Lei, Yuan. Alarms and Safety Mechanisms. Oxford University Press, 2017. http://dx.doi.org/10.1093/med/9780198784975.003.0012.

Texte intégral
Résumé :
‘Alarms and Safety Mechanisms’ breaks that puzzling black box, which includes those critical functions designed to safeguard the ventilated patient. It describes the categories of alarms, including application alarms (also called clinical alarms), which may indicate problems with the patient; a leak or occlusion in the breathing circuit; or an improper ventilator setting. It goes on to describe technical alarms that point to an actual malfunction within the ventilator system. It also discusses alarm priorities and alarm settings or alarm limits. The chapter details each common alarm, listing alternative alarm messages as they appear on different ventilators, and describing what the alarm means and how to troubleshoot it. Finally, the chapter discusses a ventilator’s emergency states, such as the ambient state or the safety valve open state, and the ventilator inoperative alarm.

Résumé :
This chapter examines the role of interest groups in shaping American social welfare policy. It outlines major theories and findings on interest group influence in American politics and comparative welfare state development and examines the activities and influence of major categories of groups, including business, labor, agriculture, professional associations, intergovernmental organizations, and citizens’ groups. Although many interest groups have helped secure policies that form a limited social safety net, this chapter suggests that the competition among a diverse array of interest groups in a fragmented political system makes policy change difficult. This tendency towards gridlock, which favors the interests of some groups over others, has constrained the size and redistribution of the American welfare state.

Ramadan, Moussa Abou. Muslim Jurists’ Criteria for the Division of the World into Dar al-Harb and Dar al-Islam. Oxford University Press, 2017. http://dx.doi.org/10.1093/oso/9780198805878.003.0011.

Résumé :
There is disagreement over the definition of dar al-Islam and dar al-harb between classical and contemporary jurists. Different authors have laid down different criteria; we find different opinions even within the same school; and at times variations may even be found with regard to the same author and/or in the same treatise. Some jurists give preference to Islamic law, while other focus on the safety of Muslims. The majority accepts that dar al-Islam can be transformed into dar al-harb, while Ibn Hajar holds that a territory belonging to dar al-Islam will never lose its status. There is also divergence on the question whether the world is divided into two or three categories.

Tree Faller's Manual. CSIRO Publishing, 2011. http://dx.doi.org/10.1071/9780643101746.

Texte intégral
Résumé :
The Tree Faller’s Manual is an essential handbook for forest operators and others who need to fell trees manually using a hand-held chainsaw. This manual builds on the information provided by the Chainsaw Operator’s Manual. Tree felling is a high risk activity. Many fatalities and serious injuries have occurred as a result of being struck by falling trees, dislodged tree limbs or other dangers in the area. Most of these accidents are caused by using unsafe felling techniques and not following safe work procedures. This manual will guide the faller to safer work techniques. The manual is based on the national competency standards for the forest and forest products industry where tree-felling is covered using three categories: basic, intermediate and advanced. Basic tree felling applies to trees that are relatively small, with a single stem and no defects. Intermediate tree felling covers trees with single or multiple stems, limited defects, and lean and weight distribution that can be adapted to felling direction. Advanced tree felling applies to larger and more complex trees and includes trees deemed to be more hazardous. Workplace safety, risk assessment and site preparation are included along with the theory, techniques and tools for each of the tree-felling categories.

Texte intégral
Résumé :
This book offers the most up-to-date research involving the treatment and management of paraphilic and nonparaphilic sex offenders with and without a comorbid mental illness or intellectual disability. It provides in-depth coverage of the problems of identification, risk assessment and management, treatment, and legal solutions. It seeks to ensure public safety while at the same time maintaining medical integrity and respect for due process. Since the first edition of this book, several states have significantly changed their laws concerning registries of sex offenders, which have been expanded to all aspects of community-based management in which clinicians are actively involved. This second edition addresses how the law has not yet caught up with the rapid development of new technology in the past few years, leaving many new categories of risk potentially unaddressed.

Résumé :
Various fields of research have developed to better understand the health and behavioral effects of environmental characteristics such as air quality and the way our homes and neighborhoods are organized. A synergy of many previously disparate fields of research is underway, aided in part by recent advances in technology. Better sensors, including wearable physiological and environmental monitors, are enabling researchers to more readily study the interactions between environment characteristics and both mental and physical well-being. This new evidence-based research direction adds a much-needed layer of quantitative data to previous, largely qualitative, findings. Moreover, an increased understanding of the environment’s effects on humans can result in not just the alleviation of negative environmental characteristics but also the promotion of positive environmental characteristics. This chapter offers samples of environmental effects on human health and well-being in the following categories: environmental quality, natural systems, sensory environments, physical activity, safety, and social connectedness.

Bannwarth, Bernard, et Francis Berenbaum. Systemic analgesics (including paracetamol and opioids). Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780199668847.003.0029.

Texte intégral
Résumé :
Apart from non-steroidal anti-inflammatory drugs (NSAIDs), there are only two categories of systemic analgesics, namely paracetamol (acetaminophen) and opioids, that are currently available worldwide for clinical use. Paracetamol is poorly effective in relieving pain and improving function in patients with symptomatic osteoarthritis (OA). Furthermore, its safety profile is less favourable than classically thought. In fact, there is evidence paracetamol acts as a weak inhibitor of the cyclooxygenase enzymes. Given that paracetamol poses a lower risk of severe adverse events than NSAIDs while being better tolerated than opioids, it is usually considered as the first-line systemic analgesic for OA. Commonly prescribed opioids are primarily agonists of the mu receptors, thereby producing similar desirable (analgesia) and untoward effects. Meta-analyses of short-term clinical trials showed that, on average, the modest clinical benefits of opioids did not outweigh the side effects in patients with knee or hip OA. Accordingly, most current guidelines support the use of opioids for selected OA patients only (e.g. patients who have not had an adequate response to other treatment modalities and are not candidates for total joint arthroplasty). In view of the limited efficacy and/or potential harms of available analgesics, particular attention was paid to novel painkillers, especially nerve growth factor (NGF) antagonists. Although these agents provided clinically meaningful improvements in pain and physical function in patients with hip or knee OA, they lead to severe side effects, including rapidly destructive arthropathies and neuropathies. Thus, if approved for marketing, NGF antagonists would be reserved for selected and well-defined patients with OA.
