Texte intégralBackground: Subjects with mental disorders have a higher risk of suicide behaviors than the general population. So, to date, researchers have investigated some biomarkers possibly related to suicidality. Despite many studies have reported a possible relationship between low lipid serum levels and suicide attempt, conflicting results have emerged. Aim We investigated whether serum total cholesterol, LDL cholesterol and triglycerides are associated with recent suicide attempts in subjects with different mental disorders. Methods We conducted a cross-sectional study, including 593 consecutively admitted inpatients with schizophrenia spectrum, bipolar, major depressive, and personality disorders. Serum lipid levels were compared between subjects admitted for a recent suicide attempt and those without such recent history. Moreover, according to hypothesis that links impulsivity and violence with low serum lipid levels, the association between lipid levels and violent suicide attempt was assessed. Results We did not find any association of total and LDL cholesterol and triglycerides with suicide attempts, also considering diagnosis and suicide methods. In addition, a post-hoc analysis showed a trend toward significance (p=0.06) in the association between high cholesterol level (≥160mg/dL) and recent suicide attempt. Conclusions Our results do not support the hypothesis of association between lipid profiles and suicide attempts in subjects with different mental disorders. Further research is needed to clarify the role of biomarkers in suicidal behaviors.

Texte intégralIntroduction: Suicidal ideation and suicidal attempts are usually complications of psychiatric disorders. The study of the frequency and associated factors may contribute for the mental health professionals to have a more appropriate approach. Aims: Estimate the prevalence of attempts and suicidal thoughts, in people who have serious mental disorder (SMD) and the association with social-demographic characteristics, diagnosis, social adjustment, psychiatric symptoms, use of psychoactive substances, family involvement and treatments they have gone though. Method: One hundred ninety two subjects, age 18 - 65, showing SMD and having been in touch with mental health services in three defined zones in the city of Sao Paulo, in the determined period, were evaluated and interviewed. The researchers used structured anamnesis with standardized questionnaires and psychometric scales, the subjects were interviewed at home and a descriptive analysis, a univariate analysis and a multivariate analysis were used. Results: From the sample we studied weve found a predominance of men (53,1%), single people (57,3%), having a diagnose of schizophrenia (58,3%). Few people presented abuse or dependence of alcohol (7,3%) and only three people (1,6%) hadnt used psychiatric medications within one year before the interview. One hundred nine (57,7%) people who where interviewed showed suicidal thoughts during their lives and during 12 months prior to the research, thirty eight (20,1%) had that kind of thoughts. Fifty nine (30,9%) people had tried to commit suicide during their lives and eight (4,2%) showed attempts to it within 12 months before the interview. The presence of guilt feelings and depressive symptoms showed statistics association to all dependant variables. The multivariate analyses showed statistics association to attempts and suicidal thoughts during the patients lives to the occasional use of neuroleptics. Conclusion: The presence of suicidal ideation and attempts to suicide were prevalent in the studied population and they should always be investigated by the health mental professionals in order for them manage the prosper procedures, such as the handling of guilt feelings and depression and be able to stablish the proper medication, for the mental disorder.

Texte intégralThe role of religiosity in determining suicidal behavior is controversial, since there are tudies where its considered a protective factor and others, a risk factor. In this study, the authors conducted a systematic literature review to assess the relationship between religiosity and suicidal behavior. Among the 154 articles published in scientific journals initially identified in Medline, Lilacs, SciELO and PsycInfo, we selected 59 articles that focused on the association between religiosity and suicidal behavior. To evaluate the qualitative attributes of the articles, a Qualitative Evaluation Script was developed. The results show that most articles found presented a lack of methodological rigor in measuring the concept of religiosity, possibly due to the subjective characteristic of this construct. However, it was found that the protective role against suicidal behavior exercised by religion, varies according to the culture in which it operates, whereas in some cultures suicidal behavior is not seen with total disapproval. However, most studies support the hypothesis that religiosity reduces the risk of suicidal behavior in individuals who profess some kind of belief, and participates in some religious space. Were not found, in this research, studies which measure the association between religiosity and suicidal behavior in religions of African origin.

Suicide is a phenomenon present in all stages of life, however, the World Health Organization points to a significant increase in suicide rates among teenagers. Adolescence can be understood as a cultural phenomenon that marks the transition of roles and responsibilities from childhood to adulthood. The literature reports that the typical events of adolescence may contribute to the occurrence of suicide at this time of life. The purpose of this dissertation is to describe aspects related to suicide rates, and identify the incidence of ideations and suicide attempts among adolescents living in the city of Goiânia. For this purpose, three chapters will be presented organized in article format. The first chapter deals with a systematic review of the literature on suicide in adolescence. In this study, we analyzed 9 articles, 2 theses and 2 dissertations. It could be observed that the list of factors associated with suicide is extensive. The second chapter aims to describe the suicide rates of teenagers living in Goiânia from 2003 to 2013, and to investigate associated factors. For this purpose, the data of 10 to 19 yearold subjects provided by the Mortality Information System were analyzed - SIM, part of Ministry of Health responsible for the provision of information on mortality in Brazil. The results indicate a higher incidence of suicide among adolescents between 15-19 years old. However, it emphasizes a trend of increasing suicide among adolescent males between 10-14 years old. The third chapter aims to carry out an assessment of the prevalence of suicidal ideation and attempted suicide among teenagers in the city of Goiania in 2013. Also in this chapter, we evaluate the relationship between ideation and attempted suicide with self-reported problems behaviors through the Youth Self Report - YSR. The results indicate an association between ideation and suicide attempt with all behavioral problems. We also found that attempted suicide rates are higher than the rates of suicidal ideation among adolescents in this county. It is emphasized the importance of studies that understand the variables involved in suicidal behavior of adolescents, since it is the first step for the implementation of contention measures for this phenomenon.
O suicídio é um fenômeno presente em todas as etapas da vida, entretanto, a Organização Mundial de Saúde chama a atenção para um aumento significativo nas taxas de suicídio entre os adolescentes. A adolescência pode ser compreendida como um fenômeno cultural que marca a troca de papéis e responsabilidades da infância para os papéis e responsabilidades típicos da vida adulta. A literatura relata que os acontecimentos típicos da adolescência podem contribuir para a ocorrência do suicídio neste momento da vida. A proposta desta dissertação é descrever aspectos relacionados às taxas de suicídio, bem como identificar a incidência de ideações e tentativas de suicídio entre adolescentes residentes na cidade de Goiânia. Para tanto, serão apresentados três capítulos organizados no formato de artigo. O primeiro capítulo trata de uma revisão sistemática da literatura sobre o suicídio na adolescência. Neste estudo, foram analisados 9 artigos, 2 teses e 2 dissertações. Foi possível observar que é vasta a lista de fatores associados ao suicídio. O segundo capítulo tem por objetivo descrever as taxas de suicídio de adolescentes residentes na cidade de Goiânia durante o período de 2003 a 2013, bem como investigar características associadas. Para tanto, foram analisados os dados sobre suicídio de sujeitos de 10 a 19 anos disponibilizados pela Sistema de Informações sobre Mortalidade - SIM, órgão do Ministério da Saúde responsável pela disponibilização das informações sobre mortalidade no Brasil. Os resultados indicam uma maior incidência do suicídio entre os adolescentes de 15 a 19 anos. Entretanto, ressalta-se uma tendência de aumento do suicídio entre adolescentes do sexo masculino de 10 a 14 anos. O terceiro capítulo tem por objetivo realizar uma avaliação da prevalência de ideação e tentativa de suicídio entre os adolescentes da cidade de Goiânia no ano de 2013. Ainda neste capítulo, avalia-se a relação entre ideação e tentativa de suicídio com o autorrelato de problemas de comportamentos por meio do Youth Self Report - YSR. Os resultados indicam uma associação entre ideação e tentativa de suicídio com todos os problemas de comportamento. Encontraram-se também taxas de tentativa de suicídio superiores às taxas de ideação suicida entre adolescentes deste município. Ressalta-se a importância de estudos que compreendam as variáveis envolvidas no comportamento suicida de adolescentes, uma vez que este é o primeiro passo para que medidas de contenção deste fenômeno sejam implementadas.

Texte intégralThe scientific literature on suicidal now displays more than a hundred of risk factors, at least five major theoretical current (themselves divided into various models) and many nosological debate regarding the existence of prodromal suicidal symptoms. The objective of this research is to trace the genesis of the suicidal process until the final patient's mental state at the time of the suicidal movement. 33 suicidal subjects were invited to complete a scale of dissociation (DES), the Rorschach Comprehensive System and a semi-structured (MEPS) to enable them to mentally go back to the the suicide attempts. The results show in most of them a preparatory phase, pathological elements of personality, a pre-suicidal phase on the day of the attempted suicide and three psychological profiles at the time of the attempted suicide. Combining some elements of personality increases the frequency of suicide attempts (emotional profiles and idiopathic), while the severity of the action and its potentially lethal depends on the type of suicide attempt performed. In addition, attempts suicide called "kinesthetic" appear very dangerous. The clinical implications, theoretical and methodological treatment are discussed

Texte intégralThis sequence starts with the decision to commit suicide, the choice of method, followed by the act itself; it is characterized by an interruption that triggers different forms of intervention: physical treatment in a hospital and psychiatric evaluation; it continues with the return of the individual to his/her familiar circle, facing the life conditions he/she had wanted to leave.
Reconstructing this experience emphasizes the solitary, the physical and the uncertain nature of the suicidal act. It also shows how individuals who do not complete their suicide will be caught in disconcerting and compromising situations, and that their act will force them to justify themselves to different audiences and will taint their relationships with others. Finally, the analysis indicates that the terms in which individuals envisage suicide are transformed in the course of their experience.

As tentativas e os suicídios têm se tornado um problema de saúde pública, sendo o autoenvenenamento um dos métodos mais utilizados de atentar contra a própria vida. Este estudo foi realizado em pacientes que tentaram suicídio por envenenamento (TSE) e foram atendidos na Restauração, o maior hospital público de Pernambuco. Objetivo: descrever o perfil sociodemográfico; identificar os principais agentes utilizados; apontar alguns estressores associados à TSE e analisar as diferenças de gênero. Método: o estudo quantitativo, descritivo, analítico, do tipo corte transversal, que avaliou 110 pacientes, com idades entre 14 e 78 anos, atendidos com TSE, no período de janeiro a agosto de 2009. Os instrumentos de avaliação utilizados foram a entrevista padronizada Mini International Neuropsychiatric Interview Brazilian, versão 5.0.0 (MINI), e formulário de entrevista especialmente desenvolvido para o estudo. Resultados: do total, 70% eram mulheres; idade média de 28,8 anos; 58,2% entre 14 e 28 anos; 63,6% encontravam-se sem convívio conjugal; 54,5% estavam desempregados; 85,5% sem profissão; 75,5% declararam-se dependentes de terceiros financeiramente; 67,3% admitiram ter uma crença religiosa; 98,2% com pelo menos um transtorno psiquiátrico. Dentre os estressores, 71,8% dos entrevistados atribuíram como a causa para cometer TSE os conflitos interpessoais (sendo 47,3% afetivos e 24,5% familiares do 1o grau); 80% relataram ideação suicida; 30,9%, TS anterior; 52,7% admitiram ter planejado o ato; 90,9% relataram doença mental na família; 62,7% alegaram fatos traumáticos ao longo da vida; 26,4%, abuso sexual e 17,3% ocorridos na infância; 61,8% declararam estar aliviados por não terem morrido. Em relação a outras variáveis, ressaltam-se 98,2% com um ou mais Transtornos Psiquiátricos; 71,8% falam do Transtorno Depressivo; 28,2% com uso nocivo de bebidas alcoólicas; 27,8% com TAG; 64,5% com comorbidade psiquiátricas; 90,9% com a doença mental na família, e 20% com tentativa de suicídio entre os familiares. Dos agentes declarados, 50,9% foram por ingesta de medicamentos; 45,5% agrotóxicos e 33,6% declararam estar sob efeito de bebidas alcoólicas no momento do envenenamento. Na comparação de gênero, a idade variou de DP=8,4 para os homens e DP=11,55 para as mulheres. As diferenças significativas foram: abuso sexual na infância, sendo 22,1% das mulheres e 6,1% dos homens; transtornos decorrentes de bebidas alcoólicas, 42,4% dos homens e 22,1% das mulheres. Admitiram estar sob efeito de bebidas alcoólicas 51,5% dos homens e 26% das mulheres. Conclusões: os resultados confirmaram outros estudos da literatura: idade média de 28,8 anos; quase três vezes mais mulheres; vida marcada por conflitos afetivo-familiares; precárias condições de autogestão (instrução deficiente, sem profissão, desemprego), por alto índice de transtornos mentais (destacando-se depressivos e alcoolismo) e suporte familiar e social precários. Restou evidente o impacto dos problemas presentes como fortes estressores. Esses dados apontam para a necessidade de planejamento de programas preventivos mais efetivos, em especial para a população mais vulnerável

Texte intégralFrom a casuistry study based on a sample of dozen subjects, this work will try to elucidate the psychological motivation of the repeated settlement into a process of suicidal act, from teenage years to early adulthood. Its purpose is to challenge three major theories on the ground of a clinical and projective methodology. The first one assumes the trans-structural aspect of this symptomatic behaviour. The second one makes a connection with the loss treatment from which can be inferred-that is the third hypothesis- failures of the fantastical development, leading to the suicidal act. We assume such failures could both result, on the one hand, a traumatic coalescence between internal and external and, on the other hand, from the inhibition of the intrapsychic conflictual aspect, particularly concerning aggressivity. We would therefore be facing two separate process of suicidal repetition. First, a reactionnal one, due to its dependence on the repetition of external events, and then a second one, rooted in compulsion

Texte intégralThe World Health Organization confirms suicide as a problem of public health. It is estimated that for each suicide there are at least ten attempts enough to demand the professionals' attention. Purpose: describe the attempted suicides in adolescents in order to analyze those cases repercussion in both family context and professional practical; identify the social, cultural and family aspects that act as facilitators for the suicides attempts and also to suggest an education approach in health in order to fight the problem. The recipients of this study were 12 adolescents admitted at an emergency Hospital, in Fortaleza, Ceará, Brazil, which had as cause the attempted suicide for any mechanism. It was conducted in the qualitative approach and the ethnography, as method. Results: it was found, as main reason, the love no corresponded (the one with affectionate connotation, friendship and the first relationship). However, it was not the only way that the "love no corresponded" was mentioned. It was also mentioned the family relationship, the affection, the respect in the family and the adolescent's valorization (their beauty aspects). It was ended that the suicide appear when we are teenager. It is necessary to consolidate the philosophical and operational beginnings of SUS (Brazilian Health System) and also that involvment of society in order to find ways to better off the life aspects.
A Organização Mundial da Saúde confirma o suicídio como um problema de saúde pública. Estima-se que, para cada suicídio, existem pelo menos dez tentativas suficientemente sérias a ponto de exigir atenção dos profissionais. Objetivos: descrever as tentativas de suicídio em adolescentes, analisando a repercussão desses casos no contexto familiar e prática profissional; identificar os fatores sociais, culturais e familiares que agem como facilitadores para as tentativas suicidas; e propor abordagem de educação em saúde para o enfrentamento do problema. Os participantes do estudo foram 12 adolescentes, admitidos em um Hospital de emergência, em Fortaleza, Ceará, Brasil, tendo como causa a tentativa de suicídio, por qualquer mecanismo. Trilhou na abordagem qualitativa e na etnografia, como método. Resultados: descortinou-se, como razão primaz, o amor não correspondido, tendo esse amor a conotação do afetivo, do namoro, do caso, da primeira entrega; contudo, não foi só nesse sentido que o amor não correspondido foi mencionado. Este também foi aludido quanto ao relacionamento familiar, ao carinho, ao respeito da família, à valorização da pessoa do adolescente, do seu físico e da sua estética. Concluiu-se que o suicídio está presente na fase da adolescência, que é preciso consolidar os princípios filosóficos e operacionais do Sistema Único de Saúde (SUS) e que a sociedade abrace as políticas que valorizam a vida.

Texte intégralIntroduction: The serotonergic route has been intensively studied regarding the involvement in several psychiatric disorders including major depression and suicide behavior. The serotonin transporter gene (5-HTT) is one of the most studied candidate genes for suicide behavior. A polymorphism corresponding to an insertion/deletion of 44 base pairs in the promoter region of this gene (5 - HTTLPR), originates two alleles (l- long e s-short). Several studies have demonstrated an evidence for the association among this polymorphism (specially the s variant) and suicide behavior. New allelic variants within the 5-HTTLPR polymorphism were described which generates a multiallelic functioning in the regulatory region of the 5- HTT gene. The analysis of an imbedded SNP (Single Nucleotide Polymorphism) was rarely studied in association with suicide behavior and the vast majority of publications involving this polymorphism analyzed only the biallelic form. Thus, we decided to perform study 1, which allowed a multiallelic analysis of this polymorphism in depressed patients who attempted suicide. The enzyme catechol - o - methyltransferase (COMT) has been shown in a few studies as being associated to such behavior. A polymorphism (rs165388) in the gene that codes for the enzyme produces a substitution of valine for methionine at codon 158 (COMT Val158Met) which results in the trimodal distribution ([AA (Met/Met), AG (Val/Met) e GG (Val/Val)] of the COMT activity. This is one of the most studied polymorphisms in psychotic and schizophrenic patients, although with few evidences of positive association. Because this polymorphism has been poorly studied in non psychotic patients with suicide behavior, we decided to investigate a possible participation of such polymorphism in our sample (study 2). Methods: These are case-control candidate gene studies for suicide behavior. Subjects were cautiously paired for sex and age and all were Caucasians. The sample of the first study consisted of 94 patients and the control group was a convenience sample comprising 94 employees of Hospital de Clínicas de Porto Alegre (HCPA) without psychiatric disorders. The second study comprised the same sample with 176 subjects also paired for sex and age (88 cases and 88 controls). Cases and controls were evaluated by a Psychiatric Interview and the Mini International Neuropsychiatry Interview (MINI). A semi-structured interview was used to access sociodemographic data and clinical history. The Beck’s Suicidal Intention Scale (SIS) was used for the patients interviewed by the psychiatrist. The region of the 5-HTT gene containing the biallelic polymorphism and the region of the COMT gene containing the Val/Met polymorphism were amplified under Polymerase Chain Reaction. The amplified products of 5-HTT polymorphism were determined by restriction fragment length polymorphism (RFLP) analysis. The digestion products were visualized by 3% agarose gel electrophoresis stained with ethidium bromide under UV light. Results: In our first study, we did not find an association with the new allelic variants of 5-HTTLPR among depressed patients with suicide attempts. Also we did not find an association with the rs165388 polymorphism (COMT) with suicide attempts in depressed patients. Conclusion: Even with a negative result, this work allowed, for the first time, the new allelic variants of 5-HTT and analyze the COMT Val158Met polymorphism with suicide attempts in depressed patients. We suggest more studies in order to understand the role of these polymorphisms in suicide behavior.

Texte intégralThe subject of this doctoral research is the suicide attempt. It is based on two observations; the first, which is clinical, is the variability in certain characteristics of the suicide attempt, such as the depressive affects, the intention and the lethality. The second, which is more theoretical, concerns psychological makeup, taking into account how masochism is assimilated at a structural and dynamic level. In accordance with a psychopathological approach, we have formulated the hypothesis that in suicidal subjects, the suicidal dynamic varies as a function of the psychological organization, and more specifically, as a function of the mode of assimilation of masochism. Our research population consisted of 100 subjects (50 suicidal subjects and 50 control subjects). As our research involves different variables at different levels of analysis, we have chosen to use seven complementary and compatible methodological tools. The latter have enabled us to address the psychological organization of the subjects and the suicide attempt, both from a quantitative and qualitative point of view. Given the results obtained, we were able to validate, within the limits of the statistical tests carried out and the size of the samples studied, our explanatory model of the variability of the suicidal dynamic based on the modes of assimilation of masochism

The rise of the phenomenon and poverty research on clinical and psychopathologic plan we gave rise to better understand the mechanisms of attempted suicide and query prevention systems and decision-making support. The results highlight the suicidal gesture during adolescence is rooted in a narcissistic problematic in the threat of others and the failure of enlarged faimly containment function. Texte intégralIt is important to investigate the attendance of people with suicidal behavior, since suicide attempt is the main predictor of future suicide death. There is a lack of studies that analyze on the subject in Brazil. This study aimed to investigate the attendances related to suicidal behavior in 2014, in an emergency mobile service and associated factors. Quantitative study, transversal, documentary. A manual consultation of nursing care records generated by Basic Support Units in which they were documented called suicidal behavior in the year 2014. The data were recorded in scripts prepared by the researchers and analyzed by means of descriptive statistics, chi-square test , Fisher\'s exact test, correlation tests, and mean comparison tests. The study was approved by the Research Ethics Committee. In the 313 files analyzed, there was a predominance of adult women, who attempted to commit suicide due to drug intoxication, at home and were referred to a pre-hospital emergency room service. Most signs had no documented signs, symptoms, or complaints. The interventions most performed by nursing were related to the monitoring of clinical parameters. There were differences related to the sex of the victim and lethality of the suicidal behavior, suicide attempt method, referral to emergency services and half of the occurrence. Attempts to commit suicide due to intoxication or self-inflicted injuries differed in relation to the time of the call, waiting time for care, lethality, documentation of clinical evaluation and interventions, referral to emergency services. This study allowed the best mapping of the demands related to suicidal behavior, knowledge relevant to the planning of nursing care and understanding of the skills required for such care

O caminho para a compreensão de qualquer tentativa de suicídio é surpreendentemente complexa e requer uma análise dos mais diversos tipos de variáveis. Atualmente se reconhece que pessoas que fazem tentativas de autoextermínio (TAE) possuem diferentes perfis associados a diferentes categorias de TAE. Pacientes que fazem tentativa de suicídio clinicamente grave (TSCG) são os mais prováveis de apresentar o perfil de pessoas que cometem suicídio. Como meio de ressaltar o Método de Rorschach (MR) como uma técnica para compreensão dessa realidade, esse estudo apresenta a análise de quatro casos de TSCG. Visa investigar a estrutura e dinâmica de personalidade que apontam para indicadores de vulnerabilidades e configuração psicopatológica, úteis para compreensão do potencial suicida. Os resultados apontaram para limitações graves e prejuízos extensos na organização de personalidade dos quatro sujeitos: inabilidade social, imaturidade cognitiva, julgamento falho da realidade, dificuldades para lidar com afetos, limitação nos recursos para o enfrentamento de demandas, percepção de si mesmo danificada e prejudicada e pessimismo. Todas essas variáveis favorecem perdas bruscas do controle e denotam maior risco de suicídio. A análise desses prejuízos auxiliam na elaboração de recomendações terapêuticas, do foco psicodinâmico, do planejamento adequado no processo psicoterapêutico além de favorecer a antecipação de possíveis resistências. Os resultados demonstram a necessidade de intervenção em crise e tratamento a longo prazo desses sujeitos. O MR é uma técnica útil na avaliação de risco, na compreensão do contexto de risco e com potencial para a identificação de características do grupo de TSCG. _______________________________________________________________________________ ABSTRACT
The path to the understanding of any suicide attempt is surprisingly complex and requires the analysis of several types of variables. Currently it is recognized that people who make suicide attempts (SA) have different profiles associated to different types of SA. Patientes who make medically serious suicide attempts (MSSA) are most likely to present the profile of people who commit suicide. In order to highlight the Rorschach Inkblot Method (RIM) as a technique to understand this reality, this study presents the analysis of four cases of MSSA. It aims at investigating the structure and dynamics of personality related to indicators of vulnerability and psychopathological configuration, useful for the understanding suicide risk. The results revealed serious limitations and extensive damage in the personality organization of the four subjects. These data refer to poor social skills, cognitive immaturity, poor test of reality, difficulties dealing with emotions, limited resources for coping with demands, low self-esteem and pessimism. All these factors foster sudden loss of control and indicate increased risk of suicide. The analysis of these difficulties helps to establish therapeutic recommendations, psychodynamic focus, proper planning in the psychotherapeutic process. It also allows for anticipation of possible resistances. Results show the need for crisis intervention and long-term treatment of these subjects. The RIM is a technique useful in the assessment of risk, the understanding of the context of risk and with potential for the identification of characteristics of the MSSA group.

Resumo: Objetivo: Verificar as freqüências de comportamento suicida, durante a vida e nos últimos 12 meses, em pessoas com epilepsia em amostra da comunidade. Identificar: características relacionadas ao comportamento suicida em pessoas com epilepsia quando comparadas a controles da mesma região pareados por sexo e idade; fatores de risco para ideação suicida em pessoas com epilepsia e fatores associados à depressão e ansiedade em indivíduos com epilepsia. Método: Foram feitas três análises, a primeira, tipo caso-controle em que pessoas com epilepsia identificadas em um levantamento prévio na região, com diagnóstico confirmado por neurologistas, foram comparadas a pessoas da comunidade. A segunda comparou indivíduos com epilepsia e ideação suicida com aqueles sem ideação. A terceira análise foi feita com pessoas com epilepsia com diagnóstico (via HADS) de ansiedade e/ou depressão comparando-as às que não tiveram ansiedade e/ou depressão. A avaliação foi feita por meio de entrevista estruturada, com escalas psicométricas, contendo informações sobre saúde mental, dados socioeconômicos e características da epilepsia. Resultados: Das 171 pessoas identificadas com epilepsia, 153 casos foram entrevistados e comparados a 154 controles. Os casos tiveram mais frequentemente ansiedade (39,4% versus 23,8%, OR 2,1 [IC95% 1,2-3,5]; p=0,006) e depressão (24,4% versus 14,7%, OR 1,9 [IC95% 1,0-3,5]; p=0,04). Também relataram mais pensamentos de suicídio (36,7% versus 23,8%, OR 1,8 [IC95% 1,1-3,1]; p=0,02), planos (18,2% versus 3,3%, OR 2,0 [IC95% 1,0-4,0]; p=0.04) e tentativas de suicídio (12,1% versus 5,3%, OR 2,4 [IC95% 1,1-3,2], p=0,04) durante a vida. Os fatores associados à ideação suicida foram: ansiedade (OR 3,3 [IC95% 1,4-7,5]; p=0,001, depressão (OR 4,8 [IC95% 1,9- 12,5]; p=0,001) e dois ou mais diagnósticos psiquiátricos (OR 21,6 [IC95% 4,4- 105,9]; p<0,0001). Os fatores relacionados com depressão e ansiedade, respectivamente, foram: baixa escolaridade (OR 3,8 [IC95% 1,6-9,0]; p=0,002 e OR 2,8 [IC95% 1,2-6,5]; p=0,02), ideação suicida (OR 4,4 [IC95% 1,9-10,3]; p<0,0001 e OR 3,6 [IC95% 1,7-7,7]; p=0,001) e tentativa de suicídio (OR 9,3 [IC95% 2,6-32,8]; p<0,0001 e OR 6,9 [IC95% 1,8-26,4]; p=0,001). Conclusão: os achados chamam a atenção para comorbidade psiquiátrica e comportamento suicida na epilepsia, ressaltando a importância da identificação e tratamento dos transtornos mentais na prática clínica, especialmente para a prevenção do suicídio
Abstract: Objective: to estimate the frequencies of suicidal behavior in people with epilepsy in a community sample. To determine: which characteristics are related to suicidal behavior in people with epilepsy when compared with controls living in the same neighborhood matched by sex and age; which are the risk factors related to suicidal thoughts in epilepsy; which characteristics are associated with depression and anxiety in people with epilepsy. Method: Three analyses were made; the first one was a case-control study comparing people who have been diagnosed with epilepsy by neurologists in a previous community-based survey, with people without epilepsy living in the same region. The second compared epilepsy subjects with suicidal thoughts with those without. The third analysis made with people with epilepsy and anxiety and depression (determined by HADS) compared them to people without depression and/or anxiety. A structured interview, using psychometric scales, and questions about mental health, socio-economic status and characteristics of epilepsy were made. Results: From 171 subjects with epilepsy, 153 were interviewed and compared with 154 controls; those have had more frequently anxiety (39.4% versus 23.8%, OR 2.1 [95% CI 1.2 - 3.5]; p=0.006) and depression (24.4% versus 14.7%, OR 1.9 [95% CI 1.01 - 3.5]; p=0.04). They also reported more suicidal thoughts (36.7% versus 23.8%, OR 1.8 [95% CI 1.1 - 3.1]; p=0.02), plans (18.2% versus 3.3%, OR 2.0 [95% CI 1.0 - 4.0]; p=0.04) and attempts (12.1% versus 5.3%, OR 2.4 [95% CI 1.1 - 3.2], p=0.04) during life than controls. Factors associated with suicidal thoughts were: anxiety (OR 3.3 [95% CI 1.4-7.5]; p=0.001), depression (OR 4.8 [95%CI 1.9-12.5]; p=0.001) and 2 or more psychiatric disorder (OR 21.6 [95%CI 4.4-105.9]; p<0.0001). The characteristics related to depression and anxiety, respectively, were: few years of study (OR 3.8 [95% CI 1.6-9.0]; p=0.002 and OR 2.8 [95% CI 1.2 to 6.5]; p=0.02), suicidal thoughts (OR 4.4 [95%CI 1.9- 10.3]; p<0.0001 and OR 3.6, [95%CI 1.7-7.7]; p=0.001) and suicide attempt (OR 9.3 [95%CI 2.6-32.8]; p<0.0001 and OR 6.9 [95%CI 1.8-26.4]; p=0.001). Conclusion: the findings call attention for psychiatric comorbidity and suicidal behavior in epilepsy, reinforcing the importance of recognition and treatment of mental disorders in clinical practice, especially for suicide prevention

OBJETIVO: Verificar a associação entre a violência por parceiro íntimo na gravidez (VPIG) com o comportamento suicida de mulheres nos últimos sete anos. MÉTODOS: Estudo de coorte realizado entre 2013 e 2014, incluindo 644 mulheres cadastradas na Estratégia de Saúde da Família do Distrito Sanitário II, da cidade do Recife-PE. As mulheres foram entrevistadas na gravidez, puerpério e nos últimos sete anos, utilizando-se um questionário adaptado do Estudo Multipaíses sobre a Saúde da Mulher e Violência Doméstica da Organização Mundial da Saúde. O comportamento suicida englobou toda atitude no sentido de pôr fim à vida ou apenas tentar pôr fim à vida, independente do potencial letal do meio utilizado. Para avaliar comportamento suicida foram utilizadas as perguntas “Já tentou pôr fim à sua vida?” e “Tem tido a ideia de acabar com a vida?”. A regressão de Poisson foi utilizada para estimar os riscos relativos (RR) brutos e ajustados. RESULTADOS: Na gravidez, 33,9% foram vítimas de algum tipo de violência. A incidência do comportamento suicida foi de 47,37% entre as mulheres que relataram violência na gravidez. O comportamento suicida mostrou-se associado à VPIG (RR: 2,22; IC95%: 1,30-3,77). O apoio social mostrou-se como fator de proteção ao comportamento suicida (RR=0,35; IC 95%: 0,20-0,60). DISCUSSÃO: O estudo mostrou associação entre VPIG e comportamento suicida, mesmo após os ajustes por fatores considerados de confundimento. Nesse contexto o apoio social apareceu como fator de proteção. Tendo em vista os inúmeros impactos negativos da violência na saúde mental das mulheres vítimas de VPIG, faz-se necessário que os serviços de saúde estejam preparados para lidar com as vítimas da violência, bem como investir em políticas que garantam acesso às diversas modalidades de terapias e prevenção de danos.
OBJECTIVE: To investigate the association between intimate partner violence in pregnancy with women`s suicidal behavior in the last seven years. METHODS: This prospective cohort study was conducted with 644 women, enrolled in the Brazilian Family Health Strategy in Recife, Northeastern Brazil, between 2013 and 2014.The women were interviewed during pregnancy, postpartum and past seven years using a questionnaire adapted from the Multi-Country Study on Women's Health and Domestic Violence of the World Health Organization. Suicidal behavior encompasses the whole attitude towards putting an end to life or just trying to put an end to life, regardless of the lethal potential of the medium used. Poisson regression was used to estimate crude and adjusted relative risks (RR). RESULTS: The study showed that during pregnancy, 33,9% of women have suffered some kind of violence. The incidence of suicidal behavior was 47.37% among women who reported violence during pregnancy. Suicidal behavior was associated with IPVG (RR: 2.22; CI: 1.30-3.77). Social support was shown as a protective factor for suicidal behavior (RR = 0.35; CI: 0.20-0.60). DISCUSSION: The study showed an association between IPV and suicidal behavior, even after adjusting for confounding factors. In this context social support appeared as a protective factor. Given the many negative impacts of violence on the mental health of women victims of IPV, it is necessary that health services are prepared to deal with victims of violence and to invest in policies to guarantee access to the various modalities of therapies and prevention of damage.

Resumo: O estudo de perfis e fatores correlacionados ao comportamento suicida despontam como um poderoso mecanismo de prevenção, principalmente na população adolescente, grupo etário intimamente vulnerável a este evento. Objetivo:Determinar a prevalência e os fatores associados a ideação e tentativa de suicídio em adolescentes. Método: Estudo transversal, com 578 jovens de 15 a 19 anos. Para coleta foram empregados quatro instrumentos a saber: Critério de Classificação Econômica do Brasil; Self Report Questionare; Escala de Avaliação do Risco de Suicídio de Columbia e Escala de Avaliação da Adaptabilidade e Coesão Familiar. Utilizaram-se os testes Qui-quadrado de Pearson, Exato de Fisher, e elaborado modelo de regressão logística. Resultados:A prevalência de ideação foi de 26,1% e de tentativa 5%. Os fatores associados a ideação foram: classe econômica desfavorável, sintomatologia positiva de transtornos mentais, escolaridade fundamental para o responsável, ser filho intermediário, residir com muitas pessoas e apresentar funcionamento familiar com coesão desligada, e flexibilidade caótica. Para a tentativa foi significativa a presença de sintomas de transtornos mentais, coesão desligada, e flexibilidade rígida. Na regressão a combinação das variáveis: filho caçula ou intermediário, escolaridade do responsável, classe econômica desfavorável, sintomatologia positiva para transtornos mentais e família com coesão de níveis extremos e em todos os tipos de flexibilidade amplia o percentual de ideação em 85,5%. Conclusão:Uma grande proporção de adolescentes, relataram ideação, bem como já realizaram alguma tentativa de suicídio, sendo considerados um grande número de fatores demográficos, sócio econômicos, psicológicos, comportamentais e de relações familiares como associados a estes episódios.
Abstract: Investigations into profiles and factors associated with suicidal behavior can serve as a powerful prevention mechanism, especially for the adolescent population, which is particularly vulnerable to such behavior. Objective:To determine the prevalence and factors associated with suicide ideation and attempted suicide in adolescents. Methods:A cross-sectional study was conducted with 578 adolescents aged 15 to 19 years. Data collection involved the administration of the Brazilian Economic Classification Criteria, SelfReporting Questionnaire, Columbia Suicide Severity Rating Scale and Family Adaptability and Cohesion Evaluation Scale. Statistical analysis involved Pearson’s chi-square test, Fisher’s exact test and the creation of a logistic regression model. Results:The prevalence of suicide ideation and attempted suicide was 26.1% and 5%, respectively. Suicide ideation was associated with an unfavorable economic status, positive symptoms of mental disorders, an elementary school level of the guardian, being a middle child, residing with many people and family functioning classified as disengaged cohesion and chaotic flexibility. Attempted suicide was significantly associated with symptoms of mental disorders, disengaged cohesion and rigid flexibility. In the multiple regression analysis, being the youngest or middle child, guardian’s schooling at the elementary level, unfavorable economic status, positive symptoms of mental disorders and extreme cases of cohesion and flexibility increased the rate of suicide ideation by up to 85.5%. Conclusion:A large portion of the adolescents reported suicide ideation and some had attempted suicide. A considerable number of demographic, socioeconomic, psychological, behavioral and familial factors were associated with such episodes.

Texte intégralBackground and objectives: Risk factors for suicide and deliberate self-harm are the subject of many studies in the epidemiology of suicidal behaviour. However few studies have focused on the recurrence of suicidal behaviours after hospitalization in the emergency department for a deliberate self-harm. The aim of this study is to identify specific risk factors for repeated deliberate self-harm among patients admitting following a deliberate self-harm by acute intoxication to assess and orient in an optimal way after patients' suicidal act. Methods: A prospective year study was conducted with 606 patients admitted to the Emergency Room as a result of self-poisoning. The assessment, in the emergencies, fell on the intensity of suicidal intent with the Beck Suicidal Intent Scale (SIS), the level of alexithymia with the Toronto Alexithymia Scale-20 (TAS 20), the intensity of depression with the The Beck Depression Inventory-13 (BDI), the level of hopelessness with the Beck Hopelessness Scale, the level of alcohol dependence with the Alcohol Use Disorders Identification Test (AUDIT), and other psychiatric disorders with the Mini International Neuropsychiatric Interview (M. I. N. I. ). For each subject included, emergency admissions for suicidal behaviour, regardless of the modality, were informed with the computerization of medical data in the year following the date of inclusion, and have created the variable "relapse" and" multiple recurrences ". Results: Among the 606 participants, 418 women (69%) and 188 men (31%), mean age 38 years (SD= 13. 2), 140 (23. 1%) readmitted in the year after the evaluation and 61 (10. 1%) were readmitted more than 2 times. Antipsychotic and anxiolytic treatment, addiction to alcohol and marital status "single" were significant independent predictors of recurrence at 1 year. Antipsychotic and anxiolytic treatment and addiction to alcohol were significant independent predictors multiple recurrence. Conclusion: Upon an initial ER admission as a result of self-poisoning, it is important to evaluate specific factors, such as alcohol abuse, that could subsequently lead to repeated deliberate self-harm. In view of improving the targeting and referral of patients towards structures that can best respond to their needs

Texte intégralBackground: Suicide attempt (SA) concerns 8% to 10% of adolescents and SA recurrence within the year 14% to 20% of adolescents. SA of young patients may have implications over the long term on an academic/professional level, as well as on an affective and psychic level. Objectives: 1) To describe psychosocial outcomes of SAers and the weight of recurrence on these outcomes, 2) to study the impact of losing contact with caregivers during the year following SA, and 3) to study how young SAers use means of communication, and perceive social support they receive and their health-related quality of life. Methods: Three researches were carried out on adolescent SAers. 1) 309 SAers were evaluated at time of SA and 10 years after SA, 2) study of the correlation between a loss of contact with caregivers and SA recurrence occurring between 1 to 10 years after the initial SA of 249 young patients, 3) how 58 SAers use means of communication, and perceive social support they receive and their health-related quality of life. Results: As an adult, the psychosocial situation of young SAers is impaired, especially for those who have a recurrence of SA during the year after index SA. The risk of recurrence increases in the ten years following index SA for early SAers, and in the year following index SA when contact has been lost with caregivers. SAers preferentially use SMS to “keep in touch” with relatives and they assess on a less positive note than peers their social support and health-related quality of life. Perspectives: We developed a monitoring device based on SMS sending in order to prevent SA recurrence which will be assessed through a randomized controlled clinical trial

Texte intégralThe suicide is a problem liable of prevention. Nursing professionals who work in emergencies have a central role in the first management of suicide attempts, and the quality of care provided by these professionals may be influenced by their attitudes front of suicidal behavior. The knowledge about the attitudes related to the suicide and its associated factors is scarce and needs to be investigated in differents countries, cultures and times. This study investigated, among the nursing professionals who work in the emergencies, the attitudes related to suicide and associated factors. Quantitative cross-sectional study, developed with a population of 146 nursing professionals from two emergency services (prehospital and hospital services) in the interior of the state of São Paulo, Brazil. The data was collected in 2015 by the self application of a social-demografic questionnaire and Suicide Behavior Attitude Questionnaire (SBAQ). This questionnaire does not have cutoffs and its analysis can be done through the isolated items and factors \"Feelings towards the patient\", \"Professional capacity of perception\" and \"Right to Suicide\". The data was analyzed with Statistical Package for Social Sciences software (SPSS) version 19.0 and R GUI 3.0.1. The Kolmogorov-Smirnov and Shapiro-Wilk normality tests, The Spearman correlation test and Mann-Whitney test were applied. The participants presented low educacional exposure about the suicide and were obtained the maximum and minimum punctuations in each one of the items of the questionnaire. More negative attitudes were associated with have worked in mental health service (p=0,00). More perception of professional capacity was associated with specific formation in mental health (p=0,00) or suicide (p=0,00) and work in hospital service (p=0,01). Attitudes less moralists and condemnatory were associated to experience of working in mental health services (p=0,01) and being nurse (p=0,00). There was a weak positive correlation between the perception of professional capacity and attitudes less negatives. This study is a pioner in the investigation of associated factors front to suicide attitudes among nursing professionals who work in Brazilian prehospital and hospital emergencies. It is important to invest in stratagies that promote professional\'s mental health, better attitudes and preparation for the qualification of care

Resumo: Este estudo foi realizado no Hospital das Clínicas da Universidade Estadual de Campinas, Campinas - SP. Teve por objetivos relacionar o grau de intencionalidade suicida com o grau de letalidade médica e verificar a existência de subgrupos de pacientes e caracterizá-Ios clinicamente. Foram avaliados 121 pacientes. Os sujeitos deste estudo foram os indivíduos atendidos no pronto-socorro por tentativa de suicídio e que aí permaneceram por 24 horas ou mais para observação e aqueles que necessitaram de internação em uma das enfermarias do hospital. Para avaliação foi utilizado um questionário ("Questionário de Tentativa de Suicídio - QTS") composto por uma anamnese estruturada e escalas psicométricas. Na análise foi utilizado o teste Qui-quadrado ou Exato de Fisher para verificar associação ou comparar proporções entre variáveis categóricas e o teste de Mann-Whitney para variáveis contínuas ou ordenáveis. Na verificação de correlação entre intencionalidade suicida e letalidade médica o Coeficiente de Correlação Linear de Spearman foi utilizado. A análise de Regressão Logística Politômica Multivariada, a Análise de Correspondência e a Análise de Agrupamentos ("Cluster Analysis") foram utilizadas para identificar as variáveis que poderiam discriminar grupos resultantes da interação entre diversas variávels. Os pacientes foram em sua maioria adultos jovens e do sexo masculino (64,5%). A média de idade foi 32,6 anos. Dois terços dos pacientes tinham menos que oito anos de escola cursados com sucesso e metade dos pacientes eram solteiros. Em 82% dos pacientes o método utilizado foi o envenenamento. Oito pacientes faleceram, todos do sexo masculino com média de idade de 27,5 anos. Três variáveis associaram-se com maior intencionalidade suicida: método mais violentos, presença de sintomas de depressão e hipótese diagnóstica de transtorno afetivo. Duas variáveis se associaram com maior letalidade médica: sexo masculino e ocorrência de uma tentativa de suicídio anterior. Três subgrupos, excluindo-se os pacientes que faleceram, foram identificados pela análise de agrupamentos. O primeiro subgrupo, com 43 pacientes: a maioria do sexo feminino com média de idade de 32,7 anos; o envenenamento foi o método utilizado; metade dos pacientes apresentou maior letalidade médica e dois terços apresentaram menor intencionalidade suicida. O segundo subgrupo, formado por 53 pacientes: a maioria do sexo masculino com média de idade de 30 anos; o envenenamento foi o único método utilizado; a maioria apresentou maior letalidade médica e maior intencionalidade suicida. O terceiro subgrupo, formado por 17 pacientes: a maioria do sexo masculino com média de idade de 42,7 anos; todos utilizaram método mais violentos; a maioria com maior letalidade médica e maior intencionalidade suicida
Abstract: This study was carried out at the general hospital of the Universidade Estadual de Campinas, in Campinas, Brazil. The aims of the study were to assess the relationship between suicide intention and medical lethality, as well as to verify the existence of different groups of suicide attempters. The study subjects were those patients admitted to the hospital after a suicide attempt and those who stayed in the casualty department for more than 24 hours. A structured interview including psychometric scales, sociodemographic and clinical data, was used. The following statistical tests were used: Chi-squared, Fisher, Mann-Whitney, Spearman correlation, multivariate politomic logistic regression, correspondence analysis and cluster analysis. Most patients were male (64.5%) and young adults. The mean age was 32.6 years. Half of the patients were single and two thirds had less than 8 years of scholarship. Poisoning was the method of suicide most frequently used (82%). There were 8 deaths in young male adults (mean age = 27.5 years). 175 Three variables were associated with greater medicallethality: the use of a violent method~ depressive symptoms and the diagnosis of an affective disorder. Two variables were associated with medicallethality: male and a previous suicide attempt. Three groups of patients (excluding 8 cases of death) were identified by cluster analysis: 1. A group of 43 subjects (most of them female, mean age = 32.7 years) who used self-poisoning as suicide method. Half presented higher degrees of medical lethality and two thirds lower leveI of suicide intention; 2. A group of 53 subjects (most males, mean age = 30 years) who used self-poisoning and presented both high medical lethality and suicide intention; 3. A group of 17 subjects (most males, mean age = 42.7) who used violent method in the suicide attempt and presented higher levels of medical lethality and suicide intention

Resumo: Objetivo: Detectar fatores associados ao comportamento suicida em pacientes que fazem uso nocivo de bebidas alcoólicas internados em hospital geral. Método: 4.352 pacientes, admitidos consecutivamente, foram avaliados utilizando-se um rastreamento do qual constavam as escalas AUDIT e HAD. Fixando-se histórico de tentativa de suicídio como variável dependente, testes de qui-quadrado foram realizados com variáveis sociodemográficas e clínicas. Resultados: Uso nocivo de álcool (AUDIT > 8) foi detectado em 423 pacientes. Dentre estes, 60 (14,2%) tinham depressão (HAD > 8) e 34 (8%) tinham histórico de tentativa de suicídio. Observou-se, também, maior frequência de tentativa de suicídio prévia em pacientes que estavam deprimidos (p = 0,0001), portadores de HIV (p = 0,0001) e que faziam uso de psicofármacos (p = 0,02). Conclusões: A elevada prevalência de depressão em pacientes com transtornos relacionados ao uso de álcool e com tentativa de suicídio pregressa reforça a hipótese de que há outros fatores, como a depressão, relacionados à co-ocorrência de alcoolismo e suicídio
Abstract: Objective: to detect factors associated to suicidal behavior among patients admitted to a general hospital who presented harmful alcohol drinking pattern. Method: 4352 patients consecutively admitted were screened by means of the AUDIT and HAD. Chi-square tests were performed taking history of previous suicide attempts as dependent variable. Results: 432 individuals presented alcohol harmful use or dependence (AUDIT > 8), 60 (14.2%) of which had depression (HAD > 8) and 34 (8%) previous suicide attempt. The latter was more frequent among those who were depressed (p = 0.0001), taking psychotropic medicines (p = 0.02) and HIV positive (p = 0.0001). Conclusion: The high prevalence of depression in these patients reinforces the hypothesis that there are other factors, such as depression, associated with the cooccurrence of alcoholism and suicide

Introduction: Suicide is one of the ten leading causes of death worldwide, emerging from a complex interaction between genetic, environmental and psychosocial factors. In this aspect studies found that individuals with suicidal tendencies have elevations in the levels of pro-inflammatory cytokines and alterations in the functioning of the HPA axis. Objective: Investigate the association of the polymorphism in the CRHR1 gene (rs110402) with the suicidal behavior, as well as the genotype effects on IL-1β levels. Methods: Sample consisted in 171 individuals that participated in the study, from which 15 were at risk of suicide, 20 had attempted suicide and 136 were controls. We perform genotyping of individuals by real-time PCR and IL-1β levels measured by ELISA technique. Results: Individuals with a clinical diagnosis of attempted suicide carrying the A allele of SNP rs110402 showed increased IL-1β levels (19.3 IQR: 6.84 to 36.47) compared to the control group (4.91 IQR: 3.22 to 6.39; p = 0.027). Ratings isolated from the levels of this interleukin and genotypes of the rs10402 SNP showed no relationship with suicidal behavior. Conclusion: Our results suggest that alterations in CRHR1 gene can affect the levels of pro-inflammatory molecules, showing the complex interactions of biological markers in suicidal behavior.
Introdução: O suicídio é umas das dez principais causas de morte no mundo, emergindo de uma interação complexa entre fatores genéticos, ambientais e psicossociais. Neste aspecto, estudos identificaram que indivíduos com tendências suicidas apresentam elevações nos níveis de citocinas pró-inflamatórias e alterações no funcionamento do eixo HPA. Objetivo: Investigar a associação de um polimorfismo gene do CRHR1 (rs110402) com o comportamento suicida, bem como os efeitos do genótipo sobre os níveis de IL-1β. Métodos: A amostra foi composta de 171 indivíduos, e destes 15 apresentavam risco de suicídio, 20 já haviam tentado o suicídio e 136 eram controles. Realizamos a genotipagem dos indivíduos por PCR em tempo real e a dosagem de IL-1β pela técnica de Elisa. Resultados: Indivíduos com um diagnóstico clínico de tentativa de suicídio que carregavam o alelo A do SNP rs110402 mostraram aumento dos níveis de IL-1β (19,3 IQR: 6,84 - 36,47) em relação ao grupo controle (4,91 IQR: 3,22-6,39; p = 0,027). Avaliações isoladas em relação aos níveis dessa interleucina e aos genótipos desse polimorfismo não mostraram relação com o comportamento suicida. Conclusão: Nossos resultados sugerem que alterações no gene do CRHR1 podem afetar os níveis de moléculas pró-inflamatórias, mostrando as interações complexas de marcadores biológicos no comportamento suicida.

Texte intégralNational and international studies and reports about preventing suicide in jail have been published, giving information about epidemiology, suicide risk profiles. They also provide prevention measures. More researches are required to improve knowledge of clinical aspects of suicidal behaviors in prison and to evaluate prevention programs. The study consisted in an assessment of 12 measures of a local preventing suicide program. We asked prisoners who had been jailed since one month for an interview. First they had to tell if they knew about the existence of a prevention program. Second, they evaluated the usefulness of each measure in (5 points - Likert scale). They freely explained their choices. We asked them about suicidal ideation’s presence since they had arrived in jail, their urgency degree at D0, D8 and D28, and the factors that contributed to these states. They filled the Reasons for Living Inventory of Linehan (RLIL) and the Mini International Neuropsychiatric Interview. Twenty of the 53 participants have had suicidal thoughts during the first month ; only 15 told they knew there was a suicide prevention program. Eight measures were noted as significantly useful. Four of them obtained a middle score : limitation of hanging points, cell doubling, special monitoring, special protection cell. The search for correlations have found different combinations of links between some measures, the suicidal states, use of the program, the RLIL and psychiatric disorders. We discuss about how psychological tools can be helpful to the evolution of each measure, to the adaptation of the global prevention program. Notably, the prisoners’ participation is in particular proposed. Moreover, preserving speaking time with freedom of expression in the application of the program was expected by the participants. At last, they proposed innovative methods that could serve as a lead for followup works

Texte intégralThe aim of this Ph.D thesis is to understand the contemporary issues about French suicidality. Two different dimensions have been investigated. First, we interrogated the dynamic of suicidal process, considering both individual motives and social causes of suicidal behaviors. Second, we studied emergency departments of the French public hospitals, that take care of the majority of suicide attempts known in France. The methodological and theoretical principles adopted in this work articulate our understanding of suicidal process with our analysis of its medical treatment. In this work, we will suggest that suicidality and its medical treatment are shaped by normative transformations of contemporary societies, which place autonomy, independence, authenticity and responsibility as preeminent values. Yet, such values enhance a new type of social bound. This individualization changes the type of problems that actors have to solve with their own economic, cultural and social resources, which are sometimes not sufficient. Thus, this new normativity tends to affect suicidal issues. In summary, we propose to enlighten a new economy of suicides and attempted suicides. To do so, we conducted an ethnographic investigation of general and psychiatric emergency services. The main hypothesis of this approach precisely supports that this new type of problems shaped through individualization arises in a specific manner in the medical treatment of suicidal behaviors
