The term ‘athlete’s heart’ refers to the structural, functional, and electrical adaptations that occur as a result of habitual exercise training. It is characterized by an increase of the internal chamber dimensions and wall thickness of both atria and ventricles. The athlete’s right ventricle also undergoes structural, functional, and electrical remodelling as a result of intense exercise training. Some research suggests that the haemodynamic stress of intense exercise is greater for the right heart and, as a result, right heart remodelling is slightly more profound when compared with the left heart. Echocardiography is the primary tool for the assessment of morphological and functional features of athlete’s heart and facilitates differentiation between physiological and pathological LV hypertrophy. Doppler myocardial and strain imaging can give additional information to the standard indices of global systolic and diastolic function and in selected cases cardiac magnetic resonance imaging may help in the diagnosis of specific myocardial diseases among athletes such as hypertrophic cardiomyopathy, dilated cardiomyopathy, or arrhythmogenic right ventricular cardiomyopathy.

This chapter examines parental responses to pregnancy complications and having a high-risk infant. Women with high-risk pregnancies have more depressive symptoms, stress, and anxiety than healthy pregnant women. They experience shock, worry, sadness, frustration, anger, guilt, and grief; perform fewer health-promoting behaviors; and have less intense maternal–fetal attachment. Parents also experience emotional distress after the birth of a high-risk infant, including worry about infant survival and outcomes, stress, anxiety, depression, post-traumatic stress, grief, hostility, and powerlessness. Distressed parents perceive their infants more negatively and are less sensitive in interactions than nondistressed parents. Several postnatal interventions have been implemented to ameliorate these negative responses. Overall, the responses of mothers to a high-risk pregnancy or birth of a high-risk infant are remarkably similar. More needs to be known about the effect of the parent’s past history, paternal responses, responses of minorities, experiences of parents of high-risk full-term infants, and interventions to ameliorate negative parental responses.

Each year, about six million children undergo surgical procedures in the United States. More than 80% of all pediatric procedures in the United States are on an outpatient basis without hospital preoperative or postoperative admission. More than 60% of these children manifest emotional distress prior to the procedures and at the induction of anesthesia. The intense distress may lead to negative postoperative physiological and psychological outcomes in children and affect parental satisfaction. Preoperative stress can also exaggerate negative memories, which can exaggerate distress at subsequent procedures. Studies have looked into the incorporation of pharmacological agents as well as nonpharmacological strategies such as parental presence, and behavioral techniques such as imagery, relaxation, and hypnosis in the management of preoperative anxiety. The goal of this chapter is to examine the effects of preoperative anxiety and the effectiveness of these strategies in preventing preoperative psychological stress in children.

Abstract Trauma is almost universal in human life, but only a minority of those exposed to adversity develop posttraumatic stress disorder (PTSD). Trauma has become a catchword for many kinds of adverse experiences; this is a construct that needs to be more narrowly and precisely defined. Moreover, most people are resilient to adversity. While exposure is a risk factor for psychopathology, PTSD tends to develop in people with high neuroticism, which describes intense reactions to adversities, based on temperament, and that is a heritable personality trait. The best model of PTSD needs to be biopsychosocial. Moreover, while childhood trauma is a risk factor for many forms of psychopathology, only some of those exposed to early adversity develop mental disorders. Failure to understand the complexity of the pathways to psychopathology can lead to well-meaning but misguided approaches to psychotherapy.

Abstract Ambulance services and paramedics perform critical roles in contemporary healthcare economies. Trained to work in the field and respond rapidly to emergencies, societies have come to increasingly rely on ambulance services to deliver urgent care, never more so than in recent years given intense social inequality, overstretched and underfunded health systems, and deadly pandemics. This monograph is the first book-length study of the paramedic profession in England. Based on in-depth interviews and ethnographic observation The Paramedic at Work provides a detailed account of the complex realities of work in this fascinating occupation. Empirical chapters explore the nature of work ‘out on the road’, the peculiarities of ambulance organizational culture, the intensity of workplace stress and burnout, and the current and future trajectory of paramedic professionalism. The book documents the unique paradoxes experienced by those employed in this line of work. Ambulance staff are trained to handle life-threatening trauma and disease, but most callouts consist of unplanned primary care. Paramedic work features wide autonomy but is also bound into an array of micromanaging performance indicators. Paramedics are trusted and respected in society but the profession is poorly understood and employers can be unsupportive. But, no matter how intense the personal struggles can be, paramedic work also offers rare opportunities for meaningful and socially valued work. The nature of the paramedic role is rapidly moving from a manual occupation rooted in first aid and transportation, to a clinical profession of increasing scope, versatility, and social respect.

Ernest Hemingway was plagued all his life by sexual arousal linked to associations developed in early childhood. In his letters and writing, Hemingway does not easily use “I” or “you.” From Paris, he writes to his boyhood friends as simply a male: “Bring a male up to date.” Psychoanalysts have given us a full range of Hemingway’s mental disabilities: latent homosexuality, posttraumatic stress disorder, bipolar disorder, narcissism, gender-identity issues, self-image issues, domestic abuse, alcoholism, and sexual inadequacy. His most profound disability might perhaps have been his disconnection from himself and others. His deepest longing was to dissolve with another in a way that did not require him to be a separate, individual person. Ultimately, Hemingway’s desire for dissolution led to self-murder, even as the chaos, rage, and agony of his bipolar mind brought with it an astonishing blessing: stark, intense, and magnificent prose.

This chapter reviews psychiatric consultation for trauma and stress in medical-surgical patients. Hospitalization can induce psychologic or psychiatric disturbance and worsen the clinical condition of patients who are suffering from medical and surgical comorbidities. Some medical conditions can be related to stress related disorders indirectly, while others, such as critical illness/intensive care unit treatment or direct physical injury, are themselves traumatic stressors that can promote trauma and stressor-related disorders (TSRDs). Given the negative impact of stress-related disorders on quality of life, mental health clinicians should diagnose TSRDs to ensure patients receive appropriate care. Treatment and management can be provided in multiple forms of psychological therapies and psychopharmacology, and within a multidisciplinary team, particularly for the medical surgical patient. Psychiatrists, psychologists, and social workers must assist patients with terminal illnesses by optimizing end-of-life care, supporting patients and their families, and encouraging approaches to allow the transformative process of dying to be meaningful.
