Abstract Background: Over the last years, the management of patients with node positive early breast cancer has gone through important innovations. On the medical side, new targeted therapies such as olaparib and abemaciclib have been introduced, with promising results on the invasive disease-free survival. Moreover, sparing axillary lymph node dissection has proven to be noninferior in terms of overall survival. However, no tools are currently available to predict lymph node involvement before definitive surgical evaluation. The aim of the study was to analyze clinical and pathological characteristics of patients with node positive early breast cancer to explore potential risk profiles associated with a ≥3 nodal involvement. Methods: The study retrospectively analyzed 335 node-positive breast cancer patients treated at the Breast Unit of the CRO Aviano National Cancer Institute, between 2017 and 2021. Data regarding primary tumor biological features, lymph node involvement and surgical approach were collected. Associations between clinico-pathological characteristics and ≥3 lymph node involvement were tested through stepwise logistic regression and the gradient boosting machine learning algorithm (GBM). Results: Among the 335 analyzed patients, 87.0% had a primary tumor < 5 cm, with a single positive lymph node in 73.3% of cases. Hormone receptors were mainly positive (respectively 93.5% and 83.4% for estrogen and progesterone receptors). Tumor grade was most frequently well differentiated (Grade 1 in 60.7%), with a Ki67 < 20% (59.5%). After multivariable logistic regression, a tumor size ≥ 3 cm (OR 3.24, CI95% 1.47-7.17, p = 0.004), the presence of massive lymphovascular stromal invasion (OR 2.50, CI95% 1.02-6.14, p = 0.045) and 2 or more positive sentinel lymph nodes at surgical evaluation (OR 6.08, CI95% 3.34-11.05, p < 0.001) were associated with a higher risk of identifying ≥ 3 positive lymph nodes after subsequent axillary dissection. Similar results were observed in the luminal-like cohort. A GBM machine learning model was then developed with a 0.77 Area Under the Curve. Features with the highest relative importance (RI) were single sentinel node involvement (RI 16.1873), followed by tumor size ≥ 3 cm (RI 10.2024), ≥2 positive sentinel lymph nodes (RI 8.5050) and lymphovascular stromal invasion (4.0217). Consistently, number of positive sentinel lymph nodes and tumor size were the predominant features in all top 20 GBM models. Conclusions: The present study explored the definition of risk profiles linked to 3 or more positive lymph nodes based on clinical and pathological features. It, moreover, tested the feasibility of developing machine learning classifiers to support future clinical decision-making. Due to the growing complexity of the adjuvant setting, finding a balance between minimally invasive surgical and staging approaches and risk definition for treatment personalization will become increasingly critical. Este artículo busca dar a conocer la existencia de dicha base de datos, así como algunos de los temas que podemos encontrar en este reservorio documental que pueden ser de utilidad para quienes estén interesados en conocer y reconstruir la historia de Chiapas e incluso, de América Latina, durante la segunda mitad del siglo XX.

Civilna družba v demokratičnih državah zahteva, da so oborožene sile profesional- ne. Torej morajo oborožene sile teh družb imeti jasno predstavo o tem, kaj vojaška profesija sploh je. Predvsem pa morajo imeti jasno izoblikovano predstavo o tem, kakšen je profesionalni častnik, saj je prav častniški zbor tisti, ki je odgovoren za vodenje oboroženih sil. Znanstvene razprave o vojaški profesiji in profesionalnem častniku so se pojavile po koncu druge svetovne vojne z deli Samuela Huntingtona, Morissa Janowitza, Bengta Abrahamssona in drugih. Njihova razmišljanja so odsevala predvsem okoli- ščine v času hladne vojne, te pa so se s padcem berlinskega zidu v devetdesetih letih prejšnjega stoletja močno spremenile. Večina avtorjev tako ugotavlja, da je nujno preveriti in posodobiti koncepte vojaške profesije in vojaškega profesionalca. V članku poskušam odgovoriti na to, kaj danes pomenita pojma vojaška profesija in profesionalni častnik ter kako bi k razvoju vojaške profesije in profesionalnega častnika morali pristopiti v Slovenski vojski (SV). Civil society in democratic countries expects its armed forces to be professional. Therefore, the armed forces in those countries need to have a clear idea of what pro- fession of arms is. They should especially have a clear idea of what a professional officer is, since the officer corps is responsible for leading the armed forces. The subjects of profession of arms and professional officer were first scientifically discussed after the Second World War by authors such as Samuel Huntington, Morris Janowitz, Bengt Abrahamson and others. Their theories reflected the circumstances of the Cold War, which, however, significantly changed with the fall of the Berlin Wall in the 1990’s. Most of the authors today have thus identified the need for revising and updating the traditional concepts of the profession of arms and military professional. The article attempts an answer to what the terms profession of arms and professional officer mean today, and how the SAF should approach the development of military profession and professional officers.

Workers in ‘conscientization’ through theatre in the so-called underdeveloped countries tend to assume that this form of theatre work has a potential unique to such circumstances. R. G. Davis, who was founding director of the San Francisco Mime Troupe in the ‘sixties, argues that the poorer sections of the richest nation in the world have no less been deprived of their cultural roots, and here describes his experiences when he was asked to direct a play dramatizing a workplace incident in the cannery town of Watsonville, northern California, when a Mexican worker was appealing for reinstatement against unfair dismissal. A regular contributor to the former Theatre Quarterly, most recently in TQ 40 (1981) on his productions of Dario Forin the USA, R. G. Davis's own version of We Won't Pay, We Won't Pay, was published last year by Samuel French, and he has recently directed a Native American play in San Jose, followed by a Brecht play in Australia.

Abstract This study focuses on the difficult ending of 1 Sam 20:41 in the Masoretic text (MT): וַיִּבְכּוּ אִישׁ אֶת רֵעֵהוּ עַד דָּוִד הִגְדִּיל, rendered normally as “They wept together; David wept the longer.” Many have pointed out the peculiarity of this phrase, and different emendations were proposed, based mainly on the LXX. The present article suggests an emendation of 1 Sam 20:41, according to which the original text read here עד בור הגדול. The mention of the “Great Cistern” at the end of the previous story in the Book of Samuel (19:22), provides strong support for this suggestion. It is proposed that the mention of the “Great Cistern” in the original text intended to narrow the geographical gaps between the different stories about David and Saul.

Abstract We examined genetic population structure of five putative subspecies of Song Sparrows (Melospiza melodia) in the San Francisco Bay region (M. m. samuelis, M. m. maxillaris, M. m. pusillula, M. m. gouldii, and M. m. heermanni) at nine microsatellite loci to assist the development of Song Sparrow conservation and management strategies. We sampled nine populations from five putative subspecies and found low estimates of differentiation between populations within subspecies and between. Despite low estimates of divergence, genetic structure at the subspecies level was indicated by the larger amount of variance accounted for by subspecies than populations. We propose that a management unit encompassing the range of M. m. pusillula be given priority for conservation on the basis of the extent of genetic divergence shown by Cavalli-Sforza and Edward's chord distance, and the topology of an unweighted pair group cluster analysis supported by 100% of bootstrap replicates across loci. Although M. m. samuelis and M. m. maxillaris appear undifferentiated from M. m. heermanni, it remains possible that adaptive differences between those types were not identified with neutral loci.

This is an example of reading a biblical text as a narrative unity. The article argues for the holistic reading of the whole of 1 Sam 25. What some scholars argue for an appended v. 1 and vv. 43-44, will be seen as integral part of the whole narrative of trying to preserve the characterization of David as a divinely-appointed king, an act that is given a suspenseful start in the loss of David’s protector in the person of Samuel (v. 1), and provided with a theological support in David’s marriage to Abigail, a marriage that is set as an ideal one compared to David other marriages mentioned in the narrative (vv. 43-44).

AbstractThe following article is based on a presentation given by Samuel I. Stupp of Northwestern University as part of Symposium X—Frontiers of Materials Research on April 13, 2004, at the Materials Research Society Spring Meeting in San Francisco. Materials designed at the molecular and supramolecular scales to interact with cells, biomolecules, and pharmaceuticals will have a profound impact on technologies targeting the regeneration of body parts. Materials science is a great partner to stem cell biology, genomics, and proteomics in crafting the scaffolds that will effectively regenerate tissues lost to trauma, disease, or genetic defects. The repair of humans should be minimally invasive, and thus the best scaffolds would be liquids programmed to create materials inside our bodies. In this regard, self-assembling materials will play a key role in future technologies. This article illustrates how molecules are designed to assemble into cell scaffolds for human repair and provides examples relevant to brain damage, fractures of the skeleton, spinal cord injuries leading to paralysis, and diabetes.

String instruments may be played either with open strings (where the string vibrates between the bridge and a hard wooden nut) or with stopped strings (where the string vibrates between the bridge and a performer's finger pressed against the fingerboard). Compared with open strings, stopped strings permit the use of vibrato and exhibit a darker timbre. Inspired by research on the timbre of sad speech, we test whether there is a tendency to use stopped strings in nominally sad music. Specifically, we compare the proportion of potentially open-to-stopped strings in a sample of slow, minor-mode movements with matched major-mode movements. By way of illustration, a preliminary analysis of Samuel Barber's famous Adagio from his Opus 11 string quartet shows that the selected key (B-flat minor) provides the optimum key for minimizing open string tones. However, examination of a broader controlled sample of quartet movements by Haydn, Mozart and Beethoven failed to exhibit the conjectured relationship. Instead, major-mode movements were found to avoid possible open strings more than slow minor-mode movements.

The study represents the theory of a literary and thematic dependency of Psalm 113 upon 1 Samuel 2:1-10. The ‘Song of Hannah’ (c. 7th Century bc ?) is older than Psalm 113 (c. 6th Century bc ?). Both psalms place emphasis upon the uniqueness of Jahwe. The theological skopos of Psalm 113 aims primarily at the affirmation of Jahwe as the incomparable God (v.5). This topic allows itself to present Psalm 113 from 1 Sam 2:1-10. The author of Psalm 113, with the help of ‘The Song of Hannah’ in 1 Sam 2:1-10 has made Ps 113 a hymn of the community of Jahwe about the incomparable God.

The ‘biographical’ notes of the Masoretic ‘Book’ of Psalms are often understood as placing the psalms in dialogue with 1-2 Samuel, and casting David as a pious exemplar. As David prayed psalms in his distress, so can anyone. Indebted to an influential article by Brevard Childs, many scholars also see early traces of midrashic exegesis. However, this is not entirely persuasive, and to inquire into these issues, the article proceeds from the observation that many of the ‘biographical’ notes cluster around similar events. In most of them, David is fleeing from Saul. Following a survey of the ‘biographical’ notes in both the Masoretic text and the Septuagint, it is argued that the often-suggested connections between the psalms and 1-2 Samuel are quite weak, and that a better way to understand the addition of ‘biographical’ notes is found when reading them in light of a resurfacing Saulide–Davidic rivalry in post-exilic times.

AbstractThe biblical portrayal of the Philistines is largely negative. Their military exploits, depicted extensively in the books of Judges and 1 Samuel—coupled with their religion, unaccustomed to the Israelite cult—have led several commentators to label them a prototypical “other.” To put it memorably, the Philistines are what Israel should not be. I attempt to nuance somewhat this overtly negative characterization of the Philistines by focusing on one incident in 1 Sam 6: the Philistines’ returning of the ark, accompanied with a peculiar offering of objects made of gold. I compare this ritual to the sacrificial actions of Eli’s sons in 1 Sam 2-4 to argue that, at least in 1 Sam 6, and with respect to what lies at the heart of Israel’s cult—the approach of the inscrutable and holy deity—the Israelites should be more like the Philistines.

Abstract Background Among children with acute otitis media (AOM) S.pneumoniae, H.influenzae, and M.catarrhalis are the predominant bacterial otopathogens. There is a high correlation between nasopharyngeal (NP) and middle ear fluid (MEF) organisms during AOM. Thus, NP samples could serve as a surrogate for detection of otopathogens and are more easily collected in a typical practice environment than MEF. Though culture is considered the gold standard for detection, it is time-consuming, which can limit its diagnostic utility to guide clinical care. We aimed to determine the sensitivity, specificity, positive (PPV) and negative predictive value (NPV) for NP qualitative PCR for bacterial otopathogens compared to NP culture. Methods Patients age 6-35 months with uncomplicated AOM who were prospectively enrolled in an AOM study in Denver, CO from Jan 2019-Dec 2020 were included. All patients had an NP flocked swab (Eswab®, Copan Diagnostics) at enrollment. Otopathogen culture was completed using standard techniques. Nucleic acids were extracted using the NucliSENS® easyMAG® system (Quidel, San Diego, CA) per manufacturer’s instructions. Multiplex RT-PCR for S.pneumoniae, H.influenzae, and M.catarrhalis was completed using Lyra® (Quidel, San Diego, CA) and AnDiaTec® assay kits (Quidel Germany GmbH, Kornwestheim, Germany). Nucleic acid amplification and detection was completed on the Applied Biosystems® (ABI) 7500 Fast Dx Real-Time PCR Instrument. Results Of the 80 children included, 18 (22.5%) had no organism detected on culture, 31 (38.8%) had one and 31 (38.8%) had multiple organisms detected. The most commonly identified organisms on culture were M.catarrhalis (42, 52.5%), followed by S.pneumoniae (30, 37.5%), and H.influenzae (17, 21.3%). Of H.influenzae isolates 8 (47.1%) produced beta-lactamase. The sensitivity of PCR was high ( >94%) for all organisms whereas the specificity was lower (50.0-77.8%) and varied by organism (Table). NPV were high ( >96%) for all otopathogens, whereas, PPV ranged from 53.3 to 68.9%. PCR detected 1.6 times more organisms than culture (149 vs. 96). Sensitivity, specificity, positive and negative predictive value of PCR compared to culture for otopathogens. Conclusion NP PCR has a high predictive value for excluding otopathogens and warrants further exploration as a diagnostic tool to evaluate for otopathogens in children. Abstract In this essay, I explore the literary background of 2 Sam 8:1b–14. The Old Sabaic royal summary inscription RES 3945/3946 exhibits significant structural and compositional parallels to the summary account of David’s achievements. I argue that the quantity and quality of such similarities firmly locates the writing practices underlying the literary history of 2 Sam 8:1b–14 within a scribal tradition of narrating royal accomplishments shared in Israel and Ancient South Arabia. Based on this historical approximation, it is possible to revisit the problem of the passage’s literary shape and position within the book of Samuel.

The functioning of human consciousness in interpreting and staging a theatrical performance is, as Gordon Armstrong argues in this article, among the most highly selective and adaptive operations known to physical science. According to this view, the theatre, as a substrate of consciousness, was part of the package that defined modern man as a reflective species: whereas for the first four million years of human existence man was silent about a probable inner life, the dawn of empathy some 200,000 years ago saw a neural explosion – the enlargement of the angular gyrus in the left hemisphere of the brain, unlocking a new kind of reflective consciousness. In isolation, this aberrant neurological connection proved so advantageous for hunting and for communication that members of a tribe who possessed this aberration prospered: and adaptation to the ice ages that began 200,000 years ago was a motivating factor in stimulating the emergence of what we can recognize as art. Gordon Armstrong is immediate past Secretary of the American Society for Theatre Research, and Review Editor for Theatre Research International. He has taught at UCLA, SUNY Stony Brook, and the University of Rhode Island, and has designed and directed productions in San Francisco, Los Angeles, and New York. In his seminal The Theatre of the Absurd, Martin Esslin suggested that Samuel Beckett, in denying his characters individualized facets of humanity, achieved an ‘alienation effect’ that was more profound and assured than Brecht's. Here, Michael David Fox, while agreeing that Beckett denies actors and audiences the kinds of identification achieved in naturalistic drama through Stanislavskian techniques, argues that he demands a different quality of empathy from his audiences – not through the artifice of a character's simulated pain but through the actuality of the performer's physical suffering. While analyzing the demands and constraints upon actors which Beckett imposes in his better-known plays, he also re-evaluates the more ‘occasional’ piece Catastrophe – written in 1982 ostensibly as a homage to the then-imprisoned Czech playwright Vaclav Havel – less as a critique of political terror than as an ironic and devastating self-critique of the terror of Beckett's own tragic representation. Michael David Fox is a doctoral student in drama and critical theory in the Joint Doctoral Program in Drama at the University of California, Irvine and the University of California, San Diego. His dissertation is on the production of empathy in Shakespeare's tragedies.
