment l’existence d’une réponse neurovégétative notable durant l’apprentissage moteur et ouvrent la voie à l’étude des corrélats neurovégétatifs de l’apprentissage moteur dans le cadre d’habiletés plus complexes et à leur interprétation dans le cadre de l’émersion.

The poly ADP ribose polymerase olaparib is currently approved in front line BRCA-associated epithelial ovarian cancer (EOC), platinum-sensitive recurrence agnostic to BRCA status and for gBRCA as treatment in the fourth line and beyond. Women who are diagnosed with advanced stage EOC face a formidable challenge in overcoming their disease and achieving long-term, disease-free survival. The qualifier here is disease free. EOC is largely exquisitely chemosensitive, especially in the treatment naive (first line) setting and the expectation is that the vast majority of women will complete front line platinum-based chemotherapy with a response. When unselected (not selected by BRCA) women are enrolled on clinical trials, the response rate among those who have measurable disease at the time of chemotherapy initiation is 48% for carboplatin/paclitaxel and 67% for carboplatin/paclitaxel plus bevacizumab. When one considers the addition of women who start chemotherapy without measurable disease, they will likely also end chemotherapy without measurable disease and the overall rate of no evidence of disease at conclusion of chemotherapy approaches 80%. Despite this, the majority of women will suffer relapse of their disease, typically within the first 3 years following completion of therapy. Once recurrent, the disease is highly treatable for many years but no longer considered curable. This review will cover indications for olaparib in ovarian cancer as well as ongoing combination trials and rationale for these combinations.

Background: In 2022, two new acute myeloid leukemia (AML) classifications (World Health Organization, WHO and International Consensus Classification, ICC) and a new edition of the European LeukemiaNet (ELN) prognostic system have been released. These advances have been driven by the need to incorporate molecular characterization into the diagnosis and management of AML. Although important similarities in the diagnostic classifications have been recently recognized by diverse validation studies, the different approach to define AML ontogenesis (primary vs secondary) remains a central issue of the classification discrepancy. While for the WHO 2022 a prior diagnosis of myelodysplastic syndrome (MDS) or MDS/myeloproliferative neoplasm (MPN) is sufficient to define AML myelodysplasia-related (MR), the ICC no longer recognizes the AML arising from MDS (sAML) as a disease entity, and instead introduces a list of “diagnostic qualifiers”, also including therapy-related AML (tAML). Nevertheless, whether these “diagnostic qualifiers” represent a mere “descriptive” information or impact on prognosis and outcome is not well defined. Methods: We studied a cohort of 936 AML patients collected through a multicenter international collaboration across 5 academic centers. An integration of clinical and cytogenetic/molecular data was used to apply the WHO 2016, WHO 2022 and ICC diagnostic classifications and the 2017 and 2022 edition of ELN prognostic risk stratification criteria. Results: Overall,220 cases were classified as secondary AML (155 sAML and 65 tAML). As per the2016 and 2022 WHO editions, the majority of sAML cases, accounting for 98.1% and 95.5% respectively, fell into the myelodysplasia-related changes (MRC) AML and AML-MR categories due to the “disease defining” presence of an antecedent MDS or MDS/MPN. According to the ICC, 81.9% of sAML cases were defined by the presence of a MDS-related genetic profile, belonging to AML with mutated TP53 (AML- TP53) and AML with MDS-related gene mutations (AML-MDSgene) and cytogenetic abnormalities (AML-MDSk) subcategories. The tAML group showed a greater diagnostic heterogeneity, with a higher frequency of KMT2A-related categories (10.8% and 7.7% according to WHO 2022 and ICC, respectively) and AML- TP53 for the ICC (21.5%, Fig. 1A). In terms of outcomes, according to the ELN 2022 risk classification the sAML and tAML groups were classified into favorable risk 5.8% and 13.9%, intermediate 12.3% and 36.9%, and adverse 81.9% and 49.2%, respectively. Notably, while the ELN 2022 edition was effective in stratifying the group of tAML patients receiving conventional treatment (p=0.002), no statistically significant differences were noticed when sAML patients were stratified based on the same ELN 2022 risk categories ( Fig. 1B). We then compared our sAML cohort to AML cases with ICC MDS-related profile not fulfilling the anamnestic criterion of a prior MDS diagnosis (233 cases, 32.54% - AML- TP53/MDSgene/MDSk subcategories), in terms of molecular characteristics and outcome. Remarkably, both groups showed a similar prevalence of MDS genes, except for RUNX1, which was more frequent in the AML cases with the ICC MDS-related profile (14%) than in sAML (6.3%, p =0.034). Finally, both groups showed similar outcomes. Conclusions: Here, we demonstrated the overarching importance of the biological ontogenetic profile in AML (&gt;80% of sAML presented an MDS-related profile) and of the risk stratification according to the new classifications (&gt;80% of sAML belonged to adverse ELN 2022 risk). As to outcomes, 2022 ELN edition confirmed its stratification capability for tAML, while sAML were not adequately stratified. This evidence supports the idea that therapy-related AML should be acknowledged and classified as “second” neoplasms, rather than being categorized as secondary AML. Considering the overlap in AML of MDS-related profile regardless of the presence of an antecedent diagnosis of MDS as “diagnostic qualifier”, these cases could potentially benefit from treatments specifically tailored for MDS-related conditions or those typically reserved for secondary AML, such as CPX-351.

Supervisors of Community Rehabilitation Workers (CRWs) have a critical role to play in supporting and developing the skills and effectiveness of CRWs. This paper reports on research conducted by the Wits/Tintswalo Community Rehabilitation, Research and Education (CORRE) Programme amongst past and present supervisors of qualified CRWs. The aim was to understand different approaches used in supervision, the problems encountered and possible solutions, and the perceived impact of CRWs within rehabilitation services.Thirteen supervisors of qualified CRWs completed a postal or hand delivered questionnaire, which contained a range of open-ended and semi-structured questions. The findings included the supervision activities, the problems around supervision and the benefit of a CRW service. The need for community based rehabilitation (CBR) practice protocols and standards was highlighted.These findings were one of a series of steps towards the development of a CBR management manual for the Northern Province.

Background: Recognizing the prognostic importance of genetic characteristics rather than clinical history, secondary acute myeloid leukemia (AML), i.e., arising from prior cytotoxic therapy and/or an antecedent hematologic disorder (AHD), is now considered a diagnostic qualifier rather than a separate disease entity (Döhner, Blood 2022). To assess the relationship between clinical history and/or adverse cytogenetics and outcome, we report herein the molecular profile of AML patients &gt;60 years with secondary AML and/or adverse cytogenetics included in a multicentric prospective trial. Methods: Fit patients &gt;60 years with secondary AML and/or adverse cytogenetics according to the European LeukemiaNet, included in the Rev-5-Aza trial (NCT01301820), received induction therapy with lomustine 200 mg/m 2 day 1, idarubicin 8 mg/m 2 days 1-5, and cytarabine 100 mg/m 2 days 1-7. Patients in complete remission (CR) after one induction cycle received consolidation therapy with 12 cycles of alternating azacitidine 75 mg/m 2 days 1-7 and lenalidomide 10 mg days 1-21 every 28 days (Hunault-Berger M, Blood Cancer J 2017). Mutational profile was determined by targeted sequencing of complete coding regions, including splice sites, of 77 genes recurrently mutated in myeloid malignancies using custom SureSelect QXT capture system (Agilent, Santa Clara, CA, USA) on NextSeq500 platform (Illumina, San Diego, CA, USA). As a validation cohort, we determined the molecular profile of 73 patients with adverse cytogenetic risk and/or secondary AML in first CR after the same induction therapy but who received standard chemotherapy consolidation with 6 cycles of idarubicin 8 mg/m 2 day 1 and cytarabine 50 mg/m 2/12 hours days 1-5. Results: Between March 2011 and February 2013, 117 fit patients &gt;60 years included in the Rev-5-Aza trial received induction therapy with 56% reaching first CR and four patients undergoing allogeneic hematopoietic cell transplantation. The molecular profile could be established in 97 of these patients (83%; median age 70 years, interquartile range (IQR): 65-73; therapy-related AML, 32%; AHD, 46%; adverse cytogenetic risk, 70%). Secondary-type mutations were the most frequently observed (49%; ASXL1, 12%; BCOR, 16%; EZH2, 2%; RUNX1, 18%; SF3B1, 8%; SRSF2, 14%; STAG2, 10%; U2AF1, 8%; and ZRSR2, 0%) followed by TP53 mutations (44% of patients). Patients with TP53 mutations were more likely to have adverse risk cytogenetics (95%) in comparison to patients with secondary-type mutations (64%). No clinical or biological characteristic was associated with the achievement of CR (secondary-type mutations, 51%, TP53 mutation, 49%). Clinical history was not associated with overall survival (OS) as patients with therapy-related AML had the same outcome as other patients (hazard ratio [HR]=1.04, P=0.9) and patients with AHD had a more favorable outcome (HR=0.57, P=0.02). On the other hand, adverse cytogenetics was associated with decreased OS (HR=3.32, P&lt;0.001). Whereas TP53 mutations were associated with decreased OS (HR=3.71, P&lt;0.001), secondary-type mutations were not (HR=0.64, P=0.049; Figure A). After multivariable analysis, both complex karyotype (CK; HR=0.006, P=0.006) and TP53 mutations (HR=2.12, P=0.03) were independently associated with OS. Considering both CK and TP53 mutations identified three groups of patients with differential OS (median 1.57 vs. 0.91 vs. 0.45 years, P&lt;0.001; Figure B). Similar results were observed in the validation cohort. Patients in first CR from both cohorts were then compared to identify molecular subgroups for which a lenalidomide/azacitidine consolidation was associated with improved OS in comparison to standard chemotherapy. No molecular subgroup had increased OS with lenalimomide/azaciditine consolidation while standard chemotherapy was associated with improved outcomes in patients with adverse risk cytogenetics. Conclusion: Together, these data suggest that the clinical history contains limited prognostic value compared to genetics, supporting the most recent approach to use cytogenetic and molecular data to assess disease risk. In this patient population, complex karyotype and TP53 mutations, rather than secondary-type mutations, were associated with outcome. Figure. e24158 Background: Despite their potential to improve patient quality of life and survival, PRO-based symptom assessments are not routinely used in clinical cancer care. Barriers to use include cost, availability to for-profit users, and data sharing mandates as well as restrictions to any modification or translation to mobile devices. A trustworthy, open, and flexible approach to PRO-based symptom assessment may create new opportunities to reduce patient burden, increase clinical insight, and maximize measurement quality. Methods: We conducted expert interviews with oncology care staff, patients, and psychometricians to develop a flexible framework for symptom assessment that could be used to provide clinical insight and, uniquely, provide unidimensional measurements supported by item response theory. We developed a scalable framework to create symptom assessments using four interchangeable components, recall period, construct (e.g., severity, frequency, control), symptom (e.g., nausea, fatigue, pain), and an optional qualifier (e.g., at its worst, on average, in your lower back) In its first iteration, we tested 18 questions measuring presence as well as the severity, frequency, and impairment related to 9 symptoms occurring over the previous 7 days. We assessed construct validity by comparing scores to the PRO-CTCAE and assessed scalability and dimensionality using Mokken analysis. We recruited cancer patients receiving therapy in the United States to complete the nascent assessment alongside the PRO-CTCAE. Results: We recruited 10 patients and 13 clinical and psychometric domain experts for the expert interviews to develop the framework. For validation, we recruited 251 patients. Fatigue (65%) was the most common symptom and vomiting (32%) the least. All items correlated with the PRO-CTCAE equivalents (r = 0.46 - 0.72, all P < 0.05). Mokken analysis confirmed the scalability and dimensionality of all symptom scales with multiple items ( Ho = 0.59 - 0.72) as well as their constituent items ( Ho = 0.49 - 0.66). Conclusions: We created a flexible framework using exemplar PRO-based symptom assessments that can flexibly provide clinical insights and robust research measurements depending on stakeholder need. In the future, we plan to share access and encourage its use and expansion. Further work is planned to increase the number of symptoms and number of questions per symptom available within the framework.

The primary purpose of this essay is to propose the most essential skills for being a new teacher. Hence, three main sections are included in this essay. The first skill this essay would like to discuss is management skill. This section will argue two primary skills: personal and classroom management. Firstly, practical individual management skills could reduce the anxiety of entrants. Newly qualified teachers should design a work plan to prevent overpressure. Reasonable use of time is likely to enhance the satisfaction of their work. Therefore, that might reduce the school’s resignation rate, and the other management skills will emphasise students’ behaviours. Several students would like to know the last defensive line of their new teachers. Consequently, they might keep challenging the tolerance of their teachers. This is the reason why classroom management skills should be paid attention. Teachers should maintain each student’s rights so that they can have a harmonious climate in the classroom. Hence, beginning teachers should lead class management—the second section associates with the relationship between teachers and other people, including students, staff and parents. When communicating with students, teachers could apply particular methods to shorten the distance between pupils, such as appropriate humour. Furthermore, the other skilful colleagues might be regarded as beginning teachers’ teachers because of their extensive experiences. On the other hand, a harmonious relationship with parents is likely to promote children’s progress. Apart from these two skills, the third one that will be argued is the teacher’s individual teaching and development skills. Accordingly, the effective delivery of knowledge skills might decide whether or not the class is booming. Positive teaching skills should stimulate students’ critical thinking to achieve academic achievements. Moreover, beginning teachers should keep the belief that they are required to continue with professional development.

Abstract This study examined the association between physical fitness and a technical execution score in rhythmic gymnasts varying in the performance level. Forty-six young rhythmic gymnasts (age: 9.9 ±1.3 years) were divided into two groups (qualifiers, n=24 and non-qualifiers, n=22) based on the results of the National Championships. Gymnasts underwent a series of physical fitness tests and technical execution was evaluated in a routine without apparatus. There were significant differences between qualifiers and non-qualifiers in the technical execution score (p=0.01, d=1.0), shoulder flexion (p=0.01, d=0.8), straight leg raise (p=0.004, d=0.9), sideways leg extension (p=0.002, d=0.9) and body fat (p=.021, d=0.7), but no differences were found in muscular endurance and jumping performance. The technical execution score for the non-qualifiers was significantly correlated with shoulder extension (r=0.423, p<0.05), sideways leg extension (r=0.687, p<0.01), push ups (r=0.437, p<0.05) and body fat (r=0.642, p<0.01), while there was only one significant correlation with sideways leg extension (r=0.467, p<0.05) for the qualifiers. Multiple regression analysis revealed that sideways leg extension, body fat, and push ups accounted for a large part (62.9%) of the variance in the technical execution score for the non-qualifiers, while for the qualifiers, only 37.3% of the variance in the technical execution score was accounted for by sideways leg extension and spine flexibility. In conclusion, flexibility and body composition can effectively discriminate between qualifiers and non-qualifiers in youth rhythmic gymnastics. At the lower level of performance (non-qualifiers), physical fitness seems to have a greater effect on the technical execution score.

This action research study used three focus groups and one interview to explore the experiences of five recently qualified occupational therapists, four supervisors and five occupational therapy managers. The participants were asked to identify the perceived development needs of new practitioners and the mechanisms that supported or hindered these being met. The findings showed that the recently qualified occupational therapists experienced satisfaction at making a difference in their first posts. However, for some, their first post had fallen short of their expectations because they faced challenges when moving from student to practitioner, sometimes with limited support. The participants identified development needs that they felt were common to other new practitioners and also the factors that constrained or enabled the meeting of these needs. These findings informed the second phase of the action research study to design a preceptorship programme that was compliant with the revised National Health Service employment contract (Department of Health 2005) and would improve the transitional experience of occupational therapists. This paper presents the focus group results and the rationale for the preceptorship programme, which was launched as a pilot evaluation study with occupational therapy staff from over 20 organisations in Autumn 2005.

Following the introduction of preceptorship as a requirement for most newly qualified practitioners in the National Health Service in the United Kingdom, a preceptorship programme for occupational therapists was designed. This was the first of its kind within occupational therapy and was endorsed by the College of Occupational Therapists. Participation in preceptorship was intended to ease the transition of newly qualified occupational therapists. This paper offers a definition of preceptorship for the profession in the United Kingdom and reports on a small qualitative study that evaluated the extent to which the interventions of the preceptorship programme ameliorated the challenges of transition. The evaluation was conducted within a realist framework. Data were collected by semi-structured interviews from four pairs of newly qualified occupational therapists and preceptors during the pilot year. The findings suggest that the preceptorship programme was successful in supporting the development of new practitioners and that implementation was optimised through strong leadership, a learning culture and positive supervision. This paper contributes to the body of knowledge relating to the transitional experience of occupational therapists and recommends changes to the programme to optimise its effectiveness for practice.

The ability to accurately describe lung sounds were tested on 146 subjects (qualified physiotherapists from a tertiary care hospital and third, and fourth year students from three universities). The effect of increased clinical time as well as the nomenclature used was also investigated. The subjects had to recognise six tape recorded lung sounds on a multiple-choice answer sheet. It was found that in total the subjects were minimally accurate with a median score of three out of six. The relationship between increasing clinical time and increasing accuracy in determining lung sounds were not significant. Significance was set at p < 0.05. Comparing third and forth years a p value of 0.0639 was found, while a p value 0.3592 was found when comparing forth years and qualified physiotherapists. Mean scores did however seem to indicate a trend, as they increased with increasing clinical time. The “Forgacs” nomenclature was used by the majority of subjects tested.

The aim of the study was to determine the state of psychophysiological functions of skilled athletes specializing in academic rowing, in relation to the level of existing stress, emotional stability and the effectiveness of mental self-regulation and adaptability. To determine the state of psychophysiological functions, the maximum rate of movement of the hand (according to the method of tapping test) used diagnostic complex "Diagnostics-1" (MV Makarenko, VS Lyzogub). To study the strategies of coping with stress (coping strategies) and to determine the priority style of coping with stressful situations in athletes, the questionnaire "Identification of individual coping strategies" by E. Heim was used. To assess the current mental state of athletes (namely – the level of existing stress), emotional stability and effectiveness of mental self-regulation and adaptability, the test of color choice M. Luscher was used. In the studied group of athletes revealed the prevalence of respondents with a high level of stress resistance, high and medium level of emotional stability, average efficiency of mental self-regulation and adaptability. The most common coping strategies among the surveyed athletes were: in the cognitive sphere – "preservation of self-control", in the emotional – "optimism", in the behavioral – "distraction". In general, the use of adaptive stress management strategies outnumbered non-adaptive and relatively adaptive strategies among athletes. Specific psychophysiological markers of stress resistance, efficiency of mental self-regulation and adaptability of the examined athletes were indicators of strength and efficiency of sensorimotor activity. Emotional stability of the surveyed athletes was associated with the strength and functional mobility of nervous processes. The identified relationships between indicators of psychophysiological functions and psychological characteristics of athletes can have prognostic value and be used to optimize sports improvement in this sport.

The significant influence of somatic and functional characteristics on the sports result makes it advisable to justify the model indices of qualified players who are considered to be ideal models in their sport. The purpose of the study was to determine the model morpho-functional characteristics of qualified male and female volleyball play- ers of the Ukrainian Super League. The study involved 12 female and 13 male volleyball players that were qualified as masters of sport, candidates for masters of sports. Morpho-functional diagnostics aimed at determining the body composition of players (fat, muscle, bone components, total water content). According to the results of the anthropometric measurement and morpho-functional diagnostics of qualified volleyball players, we came up with 18 indicators that characterized the constitution and the body composition of athletes’ weight. The analysis of the obtained data showed a slight skewness in the distribution of fat and muscle components in the athletes’ legs and arms, which was explained by the nature of the volleyball game. The significant potential of Ukrainian volleyball players, the availability of the reserved functional resources was evidenced by their anthropometric indicators, the content of fat and muscle components in the body, which were not significantly different from the similar indicators of the players of the leading national teams and professional volleyball clubs. The results of the research can serve as a guide and facilitate the selection, training and specialization of gifted Ukrainian youth.

Background and Study Aim. The aim of the research is to investigate the efficiency of designed complexes of training exercises aimed to improve the coordination abilities of qualified athletes in fencing. Material and Methods. 22 qualified athletes specializing in fencing aged from 15 to 18 took part in the research. The study design included the sequential pedagogical experiment that was realized through the several stages. At the first stage, the preliminary testing of the athletes’ coordination abilities (n=22) was conducted. During the preliminary testing the following tests were applied: jumps to the line; shuttle run (4x9 m.); T-test, straight touch from en-garde position to the target of 10 cm in diameter from close range (10 attempts); straight lunge attack from en-garde position to the target of 10 cm (10 attempts). At the second stage of the pedagogical experiment two complexes of exercises aimed at developing the coordination abilities of the athletes were applied for 3 months. The experimental complexes of exercises were used twice a week. At the third stage, the efficiency of experimental complexes aimed at the developing of coordination abilities of qualified athletes in fencing was checked through a control testing. The Statistica 10 software created by the StatSoft Company was used for statistical processing of data. Results. It was established that during the process of coordination abilities development, the greatest attention should be given to the athletes’ ability to control timing, spatial and physical parameters of moves and moves consistency. To improve the process of developing the coordination abilities of the qualified athletes in fencing two complexes of training exercises were developed. The results of the pedagogical experiment confirmed the statistical differences in the levels of the athletes’ coordination abilities manifestation before and after using experimental training complexes (p<0.05). Conclusions. It has been shown that specially developed training complexes could be used during the training process in order to develop and increase the level of coordination abilities of qualified athletes in fencing. The analysis of experts’ opinion showed that the development of coordination abilities in fencing should be oriented towards the development of athletes’ ability to control timing, spatial and physical parameters of movements.

The biomechanical indicators of a direct blow in boxing were recorded using the Kinovea program, version 0.8.15, and psychophysiological indicators using the Psychodiagnostics program. As biomechanical indicators, the speed of movement of various points of the limbs and the angles in the joints were determined. As psychophysiological indicators, the speed of a simple visual-motor reaction, a reaction of choice in various testing modes was recorded. Based on these indicators, the mobility and endurance of the nervous system were determined. The reliability of differences between groups of different fighting styles was determined. Results. Tempo-style boxers differ from representatives of other fighting styles in their lower mobility of the nervous system and greater endurance for speedy and precise actions. Tempo-style boxers have the lowest values ​​of the angle in the knee joint throughout the movement (p<0.05, p<0.01) and the highest speed of movement. Play style boxers are characterized by the greatest angle in the shoulder joint at the very beginning of the movement (p<0.05, p<0.001), the highest speed of fist and shoulder movement at the very beginning of the movement (p<0.001). Power style boxers develop maximum movement speed at the end of a straight punch compared to the beginning. Power style boxers are distinguished by the smallest values ​​of the angle in the elbow joint, which is especially noticeable at the end of the movement (p<0.001). Conclusions. Psychophysiological features of boxers of different styles of fighting are reflected in the features of the technique of a direct blow. The lack of speed when tempo-style boxers are engaged in movement is supplemented and compensated by the high speed of movement. Play style boxers are characterized by high speed of movement at the very beginning of punching. Power style boxers are distinguished by the gradual development of movement speed.

Introduction: Each development stage of the healthcare system and medicine is associated not only with the emergence of new integrated areas of knowledge, but also with radical changes in the technology of the doctor’s work with the patient, algorithms, methods for collecting, processing information, and making decisions. Also, many postulates need revision, activation of existing reserves, certain concepts also require a new interpretation. Objective: To form a high-quality medical model for the rehabilitation of highly qualified athletes. Material and method: The paper proposes a model for constructing a quality assessment of rehabilitation of athletes based on probabilistic methods. The novelty of the study underlies the transition from a qualitative to a quantitative assessment of health, which has become a new direction in the assessment and management of health. Results: The authors present a model of a qualitative increase in the managerial process by the rehabilitation of highly qualified athletes. The paper proves that it is necessary to determine the ability to measure and express the basic properties of any organism in conditionally qualitative proportions – the ability to withstand various stressful effects and adverse environmental influences. Conclusion: The practical significance of the study is determined by the fact that it is necessary to search for evaluative health criteria, and in the individual’s ability to carry out biological and social functions. The perfection of these functions in humans can also be described quantitatively, by reserves of energy, plastic, and regulatory support of functions.

Objective: The aim of this study is to determine inter-rater, test–retest and intra-rater reproducibility and responsiveness of subjective assessment of upper limb associated reactions in people with acquired brain injury using (1) the ‘Qualifiers Scale’ of the International Classification of Functioning, Disability and Health Framework, and (2) visually estimated elbow flexion angle during walking. Design: Observational study. Setting: A brain injury rehabilitation centre, Melbourne, Australia. Subjects: People with acquired brain injury and upper limb associated reactions and experienced neurological physiotherapists. Main measures: The Qualifiers Scale applied to individual upper limb joints and global associated reaction on a 5-point scale (0–4), a summed upper limb severity score and visually estimated elbow flexion angle. Results: A total of 42 people with acquired brain injury (mean age: 48.4 ± 16.5 years) were videoed walking at self-selected and fast speeds. A subset of 30 chronic brain injury participants (mean time post injury: 8.2 ± 9.3 years) were reassessed one week later for retest reproducibility. Three experienced neurological physiotherapists (mean experience: 22.7 ± 9.1 years) viewed these videos and subjectively rated the upper limb associated reactions. Strong-to-very strong test–retest, intra- and inter-rater reproducibility was found for elbow flexion angle (ICC > 0.86) and the Qualifiers Scale applied to global and individual upper limb joints (ICC > 0.60). Responsiveness of change from self-selected to fast walking speed (mean increase 0.46 m/s) was highest for elbow flexion angle (effect size = 0.83) and low-to-moderate for the Qualifiers Scale. Conclusion: Subjectively rated associated reactions during walking demonstrated strong reproducibility and moderate responsiveness to speed change. The Qualifiers Scale and elbow flexion angle can both subjectively quantify associated reactions during walking in a clinical setting.

The purpose of the study is: 1) development and unification of a system of training loads monitoring by the nature of the physiological impact on the basketball players’ body; 2) study of the strategy of preparing a highly qualified basketball team for important international competitions, namely: to find out the nature of the distribution of means of different physiological, as well as technical-tactical and game orientation at different stages of preparation; 3) test basketball players before and after training. Materials and methods. A team of highly qualified basketball players (masters of sports and masters of sports of international class) in the number of 15 people participated in the research. The assessment of physical load by the nature of the physiological impact on the body of basketball players during 18 microcycles using 4 – 1 system, that is, 4 working days and 1 day off, was carried out. Athletes were tested before and after training. Results. The dynamics of the means of training by the nature of the physiological impact on the athletes’ body have been established. The nature of the distribution of means at different stages of preparation has been established. The test results before and after training are shown. Conclusions. The research results showed that a certain accounting system based on assessments of the nature of the tasks to be solved and the degree of compliance with the specifics of their game activities has developed in basketball. At the same time, research have indicated the need for accounting and analyzing of the training loads by the nature of physiological impact on the body of athletes.

AbstractThis study examined whether anthropometric and fitness tests might successfully predict selection of young female volleyball players for a junior national team. Sixty four female players (age: 14.4 ± 0.5 y, body height: 1.76 ± 0.05 m, body mass: 63.9 ± 6.4 kg) underwent a selection procedure for the junior national team. Anthropometric data and speed and power test results were obtained and players were graded for their performance in a volleyball tournament. Selected players differed from the non-qualified in body height (3.4%; p = 0.001), standing reach height (2.6%; p = 0.001), the sum of skinfolds (15.4%; p = 0.035), body mass index (BMI; 7.1%; p = 0.005) and spike jump and reach (SJR) (2.5%; p = 0.001). Selected players were classified in the 99.2 ± 1.6 percentile in body height and in the 51.4 ± 20.6 percentile in the BMI, which were significantly different from those of the non-qualified players (95.4 ± 7.0 and 66.7 ± 18.6, p = 0.02 and p = 0.004, respectively). Stepwise discriminant analysis yielded a discriminant function (p < 0.001, η2 = 0.78) that was highly loaded by height, SJR and the BMI (r = 0.79, r = 0.74 and r = -0.53, respectively). Cross validation results showed that selection was correctly predicted in 15 out of the 20 selected players (predictive accuracy: 75.0%) and in 35 out of the 44 non-qualified players (predictive accuracy: 79.5%). In conclusion, body height, the BMI and SJR height successfully discriminated between selected and non-qualified elite young female junior national team volleyball players. The equal vertical jump, sprint and agility of selected and non-qualified players, highlight the importance of body height and the BMI for selection of elite junior female volleyball players.

The purpose of the article is to identify the psychological aspects of the training of highly qualified athletes in Kazakh Kuresi wrestling. The leading methods for the study are the Minnesota Multiphasic Personality Inventory (MMPI) for an in-depth examination of individual characteristics of psychological functioning of personality and the diagnostic method for personal motivation to success according to the questionnaire T. Ehlers to identify the psychological skills of striving for victory in athletes. The article presents the psychological and aspects of the training of highly qualified athletes in Kazakh Kuresi wrestling. The identified aspects provide the full-fledged psychological support to highly qualified Kazakh Kuresi wrestling athletes at all levels of training and during competitions to form a correct psychological attitude with a stable individual state to many pre-competitive and competitive factors. Findings highlight the importance of comprehensive psychological support in enhancing sports performance and maintaining athletes' psychological and physical health. This research offers practical insights for high-performance sports, especially in the training of elite Kazakh Kuresi wrestlers. This paper presents data that document the need for additional leadership personnel in adapted physical education. A systematic analysis of the Chronicle of Higher Education, Dissertation Abstracts International, and the Physical Education Gold Book reveals that there is currently a discrepancy between the number of advertised positions in higher education and the number of available personnel to fill these positions. The delivery of appropriate personnel preparation programs in the area of adapted physical education is dependent upon the availability of well trained and qualified personnel. Observations are made on the type of training needed and recommendations for ensuring the availability of a qualified pool of applicants.
