New first derivative ratio spectrophotometric method was developed for simultaneous estimation of telmisartan hydrochloride (TELM) and hydrochlorothiazide (HCTZ) in combined tablet dosage form without prior separation. The amplitude in the first derivative of the corresponding ratio spectra at 263.2 nm and 278.4 nm of TELM and for HCTZ at 287.2 nm and 304.8 nm were selected for the determination. The calibration graphs were established in the range of 1-10μg/mL and 2-15 μg/mL of TELM and HCTZ, respectively. The percentage recoveries of binary mixture of TELM were found to be 97.81±1.05 (263.2nm) and 96.60±1.91 (278.4nm) and for HCTZ were found to be 97.47±1.41 (287.2), 98.16± 1.45 (304.8nm) by proposed method. This method was successfully applied for the analysis of commercial tablet formulation, with no interference from excipients as indicated by the recovery study results. The proposed method was validated, statistically analyzed, compared with reported HPLC method and can be successfully applied for the routine analysis.

Twin extreme learning machine (TELM) is a phenomenon of symmetry that improves the performance of the traditional extreme learning machine classification algorithm (ELM). Although TELM has been widely researched and applied in the field of machine learning, the need to solve two quadratic programming problems (QPPs) for TELM has greatly limited its development. In this paper, we propose a novel TELM framework called Lagrangian regularized twin extreme learning machine (LRTELM). One significant advantage of our LRTELM over TELM is that the structural risk minimization principle is implemented by introducing the regularization term. Meanwhile, we consider the square of the l2-norm of the vector of slack variables instead of the usual l1-norm in order to make the objective functions strongly convex. Furthermore, a simple and fast iterative algorithm is designed for solving LRTELM, which only needs to iteratively solve a pair of linear equations in order to avoid solving two QPPs. Last, we extend LRTELM to semi-supervised learning by introducing manifold regularization to improve the performance of LRTELM when insufficient labeled samples are available, as well as to obtain a Lagrangian semi-supervised regularized twin extreme learning machine (Lap-LRTELM). Experimental results on most datasets show that the proposed LRTELM and Lap-LRTELM are competitive in terms of accuracy and efficiency compared to the state-of-the-art algorithms.

Coal gangue is the main solid waste in coal mining areas, and its annual emissions account for about 10% of coal production. The composition information of coal gangue is the basis of reasonable utilization of coal gangue, and according to the composition information of coal gangue, one can choose the appropriate application scene. The reasonable utilization of coal gangue can not only effectively alleviate the environmental problems in mining areas but also produce significant economic and social benefits. Chemical analysis techniques are the principal ones used in traditional coal gangue analysis; however, they are slow and expensive. Many researchers have used machine learning techniques to analyze the spectral data of coal gangue, primarily random forests (RFs), extreme learning machines (ELMs), and two-hidden-layer extreme learning machines (TELMs). However, these techniques are heavily reliant on the preprocessing of the spectral data. This research suggests a quick analysis approach for coal gangue based on thermal infrared spectroscopy and deep learning in light of the drawbacks of the aforementioned methodologies. The proposed deep learning model is named SR-TELM, which extracts spectral features using a convolutional neural network (CNN) consisting of a spatial attention mechanism and residual connections and implements content prediction with TELM as a regressor, which can effectively overcome the dependence on preprocessing. The usefulness and speed of SR-TELM in coal gangue analysis were demonstrated by comparing several models in order to verify the proposed coal gangue analysis model. The experimental findings show that, for the prediction tasks of moisture, ash, volatile matter, and fixed carbon content, respectively, the SR-TELM model attained an R2 of 0.947, 0.972, 0.967, and 0.981 and an RMSE of 0.274, 4.040, 1.567, and 2.557 with a test time of just 0.03 s. It offers a method for the analysis of coal gangue that is low cost, highly effective, and highly reliable.

Iron ore is an important raw material for the steel industry, so it is of great economic significance to determine the grade of the iron ore quickly and accurately. And the TFe content is the main indicator that determines the grade of the iron ore and whether the iron ore can be smelted directly. Unlike manual methods and methods for chemical analysis, the paper uses the selection of band for the near-infrared spectrum based on the pruning method and the two-hidden-layer extreme learning machine based on LU decomposition and seagull optimization algorithm (LU-TELM-SOA) to identify the TFe content. First of all, the paper proposes the selection of band based on the pruning method to retain the sensitive band of the near-infrared spectrum. Aiming at the problems of poor stability and low accuracy of a single LU-TELM (the two-hidden-layer extreme learning machine based on LU decomposition) model, the paper proposes LU-TELM-SOA. The experimental results show that LU-TELM-SOA has the advantages of high accuracy and strong stability.

The record of fossil land mammals from Antarctica has been restricted previously to the middle levels of the Eocene-?early Oligocene La Meseta Formation in Seymour Island, Antarctic Peninsula. This mostly shallow-marine sequence was divided informally into seven subunits (Tertiary Eocene La Meseta or TELM 1 to 7) by Sadler (1988). Land mammals, representing South American lineages of marsupials, edentates, and ungulates were recovered from TELM 3, 4, and 5 (Marenssi et al., 1994; Vizcaíno et al., 1994). The purpose of the present note is to report the discovery of a well-preserved ungulate tooth from the uppermost level of the La Meseta Formation (TELM 7) and to discuss its paleoenvironmental implications.

As a novel neural network learning framework, Twin Extreme Learning Machine (TELM) has received extensive attention and research in the field of machine learning. However, TELM is affected by noise or outliers in practical applications so that its generalization performance is reduced compared to robust learning algorithms. In this paper, we propose two novel distance metric optimization-driven robust twin extreme learning machine learning frameworks for pattern classification, namely, CWTELM and FCWTELM. By introducing the robust Welsch loss function and capped L2,p-distance metric, our methods reduce the effect of outliers and improve the generalization performance of the model compared to TELM. In addition, two efficient iterative algorithms are designed to solve the challenges brought by the non-convex optimization problems CWTELM and FCWTELM, and we theoretically guarantee their convergence, local optimality, and computational complexity. Then, the proposed algorithms are compared with five other classical algorithms under different noise and different datasets, and the statistical detection analysis is implemented. Finally, we conclude that our algorithm has excellent robustness and classification performance.

For the past decades, recognition technologies of multispectral palmprint have attracted more and more attention due to their abundant spatial and spectral characteristics compared with the single spectral case. Enlightened by this, an innovative robust L2 sparse representation with tensor-based extreme learning machine (RL2SR-TELM) algorithm is put forward by using an adaptive image level fusion strategy to accomplish the multispectral palmprint recognition. Firstly, we construct a robust L2 sparse representation (RL2SR) optimization model to calculate the linear representation coefficients. To suppress the affection caused by noise contamination, we introduce a logistic function into RL2SR model to evaluate the representation residual. Secondly, we propose a novel weighted sparse and collaborative concentration index (WSCCI) to calculate the fusion weight adaptively. Finally, we put forward a TELM approach to carry out the classification task. It can deal with the high dimension data directly and reserve the image spatial information well. Extensive experiments are implemented on the benchmark multispectral palmprint database provided by PolyU. The experiment results validate that our RL2SR-TELM algorithm overmatches a number of state-of-the-art multispectral palmprint recognition algorithms both when the images are noise-free and contaminated by different noises.

With the further development of the construction of “smart mine,” the establishment of three-dimensional (3D) point cloud models of mines has become very common. However, the truck operation caused the 3D point cloud model of the mining area to contain dust points, and the 3D point cloud model established by the Context Capture modeling software is a hollow structure. The previous point cloud denoising algorithms caused holes in the model. In view of the above problems, this paper proposes the point cloud denoising method based on orthogonal total least squares fitting and two-layer extreme learning machine improved by genetic algorithm (GA-TELM). The steps are to separate dust points and ground points by orthogonal total least squares fitting and use GA-TELM to repair holes. The advantages of the proposed method are listed as follows. First, this method could denoise without generating holes, which solves engineering problems. Second, GA-TELM has a better effect in repairing holes compared with the other methods considered in this paper. Finally, this method starts from actual problems and could be used in mining areas with the same problems. Experimental results demonstrate that it can remove dust spots in the flat area of the mine effectively and ensure the integrity of the model.

Twin extreme learning machine (TELM) is a classical and high-efficiency classifier. However, it neglects the statistical knowledge hidden inside the data. In this paper, in order to make full use of statistical information from sample data, we first come up with a Fisher-regularized twin extreme learning machine (FTELM) by applying Fisher regularization into TELM learning framework. This strategy not only inherits the advantages of TELM, but also minimizes the within-class divergence of samples. Further, in an effort to further boost the anti-noise ability of FTELM method, we propose a new capped L1-norm FTELM (CL1-FTELM) by introducing capped L1-norm in FTELM to dwindle the influence of abnormal points, and CL1-FTELM improves the robust performance of our FTELM. Then, for the proposed FTELM method, we utilize an efficient successive overrelaxation algorithm to solve the corresponding optimization problem. For the proposed CL1-FTELM, an iterative method is designed to solve the corresponding optimization based on re-weighted technique. Meanwhile, the convergence and local optimality of CL1-FTELM are proved theoretically. Finally, numerical experiments on manual and UCI datasets show that the proposed methods achieve better classification effects than the state-of-the-art methods in most cases, which demonstrates the effectiveness and stability of the proposed methods.

AbstractTraffic incidents in big cities are increasing alongside economic growth, causing traffic delays and deteriorating road safety conditions. Thus, developing a universal freeway automatic incident detection (AID) algorithm is a task that took the interest of researchers. This paper presents a novel automatic traffic incident detection method based on the extreme learning machine (ELM) algorithm. Furthermore, transfer learning has recently gained popularity as it can successfully generalise information across multiple tasks. This paper aimed to develop a new approach for the traffic domain-based domain adaptation. The ELM was used as a classifier for detection, and target domain adaptation transfer ELM (TELM-TDA) was used as a tool to transfer knowledge between environments to benefit from past experiences. The detection performance was evaluated by common criteria including detection rate, false alarm rate, and others. To prove the efficiency of the proposed method, a comparison was first made between back-propagation neural network and ELM; then, another comparison was made between ELM and TELM-TDA.

Recently, fossil land mammals have been recorded in the James Ross Island area, east of the Antarctic Peninsula, from the marine middle-late Eocene strata of Seymour Island. These include two endemic species of polydolopid marsupial: Antarctodolops dailyi Woodburne & Zinsmeister 1984, and Eurydolops seymourensis Case, Woodburne & Chaney 1988, and three partly determinate placentals identified as a tardigrade edentate, a sparnotheriodontid litoptern and a trigonostylopid astrapothere (Bond et al. 1989). The marsupials are represented by fragmentary jaw and complete dental material, the litoptern and astrapothere only by tooth fragments. All are recorded from Unit TELM 5 of the La Meseta Formation (Sadler 1988), the two ungulates at a lower level than the rest. The subject of the present note is a second tooth fragment from Seymour Island identified as an astrapothere. It is worth recording because such mammal remains are rare and it is from TELM 4, the unit below that yielding the other land mammals.

Due to the complicated engineering operation of the check valve in a high−pressure diaphragm pump, its vibration signal tends to show non−stationary and non−linear characteristics. These leads to difficulty extracting fault features and, hence, a low accuracy for fault diagnosis. It is difficult to extract fault features accurately and reliably using the traditional MPE method, and the ELM model has a low accuracy rate in fault classification. Multi−scale weighted permutation entropy (MWPE) is based on extracting multi−scale fault features and arrangement pattern features, and due to the combination of extracting a sequence of amplitude features, fault features are significantly enhanced, which overcomes the deficiency of the single−scale permutation entropy characterizing the complexity of vibration signals. It establishes the check valve fault diagnosis model from the twin extreme learning machine (TELM). The TELM fault diagnosis model established, based on MWPE, aims to find a pair of non−parallel classification hyperplanes in the equipment state space to improve the model’s applicability. Experiments show that the proposed method effectively extracts the characteristics of the vibration signal, and the fault diagnosis model effectively identifies the fault state of the check valve with an accuracy rate of 97.222%.

La veritable font principal d’aquest conjunt és la Història dels animals d’Elià —font alhora del Fisiòleg grec, i aquest del llatí—, part de la transmissió manuscrita de la qual remunta al segle XIII. Ultra les nombroses coincidències de Turmeda amb, per ordre de prelació dins l’actual estat de la qüestió, els bestiaris medievals, el Fisiòleg llatí i la Risala al-hayawanat, cal apreciar aquelles on la font és tan sols Elià, la qual cosa obliga a replantejar la seua recepció a la nostra tradició literària.

Extreme learning machine (ELM) is a rapid learning algorithm of the single-hidden-layer feedforward neural network, which randomly initializes the weights between the input layer and the hidden layer and the bias of hidden layer neurons and finally uses the least-squares method to calculate the weights between the hidden layer and the output layer. This paper proposes a multiple hidden layers ELM (MELM for short) which inherits the characteristics of parameters of the first hidden layer. The parameters of the remaining hidden layers are obtained by introducing a method (make the actual output zero error approach the expected hidden layer output). Based on the MELM algorithm, many experiments on regression and classification show that the MELM can achieve the satisfactory results based on average precision and good generalization performance compared to the two-hidden-layer ELM (TELM), the ELM, and some other multilayer ELM.

Carlini et al. (1990) reported the presence of Xenarthra (Mammalia) from the Eocene deposits of Seymour Island, Antarctic Peninsula, based on an ungual phalanx from locality RV 8200. The locality, referred to informally as “mammal site” by Woodburne & Zinsmeister (1984), is located at 64°14′21″S, 56°39′44″W at an elevation of 45 m in the middle levels of the shallow marine La Meseta Formation (TELM 5 of Saddler 1988). Carlini et al. (1990) initially identified the phalanx as a megatherioid sloth (Order Tardigrada). Marenssi et al. (1994) revised its identification to ?Tardigrada or ?Vermilingua, by comparison with a primitive myrmecophagid (Order Vermilingua) from the Lower Miocene of Patagonia, whose ungual phalanges are indistinguishable from those of primitive sloths (Carlini et al. 1992). We report a fragmentary tooth of a member of the Tardigrada (Family incertue sedis) confirming the presence of this group in the Eocene of Antarctica.

Health monitoring is important for densely distributed urban infrastructures, particularly in cities undergoing rapid economic progress. Permanent scatterer interferometry (PSI) is an advanced remote sensing observation technique that is commonly used in urban infrastructure monitoring. However, the rapid construction of infrastructures may easily cause a loss of coherence for radar interferometry, inducing a low density of effective permanent scatterer (PS) points, which is the main limitation of PSI. In order to address these problems, a novel time-series synthetic aperture radar interferometry (InSAR) process based on the adaptive window homogeneous pixel selection and phase optimization (AWHPSPO) algorithm and thermal expansion linear model (TELM) is proposed. Firstly, for homogeneous point selection, information on both the time-series intensity and deformation phases is considered, which can compensate for the defects of insufficient homogeneous samples and low phase quality in traditional distributed scatterer interferometric synthetic aperture radar (DS-InSAR) processing. Secondly, the physical, thermal expansion component, which reflects the material properties of the infrastructures, is introduced into the traditional linear model, which can more rationally reflect the temporal evolution of deformation variation, and the thermal expansion coefficients can be estimated simultaneously with the deformation parameters. In order to verify our proposed algorithm, the Orange Island area in Changsha City, China, was selected as the study area in this experiment. Three years of its historical time-series deformation fields and thermal expansion coefficients were regenerated. With the use of high-resolution TerraSAR-X radar satellite images, a maximum accumulated settlement of 12.3 mm and a minor uplift of 8.2 mm were detected. Crossvalidation with small baseline subset interferometric synthetic aperture radar (SBAS-InSAR) results using Sentinel 1A data proved the reliability of AWHPSPO. The proposed algorithm can provide a reference for the control of the health and safety of urban infrastructures.

Extreme Learning Machine (ELM) as a fast and efficient neural network model in pattern recognition and machine learning will decline when the labeled training sample is insufficient. Transfer learning helps the target task to learn a reliable model by using plentiful labeled samples from the different but relevant domain. In this paper, we propose a supervised Extreme Learning Machine with knowledge transferability, called Transfer Extreme Learning Machine with Output Weight Alignment (TELM-OWA). Firstly, it reduces the distribution difference between domains by aligning the output weight matrix of the ELM trained by the labeled samples from the source and target domains. Secondly, the approximation between the interdomain ELM output weight matrix is added to the objective function to further realize the cross-domain transfer of knowledge. Thirdly, we consider the objective function as the least square problem and transform it into a standard ELM model to be efficiently solved. Finally, the effectiveness of the proposed algorithm is verified by classification experiments on 16 sets of image datasets and 6 sets of text datasets, and the result demonstrates the competitive performance of our method with respect to other ELM models and transfer learning approach.

Unlike the ubiquitously expressed TEL1 gene, however,TEL2 appears to be expressed predominantly in hematopoietic tissues. Antibodies raised against the C-terminus of the TEL2 protein were used to show that TEL2 localizes to the nucleus. All ETS proteins can bind DNA via the highly conserved ETS domain, which recognizes a purine-rich DNA sequence with a GGAA core motif. DNA binding assays show that TEL2 can bind the same consensus DNA binding sequence recognized by TEL1/ETV6. Additionally, the TEL2 protein is capable of associating with itself and with TEL1 in doubly transfected Hela cells, and this interaction is mediated through the pointed (PNT) domain of TEL1. The striking similarities ofTEL2 to the oncogenic TEL1, its expression in hematopoietic tissues, and its ability to associate withTEL1 suggest that TEL2 may be an important hematopoietic regulatory protein.

OBJECTIVE: To investigate the long term survival of medical emergency team (MET) patients at an Australian regional hospital and describe associated patient and MET call characteristics. DESIGN: Retrospective cohort study. Data linkage to the statewide death registry was performed to allow for long term survival analysis, including multivariable Cox proportional hazards regression and production of Kaplan–Meier survival curves. SETTING: A large Australian regional hospital. PARTICIPANTS: Adult patients who received a MET call from 1 July 2012 to 3 March 2020. MAIN OUTCOME MEASURES: Survival to 30, 90 and 180 days; one year; and 5-years after index MET call. RESULTS: The study included 6499 eligible patients. The cohort median age was 71 years, and 52.4% of the patients were female. Surgical (39.6%) and medical (36.9%) patients comprised most of the cohort. Thirty-day survival was 86.5% one-year survival was 66.1%. Among patients aged < 75 years, factors independently associated with significantly higher long term mortality included age (hazard ratio [HR], 3.26 [95% CI, 2.63–4.06]; for patients aged 65–74 v 18–54 years), male sex (HR, 0.71 [95% CI, 0.61–0.83]; for females) and pre-existing limitation of medical therapy (HR, 2.76; 95% CI, 2.28–3.35). Among patients aged ≥ 75 years, factors independently associated with significantly higher long term mortality included age (HR, 1.46 [95% CI, 1.29–1.65]; for patients aged ≥ 85 years), male sex (HR, 0.74 [95% CI, 0.66–0.83]; for females), and altered MET criteria (HR, 1.33; 95% CI, 1.03–1.71). CONCLUSIONS: Long term survival probabilities of MET call patients are affected by factors including age, sex, and limitation of medical therapy status. These data may be useful for clinicians conducting end-of-life discussions with patients.

Cieľom príspevku je teoreticky priblížiť teóriu vtelenej kognície, zasadiť ju do širšieho filozofického rámca a uviesť jej potenciálny prínos pre pedagogiku. Príspevok postupuje od ozrejmenia samotného pojmu vtelenie, jeho pôvod vo fenomenológii (M. Merleau-Ponty) i využitie v kultúrnej antropológii (T. J. Csordas). V ďalšej časti sa venujeme filozofickému rámcu, ktorý mal vplyv na teóriu vtelenej kognície a potlačenie dualistického pohľadu na telo a myseľ, ozrejmujeme aspekty vtelenia dotvárajúce obraz vtelenia. V rámci prínosu vtelenej kognície pre súčasnú pedagogiku sa zameriavame na dve oblasti skúmania vtelenej kognície, a to na jazyk a matematiku, ktoré sú považované za prevažne kognitívne a abstraktné koncepty oddelené od tela. Ukázalo sa, že teória vtelenej kognície nedostatočne vysvetľuje to, čo vlastne telo je, a o akom tele sa tu pojednáva. Z uvedeného vyplýva, že v oboch uvádzaných oblastiach (jazyk a matematika) je významnou časťou najmä skúmanie a význam gest a telesnej skúsenosti pre proces chápania abstraktným systémom a učenia. Zistili sme, že prínos vtelenej kognície pre pedagogiku spočíva v orientácii na telesné konanie, aktivitu a vplyv prostredia a telesnej skúsenosti na kogníciu, ktoré uľahčujú proces učenia.

Background Telephone-assisted cardiopulmonary resuscitation (T-CPR) has proven to be a crucial intervention in enhancing the ability of lay responders to perform cardiopulmonary resuscitation (CPR) during telehealth emergency services. While the majority of established T-CPR protocols primarily focus on guiding individual rescuers, there is a lack of emphasis on instructing and coordinating multiple lay responders to perform resuscitation collaboratively. Objective This study aimed to develop an innovative team-based tele-instruction tool to efficiently organize and instruct multiple lay responders on the CPR process and to evaluate the effectiveness and feasibility of the tool. Methods We used a mixed methods design in this study. We conducted a randomized controlled simulation trial to conduct the quantitative analysis. The intervention groups used the team-based tele-instruction tool for team resuscitation, while the control groups did not have access to the tool. Baseline resuscitation was performed during the initial phase (phase I test). Subsequently, all teams watched a team-based CPR education training video and finished a 3-person practice session with teaching followed by a posttraining test (phase II test). In the qualitative analysis, we randomly selected an individual from each team and 4 experts in emergency medical services to conduct semistructured interviews. The purpose of these interviews was to evaluate the effectiveness and feasibility of this tool. Results The team-based tele-instruction tool significantly improved the quality of chest compression in both phase I and phase II tests. The average compression rates were more appropriate in the intervention groups compared to the control groups (median 104.5, IQR 98.8-111.8 min–1 vs median 112, IQR 106-120.8 min–1; P=.04 in phase I and median 117.5, IQR 112.3-125 min–1 vs median 111, IQR 105.3-119 min–1; P=.03 in phase II). In the intervention group, there was a delay in the emergency response time compared to that in the control group (time to first chest compression: median 20, IQR 15-24.8 seconds vs median 25, IQR 20.5-40.3 seconds; P=.03; time to open the airway: median 48, IQR 36.3-62 seconds vs median 73.5, IQR 54.5-227.8 seconds; P=.01). However, this delay was partially mitigated after the phase II test. The qualitative results confirmed the compatibility and generalizability of the team-based tele-instruction tool, demonstrating its ability to effectively guide multiple lay responders through teamwork and effective communication with telecommunicators. Conclusions The use of the team-based tele-instruction tool offers an effective solution to enhance the quality of chest compression among multiple lay responders. This tool facilitated the organization of resuscitation teams by dispatchers and enabled efficient cooperation. Further assessment of the widespread adoption and practical application of the team-based tele-instruction tools in real-life rescue scenarios within the telehealth emergency services system is warranted.

The innovative use of technologies in language learning and teaching significantly impacts language education. This paper aims to investigate technology-enhanced language learning (TeLL) and teaching (TeLT) in higher education through surveying 223 students and 93 teachers across four Hong Kong universities, and interviewing around 50 students and 20 teachers. The current study investigated the e-resources/tools frequently adopted in language learning and teaching, how participants perceived the affordances of TeLL and TeLT, and challenges they had faced in technology-enhanced language learning and teaching. The results reveal that students and teachers perceived several affordances of TeLL and TeLT, such as provision of authentic materials, enhancement of interactivity, and enhancement of learner autonomy. A list of commonly adopted e-resources/tools for language learning and teaching were identified. The students and teachers also indicated what support they would hope to receive from a Community of Practice (CoP) on technology-enhanced language learning and teaching.

The purpose of this study was to determine patients’ and families’ perceptions of care in 10 critical care units enhanced by a tele–intensive care unit (ICU) in a 5-hospital health care system. Patients and family members who had a critical care experience were approached for participation. An adapted version of the Schmidt Perception of Nursing Care Survey was administered. The Schmidt Perception of Nursing Care Survey factors–-seeing the individual patient, explaining, responding, and watching over—were analyzed for 637 participants (263 before and 374 after the tele-ICU implementation). Analysis of data from patients and family members indicated significantly higher means for the following factors: seeing the individual patient (P =.004), responding (P =.002), and watching over (P =.006) only when there was an awareness by the patient and family members that the care team was at the bedside and at the tele-ICU command center. The perceptions of care in these cases may suggest an improved patient experience when a tele-ICU is part of the care team.

e13681 Background: The onset of the coronavirus disease 2019 (COVID-19) pandemic changed the landscape of oncology practice and placed tele-oncology at the center of cancer care delivery. As tele-oncology will likely remain a key part of cancer care delivery, healthcare organizations need to determine how to implement tele-oncology programs. We conducted an instrumental case study at a National Cancer Institutes -designated comprehensive cancer center that successfully implemented tele-oncology. Methods: Data were collected from three sources between June 2020–February 2022. We conducted semi-structured interviews among stakeholders directly involved in implementing and/or managing tele-oncology and surveys of administrators of the center’s tele-oncology and clinicians who used the program. Document analysis of implementation deliverables were also analyzed. Data collection and analyses were guided by the Consolidated Framework for Implementation Research, and Clinical Transformation in Technology. We used the Implementation Research Logic Model (IRLM) to develop a framework depicting determinants and key steps for implementing tele-oncology. Results: We interviewed 40 clinicians. We also invited 364 employees to complete online surveys, of which 151 (41.5%) responded. Most respondents were physicians (41.7%), nurse practitioners (26.5%), or physician assistants (26.5%). To implement tele-oncology program, three elements should be considered: (1) platform, (2) people, and (3) process. We found hospitals should: 1) adopt a safe and adaptable platform to deliver tele-oncology; 2) build a multidisciplinary tele-oncology team to provide the administrative, legal, workflow, and clinical support; 3) provide training and resources to make sure all the team members can deliver their responsibilities; 4) identify and appreciate early adopters, departments, and clinicians with successful experiences; 5) frequently evaluate the clinical and implementation outcomes; 6) secure reimbursement comparable with in-person rate and ; 7) advocate for policies that are supportive of tele-oncology in the future. Conclusions: Using IRLM we presented the tele-oncology implementation framework that can be used by hospitals around the world to implement their own tele-oncology.

The tele–intensive care unit (ICU) provides a remote monitoring system that adds an additional layer of support for critically ill patients. However, to optimize contributions, the bedside team must incorporate this resource into the patient’s plan of care. Using the American Association of Critical-Care Nurses’ Healthy Work Environment Standards as a platform, we can create and nurture a new partnership model. Strategies that embrace the standards of skilled communication, true collaboration, and effective decision making become mutual goals for improving patient safety and outcomes. Joint communication guidelines facilitate timely and meaningful communication. Trust and the desire to cooperate encourage provider engagement to strengthen collaboration. The use of tele-ICU technology can assist in the interpretation and transformation of data to affect decision making at all levels to influence patient care. Through the lens of the healthy work environment, the tele-ICU/ICU partnership provides enhanced opportunities for improved patient care and team satisfaction.

Evidence-based practice (EBP) has become more than just a trendy buzzword in health care; EBP validates care delivery methods and grants satisfaction to nurses in knowing the care they provide is based on valid, current information. Research-based enhancements are paramount to the advancement of nursing practice and prompt the implementation of creative methods to improve care. The advent of the tele–intensive care unit (ICU) introduces new members of the health care team to assist with implementation of EBP initiatives. This new partnership results in improved length of stay, mortality rates, and ventilator times for critical care patients. Current literature suggests that a clinician-driven, standardized ventilator management protocol is of significant benefit. Tele-ICU clinicians provide an interactive element to coordinate interdisciplinary team efforts. Enhanced communication, data evaluation, and timely intervention expedite the weaning process and reduce ventilator length of stay. Consistent collaboration between tele-ICU and bedside clinicians successfully improves patient outcomes through standardized adherence to best-practice initiatives.

ABSTRACT The human ETS family gene TEL2/ETV7 is highly homologous to TEL1/ETV6, a frequent target of chromosome translocations in human leukemia and specific solid tumors. Here we report that TEL2 augments the proliferation and survival of normal mouse B cells and dramatically accelerates lymphoma development in Eμ-Myc transgenic mice. Nonetheless, inactivation of the p53 pathway was a hallmark of all TEL2/Eμ-Myc lymphomas, indicating that TEL2 expression alone is insufficient to bypass this apoptotic checkpoint. Although TEL2 is infrequently up-regulated in human sporadic Burkitt's lymphoma, analysis of pediatric B-cell acute lymphocytic leukemia (B-ALL) samples showed increased coexpression of TEL2 and MYC and/or MYCN in over one-third of B-ALL patients. Therefore, TEL2 and MYC also appear to cooperate in provoking a cadre of human B-cell malignancies.

Tele–intensive care unit (ICU) technology has been proven to bridge the gap between available resources and quality care for many health care systems across the country. Tele-ICUs allow the standardization of care and provide a second set of eyes traditionally not available in the ICU. A growing body of literature supports the use of tele-ICUs based on improved outcomes and reduction in errors. To date, the literature has not effectively outlined the limitations of this technology related to response to changes in patient care, interventions, and interaction with the care team. This information can potentially have a profound impact on service expectations. Some misconceptions about tele-ICU technology include the following: tele-ICU is “watching” 24 hours a day, 7 days a week; tele-ICU is a telemetry unit; tele-ICU is a stand-alone crisis intervention tool; tele-ICU decreases staffing at the bedside; tele-ICU clinical roles are clearly defined and understood; and tele-ICUs are not cost-effective to operate. This article outlines the purpose of tele-ICU technology, reviews outcomes, and “busts” myths about tele-ICU technology.

The tele–intensive care unit (tele-ICU) uses sophisticated telemedicine technology and a remote team of critical care experts, including nurses, to provide continuous monitoring, assessment, and interventional services to a large number of patients across multiple ICUs. This new practice environment offers experienced critical care nurses an opportunity for career and knowledge expansion while reducing some of the physical and emotional risks encountered at the bedside. The role of the tele-ICU is still evolving but focuses on 4 areas of responsibility: performing virtual rounds, managing patient alerts, providing ICU support, and coaching or providing teaching moments. The transition from the bedside into the tele-ICU role can be complex as tele-ICU nurses encounter ICU acceptance barriers and a lack of or a change in professional identity. A formal orientation program focused on competency is necessary for the successful transition from bedside nurse to tele-ICU nurse.

Para ello se llevó adelante un estudio cualitativo a partir de un cuestionario semiestructurado administrado a 49 informantes clave que permitió detectar los primeros efectos organizacionales de ese proceso de aprendizaje: los agentes tienen una percepción positiva de los primeros efectos del teletrabajo, pero ello tiene muchos matices, y no es incondicional.

Background Rapid Response Teams were developed to provide interventions for deteriorating patients. Their activation depends on timely detection of deterioration. Automated calculation of warning scores may lead to early recognition, and improvement of RRT effectiveness. Method This was a “Before” and “After” study, in the “Before” period ward nurses activated RRT after manually recording vital signs and calculating warning scores. In the “After” period, vital signs and warning calculations were automatically relayed to RRT through a wireless monitoring network. Results When compared to the before group, the after group had significantly lower incidence and rate of cardiopulmonary resuscitation (CPR) (2.3 / 1000 inpatient days versus 3.8 / 1000 inpatient days respectively, p = 0.01), significantly shorter length of hospital stay and lower hospital mortality, but significantly higher number of RRT activations. In multivariable logistic regression model, being in the “After” group decreases odds of CPR by 33% (OR = 0.67 [95% CI: 0.46–0.99]; p = 0.04). There was no difference between groups in ICU admission. Conclusion Automated activation of the RRT significantly reduced CPR events and rates, improved CPR success rate, reduced hospital length of stay and mortality, but increased the number of RRT activations. There were no differences in unplanned ICU admission or readmission.

In 1999, Human Rights Watch (HRW) and the International Federation for Human Rights (FIDH) published an extensive account of genocide in Rwanda, Leave None to Tell the Story. Based on interviews and archival work conducted by a team of researchers and written primarily by Alison Des Forges, Leave None to Tell was quickly recognised as the definitive account of the 1994 genocide. In the ensuing two decades, however, much additional research has added to our understanding of the 1994 violence. In this paper, I assess Leave None to Tell the Story in light of the research conducted since its publication, focusing in particular on three major challenges to the analysis. First, research into the organisation of the genocide disputes the degree to which it was planned in advance. Second, micro-level research into the motivations of those who participated disputes the influence of ideology on the genocide. Third, research has provided increasing evidence and details of violence perpetrated by the Rwandan Patriotic Front (RPF). I contend that despite these correctives, much of the analysis continues to hold up, such as the role of national figures in promoting genocide at the local level, the impact of the dynamics of local power struggles on the violence, and the patterns of violence, including the effort after the initial massacres to implicate a wide portion of the population. Finally, as a member of the team that researched and helped write Leave None to Tell, I reflect on the value of this rare sort of research project that engages human rights organisations in an academic research project.

In this essay, we tell the story of how a team of English and nursing professors came together to develop curriculum for a mandatory first-semester writing course in the collaborative Bachelor of Science, Nursing (BScN) at Fanshawe College and Western University, both in London, Ontario. The discussion focuses on the implementation of the course at the Fanshawe site. Following a review of literature that has informed our thinking about writing in nursing, we discuss how the team, consisting of both English/writing and nursing faculty, solved curriculum problems to develop an effective course. We also look forward to areas for future development.

Ovaj rad prvenstveno nastoji da problematizuje postojeće definicije i određenja koncepcije nezavisnih tela i nezavisnih kontrolnih organa, kao i da ispita domete i ograničenja pojmova koji su u upotrebi u svrhu označavanja nezavisnih kontrolnih institucija. Analiza ne predstavlja naprosto teorijski kuriozitet, budući da način na koji politički teoretičari, ali i praktičari politike u Srbiji misle i određuju pojam nezavisnih tela ima neposredan efekat na način na koji će ova vrsta organa biti tretirana u praksi, kao i na pitanje kakva će im uloga biti dodeljena u okviru pozitivno pravnog i političkog sistema Republike Srbije. Kako bi navedeni cilj bio postignut, analiza pronalazi utemeljenje u jednom od konkurentnih modela određivanja i klasifikacije nezavisnih tela, i sa ove ishodišne tačke vrši ispitivanje karakteristika tri pojedinačna organa koji predstavljaju granične slučajeve u odnosu na kriterijume za čiju se upotrebu rad opredeljuje. Konkretnoj analizi su podvrgnuti predsednik Republike, Ustavni sud Republike Srbije i Narodna banka Republike Srbije, dok su, u skladu sa zahtevima analogije koju postavljamo, izvršeni uvidi u rad, status i nadležnosti i drugih organa koji su u literaturi široko prihvaćeni kao nezavisna kontrolna tela različite vrste. U radu se podvlači značaj koji adekvatno razumevanje uloge i funkcija nezavisnih tela mogu imati po očuvanje sistema podele vlasti, vladavine prava, ali i očuvanja i konsolidacije demokratije u Srbiji.
