This chapter reviews recent literature related to local anesthetic sympathetic ganglion blockade (SGB) in treatment of post-traumatic stress disorder (PTSD) and its effect on the overall function of patients, with specific emphasis on male sexual and marital function. This chapter also discusses a publically available video in which a patient who had the PTSD checklist (PCL) administered before, and following, SGB as well as his wife were interviewed. A 34 year-old male Army Ranger veteran who suffered severe PTSD with associated marital and sexual difficulties after deployment, the patient had reported decreased libido and avoidance of intimacy. After SGB, he noticed marked improvement of his PTSD symptoms and resolution of his sexual dysfunction and marital difficulties. His reported improvements were echoed by his wife. The chapter also discusses known mechanisms of sexual dysfunction as related to PTSD and sympathetic nerve blockade effects as related to the sympathetic nervous system.

A screening checklist emphasizing basic principles of psychiatric history-taking is provided to help identify the patient’s requirements. The author reviews the classes of psychiatric diagnoses most likely to be present in the pain/addiction and other comorbidly-ill patients, and reviews suicide risks. Similarly, the classes of medications employed in psychiatry and their capacity for alleviation or aggravation of substance use disorders are reviewed, with notations of drug–drug interactions. A final section addresses the role of emotions and psychiatric symptoms in the perception and management of pain.

Managing the urban water cycle needs to be underpinned by key sustainability principles of water consumption, water recycling, waste minimisation and environmental protection. The integration of urban water cycle management with urban planning and design is known as Water Sensitive Urban Design (WSUD). One of the key elements of WSUD is the management of urban stormwater, both as a resource and for the protection of receiving water ecosystems. This requires strategic planning and concept designs that are underpinned by sound engineering practices in design and construction. Crisis resource management (CRM) and patient safety are fundamental to the practice of anesthesiology. Human error and system failures continue to play a substantial role in preventable errors that lead to adverse outcomes or death. Many of these deaths are not the result of inadequate medical knowledge and skill, but occur because of problems involving communication and team management. CRM addresses these patient safety issues by addressing behavioral skills for critical events. These skills provide tools to help the leader manage the team and to help the team work together; they include calling for help, establishing situation awareness, using checklists, and communicating effectively. Effective strategies to teach these skills include the use of simulation for team training and Team STEPPS.

There are helpful tips and strategies on a variety of topics, notably preparing a monitoring plan; how to identify and assess potential investigators; preparing for a Study Initiation Visit; eCRFs and remote monitoring; thorough Source Data Verification; how to report monitoring visits plus example checklists associated with site visits and review of the Investigator Site File. The guidance provided in this chapter should help CRAs perform their essential role in encouraging good, high quality research - and provide an insight into the CRA role to those whose work is being monitored.

General anaesthesia (GA) is most often indicated for category 1 (immediate threat to life of mother or baby) caesarean delivery (CD) or when neuraxial anaesthesia has failed or is contraindicated. Secure intravenous access is essential. Jugular venous cannulation (with ultrasound guidance) is required if peripheral access is inadequate. A World Health Organization surgical safety checklist must be used. The shoulders and upper back should be ramped. Left lateral table tilt or other means of uterine displacement are essential to minimize aortocaval compression, and a head-up position is recommended to improve the efficiency of preoxygenation and reduce the likelihood of gastric contents reaching the oropharynx. Cricoid pressure is controversial. In the United Kingdom, thiopental remains the induction agent of choice, although there is scant evidence upon which to avoid propofol. In pre-eclampsia, it is essential to obtund the pressor response to laryngoscopy with remifentanil or alfentanil. Rocuronium is an acceptable alternative to succinylcholine for neuromuscular blockade. Sugammadex offers the possibility of swifter reversal of rocuronium than spontaneous recovery from succinylcholine. Management of difficult tracheal intubation is focused on ‘oxygenation without aspiration’ and prevention of airway trauma. The Classic™ laryngeal mask airway is the most commonly used rescue airway in the United Kingdom. There is a large set of data from fasted women of low body mass index who have undergone elective CD safely with a Proseal™ or Supreme™ laryngeal mask airway. Sevoflurane is the most popular volatile agent for maintenance of GA. The role of electroencephalography-based depth of anaesthesia monitors at CD remains to be established. Intraoperative end-tidal carbon dioxide tension should be maintained below 4.0 kPa.
